The internal flag IP_BASED_GIDS was added to a field that was being used
to hold the port Info CapabilityMask without considering the effects this
will have. Since most drivers just use the value from the HW MAD it means
IP_BASED_GIDS will also become set on any HW that sets the IBA flag
IsOtherLocalChangesNoticeSupported - which is not intended.
Fix this by keeping port_cap_flags only for the IBA CapabilityMask value
and store unrelated flags externally. Move the bit definitions for this to
ib_mad.h to make it clear what is happening.
To keep the uAPI unchanged define a new set of flags in the uapi header
that are only used by ib_uverbs_query_port_resp.port_cap_flags which match
the current flags supported in rdma-core, and the values exposed by the
current kernel.

This patch follows the logic from ib_core but considers the internal
device state upon executing the involved commands.
Specifically, Upon internal error state modify QP to an error state can
be assumed to be success as each in-progress WR going to be flushed in
error in any case as expected by that modify command.
In addition,
As the drain should never fail the driver makes sure that post_send/recv
will succeed even if the device is already in an internal error state.
As such once the driver will supply the simulated/SW CQEs the CQE for
the drain WR will be handled as well.
In case of an internal error state the CQE for the drain WR may be
completed as part of the main task that handled the error state or by
the task that issued the drain WR.
As the above depends on scheduling the code takes the relevant locks
and actions to make sure that the completion handler for that WR will
always be called after that the post_send/recv were issued but not in
parallel to the other task that handles the error flow.

This patch replaces the ib_device_attr.max_sge with max_send_sge and
max_recv_sge. It allows ulps to take advantage of devices that have very
different send and recv sge depths. For example cxgb4 has a max_recv_sge
of 4, yet a max_send_sge of 16. Splitting out these attributes allows
much more efficient use of the SQ for cxgb4 with ulps that use the RDMA_RW
API. Consider a large RDMA WRITE that has 16 scattergather entries.
With max_sge of 4, the ulp would send 4 WRITE WRs, but with max_sge of
16, it can be done with 1 WRITE WR.

The core code now ensures that all driver callbacks that receive an
rdma_ah_attrs will have a sgid_attr's pointer if there is a GRH present.
Drivers can use this pointer instead of calling a query function with
sgid_index. This simplifies the drivers and also avoids races where a
gid_index lookup may return different data if it is changed.

This series comes to allow user space applications to monitor real time
traffic activity and events of the verbs objects it manages, e.g.:
ibv_qp, ibv_wq, ibv_flow.
This API enables generic counters creation and define mapping
to association with a verbs object, current mlx5 driver using

Any steering QP is supposed be above steering_qp_base,
see function mlx4_ib_steer_qp_alloc() for it, however in case
of misalignment between SW and FW, this qp_base can be wrong.
Use WARN() to catch such situation without killing the machine.

Update mlx4 to support user MR creation against read-only memory, previously
it required the memory to be writable.

To allow rereg_user_mr to modify the MR from read-only to writable without
using get_user_pages again, we needed to define the initial MR as writable.
However, this was originally done unconditionally, without taking into
account the writability of the underlying virtual memory.
As a result, any attempt to register a read-only MR over read-only
virtual memory failed.
To fix this, do not add the writable flag bit when the user virtual memory
is not writable (e.g. const memory).
However, when the underlying memory is NOT writable (and we therefore
do not define the initial MR as writable), the IB core adds a
"force writable" flag to its user-pages request. If this succeeds,
the reg_user_mr caller gets a writable copy of the original pages.
If the user-space caller then does a rereg_user_mr operation to enable
writability, this will succeed. This should not be allowed, since
the original virtual memory was not writable.

When the kernel was compiled using the UBSAN option,
we saw the following stack trace:

The problem was caused by calling round_up in procedure
mlx4_ib_umem_calc_optimal_mtt_size (on line 349, as noted in the stack
trace) with the second parameter (1 << block_shift) (which is an int).
The second parameter should have been (1ULL << block_shift) (which
is an unsigned long long).
(1 << block_shift) is treated by the compiler as an int (because 1 is
an integer).
Now, local variable block_shift is initialized to 31.
If block_shift is 31, 1 << block_shift is 1 << 31 = 0x80000000=-214748368.
This is the most negative int value.
Inside the round_up macro, there is a cast applied to ((1 << 31) - 1).
However, this cast is applied AFTER ((1 << 31) - 1) is calculated.
Since (1 << 31) is treated as an int, we get the negative overflow
identified by UBSAN in the process of calculating ((1 << 31) - 1).
The fix is to change (1 << block_shift) to (1ULL << block_shift) on
line 349.

ib_query_gid() fetches the GID from the software cache maintained in
ib_core for RoCE ports.
Therefore, simplify the provider drivers for RoCE to treat query_gid()
callback as never called for RoCE, and only require non-RoCE devices to
implement it.

