f07b352021 GFS2: Made logd daemon take into account log demand
Before this patch, the logd daemon only tried to flush things when
the log blocks pinned exceeded a certain threshold. But when we're
deleting very large files, it may require a huge number of journal
blocks, and that, in turn, may exceed the threshold. This patch
factors that into account.

2fcf5cc3be GFS2: Limit number of transaction blocks requested for truncates
This patch limits the number of transaction blocks requested during
file truncates. If we have very large multi-terabyte files, and want
to delete or truncate them, they might span so many resource groups
that we overflow the journal blocks, and cause an assert failure.
By limiting the number of blocks in the transaction, we prevent this
overflow and give other running processes time to do transactions.

The limiting factor I chose is sd_log_thresh2 which is currently
set to 4/5ths of the journal. This same ratio is used in function
gfs2_ail_flush_reqd to determine when a log flush is required.
If we make the maximum value less than this, we can get into a
infinite hang whereby the log stops moving because the number of
used blocks is less than the threshold and the iterative loop
needs more, but since we're under the threshold, the log daemon
never starts any IO on the log.

8b0e195314 ktime: Cleanup ktime_set() usage
ktime_set(S,N) was required for the timespec storage type and is still
useful for situations where a Seconds and Nanoseconds part of a time value
needs to be converted. For anything where the Seconds argument is 0, this
is pointless and can be replaced with a simple assignment.

43388b21e7 fix gfs2_stuffed_write_end() on short copies
a) the page is uptodate - ->write_begin() would either fail (in which
case we don't reach ->write_end()), or unstuff the inode, or find the
page already uptodate, or do a successful call of stuffed_readpage(),
which would've made it uptodate

b) zeroing the tail in pagecache is wrong.  kill -9 at the right time
while writing unmodified file contents to the same file should _not_
leave us in a situation when read() from the file will be reporting
it full of zeroes.  Especially since that effect will be transient -
at some later point the page will be evicted and then we'll be back
to the real file contents.

ef295ecf09 block: better op and flags encoding
Now that we don't need the common flags to overflow outside the range
of a 32-bit type we can encode them the same way for both the bio and
request fields.  This in addition allows us to place the operation
first (and make some room for more ops while we're at it) and to
stop having to shift around the operation values.

In addition this allows passing around only one value in the block layer
instead of two (and eventuall also in the file systems, but we can do
that later) and thus clean up a lot of code.

Last but not least this allows decreasing the size of the cmd_flags
field in struct request to 32-bits.  Various functions passing this
value could also be updated, but I'd like to avoid the churn for now.

078cd8279e fs: Replace CURRENT_TIME with current_time() for inode timestamps
CURRENT_TIME macro is not appropriate for filesystems as it
doesn't use the right granularity for filesystem timestamps.
Use current_time() instead.

CURRENT_TIME is also not y2038 safe.

This is also in preparation for the patch that transitions
vfs timestamps to use 64 bit time and hence make them
y2038 safe. As part of the effort current_time() will be
extended to do range checks. Hence, it is necessary for all
file system timestamps to use current_time(). Also,
current_time() will be transitioned along with vfs to be
y2038 safe.

Note that whenever a single call to current_time() is used
to change timestamps in different inodes, it is because they
share the same time granularity.

Fix for commit 719ee344: initialize atime of I_NEW inodes to 0 so that

inode_change_ok() will be resposible for clearing capabilities and IMA
extended attributes and as such will need dentry. Give it as an argument

modifications in addition to checks.

073931017b posix_acl: Clear SGID bit when setting file permissions
When file permissions are modified via chmod(2) and the user is not in
the owning group or capable of CAP_FSETID, the setgid bit is cleared in
inode_change_ok().  Setting a POSIX ACL via setxattr(2) sets the file
permissions as well as the new ACL, but doesn't clear the setgid bit in
a similar way; this allows to bypass the check in chmod(2).  Fix that.

