89261c5702 Here's the main MIPS pull for Linux 4.21. Core architecture changes
include:
 
  - Syscall tables & definitions for unistd.h are now generated by
    scripts, providing greater consistency with other architectures &
    making it easier to add new syscalls.
 
  - Support for building kernels with no floating point support, upon
    which any userland attempting to use floating point instructions will
    receive a SIGILL. Mostly useful to shrink the kernel & as preparation
    for nanoMIPS support which does not yet include FP.
 
  - MIPS SIMD Architecture (MSA) vector register context is now exposed
    by ptrace via a new NT_MIPS_MSA regset.
 
  - ASIDs are now stored as 64b values even for MIPS32 kernels, expanding
    the ASID version field sufficiently that we don't need to worry about
    overflow & avoiding rare issues with reused ASIDs that have been
    observed in the wild.
 
  - The branch delay slot "emulation" page is now mapped without write
    permission for the user, preventing its use as a nice location for
    attacks to execute malicious code from.
 
  - Support for ioremap_prot(), primarily to allow gdb or other
    ptrace users the ability to view their tracee's memory using the same
    cache coherency attribute.
 
  - Optimizations to more cpu_has_* macros, allowing more to be
    compile-time constant where possible.
 
  - Enable building the whole kernel with UBSAN instrumentation.
 
  - Enable building the kernel with link-time dead code & data
    elimination.

  - The Boston board gains a workaround for DMA prefetching issues with
    the EG20T Platform Controller Hub that it uses.

MIPS: Loongson3,SGI-IP27: Simplify max_low_pfn calculation
Both the Loongson3 & SGI-IP27 platforms set max_low_pfn to the last
available PFN describing memory. They both do it in paging_init() which
is later than ideal since max_low_pfn is used before that function is
called. Simplify both platforms to trivially initialize max_low_pfn
using the end address of DRAM, and do it earlier in prom_meminit().

MIPS: Simplify FP context initialization
MIPS has up until now had 3 different ways for a task's floating point
context to be initialized:

  - If the task's first use of FP involves it gaining ownership of an
    FPU then _init_fpu() is used to initialize the FPU's registers such
    that they all contain ~0, and the FPU registers will be stored to
    struct thread_info later (eg. when context switching).

  - If the task first uses FP on a CPU without an associated FPU then
    fpu_emulator_init_fpu() initializes the task's floating point
    register state in struct thread_info such that all floating point
    register contain the bit pattern 0x7ff800007ff80000, different to
    the _init_fpu() behaviour.

  - If a task's floating point context is first accessed via ptrace then
    init_fp_ctx() initializes the floating point register state in
    struct thread_info to ~0, giving equivalent state to _init_fpu().

The _init_fpu() path has 2 separate implementations - one for r2k/r3k
style systems & one for r4k style systems. The _init_fpu() path also
requires that we be careful to clear & restore the value of the
Config5.FRE bit on modern systems in order to avoid inadvertently
triggering floating point exceptions.

None of this code is in a performance critical hot path - it runs only
the first time a task uses floating point. As such it doesn't seem to
warrant the complications of maintaining the _init_fpu() path.

Remove _init_fpu() & fpu_emulator_init_fpu(), instead using
init_fp_ctx() consistently to initialize floating point register state
in struct thread_info. Upon a task's first use of floating point this
will typically mean that we initialize state in memory & then load it
into FPU registers using _restore_fp() just as we would on a context
switch. For other paths such as __compute_return_epc_for_insn() or
mipsr2_decoder() this results in a significant simplification of the
work to be done.

MIPS: Loongson-3: Fix BRIDGE irq delivery problem
After commit e509bd7da1 ("genirq: Allow migration of chained
interrupts by installing default action") Loongson-3 fails at here:

setup_irq(LOONGSON_HT1_IRQ, &cascade_irqaction);

This is because both chained_action and cascade_irqaction don't have
IRQF_SHARED flag. This will cause Loongson-3 resume fails because HPET
timer interrupt can't be delivered during S3. So we set the irqchip of
the chained irq to loongson_irq_chip which doesn't disable the chained
irq in CP0.Status.

MIPS: Loongson-3: Fix CPU UART irq delivery problem
Masking/unmasking the CPU UART irq in CP0_Status (and redirecting it to
other CPUs) may cause interrupts be lost, especially in multi-package
machines (Package-0's UART irq cannot be delivered to others). So make
mask_loongson_irq() and unmask_loongson_irq() be no-ops.

The original problem (UART IRQ may deliver to any core) is also because
of masking/unmasking the CPU UART irq in CP0_Status. So it is safe to
remove all of the stuff.