Code includes wmb() followed by writel(). writel() already has a barrier on
some architectures like arm64.
This ends up CPU observing two barriers back to back before executing the
register write.
Since code already has an explicit barrier call, changing writel() to
writel_relaxed().

Extending uverbs_ioctl header with driver_id and another reserved
field. driver_id should be used in order to identify the driver.
Since every driver could have its own parsing tree, this is necessary
for strace support.
Downstream patches take off the EXPERIMENTAL flag from the ioctl() IB
support and thus we add some reserved fields for future usage.

As a default, for Ethernet packets, the device scatters only the payload
of ingress packets. The scatter FCS feature lets the user to get the FCS
(Ethernet's frame check sequence) in the received WR's buffer as a 4
Bytes trailer following the packet's payload.

Report to the user area the TSO device capabilities, it includes the
max_tso size and the QP types that support it.
The TSO is applicable only when when of the ports is ETH and the device
supports it.
uresp logic around rss_caps is updated to fix a till-now harmless bug
computing the length of the structure to copy. The code did not handle the
implicit padding before rss_caps correctly. This is necessay to copy
tss_caps successfully.

MLX4_USER_DEV_CAP_LARGE_CQE (via mlx4_ib_alloc_ucontext_resp.dev_caps)

Due to bug fixes found by the syzkaller bot and taken into the for-rc
branch after development for the 4.17 merge window had already started
being taken into the for-next branch, there were fairly non-trivial
merge issues that would need to be resolved between the for-rc branch
and the for-next branch. This merge resolves those conflicts and
provides a unified base upon which ongoing development for 4.17 can

When adding GIDs, this gid_type field was copied over to the
hardware gid table. However, when deleting GIDs, the gid_type field
was not copied over to the hardware gid table.
As a result, when running RoCEv2, all RoCEv2 gids in the
hardware gid table were set to type RoCEv1 when any gid was deleted.
This problem would persist until the next gid was added (which would again
restore the gid_type field for all the gids in the hardware gid table).
Fix this by copying over the gid_type field to the hardware gid table
when deleting gids, so that the gid_type of all remaining gids is
preserved when a gid is deleted.

When using IPv4 addresses in RoCEv2, the GID format for the mapped
IPv4 address should be: ::ffff:<4-byte IPv4 address>.
In the cited commit, IPv4 mapped IPV6 addresses had the 3 upper dwords
zeroed out by memset, which resulted in deleting the ffff field.
However, since procedure ipv6_addr_v4mapped() already verifies that the
gid has format ::ffff:<ipv4 address>, no change is needed for the gid,
and the memset can simply be removed.

IB spec says that a lid should be ignored when link layer is Ethernet,
for example when building or parsing a CM request message (CA17-34).
However, since ib_lid_be16() and ib_lid_cpu16() validates the slid,
not only when link layer is IB, we set the slid to zero to prevent
false warnings in the kernel log.

Allocating steerable UD QPs depends on having at least one IB port,
while releasing those QPs does not.
As a result, when there are only ETH ports, the IB (RoCE) driver
requests releasing a qp range whose base qp is zero, with
qp count zero.
When SR-IOV is enabled, and the VF driver is running on a VM over
a hypervisor which treats such qp release calls as errors
(rather than NOPs), we see lines in the VM message log like:
mlx4_core 0002:00:02.0: Failed to release qp range base:0 cnt:0
Fix this by adding a check for a zero count in mlx4_release_qp_range()
(which thus treats releasing 0 qps as a nop), and eliminating the
check for device managed flow steering when releasing steerable UD QPs.
(Freeing ib_uc_qpns_bitmap unconditionally is also OK, since it
remains NULL when steerable UD QPs are not allocated).

ibmr.device is being set only after ib_alloc_mr() is successfully complete.
Therefore, in case imlx4_mr_enable() returns with error, the error flow
unwinder calls to mlx4_free_priv_pages(), which uses ibmr.device.
Such usage causes to NULL dereference oops and to fix it, the IB device
should be set in the mr struct earlier stage (e.g. prior to calling
mlx4_free_priv_pages()).

