For future work we need the struct tpi_info declaration in asm/ptrace.h.
Due to circular dependencies it cannot stay in asm/lowcore.h or asm/cio.h,
which would be the preferred location. Therefore add it in its own header
file.
Also fix a typo in the length of a reserved field that did not have a
functional effect beyond an incorrect field value in the s390_cio_tpi
tracepoint.

ccwgroup_notifier() currently listens for BUS_NOTIFY_UNBIND_DRIVER
events, and triggers an ungrouping for the affected device.
Looking at __device_release_driver(), we can wait for a little longer
until the driver has been fully unbound and eg. bus->remove() has been
called. So listen for BUS_NOTIFY_UNBOUND_DRIVER instead. Due to locking
the current code should work just fine, but this clarifies our intent.

driver_unregister() ends up calling __device_release_driver() for
each device that is bound to the driver. Thus ccwgroup_notifier() will
see a BUS_NOTIFY_UNBIND_DRIVER event for these ccwgroup devices, and
trigger the ungrouping.
So there's no need to also trigger the ungrouping from within
ccwgroup_driver_unregister(), remove it.

Userspace udev rules need an indication about the very first AP change
BINDINGS=complete uevent.
So now this uevent is extend with an additional key-value entry
COMPLETECOUNT=<counter>. The very first uevent will show counter=1 and
the following BINDINGS=complete uevents increase this counter by 1.
Here is an example how the very first BINDINGS=complete uevent
looks like:

Wiring up the cdevs is an essential part of the gdev creation. So
release them during the gdev destruction, ie. on the last put_device().
This could cause us to hold on to the cdevs a tiny bit longer, but
that's not a real concern.
As we have much less certainty of what context this put_device() is
executed from, switch to irqsave locking.

Hijacking the device_driver's probe/remove callbacks for purely
bus-internal logic is a very unconvential construct. Instead just set
up our callbacks in the AP bus struct, and really_probe() will call them
in the same way as before.

The only user of tty_ops::chars_in_buffer is tty_chars_in_buffer. And it
considers tty_ops::chars_in_buffer optional. In case it's NULL, zero is
returned. So remove all those chars_in_buffer from tty_ops which return
zero. (Zero means such driver doesn't buffer.)

Group the flow flags under a single struct called flow. The new struct
contains 'stopped' and 'tco_stopped' bools which used to be bits in a
bitfield. The struct also contains the lock protecting them to
potentially share the same cache line.
Note that commit c545b66c69 (tty: Serialize tcflow() with other tty
flow control changes) added a padding to the original bitfield. It was
for the bitfield to occupy a whole 64b word to avoid interferring stores
on Alpha (cannot we evaporate this arch with weird implications to C
code yet?). But it doesn't work as expected as the padding
(tty_struct::unused) is aligned to a 8B boundary too and occupies some
bytes from the next word.
So make it reliable by:
1) setting __aligned of the struct -- that aligns the start, and
2) making 'unsigned long unused[0]' as the last member of the struct --
pads the end.
This is also the perfect time to start the documentation of tty_struct
where all this lives. So we start by documenting what these bools
actually serve for. And why we do all the alignment dances. Only the few
up-to-date information from the Theodore's comment made it into this new
Kerneldoc comment.

Today, the stacked call to vfio_ccw_sch_io_todo() does three things:
1) Update a solicited IRB with CP information, and release the CP
if the interrupt was the end of a START operation.
2) Copy the IRB data into the io_region, under the protection of
the io_mutex
3) Reset the vfio-ccw FSM state to IDLE to acknowledge that
vfio-ccw can accept more work.
The trouble is that step 3 is (A) invoked for both solicited and
unsolicited interrupts, and (B) sitting after the mutex for step 2.
This second piece becomes a problem if it processes an interrupt
for a CLEAR SUBCHANNEL while another thread initiates a START,
thus allowing the CP and FSM states to get out of sync. That is:

fsm_do_clear()
fsm_irq()
fsm_io_request()
vfio_ccw_sch_io_todo()
fsm_io_helper()
Since the FSM state and CP should be kept in sync, let's make a
note when the CP is released, and rely on that as an indication
that the FSM should also be reset at the end of this routine and
open up the device for more work.

When an I/O request is made, the fsm_io_request() routine
moves the FSM state from IDLE to CP_PROCESSING, and then
fsm_io_helper() moves it to CP_PENDING if the START SUBCHANNEL
received a cc0. Yet, the error case to go from CP_PROCESSING
back to IDLE is done after the FSM call returns.
Let's move this up into the FSM proper, to provide some
better symmetry when unwinding in this case.

We have a really nice flag in the channel_program struct that
indicates if it had been initialized by cp_init(), and use it
as a guard in the other cp accessor routines, but not for a
duplicate call into cp_init(). The possibility of this occurring
is low, because that flow is protected by the private->io_mutex
and FSM CP_PROCESSING state. But then why bother checking it
in (for example) cp_prefetch() then?
Let's just be consistent and check for that in cp_init() too.