MIPS: kexec: Make a framework for both jumping and halting on nonboot CPUs
The existing implementation lets machine_kexec() CPU jump to reboot code
buffer, whereas other CPUs to relocated_kexec_smp_wait. The natural way to
bring up an SMP new kernel would be to let CPU0 do it while others being
halted. For those failing to do so, fall back to the jumping method.

mips: switch to NO_BOOTMEM
MIPS already has memblock support and all the memory is already registered
with it.

This patch replaces bootmem memory reservations with memblock ones and
removes the bootmem initialization.

Since memblock allocates memory in top-down mode, we ensure that memblock
limit is max_low_pfn to prevent allocations from the high memory.

To have the exceptions base in the lower 512M of the physical memory, its
allocation in arch/mips/kernel/traps.c::traps_init() is using bottom-up
mode.

MIPS: Loongson fix name confict - MEM_RESERVED
MEM_RESERVED is used as a value of enum mem_type in include/linux/
edac.h. This will make failure to build for Loongson in some case:
for example with CONFIG_RAS enabled.

So here rename MEM_RESERVED to SYSTEM_RAM_RESERVED in Loongson code.

b24413180f License cleanup: add SPDX GPL-2.0 license identifier to files with no license
Many source files in the tree are missing licensing information, which
makes it harder for compliance tools to determine the correct license.

By default all files without license information are under the default
license of the kernel, which is GPL version 2.

Update the files which contain no license information with the 'GPL-2.0'
SPDX license identifier.  The SPDX identifier is a legally binding
shorthand, which can be used instead of the full boiler plate text.

This patch is based on work done by Thomas Gleixner and Kate Stewart and
Philippe Ombredanne.

How this work was done:

Patches were generated and checked against linux-4.14-rc6 for a subset of
the use cases:
 - file had no licensing information it it.
 - file was a */uapi/* one with no licensing information in it,
 - file was a */uapi/* one with existing licensing information,

Further patches will be generated in subsequent months to fix up cases
where non-standard license headers were used, and references to license
had to be inferred by heuristics based on keywords.

The analysis to determine which SPDX License Identifier to be applied to
a file was done in a spreadsheet of side by side results from of the
output of two independent scanners (ScanCode & Windriver) producing SPDX
tag:value files created by Philippe Ombredanne.  Philippe prepared the
base worksheet, and did an initial spot review of a few 1000 files.

The 4.13 kernel was the starting point of the analysis with 60,537 files
assessed.  Kate Stewart did a file by file comparison of the scanner
results in the spreadsheet to determine which SPDX license identifier(s)
to be applied to the file. She confirmed any determination that was not
immediately clear with lawyers working with the Linux Foundation.

Criteria used to select files for SPDX license identifier tagging was:
 - Files considered eligible had to be source code files.
 - Make and config files were included as candidates if they contained >5
   lines of source
 - File already had some variant of a license header in it (even if <5
   lines).

All documentation files were explicitly excluded.

The following heuristics were used to determine which SPDX license
identifiers to apply.

 - when both scanners couldn't find any license traces, file was
   considered to have no license information in it, and the top level
   COPYING file license applied.

   For non */uapi/* files that summary was:

   and resulted in the first patch in this series.

 - when the two scanners agreed on the detected license(s), that became
   the concluded license(s).

 - when there was disagreement between the two scanners (one detected a
   license but the other didn't, or they both detected different
   licenses) a manual inspection of the file occurred.

 - In most cases a manual inspection of the information in the file
   resulted in a clear resolution of the license that should apply (and
   which scanner probably needed to revisit its heuristics).

 - When it was not immediately clear, the license identifier was
   confirmed with lawyers working with the Linux Foundation.

 - If there was any question as to the appropriate license identifier,
   the file was flagged for further research and to be revisited later
   in time.

In total, over 70 hours of logged manual review was done on the
spreadsheet to determine the SPDX license identifiers to apply to the
source files by Kate, Philippe, Thomas and, in some cases, confirmation
by lawyers working with the Linux Foundation.

Kate also obtained a third independent scan of the 4.13 code base from
FOSSology, and compared selected files where the other two scanners
disagreed against that SPDX file, to see if there was new insights.  The
Windriver scanner is based on an older version of FOSSology in part, so
they are related.

Thomas did random spot checks in about 500 files from the spreadsheets
for the uapi headers and agreed with SPDX license identifier in the
files he inspected. For the non-uapi files Thomas did random spot checks
in about 15000 files.

In initial set of patches against 4.14-rc6, 3 files were found to have
copy/paste license identifier errors, and have been fixed to reflect the
correct identifier.