Support RSS hash for inner headers according to a new flag,
MLX4_IB_RX_HASH_INNER provided by the vendor channel.
In case the flag is set, RSS hash will be done on the inner headers of
VXLAN packets (which are encapsulated).
Non-encapsulated packets will be hashed according to the outer headers.

Mistakenly the driver didn't allow RSS hash fields combinations which
involve both IPv4 and IPv6 protocols. This bug caused to failures for
user's use cases for RSS.
Consequently, this patch fixes this bug and allows any combination that
the HW can support.
Additionally, the patch fixes the driver to return an error in case the
user provides an unsupported mask for RSS hash fields.

Maximal message should be used as a limit to the max message payload allowed,
without the headers. The ConnectX-3 check is done against this value includes
the headers. When the payload is 4K this will cause the NIC to drop packets.
Increase maximal message to 8K as workaround, this shouldn't change current
behaviour because we continue to set the MTU to 4k.
To reproduce;
set MTU to 4296 on the corresponding interface, for example:
ifconfig eth0 mtu 4296 (both server and client)
On server:
ib_send_bw -c UD -d mlx4_0 -s 4096 -n 1000000 -i1 -m 4096
On client:
ib_send_bw -d mlx4_0 -c UD <server_ip> -s 4096 -n 1000000 -i 1 -m 4096

Optimize the device performance by assigning multiple physical pages,
which are contiguous, to a single MTT. As a result, the number of MTTs
is reduced and in turn save cache misses of MTTs.

In the modify QP handler the base_qpn_udp field in the RSS QPC is
overwrite later by irrelevant value assignment. Hence, ingress packets
which gets to the RSS QP will be steered then to a garbage QPN.
The patch fixes this by skipping the above assignment when a RSS QP is
modified, also, the RSS context's attributes assignments are relocated
just before the context is posted to avoid future issues like this.
Additionally, this patch takes the opportunity to change the code to be
disciplined to the device's manual and assigns the RSS QP context just at
RESET to INIT transition.

Since IB/core resolves the destination mac address for user and kernel
consumers, avoid resolving in multiple provider drivers.
Only ib_core resolves DMAC now, therefore resolve_eth_dmac is removed as

There is a need to forward FW version to user space
application through RDMA netlink. In order to make it safe, there
is need to declare nla_policy and limit the size of FW string.
The new define IB_FW_VERSION_NAME_MAX will limit the size of
FW version string. That define was chosen to be equal to
ETHTOOL_FWVERS_LEN, because many drivers anyway are limited
by that value indirectly.
The introduction of this define allows us to remove the string size
from get_fw_str function signature.

Add support to work with a RSS QP by using an indirection table object
upon QP creation. Other related QP verbs (e.g. modify/destroy/query) were
updated as well for that QP mode.
Notes:
- The RX hash properties are supplied as driver private data.
- The RSS QP port is used on the associated WQs in its indirection
table. Applying different ports during WQ life time is not allowed.
- The expected RSS QP flow is: create, modify(RST->INIT),
modify(RST->RTR), destroy.

Support create/modify/destroy WQ related verbs.
The base IB object to enable RSS functionality is a WQ (i.e. ib_wq).
This patch implements the related WQ verbs as of create, modify and
destroy.
In downstream patches the WQ will be used as part of an indirection
table (i.e. ib_rwq_ind_table) to enable RSS QP creation.
Notes:
ConnectX-3 hardware requires consecutive WQNs list as receive descriptor
queues for the RSS QP. Hence, the driver manages consecutive ranges lists
per context which the user must respect.
Destroying the WQ does not return its WQN back to its range for
reusing. However, destroying all WQs from the same range releases the
range and in turn releases its WQNs for reusing.
Since the WQ object is not a natural object in the hardware, the driver
implements the WQ by the hardware QP.
As such, the WQ inherits its port from its RSS QP parent upon its
RST->INIT transition and by that time its state is applied to the
hardware.

