Properly account for I/O in transit before returning from the RESET call.
In the absense of this patch, we could have a situation where the host may
respond to a command that was issued prior to the issuance of the RESET
command at some arbitrary time after responding to the RESET command.
Currently, the host does not do anything with the RESET command.

Currently it is possible to unmap one more block than user requested to
due to the off-by-one error in unmap_region(). This is probably due to
the fact that the end variable despite its name actually points to the
last block to unmap + 1. However in the condition it is handled as the
last block of the region to unmap.
The bug was not previously spotted probably due to the fact that the
region was not zeroed, which has changed with commit
be1dd78de5. With that commit we were able
to corrupt the ext4 file system on 256M scsi_debug device with LBPRZ
enabled using fstrim.
Since the 'end' semantic is the same in several functions there this
commit just fixes the condition to use the 'end' variable correctly in
that context.

This patch tries to shorten the path length of scsi_cmd_to_driver(). As only
REQ_TYPE_BLOCK_PC commands can be submitted without a driver, so we could
avoid the related NULL checking, as long as we make sure we don't use it for
REQ_TYPE_BLOCK_PC type commands. Plus, this fixes a bug where you get
different behaviors from REQ_TYPE_BLOCK_PC commands when a driver is and isn't
attached.

Support the LUN parameter change event. Currently, the host fires this event
when the capacity of a disk is changed from the virtual machine monitor.
The resize then appears in the kernel log like this:

virtio-scsi needs to report LUNs greater than 256 using the "flat"
format. Because the Linux SCSI layer just maps the SCSI LUN to
an u32, without any parsing, these end up in the range from 16640
to 32767. Fix max_lun to account for the possibility that logical
unit numbers are encoded with the "flat" format.

This change limits the PF/VF driver to 9.5K max jumbo frame size in order
prevent a possible Tx hang in the adapter when sending frames between
pools.
All of the parts in ixgbe support a maximum frame of 15.5K for standard
traffic, however with SR-IOV or DCB enabled they should be limiting the
MTU size to 9.5K. Instead of adding extra checks which would have to
change the MTU when we go into or out of these modes it is preferred to
just use a standard 9.5K MTU limit for all modes so that this extra
overhead can be avoided.

The driver was not setting the number of real Tx queues in the net_device
structure. This caused some serious issues such as Tx hangs and extremely
poor performance with some usages of the driver.
The issue is best observed by running:
iperf -c <host> -P <n>
Where n is greater than one. The greater the value of n the more likely
the problem is to show up.

remove_memory() will be called when hot removing a memory device. But
even if offlining memory, we cannot notice it. So the patch updates the
memory block's state and sends notification to userspace.
Additionally, the memory device may contain more than one memory block.
If the memory block has been offlined, __offline_pages() will fail. So we
should try to offline one memory block at a time.
Thus remove_memory() also check each memory block's state. So there is no
need to check the memory block's state before calling remove_memory().

remove_memory() is called in two cases:
1. echo offline >/sys/devices/system/memory/memoryXX/state
2. hot remove a memory device
In the 1st case, the memory block's state is changed and the notification
that memory block's state changed is sent to userland after calling
remove_memory(). So user can notice memory block is changed.
But in the 2nd case, the memory block's state is not changed and the
notification is not also sent to userspcae even if calling
remove_memory(). So user cannot notice memory block is changed.
For adding the notification at memory hot remove, the patch just prepare
as follows:
1st case uses offline_pages() for offlining memory.
2nd case uses remove_memory() for offlining memory and changing memory block's
state and notifing the information.
The patch does not implement notification to remove_memory().

currently it lost original meaning but still has some effects:

Some security modules and oprofile still uses VM_EXECUTABLE for retrieving
a task's executable file. After this patch they will use mm->exe_file
directly. mm->exe_file is protected with mm->mmap_sem, so locking stays
the same.

Move actual pte filling for non-linear file mappings into the new special
vma operation: ->remap_pages().
Filesystems must implement this method to get non-linear mapping support,
if it uses filemap_fault() then generic_file_remap_pages() can be used.
Now device drivers can implement this method and obtain nonlinear vma support.

