It's still an obsolete interface; don't introduce those in new drivers.
However, it's saner than the ->proc_info() and commits after this one
will convert the existing ->proc_info() users to it.
The read side is ->show_info(seq_file *, struct Scsi_Host *); use
seq_... for generating contents.
The write side is ->write_info(struct Scsi_Host *, char *, int).
Again, this is driven by procfs needs; we are going to kill ->write_proc()
and ->read_proc() and this is the main obstacle to burying that piece of
shit.

This patch adds queue steering to virtio-scsi. When a target is sent
multiple requests, we always drive them to the same queue so that FIFO
processing order is kept. However, if a target was idle, we can choose
a queue arbitrarily. In this case the queue is chosen according to the
current VCPU, so the driver expects the number of request queues to be
equal to the number of VCPUs. This makes it easy and fast to select
the queue, and also lets the driver optimize the IRQ affinity for the
virtqueues (each virtqueue's affinity is set to the CPU that "owns"
the queue).
The speedup comes from improving cache locality and giving CPU affinity
to the virtqueues, which is why this scheme was selected. Assuming that
the thread that is sending requests to the device is I/O-bound, it is
likely to be sleeping at the time the ISR is executed, and thus executing
the ISR on the same processor that sent the requests is cheap.
However, the kernel will not execute the ISR on the "best" processor
unless you explicitly set the affinity. This is because in practice
you will have many such I/O-bound processes and thus many otherwise
idle processors. Then the kernel will execute the ISR on a random
processor, rather than the one that is sending requests to the device.
The alternative to per-CPU virtqueues is per-target virtqueues. To
achieve the same locality, we could dynamically choose the virtqueue's
affinity based on the CPU of the last task that sent a request. This
is less appealing because we do not set the affinity directly---we only
provide a hint to the irqbalanced running in userspace. Dynamically
changing the affinity only works if the userspace applies the hint
fast enough.

virtio_scsi_target_state is now empty. We will find new uses for it in
the next few patches, so this patch does not drop it completely.
And as James suggested, we use entries target_alloc and target_destroy
in the host template to allocate and destroy the virtio_scsi_target_state
of each target, attach this struct to scsi_target->hostdata. Now
we can get at it from the sdev with scsi_target(sdev)->hostdata.
No messing around with fixed size arrays and bulk memory allocation
and no need to pass in the maximum target size as a parameter because
everything should now happen dynamically.

If a result of the SMP discover function is PHY VACANT,
the content of discover response structure (dr) is not valid.
It sometimes happens that dr->attached_sas_addr can contain
even SAS address of other phy. In such case an invalid phy
is created, what causes NULL pointer dereference during
destruction of expander's phys.
So if a result of SMP function is PHY VACANT, the content of discover
response structure (dr) must not be copied to phy structure.

System crashes, in initiator mode operation, with
qla2xxx_copy_atioqueues() in stack trace when firmware dump is
attempted.
Check for atio_q_length alone does not indicate if atio_ring is
allocated, make explicit check of atio_ring to avoid the crash.

This patch fixes a reference count bug in the SCSI tape driver which can be
reproduced with the following:
* Boot with slub_debug=FZPU, tape drive attached
* echo 1 > /sys/devices/... tape device pci path .../remove
* Wait for device removal
* echo 1 > /sys/kernel/slab/blkdev_queue/validate
* Slub debug complains about corrupted poison pattern
In commit 523e1d39 (block: make gendisk hold a reference to its queue)
add_disk() and disk_release() were modified to get/put an additional
reference on a disk queue to fix a reference counting discrepency
between bdev release and SCSI device removal. The ST driver never
calls add_disk(), so this commit introduced an extra kref put when the
ST driver frees its struct gendisk.
Attempts were made to fix this bug at the block level [1] but later
abandoned due to floppy driver issues [2].