23e5671a79 gfs2: Fix extended attribute readahead optimization
Commit 39b0555f didn't check for a failing bio_add_page in
gfs2_submit_bhs. This could cause I/O requests to get lost, and the
affected buffer heads to stay locked forever.  Fix that by submitting
the current bio and allocating another one when bio_add_page fails.  (It
is guaranteed that we can at least add one page to a bio.)

1c185c02f4 gfs2: Remove dirty buffer warning from gfs2_releasepage
Unlike what its documentation suggests, the releasepage address space
operation can currently be called on dirty pages via shrink_active_list.
This may eventually be changed when the remaining code relying on the
current behavior has been fixed, but until then, it makes no sense to
warn on dirty buffers in gfs2_releasepage.

In most cases, EPERM is returned on immutable inode, and there're only a
few places returning EACCES. I noticed this when running LTP on
overlayfs, setxattr03 failed due to unexpected EACCES on immutable
inode.

e1cb6be9e1 GFS2: Fix gfs2_replay_incr_blk for multiple journal sizes
Before this patch, if you used gfs2_jadd to add new journals of a
size smaller than the existing journals, replaying those new journals
would withdraw. That's because function gfs2_replay_incr_blk was
using the number of journal blocks (jd_block) from the superblock's
journal pointer. In other words, "My journal's max size" rather than
"the journal we're replaying's size." This patch changes the function
to use the size of the pertinent journal rather than always using the
journal we happen to be using.

70246286e9 block: get rid of bio_rw and READA
These two are confusing leftover of the old world order, combining
values of the REQ_OP_ and REQ_ namespaces.  For callers that don't
special case we mostly just replace bi_rw with bio_data_dir or
op_is_write, except for the few cases where a switch over the REQ_OP_
values makes more sense.  Any check for READA is replaced with an
explicit check for REQ_RAHEAD.  Also remove the READA alias for
REQ_RAHEAD.

For the last process to close a file opened for write, function
gfs2_rsqa_delete was deleting the file's inode's block reservation
out of the rgrp reservations tree. Then it was checking to make sure
rs_free was 0, but it was performing the check outside the protection
of rd_rsspin spin_lock. The rd_rsspin spin_lock protection is needed
to prevent a race between the process freeing the reservation and
another who is allocating a new set of blocks inside the same rgrp
for the same inode, thus changing its value.

fd4c5748b8 gfs2: writeout truncated pages
When gfs2 attempts to write a page to a file that is being truncated,
and notices that the page is completely outside of the file size, it
tries to invalidate it.  However, this may require a transaction for
journaled data files to revoke any buffers from the page on the active
items list. Unfortunately, this can happen inside a log flush, where a
transaction cannot be started. Also, gfs2 may need to be able to remove
the buffer from the ail1 list before it can finish the log flush.

To deal with this, when writing a page of a file with data journalling
enabled gfs2 now skips the check to see if the write is outside the file
size, and simply writes it anyway. This situation can only occur when
the truncate code still has the file locked exclusively, and hasn't
marked this block as free in the metadata (which happens later in
truc_dealloc).  After gfs2 writes this page out, the truncation code
will shortly invalidate it and write out any revokes if necessary.

To do this, gfs2 now implements its own version of block_write_full_page
without the check, and calls the newly exported __block_write_full_page.
It also no longer calls gfs2_writepage_common from gfs2_jdata_writepage.

Make the code more readable by cleaning up the different ways of
initializing lock holders and checking for initialized lock holders:
mark lock holders as uninitialized by setting the holder's glock to NULL
(gfs2_holder_mark_uninitialized) instead of zeroing out the entire
object or using a separate flag.  Recognize initialized holders by their
non-NULL glock (gfs2_holder_initialized).  Don't zero out holder objects
which are immeditiately initialized via gfs2_holder_init or
gfs2_glock_nq_init.

