As part of the removal of get_driver()/put_driver(), this patch
(as1510) changes driver_find(); it now drops the reference it acquires
before returning. The patch also adjusts all the callers of
driver_find() to remove the now unnecessary calls to put_driver().
In addition, the patch adds a warning to driver_find(): Callers must
make sure the driver they are searching for does not get unloaded
while they are using it. This has always been the case; driver_find()
has never prevented a driver from being unregistered or unloaded.
Hence the patch will not introduce any new bugs. The existing callers
all seem to be okay in this respect, however I don't understand the
video drivers well enough to be certain about them.

Remove the optimization that validate server is only called once per
LCU. If a device is set online we only know that we already know the
LCU. But if the pathgroup was lost in between we have to do a
validate server again to activate some features.
Since we have no indication when a pathgroup gets lost we have to do
a validate server every time a device is set online.

module_param(bool) used to counter-intuitively take an int. In
fddd5201 (mid-2009) we allowed bool or int/unsigned int using a messy
trick.
It's time to remove the int/unsigned int option. For this version
it'll simply give a warning, but it'll break next kernel version.

For historical reasons, we allow module_param(bool) to take an int (or
an unsigned int). That's going away.
A few drivers really want an int: they set it to -1 and a parameter
will set it to 0 or 1. This sucks: reading them from sysfs will give
'Y' for both -1 and 1, but if we change it to an int, then the users
might be broken (if they did "param" instead of "param=1").
Use a new 'bint' parser for them.
(ntfs has a different problem: it needs an int for debug_msgs because
it's also exposed via sysctl.)

We were cheating with our barriers; using the smp ones rather than the
real device ones. That was fine, until rpmsg came along, which is
used to talk to a real device (a non-SMP CPU).
Unfortunately, just putting back the real barriers (reverting
d57ed95d) causes a performance regression on virtio-pci. In
particular, Amos reports netbench's TCP_RR over virtio_net CPU
utilization increased up to 35% while throughput went down by up to
14%.
By comparison, this branch is in the noise.

<sfr@canb.auug.org.au> who was invaluable in the merge issues involved
with the large sysdev removal process in the driver-core tree.

Move invalidate_bdev, block_sync_page into fs/block_dev.c. Export
kill_bdev as well, so brd doesn't have to open code it. Reduce
buffer_head.h requirement accordingly.
Removed a rather large comment from invalidate_bdev, as it looked a bit
obsolete to bother moving. The small comment replacing it says enough.

If an IO request is build on an alias device without prefix enabled
we try to calculate with zero data from the alias device. This
triggers a BUG statement with fixpoint divide exception.
This case is very unlikely and can only happen if the pathgroup is
lost with an alias device already in use.
Prevent the alias device from being used in this case.

The queue_start_poll function pointer field in struct qdio_initialize
had to change its type and become a vector of function pointers to
support asynchronous delivery of storage blocks so rename the field to
make the type change explicit and ensure no other user of qdio tries
to use the field the old way. During setting up the qdio queues, only
dereference vector elements if the vector is actually allocated.

The 'expires' value of a ccw requests defines how long the device
driver should wait for a response from the evice after the request has
been submitted to the channel subsystem. After the expiration time
(e.g. 30 seconds) the waiting request will be cancelled and started
again. This protects the DASD devices from beeing blocked by errors
that cause the answering I/O interrupt to be lost.
In case of error recovery requests, this 'expires' value used to be
set to 0, so in case of a lost interrupt, such a recovery request
would never expire and block the device. To prevent this kind of
problem, all recovery requests need to have an expires value > 0 as
well. If not specified otherwise, this should be the same expires
value as for the original request.

This moves the 'cpu sysdev_class' over to a regular 'cpu' subsystem
and converts the devices to regular devices. The sysdev drivers are
implemented as subsystem interfaces now.
After all sysdev classes are ported to regular driver core entities, the
sysdev implementation will be entirely removed from the kernel.
Userspace relies on events and generic sysfs subsystem infrastructure
from sysdev devices, which are made available with this conversion.