The libfc discovery layer is being initialized in the
'create' paths for both legacy libfcoe module parameters
and fcoe_sysfs control interfaces. The problem is that
for VN2VN mode the discovery layer is initialized as if
it were in 'fabric' mode and it is not re-configured when
the mode is changed to 'vn2vn'.
This patch splits out code that needs to be initialized
once and code that can, and should be, re-configured when
the mode changes. Additionally this patch makes that change
so that the discovery layer can be reconfigured to the
libfcoe implementation when in 'vn2vn' mode.

libfc. This patch moves this code into libfc and sets up a split in discovery
(one time) initialization code and (re-configurable) settings that will come in

The fcoemon userspace daemon is searching for the a hostX
under the the /sys/bus/fcoe/devices/ctlrX/ entries. When
interfaces created using fcoe_sysfs and fcoe.ko this linkage
is setup correctly, but bnx2fc is not doing the same thing
and therefore fcoemon does not create the fcoe interface
for bnx2fc.
This patch sets up the correct linkage for bnx2fc such that
fcoemon will work correctly with fcoe_sysfs and bnx2fc.

More prep work for immutable bvecs/effecient bio splitting - usage of
bi_vcnt has to be auditing, so getting rid of all the unnecessary usage
makes that easier.
Plus, bio_segments() is really what this code wanted, as it respects the

USB uses the .find_bridge() callback from struct acpi_bus_type
incorrectly, because as a result of the way it is used by USB every
device in the system that doesn't have a bus type or parent is
passed to usb_acpi_find_device() for inspection.
What USB actually needs, though, is to call usb_acpi_find_device()
for USB ports that don't have a bus type defined, but have
usb_port_device_type as their device type, as well as for USB
devices.
To fix that replace the struct bus_type pointer in struct
acpi_bus_type used for matching devices to specific subsystems
with a .match() callback to be used for this purpose and update
the users of struct acpi_bus_type, including USB, accordingly.
Define the .match() callback routine for USB, usb_acpi_bus_match(),
in such a way that it will cover both USB devices and USB ports
and remove the now redundant .find_bridge() callback pointer from
usb_acpi_bus.

On architectures where wmb() is defined as mb(), a build error
happens since there is also a variable named 'mb' in lpfc_sli.c's

Also blacklist WRITE_SAME to avoid (and log) accident usage.

This happens with a guest running on Windows Server 2012, but happens to
work while running on Windows Server 2008. WRITE_SAME isnt really
supported by both versions, so disable the command usage globally.

When resetting an ipr adapter, we use scsi_block_requests to
block any new commands from scsi core, and then unblock after
the reset. When hotplug removing an adapter, we shut it down
and go through this same code, but we've seen issues with
scsi_unblock_requests running after the adapter's memory has
been freed. There is really no need to block/unblock when
the adapter is being removed, so this patch skips the
block/unblock and will immediately fail any commands that
happen to make it to queuecommand while the adapter is
being shutdown.

Fnic Trace utility is a tracing functionality built directly into fnic driver
to trace events. The benefit that trace buffer brings to fnic driver is the
ability to see what it happening inside the fnic driver. It also provides the
capability to trace every IO event inside fnic driver to debug panics, hangs
and potentially IO corruption issues. This feature makes it easy to find
problems in fnic driver and it also helps in tracking down strange bugs in a
more manageable way. Trace buffer is shared across all fnic instances for
this implementation.

The issue was observed when LUN Reset is issued through IOCTL or sg_reset
utility.
fnic driver issues LUN RESET to firmware. On successful completion of device
reset, driver cleans up all the pending IOs that were issued prior to device
reset. These pending IOs are expected to be in ABTS_PENDING state. This works
fine, when the device reset operation resulted from midlayer, but not when
device reset was triggered from IOCTL path as the pending IOs were not in
ABTS_PENDING state. execution path hits panic if the pending IO is not in
ABTS_PENDING state.
Changes:
The fix replaces BUG_ON check in fnic_clean_pending_aborts() with marking
pending IOs as ABTS_PENDING if they were not in ABTS_PENDING state and skips
if they were already in ABTS_PENDING state. An extra check is added to validate
the abort status of the commands after a delay of 2 * E_D_TOV using a
helper function. The helper function returns 1 if it finds any pending IO in
ABTS_PENDING state, belong to the LUN on which device reset was issued else 0.
With this, device reset operation returns success only if the helper funciton
returns 0, otherwise it returns failure.
Other changes:
- Removed code in fnic_clean_pending_aborts() that returns failure if it finds
io_req NULL, instead of returning failure added code to continue with next io
- Added device reset flags for debugging in fnic_terminate_rport_io,
fnic_rport_exch_reset, and fnic_clean_pending_aborts

Driver allows IOs with more SGEs than max SGEs supported by Palo. The current
max SGEs supported by the fnic driver is 1024. The current register settings
on Palo supports a max of 256 only. Palo would return any IO with more than
256 SGEs with an error indicating INVALID_SGLS. Fnic driver should limit the
max supported SGLs in the driver to 256 to avoid this error.

