0f48f26009 block: fix mismerge of the DISK_EVENT_MEDIA_CHANGE removal
Jens' back-merge commit 698567f3fa ("Merge commit 'v2.6.39' into
for-2.6.40/core") was incorrectly done, and re-introduced the
DISK_EVENT_MEDIA_CHANGE lines that had been removed earlier in commits

 - 9fd097b149 ("block: unexport DISK_EVENT_MEDIA_CHANGE for
   legacy/fringe drivers")

 - 7eec77a181 ("ide: unexport DISK_EVENT_MEDIA_CHANGE for ide-gd
   and ide-cd")

because of conflicts with the "g->flags" updates near-by by commit
d4dc210f69 ("block: don't block events on excl write for non-optical
devices")

As a result, we re-introduced the hanging behavior due to infinite disk
media change reports.

Tssk, tssk, people! Don't do back-merges at all, and *definitely* don't
do them to hide merge conflicts from me - especially as I'm likely
better at merging them than you are, since I do so many merges.

698567f3fa Merge commit 'v2.6.39' into for-2.6.40/core
Since for-2.6.40/core was forked off the 2.6.39 devel tree, we've
had churn in the core area that makes it difficult to handle
patches for eg cfq or blk-throttle. Instead of requiring that they
be based in older versions with bugs that have been fixed later
in the rc cycle, merge in 2.6.39 final.

Also fixes up conflicts in the below files.

bf2253a6f0 cdrom: always check_disk_change() on open
cdrom_open() called check_disk_change() after the rest of open path
succeeded which leads to the following bizarre behavior.

* After media change, if the device opened without O_NONBLOCK,
  open_for_data() naturally fails with -ENOMEDIA and
  check_disk_change() is never called.  The media is known to be gone
  and the open failure makes it obvious to the userland but device
  invalidation never happens.

* But if the device is opened with O_NONBLOCK, all the checks are
  bypassed and cdrom_open() doesn't notice that the media is not there
  and check_disk_change() is called and invalidation happens.

There's nothing to be gained by avoiding calling check_disk_change()
on open failure.  Common cases end up calling check_disk_change()
anyway.  All we get is inconsistent behavior.

Fix it by moving check_disk_change() invocation to the top of
cdrom_open() so that it always gets called regardless of how the rest
of open proceeds.

9fd097b149 block: unexport DISK_EVENT_MEDIA_CHANGE for legacy/fringe drivers
In-kernel disk event polling doesn't matter for legacy/fringe drivers
and may lead to infinite event loop if ->check_events() implementation
generates events on level condition instead of edge.

Now that block layer supports suppressing exporting unlisted events,
simply leaving disk->events cleared allows these drivers to keep the
internal revalidation behavior intact while avoiding weird
interactions with userland event handler.

d4dc210f69 block: don't block events on excl write for non-optical devices
Disk event code automatically blocks events on excl write.  This is
primarily to avoid issuing polling commands while burning is in
progress.  This behavior doesn't fit other types of devices with
removeable media where polling commands don't have adverse side
effects and door locking usually doesn't exist.

This patch introduces new genhd flag which controls the auto-blocking
behavior and uses it to enable auto-blocking only on optical devices.

Note for stable: 2.6.38 and later only

b8cf0e0e55 cdrom: support devices that have check_events but not media_changed
Commit 93aae17af1 ("sr: implement
sr_check_events()") replaced the media_changed op with the
check_events op in drivers/scsi/sr.c

All users that check for the CDC_MEDIA_CHANGED capbility try both
the check_events op and the media_changed op, but register_cdrom()
was requiring media_changed.

This patch fixes the capability checking.

The cdrom_select_disc ioctl is also using the two operations, so
they should be required for CDC_SELECT_DISC too.

2d9217296b cdrom: add ->check_events() support
In principle, cdrom just needs to pass through ->check_events() but
CDROM_MEDIA_CHANGED ioctl makes things a bit more complex.  Just as
with ->media_changed() support, cdrom code needs to buffer the events
and serve them to ioctl and vfs as requested.