If recovery is triggered in presence of pending asynchronous
deliveries of storage blocks we do a forced cleanup after
the corresponding tasklets are completely stopped and trigger
appropriate notifications for the correspondingerror state.

The NETIUCV device driver allows to connect a Linux guest on z/VM to
another z/VM guest based on the z/VM communication facility IUCV.
Multiple output paths to different guests are possible, as well as
multiple input paths from different guests.
With this feature, you can configure multiple point-to-point NETIUCV
interfaces between your Linux on System z instance and another z/VM
guest.

zfcp_scsi_slave_destroy erroneously always tried to finish its task
even if the corresponding previous zfcp_scsi_slave_alloc returned
early. This can lead to kernel page faults on accessing uninitialized
fields of struct zfcp_scsi_dev in zfcp_erp_lun_shutdown_wait. Take the
port field of the struct to determine if slave_alloc returned early.
This zfcp bug is exposed by 4e6c82b (in turn fixing f7c9c6b to be
compatible with 21208ae) which can call slave_destroy for a
corresponding previous slave_alloc that did not finish.

Let caller know the result of adding/removing vlan id to/from vlan

In some drivers I make those functions to just return 0. But in those
where there is able to see if hw setup went correctly, return value is

If a driver wants to do command mode IO while CIO is doing
online path verification we ignore this request and provide
a fake irb when we are done and the driver can do IO again.
For transport mode IO we have no such mechanism, giving the
driver no other chance then to retry the action until we are
done. This is not very reliable.
Provide a fake irb for transport mode IO as well.

When a driver requests to do IO, we will adjust the mask of
paths to be used to exclude varied offline paths.
Drivers trying to do IO solely on paths which are online but some
way defective may lack the information to do proper error handling.
There is no reason to allow the usage of known to be broken paths.
Thus restrict the paths a ccw driver can use for IO to a subset of
the paths cio found usable (this also excludes offline paths).

Using the generic css_schedule_eval to evaluate subchannels
while resuming from hibernation is very slow when used with
many devices. Provide a new evaluation trigger which exploits
css_sched_sch_todo and use this in the resume callback for
ccw devices.

Add a new .bus_name to virtio_config_ops then modify virtio_net to
call through to it in an ethtool .get_drvinfo routine to report

When the kernel is started in kdump mode, zfcpdump should not be
initialized because both dump methods can't be used at the same time.

A kernel panic may occur during sending or receiving network packets
on a machine without adapter interrupts since commit d36deae.
The bug is triggered by writing to the shared indicator address which
is set to 0 if the machine doesn't have adapter interrupts.
Make the reading and setting of the shared indicator dependent on the
adapter interrupt feature and while at it move the code to the
file containing the adapter interrupt related code.

Running under z/VM with QIOASSIST enabled, qdio queues could stall if EQBS
did not extract all SBAL states. Add an instant retry for EQBS and, if the
retry fails, set up a timer to ensure outstanding SBALs are processed later.
While at it, optimize qdio_do_eqbs and qdio_do_sqbs to eliminate 3 jumps on
the hot path.

Add a timestamp per queue and update the timestamp when the queue is
scanned. Add the queue timestamps and the timestamp of the last
adapter interrupt to the debugfs output. The timestamps are useful
for debugging stall conditions.

The buffer for read configuration data has to be initialized with an
EBCDIC string to show support for extended UIDs to z/VM.
If this read configuration data CQR needs to be retried, the buffer
may have changed in between. So re-initialize the buffer to get a
correct extended UID under z/VM.

After terminating a request in the dasd_sleep_on_immediatly function,
wait for the clear interrupt to be received before starting the

Split out addressing mode bits from PSW_BASE_BITS, rename PSW_BASE_BITS
to PSW_MASK_BASE, get rid of psw_user32_bits, remove unused function
enabled_wait(), introduce PSW_MASK_USER, and drop PSW_MASK_MERGE macros.
Change psw_kernel_bits / psw_user_bits to contain only the bits that
are always set in the respective mode.

