94aa033efc KVM: s390: fix get_all_floating_irqs
This fixes a bug introduced with commit c05c4186bb ("KVM: s390:
add floating irq controller").

get_all_floating_irqs() does copy_to_user() while holding
a spin lock. Let's fix this by filling a temporary buffer
first and copy it to userspace after giving up the lock.

a3ed8dae6e KVM: s390: enable more features that need no hypervisor changes
After some review about what these facilities do, the following
facilities will work under KVM and can, therefore, be reported
to the guest if the cpu model and the host cpu provide this bit.

There are plans underway to make the whole bit thing more readable,
but its not yet finished. So here are some last bit changes and
we enhance the KVM mask with:

2ba4596852 KVM: s390: store the breaking-event address on pgm interrupts
If the PER-3 facility is installed, the breaking-event address is to be
stored in the low core.

There is no facility bit for PER-3 in stfl(e) and Linux always uses the
value at address 272 no matter if PER-3 is available or not.
We can't hide its existence from the guest. All program interrupts
injected via the SIE automatically store this information if the PER-3
facility is available in the hypervisor. Also the itdb contains the
address automatically.

As there is no switch to turn this mechanism off, let's simply make it
consistent and also store the breaking event address in case of manual
program interrupt injection.

876e78818d time: Rename timekeeper::tkr to timekeeper::tkr_mono
In preparation of adding another tkr field, rename this one to
tkr_mono. Also rename tk_read_base::base_mono to tk_read_base::base,
since the structure is not specific to CLOCK_MONOTONIC and the mono
name got added to the tk_read_base instance.

3c1a3bcea9 s390/maccess: improve s390_kernel_write()
Use the sturg instruction instead of the stura instruction. This allows to
modify up to eight bytes in a row instead of only four.

For function tracer enabling and disabling this reduces the time needed to
modify the text sections by 50%, since for each mcount call site six bytes
need to be changed.

Also remove the EXTABLE entries, since calls to this function are not
supposed to fail.

8a5d8473dd s390/maccess: remove potentially broken probe_kernel_write()
Remove the s390 architecture implementation of probe_kernel_write() and
instead use a new function s390_kernel_write() to modify kernel text and
data everywhere.

The s390 implementation of probe_kernel_write() was potentially broken
since it modified memory in a read-modify-write fashion, which read four
bytes, modified the requested bytes within those four bytes and wrote
the result back.
If two cpus would modify the same four byte area at different locations
within that area, this could lead to corruption.
Right now the only places which called probe_kernel_write() did run within
stop_machine_run. Therefore the scenario can't happen right now, however
that might change at any time.

To fix this rename probe_kernel_write() to s390_kernel_write() which can
have special semantics, like only call it while running within stop_machine().

e1d12d70f7 s390/traps: panic() instead of die() on translation exception
In case of a translation exception the page tables are corrupted. If the
exception handler then calls die() which again calls show_regs()
-> show_code() -> copy_from_user(), the kernel may access the same memory
location again and generates yet another translation exception. Which in turn
will lead to a deadlock on the die_lock spinlock, which the kernel tries to
grab recursively.

Given that the page tables are corrupted anyway, if we see such an exception,
let's simply panic.

5a79859ae0 s390: remove 31 bit support
Remove the 31 bit support in order to reduce maintenance cost and
effectively remove dead code. Since a couple of years there is no
distribution left that comes with a 31 bit kernel.

The 31 bit kernel also has been broken since more than a year before
anybody noticed. In addition I added a removal warning to the kernel
shown at ipl for 5 minutes: a960062e58 ("s390: add 31 bit warning
message") which let everybody know about the plan to remove 31 bit
code. We didn't get any response.

Given that the last 31 bit only machine was introduced in 1999 let's
remove the code.
Anybody with 31 bit user space code can still use the compat mode.

The be_main.c conflict resolution was really tricky.  The conflict
hunks generated by GIT were very unhelpful, to say the least.  It
split functions in half and moved them around, when the real actual
conflict only existed solely inside of one function, that being
be_map_pci_bars().

So instead, to resolve this, I checked out be_main.c from the top
of net-next, then I applied the be_main.c changes from 'net' since
the last time I merged.  And this worked beautifully.

The inet_diag.c and sysctl_net_core.c conflicts were simple
overlapping changes, and were easily to resolve.

b97ea289cf PCI: Assign resources before drivers claim devices (pci_scan_root_bus())
Previously, pci_scan_root_bus() created a root PCI bus, enumerated the
devices on it, and called pci_bus_add_devices(), which made the devices
available for drivers to claim them.

Most callers assigned resources to devices after pci_scan_root_bus()
returns, which may be after drivers have claimed the devices.  This is
incorrect; the PCI core should not change device resources while a driver
is managing the device.

Remove pci_bus_add_devices() from pci_scan_root_bus() and do it after any
resource assignment in the callers.

