422c0d61d5 [SCSI] use scmd_id(), scmd_channel() throughout code
Wrap a highly common idiom.  Makes the code easier to read, helps pave
the way for sdev->{id,channel} removal, and adds a token that can easily
by grepped-for in the future.

There are a couple sdev_id() and scmd_printk() updates thrown in as well.

a4c8f62851 [SCSI] remove scsi_cmnd.eh_state
it's never set to anything, and just three broken drivers are looking
at it and doing odd things.

3471c28803 [SCSI] Remove no-op implementations of SCSI EH hooks
Drivers need not implement a hook that returns FAILED, and does nothing
else, since the SCSI midlayer code will do that for us.

1da177e4c3 Linux-2.6.12-rc2
Initial git repository build. I'm not bothering with the full history,
even though we have it. We can create a separate "historical" git
archive of that later if we want to, and in the meantime it's about
3.2GB when imported into git - space that would just make the early
git days unnecessarily complicated, when we don't have a lot of good
infrastructure for it.

Let it rip!
