Always stay within page boundaries when copying from user within
strlen_user_mvcos()/strncpy_from_user_mvcos(). This allows to
shorten the code a bit and may prevent unnecessary faults, since
we copy quite large amounts of memory to kernel space.
Also directly call the mvcos variants of copy_from_user() to
avoid indirect branches.

I'm not sure why, but the hlist for each entry iterators were conceived
list_for_each_entry(pos, head, member)
The hlist ones were greedy and wanted an extra parameter:
hlist_for_each_entry(tpos, pos, head, member)
Why did they need an extra pos parameter? I'm not quite sure. Not only
they don't really need it, it also prevents the iterator from looking
exactly like the list iterator, which is unfortunate.
Besides the semantic patch, there was some manual work required:
- Fix up the actual hlist iterators in linux/list.h
- Fix up the declaration of other iterators based on the hlist ones.
- A very small amount of places were using the 'node' parameter, this
was modified to use 'obj->member' instead.
- Coccinelle didn't handle the hlist_for_each_entry_safe iterator
properly, so those had to be fixed up manually.
The semantic patch which is mostly the work of Peter Senna Tschudin is here:

Pull x86/EFI changes from Peter Anvin:
- Improve the initrd handling in the EFI boot stub by allowing forward
slashes in the pathname - from Chun-Yi Lee.
- Cleanup code duplication in the EFI mixed kernel/firmware code - from
Satoru Takeuchi.
- efivarfs bug fixes for more strict filename validation, with lots of
input from Al Viro.
* 'x86-efi-for-linus' of git://git.kernel.org/pub/scm/linux/kernel/git/tip/tip:
x86, efi: remove duplicate code in setup_arch() by using, efi_is_native()
efivarfs: guid part of filenames are case-insensitive
efivarfs: Validate filenames much more aggressively
efivarfs: Use sizeof() instead of magic number
x86, efi: Allow slash in file path of initrd
Pull more x86 fixes from Peter Anvin:
"Additional x86 fixes. Three of these patches are pure documentation,
two are pretty trivial; the remaining one fixes boot problems on some
non-BIOS machines."
* 'x86-urgent-for-linus' of git://git.kernel.org/pub/scm/linux/kernel/git/tip/tip:
x86: Make sure we can boot in the case the BDA contains pure garbage
x86, efi: Mark disable_runtime as __initdata
x86, doc: Fix incorrect comment about 64-bit code segment descriptors
doc, kernel-parameters: Document 'console=hvc<n>'
doc, xen: Mention 'earlyprintk=xen' in the documentation.
ACPI: Overriding ACPI tables via initrd only works with an initrd and on X86
On non-BIOS platforms it is possible that the BIOS data area contains
garbage instead of being zeroed or something equivalent (firmware
people: we are talking of 1.5K here, so please do the sane thing.)
We need on the order of 20-30K of low memory in order to boot, which
may grow up to < 64K in the future. We probably want to avoid the
lowest of the low memory. At the same time, it seems extremely
unlikely that a legitimate EBDA would ever reach down to the 128K
(which would require it to be over half a megabyte in size.) Thus,
pick 128K as the cutoff for "this is insane, ignore." We may still
end up reserving a bunch of extra memory on the low megabyte, but that
is not really a major issue these days. In the worst case we lose
512K of RAM.
This code really should be merged with trim_bios_range() in
arch/x86/kernel/setup.c, but that is a bigger patch for a later merge
window.

The upstream kernel ABI (v3) is different from current out-of-tree (v2):
* no-legacy-syscalls

So we rev up the ABI version
Signed-off-by: Vineet Gupta <vgupta@synopsys.com>
ptrace regset interface relies on ELF_NGREG for ceiling the size of user
request. So any larger request (even if legit) would be clipped.
The existing def of ELF_NGREG didn't use user_regs_struct and was
technically one placeholder short (stop_pc) - although the current code
would still work because pt_regs includes a bunch of extra fields,
making
ELF_NGREG >= sizeof(struct user_regs_struct)/sizeof(long)
But we need to remove this ambiguity, specially since pt_regs should NOT
be directly associated with with anything userspace-ish.