When transparent huge pages were introduced, memory compaction and swap
storms were an issue, and the kernel had to be careful to not make THP
allocations cause pageout or compaction.
Now that we have working compaction deferral, kswapd is smart enough to
invoke compaction and the quadratic behaviour around isolate_free_pages
has been fixed, it should be safe to remove __GFP_NO_KSWAPD.

OMAP PM core code has moved to using the existing, generic CPU devices
for attaching OPPs, so the CPUfreq driver can now use the generic
get_cpu_device() API instead of the OMAP-specific omap_device API.
This allows us to remove the last <plat/*> include from this driver.

OMAP core code now has SoC-independent clock alias for the scalable
CPU clock. Using it means driver is SoC independent and will work for
AM3xxx SoCs as well as OMAP1/3/4.
While here, remove some unnecessary plat/ includes that are
interfering with multi-subarch ARM kernels.

Ensure the clock rate that will be used is a valid one before
attempting to scale the voltage. Currently the driver assumes it has
a valid frequency from the OPP table, but boards using different
system oscillators might not have exact matches with the OPP table,
and result in a failing call to clk_set_rate().
This is particularily bad because the voltage may be scaled even
though the frequency is not. This will obviously lead to some
unpredictable behavior, especially if the frequency is high and
the voltage is dropped.

Driver gets LINK_LOST, DEAUTHENTICATED and DISASSOCIATED events
from firmware when connection is lost in different scenarios.
Currently we are using common code WLAN_REASON_DEAUTH_LEAVING
for these cases.
This patch adds support to parse an actual reason code from
firmware event body and send it to cfg80211.
WLAN_REASON_DEAUTH_LEAVING code is used if deauth is initiated
by our device.

scan_processing flag should be reset when scan request is failed
due to some reasons Ex. memory allocation failure etc. Otherwise

There are cases we cannot scan when request is received.
For example, during WPA group key negociation the scan request
will be blocked. We should return an error code to cfg80211
because cfg80211_scan_done will never be called.

Current GRO can hold packets in gro_list for almost unlimited
time, in case napi->poll() handler consumes its budget over and over.
In this case, napi_complete()/napi_gro_flush() are not called.
Another problem is that gro_list is flushed in non friendly way :
We scan the list and complete packets in the reverse order.
(youngest packets first, oldest packets last)
This defeats priorities that sender could have cooked.
Since GRO currently only store TCP packets, we dont really notice the
bug because of retransmits, but this behavior can add unexpected
latencies, particularly on mice flows clamped by elephant flows.
This patch makes sure no packet can stay more than 1 ms in queue, and
only in stress situations.
It also complete packets in the right order to minimize latencies.

Marvell 88E8001 on an ASUS P5NSLI motherboard is unable to send/receive
packets on a system with >4gb ram unless a 32bit DMA mask is used.
This issue has been around for years and a fix was sent 3.5 years ago, but
there was some debate as to whether it should instead be fixed as a PCI quirk.

Extend the amount of character devices, such as eventX, mouseX and jsX,
from a hard limit of 32 per input handler to about 1024 shared across
all handlers.
To be compatible with legacy installations input handlers will start
creating char devices with minors in their legacy range, however once
legacy range is exhausted they will start allocating minors from the
dynamic range 256-1024.

which stores the information about logical<->physical eraseblocks mappings.
So now to get this information just read the fastmap, instead of doing full

This patch fixes up the broken suspend sequence for eMMC with sleep
support. Additionally it reworks the eMMC4.5 Power Off Notification
feature so it fits together with the existing sleep feature.
The CMD0 based re-initialization of the eMMC at resume is re-introduced
to maintain compatiblity for devices using sleep.
A host shall use MMC_CAP2_POWEROFF_NOTIFY to enable the Power Off
Notification feature. We might be able to remove this cap later on,
if we think that Power Off Notification always is preferred over
sleep, even if the host is not able to cut the eMMC VCCQ power.

