7d8d001407 blk-stat: fix blk_stat_sum() if all samples are batched

9c62110454 blk-mq-sched: don't run the queue async from blk_mq_try_issue_directly()
If we have scheduling enabled, we jump directly to insert-and-run.
That's fine, but we run the queue async and we don't pass in information
on whether we can block from this context or not. Fixup both these
cases.

0067d4b020 blk-mq: Fix tagset reinit in the presence of cpu hot-unplug
In case cpu was unplugged, we need to make sure not to assume
that the tags for that cpu are still allocated. so check
for null tags when reinitializing a tagset.

f5fe1b5190 blk: Ensure users for current->bio_list can see the full list.
Commit 79bd99596b ("blk: improve order of bio handling in generic_make_request()")
changed current->bio_list so that it did not contain *all* of the
queued bios, but only those submitted by the currently running
make_request_fn.

There are two places which walk the list and requeue selected bios,
and others that check if the list is empty.  These are no longer
correct.

So redefine current->bio_list to point to an array of two lists, which
contain all queued bios, and adjust various code to test or walk both
lists.

79bd99596b blk: improve order of bio handling in generic_make_request()
To avoid recursion on the kernel stack when stacked block devices
are in use, generic_make_request() will, when called recursively,
queue new requests for later handling.  They will be handled when the
make_request_fn for the current bio completes.

If any bios are submitted by a make_request_fn, these will ultimately
be handled seqeuntially.  If the handling of one of those generates
further requests, they will be added to the end of the queue.

This strict first-in-first-out behaviour can lead to deadlocks in
various ways, normally because a request might need to wait for a
previous request to the same device to complete.  This can happen when
they share a mempool, and can happen due to interdependencies
particular to the device.  Both md and dm have examples where this happens.

These deadlocks can be erradicated by more selective ordering of bios.
Specifically by handling them in depth-first order.  That is: when the
handling of one bio generates one or more further bios, they are
handled immediately after the parent, before any siblings of the
parent.  That way, when generic_make_request() calls make_request_fn
for some particular device, we can be certain that all previously
submited requests for that device have been completely handled and are
not waiting for anything in the queue of requests maintained in
generic_make_request().

An easy way to achieve this would be to use a last-in-first-out stack
instead of a queue.  However this will change the order of consecutive
bios submitted by a make_request_fn, which could have unexpected consequences.
Instead we take a slightly more complex approach.
A fresh queue is created for each call to a make_request_fn.  After it completes,
any bios for a different device are placed on the front of the main queue, followed
by any bios for the same device, followed by all bios that were already on
the queue before the make_request_fn was called.
This provides the depth-first approach without reordering bios on the same level.

This, by itself, it not enough to remove all deadlocks.  It just makes
it possible for drivers to take the extra step required themselves.

To avoid deadlocks, drivers must never risk waiting for a request
after submitting one to generic_make_request.  This includes never
allocing from a mempool twice in the one call to a make_request_fn.

A common pattern in drivers is to call bio_split() in a loop, handling
the first part and then looping around to possibly split the next part.
Instead, a driver that finds it needs to split a bio should queue
(with generic_make_request) the second part, handle the first part,
and then return.  The new code in generic_make_request will ensure the
requests to underlying bios are processed first, then the second bio
that was split off.  If it splits again, the same process happens.  In
each case one bio will be completely handled before the next one is attempted.

With this is place, it should be possible to disable the
punt_bios_to_recover() recovery thread for many block devices, and
eventually it may be possible to remove it completely.

c01228db4b Revert "scsi, block: fix duplicate bdi name registration crashes"
This reverts commit 0dba1314d4. It causes
leaking of device numbers for SCSI when SCSI registers multiple gendisks
for one request_queue in succession. It can be easily reproduced using
Omar's script [1] on kernel with CONFIG_DEBUG_TEST_DRIVER_REMOVE.
Furthermore the protection provided by this commit is not needed anymore
as the problem it was fixing got also fixed by commit 165a5e22fa
"block: Move bdi_unregister() to del_gendisk()".

90f16fddcc block: Make del_gendisk() safer for disks without queues
Commit 165a5e22fa "block: Move bdi_unregister() to del_gendisk()"
added disk->queue dereference to del_gendisk(). Although del_gendisk()
is not supposed to be called without disk->queue valid and
blk_unregister_queue() warns in that case, this change will make it oops
instead. Return to the old more robust behavior of just warning when
del_gendisk() gets called for gendisk with disk->queue being NULL.

6c8b232efe blk-mq: make lifetime consistent between hctx and its kobject
This patch removes kobject_put() over hctx in __blk_mq_unregister_dev(),
and trys to keep lifetime consistent between hctx and hctx's kobject.

Now blk_mq_sysfs_register() and blk_mq_sysfs_unregister() become
totally symmetrical, and kobject's refcounter drops to zero just
when the hctx is freed.

7ea5fe31c1 blk-mq: make lifetime consitent between q/ctx and its kobject
Currently from kobject view, both q->mq_kobj and ctx->kobj can
be released during one cycle of blk_mq_register_dev() and
blk_mq_unregister_dev(). Actually, sw queue's lifetime is
same with its request queue's, which is covered by request_queue->kobj.

So we don't need to call kobject_put() for the two kinds of
kobject in __blk_mq_unregister_dev(), instead we do that
in release handler of request queue.

once, instead of doing that each add_disk context.

Also this patch removes clearing of ctx in blk_mq_init_cpu_queues()
because percpu allocator fills zero to allocated variable.

This patch fixes one issue[1] reported from Omar.