Commit ff34245d switched from iget5_locked to iget_locked among other
things, but iget_locked doesn't work for filesystems larger than 2^32
blocks on 32-bit systems.  Switch back to iget5_locked.  Filesystems
larger than 2^32 blocks are unrealistic to work well on 32-bit systems,
so this is mostly a code cleanliness fix.

i_diskflags in the gfs2_ilookup case (see gfs2_lookup_by_inum); this
inconsistency goes away as well.

3ce37b2cb4 gfs2: Fix gfs2_lookup_by_inum lock inversion
The current gfs2_lookup_by_inum takes the glock of a presumed inode
identified by block number, verifies that the block is indeed an inode,
and then instantiates and reads the new inode via gfs2_inode_lookup.

However, instantiating a new inode may block on freeing a previous
instance of that inode (__wait_on_freeing_inode), and freeing an inode
requires to take the glock already held, leading to lock inversion and
deadlock.

Fix this by first instantiating the new inode, then verifying that the
block is an inode (if required), and then reading in the new inode, all
in gfs2_inode_lookup.

If the block we are looking for is not an inode, we discard the new
inode via iget_failed, which marks inodes as bad and unhashes them.
Other tasks waiting on that inode will get back a bad inode back from
ilookup or iget_locked; in that case, retry the lookup.

36e4ad0316 GFS2: don't set rgrp gl_object until it's inserted into rgrp tree
Before this patch, function read_rindex_entry would set a rgrp
glock's gl_object pointer to itself before inserting the rgrp into
the rgrp rbtree. The problem is: if another process was also reading
the rgrp in, and had already inserted its newly created rgrp, then
the second call to read_rindex_entry would overwrite that value,
then return a bad return code to the caller. Later, other functions
would reference the now-freed rgrp memory by way of gl_object.
In some cases, that could result in gfs2_rgrp_brelse being called
twice for the same rgrp: once for the failed attempt and once for
the "real" rgrp release. Eventually the kernel would panic.
There are also a number of other things that could go wrong when
a kernel module is accessing freed storage. For example, this could
result in rgrp corruption because the fake rgrp would point to a
fake bitmap in memory too, causing gfs2_inplace_reserve to search
some random memory for free blocks, and find some, since we were
never setting rgd->rd_bits to NULL before freeing it.

This patch fixes the problem by not setting gl_object until we
have successfully inserted the rgrp into the rbtree. Also, it sets
rd_bits to NULL as it frees them, which will ensure any accidental
access to the wrong rgrp will result in a kernel panic rather than
file system corruption, which is preferred.

287980e49f remove lots of IS_ERR_VALUE abuses
Most users of IS_ERR_VALUE() in the kernel are wrong, as they
pass an 'int' into a function that takes an 'unsigned long'
argument. This happens to work because the type is sign-extended
on 64-bit architectures before it gets converted into an
unsigned type.

However, anything that passes an 'unsigned short' or 'unsigned int'
argument into IS_ERR_VALUE() is guaranteed to be broken, as are
8-bit integers and types that are wider than 'unsigned long'.

Andrzej Hajda has already fixed a lot of the worst abusers that
were causing actual bugs, but it would be nice to prevent any
users that are not passing 'unsigned long' arguments.

This patch changes all users of IS_ERR_VALUE() that I could find
on 32-bit ARM randconfig builds and x86 allmodconfig. For the
moment, this doesn't change the definition of IS_ERR_VALUE()
because there are probably still architecture specific users
elsewhere.

Almost all the warnings I got are for files that are better off
using 'if (err)' or 'if (err < 0)'.
The only legitimate user I could find that we get a warning for
is the (32-bit only) freescale fman driver, so I did not remove
the IS_ERR_VALUE() there but changed the type to 'unsigned long'.
For 9pfs, I just worked around one user whose calling conventions
are so obscure that I did not dare change the behavior.