Tests with kvm and a kmemdebug kernel showed, that on hot unplug the
zcard and zqueue structs for the unplugged card or queue are not
properly freed because of a mismatch with get/put for the embedded
kref counter.
This fix now adjusts the handling of the kref counters. With init the
kref counter starts with 1. This initial value needs to drop to zero
with the unregister of the card or queue to trigger the release and
free the object.

The qdio layer currently provides its own infrastructure to scan for
Request Queue completions & to report them to the device driver. This
comes with several drawbacks - having an async tasklet & timer construct in
qdio introduces additional lifetime complexity, and makes it harder to
integrate them with the rest of the device driver. The timeouts are also
currently hard-coded, and can't be tweaked without affecting other qdio
drivers (ie. qeth).
But due to recent enhancements to the qdio layer, zfcp can actually take
full control of the Request Queue completion processing. It merely needs to
opt-out from the qdio layer mechanisms by setting the scan_threshold to 0,
and then use qdio_inspect_queue() to scan for completions.
So re-implement the tasklet & timer mechanism in zfcp, while initially
copying the scan conditions from qdio's handle_outbound() and
qdio_outbound_tasklet(). One minor behavioural change is that
zfcp_qdio_send() will unconditionally reduce the timeout to 1 HZ, rather
than leaving it at 10 Hz if it was last armed by the tasklet. This just
makes things more consistent. Also note that we can drop a lot of the
accumulated cruft in qdio_outbound_tasklet(), as zfcp doesn't even use PCI
interrupt requests any longer.
This also slightly touches the Response Queue processing, as
qdio_get_next_buffers() will no longer implicitly scan for Request Queue
completions. So complete the migration to qdio_inspect_queue() here as well
and make the tasklet_schedule() visible.

When zfcp_adapter_enqueue() fails to create the zfcp_sysfs_adapter_attrs
group, it calls zfcp_adapter_unregister() to tear down the adapter state
again. This then unconditionally attempts to remove the
zfcp_sysfs_adapter_attrs group, resulting in a "group not found" WARN from
sysfs code.
Avoid this by copying most of zfcp_adapter_unregister() into the error
path, allowing for more fine-granular roll-back. Then skip the sysfs
tear-down steps if we haven't progressed this far in the initialization.

INIT_LIST_HEAD() is only needed for actual list heads, while req->list is
used as a list entry.
Note that when the error path in zfcp_fsf_req_send() removes the request
from the adapter's list of pending requests, it actually looks up the
request from the zfcp_reqlist - rather than just calling list_del(). So
there's no risk of us calling list_del() on a request that hasn't been
added to any list yet.

The driver core standard is to pass in the properly typed object, the
properly typed attribute and the buffer data. It stems from the root
kobject method:

Each subclass of kobject should provide their own function with the same
signature but more specific types, eg struct device uses:

Where kobj is a 'struct mdev_type *' and dev is 'mdev_type->parent->dev'.

This patch fixes a lockdep splat introduced by commit f21916ec48
("s390/vfio-ap: clean up vfio_ap resources when KVM pointer invalidated").
The lockdep splat only occurs when starting a Secure Execution guest.
Crypto virtualization (vfio_ap) is not yet supported for SE guests;
however, in order to avoid this problem when support becomes available,
this fix is being provided.
The circular locking dependency was introduced when the setting of the
masks in the guest's APCB was executed while holding the matrix_dev->lock.
While the lock is definitely needed to protect the setting/unsetting of the
matrix_mdev->kvm pointer, it is not necessarily critical for setting the
masks; so, the matrix_dev->lock will be released while the masks are being
set or cleared.
Keep in mind, however, that another process that takes the matrix_dev->lock
can get control while the masks in the guest's APCB are being set or
cleared as a result of the driver being notified that the KVM pointer
has been set or unset. This could result in invalid access to the
matrix_mdev->kvm pointer by the intervening process. To avoid this
scenario, two new fields are being added to the ap_matrix_mdev struct:

The functions that handle notification that the KVM pointer value has
been set or cleared will set the kvm_busy flag to true until they are done
processing at which time they will set it to false and wake up the tasks on

access to matrix_mdev->kvm will sleep on the wait queue until they are
awakened at which time they can safely access the matrix_mdev->kvm
field.

The ifmcaddr6 has been protected by inet6_dev->lock(rwlock) so that
the critical section is atomic context. In order to switch this context,
changing locking is needed. The ifmcaddr6 actually already protected by
RTNL So if it's converted to use RCU, its control path context can be
switched to sleepable.

We are spending way too much effort on qdio-internal bookkeeping for
QAOB management & caching, and it's still not robust. Once qdio's
TX path has detached the QAOB from a PENDING buffer, we lost all
track of it until it shows up in a CQ notification again. So if the
device is torn down before that notification arrives, we leak the QAOB.
Just have the driver take care of it, and simply pass down a QAOB if
they want a TX with async-completion capability. For a buffer in PENDING
state that requires the QAOB for final completion, qeth can now also try
to recycle the buffer's QAOB rather than unconditionally freeing it.
This also eliminates the qdio_outbuf_state array, which was only needed
to transfer the aob->user1 tag from the driver to the qdio layer.

