57efa1fe59 mm/gup: prevent gup_fast from racing with COW during fork
Since commit 70e806e4e6 ("mm: Do early cow for pinned pages during
fork() for ptes") pages under a FOLL_PIN will not be write protected
during COW for fork.  This means that pages returned from
pin_user_pages(FOLL_WRITE) should not become write protected while the pin
is active.

However, there is a small race where get_user_pages_fast(FOLL_PIN) can
establish a FOLL_PIN at the same time copy_present_page() is write
protecting it:

The first attempt to resolve this by using the write protect caused
problems (and was missing a barrrier), see commit f3c64eda3e ("mm: avoid
early COW write protect games during fork()")

Instead wrap copy_p4d_range() with the write side of a seqcount and check
the read side around gup_pgd_range().  If there is a collision then
get_user_pages_fast() fails and falls back to slow GUP.

Slow GUP is safe against this race because copy_page_range() is only
called while holding the exclusive side of the mmap_lock on the src
mm_struct.

e2be2a833a x86/tboot: Don't disable swiotlb when iommu is forced on
After commit 327d5b2fee ("iommu/vt-d: Allow 32bit devices to uses DMA
domain"), swiotlb could also be used for direct memory access if IOMMU
is enabled but a device is configured to pass through the DMA translation.
Keep swiotlb when IOMMU is forced on, otherwise, some devices won't work
if "iommu=pt" kernel parameter is used.

4d213e76a3 iommu/vt-d: Avoid panic if iommu init fails in tboot system
"intel_iommu=off" command line is used to disable iommu but iommu is force
enabled in a tboot system for security reason.

However for better performance on high speed network device, a new option
"intel_iommu=tboot_noforce" is introduced to disable the force on.

By default kernel should panic if iommu init fail in tboot for security
reason, but it's unnecessory if we use "intel_iommu=tboot_noforce,off".

Fix the code setting force_on and move intel_iommu_tboot_noforce
from tboot code to intel iommu code.

e31cf2f4ca mm: don't include asm/pgtable.h if linux/mm.h is already included
Patch series "mm: consolidate definitions of page table accessors", v2.

The low level page table accessors (pXY_index(), pXY_offset()) are
duplicated across all architectures and sometimes more than once.  For
instance, we have 31 definition of pgd_offset() for 25 supported
architectures.

Most of these definitions are actually identical and typically it boils
down to, e.g.

These definitions can be shared among 90% of the arches provided
XYZ_SHIFT, PTRS_PER_XYZ and xyz_page_vaddr() are defined.

For architectures that really need a custom version there is always
possibility to override the generic version with the usual ifdefs magic.

These patches introduce include/linux/pgtable.h that replaces
include/asm-generic/pgtable.h and add the definitions of the page table
accessors to the new header.

This patch (of 12):

The linux/mm.h header includes <asm/pgtable.h> to allow inlining of the
functions involving page table manipulations, e.g.  pte_alloc() and
pmd_alloc().  So, there is no point to explicitly include <asm/pgtable.h>
in the files that include <linux/mm.h>.

The include statements in such cases are remove with a simple loop:

daedaa33d9 iommu/vtd: Cleanup dma_remapping.h header
Commit e61d98d8da ("x64, x2apic/intr-remap: Intel vt-d, IOMMU
code reorganization") moved dma_remapping.h from drivers/pci/ to
current place. It is entirely VT-d specific, but uses a generic
name. This merges dma_remapping.h with include/linux/intel-iommu.h
and removes dma_remapping.h as the result.

445b69e3b7 x86/pti: Make unpoison of pgd for trusted boot work for real
The inital fix for trusted boot and PTI potentially misses the pgd clearing
if pud_alloc() sets a PGD.  It probably works in *practice* because for two
adjacent calls to map_tboot_page() that share a PGD entry, the first will
clear NX, *then* allocate and set the PGD (without NX clear).  The second
call will *not* allocate but will clear the NX bit.

Defer the NX clearing to a point after it is known that all top-level
allocations have occurred.  Add a comment to clarify why.

262b6b3008 x86/tboot: Unbreak tboot with PTI enabled
This is another case similar to what EFI does: create a new set of
page tables, map some code at a low address, and jump to it.  PTI
mistakes this low address for userspace and mistakenly marks it
non-executable in an effort to make it unusable for userspace.