Print a message in case we do not receive an IRQ in time (for internal
I/O). Also print the ID of the last used channel path, since it is
possible that not the device itself but this specific path might have
a defect.

There is very little benefit in allowing to let a ->make_request
instance update the bios device and sector and loop around it in
__generic_make_request when we can archive the same through calling
generic_make_request from the driver and letting the loop in
generic_make_request handle it.
Note that various drivers got the return value from ->make_request and
returned non-zero values for errors.

This patch extends the HiperSockets device driver to send and receive
af_iucv traffic over HiperSockets transport.
TX: Driver uses new asynchronous delivery of storage blocks to pass
flow control/congestion information from the HiperSockets microcode
to the af_iucv socket.
RX: Memory for incoming traffic is preallocated and passed to
HiperSockets layer. If receiver is not capable to clean its buffers
shared with HiperSockets and pass new memory to the HiperSockets
layer this will cause flow control/congestion events on the
sender.

OSA cards can be configured to support 1 or 4 output queues. This
does not apply to HiperSockets. This patch limits determination of
the configured number of output queues to OSA cards only, but excludes
HiperSockets.

In layer 3 mode IPv6 over VLAN does not work on newer OSA levels in
case the sender and receiver run on the same (shared) OSA adapter.
Keep vlan info in the skb so the qdio header is filled with the
required vlan tag.

To use raw track access some special storage server commands are
needed. Older storage hardware may not support these commands.
So check if raw track access is possible while setting the DASD
online.

The size of the buffer that is used to store DASD statistics input
strings depends on the user input. If the input string is to large,
the write operation could fail with -ENOMEM. To avoid this, use
vmalloc instead of kmalloc.

The SIGA-W may return with the busy bit set which means the device was
blocked. The busy loop which retries the SIGA-W for 100us may not be
long enough when running under a heavily loaded hypervisor.
Extend the retry mechanism by adding a longer second stage which retries
the SIGA-W for up to 10s. In difference to the first retry loop the second
stage is using mdelay to stop the cpu between the retries and thereby
avoid additional preassure in on the hypervisor.
If the second stage retry is successfull a device reset is avoided.

This patch extends the DASD statistics to allow for a more detailed
analysis of DASD I/O operations. In particular we want the statistics
to provide answers to the following questions:
- How many requests used a PAV alias?
- How many requests used High Performance FICON?
- How do read request perform versus write requests?
The existing DASD statistics interface has several shortcomings
- The interface for global data is a formatted text table in procfs
(/proc/dasd/statistics). The layout is meant for human readers and
is not to easy to parse. If values get to large for the table
layout, they get scaled down.
- The statistics which are collected per block device can be
accessed via an ioctl interface, which can only be extended by
defining a new ioctl.
- There is no statistics interface for individual PAV base and alias
devices.
To overcome theses shortcomings we create a new DASD statistics
interface in debugfs. This interface will contain one entry for global
data, one per DASD block device, and one per DASD base and alias
device. Each file contains the statistic data in easy to parse
name/value and name/array pairs. The existing interfaces will remain
functional, but they will not be extended.

Fix the race by resetting the DSCI before scheduling the inbound tasklet
so the device generates an interrupt if new data arrives in the above
scenario in step 6.

This enables pm_notifier_call_chain() to get the actual error code
in the callback rather than always assume -EINVAL by converting all
PM notifier calls to return encapsulate error code with
notifier_from_errno().

The qdio SBAL entry flag is made-up of four different values that are
independent of one another. Some of the bits are reserved by the
hardware and should not be changed by qdio. Currently all four values
are overwritten since the SBAL entry flag is defined as an u32.
Split the SBAL entry flag into four u8's as defined by the hardware
and don't touch the reserved bits.