This patch introduces the mechanism to inject artificial events to the
CIO layer.
One of the main-event type which triggers the CommonIO operations are
Channel Report events. When a malfunction or other condition affecting
channel-subsystem operation is recognized, a Channel Report Word
(consisting of one or more CRWs) describing the condition is made
pending for retrieval and analysis by the program. The CRW contains
information concerning the identity and state of a facility following
the detection of the malfunction or other condition.
The patch introduces two debugfs interfaces which can be used to inject
'artificial' events from the userspace. It is intended to provide an easy
means to increase the test coverage for CIO code. And this functionality
can be enabled via a new configuration option CONFIG_CIO_INJECT.
The newly introduces debugfs interfaces can be used as mentioned below
to generate different fake-events. To use the crw_inject, first we should
enable it by using enable_inject interface.

Kernel and console/TTY messages written to the SCLP line mode console
are wrapped at 80 characters per line by the associated SCLP driver.
This makes long lines of output difficult to read, and requires editing
of wrapped lines copied from the output device.
Neither the firmware interface used to access the SCLP console, nor the
HMC "Operating System Messages" web interface that displays these
messages require such a length limit. Also other operating systems such
as z/VM do not impose similar limits on messages they emit to the same
console device.
This patch therefore increases the limit to 320 characters per line to
make SCLP line mode console output more readable. As a result 99% of
lines written during a typical boot will not be wrapped, compared to
about 50% wrapped lines at 80 characters per line. Another positive
side-effect of this change is that the HMC console interface is able to
keep more messages in its history buffer due to fewer line-breaks being
generated.
In a worst case scenario this means that a 4k console buffer is emitted
with the last ~400 bytes empty (320 text + 78 headers). This is more
than offset by the fact that each line that is not truncated saves 78
header bytes in the buffer. As a result the actual number of emitted
buffers should be about the same as with the 80 character limit.
This patch also removes the differentiation between line lengths of
SCLP line mode output on z/VM and non-z/VM systems. While the z/VM
hypervisor adds a prefix in front of each line ('xx: ' where xx is the
number of the CPU issuing the message), adjusting Linux line lengths did
not significantly increase readability of console output, and makes even
less of a difference with longer lines.

TX buffers. Pass the NAPI budget to qeth_tx_complete_buf() so that
the freeing of the completed skbs can be deferred.

qeth_init_qdio_out_buf() is typically called during initialization, and
the GFP_ATOMIC is only needed for a very specific & rare case during TX
completion.
Allow callers to specify a gfp mask, so that the initialization path can
select GFP_KERNEL. While at it also clarify the function name.

The cited commit reworked the state machine for pending TX buffers.
In qeth_iqd_tx_complete() it turned PENDING into a transient state, and
uses NEED_QAOB for buffers that get parked while waiting for their QAOB
completion.
But it missed to adjust the check in qeth_tx_complete_buf(). So if
qeth_tx_complete_pending_bufs() is called during teardown to drain
the parked TX buffers, we no longer raise a notification for af_iucv.
Instead of updating the checked state, just move this code into
qeth_tx_complete_pending_bufs() itself. This also gets rid of the
special-case in the common TX completion path.

When a QAOB notifies us that a pending TX buffer has been delivered, the
actual TX completion processing by qeth_tx_complete_pending_bufs()
is done within the context of a TX NAPI instance. We shouldn't rely on
this instance being scheduled by some other TX event, but just do it
ourselves.
qeth_qdio_handle_aob() is called from qeth_poll(), ie. our main NAPI
instance. To avoid touching the TX queue's NAPI instance
before/after it is (un-)registered, reorder the code in qeth_open()
and qeth_stop() accordingly.

The current design attaches a pending TX buffer to a custom
single-linked list, which is anchored at the buffer's slot on the
TX ring. The buffer is then checked for final completion whenever
this slot is processed during a subsequent TX NAPI poll cycle.
But if there's insufficient traffic on the ring, we might never make
enough progress to get back to this ring slot and discover the pending
buffer's final TX completion. In particular if this missing TX
completion blocks the application from sending further traffic.
So convert the custom single-linked list code to a per-queue list_head,
and scan this list on every TX NAPI cycle.

When qeth_alloc_qdio_queues() fails to allocate one of the buffers that
back an Output Queue, the 'out_freeoutqbufs' path will free all
previously allocated buffers for this queue. But it misses to free the
half-finished queue struct itself.
Move the buffer allocation into qeth_alloc_output_queue(), and deal with
such errors internally.

The zFCP/NVMe standalone dumper is supposed to release the dump save area
resource as soon as possible but might fail to do so, for instance, if it
crashes. To avoid this situation, register a reboot notifier and ensure
the dump save area resource is released on reboot or power down.