Undo the poison to allow execution.

bfd20f1cc8 x86, iommu/vt-d: Add an option to disable Intel IOMMU force on
IOMMU harms performance signficantly when we run very fast networking
workloads. It's 40GB networking doing XDP test. Software overhead is
almost unaware, but it's the IOTLB miss (based on our analysis) which
kills the performance. We observed the same performance issue even with
software passthrough (identity mapping), only the hardware passthrough
survives. The pps with iommu (with software passthrough) is only about
~30% of that without it. This is a limitation in hardware based on our
observation, so we'd like to disable the IOMMU force on, but we do want
to use TBOOT and we can sacrifice the DMA security bought by IOMMU. I
must admit I know nothing about TBOOT, but TBOOT guys (cc-ed) think not
eabling IOMMU is totally ok.

So introduce a new boot option to disable the force on. It's kind of
silly we need to run into intel_iommu_init even without force on, but we
need to disable TBOOT PMR registers. For system without the boot option,
nothing is changed.

So the e820_table->map and e820_table->nr_map names are a bit
confusing, because it's not clear what a 'map' really means
(it could be a bitmap, or some other data structure), nor is
it clear what nr_map means (is it a current index, or some
other count).

Rename the fields from:

 e820_table->map        =>     e820_table->entries
 e820_table->nr_map     =>     e820_table->nr_entries

which makes it abundantly clear that these are entries
of the table, and that the size of the table is ->nr_entries.

Propagate the changes to all affected files. Where necessary,
adjust local variable names to better reflect the new field names.

No change in functionality.

When the state names got added a script was used to add the extra argument
to the calls. The script basically converted the state constant to a
string, but the cleanup to convert these strings into meaningful ones did
not happen.

are used in all the other places already.

There is already a check for boot_params.tboot_addr prior
to paravirt_enabled(). Both Xen and lguest, which are also the
only ones that set paravirt_enabled to true, never set the
boot_params.tboot_addr. The Xen folks are sure a force disable
to 0 is not needed, we recently forced disabled this on lguest.
With this in place this check is no longer needed.

Xen folks are sure force disable to 0 is not needed because
apm_info lives in .bss, we recently forced disabled this on
lguest, and on the Xen side just to be sure Boris zeroed out
the .bss for PV guests through commit 04b6b4a568
("xen/x86: Zero out .bss for PV guests"). With this care taken
into consideration the paravirt_enabled() check is simply not
needed anymore.

   The only direct user of __pte_alloc left is userfaultfd, which has
   different expectation about atomicity wrt pmd.

148f9bb877 x86: delete __cpuinit usage from all x86 files
The __cpuinit type of throwaway sections might have made sense
some time ago when RAM was more constrained, but now the savings
do not offset the cost and complications.  For example, the fix in
commit 5e427ec2d0 ("x86: Fix bit corruption at CPU resume time")
is a good example of the nasty type of bugs that can be created
with improper use of the various __init prefixes.

After a discussion on LKML[1] it was decided that cpuinit should go
the way of devinit and be phased out.  Once all the users are gone,
we can then finally remove the macros themselves from linux/init.h.

Note that some harmless section mismatch warnings may result, since
notify_cpu_starting() and cpu_up() are arch independent (kernel/cpu.c)
are flagged as __cpuinit  -- so if we remove the __cpuinit from
arch specific callers, we will also get section mismatch warnings.
As an intermediate step, we intend to turn the linux/init.h cpuinit
content into no-ops as early as possible, since that will get rid
of these warnings.  In any case, they are temporary and harmless.

This removes all the arch/x86 uses of the __cpuinit macros from
all C files.  x86 only had the one __CPUINIT used in assembly files,
and it wasn't paired off with a .previous or a __FINIT, so we can
delete it directly w/o any corresponding additional change there.

da5a108d05 x86/kernel: remove tboot 1:1 page table creation code
For TXT boot, while Linux kernel trys to shutdown/S3/S4/reboot, it
need to jump back to tboot code and do TXT teardown work. Previously
kernel zapped all mem page identity mapping (va=pa) after booting, so
tboot code mem address was mapped again with identity mapping. Now
kernel didn't zap the identity mapping page table, so tboot related
code can remove the remapping code before trapping back now.

09f98a825a x86, acpi, tboot: Have a ACPI os prepare sleep instead of calling tboot_sleep.
The ACPI suspend path makes a call to tboot_sleep right before
it writes the PM1A, PM1B values. We replace the direct call to
tboot via an registration callback similar to __acpi_register_gsi.