Adding visibility of resource usage of QPs, CQs and counters used by
virtual functions. This feature will be used to give the PF administrator
more data while debugging VF status. Usage info was added to ALLOC_RES
command, to notify the PF if the resource which is being reserved or
allocated for the VF will be used by kernel driver or by user verbs.
Updated reservation and allocation functions of QP, CQ and counter with
additional usage parameter.

When inline-receive is enabled, the HCA may write received
data into the receive WQE.
Inline-receive is enabled by setting its matching bit in
the QP context and each single-packet message with payload
not exceeding the receive WQE size will be delivered to
the WQE.
The completion report will indicate that the payload was placed to the WQE.
It includes:
1) Return maximum supported size of inline-receive by the hardware
in query_device vendor's data part.
2) Enable the feature when requested by the vendor data input.

CM REQs cannot be successfully retried, because a new pv_cm_id is
created for each request, without checking if one already exists.
By checking if an id exists before creating one, the bug is fixed.
This bug can be provoked by running an RDMA CM user-land application,
but inserting a five seconds delay before the rdma_accept() call on
the passive side. This delay is larger than the default CMA timeout,
and triggers a retry from the active side. The retried REQ will use
another pv_cm_id (the cm_id on the wire). This confuses the CM
protocol and two REJs are sent from the passive side.

The caller to the driver marks GFP_NOIO allocations with help
of memalloc_noio-* calls now. This makes redundant to pass down
to the driver gfp flags, which can be GFP_KERNEL only.
The patch removes the gfp flags argument and updates all driver paths.

The msleep(1) may do not sleep 1 ms as expected
and will sleep longer. The simple conversion from
msleep to usleep_range between 1ms and 2ms can solve an
issue.

The cited patch added a type field to structures ib_ah and rdma_ah_attr.
Function mlx4_ib_query_ah() builds an rdma_ah_attr structure from the
data in an mlx4_ib_ah structure (which contains both an ib_ah structure
and an address vector).
For mlx4_ib_query_ah() to work properly, the type field in the contained
ib_ah structure must be set correctly.
In the outgoing MAD tunneling flow, procedure mlx4_ib_multiplex_mad()
paravirtualizes a MAD received from a slave and sends the processed
mad out over the wire. During this processing, it populates an
mlx4_ib_ah structure and calls mlx4_ib_query_ah().
The cited commit overlooked setting the type field in the contained
ib_ah structure before invoking mlx4_ib_query_ah(). As a result, the
type field remained uninitialized, and the rdma_ah_attr structure was
incorrectly built. This resulted in improperly built MADs being sent out
over the wire.
This patch properly initializes the type field in the contained ib_ah
structure before calling mlx4_ib_query_ah(). The rdma_ah_attr structure
is then generated correctly.

The last two actual parameters when calling id_map_find_by_sl_id()
from id_map_get() are swapped. However, the same formal parameters to
id_map_get() have them swapped as well, inverting the effect of the
first error.
This commit improves readability, but makes no functional change to
the code.

rdma_ah_attr can now be either ib or roce allowing
core components to use one type or the other and also
to define attributes unique to a specific type. struct
ib_ah is also initialized with the type when its first
created. This ensures that calls such as modify_ah
dont modify the type of the address handle attribute.

Size of pages are held by struct ib_umem in page_size field.
It is better to store it as an exponent, because page size by nature
is always power-of-two and used as a factor, divisor or ilog2's argument.
The conversion of page_size to be page_shift allows to have portable
code and avoid following error while compiling on ARM:

Anonymous VMA (->vm_ops == NULL) cannot be shared, otherwise
it would lead to SIGBUS.
Remove the shared flags from the vma after we change it to be
anonymous.
This is easily reproduced by doing modprobe -r while running a
user-space application such as raw_ethernet_bw.

When the driver disassociate user context, it changes the vma to
anonymous by setting the vm_ops to null and zap the vma ptes.
In order to avoid race in the kernel, we need to take write lock
before we change the vma entries.

A warning message during SRIOV multicast cleanup should have actually been
a debug level message. The condition generating the warning does no harm
and can fill the message log.
In some cases, during testing, some tests were so intense as to swamp the
message log with these warning messages, causing a stall in the console
message log output task. This stall caused an NMI to be sent to all CPUs
(so that they all dumped their stacks into the message log).
Aside from the message flood causing an NMI, the tests all passed.
Once the message flood which caused the NMI is removed (by reducing the
warning message to debug level), the NMI no longer occurs.