1. Handling overlapped firmware resets
This fix serialize multiple firmware resets to avoid situation where fnic
device fails to come up for link up event, when firmware resets are issued
back to back. If there are overlapped firmware resets are issued,
the firmware reset operation checks whether there is any firmware reset in
progress, if so it polls for its completion in a loop with 100ms delay.
2. Handling device reset timeout
fnic_device_reset code has been modified to handle Device reset timeout:
- Issue terminate on device reset timeout.
- Introduced flags field (one of the scratch fields in scsi_cmnd).
With this, device reset request would have DEVICE_RESET flag set for other
routines to determine the type of the request.
Also modified fnic_terminate_rport_io, fnic_rport_exch_rset, completion
routines to handle SCSI commands with DEVICE_RESET flag.
3. LUN/Device Reset hangs when issued through IOCTL using utilities like
sg_reset.
Each SCSI command is associated with a valid tag, fnic uses this tag to
retrieve associated scsi command on completion. the LUN/Device Reset issued
through IOCTL resulting into a SCSI command that is not associated with a
valid tag. So fnic fails to retrieve associated scsi command on completion,
which causes hang. This fix allocates tag, associates it with the
scsi command and frees the tag, when the operation completed.
4. Preventing IOs during firmware reset.
Current fnic implementation allows IO submissions during firmware reset.
This fix synchronizes IO submissions and firmware reset operations.
It ensures that IOs issued to fnic prior to reset will be issued to the
firmware before firmware reset.

The following patch for megaraid_sas will fix an issue where the
driver should not be loading the DevHandle unless FastPath is enabled.
If FastPath was not enabled, this means the hardware raid map
validation failed for some reason, or the map was corrupted, which
could mean the DevHandle could be invalid.

Currently,the driver is processes RSCNs for each new NPIV ports that is created.
Processing the RSCN includes a name server query to see what has changed at the
name server side. The name server query is performed by the physical port and
each virtual port on the physical adapter (since the RSCN is passed to each
virtual port for processing). As the number of virtual ports being created
increases, this causes a lot of traffic and busies the firmware.
Processing the RSCN for a virtual port we already have a priori knowledge of is
not necessary so check the 24-bit fabric ID of the RSCN entry and skip
processing it if the RSCN is for a virtual port we already know about.

Code in qla24xx_report_id_acquisition() incorrectly assumed that
upon completion of list iteration (with no match), the 'pos'
(vp) variable passed to list_for_each_entry() would be set to
NULL. In this context, if the firmware were to return an
unrecognized vp_idx, the follow-on assignments to vp-members
could result in corruption of the structure.

It seems that the sequence of a strcpy followed by a strlen confuses GCC
when it is keeping track of array bounds here. (It is not clear to me

code in these two functions doesn't trigger this warning.)
An easy way to silence these warnings is to use preprocessor macros and
strncpy, as that apparently gives GCC enough information to keep track
of array bounds.

Add the following error handling for loopback diagnostic mode with ISP83xx:
1. If we do not receive an MBA_DCBX_COMPLETE after our initial set port
configuration command, try to reset the port back into normal operation.
If that fails, take a FCoE dump and then reset the chip.
2. After completing the loopback diagnostic operation, if the reset of the port
back into normal operation fails then reset the port so we take a FCoE dump
and then reset the chip.
3. When we receive an IDC notification and the requested operation is loopback
extend the loop down timer so the link does not appear to down for an
extended period of time.

There's a subtle race in the loop/bus-reset handling whereby a
VHA's loop-state can get incorrectly set to 'down' after the
loop-reset and firmware's completion of link re-negotiation. The
original code incorrectly assumes that firmware AENs would arrive
only after mailbox-command execution to initiate the link-flap.

