6c777e8799 Revert "PCI, x86: Implement pcibios_alloc_irq() and pcibios_free_irq()"
991de2e590 ("PCI, x86: Implement pcibios_alloc_irq() and
pcibios_free_irq()") appeared in v4.3 and helps support IOAPIC hotplug.

Олег reported that the Elcus-1553 TA1-PCI driver worked in v4.2 but not
v4.3 and bisected it to 991de2e590.  Sunjin reported that the RocketRAID
272x driver worked in v4.2 but not v4.3.  In both cases booting with
"pci=routirq" is a workaround.

I think the problem is that after 991de2e590, we no longer call
pcibios_enable_irq() for upstream bridges.  Prior to 991de2e590, when a
driver called pci_enable_device(), we recursively called
pcibios_enable_irq() for upstream bridges via pci_enable_bridge().

After 991de2e590, we call pcibios_enable_irq() from pci_device_probe()
instead of the pci_enable_device() path, which does *not* call
pcibios_enable_irq() for upstream bridges.

Revert 991de2e590 to fix these driver regressions.

185a383ada x86/PCI: Add driver for Intel Volume Management Device (VMD)
The Intel Volume Management Device (VMD) is a Root Complex Integrated
Endpoint that acts as a host bridge to a secondary PCIe domain.  BIOS can
reassign one or more Root Ports to appear within a VMD domain instead of
the primary domain.  The immediate benefit is that additional PCIe domains
allow more than 256 buses in a system by letting bus numbers be reused
across different domains.

VMD domains do not define ACPI _SEG, so to avoid domain clashing with host
bridges defining this segment, VMD domains start at 0x10000, which is
greater than the highest possible 16-bit ACPI defined _SEG.

This driver enumerates and enables the domain using the root bus
configuration interface provided by the PCI subsystem.  The driver provides
configuration space accessor functions (pci_ops), bus and memory resources,
an MSI IRQ domain with irq_chip implementation, and DMA operations
necessary to use devices through the VMD endpoint's interface.

VMD routes I/O as follows:

   1) Configuration Space: BAR 0 ("CFGBAR") of VMD provides the base
   address and size for configuration space register access to VMD-owned
   root ports.  It works similarly to MMCONFIG for extended configuration
   space.  Bus numbering is independent and does not conflict with the
   primary domain.

   2) MMIO Space: BARs 2 and 4 ("MEMBAR1" and "MEMBAR2") of VMD provide the
   base address, size, and type for MMIO register access.  These addresses
   are not translated by VMD hardware; they are simply reservations to be
   distributed to root ports' memory base/limit registers and subdivided
   among devices downstream.

   3) DMA: To interact appropriately with an IOMMU, the source ID DMA read
   and write requests are translated to the bus-device-function of the VMD
   endpoint.  Otherwise, DMA operates normally without VMD-specific address
   translation.

   4) Interrupts: Part of VMD's BAR 4 is reserved for VMD's MSI-X Table and
   PBA.  MSIs from VMD domain devices and ports are remapped to appear as
   if they were issued using one of VMD's MSI-X table entries.  Each MSI
   and MSI-X address of VMD-owned devices and ports has a special format
   where the address refers to specific entries in the VMD's MSI-X table.
   As with DMA, the interrupt source ID is translated to VMD's
   bus-device-function.

   The driver provides its own MSI and MSI-X configuration functions
   specific to how MSI messages are used within the VMD domain, and
   provides an irq_chip for independent IRQ allocation to relay interrupts
   from VMD's interrupt handler to the appropriate device driver's handler.

   5) Errors: PCIe error message are intercepted by the root ports normally
   (e.g., AER), except with VMD, system errors (i.e., firmware first) are
   disabled by default.  AER and hotplug interrupts are translated in the
   same way as endpoint interrupts.

   6) VMD does not support INTx interrupts or IO ports.  Devices or drivers
   requiring these features should either not be placed below VMD-owned
   root ports, or VMD should be disabled by BIOS for such endpoints.

d9c3d6ff22 x86/PCI: Allow DMA ops specific to a PCI domain
The Intel Volume Management Device (VMD) is a PCIe endpoint that acts as a
host bridge to another PCI domain.  When devices below the VMD perform DMA,
the VMD replaces their DMA source IDs with its own source ID.  Therefore,
those devices require special DMA ops.