The flat DT (currently embedded in vmlinux) is in .init section.
The unflattened/binary tree doesn't copy strings through and references
them from orig flat DT - which could cause catestrohpy if of_* APIs are
called post init, say from a driver which is a loadable module.

As we can see, the instruction triggering the exception is the "vmov"

properly take account of the FPEXC_FP2V flag in FPEXC. This is because
the test for the second instruction register being valid is bogus, and
will always skip emulation of the second instruction.

Without this patch any kernel code that reads kernel memory in
non present kernel pte/pmds (as set by pageattr.c) will crash.
With this kernel code:
static struct page *crash_page;
static unsigned long *crash_address;
[..]
crash_page = alloc_pages(GFP_KERNEL, 9);
crash_address = page_address(crash_page);
if (set_memory_np((unsigned long)crash_address, 1))
printk("set_memory_np failure\n");
[..]
The kernel will crash if inside the "crash tool" one would try
to read the memory at the not present address.
crash> p crash_address
crash_address = $8 = (long unsigned int *) 0xffff88023c000000
crash> rd 0xffff88023c000000
[ *lockup* ]
The lockup happens because _PAGE_GLOBAL and _PAGE_PROTNONE
shares the same bit, and pageattr leaves _PAGE_GLOBAL set on a
kernel pte which is then mistaken as _PAGE_PROTNONE (so
pte_present returns true by mistake and the kernel fault then
gets confused and loops).
With THP the same can happen after we taught pmd_present to
check _PAGE_PROTNONE and _PAGE_PSE in commit
027ef6c878 ("mm: thp: fix pmd_present for
split_huge_page and PROT_NONE with THP"). THP has the same
problem with _PAGE_GLOBAL as the 4k pages, but it also has a
problem with _PAGE_PSE, which must be cleared too.
After the patch is applied copy_user correctly returns -EFAULT
and doesn't lockup anymore.

I got a report for a minor regression introduced by commit
027ef6c878 ("mm: thp: fix pmd_present for split_huge_page and
PROT_NONE with THP").
So the problem is, pageattr creates kernel pagetables (pte and
pmds) that breaks pte_present/pmd_present and the patch above
exposed this invariant breakage for pmd_present.
The same problem already existed for the pte and pte_present and
it was fixed by commit 660a293ea9 ("x86, mm: Make
spurious_fault check explicitly check the PRESENT bit") (if it
wasn't for that commit, it wouldn't even be a regression). That
fix avoids the pagefault to use pte_present. I could follow
through by stopping using pmd_present/pmd_huge too.
However I think it's more robust to fix pageattr and to clear
the PSE/GLOBAL bitflags too in addition to the present bitflag.
So the kernel page fault can keep using the regular
pte_present/pmd_present/pmd_huge.
The confusion arises because _PAGE_GLOBAL and _PAGE_PROTNONE are
sharing the same bit, and in the pmd case we pretend _PAGE_PSE
to be set only in present pmds (to facilitate split_huge_page
final tlb flush).

swap_lock is heavily contended when I test swap to 3 fast SSD (even
slightly slower than swap to 2 such SSD). The main contention comes
from swap_info_get(). This patch tries to fix the gap with adding a new
per-partition lock.
Global data like nr_swapfiles, total_swap_pages, least_priority and
swap_list are still protected by swap_lock.
nr_swap_pages is an atomic now, it can be changed without swap_lock. In
theory, it's possible get_swap_page() finds no swap pages but actually
there are free swap pages. But sounds not a big problem.
Accessing partition specific data (like scan_swap_map and so on) is only
protected by swap_info_struct.lock.
Changing swap_info_struct.flags need hold swap_lock and
swap_info_struct.lock, because scan_scan_map() will check it. read the
flags is ok with either the locks hold.
If both swap_lock and swap_info_struct.lock must be hold, we always hold
the former first to avoid deadlock.
swap_entry_free() can change swap_list. To delete that code, we add a
new highest_priority_index. Whenever get_swap_page() is called, we
check it. If it's valid, we use it.
It's a pity get_swap_page() still holds swap_lock(). But in practice,
swap_lock() isn't heavily contended in my test with this patch (or I can
say there are other much more heavier bottlenecks like TLB flush). And
BTW, looks get_swap_page() doesn't really need the lock. We never free
swap_info[] and we check SWAP_WRITEOK flag. The only risk without the
lock is we could swapout to some low priority swap, but we can quickly
recover after several rounds of swap, so sounds not a big deal to me.
But I'd prefer to fix this if it's a real problem.
"swap: make each swap partition have one address_space" improved the
swapout speed from 1.7G/s to 2G/s. This patch further improves the
speed to 2.3G/s, so around 15% improvement. It's a multi-process test,
so TLB flush isn't the biggest bottleneck before the patches.

