The resource path as displayed by the ipr driver is the
location string identifying a location on the SAS fabric.

e7f4fed5fa [SCSI] bnx2fc: Map the doorbell register between offload and enable requests
We used to map doorbell register after FW enable request is complete.
This causes a race condition when unsolicited event is received, and FW
sends a CQE for it. Since the doorbell is not mapped, driver does not
arm CQ, which means FW will not notify the driver for further CQ
completions.  To resolve this, map the doorbell between offload and
enable, so that driver is ready to receive the unsolicited packets and
arm the CQ as soon as FW enable is performed.

50b7186f64 [SCSI] bnx2fc: support software fcoe target
Software FCoE target always advertises RETRY bit even when there are no
tape LUNs behind the target. This causes the driver to enable FW support
for sequence level error recovery and perform REC/SRR. This patch
arrests the behavior by not enabling SLER feature for this target.

ioc->diag_trigger_mpi is an SL_WH_MPI_TRIGGERS_T struct.

There is a cut and paste error here and SL_WH_EVENT_TRIGGERS_T is used
instead of SL_WH_MPI_TRIGGERS_T.  Since the SL_WH_EVENT_TRIGGERS_T is
smaller than SL_WH_MPI_TRIGGERS_T, it means we only clear part of the
buffer.

I've changed it to use sizeof(ioc->diag_trigger_mpi) which is a bit
simpler.

3dd4849c25 [SCSI] qla4xxx: Fix memory corruption issue in qla4xxx_get_ep_fwdb.
In qla4xxx_get_ep_fwdb(), dst_addr is of type struct sockaddr.
We are copying sizeof(struct sockaddr_in6) bytes to dst_addr
which is 12 bytes larger. This will cause memory corruption.
So we change dst_addr to struct sockaddr_storage which is
of 128 byte, large enough to hold sizeof(struct sockaddr_in6).

5c19b92ae8 [SCSI] qla4xxx: Fix MBOX intr switching from polling to intr mode for ISP83XX
Issue:
Mailbox command timed out after switching from polling mode to interrupt mode.

Events:-
 1. Mailbox interrupts are disabled
 2. FW generates AEN and at same time driver enables Mailbox Interrupt
 3. Driver issues new mailbox to Firmware

In above case driver will not get AEN interrupts generated by FW in step #2 as
FW generated this AEN when interrupts are disabled. During the same time
driver enabled the mailbox interrupt, so driver will not poll for interrupt.
Driver will never process AENs generated in step #2 and issues new mailbox to
FW, but now FW is not able to post mailbox completion as AENs generated before
are not processed by driver.

Fix:
Enable Mailbox / AEN interrupts before initializing FW in case of ISP83XX.
This will make sure we process all Mailbox and AENs in interrupt mode.

8108de9739 [SCSI] gdth: Remove buggy ROM handling
The ROM address handling in gdth_init_pci() is useless and possibly
dangerous.  This patch removes it.

"pci_resource_start(pdev, 8)" is not well-defined.  PCI resources 0-5 are
standard PCI BARs and 6 is the expansion ROM.  Resource 8 is either an
SR-IOV BAR (if CONFIG_PCI_IOV=y, resources 7-12 are SR-IOV BARs) or a
bridge window (resources 7-10).

The GDT device is neither an SR-IOV device nor a bridge, so in either case
resource 8 should be zero since struct pci_dev is allocated with kzalloc().

It is illegal for a driver to write an arbitrary address to the ROM BAR
because it has no way of knowing whether the ROM will conflict with another
device.

I think the only effect of the code being removed was to:

  1) Enable the ROM at 0xFEFF0000 (possibly causing a conflict with
     another device)
  2) Delay one millisecond
  3) Write zero to the ROM BAR, disabling it

I doubt the delay is needed, but I left it since it seems innocuous.

Based on feedback and discussion with Neil Horman it seems that the above patch
may have a case where the fcoe_vport_destroy() and fcoe_destroy_work() can
race; hence that patch has been withdrawn with this patch that is trying to
solve the same problem in a different way.