On some environments, such as certain SR-IOV VF configurations, RoCE
isn't supported for mlx4 Ethernet ports. Currently the driver will
not open IB device on that port.
This is problematic since we do want user-space RAW Ethernet QPs functionality
to remain in place. For that end, enhance the relevant driver flows such that we
do create a device instance in that case.

We are going to split <linux/sched/mm.h> out of <linux/sched.h>, which

Create a trivial placeholder <linux/sched/mm.h> file that just
maps to <linux/sched.h> to make this patch obviously correct and
bisectable.

Previously, we used the HW GID index in order to search the source GID
in the software GID cached table. In some cases, for example when
the MAC Address of the network interface is changed, the GID cached table
saves the old-IPv6-link-local GID at the end of the table.
When returning the old MAC address, the software GID cached table tries
to add the new IPv6-link-local GID, and when it identifies that the GID
already exists, the software GID cached does not add it. Thus a mismatch
occurs between the HW and the SW GID tables.
It resulted with sending traffic with the wrong source GID.
This commit fixes the issue by taking both from the HW table.
The problem can be reproduced with the following scenario:

Change the drivers to call ib_query_port in their get port
immutable handler instead of their own query port handler.
Doing this required to set the core cap flags of this device
before the ib_query_port call is made, since the IB core might
need these caps to serve the port query.
Drivers are ensured by the IB core that the port attributes passed
to the port query verb implementation are zero, and hence we
removed the zeroing from the drivers.
This patch doesn't add any new functionality.

Demoting simple flow steering rule priority (for DPDK) was achieved by
wrapping FW commands MLX4_QP_FLOW_STEERING_ATTACH/DETACH for the PF
as well, and forcing the priority to MLX4_DOMAIN_NIC in the wrapper
function for the PF and all VFs.
In function mlx4_ib_create_flow(), this change caused the main rule
creation for the PF to be wrapped, while it left the associated
tunnel steering rule creation unwrapped for the PF.
This mismatch caused rule deletion failures in mlx4_ib_destroy_flow()
for the PF when the detach wrapper function did not find the associated
tunnel-steering rule (since creation of that rule for the PF did not
go through the wrapper function).
Fix this by setting MLX4_QP_FLOW_STEERING_ATTACH/DETACH to be "native"
(so that the PF invocation does not go through the wrapper), and perform
the required priority demotion for the PF in the mlx4_ib_create_flow()
code path.

If uhw->inlen is non-zero, the value of variable err is 0 if the copy
succeeds. Then, if kzalloc() or kmalloc() returns a NULL pointer, it
will return 0 to the callers. As a result, the callers cannot detect the
errors. This patch fixes the bug, assign "-ENOMEM" to err before the
NULL pointer checks, and remove the initialization of err at the
beginning.

For non-special QPs, the port value becomes non-zero only at the
RESET-to-INIT transition. If the QP has not undergone that transition,
its port number value is still zero.
If such a QP is destroyed before being moved out of the RESET state,
subtracting one from the qp port number results in a negative value.
Using that negative value as an index into the qp1_proxy array
results in an out-of-bounds array reference.
Fix this by testing that the QP type is one that uses qp1_proxy before
using the port number. For special QPs of all types, the port number is
specified at QP creation time.

Add struct ib_udata to the signature of create_ah callback that is
implemented by IB device drivers. This allows HW drivers to return extra
data to the userspace library.
This patch prepares the ground for mlx5 driver to resolve destination
mac address for a given GID and return it to userspace.
This patch was previously submitted by Knut Omang as a part of the
patch set to support Oracle's Infiniband HCA (SIF).

According to the firmware spec, FLOW_STEERING_IB_UC_QP_RANGE command is
supported only if dmfs_ipoib bit is set.
If it isn't set we want to ensure allocating NET_IF QPs fail. We do so
by filling out the allocation bitmap. By thus, the NET_IF QPs allocating
function won't find any free QP and will fail.

If OpenSM runs over a ConnectX-3, and there are ConnectX-4 or Connect-IB
VFs active on the network, the OpenSM will receive QP1 packets containing
a GRH where the destination GID is the "Well-Known GID" -- which is not a
GID in the HCA Port's GID Table.
This GID must be tested-for separately -- and packets which contain
this destination GID should be routed to slave 0 (the PF).