As the code has to deal with both ->check_events() and
->media_changed(), and vfs and ioctl event buffering, this patch adds
check_events caching on top of the existing cdi->mc_flags buffering.

It may be a good idea to deprecate CDROM_MEDIA_CHANGED ioctl and
remove all this mess.

6e9624b8ca block: push down BKL into .open and .release
The open and release block_device_operations are currently
called with the BKL held. In order to change that, we must
first make sure that all drivers that currently rely
on this have no regressions.

This blindly pushes the BKL into all .open and .release
operations for all block drivers to prepare for the
next step. The drivers can subsequently replace the BKL
with their own locks or remove it completely when it can
be shown that it is not needed.

The functions blkdev_get and blkdev_put are the only
remaining users of the big kernel lock in the block
layer, besides a few uses in the ioctl code, none
of which need to serialize with blkdev_{get,put}.

Most of these two functions is also under the protection
of bdev->bd_mutex, including the actual calls to
->open and ->release, and the common code does not
access any global data structures that need the BKL.

086fa5ff08 block: Rename blk_queue_max_sectors to blk_queue_max_hw_sectors
The block layer calling convention is blk_queue_<limit name>.
blk_queue_max_sectors predates this practice, leading to some confusion.
Rename the function to appropriately reflect that its intended use is to
set max_hw_sectors.

Also introduce a temporary wrapper for backwards compability.  This can
be removed after the merge window is closed.

e1defc4ff0 block: Do away with the notion of hardsect_size
Until now we have had a 1:1 mapping between storage device physical
block size and the logical block sized used when addressing the device.
With SATA 4KB drives coming out that will no longer be the case.  The
sector size will be 4KB but the logical block size will remain
512-bytes.  Hence we need to distinguish between the physical block size
and the logical ditto.

This patch renames hardsect_size to logical_block_size.

9934c8c045 block: implement and enforce request peek/start/fetch
Till now block layer allowed two separate modes of request execution.
A request is always acquired from the request queue via
elv_next_request().  After that, drivers are free to either dequeue it
or process it without dequeueing.  Dequeue allows elv_next_request()
to return the next request so that multiple requests can be in flight.

Executing requests without dequeueing has its merits mostly in
allowing drivers for simpler devices which can't do sg to deal with
segments only without considering request boundary.  However, the
benefit this brings is dubious and declining while the cost of the API
ambiguity is increasing.  Segment based drivers are usually for very
old or limited devices and as converting to dequeueing model isn't
difficult, it doesn't justify the API overhead it puts on block layer
and its more modern users.

Previous patches converted all block low level drivers to dequeueing
model.  This patch completes the API transition by...

* renaming elv_next_request() to blk_peek_request()

* renaming blkdev_dequeue_request() to blk_start_request()

* adding blk_fetch_request() which is combination of peek and start

* disallowing completion of queued (not started) requests

* applying new API to all LLDs

Renamings are for consistency and to break out of tree code so that
it's apparent that out of tree drivers need updating.

2343046826 gdrom: dequeue in-flight request
gdrom already dequeues and fully completes requests on normal path and
the error paths can be easily converted to do so too.  Clean it up and
dequeue requests on error paths too.

While at it remove superflous blk_fs_request() && !blk_rq_sectors()
condition check.

296b2f6ae6 block: convert to dequeueing model (easy ones)
plat-omap/mailbox, floppy, viocd, mspro_block, i2o_block and
mmc/card/queue are already pretty close to dequeueing model and can be
converted with simple changes.  Convert them.

While at it,

* xen-blkfront: !fs check moved downwards to share dequeue call with
  normal path.

* mspro_block: __blk_end_request(..., blk_rq_cur_byte()) converted to
  __blk_end_request_cur()

* mmc/card/queue: loop of __blk_end_request() converted to
  __blk_end_request_all()

83096ebf12 block: convert to pos and nr_sectors accessors
With recent cleanups, there is no place where low level driver
directly manipulates request fields.  This means that the 'hard'
request fields always equal the !hard fields.  Convert all
rq->sectors, nr_sectors and current_nr_sectors references to
accessors.

