Change the error code for an invalid SCSI opcode to DID_ERROR.
INITIATOR_ERROR is a scsi parallel message which doesn't apply for RAID

84ff9532e5 scsi: dpt_i2o: use scsi_host_complete_all_commands() to abort outstanding commands
Rather than traversing all outstanding commands manually, use the
scsi_host_complete_all_commands() helper to terminate all commands during
reset.  With that we can drop the cmd_list usage from the midlayer.

22f0ba4a28 scsi: dpt_i2o: rename adpt_i2o_to_scsi() to adpt_i2o_scsi_complete()
Rename the badly named function into adpt_i2o_scsi_complete(), and make it
a void function as the return value is never used.  This also fixes a
potential use-after-free as the return value might be evaluated from the
command result after the command has been freed.

2a3d4eb8e2 scsi: flip the default on use_clustering
Most SCSI drivers want to enable "clustering", that is merging of
segments so that they might span more than a single page.  Remove the
ENABLE_CLUSTERING define, and require drivers to explicitly set
DISABLE_CLUSTERING to disable this feature.

c79cd9a24e scsi: dpt_i2o: use dma_set_mask
The driver currently uses pci_set_dma_mask despite otherwise using the
generic DMA API.  Also move the dma_get_required_mask check before actually
setting the dma mask, so that we don't end up with inconsistent settings in
corner cases.

60d6d22d85 scsi: dpt_i2o: Remove VLA usage
On the quest to remove all VLAs from the kernel[1] this moves the
sg_list variable off the stack, as already done for other allocated
buffers in adpt_i2o_passthru(). Additionally consolidates the error path
for kfree().

447a5647c9 treewide: Align function definition open/close braces
Some functions definitions have either the initial open brace and/or
the closing brace outside of column 1.

Move those braces to column 1.

This allows various function analyzers like gnu complexity to work
properly for these modified functions.

There are two places where adpt_i2o_online_hba() is called.  Both
callers call adpt_i2o_delete_hba(pHba) if adpt_i2o_online_hba() fails
and since we also free it here that causes a double free bug.

af25756de9 scsi: dpt_i2o: double free on error path
We recently introduced a kfree() in the caller for this function.
That's where, logically, you would think the kfree() should be.
Unfortunately the code was just ugly and not buggy so the static checker
warning was a false postive and introduced a double free.

I've removed the old kfree() and left the new one.

ba3e127ec1 x86/vm86: Clean up vm86.h includes

c8b09f6fb6 scsi: don't set tagging state from scsi_adjust_queue_depth
Remove the tagged argument from scsi_adjust_queue_depth, and just let it
handle the queue depth.  For most drivers those two are fairly separate,
given that most modern drivers don't care about the SCSI "tagged" status
of a command at all, and many old drivers allow queuing of multiple
untagged commands in the driver.

Instead we start out with the ->simple_tags flag set before calling
->slave_configure, which is how all drivers actually looking at
->simple_tags except for one worke anyway.  The one other case looks
broken, but I've kept the behavior as-is for now.

Except for that we only change ->simple_tags from the ->change_queue_type,
and when rejecting a tag message in a single driver, so keeping this
churn out of scsi_adjust_queue_depth is a clear win.

Now that the usage of scsi_adjust_queue_depth is more obvious we can
also remove all the trivial instances in ->slave_alloc or ->slave_configure
that just set it to the cmd_per_lun default.

Add a use_cmd_list flag in struct Scsi_Host to request keeping track of
all outstanding commands per device.

Default behaviour is not to keep track of cmd_list per sdev, as this may
introduce lock contention. (overhead is more on multi-node NUMA.), and
only enable it on the two drivers that need it.

f281233d3e SCSI host lock push-down
Move the mid-layer's ->queuecommand() invocation from being locked
with the host lock to being unlocked to facilitate speeding up the
critical path for drivers who don't need this lock taken anyway.

The patch below presents a simple SCSI host lock push-down as an
equivalent transformation.  No locking or other behavior should change
with this patch.  All existing bugs and locking orders are preserved.

Additionally, add one parameter to queuecommand,
	struct Scsi_Host *
and remove one parameter from queuecommand,
	void (*done)(struct scsi_cmnd *)

Scsi_Host* is a convenient pointer that most host drivers need anyway,
and 'done' is redundant to struct scsi_cmnd->scsi_done.

Minimal code disturbance was attempted with this change.  Most drivers
needed only two one-line modifications for their host lock push-down.

6038f373a3 llseek: automatically add .llseek fop
All file_operations should get a .llseek operation so we can make
nonseekable_open the default for future file operations without a
.llseek pointer.

The three cases that we can automatically detect are no_llseek, seq_lseek
and default_llseek. For cases where we can we can automatically prove that
the file offset is always ignored, we use noop_llseek, which maintains
the current behavior of not returning an error from a seek.

New drivers should normally not use noop_llseek but instead use no_llseek
and call nonseekable_open at open time.  Existing drivers can be converted
to do the same when the maintainer knows for certain that no user code
relies on calling seek on the device file.

The generated code is often incorrectly indented and right now contains
comments that clarify for each added line why a specific variant was
chosen. In the version that gets submitted upstream, the comments will
be gone and I will manually fix the indentation, because there does not
seem to be a way to do that using coccinelle.

Some amount of new code is currently sitting in linux-next that should get
the same modifications, which I will do at the end of the merge window.

c45d15d24e scsi: autoconvert trivial BKL users to private mutex
All these files use the big kernel lock in a trivial
way to serialize their private file operations,
typically resulting from an earlier semi-automatic
pushdown from VFS.

None of these drivers appears to want to lock against
other code, and they all use the BKL as the top-level
lock in their file operations, meaning that there
is no lock-order inversion problem.

Consequently, we can remove the BKL completely,
replacing it with a per-file mutex in every case.
Using a scripted approach means we can avoid
typos.

adpt_i2o_delete_hba() calls kfree() so we have to save "pHba->next"
before calling it.  Also inside adpt_i2o_delete_hba() itself, there