Prevent that an IO request is build during device shutdown initiated by
a driver unbind. This request will never be able to be processed or
canceled and will hang forever. This will lead also to a hanging unbind.
Fix by checking not only if the device is in READY state but also check
that there is no device offline initiated before building a new IO request.

In case of an unbind of the DASD device driver the function
dasd_generic_remove() is called which shuts down the device.
Among others this functions removes the int_handler from the cdev.
During shutdown the device cancels all outstanding IO requests and waits
for completion of the clear request.
Unfortunately the clear interrupt will never be received when there is no
interrupt handler connected.
Fix by moving the int_handler removal after the call to the state machine
where no request or interrupt is outstanding.

CCW_CMD_READ_STATUS was introduced with revision 2 of virtio-ccw,
and drivers should only rely on it being implemented when they
negotiated at least that revision with the device.
However, virtio_ccw_get_status() issued READ_STATUS for any
device operating at least at revision 1. If the device accepts
READ_STATUS regardless of the negotiated revision (which some
implementations like QEMU do, even though the spec currently does
not allow it), everything works as intended. While a device
rejecting the command should also be handled gracefully, we will
not be able to see any changes the device makes to the status,
such as setting NEEDS_RESET or setting the status to zero after
a completed reset.
We negotiated the revision to at most 1, as we never bumped the
maximum revision; let's do that now and properly send READ_STATUS
only if we are operating at least at revision 2.

For non-QEBSM devices, get_buf_states() merges PENDING and EMPTY buffers
into a single group of finished buffers. To allow the upper-layer driver
to differentiate between the two states, qdio_check_pending() looks at
each buffer's state again and sets the sbal_state flag to
QDIO_OUTBUF_STATE_FLAG_PENDING accordingly.
So effectively we're spending overhead on _every_ Output Queue
inspection, just to avoid some additional TX completion calls in case
a group of buffers has completed with mixed EMPTY / PENDING state.
Given that PENDING buffers should rarely occur, this is a bad trade-off.
In particular so as the additional checks in get_buf_states() affect
_all_ device types (even those that don't use the PENDING state).
Rip it all out, and just report the PENDING completions separately as
we already do for QEBSM devices.

For QEBSM devices the 'merge_pending' mechanism in get_buf_states()
doesn't apply, and we can actually get SLSB_P_OUTPUT_PENDING returned.
So for this case propagating the PENDING state to the driver via the
queue's sbal_state doesn't make sense and creates unnecessary overhead.
Instead introduce a new QDIO_ERROR_* flag that gets passed to the
driver, and triggers the same processing as if the buffers were flagged
as QDIO_OUTBUF_STATE_FLAG_PENDING.

When inspecting a queue, any error is currently returned back through
the queue's qdio_error field. Turn this into a proper variable that gets
passed through the call chain, so that the lifetime is clear and the
error state can be accessed along the way.

When a msg is retried because the lower ap layer returns -EAGAIN
there is a retry limit (currently 10). When this limit is reached
the last return code from the lower layer is returned, causing
the userspace to get -1 on the ioctl with errno EAGAIN.
This EAGAIN is misleading here. After 10 retry attempts the
userspace should receive a clear failure indication like EINVAL
or EIO or ENODEV. However, the reason why these retries all
fail is unclear. On an invalid message EINVAL would be returned
by the lower layer, and if devices go away or are not available
an ENODEV is seen. So this patch now reworks the retry loops
to return EIO to userspace when the retry limit is reached.

We currently track the time of the most recent QDIO Adapter Interrupt.
This is a system-wide timestamp (as such interrupts are not bound to
one specific qdio device).
If interrupt processing stalls on one device but is functional for a
different device, the timestamp continues to be updated and is of no
help for problem diagnosis.
So for debugging purposes also track the time of the last Data IRQ on
a per-device level. Collect this data in the legacy non-AI path as well.

tiqdio_add_device() adds the device to the tiq_list of eligible targets
for a data IRQ, which gets walked on each QDIO Adapter Interrupt to
inspect their DSCIs.
But currently the tiqdio_add_device() / tiqdio_remove_device() calls
are not symmetric - the device is removed within qdio_shutdown(),
but only added by qdio_activate().
So depending on the call sequence and encountered errors, we might
be trying to remove a list entry in qdio_shutdown() that was never even
added to the list. This required additional INIT_LIST_HEAD() calls to
ensure that the list entry was always in a consistent state.
All drivers now fence the IRQ delivery via qdio_start_irq() /
qdio_stop_irq(), so we can nicely integrate this tiq_list management
with the other steps needed for QDIO Adapter IRQ (de-)registration
(qdio_establish_thinint() / qdio_shutdown_thinint()).
As the naming suggests these get called during qdio_establish() and
qdio_shutdown(), with proper symmetry and roll-back after errors.
With this we longer need to worry about misplaced list removals, and
thus can clean up the list API abuse (INIT_LIST_HEAD() should not be
called on list entries).

Input Queues with commit 0a6e634535 ("s390/qdio: extend polling
support to multiple queues") and commit 0b524abc2d ("scsi: zfcp: Lift
Input Queue tasklet from qdio").
So remove the tasklet code for Input Queues, streamline the IRQ handlers
and push the tasklet struct into struct qdio_output_q.