I was using this definition for testing:

which ends up making all 16-bit or wider types work correctly with

to return according to its users, but also causes a compile-time
warning for any users that do not pass an 'unsigned long' argument.

I suggested this approach earlier this year, but back then we ended
up deciding to just fix the users that are obviously broken. After
the initial warning that caused me to get involved in the discussion
(fs/gfs2/dir.c) showed up again in the mainline kernel, Linus
asked me to send the whole thing again.

First, it removes the parameter that specifies the transaction.
Since it's always passed in as current->journal_info, we might as well

In certain cases, the 802.11 mesh pathtable code wants to
iterate over all of the entries in the forwarding table from
the receive path, which is inside an RCU read-side critical
section.  Enable walks inside atomic sections by allowing
GFP_ATOMIC allocations for the walker state.

Change all existing callsites to pass in GFP_KERNEL.

09cbfeaf1a mm, fs: get rid of PAGE_CACHE_* and page_cache_{get,release} macros
PAGE_CACHE_{SIZE,SHIFT,MASK,ALIGN} macros were introduced *long* time
ago with promise that one day it will be possible to implement page
cache with bigger chunks than PAGE_SIZE.

This promise never materialized.  And unlikely will.

We have many places where PAGE_CACHE_SIZE assumed to be equal to
PAGE_SIZE.  And it's constant source of confusion on whether
PAGE_CACHE_* or PAGE_* constant should be used in a particular case,
especially on the border between fs and mm.

Global switching to PAGE_CACHE_SIZE != PAGE_SIZE would cause to much
breakage to be doable.

Let's stop pretending that pages in page cache are special.  They are
not.

The changes are pretty straight-forward:

 - <foo> << (PAGE_CACHE_SHIFT - PAGE_SHIFT) -> <foo>;

 - <foo> >> (PAGE_CACHE_SHIFT - PAGE_SHIFT) -> <foo>;

script below.  For some reason, coccinelle doesn't patch header files.

The only adjustment after coccinelle is revert of changes to
PAGE_CAHCE_ALIGN definition: we are going to drop it later.

There are few places in the code where coccinelle didn't reach.  I'll
fix them manually in a separate patch.  Comments and documentation also

After gfs2 has withdrawn the filesystem, it may still have many locks not
in the unlocked state.  If it is using lock_dlm, it will failed trying
the unlocks since it has already unmounted the lock manager. Instead, it
should set the SDF_SKIP_DLM_UNLOCK flag on withdraw, to signal that
it can skip the lock_manager on unlocks, and failback to lock_nolock
style unlocking.

ff34245d52 GFS2: Don't filter out I_FREEING inodes anymore
This patch basically reverts a very old patch from 2008,
7a9f53b3c1, with the title
"Alternate gfs2_iget to avoid looking up inodes being freed".
The original patch was designed to avoid a deadlock caused by lock
ordering with try_rgrp_unlink. The patch forced the function to not
find inodes that were being removed by VFS. The problem is, that
made it impossible for nodes to delete their own unlinked dinodes
after a certain point in time, because the inode needed was not found
by this filtering process. There is no longer a need for the patch,
since function try_rgrp_unlink no longer locks the inode: All it does
is queue the glock onto the delete work_queue, so there should be no
more deadlock.

2df6f47150 GFS2: Fix direct IO write rounding error
The fsx test in xfstests was failing because it was using direct IO
writes which were using a bad calculation. It was using
loff_t lstart = offset & (PAGE_CACHE_SIZE - 1); when it should be
loff_t lstart = offset & ~(PAGE_CACHE_SIZE - 1);
Thus, the write at offset 0x67e00 was calculating lstart to be
0xe00, the address of our corruption. Instead, it should have been
0x67000. This patch fixes the calculation.

