Add a driver for kvm guests that matches virtual ccw devices provided
by the host as virtio bridge devices.
These virtio-ccw devices use a special set of channel commands in order
to perform virtio functions.

Seems like everyone copied x86 and defined 4 private memory slots
that never actually get used. Even x86 only uses 3 of the 4. These
aren't exposed so there's no need to add padding.

It's easy to confuse KVM_MEMORY_SLOTS and KVM_MEM_SLOTS_NUM. One is

Merge misc VM changes from Andrew Morton:
"The rest of most-of-MM. The other MM bits await a slab merge.
This patch includes the addition of a huge zero_page. Not a
performance boost but it an save large amounts of physical memory in
some situations.
Also a bunch of Fujitsu engineers are working on memory hotplug.
Which, as it turns out, was badly broken. About half of their patches
are included here; the remainder are 3.8 material."
However, this merge disables CONFIG_MOVABLE_NODE, which was totally
broken. We don't add new features with "default y", nor do we add
Kconfig questions that are incomprehensible to most people without any
help text. Does the feature even make sense without compaction or
memory hotplug?

Pull big execve/kernel_thread/fork unification series from Al Viro:
"All architectures are converted to new model. Quite a bit of that
stuff is actually shared with architecture trees; in such cases it's
literally shared branch pulled by both, not a cherry-pick.
A lot of ugliness and black magic is gone (-3KLoC total in this one):
- kernel_thread()/kernel_execve()/sys_execve() redesign.
We don't do syscalls from kernel anymore for either kernel_thread()
or kernel_execve():
kernel_thread() is essentially clone(2) with callback run before we
return to userland, the callbacks either never return or do
successful do_execve() before returning.
kernel_execve() is a wrapper for do_execve() - it doesn't need to
do transition to user mode anymore.
As a result kernel_thread() and kernel_execve() are
arch-independent now - they live in kernel/fork.c and fs/exec.c
resp. sys_execve() is also in fs/exec.c and it's completely
architecture-independent.
- daemonize() is gone, along with its parts in fs/*.c
- struct pt_regs * is no longer passed to do_fork/copy_process/
copy_thread/do_execve/search_binary_handler/->load_binary/do_coredump.
- sys_fork()/sys_vfork()/sys_clone() unified; some architectures
still need wrappers (ones with callee-saved registers not saved in
pt_regs on syscall entry), but the main part of those suckers is in
kernel/fork.c now."

"The biggest change affects group scheduling: we now track the runnable
average on a per-task entity basis, allowing a smoother, exponential
decay average based load/weight estimation instead of the previous
binary on-the-runqueue/off-the-runqueue load weight method.
This will inevitably disturb workloads that were in some sort of
borderline balancing state or unstable equilibrium, so an eye has to
be kept on regressions.
For that reason the new load average is only limited to group
scheduling (shares distribution) at the moment (which was also hurting
the most from the prior, crude weight calculation and whose scheduling
quality wins most from this change) - but we plan to extend this to
regular SMP balancing as well in the future, which will simplify and
speed up things a bit.
Other changes involve ongoing preparatory work to extend NOHZ to the
scheduler as well, eventually allowing completely irq-free user-space
execution."

Add comments that dirty bit in storage key gets set whenever page content
is changed. Hopefully if someone will use this function, he'll have a

Benefit from pci store block instruction by writing up to 128 bytes
with a single instruction to MMIO space. Depending on the workload
this can result in a huge performance increase due to the reduced
number of instructions. The ordering guarantees of single stores
vs. one store block are identical.

The usual rules for open()/openat()/open_by_handle_at() are
1) native 32bit - don't force O_LARGEFILE in flags
2) native 64bit - force O_LARGEFILE in flags
3) compat on 64bit host - as for native 32bit
4) native 32bit ABI for 64bit system (mips/n32, x86/x32) - as for
native 64bit
There are only two exceptions - s390 compat has open() forcing
O_LARGEFILE and arm64 compat has open_by_handle_at() doing the same
thing. The same binaries on native host (s390/31 and arm resp.) will
*not* force O_LARGEFILE, so IMO both are emulation bugs.
Objections? The fix is obvious...

for every PCI function in stand-by or configured state. The PCI
functions are named after the PCI function ID (fid). By writing to
the power attribute in /sys/bus/pci/slots/<fid>/power the PCI function
is moved to stand-by or configured state. If moved to the configured
state the device is automatically scanned by the s390 PCI layer.

Support PCI adapter interrupts using the Single-IRQ-mode. Single-IRQ-mode
disables an adapter IRQ automatically after delivering it until the SIC
instruction enables it again. This is used to reduce the number of IRQs
for streaming workloads.
Up to 64 MSI handlers can be registered per PCI function.
A hash table is used to map interrupt numbers to MSI descriptors.
The interrupt vector is scanned using the flogr instruction.
Only MSI/MSI-X interrupts are supported, no legacy INTs.

The flogr instruction scans a bitmap starting from the leftmost bit.
Implement support for these bitops. This could be useful to scan
bitmaps like an interrupt vector set by the hardware starting
at the leftmost bit.

CLP instructions are used to query the firmware about detected PCI
functions, the attributes of those functions and to enable or disable
a PCI function. The CLP interface is the equivalent to a PCI bus scan.

