The current codebase makes use of the zero-length array language extension
to the C90 standard, but the preferred mechanism to declare variable-length
types such as these ones is a flexible array member[1][2], introduced in
C99:

By making use of the mechanism above, we will get a compiler warning in
case the flexible array does not occur last in the structure, which will
help us prevent some kind of undefined behavior bugs from being
inadvertently introduced[3] to the codebase from now on.
Also, notice that, dynamic memory allocations won't be affected by this
change:
"Flexible array members have incomplete type, and so the sizeof operator
may not be applied. As a quirk of the original implementation of
zero-length arrays, sizeof evaluates to zero."[1]
sizeof(flexible-array-member) triggers a warning because flexible array
members have incomplete type[1]. There are some instances of code in which
the sizeof operator is being incorrectly/erroneously applied to zero-length
arrays and the result is zero. Such instances may be hiding some bugs. So,
this work (flexible-array member conversions) will also help to get
completely rid of those sorts of issues.
This issue was found with the help of Coccinelle.

There are only two callers of blk_rq_map_sg/__blk_rq_map_sg that set
the dma_pad value in the queue. Move the handling into those callers
instead of burdening the common code, and move the ->extra_len field
from struct request to struct scsi_cmnd.

Don't burden the common block code with with specifics of the libata DMA
draining mechanism. Instead move most of the code to the scsi midlayer.
That also means the nr_phys_segments adjustments in the blk-mq fast path
can go away entirely, given that SCSI never looks at nr_phys_segments
after mapping the request to a scatterlist.

If an iSCSI connection happens to fail while the daemon isn't running (due
to a crash or for another reason), the kernel failure report is not
received. When the daemon restarts, there is insufficient kernel state in
sysfs for it to know that this happened. open-iscsi tries to reopen every
connection, but on different initiators, we'd like to know which
connections have failed.
There is session->state, but that has a different lifetime than an iSCSI
connection, so it doesn't directly reflect the connection state.

Call scsi_bios_ptable from scsi_partsize instead of requiring boilerplate
code in the callers. Also switch the calling convention to match that
of the ->bios_param instances calling this function, and use true/false
for the return value instead of the weird -1 convention.

Large queues of I/O to offline devices that are eventually submitted when
devices are unblocked result in a many repeated "rejecting I/O to offline
device" messages. These messages can fill up the dmesg buffer in crash
dumps so no useful prior messages remain. In addition, if a serial console
is used, the flood of messages can cause a hard lockup in the console code.
Introduce a flag indicating the message has already been logged for the
device, and reset the flag when scsi_device_set_state() changes the device
state.

iSCSI session destruction can be arbitrarily slow, since it might require
network operations and serialization inside the SCSI layer. This patch
adds a new user event to trigger the destruction work asynchronously,
releasing the rx_queue_mutex as soon as the operation is queued and before
it is performed. This change allows other operations to run in other
sessions in the meantime, removing one of the major iSCSI bottlenecks for
us.
To prevent the session from being used after the destruction request, we
remove it immediately from the sesslist. This simplifies the locking
required during the asynchronous removal.

The current behavior of the SCSI core is to clear driver-private data
before preparing a request for submission to the SCSI LLD. Make it possible
for SCSI LLDs to disable clearing of driver-private data.
These hooks will be used by a later patch, namely "scsi: ufs: Let the SCSI
core allocate per-command UFS data".

Connection failure processing depends on a daemon being present to (at
least) stop the connection and start recovery. This is a problem on a
multipath scenario, where if the daemon failed for whatever reason, the
SCSI path is never marked as down, multipath won't perform the failover and
IO to the device will be forever waiting for that connection to come back.
This patch performs the connection failure entirely inside the kernel.
This way, the failover can happen and pending IO can continue even if the
daemon is dead. Once the daemon comes alive again, it can execute recovery
procedures if applicable.

This series concludes the work I did for linux-5.5 on the compat_ioctl()
cleanup, killing off fs/compat_ioctl.c and block/compat_ioctl.c by moving
everything into drivers.
Overall this would be a reduction both in complexity and line count, but
as I'm also adding documentation the overall number of lines increases
in the end.
My plan was originally to keep the SCSI and block parts separate.
This did not work easily because of interdependencies: I cannot
do the final SCSI cleanup in a good way without first addressing the
CDROM ioctls, so this is one series that I hope could be merged through
either the block or the scsi git trees, or possibly both if you can
pull in the same branch.
The series comes in these steps:
1. clean up the sg v3 interface as suggested by Linus. I have
talked about this with Doug Gilbert as well, and he would
rebase his sg v4 patches on top of "compat: scsi: sg: fix v3
compat read/write interface"
2. Actually moving handlers out of block/compat_ioctl.c and
block/scsi_ioctl.c into drivers, mixed in with cleanup
patches
3. Document how to do this right. I keep getting asked about this,
and it helps to point to some documentation file.
The branch is based on another one that fixes a couple of bugs found
during the creation of this series.

