7f1a00b6fc fix up initial thread stack pointer vs thread_info confusion
The INIT_TASK() initializer was similarly confused about the stack vs
thread_info allocation that the allocators had, and that were fixed in
commit b235beea9e ("Clarify naming of thread info/stack allocators").

The task ->stack pointer only incidentally ends up having the same value
as the thread_info, and in fact that will change.

So fix the initial task struct initializer to point to 'init_stack'
instead of 'init_thread_info', and make sure the ia64 definition for
that exists.

This actually makes the ia64 tsk->stack pointer be sensible for the
initial task, but not for any other task.  As mentioned in commit
b235beea9e, that whole pointer isn't actually used on ia64, since
task_stack_page() there just points to the (single) allocation.

All the other architectures seem to have copied the 'init_stack'
definition, even if it tended to be generally unusued.

accept task_struct as a parameter.

e298c499ca ia64: Remove superfluous SMP function call
Since commit 3b9d6da67e ("cpu/hotplug: Fix rollback during error-out
in __cpu_disable()") it is ensured that callbacks of CPU_ONLINE and
CPU_DOWN_PREPARE are processed on the hotplugged CPU. Due to this SMP
function calls are no longer required.

Replace smp_call_function_single() with a direct call to
ia64_mca_cmc_vector_adjust(). The function itselfs handles disable and
enable interrupts, therefore the smp_call_function_single() calling
convention is not preserved.

 "This tree adds support for a new memory protection hardware feature
  that is available in upcoming Intel CPUs: 'protection keys' (pkeys).

  The gist is that protection keys allow the encoding of
  user-controllable permission masks in the pte.  So instead of having a
  fixed protection mask in the pte (which needs a system call to change
  and works on a per page basis), the user can map a (handful of)
  protection mask variants and can change the masks runtime relatively
  cheaply, without having to change every single page in the affected
  virtual memory range.

  This allows the dynamic switching of the protection bits of large
  amounts of virtual memory, via user-space instructions.  It also
  allows more precise control of MMU permission bits: for example the
  executable bit is separate from the read bit (see more about that
  below).

  This tree adds the MM infrastructure and low level x86 glue needed for
  that, plus it adds a high level API to make use of protection keys -
  if a user-space application calls:

        mmap(..., PROT_EXEC);

  or

        mprotect(ptr, sz, PROT_EXEC);

  (note PROT_EXEC-only, without PROT_READ/WRITE), the kernel will notice
  this special case, and will set a special protection key on this
  memory range.  It also sets the appropriate bits in the Protection
  Keys User Rights (PKRU) register so that the memory becomes unreadable
  and unwritable.

  So using protection keys the kernel is able to implement 'true'
  PROT_EXEC on x86 CPUs: without protection keys PROT_EXEC implies
  PROT_READ as well.  Unreadable executable mappings have security
  advantages: they cannot be read via information leaks to figure out
  ASLR details, nor can they be scanned for ROP gadgets - and they
  cannot be used by exploits for data purposes either.

  We know about no user-space code that relies on pure PROT_EXEC
  mappings today, but binary loaders could start making use of this new
  feature to map binaries and libraries in a more secure fashion.

  There is other pending pkeys work that offers more high level system
  call APIs to manage protection keys - but those are not part of this
  pull request.

  Right now there's a Kconfig that controls this feature
  (CONFIG_X86_INTEL_MEMORY_PROTECTION_KEYS) that is default enabled
  (like most x86 CPU feature enablement code that has no runtime
  overhead), but it's not user-configurable at the moment.  If there's
  any serious problem with this then we can make it configurable and/or
  flip the default"

 "What's wrong with the current cpu hotplug infrastructure?

   - Asymmetry

     The hotplug notifier mechanism is asymmetric versus the bringup and
     teardown.  This is mostly caused by the notifier mechanism.

   - Largely undocumented dependencies

     While some notifiers use explicitely defined notifier priorities,
     we have quite some notifiers which use numerical priorities to
     express dependencies without any documentation why.