Before this patch, we were using MMC_CAP2_BROKEN_VOLTAGE as a way to
avoid calling regulator_set_voltage() on a fixed regulator, but that's
just duplicating information that already exists -- we should test
whether the regulator is fixed directly, instead of via a capability.
This patch implements that test. We can't reclaim the capability bit
just yet, since there are still boards in arch/arm/ that reference it;
those references can be removed now.

The function sis900_probe() return 0 for success and negative value
for most of its internal tests failures. There is one exception
that is error case going to err_out_cleardev:. Fore this error case,
the function abort its success execution path, but returns non negative
value, making it difficult for a caller function to notice the error.
This patch fixes the error case that do not return negative value.
This was found by Coccinelle, but the code change was made by hand.
This patch is not robot generated.

Since the prt_names() macro is always called with an unsigned index the
((a) >= 0) condition is always true and gives a compiler warning when
compiling with W=1.
Rewrite the macro to avoid that warning, but cast the index to unsigned
just in case it is ever called with a signed argument.

The maximum size of an EDID is 32768 bytes, which is 32768 / 128 = 256 blocks.

Commit c49f34bc ("ARM: OMAP2+ Move SoC specific headers to be local to
mach-omap2") moved omap34xx.h to mach-omap2. This broke omap3isp, as it
includes omap34xx.h.
Instead of moving omap34xx to platform_data, simply add the two
definitions the driver needs and remove the include altogether.

The tda18271 datasheet says:
"The image rejection calibration and RF tracking filter
calibration must be launched exactly as described in the
flowchart, otherwise bad calibration or even blocking of the
TDA18211HD can result making it impossible to communicate
via the I2C-bus."
(yeah, tda18271 refers there to tda18211 - likely a typo at their
datasheets)
That likely explains why sometimes tda18271 stops answering. That
is now happening more often on designs with drx-k chips, as the
firmware is now loaded asyncrousnly there.
While the above text doesn't explicitly tell that the I2C bus
couldn't be used by other devices during such initialization,
that seems to be a requirement there.
So, let's explicitly use the I2C lock there, avoiding I2C bus
share during those critical moments.
Compile-tested only. Please test.

In LUN RESET testing involving NetApp targets, it is observed that LUN
RESET is failing. The fc_fcp_resp() is not completing the completion
for the LUN RESET task since fc_fcp_resp assumes that the FCP_RSP_INFO
is 8 bytes with the 4 byte reserved field, where in case of NetApp targets
the FCP_RSP to LUN RESET only has 4 bytes of FCP_RSP_INFO. This leads
fc_fcp_resp to error out w/o completing the task completion, eventually
causing LUN RESET to be escalated to host reset, which is not very nice.
Per FCP-3 r04, clause 9.5.15 and Table 23, the FCP_RSP_INFO field can be either
4 bytes or 8 bytes, with the last 4 bytes as "Reserved (if any)". Therefore it
is valid to have 4 bytes FCP_RSP_INFO like some of the NetApp targets behave.
Fixing this by validating the FCP_RSP_INFO against both the two spec allowed
length.

SCSI errors were generated while writing to LUNs
connected via NPIV ports.
Debugging this it was found that the FCoE packets
transmitted via the NPIV ports were not tagged with
correct user priority as negotiated with peer by DCB
agent. This resulted in FCoE traffic going with priority
zero(0) that did not have priority flow control (PFC)
enabled for it. The initiator after transferring data
to the target never saw any reply indicating the transfer
was complete. This resulted in error recovery (ABTS) and
SCSI command retries by the scsi-mid layer; eventually
resulting in I/O errors.
This patch fixes this issue by keeping the FCoE user
priority information in the fcoe_interface instance
that is common for both the physical port as well as
NPIV ports connected to that physical port; instead
of storing it in fcoe_port structure that has a per

- Modified the firmware naming convention to contain the
firmware image version (3.1.0.0).
- The new convention is <firmware-image>-<firmware-version>.bin
- The change will enforce loading only compatible firmware with this driver
and also avoid over-writing the old firmware image in-order to load new
version driver as the firmware names used to be the same.