Add interfaces to allow the VMD driver to set up dma_ops for the devices
below it.

Commit 4d6b4e69a2 ("x86/PCI/ACPI: Use common interface to support
PCI host bridge") converted x86 to use the common interface
acpi_pci_root_create, but the conversion missed on code piece in
arch/x86/pci/bus_numa.c, which causes regression on some legacy
AMD platforms as reported by Arthur Marsh <arthur.marsh@internode.on.net>.
The root causes is that acpi_pci_root_create() fails to insert
host bridge resources into iomem_resource/ioport_resource because
x86_pci_root_bus_resources() has already inserted those resources.
So change x86_pci_root_bus_resources() to not insert resources into
iomem_resource/ioport_resource.

8affb487d4 x86/PCI: Don't alloc pcibios-irq when MSI is enabled
The pcibios-irq and MSI both use dev->irq to store the IRQ number.  While
the MSI code checks for that and frees the pcibios-irq before overwriting
dev->irq, the pcibios_alloc_irq() function does not.

Usually this is not a problem, as the pcibios-irq is allocated before probe
time of the device and the MSI IRQ is allocted from the driver's probe
path.

But there are PCI devices handled by the core kernel and not by a standard
PCI driver, like the AMD IOMMU for example.  For the AMD IOMMU a normal PCI
device driver does not make sense, because a driver can be forcibly unbound
from its device, which is not a good idea for an IOMMU.

Nevertheless the PCI core code tries to match the PCI device implementing
the AMD IOMMU against drivers, and allocates/frees a pcibios IRQ every time
it tries out a new driver.  This overwrites the dev->irq field set by
pci_enable_msi() and sets it to 0 in the end (because the probe fails and
the pcibios-irq is freed again).

On suspend/resume this breaks the kernel, because the IRQ descriptor for
IRQ 0 is NULL.

Fix this by not allocating a pcibios-irq when MSI is already active.  This
also has the benefit, that a device claimed by the core kernel can not be
probed by a PCI driver later.

Revert dff22d2054 ("PCI: Call pci_read_bridge_bases() from core instead
of arch code").

Reading PCI bridge windows is not arch-specific in itself, but there is PCI
core code that doesn't work correctly if we read them too early.  For
example, Hannes found this case on an ARM Freescale i.mx6 board:

  pci_bus 0000:00: root bus resource [mem 0x01000000-0x01efffff]
  pci 0000:00:00.0: PCI bridge to [bus 01-ff]
  pci 0000:00:00.0: BAR 8: no space for [mem size 0x01000000] (mem window)
  pci 0000:01:00.0: BAR 2: failed to assign [mem size 0x00200000]
  pci 0000:01:00.0: BAR 1: failed to assign [mem size 0x00004000]
  pci 0000:01:00.0: BAR 0: failed to assign [mem size 0x00000100]

The 00:00.0 mem window needs to be at least 3MB: the 01:00.0 device needs
0x204100 of space, and mem windows are megabyte-aligned.

Bus sizing can increase a bridge window size, but never *decrease* it (see
d65245c329 ("PCI: don't shrink bridge resources")).  Prior to
dff22d2054, ARM didn't read bridge windows at all, so the "original size"
was zero, and we assigned a 3MB window.

After dff22d2054, we read the bridge windows before sizing the bus.  The
firmware programmed a 16MB window (size 0x01000000) in 00:00.0, and since
we never decrease the size, we kept 16MB even though we only needed 3MB.
But 16MB doesn't fit in the host bridge aperture, so we failed to assign
space for the window and the downstream devices.

I think this is a defect in the PCI core: we shouldn't rely on the firmware
to assign sensible windows.

Ray reported a similar problem, also on ARM, with Broadcom iProc.

Issues like this are too hard to fix right now, so revert dff22d2054.

991de2e590 PCI, x86: Implement pcibios_alloc_irq() and pcibios_free_irq()
To support IOAPIC hotplug, we need to allocate PCI IRQ resources on demand
and free them when not used anymore.

Implement pcibios_alloc_irq() and pcibios_free_irq() to dynamically
allocate and free PCI IRQs.

Remove mp_should_keep_irq(), which is no longer used.

