73b0140bf0 mm/gup: change GUP fast to use flags rather than a write 'bool'
To facilitate additional options to get_user_pages_fast() change the
singular write parameter to be gup_flags.

This patch does not change any functionality. New functionality will
follow in subsequent patches.

Some of the get_user_pages_fast() call sites were unchanged because they
already passed FOLL_WRITE or 0 for the write parameter.

NOTE: It was suggested to change the ordering of the get_user_pages_fast()
arguments to ensure that callers were converted. This breaks the current
GUP call site convention of having the returned pages be the final
parameter. So the suggestion was rejected.

96d4f267e4 Remove 'type' argument from access_ok() function
Nobody has actually used the type (VERIFY_READ vs VERIFY_WRITE) argument
of the user address range verification function since we got rid of the
old racy i386-only code to walk page tables by hand.

It existed because the original 80386 would not honor the write protect
bit when in kernel mode, so you had to do COW by hand before doing any
user access. But we haven't supported that in a long time, and these
days the 'type' argument is a purely historical artifact.

A discussion about extending 'user_access_begin()' to do the range
checking resulted this patch, because there is no way we're going to
move the old VERIFY_xyz interface to that model. And it's best done at
the end of the merge window when I've done most of my merges, so let's
just get this done once and for all.

This patch was mostly done with a sed-script, with manual fix-ups for
the cases that weren't of the trivial 'access_ok(VERIFY_xyz' form.

There were a couple of notable cases:

 - csky still had the old "verify_area()" name as an alias.

 - the iter_iov code had magical hardcoded knowledge of the actual
   values of VERIFY_{READ,WRITE} (not that they mattered, since nothing
   really used it)

 - microblaze used the type argument for a debug printout

but other than those oddities this should be a total no-op patch.

I tried to fix up all architectures, did fairly extensive grepping for
access_ok() uses, and the changes are trivial, but I may have missed
something. Any missed conversion should be trivially fixable, though.

aa563d7bca iov_iter: Separate type from direction and use accessor functions
In the iov_iter struct, separate the iterator type from the iterator
direction and use accessor functions to access them in most places.

Convert a bunch of places to use switch-statements to access them rather
then chains of bitwise-AND statements. This makes it easier to add further
iterator types. Also, this can be more efficient as to implement a switch
of small contiguous integers, the compiler can use ~50% fewer compare
instructions than it has to use bitwise-and instructions.

Further, cease passing the iterator type into the iterator setup function.
The iterator function can set that itself. Only the direction is required.

11924ba5e6 VMCI: Resource wildcard match fixed
When adding a VMCI resource, the check for an existing entry
would ignore that the new entry could be a wildcard. This could
result in multiple resource entries that would match a given
handle. One disastrous outcome of this is that the
refcounting used to ensure that delayed callbacks for VMCI
datagrams have run before the datagram is destroyed can be
wrong, since the refcount could be increased on the duplicate
entry. This in turn leads to a use after free bug. This issue
was discovered by Hangbin Liu using KASAN and syzkaller.

6da2ec5605 treewide: kmalloc() -> kmalloc_array()
The kmalloc() function has a 2-factor argument form, kmalloc_array(). This
patch replaces cases of:

        kmalloc(a * b, gfp)

with:
        kmalloc_array(a * b, gfp)

as well as handling cases of:

        kmalloc(a * b * c, gfp)

with:

        kmalloc(array3_size(a, b, c), gfp)

as it's slightly less ugly than:

        kmalloc_array(array_size(a, b), c, gfp)

This does, however, attempt to ignore constant size factors like:

        kmalloc(4 * 1024, gfp)

though any constants defined via macros get caught up in the conversion.

Any factors with a sizeof() of "unsigned char", "char", and "u8" were
dropped, since they're redundant.

The tools/ directory was manually excluded, since it has its own
implementation of kmalloc().

The Coccinelle script used for this was:

// Fix redundant parens around sizeof().

a9a08845e9 vfs: do bulk POLL* -> EPOLL* replacement
This is the mindless scripted replacement of kernel use of POLL*
variables as described by Al, done by this script:

    for V in IN OUT PRI ERR RDNORM RDBAND WRNORM WRBAND HUP RDHUP NVAL MSG; do
        L=`git grep -l -w POLL$V | grep -v '^t' | grep -v /um/ | grep -v '^sa' | grep -v '/poll.h$'|grep -v '^D'`
        for f in $L; do sed -i "-es/^\([^\"]*\)\(\<POLL$V\>\)/\\1E\\2/" $f; done
    done

with de-mangling cleanups yet to come.

NOTE! On almost all architectures, the EPOLL* constants have the same
values as the POLL* constants do. But they keyword here is "almost".
For various bad reasons they aren't the same, and epoll() doesn't
actually work quite correctly in some cases due to this on Sparc et al.

The next patch from Al will sort out the final differences, and we
should be all done.

b69339ba10 sched/headers: Prepare to remove spurious <linux/sched.h> inclusion dependencies
In the following patches we are going to remove various headers
from sched.h and other headers that sched.h includes.

To make those patches build cleanly prepare the scene by adding
dependencies to various files that learned to rely on those
to-be-removed dependencies.

These changes all make sense standalone: they add a header for
a data type that a particular .c or .h file is using.

5b5e0928f7 lib/vsprintf.c: remove %Z support
Now that %z is standartised in C99 there is no reason to support %Z.
Unlike %L it doesn't even make format strings smaller.

Use BUILD_BUG_ON in a couple ATM drivers.

In case anyone didn't notice lib/vsprintf.o is about half of SLUB which
is in my opinion is quite an achievement. Hopefully this patch inspires
someone else to trim vsprintf.c more.

eb94cd68ab VMCI: Doorbell create and destroy fixes
This change consists of two changes:

1) If vmci_doorbell_create is called when neither guest nor
   host personality as been initialized, vmci_get_context_id
   will return VMCI_INVALID_ID. In that case, we should fail
   the create call.
2) In doorbell destroy, we assume that vmci_guest_code_active()
   has the same return value on create and destroy. That may not
   be the case, so we may end up with the wrong refcount.
   Instead, destroy should check explicitly whether the doorbell
   is in the index table as an indicator of whether the guest
   code was active at create time.

09cbfeaf1a mm, fs: get rid of PAGE_CACHE_* and page_cache_{get,release} macros
PAGE_CACHE_{SIZE,SHIFT,MASK,ALIGN} macros were introduced *long* time
ago with promise that one day it will be possible to implement page
cache with bigger chunks than PAGE_SIZE.

This promise never materialized. And unlikely will.

We have many places where PAGE_CACHE_SIZE assumed to be equal to
PAGE_SIZE. And it's constant source of confusion on whether
PAGE_CACHE_* or PAGE_* constant should be used in a particular case,
especially on the border between fs and mm.

Global switching to PAGE_CACHE_SIZE != PAGE_SIZE would cause to much
breakage to be doable.

Let's stop pretending that pages in page cache are special. They are
not.

The changes are pretty straight-forward:

 - <foo> << (PAGE_CACHE_SHIFT - PAGE_SHIFT) -> <foo>;

 - <foo> >> (PAGE_CACHE_SHIFT - PAGE_SHIFT) -> <foo>;

 - PAGE_CACHE_{SIZE,SHIFT,MASK,ALIGN} -> PAGE_{SIZE,SHIFT,MASK,ALIGN};

 - page_cache_get() -> get_page();

 - page_cache_release() -> put_page();

This patch contains automated changes generated with coccinelle using
script below. For some reason, coccinelle doesn't patch header files.
I've called spatch for them manually.

The only adjustment after coccinelle is revert of changes to
PAGE_CAHCE_ALIGN definition: we are going to drop it later.

There are few places in the code where coccinelle didn't reach. I'll
fix them manually in a separate patch. Comments and documentation also
will be addressed with the separate patch.

f42a0fd13b VMCI: Use 32bit atomics for queue headers on X86_32
This change restricts the reading and setting of the head and tail
pointers on 32bit X86 to 32bit for both correctness and
performance reasons. On uniprocessor X86_32, the atomic64_read
may be implemented as a non-locked cmpxchg8b. This may result in
updates to the pointers done by the VMCI device being overwritten.
On MP systems, there is no such correctness issue, but using 32bit
atomics avoids the overhead of the locked 64bit operation. All this
is safe because the queue size on 32bit systems will never exceed
a 32bit value.

With well over 200+ users of this api, there are a mere 12 users that

So stop pretending like it matters, and just return void from

Other than that, there's nothing that can go wrong.

a1d88436d5 VMCI: Fix two UVA mapping bugs
(this is a resend of this patch. Originally sent last year, but post
 appears to have been lost)

This change fixes two bugs in the VMCI host driver related to mapping
the notify boolean from user space into kernel space:
- the actual UVA was rounded up to the next page boundary - resulting
  in memory corruption in the calling process whenever notifications
  would be signalled. This has been fixed by just removing the
  PAGE_ALIGN part, since get_user_pages_fast can figure this out on
  its own
- the mapped page wasn't stored anywhere, so it wasn't unmapped and
  put back when a VMCI context was destroyed. Fixed this by
  remembering the page.

94e57fea62 PCI: Move PCI_VENDOR_ID_VMWARE to pci_ids.h
Move PCI_VENDOR_ID_VMWARE from device-specific files to pci_ids.h.
It is useful to always have access to it, especially when accessing
subsystem_vendor_id on emulated devices.

e0117521e5 vmw_vmci: Use pci_enable_msix_exact() instead of pci_enable_msix()
As result of deprecation of MSI-X/MSI enablement functions
pci_enable_msix() and pci_enable_msi_block() all drivers
using these two interfaces need to be updated to use the
new pci_enable_msi_range()  or pci_enable_msi_exact()
and pci_enable_msix_range() or pci_enable_msix_exact()
interfaces.

782f244535 VMCI: fix error handling path when registering guest driver
When host capabilities check failed or when we were unable to register doorbell
bitmap we were forgetting to set error code and were returning 0 which would

Convert vmci_host_setup_notify() and qp_host_get_user_memory() to use