Root cause is libsas will clear asd_sas_port phy_mask value in sas_port_deform
after triggering destruct workqueue, but the workqueue will send sync cmd and
still need phy_mask value. Now, mvsas using asd_sas_phy setting instead of
asd_sas_port setting.

When there are multiple FCFs in the fabric, and one of them becomes
unavailable, the fabric name for the unavailable FCF becomes 0 along
with FIP_FL_AVAIL getting reset. In this case, FCF selection logic does
not select any FCF as it first checks for conflicting FCFs (since fabric
name is 0, it fails the condition), instead of first checking if it is
usable or not. Fix it by first checking if FCF is usable and skip that
FCF, and go to the next one in the list to check if it can be selected.

schedule_delayed_work() is using msec instead of jiffies. On PLOGI
reject from target, remote port retry is scheduled @ 20 sec instead
of 2sec(FC_DEF_E_D_TOV).

When a CVL is received while we wait to select best FCF, we drop it
without handling it. This causes initiator and the switch to go
out-of-sync. Initiator proceeds selecting one of the FCFs and tries to
send FIP FLOGI. However the switch may reject the FLOGI, as it has
cleared its internal state, and expects the initiator to start FIP
discovery protocol. Fix this condition by resetting the fcoe
controller.

All in-kernel users of class_find_device() don't really need mutable
data for match callback.
In two places (kernel/power/suspend_test.c, drivers/scsi/osd/osd_uld.c)
this patch changes match callbacks to use const search data.
The const is propagated to rtc_class_open() and power_supply_get_by_name()
parameters.
Note that there's a dev reference leak in suspend_test.c that's not
touched in this patch.

Originally 'efi_enabled' indicated whether a kernel was booted from
EFI firmware. Over time its semantics have changed, and it now
indicates whether or not we are booted on an EFI machine with
bit-native firmware, e.g. 64-bit kernel with 64-bit firmware.
The immediate motivation for this patch is the bug report at,
https://bugs.launchpad.net/ubuntu-cdimage/+bug/1040557
which details how running a platform driver on an EFI machine that is
designed to run under BIOS can cause the machine to become
bricked. Also, the following report,
https://bugzilla.kernel.org/show_bug.cgi?id=47121
details how running said driver can also cause Machine Check
Exceptions. Drivers need a new means of detecting whether they're
running on an EFI machine, as sadly the expression,
if (!efi_enabled)
hasn't been a sufficient condition for quite some time.
Users actually want to query 'efi_enabled' for different reasons -
what they really want access to is the list of available EFI
facilities.
For instance, the x86 reboot code needs to know whether it can invoke
the ResetSystem() function provided by the EFI runtime services, while
the ACPI OSL code wants to know whether the EFI config tables were
mapped successfully. There are also checks in some of the platform
driver code to simply see if they're running on an EFI machine (which
would make it a bad idea to do BIOS-y things).
This patch is a prereq for the samsung-laptop fix patch.

Delete successive tests to the same location. The code tested the result
of a previous allocation, that itself was already tested. It is changed to
test the result of the most recent allocation.

Boot from SAN fails in some cases where boot target login response
is slow. In this case, probe finishes early and triggers init, which
doesn't find remote disk to mount root which leads to BFS failure.

Problem:
If initialization of adapter fails for some reason, then the interrupt handlers
are released. The interrupt handlers are not registered again when initialization
of adapter is retried.
Solution:
Re-register the interrupt handler when adapter initialization is retried.

Problem:
QLA4xxx firmware may assert if given more IOCBs than it can handle.
Solution:
The driver was updated to throttle the number of active IOCBs based
on the total number of IOCB buffers received from get_firmware_status
mbx_sts[2].

The spurious interrupt messages are seen when interrupt lines are
shared between different adapters or modules. Example, interrupt
lines are shared between qlcnic and qla4xxx and USB modules, then the
console is flooded with flurry of spurious interrupt messages
which are expected in such scenario.
Hence put these messages under debug level 2 for INTx interrupt mode.