  <linux/sched.h> header footprint, to speed up the kernel build and to

  size goes down from a previous ~0.68 MB (~22K lines) to ~0.45 MB (~15K
  lines), which is around 40% faster to build on typical configs.

  eliminated quirks, backmerged fixes plus I rebased it to an upstream
  SHA1 from yesterday that includes most changes queued up in -next plus
  all sched.h changes that were pending from Andrew.

  I've re-tested the series both on x86 and on cross-arch defconfigs,
  and did a bisectability test at a number of random points.

  I tried to test as many build configurations as possible, but some
  build breakage is probably still left - but it should be mostly
  limited to architectures that have no cross-compiler binaries

165a5e22fa block: Move bdi_unregister() to del_gendisk()
Commit 6cd18e711d "block: destroy bdi before blockdev is
unregistered." moved bdi unregistration (at that time through
bdi_destroy()) from blk_release_queue() to blk_cleanup_queue() because
it needs to happen before blk_unregister_region() call in del_gendisk()
for MD. SCSI though will free up the device number from sd_remove()
called through a maze of callbacks from device_del() in
__scsi_remove_device() before blk_cleanup_queue() and thus similar races
as described in 6cd18e711d can happen for SCSI as well as reported by
Omar [1].

Moving bdi_unregister() to del_gendisk() works for MD and fixes the
problem for SCSI since del_gendisk() gets called from sd_remove() before
freeing the device number.

This also makes device_add_disk() (calling bdi_register_owner()) more
symmetric with del_gendisk().

113285b473 blk-mq: ensure that bd->last is always set correctly
When drivers are called with a request in blk-mq, blk-mq flags the
state such that the driver knows if this is the last request in
this call chain or not. The driver can then use that information
to defer kicking off IO until bd->last is true. However, with blk-mq
and scheduling, we need to allocate a driver tag for a request before
it can be issued. If we fail to allocate such a tag, we could end up
in the situation where the last request issued did not have
bd->last == true set. This can then cause a driver hang.

This fixes a hang with virtio-blk, which uses bd->last as a hint
on whether to kick the queue or not.

7b36a7189f block: don't call ioc_exit_icq() with the queue lock held for blk-mq
For legacy scheduling, we always call ioc_exit_icq() with both the
ioc and queue lock held. This poses a problem for blk-mq with
scheduling, since the queue lock isn't what we use in the scheduler.
And since we don't need the queue lock held for ioc exit there,
don't grab it and leave any extra locking up to the blk-mq scheduler.

6d2809d51a blk-mq: make blk_mq_alloc_request_hctx() allocate a scheduler request
blk_mq_alloc_request_hctx() allocates a driver request directly, unlike
its blk_mq_alloc_request() counterpart. It also crashes because it
doesn't update the tags->rqs map.

Fix it by making it allocate a scheduler request.

blk_mq_tags/requests of specific hardware queue are mostly used in
specific cpus, which might not be in the same numa node as disk. For
example, a nvme card is in node 0. half hardware queue will be used by

5b5e0928f7 lib/vsprintf.c: remove %Z support
Now that %z is standartised in C99 there is no reason to support %Z.
Unlike %L it doesn't even make format strings smaller.

Use BUILD_BUG_ON in a couple ATM drivers.

In case anyone didn't notice lib/vsprintf.o is about half of SLUB which
is in my opinion is quite an achievement.  Hopefully this patch inspires
someone else to trim vsprintf.c more.

d38d351555 blk-mq-sched: separate mark hctx and queue restart operations
In blk_mq_sched_dispatch_requests(), we call blk_mq_sched_mark_restart()
after we dispatch requests left over on our hardware queue dispatch
list. This is so we'll go back and dispatch requests from the scheduler.
In this case, it's only necessary to restart the hardware queue that we
are running; there's no reason to run other hardware queues just because
we are using shared tags.

So, split out blk_mq_sched_mark_restart() into two operations, one for
just the hardware queue and one for the whole request queue. The core
code only needs the hctx variant, but I/O schedulers will want to use
both.

This also requires adjusting blk_mq_sched_restart_queues() to always
check the queue restart flag, not just when using shared tags.

da55f2cc78 blk-mq: use sbq wait queues instead of restart for driver tags
Commit 50e1dab86a ("blk-mq-sched: fix starvation for multiple hardware
queues and shared tags") fixed one starvation issue for shared tags.
However, we can still get into a situation where we fail to allocate a
tag because all tags are allocated but we don't have any pending
requests on any hardware queue.

One solution for this would be to restart all queues that share a tag
map, but that really sucks. Ideally, we could just block and wait for a
tag, but that isn't always possible from blk_mq_dispatch_rq_list().

However, we can still use the struct sbitmap_queue wait queues with a
custom callback instead of blocking. This has a few benefits:

1. It avoids iterating over all hardware queues when completing an I/O,
   which the current restart code has to do.
2. It benefits from the existing rolling wakeup code.
3. It avoids punting to another thread just to have it block.

4f1244c829 block/sed-opal: allocate struct opal_dev dynamically
Insted of bloating the containing structure with it all the time this
allocates struct opal_dev dynamically.  Additionally this allows moving
the definition of struct opal_dev into sed-opal.c.  For this a new
private data field is added to it that is passed to the send/receive
callback.  After that a lot of internals can be made private as well.

7520872c0c block: don't defer flushes on blk-mq + scheduling
For blk-mq with scheduling, we can potentially end up with ALL
driver tags assigned and sitting on the flush queues. If we
defer because of an inlfight data request, then we can deadlock
if that data request doesn't already have a tag assigned.