I was chasing down a bug of random validity intercepts on s390.
(guest prefix page not mapped in the host virtual aspace). Turns out
that the problem was a wrong address space control element. The
cause was quite complex:
During paging activity a DAT protection during SIE caused a program
interrupt. Normally, the sie retry loop tries to catch all
interrupts during and shortly before sie to rerun the setup. The
problem is now that protection causes a suppressing program interrupt,
causing the PSW to point to the instruction AFTER SIE in case of DAT
protection. This confused the logic of the retry loop to not trigger,
instead we jumped directly back to SIE after return from
the program interrupt. (the protection fault handler itself did
a rewind of the psw). This usually works quite well, but:
If now the protection fault handler has to wait, another program
might be scheduled in. Later on the sie process will be schedules
in again. In that case the content of CR1 (primary address space)
will be wrong because switch_to will put the user space ASCE into CR1
and not the guest ASCE.
In addition the program parameter is also wrong for every protection
fault of a guest, since we dont issue the SPP instruction.
So lets also check for PSW == instruction after SIE in the program
check handler. Instead of expensively checking all program
interruption codes that might be suppressing we assume that a program
interrupt pointing after SIE was always a program interrupt in SIE.
(Otherwise we have a kernel bug anyway).
We also have to compensate the rewinding, since the C-level handlers
will do that. Therefore we need to add a nop with the same length
as SIE before the sie_loop.

The current single step code is racy in regard to concurrent delivery
of signals. If a signal is delivered after a PER program check occurred
but before the TIF_PER_TRAP bit has been checked in entry[64].S the code
clears TIF_PER_TRAP and then calls do_signal. This is wrong, if the
instruction completed (or has been suppressed) a SIGTRAP should be
delivered to the debugger in any case. Only if the instruction has been
nullified the SIGTRAP may not be send.
The new logic always sets TIF_PER_TRAP if the program check indicates PER
tracing but removes it again for all program checks that are nullifying.
The effect is that for each change in the PSW address we now get a
single SIGTRAP.

Allow to generate code that only runs on zEC12 machines.
Also add a check which prevents the kernel to run on machines which
do not have any of the following new facilities installed:
- (48) decimal-floating-point zoned-conversion
- (49) execution-hint
- (49) load-and-trap
- (49) miscellaneous-instruction-extensions
- (49) processor-assist
- (50) constrained transactional-execution
- (73) transactional-execution
48, 49, 50 and 73 are the bit numbers of the facility indications for
each of the required facilities.
Note that we assume that user-space gets compiled with the same
compiler options, therefore we also test for a dfp facility even
if the kernel doesn't make use of it.

Mainly merge all different per-cpu arrays into a single array which
holds all topology information per logical cpu.
Also fix the broken core vs socket variable naming and simplify the
locking a bit.
When running in environments without topology information also
invent book, socket and core ids, so that not all ids are zero.

Preinitialize the program check table, so we can put it into the
read-only data section.
Also use only four byte entries for the table, since each program
check handler resides within the first 2GB. Therefore this reduces
the size of the table by 50% on 64 bit builds.

Use 1MB frames for vmemmap if EDAT1 is available in order to
reduce TLB pressure
Always use a 1MB frame even if its only partially needed for
struct pages. Otherwise we would end up with a mix of large
frame and page mappings, because vmemmap_populate gets called
for each section (256MB -> 3.5MB memmap) separately.
Worst case is that we would waste 512KB.

System time accounting APIs such as vtime_account_system() and
vtime_account_idle() need to be irqsafe. Current callers include
irq entry, exit and kvm, all of which have been checked against that
requirement. Now it's better to grow that with an automatic check
in case we have further callers or we missed something.

On ia64 and powerpc, vtime context switch only consists
in flushing system and user pending time, plus a few
arch housekeeping.
Consolidate that into a generic implementation. s390 is
a special case because pending user and system time accounting
there is hard to dissociate. So it's keeping its own implementation.

All vtime implementations just flush the user time on process
tick. Consolidate that in generic code by calling a user time
accounting helper. This avoids an indirect call in ia64 and
prepare to also consolidate vtime context switch code.

Prepending irq-unsafe vtime APIs with underscores was actually
a bad idea as the result is a big mess in the API namespace that
is even waiting to be further extended. Also these helpers
are always called from irq safe callers except kvm. Just
provide a vtime_account_system_irqsafe() for this specific
case so that we can remove the underscore prefix on other
vtime functions.

In order to promote interoperability between userspace tracers and ftrace,
add a trace_clock that reports raw TSC values which will then be recorded
in the ring buffer. Userspace tracers that also record TSCs are then on
exactly the same time base as the kernel and events can be unambiguously
interlaced.
Tested: Enabled a tracepoint and the "tsc" trace_clock and saw very large
timestamp values.

Select HAVE_ALIGNED_STRUCT_PAGE on s390, so that the slub allocator can make
use of compare and swap double for lockless updates. This increases the size
of struct page to 64 bytes (instead of 56 bytes), however the performance gain
justifies the increased size:
- now excactly four struct pages fit into a single cache line; the
case that accessing a struct page causes two cache line loads
does not exist anymore.
- calculating the offset of a struct page within the memmap array
is only a simple shift instead of a more expensive multiplication.
A "hackbench 200 process 200" run on a 32 cpu system did show an 8% runtime
improvement.

invocations are rather pointless.
However when walking page tables we need to make sure that everything
is within bounds of the ASCE limit of the task's address space.
So remove the access_ok() call and add the same check we have in
get_user_pages_fast().

When walking page tables we need to make sure that everything
is within bounds of the ASCE limit of the task's address space.
Otherwise we might calculate e.g. a pud pointer which is not
within a pud and dereference it.
So check against TASK_SIZE (which is the ASCE limit) before
walking page tables.

The current topology code confuses core id vs physical package id.
In other words /sys/devices/system/cpu/cpuX/topology/core_id
displays the physical_package_id (aka socket id) instead of the
core id.
The physical_package_id sysfs attribute always displays "-1"
instead of the socket id.
Fix this mix-up with a small patch which defines and initializes
topology_physical_package_id correctly and fixes the broken
core id handling.