In the current approach instead of removing the fcoe_config_mutex from the
vport_delete callback function; I've chosen to delete all the NPIV ports first
on a given root lport before continuing with the removal of the root lport.

f9184df3b9 fcoe: close race on link speed detection in fcoe code
When creating an fcoe interfce, we call fcoe_link_speed_update before we add the
lports fcoe interface to the fc_hostlist.  Since network device events like
NETDEV_CHANGE are only processed if an fcoe interface is found with an
underlying netdev that matches the netdev of the event.  Since this processing
in fcoe_device_notification is how link_speed changes get communicated to the
libfc  code (via fcoe_link_speed_update), we have a race condition - if a
NETDEV_CHANGE event is sent after the call to fcoe_link_speed_update in
fcoe_netdev_config, but before we add the interface to the fc_hostlist, we will
loose the event and attributes like /sys/class/fc_host/hostX/speed will not get
updated properly.

Fix this by moving the add to the fc_hostlist above the serialized call to
fcoe_netdev_config, ensuring that we catch netdev envents before we make a
direct call to fcoe_link_speed_update.

Also use this opportunity to clean up access to the fc_hostlist a bit by
creating a fcoe_hostlist_del accessor and replacing the cleanup in fcoe_exit to
use it properly.

Tested by myself successfully

6f4c827e68 [libata] scsi: no poll when ODD is powered off
When the ODD is powered off, any action the user did to the ODD that
would generate a media event will trigger an ACPI interrupt, so the
poll for media event is no longer necessary. And the poll will also
cause a runtime status change, which will stop the ODD from staying in
powered off state, so the poll should better be stopped.

But since we don't have access to the gendisk structure in LLDs, here
comes the disk_events_disable_depth for scsi device. This field is a
hint set by LLDs to convey information to upper layer drivers. A value
of 0 means media poll is necessary for the device, while values above 0
means media poll is not needed and should better be skipped. So we can
increase its value when we are to power off the ODD in ATA layer and

6c7f1e2f12 [SCSI] sr: support runtime pm
This patch adds runtime pm support for sr.

It did this by increasing the runtime usage_count of the device when
its block device is accessed. And decreasing the runtime usage_count
of the device when the access is done.

If there is media inside, runtime suspend is not allowed as we don't
always know if the ODD is being used or not.

5be78ee924 RDMA/cxgb4: Fix LE hash collision bug for active open connection
It enables establishing active open connection using fw_ofld_connection work
request when cpl_act_open_rpl says TCAM full error which may be because
of LE hash collision. Current support is only for IPv4 active open connections.

Sets ntuple bits in active open requests. For T4 firmware greater than 1.4.10.0
ntuple bits are required to be set.

Adds nocong and enable_ecn module parameter options.

This renames all random32 functions to have 'prandom_' prefix as follows:

The purpose of this renaming is to prevent some kernel developers from
assuming that prandom32() and random32() might imply that only
prandom32() was the one using a pseudo-random number generator by
prandom32's "p", and the result may be a very embarassing security
exposure.  This concern was expressed by Theodore Ts'o.

And furthermore, I'm going to introduce new functions for getting the
requested number of pseudo-random bytes.  If I continue to use both
prandom32 and random32 prefixes for these functions, the confusion
is getting worse.

As a result of this renaming, "prandom_" is the common prefix for
pseudo-random number library.

Currently, srandom32() and random32() are preserved because it is
difficult to rename too many users at once.

6a891b071b libfcoe, fcoe, bnx2fc: Add new fcoe control interface
This patch does a few things.

1) Makes /sys/bus/fcoe/ctlr_{create,destroy} interfaces.
   These interfaces take an <ifname> and will either
   create an FCoE Controller or destroy an FCoE
   Controller depending on which file is written to.

   The new FCoE Controller will start in a DISABLED
   state and will not do discovery or login until it
   is ENABLED. This pause will allow us to configure
   the FCoE Controller before enabling it.