   - Control processor driven

     Most of the bringup/teardown of a cpu is driven by a control
     processor.  While it is understandable, that preperatory steps,
     like idle thread creation, memory allocation for and initialization
     of essential facilities needs to be done before a cpu can boot,
     there is no reason why everything else must run on a control
     processor.  Before this patch series, bringup looks like this:

       Control CPU                     Booting CPU

       do preparatory steps
       kick cpu into life

                                       do low level init

       sync with booting cpu           sync with control cpu

       bring the rest up

   - All or nothing approach

     There is no way to do partial bringups.  That's something which is
     really desired because we waste e.g.  at boot substantial amount of
     time just busy waiting that the cpu comes to life.  That's stupid
     as we could very well do preparatory steps and the initial IPI for
     other cpus and then go back and do the necessary low level
     synchronization with the freshly booted cpu.

   - Minimal debuggability

     Due to the notifier based design, it's impossible to switch between
     two stages of the bringup/teardown back and forth in order to test
     the correctness.  So in many hotplug notifiers the cancel
     mechanisms are either not existant or completely untested.

   - Notifier [un]registering is tedious

     To [un]register notifiers we need to protect against hotplug at
     every callsite.  There is no mechanism that bringup/teardown
     callbacks are issued on the online cpus, so every caller needs to
     do it itself.  That also includes error rollback.

  What's the new design?

     The base of the new design is a symmetric state machine, where both
     the control processor and the booting/dying cpu execute a well
     defined set of states.  Each state is symmetric in the end, except
     for some well defined exceptions, and the bringup/teardown can be
     stopped and reversed at almost all states.

     So the bringup of a cpu will look like this in the future:

                                       bring itself up

     The synchronization step does not require the control cpu to wait.
     That mechanism can be done asynchronously via a worker or some
     other mechanism.

     The teardown can be made very similar, so that the dying cpu cleans
     up and brings itself down.  Cleanups which need to be done after
     the cpu is gone, can be scheduled asynchronously as well.

  There is a long way to this, as we need to refactor the notion when a
  cpu is available.  Today we set the cpu online right after it comes
  out of the low level bringup, which is not really correct.

  The proper mechanism is to set it to available, i.e. cpu local
  threads, like softirqd, hotplug thread etc. can be scheduled on that
  cpu, and once it finished all booting steps, it's set to online, so
  general workloads can be scheduled on it.  The reverse happens on
  teardown.  First thing to do is to forbid scheduling of general
  workloads, then teardown all the per cpu resources and finally shut it
  off completely.

  This patch series implements the basic infrastructure for this at the
  core level.  This includes the following:

   - Basic state machine implementation with well defined states, so
     ordering and prioritization can be expressed.

   - Interfaces to [un]register state callbacks

     This invokes the bringup/teardown callback on all online cpus with
     the proper protection in place and [un]installs the callbacks in
     the state machine array.

     For callbacks which have no particular ordering requirement we have
     a dynamic state space, so that drivers don't have to register an
     explicit hotplug state.

     If a callback fails, the code automatically does a rollback to the
     previous state.

   - Sysfs interface to drive the state machine to a particular step.

     This is only partially functional today.  Full functionality and
     therefor testability will be achieved once we converted all
     existing hotplug notifiers over to the new scheme.

   - Run all CPU_ONLINE/DOWN_PREPARE notifiers on the booting/dying
     processor:

     In a previous step of this work we've done a full tree mechanical
     conversion of all hotplug notifiers to the new scheme.  The balance
     is a net removal of about 4000 lines of code.

     This is not included in this series, as we decided to take a
     different approach.  Instead of mechanically converting everything
     over, we will do a proper overhaul of the usage sites one by one so
     they nicely fit into the symmetric callback scheme.

     I decided to do that after I looked at the ugliness of some of the
     converted sites and figured out that their hotplug mechanism is
     completely buggered anyway.  So there is no point to do a
     mechanical conversion first as we need to go through the usage
     sites one by one again in order to achieve a full symmetric and
     testable behaviour"

fc6d73d674 arch/hotplug: Call into idle with a proper state
Let the non boot cpus call into idle with the corresponding hotplug state, so
the hotplug core can handle the further bringup. That's a first step to
convert the boot side of the hotplugged cpus to do all the synchronization
with the other side through the state machine. For now it'll only start the
hotplug thread and kick the full bringup of the cpu.

d4edcf0d56 mm/gup: Switch all callers of get_user_pages() to not pass tsk/mm
We will soon modify the vanilla get_user_pages() so it can no
longer be used on mm/tasks other than 'current/current->mm',
which is by far the most common way it is called.  For now,
we allow the old-style calls, but warn when they are used.
(implemented in previous patch)

This patch switches all callers of:

	get_user_pages()
	get_user_pages_unlocked()
	get_user_pages_locked()

to stop passing tsk/mm so they will no longer see the warnings.