39d9b77b8d x86/pci/intel_mid_pci: Work around for IRQ0 assignment
On Intel Tangier the MMC host controller is wired up to irq 0. But
several other devices have irq 0 associated as well due to a bogus PCI
configuration.

The first initialized driver will acquire irq 0 and make it
unavailable for other devices. If the sdhci driver is not the first
one it will fail to acquire the interrupt and therefor be non
functional.

Add a quirk to the pci irq enable function which denies irq 0 to
anything else than the MMC host controller driver on Tangier
platforms.

dff22d2054 PCI: Call pci_read_bridge_bases() from core instead of arch code
When we scan a PCI bus, we read PCI-PCI bridge window registers with
pci_read_bridge_bases() so we can validate the resource hierarchy.  Most
architectures call pci_read_bridge_bases() from pcibios_fixup_bus(), but
PCI-PCI bridges are not arch-specific, so this doesn't need to be in
arch-specific code.

Call pci_read_bridge_bases() directly from the PCI core instead of from
arch code.

For alpha and mips, we now call pci_read_bridge_bases() always; previously
we only called it if PCI_PROBE_ONLY was set.

e6323e3c51 PCI: Fix generic NCR 53c810 class code quirk
In the generic quirk fixup_rev1_53c810(), added by a5312e28c1 ("[PATCH]
PCI: NCR 53c810 quirk"), we assigned "class = PCI_CLASS_STORAGE_SCSI".  But
PCI_CLASS_STORAGE_SCSI is only the two-byte base class/sub-class and needs
to be shifted to make space for the low-order interface byte.

Furthermore, we had a similar quirk, pci_fixup_ncr53c810(), for arch/x86,
which assigned class correctly.  The arch code is linked before the PCI
core, so arch quirks run before generic quirks.  Therefore, on x86, the x86
arch quirk ran first, and the generic quirk did nothing because it saw that
dev->class was already set.  But on other arches, the generic quirk set the
wrong class code.

Fix the generic quirk to set the correct class code and remove the
now-unnecessary x86-specific quirk.

3d9fecf6bf x86/PCI: Use host bridge _CRS info on systems with >32 bit addressing
We enable _CRS on all systems from 2008 and later.  On older systems, we
ignore _CRS and assume the whole physical address space (excluding RAM and
other devices) is available for PCI devices, but on systems that support
physical address spaces larger than 4GB, it's doubtful that the area above
4GB is really available for PCI.

After d56dbf5bab ("PCI: Allocate 64-bit BARs above 4G when possible"), we
try to use that space above 4GB *first*, so we're more likely to put a
device there.

On Juan's Toshiba Satellite Pro U200, BIOS left the graphics, sound, 1394,
and card reader devices unassigned (but only after Windows had been
booted).  Only the sound device had a 64-bit BAR, so it was the only device
placed above 4GB, and hence the only device that didn't work.

Keep _CRS enabled even on pre-2008 systems if they support physical address
space larger than 4GB.

dc4fdaf0e4 PCI / ACPI: Do not set ACPI companions for host bridges with parents
Commit 97badf873a (device property: Make it possible to use
secondary firmware nodes) uncovered a bug in the x86 (and ia64) PCI
host bridge initialization code that assumes bridge->bus->sysdata
to always point to a struct pci_sysdata object which need not be
the case (in particular, the Xen PCI frontend driver sets it to point
to a different data type).  If it is not the case, an incorrect
pointer (or a piece of data that is not a pointer at all) will be
passed to ACPI_COMPANION_SET() and that may cause interesting
breakage to happen going forward.

To work around this problem use the observation that the ACPI
host bridge initialization always passes NULL as parent to
pci_create_root_bus(), so if pcibios_root_bridge_prepare() sees
a non-NULL parent of the bridge, it should not attempt to set
an ACPI companion for it, because that means that
pci_create_root_bus() has been called by someone else.

cb32edf65b x86/mm/pat: Wrap pat_enabled into a function API
We use pat_enabled in x86-specific code to see if PAT is enabled
or not but we're granting full access to it even though readers
do not need to set it. If, for instance, we granted access to it
to modules later they then could override the variable
setting... no bueno.

This renames pat_enabled to a new static variable __pat_enabled.
Folks are redirected to use pat_enabled() now.