Stop maintaining the skb_send_q list for TRANS_HIPER sockets.
Not only is it extra overhead, but keeping around a list of skb clones
means that we later also have to match the ->sk_txnotify() calls
against these clones and free them accordingly.
The current matching logic (comparing the skbs' shinfo location) is
frustratingly fragile, and breaks if the skb's head is mangled in any
sort of way while passing from dev_queue_xmit() to the device's
HW queue.
Also adjust the interface for ->sk_txnotify(), to make clear that we
don't actually care about any skb internals.

When do_qdio() returns with an unexpected error, qeth_flush_buffers()
kicks off a recovery action.
In such a case there's no point in starting TX completion processing,
the device gets torn down anyway. So take a closer look at do_qdio()'s
return value, and skip the TX completion processing accordingly.

As part of the TX queue selection for af_iucv skbs,
qeth_l3_get_cast_type_rcu() ends up calling qeth_get_ether_cast_type().
Which is rather fragile, since such skbs don't have a proper ETH header
and we rely on it being zeroed out in the right places. Add a separate
case for ETH_P_AF_IUCV instead that does the right thing.
When later building the HW header for such skbs, don't hard-code the
cast type but follow the same path as for other protocol types. Here
the cast type should naturally come from the skb's queue mapping.

Replace our home-grown helper with the more robust vlan_get_protocol().
This is pretty much a 1:1 replacement, we just need to pass around a
proper ETH_P_* everyhwere and convert the old value range.
For readability also convert the protocol checks in
qeth_l3_hard_start_xmit() to a switch statement.

So now the low level cca functions return -EAGAIN when this
error condition is seen and the paes retry function will
evaluate the return value. Seeing EAGAIN and running in
process context results in trying to sleep for 1 s now.

The queues assigned to a matrix mediated device are currently reset when:
* The VFIO_DEVICE_RESET ioctl is invoked
* The mdev fd is closed by userspace (QEMU)
* The mdev is removed from sysfs.
Immediately after the reset of a queue, a call is made to disable
interrupts for the queue. This is entirely unnecessary because the reset of
a queue disables interrupts, so this will be removed.
Furthermore, vfio_ap_irq_disable() does an unconditional PQAP/AQIC which
can result in a specification exception (when the corresponding facility
is not available), so this is actually a bugfix.

The vfio_ap device driver registers a group notifier with VFIO when the
file descriptor for a VFIO mediated device for a KVM guest is opened to
receive notification that the KVM pointer is set (VFIO_GROUP_NOTIFY_SET_KVM
event). When the KVM pointer is set, the vfio_ap driver takes the
following actions:
1. Stashes the KVM pointer in the vfio_ap_mdev struct that holds the state
of the mediated device.
2. Calls the kvm_get_kvm() function to increment its reference counter.
3. Sets the function pointer to the function that handles interception of
the instruction that enables/disables interrupt processing.
4. Sets the masks in the KVM guest's CRYCB to pass AP resources through to
the guest.
In order to avoid memory leaks, when the notifier is called to receive
notification that the KVM pointer has been set to NULL, the vfio_ap device
driver should reverse the actions taken when the KVM pointer was set.

Our intention was to only remove path kobjects whenever a device is
being set offline. However, one corner case was missing.
If a device is disabled and enabled (using the IOCTLs BIODASDDISABLE and
BIODASDENABLE respectively), the enabling process will call
dasd_eckd_reload_device() which itself calls dasd_eckd_read_conf() in
order to update path information. During that update,
dasd_eckd_clear_conf_data() clears all old data and also removes all
kobjects. This will leave us with an inconsistent state of path kobjects
and a subsequent path verification leads to a failing kobject creation.
Fix this by removing kobjects only in the context of offlining a device
as initially intended.

Replace the gendisk pointer in struct bio with a pointer to the newly
improved struct block device. From that the gendisk can be trivially
accessed with an extra indirection, but it also allows to directly
look up all information related to partition remapping.

Bypassing the DMA API is bad style, even when we don't expect any
actual problems. Let's utilize the right API helpers for setting the
DMA masks and check for returned errors, so that we benefit from
common sanity checks.
io_subchannel_allocate_dev() required some extra massaging, so that we
can return an errno other than -ENOMEM.

ip_finish_output_gso() may call .ndo_features_check() even before the
skb has a L2 header. This conflicts with qeth_get_ip_version()'s attempt
to inspect the L2 header via vlan_eth_hdr().
Switch to vlan_get_protocol(), as already used further down in the
common qeth_features_check() path.

Due to insufficient locking, qeth_core_set_online() and
qeth_dev_layer2_store() can run in parallel, both attempting to load &
setup the discipline (and stepping on each other toes along the way).
A similar race can also occur between qeth_core_remove_device() and
qeth_dev_layer2_store().
Access to .discipline is meant to be protected by the discipline_mutex,
so add/expand the locking in qeth_core_remove_device() and
qeth_core_set_online().
Adjust the locking in qeth_l*_remove_device() accordingly, as it's now
handled by the callers in a consistent manner.