Interrupt sources like pfault, sclp, dasd_diag and virtio all use the
service signal external interrupt subclass mask in control register 0
to enable and disable the corresponding interrupt.
Because no reference counting is implemented each subsystem thinks it
is the only user of subclass and sets and clears the bit like it wants.
This leads to case that unloading the dasd diag module under z/VM
causes both sclp and pfault interrupts to be masked. The result will
be locked up system sooner or later.
Fix this by introducing a new way to set (register) and clear
(unregister) the service signal subclass mask bit in cr0.
Also convert all drivers.

Provide sysfs attributes that contain the CPC name and the HMC network
name of the machine the operating system is running on. This information
is retrieved with the operation communication parameters (OCF) sclp

Remove the tape block device driver. It's not of real use but has
already created some confusion when users wanted to access tape devices
and used the block device nodes instead of the character device nodes.
Also remove the whole tape documentation since it's completely outdated
and we have the device drivers book which is the place where everything
is properly documented.

In ccw_device_set_online we basically start path verification and
wait for the device to reach a final state. If it turns out that the
device has no useable path we schedule the deregistration of the
device (which is still in an non-final state) and wake up the waiting
process. The deregistration process will set a final state, but if
the wake up happens to be prior to this, the device will hang forever
in ccw_device_set_online.
To fix this just set the final NOT_OPER state prior to the scheduled
deregistration of the device.

This patch uses the ndo_set_features callback during normal device

Set rx csum default to hw checksumming again.
Remove sysfs interface for rx csum (checksumming) and TSO (large_send).
With the new hw_features it does not work to keep the old sysfs
interface in parallel. Convert options.checksum_type to new hw_features.

The termination of running CQR caused by reserve/release operations
may lead to an IO error if reserve/release is done in a tight loop.
Prevent this by increasing the retry counter after termination.

Force dev_alloc_name() to be called from register_netdevice() by
dev_get_valid_name(). That allows to remove multiple explicit
dev_alloc_name() calls.
The possibility to call dev_alloc_name in advance remains.
This also fixes veth creation regresion caused by

pfault, dasd diag and virtio all use the same external interrupt number.
The respective interrupt handlers decide by the subcode if they are
meant to handle the interrupt.
Counting is currently done before looking at the subcode which means
each handler counts an interrupt even if it is not handling it.
Fix this by moving the kstat code after the code which looks at the
subcode.

In-kernel disk event polling doesn't matter for legacy/fringe drivers
and may lead to infinite event loop if ->check_events() implementation
generates events on level condition instead of edge.
Now that block layer supports suppressing exporting unlisted events,
simply leaving disk->events cleared allows these drivers to keep the
internal revalidation behavior intact while avoiding weird
interactions with userland event handler.

The qdio hardware may surpress further interrupts as long as a SBAL is in
the error state. That can lead to unnotified data in the SBALs following
the error state. To prevent this behaviour change the SBAL[s] in error
state immediately to another program owned state so interrupts are again
received for further traffic on the device.

The dasd_open function uses the private_data pointer of the gendisk to
find the dasd_block structure that matches the gendisk. When a DASD
device is set offline, we set the private_data pointer of the gendisk
to NULL and later remove the dasd_block structure, but there is still
a small race window, in which dasd_open could first read a pointer
from the private_data field and then try to use it, after the structure
has already been freed.
To close this race window, we will store a pointer to the dasd_devmap
structure of the base device in the private_data field. The devmap
entries are not deleted, and we already have proper locking and
reference counting in place, so that we can safely get from a devmap
pointer to the dasd_device and dasd_block structures of the device.

The cio_ignore purge function is intended to only remove CCW devices
which are in the offline state. There is a time frame after the purge
function finished where a CCW device is scheduled for removal but
still accessible. When the device is set online during this time
frame, it may first appear online before it is then removed.
Fix this by preventing that CCW devices can be set online while there
is work (such as removal triggered by the purge function) for it
pending. Also ensure that the purge function does not schedule devices
for removal which are in the process of being set online.