When doing a hotplug remove of a cxgb4 device, there are still dandling
symlinks at /sys/class/scsi_host/hostX to the removed PCI device. The
upper layer device may also try to send data, which may crash the
system.
The DETACH message from the lower level driver is sent to the ULD when
the device is removed, when the scsi host should be removed from the
system, avoiding any problems.
After this patch, there are no more dangling symlinks and many attempts
to crash the system while there is SCSI activity and removing the device
have failed. Adding the device back again works as expected, with the
scsi hosts showing up again.

We used to map doorbell register after FW enable request is complete.
This causes a race condition when unsolicited event is received, and FW
sends a CQE for it. Since the doorbell is not mapped, driver does not
arm CQ, which means FW will not notify the driver for further CQ
completions. To resolve this, map the doorbell between offload and
enable, so that driver is ready to receive the unsolicited packets and
arm the CQ as soon as FW enable is performed.

Software FCoE target always advertises RETRY bit even when there are no
tape LUNs behind the target. This causes the driver to enable FW support
for sequence level error recovery and perform REC/SRR. This patch

ioc->diag_trigger_mpi is an SL_WH_MPI_TRIGGERS_T struct.
There is a cut and paste error here and SL_WH_EVENT_TRIGGERS_T is used
instead of SL_WH_MPI_TRIGGERS_T. Since the SL_WH_EVENT_TRIGGERS_T is
smaller than SL_WH_MPI_TRIGGERS_T, it means we only clear part of the
buffer.
I've changed it to use sizeof(ioc->diag_trigger_mpi) which is a bit
simpler.

In qla4xxx_get_ep_fwdb(), dst_addr is of type struct sockaddr.
We are copying sizeof(struct sockaddr_in6) bytes to dst_addr
which is 12 bytes larger. This will cause memory corruption.
So we change dst_addr to struct sockaddr_storage which is
of 128 byte, large enough to hold sizeof(struct sockaddr_in6).

Issue:
Mailbox command timed out after switching from polling mode to interrupt mode.
Events:-
1. Mailbox interrupts are disabled
2. FW generates AEN and at same time driver enables Mailbox Interrupt
3. Driver issues new mailbox to Firmware
In above case driver will not get AEN interrupts generated by FW in step #2 as
FW generated this AEN when interrupts are disabled. During the same time
driver enabled the mailbox interrupt, so driver will not poll for interrupt.
Driver will never process AENs generated in step #2 and issues new mailbox to
FW, but now FW is not able to post mailbox completion as AENs generated before
are not processed by driver.
Fix:
Enable Mailbox / AEN interrupts before initializing FW in case of ISP83XX.
This will make sure we process all Mailbox and AENs in interrupt mode.

The ROM address handling in gdth_init_pci() is useless and possibly
dangerous. This patch removes it.
"pci_resource_start(pdev, 8)" is not well-defined. PCI resources 0-5 are
standard PCI BARs and 6 is the expansion ROM. Resource 8 is either an
SR-IOV BAR (if CONFIG_PCI_IOV=y, resources 7-12 are SR-IOV BARs) or a
bridge window (resources 7-10).
The GDT device is neither an SR-IOV device nor a bridge, so in either case
resource 8 should be zero since struct pci_dev is allocated with kzalloc().
It is illegal for a driver to write an arbitrary address to the ROM BAR
because it has no way of knowing whether the ROM will conflict with another
device.
I think the only effect of the code being removed was to:
1) Enable the ROM at 0xFEFF0000 (possibly causing a conflict with
another device)
2) Delay one millisecond
3) Write zero to the ROM BAR, disabling it
I doubt the delay is needed, but I left it since it seems innocuous.

Based on feedback and discussion with Neil Horman it seems that the above patch
may have a case where the fcoe_vport_destroy() and fcoe_destroy_work() can
race; hence that patch has been withdrawn with this patch that is trying to
solve the same problem in a different way.
In the current approach instead of removing the fcoe_config_mutex from the
vport_delete callback function; I've chosen to delete all the NPIV ports first
on a given root lport before continuing with the removal of the root lport.