If user space is running in primary mode it can switch to secondary
or access register mode, this is used e.g. in the clock_gettime code
of the vdso. If a signal is delivered to the user space process while
it has been running in access register mode the signal handler is
executed in access register mode as well which will result in a crash
most of the time.
Set the address space control bits in the PSW to the default for the
execution of the signal handler and make sure that the previous
address space control is restored on signal return. Take care
that user space can not switch to the kernel address space by
modifying the registers in the signal frame.

The early mini sclp driver may be called in zArch mode either in
31 or 64 bit addressing mode.
If called in 31 bit addressing mode the new external interrupt psw
however would switch to 64 bit addressing mode. This would cause an
addressing exception within the interrupt handler, since the code
didn't expect the zArch/31 bit addressing mode combination.
Fix this by setting the new psw addressing mode bits so they fit
the current addressing mode.

The SO_ATTACH_FILTER option is set only. I propose to add the get
ability by using SO_ATTACH_FILTER in getsockopt. To be less
irritating to eyes the SO_GET_FILTER alias to it is declared. This
ability is required by checkpoint-restore project to be able to
save full state of a socket.
There are two issues with getting filter back.
First, kernel modifies the sock_filter->code on filter load, thus in
order to return the filter element back to user we have to decode it
into user-visible constants. Fortunately the modification in question
is interconvertible.
Second, the BPF_S_ALU_DIV_K code modifies the command argument k to
speed up the run-time division by doing kernel_k = reciprocal(user_k).
Bad news is that different user_k may result in same kernel_k, so we
can't get the original user_k back. Good news is that we don't have
to do it. What we need to is calculate a user2_k so, that
reciprocal(user2_k) == reciprocal(user_k) == kernel_k
i.e. if it's re-loaded back the compiled again value will be exactly
the same as it was. That said, the user2_k can be calculated like this
user2_k = reciprocal(kernel_k)
with an exception, that if kernel_k == 0, then user2_k == 1.
The optlen argument is treated like this -- when zero, kernel returns
the amount of sock_fprog elements in filter, otherwise it should be
large enough for the sock_fprog array.

Switching to or from guest context is done on ioctl context.
So by the time we call kvm_guest_enter() or kvm_guest_exit()
we know we are not running the idle task.
As a result, we can directly account the cputime using
vtime_account_system().
There are two good reasons to do this:
* We avoid some useless checks on guest switch. It optimizes
a bit this fast path.
* In the case of CONFIG_IRQ_TIME_ACCOUNTING, calling vtime_account()
checks for irq time to account. This is pointless since we know
we are not in an irq on guest switch. This is wasting cpu cycles
for no good reason. vtime_account_system() OTOH is a no-op in
this config option.
* We can remove the irq disable/enable around kvm guest switch in s390.
A further optimization may consist in introducing a vtime_account_guest()
that directly calls account_guest_time().

vtime_account_system() currently has only one caller with
vtime_account() which is irq safe.
Now we are going to call it from other places like kvm where
irqs are not always disabled by the time we account the cputime.
So let's make it irqsafe. The arch implementation part is now
prefixed with "__".
vtime_account_idle() arch implementation is prefixed accordingly
to stay consistent.

Without CONFIG_HUGETLB_PAGE, pmd_huge() will always return 0. So

It turns out that S390 shouldn't use uapi/asm-generic/kvm_para.h since it
defines the functions in its non-UAPI kvm_para.h file.
#including <asm-generic/kvm_para.h> will first pick up
include/asm-generic/kvm_para.h, which defines conflicting functions.
Instead, partially revert commit 0420c87e64,
ungenericising this file and just inserting a comment to prevent the patch
program from deleting it.

The sysfs and procfs output of the instruction and data caches were
wrong: the output of the data cache provided that instruction cache
values and vice versa.
Fix this by using the correct type indication when issueing the
ecag instruction.

Because of a change in the s390 arch backend of binutils (commit 23ecd77
"Pick the default arch depending on the target size" in binutils repo)
31 bit builds will fail since the linker would now try to create 64 bit
binary output.
Fix this by setting OUTPUT_ARCH to s390:31-bit instead of s390.
Thanks to Andreas Krebbel for figuring out the issue.

Make uapi/asm-generic/kvm_para.h non-empty by addition of a comment to stop
the patch program from deleting it when it creates it.
Then delete empty arch-specific uapi/asm/kvm_para.h files and tell the Kbuild
files to use the generic instead.
Should this perhaps instead be a #warning or #error that the facility is
unsupported on this arch?

Newer kernels (linux-next with the transparent huge page patches)
use rrbm if the feature is announced via feature bit 66.
RRBM will cause intercepts, so KVM does not handle it right now,
causing an illegal instruction in the guest.

The load of the svc number in the TIF_SYSCALL restart path needs to be
done with an instruction that loads all 64 bits of %r1, 'lh' only loads
32 bits. If the upper half of %r1 is not zero and has the msb set,
entry64.S will try to execute an svc with a really large number.
What will be in the upper half of %r1 depends on the code generated by
gcc for the functions on the do_signal() callchain.

vmem_add_mem() should only then insert a large page if pmd_none() is true
for the specific entry. We might have a leftover from a previous mapping.
In addition make vmem_remove_range()'s page table walk code more complete
and fix a couple of potential endless loops (which can never happen :).

Add a special module area on top of the vmalloc area, which may be only
used for modules and bpf jit generated code.
This makes sure that inter module branches will always happen without a
trampoline and in addition having all the code within a 2GB frame is
branch prediction unit friendly.