When qeth_dev_layer2_store() - holding the discipline_mutex - waits
inside qeth_l*_remove_device() for a qeth_do_reset() thread to complete,
we can hit a deadlock if qeth_do_reset() concurrently calls
qeth_set_online() and thus tries to aquire the discipline_mutex.
Move the discipline_mutex locking outside of qeth_set_online() and
qeth_set_offline(), and turn the discipline into a parameter so that
callers understand the dependency.
To fix the deadlock, we can now relax the locking:
As already established, qeth_l*_remove_device() waits for
qeth_do_reset() to complete. So qeth_do_reset() itself is under no risk
of having card->discipline ripped out while it's running, and thus
doesn't need to take the discipline_mutex.

The main purpose of tty_port::low_latency was removed in commit
a9c3f68f3c (tty: Fix low_latency BUG) back in 2014. It was left in
place for drivers as an optional tune knob. But only one driver has been
using it until the previous commit. So remove this misconcept
completely, given there are no users.

In dasd_alias_disconnect_device_from_lcu the device is removed from any
list on the LCU. Afterwards the LCU is removed from the lcu list if it
does not contain devices any longer.
The lcu->lock protects the lcu from parallel updates. But to cancel all
workers and wait for completion the lcu->lock has to be unlocked.
If two devices are removed in parallel and both are removed from the LCU
the first device that takes the lcu->lock again will delete the LCU because
it is already empty but the second device also tries to free the LCU which
leads to a list corruption of the lcu list.
Fix by removing the device right before the lcu is checked without
unlocking the lcu->lock in between.

dasd_alias_add_device() moves devices to the active_devices list in case
of a scheduled LCU update regardless if they have previously been in a
pavgroup or not.
Example: device A and B are in the same pavgroup.
Device A has already been in a pavgroup and the private->pavgroup pointer
is set and points to a valid pavgroup. While going through dasd_add_device
it is moved from the pavgroup to the active_devices list.
In parallel device B might be removed from the same pavgroup in
remove_device_from_lcu() which in turn checks if the group is empty
and deletes it accordingly because device A has already been removed from
there.
When now device A enters remove_device_from_lcu() it is tried to remove it
from the pavgroup again because the pavgroup pointer is still set and again
the empty group will be cleaned up which leads to a list corruption.
Fix by setting private->pavgroup to NULL in dasd_add_device.
If the device has been the last device on the pavgroup an empty pavgroup
remains but this will be cleaned up by the scheduled lcu_update which
iterates over all existing pavgroups.

Prevent _lcu_update from adding a device to a pavgroup if the LCU still
requires an update. The data is not reliable any longer and in parallel
devices might have been moved on the lists already.
This might lead to list corruptions or invalid PAV grouping.
Only add devices to a pavgroup if the LCU is up to date. Additional steps
are taken by the scheduled lcu update.

For an LCU update a read unit address configuration IO is required.
This is started using sleep_on(), which has early exit paths in case the
device is not usable for IO. For example when it is in offline processing.
In those cases the LCU update should fail and not be retried.
Therefore lcu_update_work checks if EOPNOTSUPP is returned or not.
Commit 41995342b4 ("s390/dasd: fix endless loop after read unit address configuration")
accidentally removed the EOPNOTSUPP return code from
read_unit_address_configuration(), which in turn might lead to an endless
loop of the LCU update in offline processing.
Fix by returning EOPNOTSUPP again if the device is not able to perform the
request.

When qeth_qdio_handle_aob() frees dangling allocations in the notified
TX buffer, there are rare tear-down cases where
qeth_drain_output_queue() would later call qeth_clear_output_buffer()
for the same buffer - and thus end up walking the buffer a second time
to check for dangling kmem_cache allocations.
Luckily current code previously scrubs such a buffer, so
qeth_clear_output_buffer() would find buf->buffer->element[i].addr as
NULL and not do anything. But this is fragile, and we can easily improve
it by consistently clearing the ->is_header flag after freeing the
allocation.

For TX buffers that require an additional async notification via QAOB, the
TX completion code can now manage all the necessary processing if the
notification has already occurred (or is occurring concurrently).
In such cases we can avoid replacing the metadata that is associated
with the buffer's slot on the ring, and just keep using the current one.
As qeth_clear_output_buffer() will also handle any kmem cache-allocated
memory that was mapped into the TX buffer, qeth_qdio_handle_aob()
doesn't need to worry about it.
While at it, also remove the unneeded forward declaration for
qeth_init_qdio_out_buf().

All qeth devices have a minimum set of sysfs attributes, and non-OSN
devices share a group of additional attributes. Depending on whether
the device is forced to use a specific discipline, the device_type then
specifies further attributes.
Shift the common attributes into dev->groups, so that the device_type
only contains the discipline-specific attributes. This avoids exposing
the common attributes to the disciplines, and nicely cleans up our
sysfs code.
While replacing the qeth_l*_*_device_attributes() helpers, switch from
sysfs_*_groups() to the more generic device_*_groups().