5d097056c9 kmemcg: account certain kmem allocations to memcg
Mark those kmem allocations that are known to be easily triggered from
userspace as __GFP_ACCOUNT/SLAB_ACCOUNT, which makes them accounted to
memcg.  For the list, see below:

 - threadinfo
 - task_struct
 - task_delay_info
 - pid
 - cred
 - mm_struct
 - vm_area_struct and vm_region (nommu)
 - anon_vma and anon_vma_chain
 - signal_struct
 - sighand_struct
 - fs_struct
 - files_struct
 - fdtable and fdtable->full_fds_bits
 - dentry and external_name
 - inode for all filesystems. This is the most tedious part, because
   most filesystems overwrite the alloc_inode method.

The list is far from complete, so feel free to add more objects.
Nevertheless, it should be close to "account everything" approach and
keep most workloads within bounds.  Malevolent users will be able to
breach the limit, but this was possible even with the former "account
everything" approach (simply because it did not account everything in
fact).

7508abc4bd GFS2: Check if iopen is held when deleting inode
This patch fixes an error condition in which an inode is partially
created in gfs2_create_inode() but then some error is discovered,
which causes it to fail and call iput() before the iopen glock is
created or held. In that case, gfs2_delete_inode would try to
unlock an iopen glock that doesn't yet exist. Therefore, we test
its holder (which must exist) for the HIF_HOLDER bit before trying
to dq it.

  One piece probably deserves special mention - Neil's
  lookup_one_len_unlocked().  Similar to lookup_one_len(), but gets

  course, means that it's not useful for any directory modifications,
  but things like getting inode attributes in nfds readdirplus are fine
  with that.  I really should've asked for moratorium on lookup-related
  changes this cycle, but since I hadn't done that early enough...  I
  *am* asking for that for the coming cycle, though - I'm going to try
  and get conversion of i_mutex to rwsem with ->lookup() done under lock
  taken shared.

f39814f60a gfs2: Invalid security labels of inodes when they go invalid
When gfs2 releases the glock of an inode, it must invalidate all
information cached for that inode, including the page cache and acls.
Use the new security_inode_invalidate_secctx hook to also invalidate
security labels in that case.  These items will be reread from disk
when needed after reacquiring the glock.

6cc4b6e801 GFS2: Don't do glock put on when inode creation fails
Currently the error path of function gfs2_inode_lookup calls function
gfs2_glock_put corresponding to an earlier call to gfs2_glock_get for
the inode glock. That's wrong because the error path also calls
iget_failed() which eventually calls iput, which eventually calls
gfs2_evict_inode, which does another gfs2_glock_put. This double-put
can cause the glock reference count to get off.

5ea31bc0a6 GFS2: Always use iopen glock for gl_deletes
Before this patch, when function try_rgrp_unlink queued a glock for
delete_work to reclaim the space, it used the inode glock to do so.
That's different from the iopen callback which uses the iopen glock
for the same purpose. We should be consistent and always use the
iopen glock. This may also save us reference counting problems with
the inode glock, since clear_glock does an extra glock_put() for the
inode glock.

Some error cases in gfs2_create_inode were not unlocking the iopen
glock, getting the reference count off. This adds the proper unlock.
The error logic in function gfs2_create_inode was also convoluted,
so this patch simplifies it. It also takes care of a bug in
which gfs2_qa_delete() was not called in an error case.

ee530beafe GFS2: Truncate address space mapping when deleting an inode
In function gfs2_delete_inode() we write and flush the mapping for
a glock, among other things. We truncate the mapping for the inode,
but we never truncate the mapping for the glock. This patch makes it
also truncate the metamapping. This avoid cases where the glock is
reused by another process who is trying to recreate an inode in its
place using the same block.

86d067a797 GFS2: Wait for iopen glock dequeues
This patch changes every glock_dq for iopen glocks into a dq_wait.
This makes sure that iopen glocks do not outlive the inode itself.
In turn, that ensures that anyone trying to unlink the glock will
be able to find the inode when it receives a remote iopen callback.