This fixes a deadlock with running the xfs/297 xfstest, where
thousands of syncs can cause the drive queue to stall.

64765a75ef blk-mq-sched: ask scheduler for work, if we failed dispatching leftovers
Usually we don't ask the scheduler for work, if we already have
leftovers on the dispatch list. This is done to leave work on
the scheduler side for as long as possible, for proper merging.
But if we do have work leftover but didn't dispatch anything,
then we should ask the scheduler since we could potentially
issue requests from that.

wbt_disable_default() calls del_timer_sync() to wait for the wbt
timer to finish before disabling throttling. We can't do this with

b410aff2bd block: do not allow updates through sysfs until registration completes
When a new disk shows up, sysfs queue directory is created before elevator
is registered. This allows a user to attempt a scheduler switch even though
the initial registration hasn't completed yet.

In one scenario, blk_register_queue() calls elv_register_queue() and
right before cfq_registered_queue() is called, another process executes
elevator_switch() and replaces q->elevator with deadline scheduler. When
cfq_registered_queue() executes it interprets e->elevator_data as struct
cfq_data even though it is actually struct deadline_data.

Grab q->sysfs_lock in blk_register_queue() to synchronize with sysfs
callers.

1e739730c5 block: optionally merge discontiguous discard bios into a single request
Add a new merge strategy that merges discard bios into a request until the
maximum number of discard ranges (or the maximum discard size) is reached
from the plug merging code.  I/O scheduler merging is not wired up yet
but might also be useful, although not for fast devices like NVMe which
are the only user for now.

Note that for now we don't support limiting the size of each discard range,
but if needed that can be added later.

80c6b15732 blk-mq-sched: (un)register elevator when (un)registering queue
I noticed that when booting with a default blk-mq I/O scheduler, the
/sys/block/*/queue/iosched directory was missing. However, switching
after boot did create the directory. This is because we skip the initial
elevator register/unregister when we don't have a ->request_fn(), but we
should still do it for the ->mq_ops case.

eeeefd4184 block: don't try Write Same from __blkdev_issue_zeroout
Write Same can return an error asynchronously if it turns out the
underlying SCSI device does not support Write Same, which makes a
proper fallback to other methods in __blkdev_issue_zeroout impossible.

at all from __blkdev_issue_zeroout as a non-invasive workaround.

e4d750c977 block: free merged request in the caller
If we end up doing a request-to-request merge when we have completed
a bio-to-request merge, we free the request from deep down in that
path. For blk-mq-sched, the merge path has to hold the appropriate
lock, but we don't need it for freeing the request. And in fact
holding the lock is problematic, since we are now calling the
mq sched put_rq_private() hook with the lock held. Other call paths
do not hold this lock.

Fix this inconsistency by ensuring that the caller frees a merged
request. Then we can do it outside of the lock, making it both more
efficient and fixing the blk-mq-sched problem of invoking parts of
the scheduler with an unknown lock state.

When we attempt to merge request-to-request, we return a 0/1 if we
ended up merging or not. Change that to return the pointer to the
request that we freed. We will use this to move the freeing of
that request out of the merge logic, so that callers can drop
locks before freeing the request.

There should be no functional changes in this patch.

f44f1ab5a2 block: Unhash block device inodes on gendisk destruction
Currently, block device inodes stay around after corresponding gendisk
hash died until memory reclaim finds them and frees them. Since we will
make block device inode pin the bdi, we want to free the block device
inode as soon as the device goes away so that bdi does not stay around
unnecessarily. Furthermore we need to avoid issues when new device with
the same major,minor pair gets created since reusing the bdi structure
would be rather difficult in this case.

Unhashing block device inode on gendisk destruction nicely deals with
these problems. Once last block device inode reference is dropped (which
may be directly in del_gendisk()), the inode gets evicted. Furthermore if
the major,minor pair gets reallocated, we are guaranteed to get new
block device inode even if old block device inode is not yet evicted and
thus we avoid issues with possible reuse of bdi.

12d70958a2 blk-mq: don't fail allocating driver tag for stopped hw queue
We rely on blk_mq_get_driver_tag() not failing if 'wait' is true,
but it currently fails in that case if the queue happens to be
stopped at the time of the call.

We don't need to check for stopped here, it's just assigning
the tag. If the queue is stopped, we'll handle it when
attempting to run the queue.

This fixes a stall/crash on flush intensive workloads, where
we proceed to process a flush that doesn't have a valid tag
assigned.

aebf526b53 block: fold cmd_type into the REQ_OP_ space
Instead of keeping two levels of indirection for requests types, fold it
all into the operations.  The little caveat here is that previously
cmd_type only applied to struct request, while the request and bio op
fields were set to plain REQ_OP_READ/WRITE even for passthrough
operations.

Instead this patch adds new REQ_OP_* for SCSI passthrough and driver
private requests, althought it has to add two for each so that we
can communicate the data in/out nature of the request.

5ea708d15a block: simplify blk_init_allocated_queue
Return an errno value instead of the passed in queue so that the callers
don't have to keep track of two queues, and move the assignment of the
request_fn and lock to the caller as passing them as argument doesn't
simplify anything.  While we're at it also remove two pointless NULL
assignments, given that the request structure is zeroed on allocation.

e6f7f93d58 block: fix elevator init check
We can't initalize the elevator fields for flushes as flush share space
in struct request with the elevator data.  But currently we can't
communicate that a request is a flush through blk_get_request as we
can only pass READ or WRITE, and the low-level code looks at the
possible NULL bio to check for a flush.

