Remove the unused ptype struct value, which makes table init easier for
the zero entries, and use ranged initializer to remove a bunch of code
(works with gcc and clang). There is no significant functional change.

Some small things were missed with recent name changes
from i40e to iavf. Having a separate patch allows to
correct the small misses in one place.

With the rename of the iavf driver, there were some
files that were missed in renaming. Update these to
be iavf as well.

This finishes the process of renaming the files that
make sense to rename (skipping adminq related files that
talk to i40e), and fixes up the build and the #includes
so that everything builds nicely.