Note that ARM's pci_common_init_dev() already called pci_bus_add_devices()
after pci_scan_root_bus(), so we only need to remove the first call:

The patch represents capability KVM_CAP_S390_VECTOR_REGISTERS by means
of the SIMD facility bit. This allows to a) disable the use of SIMD when
used in conjunction with a not-SIMD-aware QEMU, b) to enable SIMD when
used with a SIMD-aware version of QEMU and c) finally by means of a QEMU
version using the future cpu model ioctls.

e44fc8c9da KVM: s390: introduce post handlers for STSI
The Store System Information (STSI) instruction currently collects all
information it relays to the caller in the kernel. Some information,
however, is only available in user space. An example of this is the
guest name: The kernel always sets "KVMGuest", but user space knows the
actual guest name.

This patch introduces a new exit, KVM_EXIT_S390_STSI, guarded by a
capability that can be enabled by user space if it wants to be able to
insert such data. User space will be provided with the target buffer
and the requested STSI function code.

41408c28f2 KVM: s390: Add MEMOP ioctls for reading/writing guest memory
On s390, we've got to make sure to hold the IPTE lock while accessing
logical memory. So let's add an ioctl for reading and writing logical
memory to provide this feature for userspace, too.
The maximum transfer size of this call is limited to 64kB to prevent
that the guest can trigger huge copy_from/to_user transfers. QEMU
currently only requests up to one or two pages so far, so 16*4kB seems
to be a reasonable limit here.

The kvm_s390_check_low_addr_protection() function is used only with real
addresses. According to the POP (the "Low-Address Protection"
paragraph in chapter 3), if the effective address is real or absolute,
the low-address protection procedure should raise a PROTECTION exception
only when the low-address protection is enabled in the control register
0 and the address is low.
This patch removes ASCE checks from the function and renames it to
better reflect its behavior.

f8935983f1 clocksource: Mostly kill clocksource_register()
A long running project has been to clean up remaining uses
of clocksource_register(), replacing it with the simpler
clocksource_register_khz/hz() functions.

However, there are a few cases where we need to self-define
our mult/shift values, so switch the function to a more
obviously internal __clocksource_register() name, and
consolidate much of the internal logic so we don't have
duplication.

dc9be0fac7 kvm: move advertising of KVM_CAP_IRQFD to common code
POWER supports irqfds but forgot to advertise them.  Some userspace does
not check for the capability, but others check it---thus they work on
x86 and s390 but not POWER.

To avoid that other architectures in the future make the same mistake, let
common code handle KVM_CAP_IRQFD the same way as KVM_CAP_IRQFD_RESAMPLE.

We finally have all the pieces in place, so let's include the
vector facility bit in the mask of available hardware facilities
for the guest to recognize.  Also, enable the vector functionality
in the guest control blocks, to avoid a possible vector data
exception that would otherwise occur when a vector instruction
is issued by the guest operating system.

68c557501b KVM: s390: Allocate and save/restore vector registers
Define and allocate space for both the host and guest views of
the vector registers for a given vcpu.  The 32 vector registers
occupy 128 bits each (512 bytes total), but architecturally are
paired with 512 additional bytes of reserved space for future
expansion.

The kvm_sync_regs structs containing the registers are union'ed
with 1024 bytes of padding in the common kvm_run struct.  The
addition of 1024 bytes of new register information clearly exceeds
the existing union, so an expansion of that padding is required.

When changing environments, we need to appropriately save and
restore the vector registers viewed by both the host and guest,
into and out of the sync_regs space.

The floating point registers overlay the upper half of vector
registers 0-15, so there's a bit of data duplication here that
needs to be carefully avoided.

91520f1af8 KVM: s390: perform vcpu model setup in a function
The function kvm_s390_vcpu_setup_model() now performs all cpu model realated
setup tasks for a vcpu. Besides cpuid and ibc initialization, facility list
assignment takes place during the setup step as well. The model setup has been
pulled to the begin of vcpu setup to allow kvm facility tests.

There is no need to protect the cpu model setup with a lock since the attributes
can't be changed anymore as soon the first vcpu is online.

492d8642ea KVM: s390: Forward PSW to next instruction for addressing exceptions
When the SIE exited by a DAT access exceptions which we can not
resolve, the guest tried to access a page which is out of bounds
and can not be paged-in. In this case we have to signal the bad
access by injecting an address exception. However, address exceptions
are either suppressing or terminating, i.e. the PSW has to point to
the next instruction when the exception is delivered. Since the
originating DAT access exception is nullifying, the PSW still
points to the offending instruction instead, so we've got to forward
the PSW to the next instruction.
Having fixed this issue, we can now also enable the TPROT
interpretation facility again which had been disabled because
of this problem.

a9a846fd5c KVM: s390: Nullify instruction for certain program exceptions
When certain program exceptions (e.g. DAT access exceptions) occur,
the current instruction has to be nullified, i.e. the old PSW that
gets written into the low-core has to point to the beginning of the
instruction again, and not to the beginning of the next instruction.
Thus we have to rewind the PSW before writing it into the low-core.
The list of nullifying exceptions can be found in the POP, chapter 6,
figure 6-1 ("Interruption Action").

15462e37ca KVM: s390: reinjection of irqs can fail in the tpi handler
The reinjection of an I/O interrupt can fail if the list is at the limit
and between the dequeue and the reinjection, another I/O interrupt is
injected (e.g. if user space floods kvm with I/O interrupts).

This patch avoids this memory leak and returns -EFAULT in this special
case. This error is not recoverable, so let's fail hard. This can later
be avoided by not dequeuing the interrupt but working directly on the
locked list.