2) Makes the 'mode' attribute of a fcoe_ctlr_device
   writale. This allows the user to configure the mode
   in which the FCoE Controller will start in when it
   is ENABLED.

   Possible modes are 'Fabric', or 'VN2VN'.

   The default mode for a fcoe_ctlr{,_device} is 'Fabric'.
   Drivers must implement the set_fcoe_ctlr_mode routine
   to support this feature.

   libfcoe offers an exported routine to set a FCoE
   Controller's mode. The mode can only be changed
   when the FCoE Controller is DISABLED.

   This patch also removes the get_fcoe_ctlr_mode pointer
   in the fcoe_sysfs function template, the code in
   fcoe_ctlr.c to get the mode and the assignment of
   the fcoe_sysfs function pointer to the fcoe_ctlr.c
   implementation (in fcoe and bnx2fc). fcoe_sysfs can
   return that value for the mode without consulting the
   LLD.

3) Make a 'enabled' attribute of a fcoe_ctlr_device. On a
   read, fcoe_sysfs will return the attribute's value. On
   a write, fcoe_sysfs will call the LLD (if there is a
   callback) to notifiy that the enalbed state has changed.

This patch maintains the old FCoE control interfaces as
module parameters, but it adds comments pointing out that
the old interfaces are deprecated.

QUEUE_FLAG_DEAD is used to indicate that queuing new requests must
stop. After this flag has been set queue draining starts. However,
during the queue draining phase it is still safe to invoke the
queue's request_fn, so QUEUE_FLAG_DYING is a better name for this
flag.

ef60f67434 libfcoe: Save some memory and optimize name lookups
Instead of creating a structure with an enum and a pointer
to a string, simply allocate an array of strings and use
the enum values for the indicies.

This means that we do not need to iterate through the list
of entries when looking up a string name by its enum key.

This will also help with a latter patch that will add
more fcoe_sysfs attributes that will also use the
fcoe_enum_name_search macro. One attribute will also do
a reverse lookup which requires less code when the
enum-to-string mappings are organized as this patch makes
them to be.

5b97fabdc8 libfc: fix REC handling
Currently fc_fcp_timeout doesn't check FC_RP_FLAGS_REC_SUPPORTED
flag first, this prevents REC request ever going out at all
to the target having REC support. So this patches fixes the
fc_fcp_timeout by checking FC_RP_FLAGS_REC_SUPPORTED flag first.

The changed order won't cause any issue during clearing
FC_RP_FLAGS_REC_SUPPORTED on failed IO with target not supporting
FC_RP_FLAGS_REC_SUPPORTED, since retry on failed IO would succeed.

c1d390d8e6 megaraid: fix BUG_ON() from incorrect use of delayed work
megaraid use INIT_WORK to declare a hotplug_work, but cast the
hotplug_work from work_struct to delayed_work and
schedule_delayed_work on it.  This is very dangerous, as other part of
delayed_work might be kernel memories allocated by others.

With commit 8852aac ("workqueue: mod_delayed_work_on() shouldn't queue
timer on 0 delay"), schedule_delayed_work() will check dwork->timer
before queue_work even when @delay is 0, this causes megaraid code to
hit the BUG_ON() in workqueue code.  Change megaraid code to use
delayed work.

Eliminate the private_rport_attrs[] array and the SETUP_*() macros
used to set up that array since the information in that array
duplicates the information in the static device attributes. Also,

ac9be30e91 srp_transport: Fix attribute registration
Register transport attributes after the attribute array has been set
up instead of before.  The current code can trigger a race condition
because the code reading the attribute array can run on another thread
than the code that initialized that array.  Make sure that any code
reading the attribute array will see all values written into that
array.

a394aac885 [SCSI] qla2xxx: Test and clear FCPORT_UPDATE_NEEDED atomically.
When the qla2xxx driver loses access to multiple, remote ports, there is a race
condition which can occur which will keep the request stuck on a scsi request
queue indefinitely.