When MAD arrives to the hypervisor, we need to identify which slave it
should be sent by destination GID. When L3 protocol is IPv4 the
GRH is replaced by an IPv4 header. This patch detects when IPv4 header
needs to be parsed instead of GRH.

In MLX qp packets, the LRH (built by the driver) has both a VL field
and an SL field. When building a QP1 packet, the VL field should
reflect the SLtoVL mapping and not arbitrarily contain zero (as is
done now). This bug causes credit problems in IB switches at
high rates of QP1 packets.
The fix is to cache the SL to VL mapping in the driver, and look up
the VL mapped to the SL provided in the send request when sending
QP1 packets.
For FW versions which support generating a port_management_config_change
event with subtype sl-to-vl-table-change, the driver uses that event
to update its sl-to-vl mapping cache. Otherwise, the driver snoops
incoming SMP mads to update the cache.
There remains the case where the FW is running in secure-host mode
(so no QP0 packets are delivered to the driver), and the FW does not
generate the sl2vl mapping change event. To support this case, the
driver updates (via querying the FW) its sl2vl mapping cache when
running in secure-host mode when it receives either a Port Up event
or a client-reregister event (where the port is still up, but there
may have been an opensm failover).
OpenSM modifies the sl2vl mapping before Port Up and Client-reregister
events occur, so if there is a mapping change the driver's cache will
be properly updated.

This patch moves mlx4 vendor's specific structures to
common UAPI folder which will be visible to all consumers.
These structures are used by user-space library driver
(libmlx4) and currently manually copied to that library.
This move will allow cross-compile against these files and
simplify introduction of vendor specific data.

Instead of exposing ib_get_dma_mr to ULPs and letting them use it more or
less unchecked, this moves the capability of creating a global rkey into
the RDMA core, where it can be easily audited. It also prints a warning
everytime this feature is used as well.

When sending QP1 MAD packets which use a GRH, the source GID
(which consists of the 64-bit subnet prefix, and the 64 bit port GUID)
must be included in the packet GRH.
For SR-IOV, a GID cache is used, since the source GID needs to be the
slave's source GID, and not the Hypervisor's GID. This cache also
included a subnet_prefix. Unfortunately, the subnet_prefix field in
the cache was never initialized (to the default subnet prefix 0xfe80::0).
As a result, this field remained all zeroes. Therefore, when SR-IOV
was active, all QP1 packets which included a GRH had a source GID
subnet prefix of all-zeroes.
However, the subnet-prefix should initially be 0xfe80::0 (the default
subnet prefix). In addition, if OpenSM modifies a port's subnet prefix,
the new subnet prefix must be used in the GRH when sending QP1 packets.
To fix this we now initialize the subnet prefix in the SR-IOV GID cache
to the default subnet prefix. We update the cached value if/when OpenSM
modifies the port's subnet prefix. We take this cached value when sending
QP1 packets when SR-IOV is active.
Note that the value is stored as an atomic64. This eliminates any need
for locking when the subnet prefix is being updated.
Note also that we depend on the FW generating the "port management change"
event for tracking subnet-prefix changes performed by OpenSM. If running
early FW (before 2.9.4630), subnet prefix changes will not be tracked (but
the default subnet prefix still will be stored in the cache; therefore
users who do not modify the subnet prefix will not have a problem).
IF there is a need for such tracking also for early FW, we will add that
capability in a subsequent patch.

Because of an incorrect bit-masking done on the join state bits, when
handling a join request we failed to detect a difference between the
group join state and the request join state when joining as send only
full member (0x8). This caused the MC join request not to be sent.
This issue is relevant only when SRIOV is enabled and SM supports
send only full member.
This fix separates scope bits and join states bits a nibble each.

By Mellanox HW design and SW implementation, poll_cq never
fails and returns errors, so all these printks are to catch ULP bugs.
In case of such bug, the reverted patch will cause reentry of the
function, resulting in a printk storm.