Let the kernel text section always begin at 1MB. This allows to always have
a large frame in the identity mapping of the kernel image for beginning of
the text section, if the machine has EDAT1 support.
Moving the beginning from 64K to 1MB doesn't cost any memory, since we make
the memory between 64K and 1MB available for the page allocator.

Within the identity mapping the kernel text section is mapped read-only.
However when mapping the first and last page of the text section we must
round upwards and downwards respectively, if only parts of a page belong
to the section.
Otherwise potential rw data can be mapped read-only. So the rounding must
be done just the other way we have it right now.

This usually makes only sense for kernel developers. The output
with CONFIG_DEBUG_PAGEALLOC is not very helpful, because of the
huge number of mapped out pages, however I decided for the time
being to not add a !DEBUG_PAGEALLOC dependency.
Maybe it's helpful for somebody even with that option.

Do the switch to z/Architecture (alias 64 bit) mode early in head.S.
If the machine is already running in 64 bit mode the sigp turns into
a nop. With this change it doesn't matter in which mode the kernel
is started.

The current page table walk code in pageattr.c only checks for large pages
while walking the kernel page table, but happily assumes that everything
else is just fine.
Add more checks so we never access invalid memory regions.

pmd_huge() will always return 0 on !HUGETLBFS, however we use that helper
function when walking the kernel page tables to decide if we have a
1MB page frame or not.
Since we create 1MB frames for the kernel 1:1 mapping independently of
HUGETLBFS this can lead to incorrect storage accesses since the code
can assume that we have a pointer to a page table instead of a pointer
to a 1MB frame.
Fix this by adding a pmd_large() primitive like other architectures have
it already and remove all references to HUGETLBFS/HUGETLBPAGE from the
code that walks kernel page tables.

MACHINE_HAS_PFMF and MACHINE_HAS_HPAGE actually have the same semantics:
the cpu has the EDAT1 facility installed in zArch mode. So remove one
of the feature flags in machine_flags and rename the other one to EDAT1.
The two old macros simply get mapped to MACHINE_HAS_EDAT1.

The transparent huge page code passes a PMD pointer in as the third
argument of update_mmu_cache(), which expects a PTE pointer.
This never got noticed because X86 implements update_mmu_cache() as a
macro and thus we don't get any type checking, and X86 is the only
architecture which supports transparent huge pages currently.
Before other architectures can support transparent huge pages properly we
need to add a new interface which will take a PMD pointer as the third
argument rather than a PTE pointer.

.fault now can retry. The retry can break state machine of .fault. In
filemap_fault, if page is miss, ra->mmap_miss is increased. In the second
try, since the page is in page cache now, ra->mmap_miss is decreased. And
these are done in one fault, so we can't detect random mmap file access.
Add a new flag to indicate .fault is tried once. In the second try, skip
ra->mmap_miss decreasing. The filemap_fault state machine is ok with it.
I only tested x86, didn't test other archs, but looks the change for other
archs is obvious, but who knows :)

This patch is part of the architecture backend for thp on s390. It
disables thp for kvm hosts, because there is no kvm host hugepage support
so far. Existing thp mappings are split by follow_page() with FOLL_SPLIT,
and future thp mappings are prevented by setting VM_NOHUGEPAGE in
mm->def_flags.

This patch is part of the architecture backend for thp on s390. It
provides the pagetable pre-allocation functions
pgtable_trans_huge_deposit() and pgtable_trans_huge_withdraw(). Unlike
other archs, s390 has no struct page * as pgtable_t, but rather a pointer
to the page table. So instead of saving the pagetable pre- allocation
list info inside the struct page, it is being saved within the pagetable
itself.

This patch is part of the architecture backend for thp on s390. It
provides the functions related to thp splitting, including serialization
against gup. Unlike other archs, pmdp_splitting_flush() cannot use a tlb
flushing operation to serialize against gup on s390, because that wouldn't
be stopped by the disabled IRQs. So instead, smp_call_function() is
called with an empty function, which will have the expected effect.

The core page allocator ensures that page flags are zeroed when freeing
pages via free_pages_check. A number of architectures (ARM, PPC, MIPS)
rely on this property to treat new pages as dirty with respect to the data
cache and perform the appropriate flushing before mapping the pages into
userspace.
This can lead to cache synchronisation problems when using hugepages,
since the allocator keeps its own pool of pages above the usual page
allocator and does not reset the page flags when freeing a page into the
pool.
This patch adds a new architecture hook, arch_clear_hugepage_flags, so
that architectures which rely on the page flags being in a particular
state for fresh allocations can adjust the flags accordingly when a page
is freed into the pool.

This is a preparatory patch for the introduction of NT_SIGINFO elf note.
Make the location of compat_siginfo_t uniform across eight architectures
which have it. Now it can be pulled in by including asm/compat.h or
linux/compat.h.
Most of the copies are verbatim. compat_uid[32]_t had to be replaced by
__compat_uid[32]_t. compat_uptr_t had to be moved up before
compat_siginfo_t in asm/compat.h on a several architectures (tile already
had it moved up). compat_sigval_t had to be relocated from linux/compat.h
to asm/compat.h.

UAPI: Refer to the DRM UAPI headers with <...> and from certain headers only
Ease the deployment of clkdev by providing a default asm/clkdev.h for
use if the arch does not have an include/asm/clkdev.h.
Due to limitations in Kbuild we manually add clkdev.h to all
architectures that don't have one rather than having the header appear
by default.