Change efi_initialize_iomem_resources() to set 'flags' and
'desc' for EFI memory types. IORESOURCE_SYSRAM, a modifier bit,
is set for System RAM as IORESOURCE_MEM is already set.
IORESOURCE_SYSTEM_RAM is defined as
(IORESOURCE_MEM|IORESOURCE_SYSRAM). I/O resource descriptor is
set for "ACPI Non-volatile Storage" and "Persistent Memory".

Also set IORESOURCE_SYSTEM_RAM for "Kernel code", "Kernel data",
and "Kernel bss".

8463833590 mm: rework virtual memory accounting
When inspecting a vague code inside prctl(PR_SET_MM_MEM) call (which
testing the RLIMIT_DATA value to figure out if we're allowed to assign
new @start_brk, @brk, @start_data, @end_data from mm_struct) it's been
commited that RLIMIT_DATA in a form it's implemented now doesn't do
anything useful because most of user-space libraries use mmap() syscall
for dynamic memory allocations.

Linus suggested to convert RLIMIT_DATA rlimit into something suitable
for anonymous memory accounting.  But in this patch we go further, and
the changes are bundled together as:

 * keep vma counting if CONFIG_PROC_FS=n, will be used for limits
 * replace mm->shared_vm with better defined mm->data_vm
 * account anonymous executable areas as executable
 * account file-backed growsdown/up areas as stack
 * drop struct file* argument from vm_stat_account
 * enforce RLIMIT_DATA for size of data areas

This way code looks cleaner: now code/stack/data classification depends
only on vm_flags state:

 VM_EXEC & ~VM_WRITE            -> code  (VmExe + VmLib in proc)
 VM_GROWSUP | VM_GROWSDOWN      -> stack (VmStk)
 VM_WRITE & ~VM_SHARED & !stack -> data  (VmData)

The rest (VmSize - VmData - VmStk - VmExe - VmLib) could be called
"shared", but that might be strange beast like readonly-private or VM_IO
area.

 - RLIMIT_AS            limits whole address space "VmSize"
 - RLIMIT_STACK         limits stack "VmStk" (but each vma individually)
 - RLIMIT_DATA          now limits "VmData"

cbbe12c43d ia64: ftrace: Fix the comments for ftrace_modify_code()
There is no need to worry about module and __init text disappearing
case, because that ftrace has a module notifier that is called when
a module is being unloaded and before the text goes away and this
code grabs the ftrace_lock mutex and removes the module functions
from the ftrace list, such that it will no longer do any
modifications to that module's text, the update to make functions
be traced or not is done under the ftrace_lock mutex as well.
And by now, __init section codes should not been modified
by ftrace, because it is black listed in recordmcount.c and
ignored by ftrace.

alloc_pages_exact_node() was introduced in commit 6484eb3e2a ("page
allocator: do not check NUMA node ID when the caller knows the node is
valid") as an optimized variant of alloc_pages_node(), that doesn't
fallback to current node for nid == NUMA_NO_NODE.  Unfortunately the
name of the function can easily suggest that the allocation is
restricted to the given node and fails otherwise.  In truth, the node is
only preferred, unless __GFP_THISNODE is passed among the gfp flags.

The misleading name has lead to mistakes in the past, see for example
commits 5265047ac3 ("mm, thp: really limit transparent hugepage
allocation to local node") and b360edb43f ("mm, mempolicy:
migrate_to_node should only migrate to node").

Another issue with the name is that there's a family of
alloc_pages_exact*() functions where 'exact' means exact size (instead
of page order), which leads to more confusion.

To prevent further mistakes, this patch effectively renames
alloc_pages_exact_node() to __alloc_pages_node() to better convey that
it's an optimized variant of alloc_pages_node() not intended for general
usage.  Both functions get described in comments.

It has been also considered to really provide a convenience function for
allocations restricted to a node, but the major opinion seems to be that
__GFP_THISNODE already provides that functionality and we shouldn't
duplicate the API needlessly.  The number of users would be small
anyway.