While at it, drop superflous blk_rq_pos() < 0 test in swim.c.

f06d9a2b52 block: replace end_request() with [__]blk_end_request_cur()
end_request() has been kept around for backward compatibility;
however, it's about time for it to go away.

* There aren't too many users left.

* Its use of @updtodate is pretty confusing.

* In some cases, newer code ends up using mixture of end_request() and
  [__]blk_end_request[_all](), which is way too confusing.

So, add [__]blk_end_request_cur() and replace end_request() with it.
Most conversions are straightforward.  Noteworthy ones are...

* paride/pcd: next_request() updated to take 0/-errno instead of 1/0.

* paride/pf: pf_end_request() and next_request() updated to take
  0/-errno instead of 1/0.

* xd: xd_readwrite() updated to return 0/-errno instead of 1/0.

* mtd/mtd_blkdevs: blktrans_discard_request() updated to return
  0/-errno instead of 1/0.  Unnecessary local variable res
  initialization removed from mtd_blktrans_thread().

40cbbb781d block: implement and use [__]blk_end_request_all()
There are many [__]blk_end_request() call sites which call it with
full request length and expect full completion.  Many of them ensure
that the request actually completes by doing BUG_ON() the return
value, which is awkward and error-prone.

This patch adds [__]blk_end_request_all() which takes @rq and @error
and fully completes the request.  BUG_ON() is added to to ensure that
this actually happens.

Most conversions are simple but there are a few noteworthy ones.

* cdrom/viocd: viocd_end_request() replaced with direct calls to
  __blk_end_request_all().

* s390/block/dasd: dasd_end_request() replaced with direct calls to
  __blk_end_request_all().

* s390/char/tape_block: tapeblock_end_request() replaced with direct
  calls to blk_end_request_all().

febd7a5c13 Commands needing to be retried require a complete re-initialization.
The test-unit-ready portion of this patch was causing boots to fail on
my test machine (as in http://lkml.org/lkml/2008/12/5/161). With this
patch in place, the system is booting reliably.

Mike Anderson found the same problem in the hp_hw_start_stop code,
and I applied the same solution in cdrom_read_cdda_bpc.

d4430d62fa [PATCH] beginning of methods conversion
To keep the size of changesets sane we split the switch by drivers;
to keep the damn thing bisectable we do the following:
	1) rename the affected methods, add ones with correct
prototypes, make (few) callers handle both.  That's this changeset.
	2) for each driver convert to new methods.  *ALL* drivers
are converted in this series.
	3) kill the old (renamed) methods.

Note that it _is_ a flagday; all in-tree drivers are converted and by the
end of this series no trace of old methods remain.  The only reason why
we do that this way is to keep the damn thing bisectable and allow per-driver
debugging if anything goes wrong.

New methods:
	open(bdev, mode)
	release(disk, mode)
	ioctl(bdev, mode, cmd, arg)		/* Called without BKL */
	compat_ioctl(bdev, mode, cmd, arg)

7afb3a6e75 gdrom: change to use __blk_end_request()
This patch converts gdrom to use __blk_end_request() directly
so that end_{queued|dequeued}_request() can be removed.

gd.transfer is '1' in error cases and '0' in non-error cases,
so gdrom hasn't been propagating any error code to the block layer.
We can just convert error cases to '-EIO'.

152e283fdf block: introduce struct rq_map_data to use reserved pages
This patch introduces struct rq_map_data to enable bio_copy_use_iov()
use reserved pages.

Currently, bio_copy_user_iov allocates bounce pages but
drivers/scsi/sg.c wants to allocate pages by itself and use
them. struct rq_map_data can be used to pass allocated pages to
bio_copy_user_iov.

The current users of bio_copy_user_iov simply passes NULL (they don't
want to use pre-allocated pages).