Fix this by allowing to pass any block op and flags, and by checking for
the flush flags in __get_request.

c13660a08c blk-mq-sched: change ->dispatch_requests() to ->dispatch_request()
When we invoke dispatch_requests(), the scheduler empties everything
into the passed in list. This isn't always a good thing, since it
means that we remove items that we could have potentially merged
with.

Change the function to dispatch single requests at the time. If
we do that, we can backoff exactly at the point where the device
can't consume more IO, and leave the rest with the scheduler for
better merging and future dispatch decision making.

50e1dab86a blk-mq-sched: fix starvation for multiple hardware queues and shared tags
If we have both multiple hardware queues and shared tag map between
devices, we need to ensure that we propagate the hardware queue
restart bit higher up. This is because we can get into a situation
where we don't have any IO pending on a hardware queue, yet we fail
getting a tag to start new IO. If that happens, it's not enough to
mark the hardware queue as needing a restart, we need to bubble
that up to the higher level queue as well.

Add support for growing the tags associated with a hardware queue, for
the scheduler tags. Currently we only support resizing within the
limits of the original depth, change that so we can grow it as well by
allocating and replacing the existing scheduler tag set.

This is similar to how we could increase the software queue depth with
the legacy IO stack and schedulers.

7e79dadce2 blk-mq: stop hardware queue in blk_mq_delay_queue()
The run handler we register for the delayed work requires that the
queue be stopped, yet we leave that up to the caller. Let's move
it into blk_mq_delay_queue() itself, so that the API is sane.

This fixes a stall with SCSI, where it calls blk_mq_delay_queue()
without having stopped the queue. Hence the queue is never run.

38dbb7dd4d blk-cgroup: don't quiesce the queue on policy activate/deactivate
There's no potential harm in quiescing the queue, but it also doesn't
buy us anything. And we can't run the queue async for policy
deactivate, since we could be in the path of tearing the queue down.
If we schedule an async run of the queue at that time, we're racing
with queue teardown AFTER having we've already torn most of it down.

This adds a set of hooks that intercepts the blk-mq path of
allocating/inserting/issuing/completing requests, allowing
us to develop a scheduler within that framework.

We reuse the existing elevator scheduler API on the registration
side, but augment that with the scheduler flagging support for
the blk-mq interfce, and with a separate set of ops hooks for MQ
devices.

We split driver and scheduler tags, so we can run the scheduling
independently of device queue depth.

c5082b70ad partitions/efi: Fix integer overflow in GPT size calculation
If a GUID Partition Table claims to have more than 2**25 entries, the
calculation of the partition table size in alloc_read_gpt_entries() will
overflow a 32-bit integer and not enough space will be allocated for the
table.

Nothing seems to get written out of bounds, but later efi_partition() will
read up to 32768 bytes from a 128 byte buffer, possibly OOPSing or exposing
information to /proc/partitions and uevents.

The problem exists on both 64-bit and 32-bit platforms.

Fix the overflow and also print a meaningful debug message if the table
size is too large.

bef13315e9 block: don't try to discard from __blkdev_issue_zeroout
Discard can return -EIO asynchronously if the alignment for the request
isn't suitable for the driver, which makes a proper fallback to other
methods in __blkdev_issue_zeroout impossible.  Thus only issue a sync
discard from blkdev_issue_zeroout an don't try discard at all from
__blkdev_issue_zeroout as a non-invasive workaround.

One more reason why abusing discard for zeroing must die..

f99e86485c block: Rename blk_queue_zone_size and bdev_zone_size
All block device data fields and functions returning a number of 512B
sectors are by convention named xxx_sectors while names in the form
xxx_size are generally used for a number of bytes. The blk_queue_zone_size
and bdev_zone_size functions were not following this convention so rename
them.

No functional change is introduced by this patch.

8b0e195314 ktime: Cleanup ktime_set() usage
ktime_set(S,N) was required for the timespec storage type and is still
useful for situations where a Seconds and Nanoseconds part of a time value
needs to be converted. For anything where the Seconds argument is 0, this

Partitions that are not aligned to the blocksize of a device may cause
invalid I/O requests because the blocklayer cares only about alignment
within the partition when building requests on partitions.

When reading/writing one 4k block of the partition this maps to
reading/writing with an offset of 512 byte of the device leading to
unaligned requests for the device which in turn may cause unexpected
behavior of the device driver.

For DASD devices we have to translate the block number into a cylinder,
head, record format. The unaligned requests lead to wrong calculation
and therefore to misdirected I/O. In a "good" case this leads to I/O
errors because the underlying hardware detects the wrong addressing.
In a worst case scenario this might destroy data on the device.

To prevent partitions that are not aligned to the physical blocksize
of a device check for the alignment in the blkpg_ioctl.

The WRITE_SAME commands are not present in the blk_default_cmd_filter
write_ok list, and thus are failed with -EPERM when the SG_IO ioctl()
is executed without CAP_SYS_RAWIO capability (e.g., unprivileged users).
[ sg_io() -> blk_fill_sghdr_rq() > blk_verify_command() -> -EPERM ]

The problem can be reproduced with the sg_write_same command

So, this patch adds the WRITE_SAME commands to the list, in order
for the SG_IO ioctl to finish successfully:

That case happens to be exercised by QEMU KVM guests with 'scsi-block' devices
(qemu "-device scsi-block" [1], libvirt "<disk type='block' device='lun'>" [2]),
which employs the SG_IO ioctl() and runs as an unprivileged user (libvirt-qemu).