We allocate a small tracking structure as part of mlx4_ib_resize_cq().
However, we don't need to use GFP_ATOMIC -- immediately after the
allocation, we call mlx4_cq_resize(), which allocates a command
mailbox with GFP_KERNEL and then sleeps on a firmware command, so we
better not be in an atomic context.
This actually has a real impact, because when this GFP_ATOMIC
allocation fails (and GFP_ATOMIC does fail in practice) then a
userspace consumer resizing a CQ will get a spurious failure that we
can easily avoid.

In procedure mlx4_ib_create_flow, passing an invalid port number
will cause an out-of-bounds array access. Data passed to this procedure
can come from user-space. Therefore, need to validate port number
before proceeding onwards.
Note that we check against the number of physical ports declared at
the verbs (ib core) level; When bonding is active, the verbs level
sees one physical port, even though the low-level driver sees two ports.

Fix mad send error flow to prevent double freeing address handles,
and leaking tx_ring entries when SRIOV is active.
If ib_mad_post_send fails, the address handle pointer in the tx_ring entry
must be set to NULL (or there will be a double-free) and tx_tail must be
incremented (or there will be a leak of tx_ring entries).
The tx_ring is handled the same way in the send-completion handler.

When this code was reworked for IBoE support the order of assignments
for the sl_tclass_flowlabel got flipped around resulting in
TClass & FlowLabel being permanently set to 0 in the packet headers.
This breaks IB routers that rely on these headers, but only affects
kernel users - libmlx4 does this properly for user space.

The problem is that the function 'send_reply_to_slave' gets the
'req_sa_mad' as a pointer whose address is only aliged to 4 bytes
but is 8 bytes in size. This can result in unaligned access faults
on certain architectures.
Sowmini Varadhan pointed to this reply from Dave Miller that say
that memcpy should not be used to solve alignment issues:

Optimization of memcpy to 'ldx' instruction can only happen if the
compiler knows that the size of the data we are copying is 8 bytes
and it assumes it is aligned to 8 bytes. If the compiler know the
type is not aligned to 8 it must not optimize the 8 byte copy.
Defining the data type as aligned to 4 forces the compiler to treat
all accesses as though they aren't aligned and avoids the 'ldx'
optimization.

The SRP initiator allows to set max_sectors to a value that exceeds
the largest amount of data that can be mapped at once with an mlx4
HCA using fast registration and a page size of 4 KB. Hence modify
ib_map_mr_sg() such that it can map partial sg-elements. If an
sg-element has been mapped partially, let the caller know
which fraction has been mapped by adjusting *sg_offset.

The dma_alloc_coherent() function returns a virtual address which can
be used for coherent access to the underlying memory. On some
architectures, like arm64, undefined behavior results if this memory is
also accessed via virtual mappings that are not coherent. Because of
their undefined nature, operations like virt_to_page() return garbage
when passed virtual addresses obtained from dma_alloc_coherent(). Any
subsequent mappings via vmap() of the garbage page values are unusable
and result in bad things like bus errors (synchronous aborts in ARM64
speak).
The mlx4 driver contains code that does the equivalent of:
vmap(virt_to_page(dma_alloc_coherent)), this results in an OOPs when the
device is opened.
Prevent Ethernet driver to run this problematic code by forcing it to
allocate contiguous memory. As for the Infiniband driver, at first we
are trying to allocate contiguous memory, but in case of failure roll
back to work with fragmented memory.

The new NET_DEVLINK infrastructure can be a loadable module, but the drivers
using it might be built-in, which causes link errors like:

There are multiple ways to avoid this:

c) make NET_DEVLINK a 'bool' option so we don't have to
list it as a dependency, and rely on the APIs to be
stubbed out when it is disabled
d) use IS_REACHABLE() rather than IS_ENABLED() to check for
NET_DEVLINK in include/net/devlink.h
This implements a variation of approach a) by adding an
intermediate symbol that drivers can depend on, and changes
the three drivers using it.

Add support for receiving multicast/unicast traffic with
the don't trap rule.
Sniffing these packets requires a flow steering rule of type NORMAL
at priority 0 with flag IB_FLOW_ATTR_FLAGS_DONT_TRAP set.
Choosing between multicast or unicast is done via ethernet L2 dest_mac
mask and value:
- If mask is all zeros - unicast and multicast are set.
- If mask non zero - only mask with multicast bit 1 and rest 0 is
supported, the mac value will choose if it is
multicast or unicast rule.
If the mask multicast bit is on and some other bits are on too, it means
a request for specific multicast or unicast, this is not supported,
either receive all multicast or all unicast.
Only when limitations are met registered QP will receive requested type
but other QPs can receive same traffic if registered for it.
Otherwise, if limitations are not met, an error will be returned.
Limitations:
- Rule must be with priority 0.
- A0 mode is not supported.
- Sniffer QP cannot appear in any other flow steering rule.