We now provide an option for users who don't want to specify physical
memory address in kernel commandline.
/*
* For movablemem_map=acpi:
*
* SRAT: |_____| |_____| |_________| |_________| ......
* node id: 0 1 1 2
* hotpluggable: n y y n
* movablemem_map: |_____| |_________|
*
* Using movablemem_map, we can prevent memblock from allocating memory
* on ZONE_MOVABLE at boot time.
*/
So user just specify movablemem_map=acpi, and the kernel will use
hotpluggable info in SRAT to determine which memory ranges should be set
as ZONE_MOVABLE.
If all the memory ranges in SRAT is hotpluggable, then no memory can be
used by kernel. But before parsing SRAT, memblock has already reserve
some memory ranges for other purposes, such as for kernel image, and so
on. We cannot prevent kernel from using these memory. So we need to
exclude these ranges even if these memory is hotpluggable.
Furthermore, there could be several memory ranges in the single node
which the kernel resides in. We may skip one range that have memory
reserved by memblock, but if the rest of memory is too small, then the
kernel will fail to boot. So, make the whole node which the kernel
resides in un-hotpluggable. Then the kernel has enough memory to use.
NOTE: Using this way will cause NUMA performance down because the
whole node will be set as ZONE_MOVABLE, and kernel cannot use memory
on it. If users don't want to lose NUMA performance, just don't use
it.

When implementing movablemem_map boot option, we introduced an array
movablemem_map.map[] to store the memory ranges to be set as
ZONE_MOVABLE.
Since ZONE_MOVABLE is the latst zone of a node, if user didn't specify
the whole node memory range, we need to extend it to the node end so
that we can use it to prevent memblock from allocating memory in the
ranges user didn't specify.
We now implement movablemem_map boot option like this:
/*
* For movablemem_map=nn[KMG]@ss[KMG]:
*
* SRAT: |_____| |_____| |_________| |_________| ......
* node id: 0 1 1 2

*
* Using movablemem_map, we can prevent memblock from allocating memory
* on ZONE_MOVABLE at boot time.
*
* NOTE: In this case, SRAT info will be ingored.

On linux, the pages used by kernel could not be migrated. As a result,
if a memory range is used by kernel, it cannot be hot-removed. So if we
want to hot-remove memory, we should prevent kernel from using it.
The way now used to prevent this is specify a memory range by
movablemem_map boot option and set it as ZONE_MOVABLE.
But when the system is booting, memblock will allocate memory, and
reserve the memory for kernel. And before we parse SRAT, and know the
node memory ranges, memblock is working. And it may allocate memory in
ranges to be set as ZONE_MOVABLE. This memory can be used by kernel,
and never be freed.
So, let's parse SRAT before memblock is called first. And it is early
enough.
The first call of memblock_find_in_range_node() is in:
setup_arch()
|-->setup_real_mode()
so, this patch add a function early_parse_srat() to parse SRAT, and call
it before setup_real_mode() is called.
NOTE:
1) early_parse_srat() is called before numa_init(), and has initialized
numa_meminfo. So DO NOT clear numa_nodes_parsed in numa_init() and DO
NOT zero numa_meminfo in numa_init(), otherwise we will lose memory
numa info.
2) I don't know why using count of memory affinities parsed from SRAT
as a return value in original acpi_numa_init(). So I add a static
variable srat_mem_cnt to remember this count and use it as the return
value of the new acpi_numa_init()