- Made changes such that once the PWWN is acquired from the fabric through FAA,
and if the FAPWWN configuration is modified on the switch side, driver should
show relevant information to the user.
- Added logic to cache the reason code when the given port is disabled implicitl
due to FAA error condition.
- If the port is disabled, while sending SCN to upper layer, update the
reason code appropriately. With this, BFA FC port state machine will enter
into faa_err_config state. This state will be shown to the user.

- Introduced support for D-Port which is a new port mode during which
link level diagnostics can be run.
- Provided mechanism to dynamically configure D-Port and initiate diagnostic
tests to isolate any link level issues.

- Add private loop topology support at 2G/4G/8G speeds with following
limitations
1. No support for multiple initiators in the loop
2. No public loop support. If attached to a loop with an FL_Port,
device continues to work as a private NL_Port in the loop
3. No auto topology detection. User has to manually set the
configured topology to loop if attaching to loop.
- When loop topology is configured, enabling FC port features
QoS/Trunk/TRL are not allowed and vice versa.

This series contain:
- A major tree renaming patch series: now, drivers are organized
internally by their used bus, instead of by V4L2 and/or DVB API,
providing a cleaner driver location for hybrid drivers that
implement both APIs, and allowing to cleanup the Kconfig items and
make them more intuitive for the end user;
- Media Kernel developers are typically very lazy with their duties
of keeping the MAINTAINERS entries for their drivers updated. As
now the tree is more organized, we're doing an effort to add/update
those entries for the drivers that aren't currently orphan;
- Several DVB USB drivers got moved to a new DVB USB v2 core; the new
core fixes several bugs (as the existing one that got bitroted).
Now, suspend/resume finally started to work fine (at least with
some devices - we should expect more work with regards to it);
- added multistream support for DVB-T2, and unified the API for
DVB-S2 and ISDB-S. Backward binary support is preserved;
- as usual, a few new drivers, some V4L2 core improvements and lots
of drivers improvements and fixes.
There are some points to notice on this series:
1) you should expect a trivial merge conflict on your tree, with the
removal of Documentation/feature-removal-schedule.txt: this series
would be adding two additional entries there. I opted to not
rebase it due to this recent change;
2) With regards to the PCTV 520e udev-related breakage, I opted to
fix it in a way that the patches can be backported to 3.5 even
without your firmware fix patch. This way, Greg doesn't need to
rush backporting your patch (as there are still the firmware cache
and firmware path customization issues to be addressed there).
I'll send later a patch (likely after the end of the merge window)
reverting the rest of the DRX-K async firmware request, fully
restoring its original behaviour to allow media drivers to
initialize everything serialized as before for 3.7 and upper.
3) I'm planning to work on this weekend to test the DMABUF patches
for V4L2. The patches are on my queue for several Kernel cycles,
but, up to now, there is/was no way to test the series locally.
I have some concerns about this particular changeset with regards
to security issues, and with regards to the replacement of the old
VIDIOC_OVERLAY ioctl's that is broken on modern systems, due to
GPU drivers change. The Overlay API allows direct PCI2PCI
transfers from a media capture card into the GPU framebuffer, but
its API is crappy. Also, the only existing X11 driver that
implements it requires a XV extension that is not available
anymore on modern drivers. The DMABUF can do the same thing, but
with it is promising to be a properly-designed API. If I can
successfully test this series and be happy with it, I should be
asking you to pull them next week."

Over time, skb recycling infrastructure got litle interest and
many bugs. Generic rx path skb allocation is now using page
fragments for efficient GRO / TCP coalescing, and recyling
a tx skb for rx path is not worth the pain.
Last identified bug is that fat skbs can be recycled
and it can endup using high order pages after few iterations.
With help from Maxime Bizon, who pointed out that commit
87151b8689 (net: allow pskb_expand_head() to get maximum tailroom)
introduced this regression for recycled skbs.
Instead of fixing this bug, lets remove skb recycling.
Drivers wanting really hot skbs should use build_skb() anyway,
to allocate/populate sk_buff right before netif_receive_skb()

operations (so no PV MMU) - and use existing PV drivers for I/Os
(network, block, console, etc). This is similar to how PVHVM guests
operate in X86 platform nowadays - except that on ARM there is no need
for QEMU. The end result is that we share a lot of the generic Xen
drivers and infrastructure.

Add support to export the device description obtained from the ACPI _STR
method, if one exists for a device, to user-space via a sysfs interface.
This new interface provides a standard and platform neutral way for users
to obtain the description text stored in the ACPI _STR method. If no
_STR method exists for the device, no sysfs 'description' file will be
created. The 'description' file will be located in the /sys/devices/
directory using the device's path.
/sys/device/<bus>/<bridge path>/<device path>.../firmware_node/description
Example:

It can also be located using the ACPI device path, for example:

Parsing acpi table entries may fall into an infinite loop on a buggy BIOS
which has entry length=0 in acpi table.
Instead of kernel hang with few failure clue which leads to heavy lifting debug
effort, this patch hardens kernel boot by booting into non NUMA mode. The debug
info left in log buffer helps people identify the issue.

The Linux EC driver includes a mechanism to detect GPE storms,
and switch from interrupt-mode to polling mode. However, polling
mode sometimes doesn't work, so the workaround is problematic.
Also, different systems seem to need the threshold for detecting
the GPE storm at different levels.
ACPI_EC_STORM_THRESHOLD was initially 20 when it's created, and
was changed to 8 in 2.6.28 commit 06cf7d3c7 "ACPI: EC: lower interrupt storm
threshold" to fix kernel bug 11892 by forcing the laptop in that bug to
work in polling mode. However in bug 45151, it works fine in interrupt
mode if we lift the threshold back to 20.
This patch makes the threshold a module parameter so that user has a
flexible option to debug/workaround this issue.
The default is unchanged.

Added V4L2_CID_COLOR_KILLER control to the bt8xx driver.
The control V4L2_CID_PRIVATE_CHROMA_AGC was changed too because
with this change the bttv driver must touch two bits in the
SC Loop Control Registers, for controls V4L2_CID_COLOR_KILLER
and V4L2_CID_PRIVATE_CHROMA_AGC.

"carrier" comes from a get_user() in ir_lirc_ioctl(). We need to test
that it's not zero before using it as a divisor. It might have been
nice to test for this ir_lirc_ioctl() but the mceusb driver uses zero to
disable carrier modulation.
The bug in redrat3 is a little more subtle. The ->carrier is passed to
mod_freq_to_val() which uses it as a divisor.

We currently don't select the register bank in ov2640_s_ctrl, so we can end up
writing to DSP register 0x04 instead of sensor register 0x04.
This happens for example when calling ov2640_s_ctrl after ov2640_s_fmt.

Convert the driver from the outdated omap_pm_set_max_mpu_wakeup_lat
API to the new PM QoS API.
Since the constraint is on the MPU subsystem, use the PM_QOS_CPU_DMA_LATENCY
class of PM QoS. The resulting MPU constraints are used by cpuidle to
decide the next power state of the MPU subsystem.
The I2C device latency timing is derived from the FIFO size and the
clock speed and so is applicable to all OMAP SoCs.

The parameter passed to pca9665_reset is adap->data (which is bus driver
specific), not i2c_algp_pca_data *adap. pca9665_reset expects it to be
i2c_algp_pca_data *adap. All other wrappers from the algo call back to
the bus driver, which knows to handle its custom data. Only pca9665_reset
resides inside the algorithm code and does not know how to handle a custom
data structure. This can result in a kernel crash.
Fix by re-factoring pca_reset() from a macro to a function handling chip
specific code, and only call adap->reset_chip() if the chip is not PCA9665.

The vfl_dir flag is presently set to VFL_DIR_M2M only for decoder.
The encoder is not working because of this. So adding this flag