When creating an fcoe interfce, we call fcoe_link_speed_update before we add the
lports fcoe interface to the fc_hostlist. Since network device events like
NETDEV_CHANGE are only processed if an fcoe interface is found with an
underlying netdev that matches the netdev of the event. Since this processing
in fcoe_device_notification is how link_speed changes get communicated to the
libfc code (via fcoe_link_speed_update), we have a race condition - if a
NETDEV_CHANGE event is sent after the call to fcoe_link_speed_update in
fcoe_netdev_config, but before we add the interface to the fc_hostlist, we will
loose the event and attributes like /sys/class/fc_host/hostX/speed will not get
updated properly.
Fix this by moving the add to the fc_hostlist above the serialized call to
fcoe_netdev_config, ensuring that we catch netdev envents before we make a
direct call to fcoe_link_speed_update.
Also use this opportunity to clean up access to the fc_hostlist a bit by
creating a fcoe_hostlist_del accessor and replacing the cleanup in fcoe_exit to
use it properly.

When the ODD is powered off, any action the user did to the ODD that
would generate a media event will trigger an ACPI interrupt, so the
poll for media event is no longer necessary. And the poll will also
cause a runtime status change, which will stop the ODD from staying in
powered off state, so the poll should better be stopped.
But since we don't have access to the gendisk structure in LLDs, here
comes the disk_events_disable_depth for scsi device. This field is a
hint set by LLDs to convey information to upper layer drivers. A value
of 0 means media poll is necessary for the device, while values above 0
means media poll is not needed and should better be skipped. So we can
increase its value when we are to power off the ODD in ATA layer and
decrease its value when the ODD is powered on, effectively silence the
media events poll.

This patch adds runtime pm support for sr.
It did this by increasing the runtime usage_count of the device when
its block device is accessed. And decreasing the runtime usage_count
of the device when the access is done.
If there is media inside, runtime suspend is not allowed as we don't
always know if the ODD is being used or not.

CONFIG_HOTPLUG is going away as an option. As a result, the __dev*
markings need to be removed.
This change removes the use of __devinit, __devexit_p, __devinitdata,

It enables establishing active open connection using fw_ofld_connection work
request when cpl_act_open_rpl says TCAM full error which may be because
of LE hash collision. Current support is only for IPv4 active open connections.
Sets ntuple bits in active open requests. For T4 firmware greater than 1.4.10.0

The purpose of this renaming is to prevent some kernel developers from
assuming that prandom32() and random32() might imply that only
prandom32() was the one using a pseudo-random number generator by
prandom32's "p", and the result may be a very embarassing security
exposure. This concern was expressed by Theodore Ts'o.
And furthermore, I'm going to introduce new functions for getting the
requested number of pseudo-random bytes. If I continue to use both
prandom32 and random32 prefixes for these functions, the confusion
is getting worse.
As a result of this renaming, "prandom_" is the common prefix for
pseudo-random number library.
Currently, srandom32() and random32() are preserved because it is
difficult to rename too many users at once.

All told, hch was able to shave off another ~1K LOC by killing the
se_subsystem_dev abstraction, along with a number of PR + ALUA

emulation support for virtual backends.
Sebastian found a long-standing issue wrt to allocation of full
config_group instead of pointers for config_group->default_group[]
setup in a number of areas, which ends up saving memory with big
configurations. He also managed to fix another long-standing BUG wrt
to broken 32-bit highmem support within the FILEIO backend driver.

This patch does a few things.
1) Makes /sys/bus/fcoe/ctlr_{create,destroy} interfaces.
These interfaces take an <ifname> and will either
create an FCoE Controller or destroy an FCoE
Controller depending on which file is written to.
The new FCoE Controller will start in a DISABLED
state and will not do discovery or login until it
is ENABLED. This pause will allow us to configure
the FCoE Controller before enabling it.
2) Makes the 'mode' attribute of a fcoe_ctlr_device
writale. This allows the user to configure the mode
in which the FCoE Controller will start in when it
is ENABLED.
Possible modes are 'Fabric', or 'VN2VN'.
The default mode for a fcoe_ctlr{,_device} is 'Fabric'.
Drivers must implement the set_fcoe_ctlr_mode routine
to support this feature.
libfcoe offers an exported routine to set a FCoE
Controller's mode. The mode can only be changed
when the FCoE Controller is DISABLED.
This patch also removes the get_fcoe_ctlr_mode pointer
in the fcoe_sysfs function template, the code in
fcoe_ctlr.c to get the mode and the assignment of
the fcoe_sysfs function pointer to the fcoe_ctlr.c
implementation (in fcoe and bnx2fc). fcoe_sysfs can
return that value for the mode without consulting the
LLD.
3) Make a 'enabled' attribute of a fcoe_ctlr_device. On a
read, fcoe_sysfs will return the attribute's value. On
a write, fcoe_sysfs will call the LLD (if there is a
callback) to notifiy that the enalbed state has changed.
This patch maintains the old FCoE control interfaces as
module parameters, but it adds comments pointing out that
the old interfaces are deprecated.