During the implementation of SRAT support, we met a problem. In
setup_arch(), we have the following call series:
1) memblock is ready;
2) some functions use memblock to allocate memory;
3) parse ACPI tables, such as SRAT.
Before 3), we don't know which memory is hotpluggable, and as a result,
we cannot prevent memblock from allocating hotpluggable memory. So, in
2), there could be some hotpluggable memory allocated by memblock.
Now, we are trying to parse SRAT earlier, before memblock is ready. But
I think we need more investigation on this topic. So in this v5, I
dropped all the SRAT support, and v5 is just the same as v3, and it is
based on 3.8-rc3.
As we planned, we will support getting info from SRAT without users'
participation at last. And we will post another patch-set to do so.
And also, I think for now, we can add this boot option as the first step
of supporting movable node. Since Linux cannot migrate the direct
mapped pages, the only way for now is to limit the whole node containing
only movable memory.
Using SRAT is one way. But even if we can use SRAT, users still need an
interface to enable/disable this functionality if they don't want to
loose their NUMA performance. So I think, a user interface is always
needed.
For now, users can disable this functionality by not specifying the boot
option. Later, we will post SRAT support, and add another option value
"movablecore_map=acpi" to using SRAT.
This patch:
If system can create movable node which all memory of the node is
allocated as ZONE_MOVABLE, setup_node_data() cannot allocate memory for
the node's pg_data_t. So, use memblock_alloc_try_nid() instead of
memblock_alloc_nid() to retry when the first allocation fails.

The reason is that the cpu's node is not NUMA_NO_NODE, we will call
alloc_pages_exact_node() to alloc memory on the node, but the node is
offlined.
If the node is onlined, we still need cpu's node. For example: a task
on the cpu is sleeped when the cpu is hotremoved. We will choose
another cpu to run this task when it is waked up. If we know the cpu's
node, we will choose the cpu on the same node first. So we should clear
cpu-to-node mapping when the node is offlined.
This patch only clears apicid-to-node mapping when the cpu is
hotremoved.

Introduce a new API vmemmap_free() to free and remove vmemmap
pagetables. Since pagetable implements are different, each architecture
has to provide its own version of vmemmap_free(), just like
vmemmap_populate().
Note: vmemmap_free() is not implemented for ia64, ppc, s390, and sparc.

When memory is removed, the corresponding pagetables should alse be
removed. This patch introduces some common APIs to support vmemmap
pagetable and x86_64 architecture direct mapping pagetable removing.
All pages of virtual mapping in removed memory cannot be freed if some
pages used as PGD/PUD include not only removed memory but also other
memory. So this patch uses the following way to check whether a page
can be freed or not.
1) When removing memory, the page structs of the removed memory are
filled with 0FD.
2) All page structs are filled with 0xFD on PT/PMD, PT/PMD can be
cleared. In this case, the page used as PT/PMD can be freed.
For direct mapping pages, update direct_pages_count[level] when we freed
their pagetables. And do not free the pages again because they were
freed when offlining.
For vmemmap pages, free the pages and their pagetables.
For larger pages, do not split them into smaller ones because there is
no way to know if the larger page has been split. As a result, there is
no way to decide when to split. We deal the larger pages in the
following way:
1) For direct mapped pages, all the pages were freed when they were
offlined. And since menmory offline is done section by section, all
the memory ranges being removed are aligned to PAGE_SIZE. So only need
to deal with unaligned pages when freeing vmemmap pages.
2) For vmemmap pages being used to store page_struct, if part of the
larger page is still in use, just fill the unused part with 0xFD. And
when the whole page is fulfilled with 0xFD, then free the larger page.