Bus drivers have their own way of describing the sysfs attributes that
all devices on a bus should provide.
Switch ccwgroup_attr_groups over to use bus->dev_groups, and thus
free up dev->groups for usage by the ccwgroup device drivers.
While adjusting the attribute naming, use ATTRIBUTE_GROUPS() to get rid
of some boilerplate code.

The device is being unplugged, so pass the request to userspace to
ask for a graceful cleanup. This should free up the thread that

The usage of in_interrupt() in drivers is phased out and Linus clearly
requested that code which changes behaviour depending on context should
either be separated or the context be conveyed in an argument passed by the
caller, which usually knows the context.
ctcmpc_tx() is used as net_device_ops::ndo_start_xmit. This callback is
invoked with disabled bottom halves.
Use GFP_ATOMIC for memory allocation in ctcmpc_tx().
Remove gfp_type() since the last user is gone.

values directly to skb's data part instead of using memcpy() for it.

Now that the hd_struct always has a block device attached to it, there is
no need for having two size field that just get out of sync.
Additionally the field in hd_struct did not use proper serialization,
possibly allowing for torn writes. By only using the block_device field
this problem also gets fixed.

When qeth_iqd_tx_complete() detects that a TX buffer requires additional
async completion via QAOB, it might fail to replace the queue entry's
metadata (and ends up triggering recovery).
Assume now that the device gets torn down, overruling the recovery.
If the QAOB notification then arrives before the tear down has
sufficiently progressed, the buffer state is changed to
QETH_QDIO_BUF_HANDLED_DELAYED by qeth_qdio_handle_aob().
The tear down code calls qeth_drain_output_queue(), where
qeth_cleanup_handled_pending() will then attempt to replace such a
buffer _again_. If it succeeds this time, the buffer ends up dangling in
its replacement's ->next_pending list ... where it will never be freed,
since there's no further call to qeth_cleanup_handled_pending().
But the second attempt isn't actually needed, we can simply leave the
buffer on the queue and re-use it after a potential recovery has
completed. The qeth_clear_output_buffer() in qeth_drain_output_queue()
will ensure that it's in a clean state again.

The two expected notification sequences are
1. TX_NOTIFY_PENDING with a subsequent TX_NOTIFY_DELAYED_*, when
our TX completion code first observed the pending TX and the QAOB
then completes at a later time; or
2. TX_NOTIFY_OK, when qeth_qdio_handle_aob() picked up the QAOB
completion before our TX completion code even noticed that the TX
was pending.
But as qeth_iqd_tx_complete() and qeth_qdio_handle_aob() can run
concurrently, we may end up with a race that results in a sequence of
TX_NOTIFY_DELAYED_* followed by TX_NOTIFY_PENDING. Which would confuse
the af_iucv code in its tracking of pending transmits.
Rework the notification code, so that qeth_qdio_handle_aob() defers its
notification if the TX completion code is still active.

Currently we have to consider too many different values which
in the end only affect identity mapping size. These are:
1. max_physmem_end - end of physical memory online or standby.
Always <= end of the last online memory block (get_mem_detect_end()).
2. CONFIG_MAX_PHYSMEM_BITS - the maximum size of physical memory the
kernel is able to support.
3. "mem=" kernel command line option which limits physical memory usage.
4. OLDMEM_BASE which is a kdump memory limit when the kernel is executed as
crash kernel.
5. "hsa" size which is a memory limit when the kernel is executed during
zfcp/nvme dump.
Through out kernel startup and run we juggle all those values at once
but that does not bring any amusement, only confusion and complexity.
Unify all those values to a single one we should really care, that is
our identity mapping size.

The link mode is a combination of port speed and port mode. But we
currently only consider the speed, and then typically select the
corresponding TP-based link mode. For 1G and 10G Fibre links this means
we display the wrong link modes.
Move the SPEED_* switch statements inside the PORT_* cases, and only
consider valid combinations where we can select the corresponding
link mode. Add the relevant link modes (1000baseX, 10000baseSR and
1000baseLR) that were introduced back with
commit 5711a98221 ("net: ethtool: add support for 1000BaseX and missing 10G link modes").
To differentiate between 10000baseSR and 10000baseLR, use the detailed
media_type information that QUERY OAT provides.

Improve the initial link info with data obtained from QUERY OAT.
Doing so _only_ at initialization time avoids
1. dealing with multi-part replies, and
2. sifting through all the data that may get returned at runtime.
This allows us to determine the correct port type for the 1000BT variant
of recent OSA adapter generations (where the .card_type field in
QUERY CARD INFO is no longer sufficient).

Remove the default case for PORT_* and SPEED_* in our ethtool code.
The only time these could be hit is if qeth_init_link_info() was unable
to determine the port type from an OSA adapter's link_type.
We already throw a message in this case, so reduce the noise and don't
report bad data (ie. it's much more likely that any future link_type
will represent a PORT_FIBRE link ...).