Additionally Philippe spent 10 hours this week doing a detailed manual
inspection and review of the 12,461 patched files from the initial patch
version early this week with:
 - a full scancode scan run, collecting the matched texts, detected
   license ids and scores
 - reviewing anything where there was a license detected (about 500+
   files) to ensure that the applied SPDX license was correct
 - reviewing anything where there was no detection but the patch license
   was not GPL-2.0 WITH Linux-syscall-note to ensure that the applied
   SPDX license was correct

This produced a worksheet with 20 files needing minor correction.  This
worksheet was then exported into 3 different .csv files for the
different types of files to be modified.

These .csv files were then reviewed by Greg.  Thomas wrote a script to
parse the csv files and add the proper SPDX tag to the file, in the
format that the file expected.  This script was further refined by Greg
based on the output to detect more types of files automatically and to
distinguish between header and source .c files (which need different
comment types.)  Finally Greg ran the script using the .csv files to
generate the patches.

f875a832d2 MIPS: Abstract CPU core & VP(E) ID access through accessor functions
We currently have fields in struct cpuinfo_mips for the core & VP(E) ID
of a particular CPU, and various pieces of code directly access those
fields. This patch abstracts such access by introducing accessor
functions cpu_core(), cpu_set_core(), cpu_vpe_id() & cpu_set_vpe_id()
and having code that needs to access these values call those functions
rather than directly accessing the struct cpuinfo_mips fields. This
prepares us for changes to the way in which those values are stored in
later patches.

The cpu_vpe_id() function is introduced even though we already had a
cpu_vpe_id() macro for a couple of reasons:

  1) It's more consistent with the core, and future cluster, accessors.

  2) It ensures a sensible return type without explicit casts.

  3) It's generally preferable to use functions rather than macros.

ff2c8252bf MIPS: SMP: Constify smp ops
smp_ops providers do not modify their ops structures, so they should be
made const for robustness. Since currently the MIPS kernel is not mapped
with memory protection, this does not in itself provide any security
benefit, but it still makes sense to make this change.

There are also slight code size efficincies from the structure being
made read-only, saving 128 bytes of kernel text on a
pistachio_defconfig.

IRQ0 (HPET), IRQ1 (Keyboard), IRQ2 (Cascade), IRQ7 (SCI), IRQ8 (RTC)
and IRQ12 (Mouse) are handled by core-0 locally. Other PCI IRQs (3, 4,
5, 6, 14, 15) are balanced by all cores from Node-0. This can improve
I/O performance significantly.

e4db9253d6 MIPS: clockevent drivers: Set ->min_delta_ticks and ->max_delta_ticks
In preparation for making the clockevents core NTP correction aware,
all clockevent device drivers must set ->min_delta_ticks and
->max_delta_ticks rather than ->min_delta_ns and ->max_delta_ns: a
clockevent device's rate is going to change dynamically and thus, the
ratio of ns to ticks ceases to stay invariant.

Make the MIPS arch's clockevent drivers initialize these fields properly.

This patch alone doesn't introduce any change in functionality as the
clockevents core still looks exclusively at the (untouched) ->min_delta_ns
and ->max_delta_ns. As soon as this has changed, a followup patch will
purge the initialization of ->min_delta_ns and ->max_delta_ns from these
drivers.

likely a combination of multiple patches in that branch.

26dd3e4ff9 MIPS: Audit and remove any unnecessary uses of module.h
Historically a lot of these existed because we did not have
a distinction between what was modular code and what was providing
support to modules via EXPORT_SYMBOL and friends.  That changed
when we forked out support for the latter into the export.h file.

This means we should be able to reduce the usage of module.h
in code that is obj-y Makefile or bool Kconfig.  In the case of
some code where it is modular, we can extend that to also include
files that are building basic support functionality but not related
to loading or registering the final module; such files also have
no need whatsoever for module.h

The advantage in removing such instances is that module.h itself
sources about 15 other headers; adding significantly to what we feed
cpp, and it can obscure what headers we are effectively using.

Since module.h might have been the implicit source for init.h
(for __init) and for export.h (for EXPORT_SYMBOL) we consider each
instance for the presence of either and replace/add as needed.

Also note that MODULE_DEVICE_TABLE is a no-op for non-modular code.

Build coverage of all the mips defconfigs revealed the module.h
header was masking a couple of implicit include instances, so
we add the appropriate headers there.

When a CPU is disabled via CPU hotplug, cpu_foreign_map is not updated.
This could result in cache management SMP calls being sent to offline
CPUs instead of online siblings in the same core.

Add a call to calculate_cpu_foreign_map() in the various MIPS cpu
disable callbacks after set_cpu_online(). All cases are updated for
consistency and to keep cpu_foreign_map strictly up to date, not just
those which may support hardware multithreading.