For removing memmap region of sparse-vmemmap which is allocated bootmem,
memmap region of sparse-vmemmap needs to be registered by
get_page_bootmem(). So the patch searches pages of virtual mapping and
registers the pages by get_page_bootmem().
NOTE: register_page_bootmem_memmap() is not implemented for ia64,
ppc, s390, and sparc. So introduce CONFIG_HAVE_BOOTMEM_INFO_NODE
and revert register_page_bootmem_info_node() when platform doesn't
support it.
It's implemented by adding a new Kconfig option named
CONFIG_HAVE_BOOTMEM_INFO_NODE, which will be automatically selected
by memory-hotplug feature fully supported archs(currently only on
x86_64).
Since we have 2 config options called MEMORY_HOTPLUG and
MEMORY_HOTREMOVE used for memory hot-add and hot-remove separately,
and codes in function register_page_bootmem_info_node() are only
used for collecting infomation for hot-remove, so reside it under
MEMORY_HOTREMOVE.
Besides page_isolation.c selected by MEMORY_ISOLATION under
MEMORY_HOTPLUG is also such case, move it too.

Allocating a file structure in function get_empty_filp() might fail because
of several reasons:
- not enough memory for file structures
- operation is not allowed
- user is over its limit
Currently the function returns NULL in all cases and we loose the exact
reason of the error. All callers of get_empty_filp() assume that the function
can fail with ENFILE only.
Return error through pointer. Change all callers to preserve this error code.

We are trying to figure out whether we need to up-date the user PGD as
well. Please keep in mind that under 64-bit PV guests we have a limited
amount of rings: 0 for the Hypervisor, and 1 for both the Linux kernel
and user-space. As such the Linux pvops'fied version of write_cr3
checks if it has to update the user-space cr3 as well.
That clearly is not needed during early bootup. The recent changes (see
above git commit) streamline the x86 page table allocation to be much
simpler (And also incidentally the #PF handler ends up in spirit being
similar to how the Xen toolstack sets up the initial page-tables).
The fix is to have an early-bootup version of cr3 that just loads the
kernel %cr3. The later version - which also handles user-page
modifications will be used after the initial page tables have been
setup.
[ hpa: removed a redundant #ifdef and made the new function __init.
Also note that x86-32 already has such an early xen_write_cr3. ]

The code requires the use of the proper per-exception-vector stub

Pull x86 mm changes from Peter Anvin:
"This is a huge set of several partly interrelated (and concurrently
developed) changes, which is why the branch history is messier than
one would like.
The *really* big items are two humonguous patchsets mostly developed
by Yinghai Lu at my request, which completely revamps the way we
create initial page tables. In particular, rather than estimating how
much memory we will need for page tables and then build them into that
memory -- a calculation that has shown to be incredibly fragile -- we
now build them (on 64 bits) with the aid of a "pseudo-linear mode" --
a #PF handler which creates temporary page tables on demand.
This has several advantages:
1. It makes it much easier to support things that need access to data
very early (a followon patchset uses this to load microcode way
early in the kernel startup).
2. It allows the kernel and all the kernel data objects to be invoked
from above the 4 GB limit. This allows kdump to work on very large
systems.
3. It greatly reduces the difference between Xen and native (Xen's
equivalent of the #PF handler are the temporary page tables created
by the domain builder), eliminating a bunch of fragile hooks.
The patch series also gets us a bit closer to W^X.
Additional work in this pull is the 64-bit get_user() work which you
were also involved with, and a bunch of cleanups/speedups to
__phys_addr()/__pa()."

This provides a band-aid to provide stable page writes on jbd without
needing to backport the fixed locking and page writeback bit handling
schemes of jbd2. The band-aid works by using bounce buffers to snapshot
page contents instead of waiting.
For those wondering about the ext3 bandage -- fixing the jbd locking
(which was done as part of ext4dev years ago) is a lot of surgery, and
setting PG_writeback on data pages when we actually hold the page lock
dropped ext3 performance by nearly an order of magnitude. If we're
going to migrate iscsi and raid to use stable page writes, the
complaints about high latency will likely return. We might as well
centralize their page snapshotting thing to one place.

On cris-linux-gcc, __SIZE_TYPE__ expands to "unsigned int", as
gcc-4.6.3-nolibc/cris-linux/lib/gcc/cris-linux/4.6.3/plugin/include/config/cris/linux.h
has
#define SIZE_TYPE "unsigned int"
Hence __kernel_size_t is also "unsigned int". But __kernel_ssize_t is
"long", which has a different base type, causing compiler warnings like:
fs/quota/quota_tree.c:372:4: warning: format '%zd' expects argument of type 'signed size_t', but argument 4 has type 'ssize_t' [-Wformat]
To fix this, __kernel_ssize_t should be changed to "int". Hence cris can
just use the generic 32-bit versions from include/asm-generic/posix_types.h
for all size-related types.