In that scenario, when a filesystem (e.g., ext4) performs its zero-out calls,
which are translated to write-same calls in the guest kernel, and then into
SG_IO ioctls to the host kernel, SCSI I/O errors may be observed in the guest:

d1b1cea1e5 blk-mq: Fix failed allocation path when mapping queues
In blk_mq_map_swqueue, there is a memory optimization that frees the
tags of a queue that has gone unmapped.  Later, if that hctx is remapped
after another topology change, the tags need to be reallocated.

If this allocation fails, a simple WARN_ON triggers, but the block layer
ends up with an active hctx without any corresponding set of tags.
Then, any income IO to that hctx can trigger an Oops.

I can reproduce it consistently by running IO, flipping CPUs on and off
and eventually injecting a memory allocation failure in that path.

In the fix below, if the system experiences a failed allocation of any
hctx's tags, we remap all the ctxs of that queue to the hctx_0, which
should always keep it's tags.  There is a minor performance hit, since
our mapping just got worse after the error path, but this is
the simplest solution to handle this error path.  The performance hit
will disappear after another successful remap.

I considered dropping the memory optimization all together, but it
seemed a bad trade-off to handle this very specific error case.

This should apply cleanly on top of Jens' for-next branch.

36e1f3d107 blk-mq: Avoid memory reclaim when remapping queues
While stressing memory and IO at the same time we changed SMT settings,
we were able to consistently trigger deadlocks in the mm system, which
froze the entire machine.

I think that under memory stress conditions, the large allocations
performed by blk_mq_init_rq_map may trigger a reclaim, which stalls
waiting on the block layer remmaping completion, thus deadlocking the
system.  The trace below was collected after the machine stalled,
waiting for the hotplug event completion.

The simplest fix for this is to make allocations in this path
non-reclaimable, with GFP_NOIO.  With this patch, We couldn't hit the
issue anymore.

This should apply on top of Jens's for-next branch cleanly.

9491ae4aad mm: don't cap request size based on read-ahead setting
We ran into a funky issue, where someone doing 256K buffered reads saw
128K requests at the device level.  Turns out it is read-ahead capping
the request size, since we use 128K as the default setting.  This
doesn't make a lot of sense - if someone is issuing 256K reads, they
should see 256K reads, regardless of the read-ahead setting, if the
underlying device can support a 256K read in a single command.

This patch introduces a bdi hint, io_pages.  This is the soft max IO
size for the lower level, I've hooked it up to the bdev settings here.
Read-ahead is modified to issue the maximum of the user request size,
and the read-ahead max size, but capped to the max request size on the
device side.  The latter is done to avoid reading ahead too much, if the
application asks for a huge read.  With this patch, the kernel behaves
like the application expects.

f9d03f96b9 block: improve handling of the magic discard payload
Instead of allocating a single unused biovec for discard requests, send
them down without any payload.  Instead we allow the driver to add a
"special" payload using a biovec embedded into struct request (unioned
over other fields never used while in the driver), and overloading
the number of segments for this case.

This has a couple of advantages:

 - we don't have to allocate the bio_vec
 - the amount of special casing for discard requests in the block
   layer is significantly reduced
 - using this same scheme for other request types is trivial,
   which will be important for implementing the new WRITE_ZEROES
   op on devices where it actually requires a payload (e.g. SCSI)
 - we can get rid of playing games with the request length, as
   we'll never touch it and completions will work just fine
 - it will allow us to support ranged discard operations in the
   future by merging non-contiguous discard bios into a single
   request
 - last but not least it removes a lot of code

This patch is the common base for my WIP series for ranges discards and to
remove discard_zeroes_data in favor of always using REQ_OP_WRITE_ZEROES,
so it would be good to get it in quickly.

  EXT4-fs (dm-1): This should not happen!! Data will be lost

Due to the following fallthrough introduced with
commit 2d253440b5 ("block: Define zoned block device operations"),
generic_make_request_checks() would accept a REQ_OP_WRITE_SAME bio only
if the block device supports "write same" *and* is a zoned one:

comment, seems to be unintended anyway.

a6f0788ec2 block: add support for REQ_OP_WRITE_ZEROES
This adds a new block layer operation to zero out a range of
LBAs. This allows to implement zeroing for devices that don't use
either discard with a predictable zero pattern or WRITE SAME of zeroes.
The prominent example of that is NVMe with the Write Zeroes command,
but in the future, this should also help with improving the way
zeroing discards work. For this operation, suitable entry is exported in
sysfs which indicate the number of maximum bytes allowed in one
write zeroes operation by the device.

b02d8aaea1 block: Check partition alignment on zoned block devices
Both blkdev_report_zones and blkdev_reset_zones can operate on a partition of
a zoned block device. However, the first and last zones reported for a
partition make sense only if the partition start sector and size are aligned
on the device zone size. The same applies for zone reset. Resetting the first
or the last zone of a partition straddling zones may impact neighboring
partitions. Finally, if a partition start sector is not at the beginning of a
sequential zone, it will be impossible to write to the first sectors of the
partition on a host-managed device.
Avoid all these problems and incoherencies by ignoring partitions that are not
zone aligned.

Note: Even with CONFIG_BLK_DEV_ZONED disabled, bdev_is_zoned() will report the
correct disk zoning type (host-aware, host-managed or none) but
bdev_zone_size() will always return 0 for zoned block devices (i.e. the zone
size is unknown). So test this as a way to ensure that a zoned block device is
being handled as such. As a result, for a host-aware devices, unaligned zone
partitions will be accepted with CONFIG_BLK_DEV_ZONED disabled. That is, the
disk will be treated as a regular block device (as it should). If zoned block
device support is enabled, only aligned partitions will be accepted.