This bad state occurred do to a race condition with how the FCPORT_UPDATE_NEEDED
bit is set in qla2x00_schedule_rport_del(), and how it is cleared in
qla2x00_do_dpc().  The problem port has its drport pointer set, but it has never
been processed by the driver to inform the fc transport that the port has been
lost.  qla2x00_schedule_rport_del() sets drport, and then sets the
FCPORT_UPDATE_NEEDED bit.  In qla2x00_do_dpc(), the port lists are walked and
any drport pointer is handled and the fc transport informed of the port loss,
then the FCPORT_UPDATE_NEEDED bit is cleared.  This leaves a race where the
dpc thread is processing one port removal, another port removal is marked
with a call to qla2x00_schedule_rport_del(), and the dpc thread clears the
bit for both removals, even though only the first removal was actually
handled.  Until another event occurs to set FCPORT_UPDATE_NEEDED, the later
port removal is never finished and qla2xxx stays in a bad state which causes
requests to become stuck on request queues.

This patch updates the driver to test and clear FCPORT_UPDATE_NEEDED
atomically.  This ensures the port state changes are processed and not lost.

Update sd driver to use the callbacks defined in dev_pm_ops.

sd_freeze is NULL, the bus level callback has taken care of quiescing
the device so there should be nothing needs to be done here.
Consequently, sd_thaw is not needed here either.

suspend, poweroff and runtime suspend share the same routine sd_suspend,
which will sync flush and then stop the drive, this is the same as before.

resume, restore and runtime resume share the same routine sd_resume,
which will start the drive by putting it into active power state, this
is also the same as before.

80d2fd48cc [SCSI] scsi_pm: use callbacks from dev_pm_ops for scsi devices
Use of pm_message_t is deprecated and device driver is not supposed
to use that. This patch migrates the SCSI bus level pm callbacks
to call device's pm callbacks defined in its driver's dev_pm_ops.

This is achieved by finding out which device pm callback should be used
in bus callback function, and then pass that callback function pointer
as a param to the scsi_bus_{suspend,resume}_common routine, which will
further pass that callback to scsi_dev_type_{suspend,resume} after
proper handling.

The special case for freeze in scsi_bus_suspend_common is not necessary
since there is no high level SCSI driver has implemented freeze, so no
need to runtime resume the device if it is in runtime suspended state
for system freeze, just return like the system suspend/hibernate case.

Since only sd has implemented drv->suspend/drv->resume, and I'll update
sd driver to use the new callbacks in the following patch, there is no
need to fallback to call drv->suspend/drv->resume if dev_pm_ops is NULL.

a01475637c [SCSI] sd: put to stopped power state when runtime suspend
When device is runtime suspended, put it to stopped power state to save
some power.

This will also make the behaviour consistent with what the scsi_pm.c
thinks about sd as the comment says:
sd treats runtime suspend, system suspend and system hibernate identical.
With this patch, it is now identical.
And sd_shutdown will also do nothing when it finds the device has been
runtime suspended, if we do not spin down the disk in runtime suspend
by putting it into stopped power state, the disk will be shut down
incorrectly.
And the the same problem can be solved for runtime power off after
runtime suspended case by this change.

With the current runtime scheme for disk, it will only be runtime
suspended when no process opens the disk, so this shouldn't happen a
lot, which makes it acceptable to spin down the disk when runtime
suspended. If some day a more aggressive runtime scheme is used, like
the 'request based runtime pm for disk' that Alan Stern and Lin Ming
has been working, we can introduce some policy to control this. But for
now, make it simple and correct by spinning down the disk.

The macro bit(n) is defined as ((u32)1 << n), and thus it doesn't work
with n >= 32, such as in mvs_94xx_assign_reg_set():

	if (i >= 32) {
		mvi->sata_reg_set |= bit(i);
		...
	}

The shift ((u32)1 << n) with n >= 32 also leads to undefined behavior.
The result varies depending on the architecture.