Hard-code the minimal link info at initialization time, after we
obtained the link_type. qeth_get_link_ksettings() can still override
this with more accurate data from QUERY CARD INFO later on.
Don't set arbitrary defaults for unknown OSA link types, they
certainly won't match any future type.

Move all the HW reply data parsing into qeth_query_card_info_cb(), and
use common ethtool enums for transporting the information back to the
caller.
Also only look at the .port_speed field when we couldn't determine the
speed from the .card_type field, and introduce some 'default' cases for
SPEED_UNKNOWN, PORT_OTHER and DUPLEX_UNKNOWN.

By the time that our .get_link_ksettings() code issues a QUERY CARD INFO
cmd to get link-related information, we already set up a good amount of
static link data.
Return this data when the cmd fails, same as when the cmd is not
supported.

call_switchdev_notifiers() doesn't require holding the RTNL lock since
commit ff5cf10011 ("net: switchdev: Change notifier chain to be atomic").
We still need it for the "lost event" slow path, to avoid racing against
a concurrent .ndo_bridge_setlink().

As the number of cpus increases, the sccb response can exceed 4k for
read cpu and read scp info sclp commands. Hence, all cpu info entries
cant be embedded within a sccb response
Solution:
To overcome this limitation, extended sccb facility is provided by sclp.
1. Check if the extended sccb facility is installed.
2. If extended sccb is installed, perform the read scp and read cpu
command considering a max sccb length of three page size. This max
length is based on factors like max cpus, sccb header.
3. If extended sccb is not installed, perform the read scp and read cpu
sclp command considering a max sccb length of one page size.

sclp early read cpu info is used to detect the number of configured
cpus, which is utilized by smp_detect_cpus() in early startup.
* For read cpu info, the sccb block should be below 2gb.
* smp_detect_cpus() utilizes read cpu info early, but after memblock
initialization. Thus use memblock_allow_low() instead.
* Avoid copy of sclp_core_info structure.
* sclp_early_init_core_info(), sclp_early_core_info and
sclp_early_core_info_valid initdata are no longer required.
* smp_get_core_info() is called only once during early stage. Hence for
early sclp_get_core_info(), directly call read cpu command. No need to
maintain sclp_early_core_info_valid.

When requeueing all requests on the device request queue to the blocklayer
we might get to an ERP (error recovery) request that is a copy of an
original CQR.
Those requests do not have blocklayer request information or a pointer to
the dasd_queue set. When trying to access those data it will lead to a
null pointer dereference in dasd_requeue_all_requests().
Fix by checking if the request is an ERP request that can simply be
ignored. The blocklayer request will be requeued by the original CQR that
is on the device queue right behind the ERP request.

If the Fibre Channel Endpoint-Security status of a path changes, a
corresponding path event is received from the CIO layer.
Process this event by re-reading the FCES information.
As the information is retrieved for all paths on a single CU in one
call, the internal status can also be updated for all paths and no
processing per path is necessary.

As more path events need to be handled for ECKD the current path
verification infrastructure can be reused. Rename all path verifcation
code to fit the more broadly based task of path event handling and put
the path verification in a new separate function.

Add a new sysfs attribute (fc_security) per device and per operational
channel path. The information of the current FC Endpoint Security state
is received through the CIO layer.
The state of the FC Endpoint Security can be either "Unsupported",
"Authentication", or "Encryption".
For example:
$ cat /sys/bus/ccw/devices/0.0.c600/fc_security
Encryption
If any of the operational paths is in a state different from all
others, the device sysfs attribute will display the additional state
"Inconsistent".
The sysfs attributes per paths are organised in a new directory called
"paths_info" with subdirectories for each path.

During online processing and setting up a DASD device, the configuration
data for operational paths is read and validated two times
(dasd_eckd_read_conf()). The first time to provide information that are
necessary for the LCU setup. A second time after the LCU setup as a
device might report different configuration data then.
When the configuration setup for each operational path is being
validated, an initial call to dasd_eckd_clear_conf_data() is issued.
This call wipes all previously available configuration data and path
information for each path.
However, the operational path mask is not updated during this process.
As a result, the stored operational path mask might no longer correspond
to the operational paths mask reported by the CIO layer, as several
paths might be gone between the two dasd_eckd_read_conf() calls.
This inconsistency leads to more severe issues in later path handling
changes. Fix this by removing the channel paths from the operational
path mask during the dasd_eckd_clear_conf_data() call.

Currently, the configuration data for a path is retrieved during a path
verification and used only temporarily. If a path is newly added to the
I/O setup after a boot, no configuration data will be stored for this
particular path.
However, this data is required for later use and should be present for
a valid I/O path anyway. Store this data during the path verification so
that newly added paths can provide all information necessary.

For storing retrieved path information both the if and else block in
dasd_eckd_read_conf() use the same code. To avoid duplicate code this
should be done after the if/else block. To further increase readability,
move the code to a new function, dasd_eckd_store_conf_data().