This contains parts of the ARM KVM support that have dependencies on
other patches merged through the arm-soc tree. In combination with
patches coming through Russell's tree, this will finally add full
support for the kernel based virtual machine on ARM, which has
been awaited for some time now.
Further, we now have a separate platform for virtual machines
and qemu booting that is used by both Xen and KVM, separating
these from the Versatile Express reference implementation.
Obviously, this new platform is multiplatform capable so it
can be combined with existing machines in the same kernel.

Converting more ARM platforms to multiplatform support. This time, OMAP
gets converted, which is a major step since this is by far the largest
platform in terms of code size. The same thing happens to the vt8500
platform.
Conflicts include:
* Two mach/uncompress.h files are removed, the changes made to them
elsewhere can be discarded now.
* Moving the OMAP4 irq_match array has context clashes with turning
omap4_sar_ram_init into an omap_early_initcall()

This is another cleanup series, containing the move of the Renesas
SH-Mobile pin controller code from arch/arm/mach-shmobile over to the
generic pinctrl subsystem, changing it over to the common interfaces in
the process.
Based on agreement between Olof, Paul Mundt, Linus Walleij and Simon,
we're merging this large branch of pinctrl conversion through arm-soc,
even though it contains the corresponding conversions for arch/sh. Main
reason for this is tight dependencies (that will now mostly be broken)
between the arch/sh and mach-shmobile implementations.
There will be more of this in 3.10 to do device-tree bindings, but this
is the initial conversion.

A large number of cleanups, all over the platforms. This is dominated
largely by the Samsung platforms (s3c, s5p, exynos) and a few of the
others moving code out of arch/arm into more appropriate subsystems.
The clocksource and irqchip drivers are now abstracted to the point
where platforms that are already cleaned up do not need to even specify
the driver they use, it can all get configured from the device tree
as we do for normal device drivers. The clocksource changes basically
touch every single platform in the process.
We further clean up the use of platform specific header files here,
with the goal of turning more of the platforms over to being
"multiplatform" enabled, which implies that they cannot expose
their headers to architecture independent code any more.
It is expected that no functional changes are part of the cleanup.
The overall reduction in total code lines is mostly the result of
removing broken and obsolete code.

Here is the big driver core merge for 3.9-rc1
There are two major series here, both of which touch lots of drivers all
over the kernel, and will cause you some merge conflicts:
- add a new function called devm_ioremap_resource() to properly be
able to check return values.
- remove CONFIG_EXPERIMENTAL
If you need me to provide a merged tree to handle these resolutions,
please let me know.
Other than those patches, there's not much here, some minor fixes and
updates.

The biggest change in this update is the unification of HD-audio codec
parsers. Now the HD-audio codec is parsed in a generic parser code
which is invoked by each HD-audio codec driver. Some background
information is found in David Henningsson's blog entry:
http://voices.canonical.com/david.henningsson/2013/01/18/upcoming-changes-to-the-intel-hda-drivers/
Other than that, some random updates/fixes like USB-audio and a bunch
of small AoC updates as usual.
Highlights:
- Unification of HD-audio parser code (aka generic parser)
- Support of new Intel HD-audio controller, new IDT codecs
- Fixes for HD-audio HDMI audio hotplug
- Haswell HDMI audio fixup
- Support of Creative CA0132 DSP code
- A few fixes of HDSP driver
- USB-audio fix for Roland A-PRO, M-Audio FT C600
- Support PM for aloop driver (and fixes Oops)
- Compress API updates for gapless playback support
For ASoC part:
- Support for a wider range of hardware in the compressed stream code
- The ability to mute capture streams as well as playback streams while
inactive
- DT support for AK4642, FSI, Samsung I2S and WM8962
- AC'97 support for Tegra
- New driver for max98090, replacing the stub which was there
- A new driver from Dialog
Note that due to dependencies, DTification of DMA support for Samsung
platforms (used only by the and I2S driver and SPI) is merged here as
well.