Pull vfs update from Al Viro:
- big one - consolidation of descriptor-related logics; almost all of
that is moved to fs/file.c
(BTW, I'm seriously tempted to rename the result to fd.c. As it is,
we have a situation when file_table.c is about handling of struct
file and file.c is about handling of descriptor tables; the reasons
are historical - file_table.c used to be about a static array of
struct file we used to have way back).
A lot of stray ends got cleaned up and converted to saner primitives,
disgusting mess in android/binder.c is still disgusting, but at least
doesn't poke so much in descriptor table guts anymore. A bunch of
relatively minor races got fixed in process, plus an ext4 struct file
leak.
- related thing - fget_light() partially unuglified; see fdget() in
there (and yes, it generates the code as good as we used to have).
- also related - bits of Cyrill's procfs stuff that got entangled into
that work; _not_ all of it, just the initial move to fs/proc/fd.c and
switch of fdinfo to seq_file.
- Alex's fs/coredump.c spiltoff - the same story, had been easier to
take that commit than mess with conflicts. The rest is a separate
pile, this was just a mechanical code movement.
- a few misc patches all over the place. Not all for this cycle,
there'll be more (and quite a few currently sit in akpm's tree)."

Pull user namespace changes from Eric Biederman:
"This is a mostly modest set of changes to enable basic user namespace
support. This allows the code to code to compile with user namespaces
enabled and removes the assumption there is only the initial user
namespace. Everything is converted except for the most complex of the
filesystems: autofs4, 9p, afs, ceph, cifs, coda, fuse, gfs2, ncpfs,
nfs, ocfs2 and xfs as those patches need a bit more review.
The strategy is to push kuid_t and kgid_t values are far down into
subsystems and filesystems as reasonable. Leaving the make_kuid and
from_kuid operations to happen at the edge of userspace, as the values
come off the disk, and as the values come in from the network.
Letting compile type incompatible compile errors (present when user
namespaces are enabled) guide me to find the issues.
The most tricky areas have been the places where we had an implicit
union of uid and gid values and were storing them in an unsigned int.
Those places were converted into explicit unions. I made certain to
handle those places with simple trivial patches.
Out of that work I discovered we have generic interfaces for storing
quota by projid. I had never heard of the project identifiers before.
Adding full user namespace support for project identifiers accounts
for most of the code size growth in my git tree.
Ultimately there will be work to relax privlige checks from
"capable(FOO)" to "ns_capable(user_ns, FOO)" where it is safe allowing
root in a user names to do those things that today we only forbid to
non-root users because it will confuse suid root applications.
While I was pushing kuid_t and kgid_t changes deep into the audit code
I made a few other cleanups. I capitalized on the fact we process
netlink messages in the context of the message sender. I removed
usage of NETLINK_CRED, and started directly using current->tty.
Some of these patches have also made it into maintainer trees, with no
problems from identical code from different trees showing up in
linux-next.
After reading through all of this code I feel like I might be able to
win a game of kernel trivial pursuit."

Make default just return 0. The current default (checking
TIF_POLLING_NRFLAG) is taken to architectures that need it;
ones that don't do polling in their idle threads don't need
to defined TIF_POLLING_NRFLAG at all.
ia64 defined both TS_POLLING (used by its tsk_is_polling())
and TIF_POLLING_NRFLAG (not used at all). Killed the latter...

Use the mapping of Elf_[SPE]hdr, Elf_Addr, Elf_Sym, Elf_Dyn, Elf_Rel/Rela,
ELF_R_TYPE() and ELF_R_SYM() to either the 32-bit version or the 64-bit version
into asm-generic/module.h for all arches bar MIPS.
Also, use the generic definition mod_arch_specific where possible.
To this end, I've defined three new config bools:
(*) HAVE_MOD_ARCH_SPECIFIC
Arches define this if they don't want to use the empty generic
mod_arch_specific struct.
(*) MODULES_USE_ELF_RELA
Arches define this if their modules can contain RELA records. This causes
the Elf_Rela mapping to be emitted and allows apply_relocate_add() to be
defined by the arch rather than have the core emit an error message.
(*) MODULES_USE_ELF_REL
Arches define this if their modules can contain REL records. This causes
the Elf_Rel mapping to be emitted and allows apply_relocate() to be
defined by the arch rather than have the core emit an error message.
Note that it is possible to allow both REL and RELA records: m68k and mips are
two arches that do this.
With this, some arch asm/module.h files can be deleted entirely and replaced
with a generic-y marker in the arch Kbuild file.
Additionally, I have removed the bits from m32r and score that handle the
unsupported type of relocation record as that's now handled centrally.

Recent changes (KVM: make processes waiting on vcpu mutex killable)
now requires to check the return value of vcpu_load. This triggered
a warning in s390 specific kvm code. Turns out that we can actually
remove the put/load, since schedule will do the right thing via
the preempt notifiers.

We can have either shared kernel or jump label support, but not both.
If a kernel gets IPL'ed from an NSS it's not possible to patch the
text segment, since it's read-only.
Therefore any static branches cannot be updated. So we need to make
sure that shared kernel support is disabled if jump label support
is enabled.

The generic variant has a local_irq_save/restore pair which is quite
expensive. It is sufficient to disable preemption, which is a no-op
with !CONFIG_PREEMPT and then use the regular xchg macro.

Returning -ENOSYS on kexec_load() is a bad idea since user space cannot
tell if the system call is not implmented or if it failed.
Use -EOPNOTSUPP in case somebody tries a kexec_load on a NSS image based
kernel instead.

Change return code handling of the stsi() function:
In case function code 0 was specified the return value is the
current configuration level (already shifted). That way all
the code that actually copied the stsi_0() function can go
away.
Otherwise the return value is 0 (success) or negative to
indicate an error (currently only -EOPNOTSUPP).
Also stsi() is no longer an inline function. The function is
not performance critical, but every caller would generate an
exception table entry for this function.

The maximum nesting of the cpu topology is evaluated when /proc/sysinfo
is the first time read. This happens without a lock and a concurrent
reader on a different cpu can see and use an invalid intermediate value.
Besides the fact that this race is quite unlikely the worst thing that
could happen is that /proc/sysinfo would contain bogus information about
the machine's cpu topology.
Nevertheless this should be fixed. So move the detection code to the
early machine detection code and since now the value is early available
use it in the topology code as well.

The current proc implementation of the /proc/sysinfo file writes all
informations contained in all system information blocks to a single
page.
This is done by calling sprintf all the time in the expectation that
everything will fit into a single page. This however is not necessarily
true if the configuration of a machine is very large.
So convert /proc/sysinfo to avoid writing into random memory regions.
For readability reasons a couple of lines are longer than 80 characters.

Any change to sysinfo.h causes a whole kernel recompile since sysinfo.h is
included by topology.h, which again is used nearly everywhere.
So remove that include and add a forward declaration instead.

Renaming the globally visible variable "user_mode" to "addressing_mode" in
order to fix a name clash was not a good idea. (Commit 37fe1d73 "s390/mm:
rename user_mode variable to addressing_mode")
Looking at the code after a couple of weeks one thinks: addressing mode of
what?
So rename the variable again. This time to s390_user_mode. Which hopefully
makes more sense.

Change the default addressing mode so that user space runs in primary space
and the kernel runs in home space.
In addition remove the "switch_amode" kernel parameter so all users who
already specified they want the new default behaviour will stay in the
"switched" mode instead of in the opposite they intended.
If there is a need to switch addressing modes, this can be done with the
"user_mode" kernel parameter: user_mode=home

When a sigp instruction is issued it may store a status. This status is
currently stored in a per cpu field of the target cpu.
If multiple cpus issue a sigp instruction with the same target cpu
and a status is stored the result is not necessarily as expected.
Currently this is not an issue:
- on cpu hotplug no sigps, except "restart" and "sense" are sent to the
target cpu.
- on external call we don't look at the status if it is stored
- on sense running the condition code "status stored" is sufficient to
tell if a cpu is running or not

Add a line for each cpu cache to /proc/cpuinfo.
Since we only have information of private cpu caches in sysfs we
add a line for each cpu cache in /proc/cpuinfo which will also
contain information about shared caches.
For a z196 machine /proc/cpuinfo now looks like:

Allow user-space processes to use transactional execution (TX).
If the TX facility is available user space programs can use
transactions for fine-grained serialization based on the data
objects that are referenced during a transaction. This is
useful for lockless data structures and speculative compiler
optimizations.

Allow user-space threads to use runtime instrumentation (RI). To enable RI
for a thread there is a new s390 specific system call, sys_s390_runtime_instr,
that takes as parameter a realtime signal number. If the RI facility is
available the system call sets up a control block for the calling thread with
the appropriate permissions for the thread to modify the control block.
The user-space thread can then use the store and modify RI instructions to
alter the control block and start/stop the instrumentation via RION/RIOFF.
If the user specified program buffer runs full RI triggers an external
interrupt. The external interrupt is translated to a real-time signal that
is delivered to the thread that enabled RI on that CPU. The number of
the real-time signal is the number specified in the RI system call. So,
user-space can select any available real-time signal number in case the
application itself uses real-time signals for other purposes.
The kernel saves the RI control blocks on task switch only if the running
thread was enabled for RI. Therefore, the performance impact on task switch
should be negligible if RI is not used.
RI is only enabled for user-space mode and is disabled for the supervisor
state.

Expose cpu cache topology via sysfs.
The created sysfs directory structure is compatible to what x86, ia64
and powerpc have.
On s390 we expose only information about cpu caches which are private
to a cpu via sysfs . Caches which are shared between cpus do not have
a sysfs representation.
The reason for that is that the file "shared_cpu_map" is mandatory
and only if running under LPAR it is possible to tell which cpus
share which cache. Second level hypervisors however do not and cannot
expose that information to guests.
In order to have a consistent view we made the choice to always only
expose information about private cpu caches via sysfs.

So far, large page support was completely disabled with
CONFIG_DEBUG_PAGEALLOC, although it would be sufficient if only the
large page kernel mapping was disabled. This patch enables large page
support with CONFIG_DEBUG_PAGEALLOC, while it prevents the large kernel
mapping in that case.

Our memcpy and memcmp variants were implemented by calling the corresponding
gcc builtin variants.
However gcc is free to replace a call to __builtin_memcmp with a call to memcmp
which, when called, will result in an endless recursion within memcmp.
So let's provide asm variants and also fix the variants that are used for
uncompressing the kernel image.
In addition remove all other occurences of builtin function calls.

Move the code that finds out to which context we account the
cputime into generic layer.
Archs that consider the whole time spent in the idle task as idle
time (ia64, powerpc) can rely on the generic vtime_account()
and implement vtime_account_system() and vtime_account_idle(),
letting the generic code to decide when to call which API.
Archs that have their own meaning of idle time, such as s390
that only considers the time spent in CPU low power mode as idle
time, can just override vtime_account().

Use a naming based on vtime as a prefix for virtual based
cputime accounting APIs:
- account_system_vtime() -> vtime_account()
- account_switch_vtime() -> vtime_task_switch()
It makes it easier to allow for further declension such
as vtime_account_system(), vtime_account_idle(), ... if we
want to find out the context we account to from generic code.
This also make it better to know on which subsystem these APIs
refer to.