QUEUE_FLAG_DEAD is used to indicate that queuing new requests must
stop. After this flag has been set queue draining starts. However,
during the queue draining phase it is still safe to invoke the
queue's request_fn, so QUEUE_FLAG_DYING is a better name for this

Instead of creating a structure with an enum and a pointer
to a string, simply allocate an array of strings and use
the enum values for the indicies.
This means that we do not need to iterate through the list
of entries when looking up a string name by its enum key.
This will also help with a latter patch that will add
more fcoe_sysfs attributes that will also use the
fcoe_enum_name_search macro. One attribute will also do
a reverse lookup which requires less code when the
enum-to-string mappings are organized as this patch makes
them to be.

Currently fc_fcp_timeout doesn't check FC_RP_FLAGS_REC_SUPPORTED
flag first, this prevents REC request ever going out at all
to the target having REC support. So this patches fixes the
fc_fcp_timeout by checking FC_RP_FLAGS_REC_SUPPORTED flag first.
The changed order won't cause any issue during clearing
FC_RP_FLAGS_REC_SUPPORTED on failed IO with target not supporting
FC_RP_FLAGS_REC_SUPPORTED, since retry on failed IO would succeed.

megaraid use INIT_WORK to declare a hotplug_work, but cast the
hotplug_work from work_struct to delayed_work and
schedule_delayed_work on it. This is very dangerous, as other part of
delayed_work might be kernel memories allocated by others.
With commit 8852aac ("workqueue: mod_delayed_work_on() shouldn't queue
timer on 0 delay"), schedule_delayed_work() will check dwork->timer
before queue_work even when @delay is 0, this causes megaraid code to
hit the BUG_ON() in workqueue code. Change megaraid code to use
delayed work.

These driver files are initially, substantially similar to mpt2sas but,
because mpt2sas is going into maintenance mode and mp3sas will become heavily
developed, we elected to keep the code bases separate.

Make it possible to disconnect the IB RC connection used by the SRP
protocol to communicate with a target.
Have the SRP transport layer create a sysfs "delete" attribute for
initiator drivers that support this functionality.

Eliminate the private_rport_attrs[] array and the SETUP_*() macros
used to set up that array since the information in that array
duplicates the information in the static device attributes. Also,
verify whether SRP_RPORT_ATTRS is large enough since it is easy to
forget to update that macro when adding new attributes.

Register transport attributes after the attribute array has been set
up instead of before. The current code can trigger a race condition
because the code reading the attribute array can run on another thread
than the code that initialized that array. Make sure that any code
reading the attribute array will see all values written into that
array.

Original code was not corrected for ISP83xx support. Instead code should used
the cached reference from the req_que structure, rather than the legacy
'if (ISP-TYPE(X))... else if (ISP-TYPE(Y))...' codes.

When the qla2xxx driver loses access to multiple, remote ports, there is a race
condition which can occur which will keep the request stuck on a scsi request
queue indefinitely.
This bad state occurred do to a race condition with how the FCPORT_UPDATE_NEEDED
bit is set in qla2x00_schedule_rport_del(), and how it is cleared in
qla2x00_do_dpc(). The problem port has its drport pointer set, but it has never
been processed by the driver to inform the fc transport that the port has been
lost. qla2x00_schedule_rport_del() sets drport, and then sets the
FCPORT_UPDATE_NEEDED bit. In qla2x00_do_dpc(), the port lists are walked and
any drport pointer is handled and the fc transport informed of the port loss,
then the FCPORT_UPDATE_NEEDED bit is cleared. This leaves a race where the
dpc thread is processing one port removal, another port removal is marked
with a call to qla2x00_schedule_rport_del(), and the dpc thread clears the
bit for both removals, even though only the first removal was actually
handled. Until another event occurs to set FCPORT_UPDATE_NEEDED, the later
port removal is never finished and qla2xxx stays in a bad state which causes
requests to become stuck on request queues.
This patch updates the driver to test and clear FCPORT_UPDATE_NEEDED
atomically. This ensures the port state changes are processed and not lost.