Following commit 26ffd0d4 (ARM: mm: introduce present, faulting entries
for PAGE_NONE), if a page has been mapped as PROT_NONE, the L_PTE_VALID
bit is cleared by the set_pte_ext() code. With LPAE the software and
hardware pte share the same location and subsequent modifications of pte
range (change_protection()) will leave the L_PTE_VALID bit cleared.
This patch adds the L_PTE_VALID bit to the newprot mask in pte_modify().

When udelay() is implemented using an architected timer, it is wrong
to scale loops_per_jiffy when changing the CPU clock frequency since
the timer clock remains constant.
The lpj should probably become an implementation detail relevant to
the CPU loop based delay routine only and more confined to it. In the
mean time this is the minimal fix needed to have expected delays with
the timer based implementation when cpufreq is also in use.

Process personality is stored together with other flags like UNAME26 in
an integer variable. Overwriting this value with PER_LINUX drops all
other existing flags and as such broke tools like the uname26 tool.
Actually this was only broken on 32bit kernels, since for 32bit-ELF
binaries on 64-bit kernels the SET_PERSONALITY macro from
arch/parisc/kernel/binfmt_elf32.c is used which does not modifies the
personality value at all (which is wrong as long as we don't run HPUX
binaries or similiar).

1) PTRACE_SYSCALL doesn't work for 64bit process on parisc64.
Compat syscall table is used instead of 64bit one. IMO we should either
refuse to allow PTRACE_SYSCALL for 64bit processes or duplicate the
logics choosing the right syscall table into .Ltracesys.
2) if you have let the tracee run with PTRACE_SYSCALL and
it had stopped, you can use PTRACE_POKEUSR to modify syscall number
(r20) and arguments 1--4 (r26--r23). Modifications will have effect.
However, modifying arguments 5 and 6 (r22 and r21 resp.) works only
when process (32bit one) runs on 64bit host - on 32bit one it has no
effect. AFAICS, the diff below should fix that one.

People are playing odd games with IRQF_DISABLED, remove it.
Its not reliable, since shared interrupt lines could disable it for you,
and its possible and allowed for archs to disable IRQs to limit IRQ nesting.
Therefore, simply mandate that _ALL_ IRQ handlers are run with IRQs disabled.
[ This _should_ not break anything, since we've mandated that IRQ handlers
_must_ be able to deal with this for a _long_ time ]
IRQ handlers should be fast, no if buts and any other exceptions. We also have
plenty instrumentation to find any offending IRQ latency sources.

CONFIG_PARISC_TMPALIAS enables clear_user_highpage and copy_user_highpage.
These are essentially alternative implementations of clear_user_page and
copy_user_page. They don't have anything to do with x86 high pages, but they
build on the infrastructure to save a few instructions. Read the comment in
clear_user_highpage as it is very important to the implementation. For this
reason, there isn't any gain in using the TMPALIAS/highpage approach.

flush_cache_mm, for the non current case also uses flush_dcache_page_asm
and flush_icache_page_asm which are TMPALIAS flushes.
For the non current case, the algorithm used by get_ptep is derived from the
vmalloc_to_page implementation in vmalloc.c. It is essentially a generic page
table lookup. The other alternative was to duplicate the lookup in entry.S.
The break point for switching to a full cache flush is somewhat arbitrary. The
same approach is used in flush_cache_range for non current case. In a GCC
build and check, many small programs are executed and this change provided a
significant performance enhancement, e.g. GCC build time was cut almost in half
on a rp3440 at j4. Previously, we always flushed the entire cache.

