d49f8a52b1 SCSI misc on 20181024
This is mostly updates of the usual drivers: UFS, esp_scsi, NCR5380,
 qla2xxx, lpfc, libsas, hisi_sas.  In addition there's a set of mostly
 small updates to the target subsystem a set of conversions to the
 generic DMA API, which do have some potential for issues in the older
 drivers but we'll handle those as case by case fixes. A new myrs for
 the DAC960/mylex raid controllers to replace the block based DAC960
 which is also being removed by Jens in this merge window. Plus the
 usual slew of trivial changes.

0153167aeb net/ipv6: Remove rt6i_prefsrc
After the conversion to fib6_info, rt6i_prefsrc has a single user that
reads the value and otherwise it is only set. The one reader can be
converted to use rt->from so rt6i_prefsrc can be removed, reducing
rt6_info by another 20 bytes.

Smatch marks skb->data as untrusted so it complains that there is a
potential overflow here:

	drivers/scsi/cxgbi/cxgb4i/cxgb4i.c:2111 t4_uld_rx_handler()
	error: buffer overflow 'cxgb4i_cplhandlers' 239 <= 255.

In this case, skb->data comes from the hardware or firmware so it's not
going to overflow unless there is a firmware bug.

841b86f328 treewide: Remove TIMER_FUNC_TYPE and TIMER_DATA_TYPE casts
With all callbacks converted, and the timer callback prototype
switched over, the TIMER_FUNC_TYPE cast is no longer needed,
so remove it. Conversion was done with the following scripts:

2269848386 scsi: libcxgbi: simplify task->hdr allocation for mgmt cmds
In case of mgmt cmds, task->hdr is dereferenced after transmitting the
pdu in iscsi_tcp_task_xmit(). To handle this case current code
increments the Tx skb reference count and frees the skb in
cxgbi_cleanup_task(). In some error cases this results in skb leak.

To fix this in case of mgmt cmds, allocate a separate buffer for iSCSI
hdr and free this buffer in cxgbi_cleanup_task().

In case of vlan pass 0 as ifindex to find route instead of passing
real_dev ifindex, if we pass real_dev ifindex then
ip_route_output_ports() and ip6_route_output() will check for route
through real_dev not through vlan interface.

It seems like a historic accident that these return unsigned char *,
and in many places that means casts are required, more often than not.

Make these functions return void * and remove all the casts across
the tree, adding a (u8 *) cast only where the unsigned char pointer
was used directly, all done with the following spatch:

1dec4cec9f cxgb4: Fix tids count for ipv6 offload connection
the adapter consumes two tids for every ipv6 offload
connection be it active or passive, calculate tid usage
count accordingly.

Also change the signatures of relevant functions to get
the address family.

e0f8e8cf3b scsi: cxgb4i: libcxgbi: in error case RST tcp conn
If logout response is not received and ->ep_disconnect() is called then
close tcp conn by RST instead of FIN to cleanup conn resources
immediately.

Also move ->csk_push_tx_frames() above 'done:' to avoid calling
->csk_push_tx_frames() in error cases.

skb->data is assigned to task->hdr in cxgbi_conn_alloc_pdu(),
skb gets freed after tx but task->hdr is still dereferenced in
iscsi_tcp_task_xmit() to avoid this call skb_get() after allocating skb
and free the skb in cxgbi_cleanup_task() or before allocating new skb in
cxgbi_conn_alloc_pdu().

b6a05c823f scsi: remove eh_timed_out methods in the transport template
Instead define the timeout behavior purely based on the host_template
eh_timed_out method and wire up the existing transport implementations
in the host templates.  This also clears up the confusion that the
transport template method overrides the host template one, so some
drivers have to re-override the transport template one.

Allocate resources dynamically for Upper layer driver's (ULD) like
cxgbit, iw_cxgb4, cxgb4i and chcr. The resources allocated include Tx
queues which are allocated when ULD register with cxgb4 driver and freed
while un-registering. The Tx queues which are shared by ULD shall be
allocated by first registering driver and un-allocated by last
unregistering driver.

3d6d30d60a cxgbi: fix uninitialized flowi6
ip6_route_output looks into different fields in the passed flowi6 structure,
yet cxgbi passes garbage in nearly all those fields. Zero the structure out
first.