261520dcfc KVM: s390: fix handling of write errors in the tpi handler
If the I/O interrupt could not be written to the guest provided
area (e.g. access exception), a program exception was injected into the
guest but "inti" wasn't freed, therefore resulting in a memory leak.

In addition, the I/O interrupt wasn't reinjected. Therefore the dequeued
interrupt is lost.

This patch fixes the problem while cleaning up the function and making the
cc and rc logic easier to handle.

981467c930 KVM: s390: include guest facilities in kvm facility test
Most facility related decisions in KVM have to take into account:

- the facilities offered by the underlying run container (LPAR/VM)
- the facilities supported by the KVM code itself
- the facilities requested by a guest VM

This patch adds the KVM driver requested facilities to the test routine.

It additionally renames struct s390_model_fac to kvm_s390_fac and its field
names to be more meaningful.

The semantics of the facilities stored in the KVM architecture structure
is changed. The address arch.model.fac->list now points to the guest
facility list and arch.model.fac->mask points to the KVM facility mask.

This patch fixes the behaviour of KVM for some facilities for guests
that ignore the guest visible facility bits, e.g. guests could use
transactional memory intructions on hosts supporting them even if the
chosen cpu model would not offer them.

The userspace interface is not affected by this change.

a9ca8eb7af s390/ftrace: fix crashes when switching tracers / add notrace to cpu_relax()
With git commit 4d92f50249 ("s390: reintroduce diag 44 calls for
cpu_relax()") I reintroduced a non-trivial cpu_relax() variant on s390.

The difference to the previous variant however is that the new version is
an out-of-line function, which will be traced if function tracing is enabled.

Switching to different tracers includes instruction patching. Therefore this
is done within stop_machine() "context" to prevent that any function tracing
is going on while instructions are being patched.
With the new out-of-line variant of cpu_relax() this is not true anymore,
since cpu_relax() gets called in a busy loop by all waiting cpus within
stop_machine() until function patching is finished.
Therefore cpu_relax() must be marked notrace.

This fixes kernel crashes when frequently switching between "function" and
"function_graph" tracers.

Moving cpu_relax() to a header file again, doesn't work because of header
include order dependencies.

c07af4f1ce mm: add missing __PAGETABLE_{PUD,PMD}_FOLDED defines
Core mm expects __PAGETABLE_{PUD,PMD}_FOLDED to be defined if these page
table levels folded.  Usually, these defines are provided by
<asm-generic/pgtable-nopmd.h> and <asm-generic/pgtable-nopud.h>.

But some architectures fold page table levels in a custom way.  They
need to define these macros themself.  This patch adds missing defines.

The patch fixes mm->nr_pmds underflow and eliminates dead __pmd_alloc()
and __pud_alloc() on architectures without these page table levels.

fb3d1c085c s390: let the compiler do page clearing
The hardware folks told me that for page clearing "when you exactly
know what to do, hand written xc+pfd is usally faster then mvcl for
page clearing, as it saves millicode overhead and parameter parsing
and checking" as long as you dont need the cache bypassing.
Turns out that gcc already does a proper xc,pfd loop.

A small test on z196 that does

for ( i = 0; i < bufsize; i+= 256)
    buff[i] = 0x5;

gets 20% faster (touches every cache line of a page)

buff = mmap(NULL, bufsize,PROT_EXEC|PROT_WRITE|PROT_READ,AP_PRIVATE| MAP_ANONYMOUS,0,0);
for ( i = 0; i < bufsize; i+= 4096)
    buff[i] = 0x5;

is within noise ratio (touches one cache line of a page).

As the clear_page is usually called for first memory accesses
we can assume that at least one cache line is used afterwards,
so this change should be always better.
Another benchmark, a make -j 40 of my testsuite in tmpfs with
hot caches on a 32cpu system:

real     0m1.017s     real     0m0.994s   (~2% faster, but in noise)
user     0m5.339s     user     0m5.016s   (~6% faster)

Convert the following where appropriate:

 (1) S_ISLNK(dentry->d_inode) to d_is_symlink(dentry).

 (2) S_ISREG(dentry->d_inode) to d_is_reg(dentry).

 (3) S_ISDIR(dentry->d_inode) to d_is_dir(dentry).  This is actually more
     complicated than it appears as some calls should be converted to
     d_can_lookup() instead.  The difference is whether the directory in
     question is a real dir with a ->lookup op or whether it's a fake dir with
     a ->d_automount op.

In some circumstances, we can subsume checks for dentry->d_inode not being
NULL into this, provided we the code isn't in a filesystem that expects
d_inode to be NULL if the dirent really *is* negative (ie. if we're going to
use d_inode() rather than d_backing_inode() to get the inode pointer).

Note that the dentry type field may be set to something other than
DCACHE_MISS_TYPE when d_inode is NULL in the case of unionmount, where the VFS
manages the fall-through from a negative dentry to a lower layer.  In such a
case, the dentry type of the negative union dentry is set to the same as the
type of the lower dentry.

However, if you know d_inode is not NULL at the call site, then you can use
the d_is_xxx() functions even in a filesystem.