Currently there's no way to enable wbt if it's not enabled in the
kernel config by default for a device. Allow a write to the
'wbt_lat_usec' queue sysfs file to enable wbt.

device is CFQ managed and it was turned off by default.

blkcg allocates some per-cgroup data structures with GFP_NOWAIT and
when that fails falls back to operations which aren't specific to the
cgroup.  Occassional failures are expected under pressure and falling
back to non-cgroup operation is the right thing to do.

Unfortunately, I forgot to add __GFP_NOWARN to these allocations and
these expected failures end up creating a lot of noise.  Add
__GFP_NOWARN.

If a ZBC device is partitioned and operations are performed on the partition
the zone information is rebased to the partition, however the zone reset
is not mapped from the partition to device as are other operations.

This causes the API (report zones / reset zone) to be unbalanced in this
regard. Checking for the zone reset op code explicitly will balance the
API.

64f1c21e86 blk-mq: make the polling code adaptive
The previous commit introduced the hybrid sleep/poll mode. Take
that one step further, and use the completion latencies to
automatically sleep for half the mean completion time. This is
a good approximation.

This changes the 'io_poll_delay' sysfs file a bit to expose the
various options. Depending on the value, the polling code will
behave differently:

-1	Never enter hybrid sleep mode
 0	Use half of the completion mean for the sleep delay
>0	Use this specific value as the sleep delay

06426adf07 blk-mq: implement hybrid poll mode for sync O_DIRECT
This patch enables a hybrid polling mode. Instead of polling after IO
submission, we can induce an artificial delay, and then poll after that.
For example, if the IO is presumed to complete in 8 usecs from now, we
can sleep for 4 usecs, wake up, and then do our polling. This still puts
a sleep/wakeup cycle in the IO path, but instead of the wakeup happening
after the IO has completed, it'll happen before. With this hybrid
scheme, we can achieve big latency reductions while still using the same
(or less) amount of CPU.

0a6219a95f block: deal with stale req count of plug list
In both legacy and mq path, req count of plug list is computed
before allocating request, so the number can be stale when falling
back to slept allocation, also the new introduced wbt can sleep
too.

This patch deals with the case by checking if plug list becomes
empty, and fixes the KASAN report of 'BUG: KASAN: stack-out-of-bounds'
which is introduced by Shaohua's patches of dispatching big request.

066a4a73ce blk-mq: blk_mq_try_issue_directly() should lookup hardware queue
A previous commit changed this to pass in the hardware queue, but
it was using the wrong hardware queue. Hence a request that was
allocated on one hardware queue ended up being issued on another
one, and that caused IO timeouts and oopses on some drivers. Since
the request holds hardware queue private resources, like a tag,
we can't just issue it on a different hardware queue.

87760e5eef block: hook up writeback throttling
Enable throttling of buffered writeback to make it a lot
more smooth, and has way less impact on other system activity.
Background writeback should be, by definition, background
activity. The fact that we flush huge bundles of it at the time
means that it potentially has heavy impacts on foreground workloads,
which isn't ideal. We can't easily limit the sizes of writes that
we do, since that would impact file system layout in the presence
of delayed allocation. So just throttle back buffered writeback,
unless someone is waiting for it.

The algorithm for when to throttle takes its inspiration in the
CoDel networking scheduling algorithm. Like CoDel, blk-wb monitors
the minimum latencies of requests over a window of time. In that
window of time, if the minimum latency of any request exceeds a
given target, then a scale count is incremented and the queue depth
is shrunk. The next monitoring window is shrunk accordingly. Unlike
CoDel, if we hit a window that exhibits good behavior, then we
simply increment the scale count and re-calculate the limits for that
scale value. This prevents us from oscillating between a
close-to-ideal value and max all the time, instead remaining in the
windows where we get good behavior.

Unlike CoDel, blk-wb allows the scale count to to negative. This
happens if we primarily have writes going on. Unlike positive
scale counts, this doesn't change the size of the monitoring window.
When the heavy writers finish, blk-bw quickly snaps back to it's
stable state of a zero scale count.

The patch registers a sysfs entry, 'wb_lat_usec'. This sets the latency
target to me met. It defaults to 2 msec for non-rotational storage, and
75 msec for rotational storage. Setting this value to '0' disables
blk-wb. Generally, a user would not have to touch this setting.

We don't enable WBT on devices that are managed with CFQ, and have
a non-root block cgroup attached. If we have a proportional share setup
on this particular disk, then the wbt throttling will interfere with
that. We don't have a strong need for wbt for that case, since we will
rely on CFQ doing that for us.

We can hook this up to the block layer, to help throttle buffered
writes.

wbt registers a few trace points that can be used to track what is
happening in the system:

wbt_lat: 259:0: latency 2446318
wbt_stat: 259:0: rmean=2446318, rmin=2446318, rmax=2446318, rsamples=1,
               wmean=518866, wmin=15522, wmax=5330353, wsamples=57
wbt_step: 259:0: step down: step=1, window=72727272, background=8, normal=16, max=32

This shows a sync issue event (wbt_lat) that exceeded it's time. wbt_stat
dumps the current read/write stats for that window, and wbt_step shows a
step down event where we now scale back writes. Each trace includes the
device, 259:0 in this case.

cf43e6be86 block: add scalable completion tracking of requests
For legacy block, we simply track them in the request queue. For
blk-mq, we track them on a per-sw queue basis, which we can then
sum up through the hardware queues and finally to a per device
state.

The stats are tracked in, roughly, 0.1s interval windows.

Add sysfs files to display the stats.

The feature is off by default, to avoid any extra overhead. In-kernel
users of it can turn it on by setting QUEUE_FLAG_STATS in the queue
flags. We currently don't turn it on if someone just reads any of
the stats files, that is something we could add as well.

