Apart from public and internal header files, there is a third type called
local header files, which are located next to source files in the source
directory. Currently, they have different suffixes like
'*_lcl.h', '*_local.h', or '*_int.h'
This commit changes the different suffixes to '*_local.h' uniformly.

Currently, there are two different directories which contain internal
header files of libcrypto which are meant to be shared internally:
While header files in 'include/internal' are intended to be shared
between libcrypto and libssl, the files in 'crypto/include/internal'
are intended to be shared inside libcrypto only.
To make things complicated, the include search path is set up in such
a way that the directive #include "internal/file.h" could refer to
a file in either of these two directoroes. This makes it necessary
in some cases to add a '_int.h' suffix to some files to resolve this
ambiguity:
#include "internal/file.h" # located in 'include/internal'
#include "internal/file_int.h" # located in 'crypto/include/internal'
This commit moves the private crypto headers from
'crypto/include/internal' to 'include/crypto'
As a result, the include directives become unambiguous
#include "internal/file.h" # located in 'include/internal'
#include "crypto/file.h" # located in 'include/crypto'
hence the superfluous '_int.h' suffixes can be stripped.
The files 'store_int.h' and 'store.h' need to be treated specially;
they are joined into a single file.

The output C code was made to use ERR_func_error_string() to see if a
string table was already loaded or not. Since this function returns
NULL always, this check became useless.
Change it to use ERR_reason_error_string() instead, as there's no
reason to believe we will get rid of reason strings, ever.
To top it off, we rebuild all affected C sources.

A CAdES Basic Electronic Signature (CAdES-BES) contains, among other
specifications, a collection of Signing Certificate reference attributes,
stored in the signedData ether as ESS signing-certificate or as
ESS signing-certificate-v2. These are described in detail in Section 5.7.2
of RFC 5126 - CMS Advanced Electronic Signatures (CAdES).
This patch adds support for adding ESS signing-certificate[-v2] attributes
to CMS signedData. Although it implements only a small part of the RFC, it
is sufficient many cases to enable the `openssl cms` app to create signatures
which comply with legal requirements of some European States (e.g Italy).

The prevailing style seems to not have trailing whitespace, but a few
lines do. This is mostly in the perlasm files, but a few C files got
them after the reformat. This is the result of:
find . -name '*.pl' | xargs sed -E -i '' -e 's/( |'$'\t'')*$//'
find . -name '*.c' | xargs sed -E -i '' -e 's/( |'$'\t'')*$//'
find . -name '*.h' | xargs sed -E -i '' -e 's/( |'$'\t'')*$//'
Then bn_prime.h was excluded since this is a generated file.
Note mkerr.pl has some changes in a heredoc for some help output, but
other lines there lack trailing whitespace too.

Deprecate the function ASN1_STRING_data() and replace with a new function
ASN1_STRING_get0_data() which returns a constant pointer. Update library
to use new function.

TS_OBJ_print_bio() misuses OBJ_txt2obj: it should print the result
as a null terminated buffer. The length value returned is the total
length the complete text reprsentation would need not the amount of
data written.

The TS_RESP_verify_response() function is used for verifying the response
from a TSA. You can set the provided TS_VERIFY_CTX with different flags
depending on what aspects of the response you wish to verify.
A seg fault will occur if you supply the TS_VFY_SIGNER or TS_VFY_TSA_NAME
flags without also specifying TS_VFY_SIGNATURE.

This takes us away from the idea that we know exactly how our static
libraries are going to get used. Instead, we make them available to
build shareable things with, be it other shared libraries or DSOs.
On the other hand, we also have greater control of when the shared
library cflags. They will never be used with object files meant got
binaries, such as apps/openssl or test/test*.
With unified, we take this a bit further and prepare for having to
deal with extra cflags specifically to be used with DSOs (dynamic
engines), libraries and binaries (applications).

All those flags existed because we had all the dependencies versioned
in the repository, and wanted to have it be consistent, no matter what
the local configuration was. Now that the dependencies are gone from
the versioned Makefile.ins, it makes much more sense to use the exact
same flags as when compiling the object files.

INSTALL_PREFIX is a confusing name, as there's also --prefix.
Instead, tag along with the rest of the open source world and adopt
the Makefile variable DESTDIR to designate the desired staging
directory.
The Configure option --install_prefix is removed, the only way to
designate a staging directory is with the Makefile variable (this is
also implemented for VMS' descrip.mms et al).

Some time ago, we had a ex_libs configuration setting that could be
divided into lflags and ex_libs. These got divided in two settings,
lflags and ex_libs, and the former was interpreted to be general
linking flags.
Unfortunately, that conclusion wasn't entirely accurate. Most of
those linking were meant to end up in a very precise position on the
linking command line, just before the spec of libraries the linking
depends on.
Back to the drawing board, we're diving things further, now having
lflags, which are linking flags that aren't depending on command line
position, plib_lflags, which are linking flags that should show up just
before the spec of libraries to depend on, and finally ex_libs, which
is the spec of extra libraries to depend on.
Also, documentation is changed in Configurations/README. This was
previously forgotten.

place, we add build.info files. They have been generated from the
Makefiles in the same directories. Things that are platform specific
will appear in later commits.

The lflags configuration had a weird syntax with a % as separator. If
it was present, whatever came before ended up as PEX_LIBS in Makefile
(usually, this is LDFLAGS), while whatever came after ended up as
EX_LIBS.
This change splits that item into lflags and ex_libs, making their use
more explicit.
Also, PEX_LIBS in all the Makefiles are renamed to LDFLAGS.