The s390 page-table walk code, used for user copy and futex, currently
cannot handle huge pages. As far as user copy is concerned, that is
not really a problem because those functions will only be used on old
hardware that has no huge page support. But the futex code will also
use pagetable walk functions on current hardware when user space runs
in primary space mode. So, if a futex sits in a huge page, the futex
operation on it will result in a page fault loop or even data
corruption.
This patch adds the code for resolving huge page mappings in the user
access pagetable walk code on s390.

huge_ptep_get_and_clear() is either missing a TLB invalidation or
an mm->context.attach_count check. Since the attach_count logic was
introduced with normal ptes in mind, let's just use direct TLB
flushing for hugetlbfs pages.

git commit cd2934a3 moved the flush_tlb_range() within
__unmap_hugepage_range() inside the mm->page_table_lock, which
triggered a deadlock in s390 tlb flushing code. __tlb_flush_mm_cond()
also tries to acquire the mm->page_table_lock, but that is not needed
because all callers already have mm->mmap_sem or mm->page_table_lock,
so it can be safely removed to fix the deadlock.

On s390x-linux-gcc, __SIZE_TYPE__ expands to "long unsigned int" for both
32-bit s390 and 64-bit s390x, as
gcc-4.6.3-nolibc/s390x-linux/lib/gcc/s390x-linux/4.6.3/plugin/include/config/s390/linux.h
has
#define SIZE_TYPE (TARGET_64BIT ? "long unsigned int" : "long unsigned int")
To match this, __kernel_size_t is always set to "long unsigned int".
But while __kernel_ssize_t is "long" on 64-bit s390x, it is "int" on 32-bit
s390, causing compiler warnings like:
fs/quota/quota_tree.c:372:4: warning: format '%zd' expects argument of type 'signed size_t', but argument 4 has type 'ssize_t' [-Wformat]
To fix this, __kernel_ssize_t should be "long", irrespective of word size.

The archs that implement virtual cputime accounting all
flush the cputime of a task when it gets descheduled
and sometimes set up some ground initialization for the
next task to account its cputime.
These archs all put their own hooks in their context
switch callbacks and handle the off-case themselves.
Consolidate this by creating a new account_switch_vtime()
callback called in generic code right after a context switch
and that these archs must implement to flush the prev task
cputime and initialize the next task cputime related state.

The native 31 bit and the compat behaviour for the mmap system calls differ:
In native 31 bit mode the passed in address for the mmap system call will be
unmodified passed to sys_mmap_pgoff().
In compat mode however the passed in address will be modified with
compat_ptr() which masks out the most significant bit.
The result is that in native 31 bit mode each mmap request (with MAP_FIXED)
will fail where the most significat bit is set, while in compat mode it
may succeed.
This odd behaviour was introduced with d3815898 "[S390] mmap: add missing
compat_ptr conversion to both mmap compat syscalls".
To restore a consistent behaviour accross native and compat mode this
patch functionally reverts the above mentioned commit.

There are multiple errors in how sys_32_personality() handles personality
flags stored in top three bytes.
- directly comparing current->personality against PER_LINUX32 doesn't work
in cases when any of the personality flags stored in the top three bytes
are used.
- directly forcefully setting personality to PER_LINUX32 or PER_LINUX
discards any flags stored in the top three bytes
Fix the first one by properly using personality() macro to compare only
PER_MASK bytes.
Fix the second one by setting only the bits that should be set, instead of
overwriting the whole value.

Initialization of cra_list is currently mixed, most ciphers initialize this
field and most shashes do not. Initialization however is not needed at all
since cra_list is initialized/overwritten in __crypto_register_alg() with
list_add(). Therefore perform cleanup to remove all unneeded initializations
of this field in 'arch/s390/crypto/'

Rather than #define the options manually in the architecture code, add

Make sure the kernel does not incorrectly create a SIGBUS signal during
user space accesses:
For user space accesses in the switched addressing mode case the kernel
may walk page tables and access user address space via the kernel
mapping. If a page table entry is invalid the function __handle_fault()
gets called in order to emulate a page fault and trigger all the usual
actions like paging in a missing page etc. by calling handle_mm_fault().
If handle_mm_fault() returns with an error fixup handling is necessary.
For the switched addressing mode case all errors need to be mapped to
-EFAULT, so that the calling uaccess function can return -EFAULT to
user space.
Unfortunately the __handle_fault() incorrectly calls do_sigbus() if
VM_FAULT_SIGBUS is set. This however should only happen if a page fault
was triggered by a user space instruction. For kernel mode uaccesses
the correct action is to only return -EFAULT.
So user space may incorrectly see SIGBUS signals because of this bug.
For current machines this would only be possible for the switched
addressing mode case in conjunction with futex operations.

The downgrade of the 4 level page table created by init_new_context is
currently done only in start_thread31. If a 31 bit process forks the
new mm uses a 4 level page table, including the task size of 2<<42
that goes along with it. This is incorrect as now a 31 bit process
can map memory beyond 2GB. Define arch_dup_mmap to do the downgrade
after fork.

Currently the vmcmd shutdown action is parsed by the kernel and
if multiple cp commands have been specified, they are issued
separately with the cpcmd() function.
The underlying diagnose 8 instruction already allows to specify
multiple commands that are separated by 0x15. The ASCEBC() function
used by cpcmd() translates '\n' to 0x15. The '\n' character is
currently used as vmcmd command separator and therefore the vmcmd
string can be passed directly to the cpcmd() function.
Using the diagnose 8 command separation has the advantage that also
after disruptive commands that stop Linux, for example "def store",
additional commands can be executed.