This patch changes bit(n) to do a 64-bit shift.  It also simplifies
mv_ffc64() using __ffs64(), since invoking ffz() with ~0 is undefined.

Fixed boot from san failure when SLI4 FC device presented on the same PCI bus

The request_firmware interface can induce delays while looking
for firmware files, even if no fw file is present. In some situations
the delays exceeded scan_wait timeouts, resulting in situations in which
the boot device had not been discovered in time.  Boot Device does not
need to be on a lpfc device.

Change request_firmware use to be module paramater driven. Default is to
not attempt firmware download on boot. Add sysfs parameter to invoke
firmware update.

72fb46a9d5 [SCSI] be2iscsi: Fix kernel panic in blk_iopoll disable mode.
Kernel used to panic while running IO is disable mode, as there
was an issue with getting the correct EQ on which completion has come.
Fix done is create workqueue per hba and work item for each EQ created.

51976a8c85 [SCSI] osd_uld: Add osdname & systemid sysfs at scsi_osd class
This patch adds the support for the following two read-only sysfs attributes
to scsi_osd class members : osdname & systemid

These attributes will show up as below in sysfs class hierarchy:
/sys/class/scsi_osd/osdX/osdname
/sys/class/scsi_osd/osdX/systemid

The osdname & systemid are OSD device attributes which uniquely
identify a device on the network, while it's IP and certainly
it's /dev/osdX device path might change.
Userspace utilities (e.g. mkfs.exofs) can parse these attributes to
identify the correct OSD in safer and faster way.

(Today osd apps open each device in the system and send a
 attributes query for these, in order to access the user
 requested device)

afd5e34b2b [SCSI] sd: Reshuffle init_sd to avoid crash
scsi_register_driver will register a prep_fn() function, which
in turn migh need to use the sd_cdp_pool for DIF.
Which hasn't been initialised at this point, leading to
a crash. So reshuffle the init_sd() and exit_sd() paths
to have the driver registered last.

5db44863b6 [SCSI] sd: Implement support for WRITE SAME
Implement support for WRITE SAME(10) and WRITE SAME(16) in the SCSI disk
driver.

 - We set the default maximum to 0xFFFF because there are several
   devices out there that only support two-byte block counts even with
   WRITE SAME(16). We only enable transfers bigger than 0xFFFF if the
   device explicitly reports MAXIMUM WRITE SAME LENGTH in the BLOCK
   LIMITS VPD.

 - max_write_same_blocks can be overriden per-device basis in sysfs.

 - The UNMAP discovery heuristics remain unchanged but the discard
   limits are tweaked to match the "real" WRITE SAME commands.

 - In the error handling logic we now distinguish between WRITE SAME
   with and without UNMAP set.

The discovery process heuristics are:

 - If the device reports a SCSI level of SPC-3 or greater we'll issue
   READ SUPPORTED OPERATION CODES to find out whether WRITE SAME(16) is
   supported. If that's the case we will use it.

 - If the device supports the block limits VPD and reports a MAXIMUM
   WRITE SAME LENGTH bigger than 0xFFFF we will use WRITE SAME(16).

 - Otherwise we will use WRITE SAME(10) unless the target LBA is beyond
   0xFFFFFFFF or the block count exceeds 0xFFFF.

 - no_write_same is set for ATA, FireWire and USB.

3c6bdaeab4 [SCSI] Add a report opcode helper
The REPORT SUPPORTED OPERATION CODES command can be used to query
whether a given opcode is supported by a device. Add a helper function
that allows us to look up commands.

We only issue RSOC if the device reports compliance with SPC-3 or
later. But to err on the side of caution we disable the command for ATA,
FireWire and USB.

49bd665c54 [SCSI] isci: copy fis 0x34 response into proper buffer
SATA MICROCODE DOWNALOAD fails on isci driver. After receiving Register
Device to Host (FIS 0x34) frame Initiator resets phy.
In the frame handler routine response (FIS 0x34) was copied into wrong
buffer and upper layer did not receive any answer which resulted in
timeout and reset.
This patch corrects this bug.