If we insert a flush request, we clear REQ_PREFLUSH and/or REQ_FUA,
depending on flush settings. Since op_is_sync() factors those flags
in for deciding whether this request is sync or not, we should
set REQ_SYNC to avoid screwing up this accounting.

This should be less fragile.

c02ebfdddb blk-mq: Always schedule hctx->next_cpu
Commit 0e87e58bf6 ("blk-mq: improve warning for running a queue on the
wrong CPU") attempts to avoid triggering the WARN_ON in
__blk_mq_run_hw_queue when the expected CPU is dead.  Problem is, in the
last batch execution before round robin, blk_mq_hctx_next_cpu can
schedule a dead CPU and also update next_cpu to the next alive CPU in
the mask, which will trigger the WARN_ON despite the previous
workaround.

The following patch fixes this scenario by always scheduling the value
in hctx->next_cpu.  This changes the moment when we round-robin the CPU
running the hctx, but it really doesn't matter, since it still executes
BLK_MQ_CPU_WORK_BATCH times in a row before switching to another CPU.

d278d4a889 block: add code to track actual device queue depth
For blk-mq, ->nr_requests does track queue depth, at least at init
time. But for the older queue paths, it's simply a soft setting.
On top of that, it's generally larger than the hardware setting
on purpose, to allow backup of requests for merging.

Fill a hole in struct request with a 'queue_depth' member, that
drivers can call to more closely inform the block layer of the
real queue depth.

50d24c3440 block: immediately dispatch big size request
Currently block plug holds up to 16 non-mergeable requests. This makes
sense if the request size is small, eg, reduce lock contention. But if
request size is big enough, we don't need to worry about lock
contention. Holding such request makes no sense and it lows the disk
utilization.

In practice, this improves 10% throughput for my raid5 sequential write
workload.

The size (128k) is arbitrary right now, but it makes sure lock
contention is small. This probably could be more intelligent, eg, check
average request size holded. Since this is mainly for sequential IO,
probably not worthy.

V2: check the last request instead of the first request, so as long as
there is one big size request we flush the plug.

6a83e74d21 blk-mq: Introduce blk_mq_quiesce_queue()
blk_mq_quiesce_queue() waits until ongoing .queue_rq() invocations
have finished. This function does *not* wait until all outstanding
requests have finished (this means invocation of request.end_io()).
The algorithm used by blk_mq_quiesce_queue() is as follows:
* Hold either an RCU read lock or an SRCU read lock around
  .queue_rq() calls. The former is used if .queue_rq() does not
  block and the latter if .queue_rq() may block.
* blk_mq_quiesce_queue() first calls blk_mq_stop_hw_queues()
  followed by synchronize_srcu() or synchronize_rcu(). The latter
  call waits for .queue_rq() invocations that started before
  blk_mq_quiesce_queue() was called.
* The blk_mq_hctx_stopped() calls that control whether or not
  .queue_rq() will be called are called with the (S)RCU read lock
  held. This is necessary to avoid race conditions against
  blk_mq_quiesce_queue().

52d7f1b5c2 blk-mq: Avoid that requeueing starts stopped queues
Since blk_mq_requeue_work() starts stopped queues and since
execution of this function can be scheduled after a queue has
been stopped it is not possible to stop queues without using
an additional state variable to track whether or not the queue
has been stopped. Hence modify blk_mq_requeue_work() such that it
does not start stopped queues. My conclusion after a review of
the blk_mq_stop_hw_queues() and blk_mq_{delay_,}kick_requeue_list()
callers is as follows:
* In the dm driver starting and stopping queues should only happen
  if __dm_suspend() or __dm_resume() is called and not if the
  requeue list is processed.
* In the SCSI core queue stopping and starting should only be
  performed by the scsi_internal_device_block() and
  scsi_internal_device_unblock() functions but not by any other
  function. Although the blk_mq_stop_hw_queue() call in
  scsi_queue_rq() may help to reduce CPU load if a LLD queue is
  full, figuring out whether or not a queue should be restarted
  when requeueing a command would require to introduce additional
  locking in scsi_mq_requeue_cmd() to avoid a race with
  scsi_internal_device_block(). Avoid this complexity by removing
  the blk_mq_stop_hw_queue() call from scsi_queue_rq().
* In the NVMe core only the functions that call
  blk_mq_start_stopped_hw_queues() explicitly should start stopped
  queues.
* A blk_mq_start_stopped_hwqueues() call must be added in the
  xen-blkfront driver in its blkif_recover() function.

a2b809672e block: replace REQ_NOIDLE with REQ_IDLE
Noidle should be the default for writes as seen by all the compounds
definitions in fs.h using it.  In fact only direct I/O really should
be using NODILE, so turn the whole flag around to get the defaults
right, which will make our life much easier especially onces the
WRITE_* defines go away.

This assumes all the existing "raw" users of REQ_SYNC for writes
want noidle behavior, which seems to be spot on from a quick audit.

ef295ecf09 block: better op and flags encoding
Now that we don't need the common flags to overflow outside the range
of a 32-bit type we can encode them the same way for both the bio and
request fields.  This in addition allows us to place the operation
first (and make some room for more ops while we're at it) and to
stop having to shift around the operation values.

In addition this allows passing around only one value in the block layer
instead of two (and eventuall also in the file systems, but we can do
that later) and thus clean up a lot of code.

Last but not least this allows decreasing the size of the cmd_flags
field in struct request to 32-bits.  Various functions passing this
value could also be updated, but I'd like to avoid the churn for now.