These files were implicitly getting EXPORT_SYMBOL via device.h
which was including module.h, but that will be fixed up shortly.

b7f080cfe2 net: remove mm.h inclusion from netdevice.h
Remove linux/mm.h inclusion from netdevice.h -- it's unused (I've checked manually).

To prevent mm.h inclusion via other channels also extract "enum dma_data_direction"
definition into separate header. This tiny piece is what gluing netdevice.h with mm.h
via "netdevice.h => dmaengine.h => dma-mapping.h => scatterlist.h => mm.h".
Removal of mm.h from scatterlist.h was tried and was found not feasible
on most archs, so the link was cutoff earlier.

Hope people are OK with tiny include file.

Note, that mm_types.h is still dragged in, but it is a separate story.

de03c72cfc mm: convert mm->cpu_vm_cpumask into cpumask_var_t
cpumask_t is very big struct and cpu_vm_mask is placed wrong position.
It might lead to reduce cache hit ratio.

This patch has two change.
1) Move the place of cpumask into last of mm_struct. Because usually cpumask
   is accessed only front bits when the system has cpu-hotplug capability
2) Convert cpu_vm_mask into cpumask_var_t. It may help to reduce memory
   footprint if cpumask_size() will use nr_cpumask_bits properly in future.

In addition, this patch change the name of cpu_vm_mask with cpu_vm_mask_var.
It may help to detect out of tree cpu_vm_mask users.

This patch has no functional change.

8ac1f8320a thp: pte alloc trans splitting
pte alloc routines must wait for split_huge_page if the pmd is not present
and not null (i.e.  pmd_trans_splitting).  The additional branches are
optimized away at compile time by pmd_trans_splitting if the config option
is off.  However we must pass the vma down in order to know the anon_vma
lock to wait for.

Per document, for feature control MSR:

  Bit 1 enables VMXON in SMX operation. If the bit is clear, execution
        of VMXON in SMX operation causes a general-protection exception.
  Bit 2 enables VMXON outside SMX operation. If the bit is clear, execution
        of VMXON outside SMX operation causes a general-protection exception.

This patch is to enable this kind of check with SMX for VMXON in KVM.

4bd96a7a81 x86, tboot: Add support for S3 memory integrity protection
This patch adds support for S3 memory integrity protection within an Intel(R)
TXT launched kernel, for all kernel and userspace memory.  All RAM used by the
kernel and userspace, as indicated by memory ranges of type E820_RAM and
E820_RESERVED_KERN in the e820 table, will be integrity protected.

The MAINTAINERS file is also updated to reflect the maintainers of the
TXT-related code.

All MACing is done in tboot, based on a complexity analysis and tradeoff.

v3: Compared with v2, this patch adds a check of array size in
tboot.c, and a note to specify which c/s of tboot supports this kind
of MACing in intel_txt.txt.

On 32 bits, we can have CONFIG_ACPI_SLEEP set without implying
CONFIG_X86_TRAMPOLINE.  In that case, we simply do not need to mark
the trampoline as a MAC region.

58c41d2825 x86, intel_txt: Factor out the code for S3 setup
S3 sleep requires special setup in tboot.  However, the data
structures needed to do such setup are only available if
CONFIG_ACPI_SLEEP is enabled.  Abstract them out as much as possible,
so we can have a single tboot_setup_sleep() which either is a proper
implementation or a stub which simply calls BUG().

This patch adds kernel configuration and boot support for Intel Trusted
Execution Technology (Intel TXT).

Intel's technology for safer computing, Intel Trusted Execution
Technology (Intel TXT), defines platform-level enhancements that
provide the building blocks for creating trusted platforms.

Intel TXT was formerly known by the code name LaGrande Technology (LT).

Intel TXT in Brief:
o  Provides dynamic root of trust for measurement (DRTM)
o  Data protection in case of improper shutdown
o  Measurement and verification of launched environment

Intel TXT is part of the vPro(TM) brand and is also available some
non-vPro systems.  It is currently available on desktop systems based on
the Q35, X38, Q45, and Q43 Express chipsets (e.g. Dell Optiplex 755, HP
dc7800, etc.) and mobile systems based on the GM45, PM45, and GS45
Express chipsets.