Code that sets this can only be internal to pat.c. Apart from
the early kernel parameter "nopat" to disable PAT, we also have
a few cases that disable it later and make use of a helper
pat_disable(). It is wrapped under an ifdef but since that code
cannot run unless PAT was enabled its not required to wrap it
with ifdefs, unwrap that. Likewise, since "nopat" doesn't really
change non-PAT systems just remove that ifdef as well.

Although we could add and use an early_param_off(), these
helpers don't use __read_mostly but we want to keep
__read_mostly for __pat_enabled as this is a hot path -- upon
boot, for instance, a simple guest may see ~4k accesses to
pat_enabled(). Since __read_mostly early boot params are not
that common we don't add a helper for them just yet.

This routine has been around for over a decade, but with EISA
being dead and abandoned for about twice that long, the name can
be kind of confusing.  The function is going at the PIC Edge/Level
Configuration Registers (ELCR), so rename it as such and mentally
decouple it from the long since dead EISA bus.

2c62e8492e x86/PCI/ACPI: Make all resources except [io 0xcf8-0xcff] available on PCI bus
An IO port or MMIO resource assigned to a PCI host bridge may be
consumed by the host bridge itself or available to its child
bus/devices. The ACPI specification defines a bit (Producer/Consumer)
to tell whether the resource is consumed by the host bridge itself,
but firmware hasn't used that bit consistently, so we can't rely on it.

Before commit 593669c2ac ("x86/PCI/ACPI: Use common ACPI resource
interfaces to simplify implementation"), arch/x86/pci/acpi.c ignored
all IO port resources defined by acpi_resource_io and
acpi_resource_fixed_io to filter out IO ports consumed by the host
bridge itself.

Commit 593669c2ac ("x86/PCI/ACPI: Use common ACPI resource interfaces
to simplify implementation") started accepting all IO port and MMIO
resources, which caused a regression that IO port resources consumed
by the host bridge itself became available to its child devices.

Then commit 63f1789ec7 ("x86/PCI/ACPI: Ignore resources consumed by
host bridge itself") ignored resources consumed by the host bridge
itself by checking the IORESOURCE_WINDOW flag, which accidently removed
MMIO resources defined by acpi_resource_memory24, acpi_resource_memory32
and acpi_resource_fixed_memory32.

On x86 and IA64 platforms, all IO port and MMIO resources are assumed
to be available to child bus/devices except one special case:
    IO port [0xCF8-0xCFF] is consumed by the host bridge itself
    to access PCI configuration space.

So explicitly filter out PCI CFG IO ports[0xCF8-0xCFF]. This solution
will also ease the way to consolidate ACPI PCI host bridge common code
from x86, ia64 and ARM64.

9e8ce4b96b Revert "x86/PCI: Refine the way to release PCI IRQ resources"
Commit b4b55cda58 (Refine the way to release PCI IRQ resources)
introduced a regression in the PCI IRQ resource management by causing
the IRQ resource of a device, established when pci_enabled_device()
is called on a fully disabled device, to be released when the driver
is unbound from the device, regardless of the enable_cnt.

This leads to the situation that an ill-behaved driver can now make a
device unusable to subsequent drivers by an imbalance in their use of
pci_enable/disable_device().  That is a serious problem for secondary
drivers like vfio-pci, which are innocent of the transgressions of
the previous driver.

Since the solution of this problem is not immediate and requires
further discussion, revert commit b4b55cda58 and the issue it was
supposed to address (a bug related to xen-pciback) will be taken
care of in a different way going forward.

b97ea289cf PCI: Assign resources before drivers claim devices (pci_scan_root_bus())
Previously, pci_scan_root_bus() created a root PCI bus, enumerated the
devices on it, and called pci_bus_add_devices(), which made the devices
available for drivers to claim them.

Most callers assigned resources to devices after pci_scan_root_bus()
returns, which may be after drivers have claimed the devices.  This is
incorrect; the PCI core should not change device resources while a driver
is managing the device.

Remove pci_bus_add_devices() from pci_scan_root_bus() and do it after any
resource assignment in the callers.

Note that ARM's pci_common_init_dev() already called pci_bus_add_devices()
after pci_scan_root_bus(), so we only need to remove the first call:
