53e62fb5a4 btrfs: dev-replace: add explicit check for replace result "no error"
We recast the replace return status
BTRFS_IOCTL_DEV_REPLACE_RESULT_SCRUB_INPROGRESS to 0, to indicate no
error.
And since BTRFS_IOCTL_DEV_REPLACE_RESULT_NO_ERROR should also return 0,
which is also declared as 0, so we just return. Instead add it to the if
statement so that there is enough clarity while reading the code.

fe97e2e173 btrfs: dev-replace: replace's scrub must not be running in suspended state
When the replace state is in the suspended state, btrfs_scrub_cancel()
should fail with -ENOTCONN as there is no scrub running. As a safety
catch check if btrfs_scrub_cancel() returns -ENOTCONN and assert if it
doesn't.

b47dda2ef6 btrfs: dev-replace: set result code of cancel by status of scrub
The device-replace needs to check the result code of the scrub workers
in btrfs_dev_replace_cancel and distinguish if successful cancel
operation and when the there was no operation running.

If btrfs_scrub_cancel() fails, return
BTRFS_IOCTL_DEV_REPLACE_RESULT_NOT_STARTED so that user can try
to cancel the replace again.

d189dd70e2 btrfs: fix use-after-free due to race between replace start and cancel
The device replace cancel thread can race with the replace start thread
and if fs_info::scrubs_running is not yet set, btrfs_scrub_cancel() will
fail to stop the scrub thread.

The scrub thread continues with the scrub for replace which then will
try to write to the target device and which is already freed by the
cancel thread.

scrub_setup_ctx() warns as tgtdev is NULL.

Fix this by letting the btrfs_dev_replace_finishing() to do the job of
cleaning after the cancel, including freeing of the target device.
btrfs_dev_replace_finishing() is called when btrfs_scub_dev() returns
along with the scrub return status.

05c49e6bc1 btrfs: dev-replace: go back to suspend state if another EXCL_OP is running
In a secnario where balance and replace co-exists as below,

  - start balance
  - pause balance
  - start replace
  - reboot

and when system restarts, balance resumes first. Then the replace is
attempted to restart but will fail as the EXCL_OP lock is already held
by the balance. If so place the replace state back to
BTRFS_IOCTL_DEV_REPLACE_STATE_SUSPENDED state.

0d228ece59 btrfs: dev-replace: go back to suspended state if target device is missing
At the time of forced unmount we place the running replace to
BTRFS_IOCTL_DEV_REPLACE_STATE_SUSPENDED state, so when the system comes
back and expect the target device is missing.

Then let the replace state continue to be in
BTRFS_IOCTL_DEV_REPLACE_STATE_SUSPENDED state instead of
BTRFS_IOCTL_DEV_REPLACE_STATE_STARTED as there isn't any matching scrub
running as part of replace.

eede2bf34f Btrfs: prevent ioctls from interfering with a swap file
A later patch will implement swap file support for Btrfs, but before we
do that, we need to make sure that the various Btrfs ioctls cannot
change a swap file.

When a swap file is active, we must make sure that the extents of the
file are not moved and that they don't become shared. That means that
the following are not safe:

- chattr +c (enable compression)
- reflink
- dedupe
- snapshot
- defrag

Don't allow those to happen on an active swap file.

Additionally, balance, resize, device remove, and device replace are
also unsafe if they affect an active swapfile. Add a red-black tree of
block groups and devices which contain an active swapfile. Relocation
checks each block group against this tree and skips it or errors out for
balance or resize, respectively. Device remove and device replace check
the tree for the device they will operate on.

Note that we don't have to worry about chattr -C (disable nocow), which
we ignore for non-empty files, because an active swapfile must be
non-empty and can't be truncated. We also don't have to worry about
autodefrag because it's only done on COW files. Truncate and fallocate
are already taken care of by the generic code. Device add doesn't do
relocation so it's not an issue, either.

The replace_wait and bio_counter were mistakenly added to fs_info in

confusing in bare fs_info context.

aa144bfeaa btrfs: dev-replace: avoid useless lock on error handling path
The exit sequence in btrfs_dev_replace_start does not allow to simply
add a label to the right place so the error handling after starting
transaction failure jumps there. Currently there's a lock that pairs
with the unlock in the section, which is unnecessary and only raises
questions.  Add a variable to track the locking status and avoid the
extra locking.

7fb2eced10 btrfs: open code btrfs_dev_replace_clear_lock_blocking
There's a single caller and the function name does not say it's actually
taking the lock, so open coding makes it more explicit.

For now, btrfs_dev_replace_read_lock is used instead of read_lock so
it's paired with the unlocking wrapper in the same block.

3280f87457 btrfs: remove btrfs_dev_replace::read_locks
This member seems to be copied from the extent_buffer locking scheme and
is at least used to assert that the read lock/unlock is properly nested.
In some way. While the _inc/_dec are called inside the read lock
section, the asserts are both inside and outside, so the ordering is not
guaranteed and we can see read/inc/dec ordered in any way
(theoretically).

A missing call of btrfs_dev_replace_clear_lock_blocking could cause
unexpected read_locks count, so this at least looks like a valid
assertion, but this will become unnecessary with later updates.

5c06147128 btrfs: fix error handling in btrfs_dev_replace_start
When we fail to start a transaction in btrfs_dev_replace_start, we leave
dev_replace->replace_start set to STARTED but clear ->srcdev and
->tgtdev.  Later, that can result in an Oops in
btrfs_dev_replace_progress when having state set to STARTED or SUSPENDED
implies that ->srcdev is valid.

Also fix error handling when the state is already STARTED or SUSPENDED
while starting.  That, too, will clear ->srcdev and ->tgtdev even though
it doesn't own them.  This should be an impossible case to hit since we
should be protected by the BTRFS_FS_EXCL_OP bit being set.  Let's add an
ASSERT there while we're at it.

a27a94c2b0 btrfs: Make btrfs_find_device_by_devspec return btrfs_device directly
Instead of returning an error value and using one of the parameters for
returning the actual object we are interested in just refactor the
function to directly return btrfs_device *. Also bubble up the error
handling for the special BTRFS_ERROR_DEV_MISSING_NOT_FOUND value into
btrfs_rm_device. No functional changes.

1e7e1f9e3a btrfs: replace: Reset on-disk dev stats value after replace
on-disk devs stats value is updated in btrfs_run_dev_stats(),
which is called during commit transaction, if device->dev_stats_ccnt
is not zero.

Since current replace operation does not touch dev_stats_ccnt,
on-disk dev stats value is not updated. Therefore "btrfs device stats"
may return old device's value after umount/mount
(Example: See "btrfs ins dump-t -t DEV $DEV" after btrfs/100 finish).

Fix this by just incrementing dev_stats_ccnt in
btrfs_dev_replace_finishing() when replace is succeeded and this will
update the values.

b25e59e2b2 btrfs: drop uuid_mutex in btrfs_dev_replace_finishing
btrfs_dev_replace_finishing updates devices (soruce and target) which
are within the btrfs_fs_devices::devices or withint the cloned seed
devices (btrfs_fs_devices::seed::devices), so we don't need the global
uuid_mutex.

The device replace context is also locked by its own locks.

82b3e53b8d btrfs: Remove delayed_iput parameter of btrfs_start_delalloc_roots
This parameter was introduced alongside the function in
eb73c1b7ce ("Btrfs: introduce per-subvolume delalloc inode list") to
avoid deadlocks since this function was used in the transaction commit
path. However, commit 8d875f95da ("btrfs: disable strict file flushes
for renames and truncates") removed that usage, rendering the parameter
obsolete.

a0fecc2371 btrfs: remove wrong use of volume_mutex from btrfs_dev_replace_start
The volume mutex does not protect against anything in this case, the
comment about scrub is right but not related to locking and looks
confusing. The comment in btrfs_find_device_missing_or_by_path is wrong
and confusing too.