Don't trap flag (i.e. IB_FLOW_ATTR_FLAGS_DONT_TRAP) indicates that QP
will receive traffic, but will not steal it.
When a packet matches a flow steering rule that was created with
the don't trap flag, the QPs assigned to this rule will get this
packet, but matching will continue to other equal/lower priority
rules. This will let other QPs assigned to those rules to get the
packet too.
If both don't trap rule and other rules have the same priority
and match the same packet, the behavior is undefined.
The don't trap flag can't be set with default rule types
(i.e. IB_FLOW_ATTR_ALL_DEFAULT, IB_FLOW_ATTR_MC_DEFAULT) as default rules
don't have rules after them and don't trap has no meaning here.

Wall time obtained from ktime_get_real_ns is susceptible to sudden jumps due to
user setting the time or due to NTP. Boot time is constantly increasing time
better suited for comparing two timestamps.

problem description:
The current code sets UAR page size equal to system page size.
The ConnectX-3 and ConnectX-3 Pro HWs require minimum 128 UAR pages.
The mlx4 kernel drivers are not loaded if there is less than 128 UAR pages.
solution:
Always set UAR page to 4KB. This allows more UAR pages if the OS
has PAGE_SIZE larger than 4KB. For example, PowerPC kernel use 64KB
system page size, with 4MB uar region, there are 4MB/2/64KB = 32
uars (half for uar, half for blueflame). This does not meet minimum 128
UAR pages requirement. With 4KB UAR page, there are 4MB/2/4KB = 512 uars
which meet the minimum requirement.
Note that only codes in mlx4_core that deal with firmware know that uar
page size is 4KB. Codes that deal with usr page in cq and qp context
(mlx4_ib, mlx4_en and part of mlx4_core) still have the same assumption
that uar page size equals to system page size.
Note that with this implementation, on 64KB system page size kernel, there
are 16 uars per system page but only one uars is used. The other 15
uars are ignored because of the above assumption.
Regarding SR-IOV, mlx4_core in hypervisor will set the uar page size
to 4KB and mlx4_core code in virtual OS will obtain the uar page size from
firmware.
Regarding backward compatibility in SR-IOV, if hypervisor has this new code,
the virtual OS must be updated. If hypervisor has old code, and the virtual
OS has this new code, the new code will be backward compatible with the
old code. If the uar size is big enough, this new code in VF continues to
work with 64 KB uar page size (on PowerPc kernel). If the uar size does not
meet 128 uars requirement, this new code not loaded in VF and print the same
error message as the old code in Hypervisor.

The mlx4 driver uses a special QP to implement the GSI QP. This kind
of QP allows to build the InfiniBand headers in software.
When mlx4 hardware builds the packet, it calculates the ICRC and puts
it at the end of the payload. However, this ICRC calculation depends
on the QP configuration, which is determined when the QP is modified
(roce_mode during INIT->RTR).
When receiving a packet, the ICRC verification doesn't depend on this
configuration.
Therefore, using two GSI QPs for send (one for each RoCE version) and
one GSI QP for receive are required.

RoCEv2 packets are sent over IP/UDP protocols.
The mlx4 driver uses a type of RAW QP to send packets for QP1 and
therefore needs to build the network headers below BTH in software.
This patch adds option to build QP1 packets with IP and UDP headers if
RoCEv2 is requested.

port according to the RoCE v2 Annex when mlx4_ib device is added.

To tell hardware about a gid with type RoCEv2, software needs a new
modifier to the SET_PORT command: MLX4_SET_PORT_ROCE_ADDR. This can
replace the old method, MLX4_SET_PORT_GID_TABLE, for RoCEv1 gids.

IB core driver adds a property of type to struct ib_gid_attr.
The mlx4 driver should take that in consideration when modifying or
querying the hardware gid table.

Hop limit value wasn't copied from attributes when ah was created.
This may influence packets for unconnected services to get dropped in
routers when endpoints are not in the same subnet.