3ef0665398 MIPS: hpet: Increase HPET_MIN_PROG_DELTA and decrease HPET_MIN_CYCLES
At first, we prefer to use mips clockevent device, so we decrease the
rating of hpet clockevent device.

For hpet, if HPET_MIN_PROG_DELTA (minimum delta of hpet programming) is
too small and HPET_MIN_CYCLES (threshold of -ETIME checking) is too
large, then hpet_next_event() can easily return -ETIME. After commit
c6eb3f70d4 ("hrtimer: Get rid of hrtimer softirq") this will cause
a RCU stall.

So, HPET_MIN_PROG_DELTA must be sufficient that we don't re-trip the
-ETIME check -- if we do, we will return -ETIME, forward the next event
time, try to set it, return -ETIME again, and basically lock the system
up. Meanwhile, HPET_MIN_CYCLES doesn't need to be too large, 16 cycles
is enough.

This solution is similar to commit f9eccf2461 ("clocksource/drivers
/vt8500: Increase the minimum delta").

By the way, this patch ensures hpet count/compare to be 32-bit long.

Due to datasheet, reserving 0xff800000~0xffffffff (8MB below 4GB) is
not enough for RS780E integrated GPU's TOM (top of memory) registers
and MSI/MSI-x memory region, so we reserve 0xfe000000~0xffffffff (32MB
below 4GB).

5610b1254e MIPS: hpet: Choose a safe value for the ETIME check
This patch is borrowed from x86 hpet driver and explaind below:

Due to the overly intelligent design of HPETs, we need to workaround
the problem that the compare value which we write is already behind
the actual counter value at the point where the value hits the real
compare register. This happens for two reasons:

1) We read out the counter, add the delta and write the result to the
   compare register. When a NMI hits between the read out and the write
   then the counter can be ahead of the event already.

2) The write to the compare register is delayed by up to two HPET
   cycles in AMD chipsets.

We can work around this by reading back the compare register to make
sure that the written value has hit the hardware. But that is bad
performance wise for the normal case where the event is far enough in
the future.

As we already know that the write can be delayed by up to two cycles
we can avoid the read back of the compare register completely if we
make the decision whether the delta has elapsed already or not based
on the following calculation:

  cmp = event - actual_count;

If cmp is less than 64 HPET clock cycles, then we decide that the event
has happened already and return -ETIME. That covers the above #1 and #2
problems which would cause a wait for HPET wraparound (~306 seconds).

5754843225 MIPS: Loongson-3: Fix SMP_ASK_C0COUNT IPI handler
When Core-0 handle SMP_ASK_C0COUNT IPI, we should make other cores to
see the result as soon as possible (especially when Store-Fill-Buffer
is enabled). Otherwise, C0_Count syncronization makes no sense.

BTW, array is more suitable than per-cpu variable for syncronization,
and there is a corner case should be avoid: C0_Count of Core-0 can be
really 0.

Migrate loongson driver to the new 'set-state' interface provided by
clockevents core, the earlier 'set-mode' interface is marked obsolete
now.

This also enables us to implement callbacks for new states of clockevent
devices, for example: ONESHOT_STOPPED.

4ace6139bf MIPS: SMP: Don't increment irq_count multiple times for call function IPIs
The majority of SMP platforms handle their IPIs through do_IRQ()
which calls irq_{enter/exit}(). When a call function IPI is received,
smp_call_function_interrupt() is called which also calls
irq_{enter,exit}(), meaning irq_count is raised twice.

When tick broadcasting is used (which is implemented via a call
function IPI), this incorrectly causes all CPU idle time on the core
receiving broadcast ticks to be accounted as time spent servicing
IRQs, as account_process_tick() will account as such if irq_count is
greater than 1. This results in 100% CPU usage being reported on a
core which receives its ticks via broadcast.

This patch removes the SMP smp_call_function_interrupt() wrapper which
calls irq_{enter,exit}(). Platforms which handle their IPIs through
do_IRQ() now call generic_smp_call_function_interrupt() directly to
avoid incrementing irq_count a second time. Platforms which don't
(loongson, sgi-ip27, sibyte) call generic_smp_call_function_interrupt()
wrapped in irq_{enter,exit}().

30ad29bb48 MIPS: Loongson: Naming style cleanup and rework
Currently, code of Loongson-2/3 is under loongson directory and code of
Loongson-1 is under loongson1 directory. Besides, there are Kconfig
options such as MACH_LOONGSON and MACH_LOONGSON1. This naming style is
very ugly and confusing. Since Loongson-2/3 are both 64-bit general-
purpose CPU while Loongson-1 is 32-bit SoC, we rename both file names
and Kconfig symbols from loongson/loongson1 to loongson64/loongson32.