A lot of the REQ_* flags are only used on struct requests, and only of
use to the block layer and a few drivers that dig into struct request
internals.

This patch adds a new req_flags_t rq_flags field to struct request for
them, and thus dramatically shrinks the number of common requests.  It
also removes the unfortunate situation where we have to fit the fields
from the same enum into 32 bits for struct bio and 64 bits for
struct request.

-Wmaybe-uninitialized is set, after gcc gets a little confused
about the multiple 'goto' here:

Moving the assignment to nr_zones makes this a little simpler
while also avoiding the warning reliably. I'm removing the
extraneous initialization of 'int ret' in the same patch, as
that is semi-related and could cause an uninitialized use of
that variable to not produce a warning.

5dc8b362a2 block: Add iocontext priority to request
Patch adds an association between iocontext ioprio and the ioprio of a
request. This is done to enable request based drivers the ability to
act on priority information stored in the request. An example being
ATA devices that support command priorities. If the ATA driver discovers
that the device supports command priorities and the request has valid
priority information indicating the request is high priority, then a high
priority command can be sent to the device. This should improve tail
latencies for high priority IO on any device that queues requests
internally and can make use of the priority information stored in the
request.

The ioprio of the request is set in blk_rq_set_prio which takes the
request and the ioc as arguments. If the ioc is valid in blk_rq_set_prio
then the iopriority of the request is set as the iopriority of the ioc.
In init_request_from_bio a check is made to see if the ioprio of the bio
is valid and if so then the request prio comes from the bio.

Adds the new BLKREPORTZONE and BLKRESETZONE ioctls for respectively
obtaining the zone configuration of a zoned block device and resetting
the write pointer of sequential zones of a zoned block device.

The BLKREPORTZONE ioctl maps directly to a single call of the function
blkdev_report_zones. The zone information result is passed as an array
of struct blk_zone identical to the structure used internally for
processing the REQ_OP_ZONE_REPORT operation.  The BLKRESETZONE ioctl
maps to a single call of the blkdev_reset_zones function.

Implement zoned block device zone information reporting and reset.
Zone information are reported as struct blk_zone. This implementation
does not differentiate between host-aware and host-managed device
models and is valid for both. Two functions are provided:
blkdev_report_zones for discovering the zone configuration of a
zoned block device, and blkdev_reset_zones for resetting the write
pointer of sequential zones. The helper function blk_queue_zone_size
and bdev_zone_size are also provided for, as the name suggest,
obtaining the zone size (in 512B sectors) of the zones of the device.

Add the zoned queue limit to indicate the zoning model of a block device.
Defined values are 0 (BLK_ZONED_NONE) for regular block devices,
1 (BLK_ZONED_HA) for host-aware zone block devices and 2 (BLK_ZONED_HM)
for host-managed zone block devices. The standards defined drive managed
model is not defined here since these block devices do not provide any
command for accessing zone information. Drive managed model devices will
be reported as BLK_ZONED_NONE.

The helper functions blk_queue_zoned_model and bdev_zoned_model return
the zoned limit and the functions blk_queue_is_zoned and bdev_is_zoned
return a boolean for callers to test if a block device is zoned.

The zoned attribute is also exported as a string to applications via
sysfs. BLK_ZONED_NONE shows as "none", BLK_ZONED_HA as "host-aware" and
BLK_ZONED_HM as "host-managed".

Current bad block clear implementation assumes the range to clear
overlaps with at least one bad block already stored. If given range to
clear precedes first bad block in a list, the first entry is incorrectly
updated.

Check not only if stored block end is past clear block end but also if
stored block start is before clear block end.

Make sure that the offset and length arguments that we're using to
construct WRITE SAME and DISCARD requests are actually aligned to the
logical block size.  Failure to do this causes other errors in other parts
of the block layer or the SCSI layer because disks don't support partial
logical block writes.

22dd6d3566 block: invalidate the page cache when issuing BLKZEROOUT
Patch series "fallocate for block devices", v11.

This is a patchset to fix page cache coherency with BLKZEROOUT and
implement fallocate for block devices.

The first patch is a fix to the existing BLKZEROOUT ioctl to invalidate
the page cache if the zeroing command to the underlying device succeeds.
Without this patch we still have the pagecache coherence bug that's been
in the kernel forever.

The second patch changes the internal block device functions to reject
attempts to discard or zeroout that are not aligned to the logical block
size.  Previously, we only checked that the start/len parameters were
512-byte aligned, which caused kernel BUG_ONs for unaligned IOs to 4k-LBA
devices.

The third patch creates an fallocate handler for block devices, wires up
the FALLOC_FL_PUNCH_HOLE flag to zeroing-discard, and connects
FALLOC_FL_ZERO_RANGE to write-same so that we can have a consistent
fallocate interface between files and block devices.  It also allows the
combination of PUNCH_HOLE and NO_HIDE_STALE to invoke non-zeroing discard.

Test cases for the new block device fallocate are now in xfstests as
generic/349-351.

This patch (of 3):

Invalidate the page cache (as a regular O_DIRECT write would do) to avoid
returning stale cache contents at a later time.

0766f788eb latent_entropy: Mark functions with __latent_entropy
The __latent_entropy gcc attribute can be used only on functions and
variables.  If it is on a function then the plugin will instrument it for
gathering control-flow entropy. If the attribute is on a variable then
the plugin will initialize it with random contents.  The variable must
be an integer, an integer array type or a structure with integer fields.

These specific functions have been selected because they are init
functions (to help gather boot-time entropy), are called at unpredictable
times, or they have variable loops, each of which provide some level of
latent entropy.