Existing callers of alloc_pages_exact_node() are simply converted to
call __alloc_pages_node(), with the exception of sba_alloc_coherent()
which open-codes the check for NUMA_NO_NODE, so it is converted to use
alloc_pages_node() instead.  This means it no longer performs some
VM_BUG_ON checks, and since the current check for nid in
alloc_pages_node() uses a 'nid < 0' comparison (which includes
NUMA_NO_NODE), it may hide wrong values which would be previously
exposed.

Both differences will be rectified by the next patch.

To sum up, this patch makes no functional changes, except temporarily
hiding potentially buggy callers.  Restricting the checks in
alloc_pages_node() is left for the next patch which can in turn expose
more existing buggy callers.

Pull x86 core updates from Ingo Molnar:
 "There were so many changes in the x86/asm, x86/apic and x86/mm topics
  in this cycle that the topical separation of -tip broke down somewhat -
  so the result is a more traditional architecture pull request,
  collected into the 'x86/core' topic.

  The topics were still maintained separately as far as possible, so
  bisectability and conceptual separation should still be pretty good -
  but there were a handful of merge points to avoid excessive
  dependencies (and conflicts) that would have been poorly tested in the
  end.

  The next cycle will hopefully be much more quiet (or at least will
  have fewer dependencies).

       is in preparation to move a good chunk of the low level entry
       code from assembly to C code (Denys Vlasenko, Andy Lutomirski,
       Brian Gerst)

     - Moved all system entry related code to a new home under
       arch/x86/entry/ (Ingo Molnar)

     - Removal of the fragile and ugly CFI dwarf debuginfo annotations.
       Conversion to C will reintroduce many of them - but meanwhile
       they are only getting in the way, and the upstream kernel does
       not rely on them (Ingo Molnar)

     - NOP handling refinements. (Borislav Petkov)

   * x86/mm changes:

     - Big PAT and MTRR rework: making the code more robust and
       preparing to phase out exposing direct MTRR interfaces to drivers -
       in favor of using PAT driven interfaces (Toshi Kani, Luis R

     - New ioremap_wt()/set_memory_wt() interfaces to support
       Write-Through cached memory mappings.  This is especially
       important for good performance on NVDIMM hardware (Toshi Kani)

     - Add support for deferred errors on AMD (Aravind Gopalakrishnan)

       This is an important RAS feature which adds hardware support for
       poisoned data.  That means roughly that the hardware marks data
       which it has detected as corrupted but wasn't able to correct, as
       poisoned data and raises an APIC interrupt to signal that in the
       form of a deferred error.  It is the OS's responsibility then to
       take proper recovery action and thus prolonge system lifetime as
       far as possible.

     - Add support for Intel "Local MCE"s: upcoming CPUs will support
       CPU-local MCE interrupts, as opposed to the traditional system-
       wide broadcasted MCE interrupts (Ashok Raj)

     - Misc cleanups (Borislav Petkov)

   * x86/platform changes:

e55645ec57 ia64: remove paravirt code
All the ia64 pvops code is now dead code since both
xen and kvm support have been ripped out [0] [1]. Just
that no one had troubled to rip this stuff out. The only
useful remaining pieces were the old pvops docs but that
was recently also generalized and moved out from ia64 [2].

This has been run time tested on an ia64 Madison system.

cpumask_test_cpu() doesn't take volatile, unlike the obsoleted

ad5fb870c4 e820, efi: add ACPI 6.0 persistent memory types
ACPI 6.0 formalizes e820-type-7 and efi-type-14 as persistent memory.
Mark it "reserved" and allow it to be claimed by a persistent memory
device driver.

This definition is in addition to the Linux kernel's existing type-12
definition that was recently added in support of shipping platforms with
NVDIMM support that predate ACPI 6.0 (which now classifies type-12 as
OEM reserved).

Note, /proc/iomem can be consulted for differentiating legacy
"Persistent Memory (legacy)" E820_PRAM vs standard "Persistent Memory"
E820_PMEM.

  from EFI.  We don't support a hybrid ACPI/FDT scheme.  Of course,
  there is still plenty of work to do (a serial console would be nice!)
  but I expect that to happen on a per-driver basis after this core
  series has been merged.

  Anyway, the diff stat here is fairly horrible, but splitting this up
  and merging it via all the different subsystems would have been