The device_list_mutex is not held here to protect device lookup, but in
this case device replace cannot run in parallel with device removal (due
to exclusive op protection), so we don't need further locking here.

010a47bde9 btrfs: add proper safety check before resuming dev-replace
The device replace is paused by unmount or read only remount, and
resumed on next mount or write remount.

The exclusive status should be checked properly as it's a global
invariant and we must not allow 2 operations run. In this case, the
balance can be also paused and resumed under same conditions. It's
always checked first so dev-replace could see the EXCL_OP already taken,
BUT, the ioctl would never let start both at the same time.

Replace the WARN_ON with message and return 0, indicating no error as
this is purely theoretical and the user will be informed. Resolving that
manually should be possible by waiting for the other operation to finish
or cancel the paused state.

f1b8a1e8c0 btrfs: simplify btrfs_dev_replace_kthread
This function prints an informative message and then continues
dev-replace. The message contains a progress percentage which is read
from the status. The status is allocated dynamically, about 2600 bytes,
just to read the single value. That's an overkill. We'll use the new
helper and drop the allocation.

6374e57ad8 btrfs: fix integer overflow in calc_reclaim_items_nr
Dave Jones hit a WARN_ON(nr < 0) in btrfs_wait_ordered_roots() with
v4.12-rc6.  This was because commit 70e7af244 made it possible for
calc_reclaim_items_nr() to return a negative number.  It's not really a
bug in that commit, it just didn't go far enough down the stack to find
all the possible 64->32 bit overflows.

This switches calc_reclaim_items_nr() to return a u64 and changes everyone
that uses the results of that math to u64 as well.

In btrfs_dev_replace_finishing(), we will call
btrfs_rm_dev_replace_blocked() to wait bios before destroying the target
device when scrub is finished normally.

However when dev-replace is aborted, either due to error or cancelled by
scrub, we didn't wait for bios, this can lead to use-after-free if there
are bios holding the target device.

Furthermore, for raid56 scrub, at least 2 places are calling
btrfs_map_sblock() without protection of bio_counter, leading to the
problem.

This patch fixes the problem:
1) Wait for bio_counter before freeing target device when canceling
   replace
2) When calling btrfs_map_sblock() for raid56, use bio_counter to
   protect the call.

171938e528 btrfs: track exclusive filesystem operation in flags
There are several operations, usually started from ioctls, that cannot
run concurrently. The status is tracked in
mutually_exclusive_operation_running as an atomic_t. We can easily track
the status as one of the per-filesystem flag bits with same
synchronization guarantees.

The conversion replaces:

* atomic_xchg(..., 1)    ->   test_and_set_bit(FLAG, ...)
* atomic_set(..., 0)     ->   clear_bit(FLAG, ...)

578def7c50 Btrfs: don't wait for unrelated IO to finish before relocation
Before the relocation process of a block group starts, it sets the block
group to readonly mode, then flushes all delalloc writes and then finally
it waits for all ordered extents to complete. This last step includes
waiting for ordered extents destinated at extents allocated in other block
groups, making us waste unecessary time.

So improve this by waiting only for ordered extents that fall into the
block group's range.

8ed01abe7d btrfs: s_bdev is not null after missing replace
Yauhen reported in the ML that s_bdev is null at mount, and
s_bdev gets updated to some device when missing device is
replaced, as because bdev is null for missing device, things
gets matched up. Fix this by checking if s_bdev is set. I
didn't want to completely remove updating s_bdev because
the future multi device support at vfs layer may need it.

However, with high memory pressure, tasks which hold dev_replace.lock can
be interrupted by kswapd and then kswapd is intended to release memory occupied

delayed_node->mutex may be acquired in __btrfs_release_delayed_node(), and it leads
to a ABBA deadlock.

To fix this, we can use "blocking rwlock" used in the case of extent_buffer, but
things are simpler here since we only needs read's spinlock to blocking lock.

With this, btrfs/011 no more produces warnings in dmesg.