Implement clear_page_asm and copy_page_asm. These are optimized routines to
clear and copy a page. I tested prefetch optimizations in clear_page_asm and
copy_page_asm but didn't see any significant performance improvement on rp3440.
I'm not sure if these are routines are significantly faster than memset and/or
memcpy, but they are there for further performance evaluation.
TLB purge operations on PA 1.X SMP machines are now serialized with the help of
the new tlb_lock() and tlb_unlock() macros, since on some PA-RISC machines, TLB
purges need to be serialized in software. Obviously, lock isn't needed in UP
kernels. On PA 2.0 machines, there is a local TLB instruction which is much
less disruptive to the memory subsystem. No lock is needed for local purge.
Loops are also unrolled in flush_instruction_cache_local and
flush_data_cache_local.
The implementation of what used to be copy_user_page (now copy_user_page_asm)
is now fixed. Additionally 64-bit support is now added. Read the preceding
comment which I didn't change. I left the comment but it is now inaccurate.

This is the first patch in a series of 4, with which the page cache flushing of
parisc will gets fixed and enhanced. This even fixes the nasty "minifail" bug
(http://wiki.parisc-linux.org/TestCases?highlight=%28minifail%29) which
prevented parisc to stay an official debian port. Basically the flush in
copy_user_page together with the TLB patch from commit
7139bc1579 is what fixes the minifail bug.
This patch still uses the TMPALIAS approach. The new copy_user_page
implementation calls flush_dcache_page_asm to flush the user dcache page
(crucial for minifail fix) via a kernel TMPALIAS mapping. After that, it just
copies the page using the kernel mapping. It does a final flush if needed.
Generally it is hard to avoid doing some cache flushes using the kernel mapping
(e.g., copy_to_user_page and copy_from_user_page).
This patch depends on a subsequent change to pacache.S implementing
clear_page_asm and copy_page_asm. These are optimized routines to clear and
copy a page. The calls in clear_user_page and copy_user_page could be replaced
by calls to memset and memcpy, respectively. I tested prefetch optimizations
in clear_page_asm and copy_page_asm but didn't see any significant performance
improvement on rp3440. I'm not sure if these are routines are significantly
faster than memset and/or memcpy, but they are there for further performance
evaluation.

When we set the sun4u version of the PTE execute bit, it's:
or REG, _PAGE_EXEC_4U, REG
_PAGE_EXEC_4U is 0x1000, unfortunately the immedate field of the
'or' instruction is a signed 13-bit value. So the above actually
assembles into:
or REG, -4096, REG
completely corrupting the final PTE value.
Set it with a:
sethi %hi(_PAGE_EXEC_4U), TMP
or REG, TMP, REG
sequence instead.
This fixes "git gc" crashes on sun4u machines.

commit 1de63d60cd ("efi: Clear EFI_RUNTIME_SERVICES rather than

its documentation and disable EFI runtime services to prevent the
bricking bug described in commit e0094244e4 ("samsung-laptop:
Disable on EFI hardware"). However, it's not possible to clear
EFI_RUNTIME_SERVICES from an early param function because
EFI_RUNTIME_SERVICES is set in efi_init() *after* parse_early_param().
This resulted in "noefi" effectively becoming a no-op and no longer
providing users with a way to disable EFI, which is bad for those
users that have buggy machines.

If our first THP installation for an MM is via the set_pmd_at() done
during khugepaged's collapsing we'll end up in tsb_grow() trying to do
a GFP_KERNEL allocation with several locks held.
Simply using GFP_ATOMIC in this situation is not the best option
because we really can't have this fail, so we'd really like to keep
this an order 0 GFP_KERNEL allocation if possible.
Also, doing the TSB allocation from khugepaged is a really bad idea
because we'll allocate it potentially from the wrong NUMA node in that
context.
So what we do is defer the hugepage TSB allocation until the first TLB
miss we take on a hugepage. This is slightly tricky because we have
to handle two unusual cases:
1) Taking the first hugepage TLB miss in the window trap handler.
We'll call the winfix_trampoline when that is detected.
2) An initial TSB allocation via TLB miss races with a hugetlb
fault on another cpu running the same MM. We handle this by
unconditionally loading the TSB we see into the current cpu
even if it's non-NULL at hugetlb_setup time.

Accomodate the possibility that the TSB might be NULL at
the point that update_mmu_cache() is invoked. This is
necessary because we will sometimes need to defer the TSB
allocation to the first fault that happens in the 'mm'.
Seperate out the hugepage PTE test into a seperate function
so that the logic is clearer.