There is one further complication: a 0,0 chardev dentry may be labelled
DCACHE_WHITEOUT_TYPE rather than DCACHE_SPECIAL_TYPE.  Strictly, this was
intended for special directory entry types that don't have attached inodes.

4ba2815d3b s390/mm: align 64-bit PIE binaries to 4GB
The base address (STACK_TOP / 3 * 2) for a 64-bit program is two thirds
into the 4GB segment at 0x2aa00000000. The randomization added on z13
can eat another 1GB of the remaining 1.33GB to the next 4GB boundary.
In the worst case 300MB are left for the executable + bss which may
cross into the next 4GB segment. This is bad for branch prediction,
therefore align the base address to 4GB to give the program more room
before it crosses the 4GB boundary.

cb9e3c292d mm: vmalloc: pass additional vm_flags to __vmalloc_node_range()
For instrumenting global variables KASan will shadow memory backing memory
for modules.  So on module loading we will need to allocate memory for
shadow and map it at address in shadow that corresponds to the address
allocated in module_alloc().

__vmalloc_node_range() could be used for this purpose, except it puts a
guard hole after allocated area.  Guard hole in shadow memory should be a
problem because at some future point we might need to have a shadow memory
at address occupied by guard hole.  So we could fail to allocate shadow
for module_alloc().

Now we have VM_NO_GUARD flag disabling guard page, so we need to pass into
__vmalloc_node_range().  Add new parameter 'vm_flags' to
__vmalloc_node_range() function.

f56141e3e2 all arches, signal: move restart_block to struct task_struct
If an attacker can cause a controlled kernel stack overflow, overwriting
the restart block is a very juicy exploit target.  This is because the
restart_block is held in the same memory allocation as the kernel stack.

Moving the restart block to struct task_struct prevents this exploit by
making the restart_block harder to locate.

Note that there are other fields in thread_info that are also easy
targets, at least on some architectures.

It's also a decent simplification, since the restart code is more or less
identical on all architectures.

2f859d0dad s390/smp: reduce size of struct pcpu
Reduce the size of struct pcpu, since the pcpu_devices array consists
of NR_CPUS elements of type struct pcpu. For most machines this is just
a waste of memory.
So let's try to make it a bit smaller.
This saves 16k with performance_defconfig.

At least for machines which do have less possible cpus than NR_CPUS this can

There is no reason to initialize the topology cpu masks already while
setup_arch() is being called. It is sufficient to initialize the masks
before the scheduler becomes SMP aware.
Therefore a pre-SMP initcall aka early_initcall is suffucient.

This also allows to convert the cpu_topology array into a per cpu
variable with a later patch. Without this patch this wouldn't be
possible since the per cpu memory areas are not allocated while setup_arch
is executed.

introduced the ECTG for clock id -2. Correct would have been
clock id -3.

Fix the whole mess, CLOCK_THREAD_CPUTIME_ID is based on
CPUCLOCK_SCHED and can not be speed up by the vdso. A speedup
is only available for clock id -3 which is CPUCLOCK_VIRT for
the task currently running on the CPU.

In this, on tile, we have FIRST_USER_ADDRESS defined as 0.  round_up() has
the same type -- int.  PUD_SHIFT.

I think the best way to fix it is to define FIRST_USER_ADDRESS as unsigned
long.  On every arch for consistency.

61f77eda9b mm/hugetlb: reduce arch dependent code around follow_huge_*
Currently we have many duplicates in definitions around
follow_huge_addr(), follow_huge_pmd(), and follow_huge_pud(), so this
patch tries to remove the m.  The basic idea is to put the default
implementation for these functions in mm/hugetlb.c as weak symbols
(regardless of CONFIG_ARCH_WANT_GENERAL_HUGETL B), and to implement
arch-specific code only when the arch needs it.

For follow_huge_addr(), only powerpc and ia64 have their own
implementation, and in all other architectures this function just returns
ERR_PTR(-EINVAL).  So this patch sets returning ERR_PTR(-EINVAL) as
default.

As for follow_huge_(pmd|pud)(), if (pmd|pud)_huge() is implemented to
always return 0 in your architecture (like in ia64 or sparc,) it's never
called (the callsite is optimized away) no matter how implemented it is.
So in such architectures, we don't need arch-specific implementation.

In some architecture (like mips, s390 and tile,) their current
arch-specific follow_huge_(pmd|pud)() are effectively identical with the
common code, so this patch lets these architecture use the common code.

One exception is metag, where pmd_huge() could return non-zero but it
expects follow_huge_pmd() to always return NULL.  This means that we need
arch-specific implementation which returns NULL.  This behavior looks
strange to me (because non-zero pmd_huge() implies that the architecture
supports PMD-based hugepage, so follow_huge_pmd() can/should return some
relevant value,) but that's beyond this cleanup patch, so let's keep it.

Justification of non-trivial changes:
- in s390, follow_huge_pmd() checks !MACHINE_HAS_HPAGE at first, and this
  patch removes the check. This is OK because we can assume MACHINE_HAS_HPAGE
  is true when follow_huge_pmd() can be called (note that pmd_huge() has
  the same check and always returns 0 for !MACHINE_HAS_HPAGE.)
- in s390 and mips, we use HPAGE_MASK instead of PMD_MASK as done in common
  code. This patch forces these archs use PMD_MASK, but it's OK because
  they are identical in both archs.
  In s390, both of HPAGE_SHIFT and PMD_SHIFT are 20.
  In mips, HPAGE_SHIFT is defined as (PAGE_SHIFT + PAGE_SHIFT - 3) and
  PMD_SHIFT is define as (PAGE_SHIFT + PAGE_SHIFT + PTE_ORDER - 3), but
  PTE_ORDER is always 0, so these are identical.

a178220df3 s390/hypfs: Eliminate hypfs interval
Currently the binary hypfs interfaces provides new data only once within
an interval time of one second. This patch removes this restriction and
now new data is returned immediately on every read on a hypfs binary file.

This is done in order to allow more consistent snapshots for programs
that read multiple hypfs binary files.

With this feature, you can read the CPU performance metrics provided by the
z/VM diagnose 0C. This then allows to get the management time for each
online CPU of the guest where the diagnose is executed.

The new debugfs file /sys/kernel/debug/s390_hypfs/diag_0c exports the
diag0C binary data to user space via an open/read/close interface.

The binary data consists out of a header structure followed by an
array that contains the diagnose 0c data for each online CPU.

45cce4ccaf s390/cacheinfo: don't use smp_processor_id() in preemptible context
show_cacheinfo() needs to access the cacheinfo structure of any online
cpu. This was done with using smp_processor_id() as in index while in
preemtible context.
This means the cpu could be offline and the data be gone when it would
be accessed.
Better use any online cpu address and protect the data by get_online_cpus()
and put_online_cpus().

658b6eda20 KVM: s390: add cpu model support
This patch enables cpu model support in kvm/s390 via the vm attribute
interface.

During KVM initialization, the host properties cpuid, IBC value and the
facility list are stored in the architecture specific cpu model structure.

During vcpu setup, these properties are taken to initialize the related SIE
state. This mechanism allows to adjust the properties from user space and thus
to implement different selectable cpu models.

This patch uses the IBC functionality to block instructions that have not
been implemented at the requested CPU type and GA level compared to the
full host capability.

Userspace has to initialize the cpu model before vcpu creation. A cpu model
change of running vcpus is not possible.

f781951299 kvm: add halt_poll_ns module parameter
This patch introduces a new module parameter for the KVM module; when it
is present, KVM attempts a bit of polling on every HLT before scheduling
itself out via kvm_vcpu_block.

This parameter helps a lot for latency-bound workloads---in particular
I tested it with O_DSYNC writes with a battery-backed disk in the host.
In this case, writes are fast (because the data doesn't have to go all
the way to the platters) but they cannot be merged by either the host or
the guest.  KVM's performance here is usually around 30% of bare metal,
or 50% if you use cache=directsync or cache=writethrough (these
parameters avoid that the guest sends pointless flush requests, and
at the same time they are not slow because of the battery-backed cache).
The bad performance happens because on every halt the host CPU decides
to halt itself too.  When the interrupt comes, the vCPU thread is then
migrated to a new physical CPU, and in general the latency is horrible
because the vCPU thread has to be scheduled back in.

With this patch performance reaches 60-65% of bare metal and, more
important, 99% of what you get if you use idle=poll in the guest.  This
means that the tunable gets rid of this particular bottleneck, and more
work can be done to improve performance in the kernel or QEMU.

Of course there is some price to pay; every time an otherwise idle vCPUs
is interrupted by an interrupt, it will poll unnecessarily and thus
impose a little load on the host.  The above results were obtained with
a mostly random value of the parameter (500000), and the load was around
1.5-2.5% CPU usage on one of the host's core for each idle guest vCPU.

The patch also adds a new stat, /sys/kernel/debug/kvm/halt_successful_poll,
that can be used to tune the parameter.  It counts how many HLT
instructions received an interrupt during the polling period; each
successful poll avoids that Linux schedules the VCPU thread out and back
in, and may also avoid a likely trip to C1 and back for the physical CPU.

While the VM is idle, a Linux 4 VCPU VM halts around 10 times per second.
Of these halts, almost all are failed polls.  During the benchmark,
instead, basically all halts end within the polling period, except a more
or less constant stream of 50 per second coming from vCPUs that are not
running the benchmark.  The wasted time is thus very low.  Things may
be slightly different for Windows VMs, which have a ~10 ms timer tick.

The effect is also visible on Marcelo's recently-introduced latency
test for the TSC deadline timer.  Though of course a non-RT kernel has
awful latency bounds, the latency of the timer is around 8000-10000 clock
cycles compared to 20000-120000 without setting halt_poll_ns.  For the TSC
deadline timer, thus, the effect is both a smaller average latency and
a smaller variance.

33692f2759 vm: add VM_FAULT_SIGSEGV handling support
The core VM already knows about VM_FAULT_SIGBUS, but cannot return a
"you should SIGSEGV" error, because the SIGSEGV case was generally
handled by the caller - usually the architecture fault handler.

That results in lots of duplication - all the architecture fault
handlers end up doing very similar "look up vma, check permissions, do
retries etc" - but it generally works.  However, there are cases where
the VM actually wants to SIGSEGV, and applications _expect_ SIGSEGV.

In particular, when accessing the stack guard page, libsigsegv expects a
SIGSEGV.  And it usually got one, because the stack growth is handled by
that duplicated architecture fault handler.

However, when the generic VM layer started propagating the error return
from the stack expansion in commit fee7e49d45 ("mm: propagate error
from stack expansion even for guard page"), that now exposed the
existing VM_FAULT_SIGBUS result to user space.  And user space really
expected SIGSEGV, not SIGBUS.

To fix that case, we need to add a VM_FAULT_SIGSEGV, and teach all those
duplicate architecture fault handlers about it.  They all already have
the code to handle SIGSEGV, so it's about just tying that new return
value to the existing code, but it's all a bit annoying.

This is the mindless minimal patch to do this.  A more extensive patch
would be to try to gather up the mostly shared fault handling logic into
one generic helper routine, and long-term we really should do that
cleanup.

Just from this patch, you can generally see that most architectures just
copied (directly or indirectly) the old x86 way of doing things, but in
the meantime that original x86 model has been improved to hold the VM
semaphore for shorter times etc and to handle VM_FAULT_RETRY and other
"newer" things, so it would be a good idea to bring all those
improvements to the generic case and teach other architectures about
them too.

e6d60b368b s390/ftrace: hotpatch support for function tracing
Make use of gcc's hotpatch support to generate better code for ftrace
function tracing.
The generated code now contains only a six byte nop in each function
prologue instead of a 24 byte code block which will be runtime patched to
support function tracing.
With the new code generation the runtime overhead for supporting function
tracing is close to zero, while the original code did show a significant
performance impact.

4d92f50249 s390: reintroduce diag 44 calls for cpu_relax()
Christian Borntraeger reported that the now missing diag 44 calls (voluntary
time slice end) does cause a performance regression for stop_machine() calls
if a machine has more virtual cpus than the host has physical cpus.

This patch mainly reverts 57f2ffe14f ("s390: remove diag 44 calls from
cpu_relax()") with the exception that we still do not issue diag 44 calls if
running with smt enabled. Due to group scheduling algorithms when running in
LPAR this would lead to significant latencies.
However, when running in LPAR we do not have more virtual than physical cpus.

retry loops. A CPU executing the compare-and-delay instruction stops
until the lock value has changed. This is done to make the locking
code for contended locks to behave better in regard to the multi-
hreading facility. A thread of a core executing a compare-and-delay
will allow the other threads of a core to get a larger share of the
core resources.

a374e892c3 KVM: s390/cpacf: Enable/disable protected key functions for kvm guest
Created new KVM device attributes for indicating whether the AES and
DES/TDES protected key functions are available for programs running
on the KVM guest.  The attributes are used to set up the controls in

49538d1238 KVM: s390: fix bug in sigp emergency signal injection
Currently we are always setting the wrong bit in the
bitmap for pending emergency signals. Instead of using
emerg.code from the passed in irq parameter, we use the
value in our per-vcpu local_int structure, which is always zero.
That means all emergency signals will have address 0 as parameter.
If two CPUs send a SIGP to the same target, one might be lost.

Let's fix this by using the value from the parameter and
also trace the correct value.

3cfad02380 KVM: s390: Take addressing mode into account for MVPG interception
The handler for MVPG partial execution interception does not take
the current CPU addressing mode into account yet, so addresses are
always treated as 64-bit addresses. For correct behaviour, we should
properly handle 24-bit and 31-bit addresses, too.

Since MVPG is defined to work with logical addresses, we can simply
use guest_translate_address() to achieve the required behaviour
(since DAT is disabled here, guest_translate_address() skips the MMU
translation and only translates the address via kvm_s390_logical_to_effective()
and kvm_s390_real_to_abs(), which is exactly what we want here).

2444b352c3 KVM: s390: forward most SIGP orders to user space
Most SIGP orders are handled partially in kernel and partially in
user space. In order to:
- Get a correct SIGP SET PREFIX handler that informs user space
- Avoid race conditions between concurrently executed SIGP orders
- Serialize SIGP orders per VCPU

We need to handle all "slow" SIGP orders in user space. The remaining
ones to be handled completely in kernel are:
- SENSE
- SENSE RUNNING
- EXTERNAL CALL
- EMERGENCY SIGNAL
- CONDITIONAL EMERGENCY SIGNAL
According to the PoP, they have to be fast. They can be executed
without conflicting to the actions of other pending/concurrently
executing orders (e.g. STOP vs. START).

This patch introduces a new capability that will - when enabled -
forward all but the mentioned SIGP orders to user space. The
instruction counters in the kernel are still updated.

ea5f496925 KVM: s390: only one external call may be pending at a time
Only one external call may be pending at a vcpu at a time. For this
reason, we have to detect whether the SIGP externcal call interpretation
facility is available. If so, all external calls have to be injected
using this mechanism.

SIGP EXTERNAL CALL orders have to return whether another external
call is already pending. This check was missing until now.

SIGP SENSE hasn't returned yet in all conditions whether an external
call was pending.

If a SIGP EXTERNAL CALL irq is to be injected and one is already
pending, -EBUSY is returned.

As a SIGP STOP is an interrupt with the least priority, it may only result
in stop of the vcpu when no other interrupts are left pending.

To detect whether a non-stop irq is pending, we need a way to mask out
stop irqs from the general kvm_cpu_has_interrupt() function. For this
reason, the existing function (with an outdated name) is replaced by
kvm_s390_vcpu_has_irq() which allows to mask out pending stop irqs.

This patch removes the famous action_bits and moves the handling of
SIGP STOP AND STORE STATUS directly into the SIGP STOP interrupt.

The new local interrupt infrastructure is used to track pending stop
requests.

STOP irqs are the only irqs that don't get actively delivered. They
remain pending until the stop function is executed (=stop intercept).

If another STOP irq is already pending, -EBUSY will now be returned
(needed for the SIGP handling code).

Migration of pending SIGP STOP (AND STORE STATUS) orders should now
be supported out of the box.

2d00f75942 KVM: s390: forward hrtimer if guest ckc not pending yet
Patch 0759d0681c ("KVM: s390: cleanup handle_wait by reusing
kvm_vcpu_block") changed the way pending guest clock comparator
interrupts are detected. It was assumed that as soon as the hrtimer
wakes up, the condition for the guest ckc is satisfied.

This is however only true as long as adjclock() doesn't speed
up the monotonic clock. Reason is that the hrtimer is based on
CLOCK_MONOTONIC, the guest clock comparator detection is based
on the raw TOD clock. If CLOCK_MONOTONIC runs faster than the
TOD clock, the hrtimer wakes the target VCPU up too early and
the target VCPU will not detect any pending interrupts, therefore
going back to sleep. It will never be woken up again because the
hrtimer has finished. The VCPU is stuck.

As a quick fix, we have to forward the hrtimer until the guest
clock comparator is really due, to guarantee properly timed wake
ups.

As the hrtimer callback might be triggered on another cpu, we
have to make sure that the timer is really stopped and not currently
executing the callback on another cpu. This can happen if the vcpu
thread is scheduled onto another physical cpu, but the timer base
is not migrated. So lets use hrtimer_cancel instead of try_to_cancel.

A proper fix might be to introduce a RAW based hrtimer.

8c0a7ce606 KVM: s390: Allow userspace to limit guest memory size
With commit c6c956b80b ("KVM: s390/mm: support gmap page tables with less
than 5 levels") we are able to define a limit for the guest memory size.

As we round up the guest size in respect to the levels of page tables
we get to guest limits of: 2048 MB, 4096 GB, 8192 TB and 16384 PB.
We currently limit the guest size to 16 TB, which means we end up
creating a page table structure supporting guest sizes up to 8192 TB.

This patch introduces an interface that allows userspace to tune
this limit. This may bring performance improvements for small guests.

Commit 725908110a1f ("s390: add SMT support") added a check for
CONFIG_ZFCPDUMP. But the Kconfig symbol ZFCPDUMP was removed in v3.16

this check will always evaluate to false. No one noticed probably
because the code also checks for CONFIG_CRASH_DUMP which "also enables
s390 zfcpdump".

10ad34bc76 s390: add SMT support
The multi-threading facility is introduced with the z13 processor family.
This patch adds code to detect the multi-threading facility. With the
facility enabled each core will surface multiple hardware threads to the
system. Each hardware threads looks like a normal CPU to the operating
system with all its registers and properties.

The SCLP interface reports the SMT topology indirectly via the maximum
thread id. Each reported CPU in the result of a read-scp-information
is a core representing a number of hardware threads.

To reflect the reduced CPU capacity if two hardware threads run on a
single core the MT utilization counter set is used to normalize the
raw cputime obtained by the CPU timer deltas. This scaled cputime is
reported via the taskstats interface. The normal /proc/stat numbers
are based on the raw cputime and are not affected by the normalization.

To this end, support mapping ranges within BAR on s390.
Since multiple ranges can now be mapped within a BAR, we keep track of

d453cded05 module_arch_freeing_init(): new hook for archs before module->module_init freed.
Archs have been abusing module_free() to clean up their arch-specific
allocations.  Since module_free() is also (ab)used by BPF and trace code,
let's keep it to simple allocations, and provide a hook called before
that.

This means that avr32, ia64, parisc and s390 no longer need to implement
their own module_free() at all.  avr32 doesn't need module_finalize()
either.

Currently the signed COMPARE HALFWORD IMMEDIATE (chi) and COMPARE (c)
instructions are used to compare "A" with "K". This is not correct
because "A" and "K" are both unsigned. To fix this remove the
chi instruction (no unsigned analogon available) and use the
unsigned COMPARE LOGICAL (cl) instruction instead of COMPARE (c).

8d1a2427d8 s390/uprobes: fix user space PER events
If uprobes are single stepped for example with gdb, the behavior should
now be correct. Before this patch, when gdb was single stepping a uprobe,
the result was a SIGILL.
When PER is active for any storage alteration and a uprobe is hit, a storage
alteration event is indicated. These over indications are filterd out by gdb,
if no change has happened within the observed area.

- renamed CLOCK_STORE_SIZE to STORE_CLOCK_EXT_SIZE

SRCU is not necessary to be compiled by default in all cases. For tinification
efforts not compiling SRCU unless necessary is desirable.

The current patch tries to make compiling SRCU optional by introducing a new
Kconfig option CONFIG_SRCU which is selected when any of the components making
use of SRCU are selected.

If we do not select CONFIG_SRCU, srcu.o will not be compiled at all.

032014bc04 s390/vtime: Get rid of redundant WARN_ON
in the cpu time accounting function vtime_account_irq_enter
(vtime_account_system) we use a WARN_ON_ONCE(!irqs_disabled()).
This is redundant as the function virt_timer_forward is always
called and has a BUG_ON(!irqs_disabled()).

This saves several nanoseconds in my specific testcase (KVM entry/exit)
and probably all other callers like (soft)irq entry/exit.

ACCESS_ONCE does not work reliably on non-scalar types. For
example gcc 4.6 and 4.7 might remove the volatile tag for such
accesses during the SRA (scalar replacement of aggregates) step

Commit 1365039d0c ("KVM: s390: Fix ipte locking") replace
ACCESS_ONCE with barriers. Lets use READ_ONCE instead.

  backporting to stable.

  The first change (implicitly adding MNT_NODEV on remount) addresses a
  regression that was created when security issues with unprivileged
  remount were closed.  I go on to update the remount test to make it
  easy to detect if this issue reoccurs.

  Then there are a handful of mount and umount related fixes.

  Then half of the changes deal with the a recently discovered design
  bug in the permission checks of gid_map.  Unix since the beginning has
  allowed setting group permissions on files to less than the user and
  other permissions (aka ---rwx---rwx).  As the unix permission checks
  stop as soon as a group matches, and setgroups allows setting groups
  that can not later be dropped, results in a situtation where it is
  possible to legitimately use a group to assign fewer privileges to a
  process.  Which means dropping a group can increase a processes
  privileges.

  The fix I have adopted is that gid_map is now no longer writable
  without privilege unless the new file /proc/self/setgroups has been
  set to permanently disable setgroups.

  The bulk of user namespace using applications even the applications
  using applications using user namespaces without privilege remain
  unaffected by this change.  Unfortunately this ix breaks a couple user
  space applications, that were relying on the problematic behavior (one
  of which was tools/selftests/mount/unprivileged-remount-test.c).

  To hopefully prevent needing a regression fix on top of my security
  fix I rounded folks who work with the container implementations mostly
  like to be affected and encouraged them to test the changes.

957e3facd1 gcov: enable GCOV_PROFILE_ALL from ARCH Kconfigs
Following the suggestions from Andrew Morton and Stephen Rothwell,
Dont expand the ARCH list in kernel/gcov/Kconfig. Instead,
define a ARCH_HAS_GCOV_PROFILE_ALL bool which architectures
can enable.

set ARCH_HAS_GCOV_PROFILE_ALL on Architectures where it was
previously allowed + ARM64 which I tested.

031bc5743f mm/debug-pagealloc: make debug-pagealloc boottime configurable
Now, we have prepared to avoid using debug-pagealloc in boottime.  So
introduce new kernel-parameter to disable debug-pagealloc in boottime, and
makes related functions to be disabled in this case.

Only non-intuitive part is change of guard page functions.  Because guard
page is effective only if debug-pagealloc is enabled, turning off
according to debug-pagealloc is reasonable thing to do.

1077fa36f2 arch: Add lightweight memory barriers dma_rmb() and dma_wmb()
There are a number of situations where the mandatory barriers rmb() and
wmb() are used to order memory/memory operations in the device drivers
and those barriers are much heavier than they actually need to be.  For
example in the case of PowerPC wmb() calls the heavy-weight sync
instruction when for coherent memory operations all that is really needed
is an lsync or eieio instruction.

This commit adds a coherent only version of the mandatory memory barriers
rmb() and wmb().  In most cases this should result in the barrier being the
same as the SMP barriers for the SMP case, however in some cases we use a
barrier that is somewhere in between rmb() and smp_rmb().  For example on
ARM the rmb barriers break down as follows:

  dma_rmb() dmb(osh) data memory barrier - outer sharable
  smp_rmb() dmb(ish) data memory barrier - inner sharable

These new barriers are not as safe as the standard rmb() and wmb().
Specifically they do not guarantee ordering between coherent and incoherent
memories.  The primary use case for these would be to enforce ordering of
reads and writes when accessing coherent memory that is shared between the
CPU and a device.

It may also be noted that there is no dma_mb().  Most architectures don't
provide a good mechanism for performing a coherent only full barrier without
resorting to the same mechanism used in mb().  As such there isn't much to
be gained in trying to define such a function.

8a44971841 arch: Cleanup read_barrier_depends() and comments
This patch is meant to cleanup the handling of read_barrier_depends and
smp_read_barrier_depends.  In multiple spots in the kernel headers
read_barrier_depends is defined as "do {} while (0)", however we then go
into the SMP vs non-SMP sections and have the SMP version reference
read_barrier_depends, and the non-SMP define it as yet another empty
do/while.

With this commit I went through and cleaned out the duplicate definitions
and reduced the number of definitions down to 2 per header.  In addition I
moved the 50 line comments for the macro from the x86 and mips headers that
defined it as an empty do/while to those that were actually defining the
macro, alpha and blackfin.