FC transport on receiving bsg_job submission failure, calls bsg_job->job_done()
and sets the bsg_job->reply->result the returned value. In contrast, when the
success code (0) is returned fc transport doesn't call bsg_job->job_done() and
doesn't populate bsg_job->reply->result.

Update sd driver to use the callbacks defined in dev_pm_ops.
sd_freeze is NULL, the bus level callback has taken care of quiescing
the device so there should be nothing needs to be done here.
Consequently, sd_thaw is not needed here either.
suspend, poweroff and runtime suspend share the same routine sd_suspend,
which will sync flush and then stop the drive, this is the same as before.
resume, restore and runtime resume share the same routine sd_resume,
which will start the drive by putting it into active power state, this
is also the same as before.

Use of pm_message_t is deprecated and device driver is not supposed
to use that. This patch migrates the SCSI bus level pm callbacks
to call device's pm callbacks defined in its driver's dev_pm_ops.
This is achieved by finding out which device pm callback should be used
in bus callback function, and then pass that callback function pointer
as a param to the scsi_bus_{suspend,resume}_common routine, which will
further pass that callback to scsi_dev_type_{suspend,resume} after
proper handling.
The special case for freeze in scsi_bus_suspend_common is not necessary
since there is no high level SCSI driver has implemented freeze, so no
need to runtime resume the device if it is in runtime suspended state
for system freeze, just return like the system suspend/hibernate case.
Since only sd has implemented drv->suspend/drv->resume, and I'll update
sd driver to use the new callbacks in the following patch, there is no
need to fallback to call drv->suspend/drv->resume if dev_pm_ops is NULL.

When device is runtime suspended, put it to stopped power state to save
some power.
This will also make the behaviour consistent with what the scsi_pm.c
thinks about sd as the comment says:
sd treats runtime suspend, system suspend and system hibernate identical.
With this patch, it is now identical.
And sd_shutdown will also do nothing when it finds the device has been
runtime suspended, if we do not spin down the disk in runtime suspend
by putting it into stopped power state, the disk will be shut down
incorrectly.
And the the same problem can be solved for runtime power off after
runtime suspended case by this change.
With the current runtime scheme for disk, it will only be runtime
suspended when no process opens the disk, so this shouldn't happen a
lot, which makes it acceptable to spin down the disk when runtime
suspended. If some day a more aggressive runtime scheme is used, like
the 'request based runtime pm for disk' that Alan Stern and Lin Ming
has been working, we can introduce some policy to control this. But for
now, make it simple and correct by spinning down the disk.

The macro bit(n) is defined as ((u32)1 << n), and thus it doesn't work
with n >= 32, such as in mvs_94xx_assign_reg_set():
if (i >= 32) {
mvi->sata_reg_set |= bit(i);
...
}
The shift ((u32)1 << n) with n >= 32 also leads to undefined behavior.
The result varies depending on the architecture.
This patch changes bit(n) to do a 64-bit shift. It also simplifies
mv_ffc64() using __ffs64(), since invoking ffz() with ~0 is undefined.

store_host_reset() has tried to re-invent the wheel to compare sysfs strings.
Unfortunately it did so poorly and never bothered to check the input from
userspace before overwriting stack with it, so something simple as:

Force large capacity (> 0xFFFFFFFF blocks) drives to use READ/WRITE(16) instead
of READ/WRITE(10). Some(most/all?) USB enclosures do not like READ(10) commands
when a large capacity drive is installed. This issue was reported and discussed

virtscsi_queuecommand was leaking memory when the virtio queue was full.
Tested: Guest operates correctly even with very small queue sizes, validated
we're not leaking kmalloc-192 sized allocations anymore.