The current virtual timer interface is inherently per-cpu and hard to
use. The sole user of the interface is appldata which uses it to execute
a function after a specific amount of cputime has been used over all cpus.
Rework the virtual timer interface to hook into the cputime accounting.
This makes the interface independent from the CPU timer interrupts, and
makes the virtual timers global as opposed to per-cpu.
Overall the code is greatly simplified. The downside is that the accuracy
is not as good as the original implementation, but it is still good enough
for appldata.

Remove the file name from the comment at top of many files. In most
cases the file name was wrong anyway, so it's rather pointless.
Also unify the IBM copyright statement. We did have a lot of sightly
different statements and wanted to change them one after another
whenever a file gets touched. However that never happened. Instead
people start to take the old/"wrong" statements to use as a template
for new files.
So unify all of them in one go.

If lgr has not been initialized, the lgr_info_log() function currently
crashes because 'lgr_page' is not allocated. To fix this 'lgr_page'

The s390 idle accounting code uses a sequence counter which gets used
when the per cpu idle statistics get updated and read.
One assumption on read access is that only when the sequence counter is
even and did not change while reading all values the result is valid.
On cpu hotplug however the per cpu data structure gets initialized via
a cpu hotplug notifier on CPU_ONLINE.
CPU_ONLINE however is too late, since the onlined cpu is already running
and might access the per cpu data. Worst case is that the data structure
gets initialized while an idle thread is updating its idle statistics.
This will result in an uneven sequence counter after an update.
As a result user space tools like top, which access /proc/stat in order
to get idle stats, will busy loop waiting for the sequence counter to
become even again, which will never happen until the queried cpu will
update its idle statistics again. And even then the sequence counter
will only have an even value for a couple of cpu cycles.
Fix this by moving the initialization of the per cpu idle statistics
to cpu_init(). I prefer that solution in favor of changing the
notifier to CPU_UP_PREPARE, which would be a different solution to
the problem.

With git commit 85fe4025c6 ("fs: do not
assign default i_ino in new_inode") the get_next_ino() function was
moved from the new_inode() to the different file system implementations.
Unfortunately hypfs has been overlooked by the author. With this
patch the get_next_ino() is added also to hypfs.

If sigp sense doesn't have any status bits to report, it should set
cc 0 and leave the register as-is.
Since we know about the external call pending bit, we should report

If an invalid parameter is passed or the addressed cpu is in an
incorrect state sigp set prefix will store a status.
This status must only have bits set as defined by the architecture.
The current kvm implementation missed to clear bits and also did
not set the intended status bit ("and" instead of "or" operation).

Only if the sensed cpu is not running a status is stored, which
is reflected by condition code 1. If the cpu is running, condition
code 0 should be returned.
Just the opposite of what the code is doing.

Setting the cpu restart parameters is done in three different fashions:
- directly setting the four parameters individually
- copying the four parameters with memcpy (using 4 * sizeof(long))
- copying the four parameters using a private structure
In addition code in entry*.S relies on a certain order of the restart
members of struct _lowcore.
Make all of this more robust to future changes by adding a
mem_absolute_assign(dest, val) define, which assigns val to dest
using absolute addressing mode. Also the load multiple instructions
in entry*.S have been split into separate load instruction so the
order of the struct _lowcore members doesn't matter anymore.
In addition move the prototypes of memcpy_real/absolute from uaccess.h
to processor.h. These memcpy* variants are not related to uaccess at all.
string.h doesn't seem to match as well, so lets use processor.h.
Also replace the eight byte array in struct _lowcore which represents a
misaliged u64 with a u64. The compiler will always create code that
handles the misaligned u64 correctly.

Commit 98971f8439 ("crypto: s390 - cleanup

The initial cpu reset sets the cpu in the stopped state.
Several places check for the cpu state (e.g. sigp set prefix) and
not setting the STOPPED state triggered errors with newer guest
kernels after reboot.

The smp and the kvm code have different defines for the sigp order codes.
Let's just have a single place where these are defined.
Also move the sigp condition code and sigp cpu status bits to the new
sigp.h header file.

Does block_sigmask() + tracehook_signal_handler(); called when
sigframe has been successfully built. All architectures converted
to it; block_sigmask() itself is gone now (merged into this one).
I'm still not too happy with the signature, but that's a separate
story (IMO we need a structure that would contain signal number +
siginfo + k_sigaction, so that get_signal_to_deliver() would fill one,
signal_delivered(), handle_signal() and probably setup...frame() -
take one).

On s390 access_ok is a macro which discards all parameters and always
returns 1. This can result in compile warnings which warn about unused
variables like this:

Fix this by adding a __range_ok() function which consumes all parameters
but still always returns 1.

Now that hopefully all cmpxchg/xchg bugs have been fixed select
HAVE_CMPXCHG_LOCAL option which uncovered a couple of bugs on s390.
The only call site which is affected seems to be within mm/vmstat.c.

For 1 and 2 byte operands for xchg and cmpxchg the old and new values
get or'ed into the larger 4 byte old value before the compare and swap
instruction gets executed. This is done without using the proper byte
mask before or'ing the values.
If the caller passed in negative old or new values these got sign
extended by the caller. Which in turn means that either the old value
never matches, or, even worse, unrelated bytes would be changed in memory.
Luckily there don't seem to be any callers around yet, since that would
have resulted in the specification exception fixed in an earlies patch.

When accessing a 1 or 2 byte memory operand we cannot use the
passed address since the compare and swap instruction only works
for 4 byte aligned memory operands.
Hence we calculate an aligned address so that compare and swap works
correctly. However we don't pass the calculated address to the inline
assembly. This results in incorrect memory accesses and in a
specification exception if used on non 4 byte aligned memory operands.
Since this didn't happen until now, there don't seem to be
too many users of cmpxchg on unaligned addresses.