58c4e17384 btrfs: scrub: use GFP_KERNEL on the submission path
Scrub is not on the critical writeback path we don't need to use
GFP_NOFS for all allocations. The failures are handled and stats passed
back to userspace.

Let's use GFP_KERNEL on the paths where everything is ok, ie. setup the
global structures and the IO submission paths.

Functions that do the repair and fixups still use GFP_NOFS as we might
want to skip any other filesystem activity if we encounter an error.
This could turn out to be unnecessary, but requires more review compared
to the easy cases in this patch.

To avoid deadlock described in commit 084b6e7c76 ("btrfs: Fix a
lockdep warning when running xfstest."), we should move kobj stuff out
of dev_replace lock range.

  "It is because the btrfs_kobj_{add/rm}_device() will call memory
  allocation with GFP_KERNEL,
  which may flush fs page cache to free space, waiting for it self to do
  the commit, causing the deadlock.

  To solve the problem, move btrfs_kobj_{add/rm}_device() out of the
  dev_replace lock range, also involing split the
  btrfs_rm_dev_replace_srcdev() function into remove and free parts.

  Now only btrfs_rm_dev_replace_remove_srcdev() is called in dev_replace
  lock range, and kobj_{add/rm} and btrfs_rm_dev_replace_free_srcdev() are
  called out of the lock range."

f8c269d722 btrfs: cleanup 64bit/32bit divs, compile time constants
Switch to div_u64 if the divisor is a numeric constant or sum of
sizeof()s. We can remove a few instances of do_div that has the hidden
semtantics of changing the 1st argument.

Small power-of-two divisors are converted to bitshifts, large values are
kept intact for clarity.

4245215d6a Btrfs, raid56: fix use-after-free problem in the final device replace procedure on raid56
The commit c404e0dc (Btrfs: fix use-after-free in the finishing
procedure of the device replace) fixed a use-after-free problem
which happened when removing the source device at the end of device
replace, but at that time, btrfs didn't support device replace
on raid56, so we didn't fix the problem on the raid56 profile.
Currently, we implemented device replace for raid56, so we need
kick that problem out before we enable that function for raid56.

The fix method is very simple, we just increase the bio per-cpu
counter before we submit a raid56 io, and decrease the counter
when the raid56 io ends.

device replace could fail due to another running scrub process or any
other errors btrfs_scrub_dev() may hit, but this failure doesn't get
returned to userspace.

					unlock_chunk()
					mutex_unlock(&device_list_mutex)
	check device

Destroying the target device if device replace fails also has the same problem.

We fix this problem by locking uuid_mutex during destroying source device or
target device, just like the device remove operation.

It is a temporary solution, we can fix this problem and make the code more
clear by atomic counter in the future.

2196d6e8a7 Btrfs: Fix misuse of chunk mutex
There were several problems about chunk mutex usage:
- Lock chunk mutex when updating metadata. It would cause the nested
  deadlock because updating metadata might need allocate new chunks
  that need acquire chunk mutex. We remove chunk mutex at this case,
  because b-tree lock and other lock mechanism can help us.
- ABBA deadlock occured between device_list_mutex and chunk_mutex.
  When we update device status, we must acquire device_list_mutex at the
  beginning, and then we might get chunk_mutex during the device status
  update because we need allocate new chunks for metadata COW. But at
  most place, we acquire chunk_mutex at first and then acquire device list
  mutex. We need change the lock order.
- Some place we needn't acquire chunk_mutex. For example we needn't get
  chunk_mutex when we free a empty seed fs_devices structure.

ce7213c70c Btrfs: fix wrong device bytes_used in the super block
device->bytes_used will be changed when allocating a new chunk, and
disk_total_size will be changed if resizing is successful.
Meanwhile, the on-disk super blocks of the previous transaction
might not be updated. Considering the consistency of the metadata
in the previous transaction, We should use the size in the previous
transaction to check if the super block is beyond the boundary
of the device.

Though it is not big problem because we don't use it now, but anyway
it is better that we make it be consistent with the common metadata,
maybe we will use it in the future.