Merge tag 'block-ioctl-cleanup-5.6' into 5.6/scsi-queue
Pull compat_ioctl cleanup from Arnd. Here's his description:
This series concludes the work I did for linux-5.5 on the compat_ioctl()
cleanup, killing off fs/compat_ioctl.c and block/compat_ioctl.c by moving
everything into drivers.
Overall this would be a reduction both in complexity and line count, but
as I'm also adding documentation the overall number of lines increases
in the end.
My plan was originally to keep the SCSI and block parts separate.
This did not work easily because of interdependencies: I cannot
do the final SCSI cleanup in a good way without first addressing the
CDROM ioctls, so this is one series that I hope could be merged through
either the block or the scsi git trees, or possibly both if you can
pull in the same branch.
The series comes in these steps:
1. clean up the sg v3 interface as suggested by Linus. I have
talked about this with Doug Gilbert as well, and he would
rebase his sg v4 patches on top of "compat: scsi: sg: fix v3
compat read/write interface"
2. Actually moving handlers out of block/compat_ioctl.c and
block/scsi_ioctl.c into drivers, mixed in with cleanup
patches
3. Document how to do this right. I keep getting asked about this,
and it helps to point to some documentation file.
The branch is based on another one that fixes a couple of bugs found
during the creation of this series.

In order to move the compat handling for SCSI ioctl commands out of
fs/compat_ioctl.c into the individual drivers, we need a helper function
first to match the native ioctl handler called by sd, sr, st, etc.

In the v5.4 merge window, a cleanup patch from Al Viro conflicted
with my rework of the compat handling for sg.c read(). Linus Torvalds
did a correct merge but pointed out that the resulting code is still
unsatisfactory.
I later noticed that the sg_new_read() function still gets the compat
mode wrong, when the 'count' argument is large enough to pass a
compat_sg_io_hdr object, but not a nativ sg_io_hdr.
To address both of these, move the definition of compat_sg_io_hdr
into a scsi/sg.h to make it visible to sg.c and rewrite the logic
for reading req_pack_id as well as the size check to a simpler
version that gets the expected results.

struct scsi_cmnd cmd->req.resid_len which is returned and set respectively
by the helper functions scsi_get_resid() and scsi_set_resid() is an
unsigned int. Reflect this fact in the interface of these helper functions.
Also fix compilation errors due to min() and max() type mismatch introduced
by this change in scsi debug code, usb transport code and in the USB ENE
card reader driver.

It isn't necessary to check the host depth in scsi_queue_rq() any more
since it has been respected by blk-mq before calling scsi_queue_rq() via
getting driver tag.
Lots of LUNs may attach to same host and per-host IOPS may reach millions,
so we should avoid expensive atomic operations on the host-wide counter in
the IO path.
This patch implements scsi_host_busy() via blk_mq_tagset_busy_iter() with
one scsi command state for reading the count of busy IOs for scsi_mq.
It is observed that IOPS is increased by 15% in IO test on scsi_debug (32
LUNs, 32 submit queues, 1024 can_queue, libaio/dio) in a dual-socket
system.

When a non-passthrough command is terminated with CHECK CONDITION, request
sense is executed by hijacking the command descriptor. Since
scsi_eh_prep_cmnd() and scsi_eh_restore_cmnd() do not save/restore the
original command resid, the value returned on failure of the original
command is lost and replaced with the value set by the execution of the
request sense command. This value may in many instances be unaligned to the
device sector size, causing sd_done() to print a warning message about the
incorrect unaligned resid before the command is retried.
Fix this problem by saving the original command residual in struct
scsi_eh_save using scsi_eh_prep_cmnd() and restoring it in
scsi_eh_restore_cmnd(). In addition, to make sure that the request sense
command is executed with a correctly initialized command structure, also
reset the residual to 0 in scsi_eh_prep_cmnd() after saving the original
command value in struct scsi_eh_save.

Until now the scsi mid-layer forbids runtime suspend till userspace enables
it. This is mainly to quarantine some disks with broken runtime power
management or have high latencies executing suspend resume callbacks. If
the userspace doesn't enable the runtime suspend the underlying hardware
will be always on even when it is not doing any useful work and thus
wasting power.
Some low-level drivers for the controllers can efficiently use runtime
power management to reduce power consumption and improve battery life.
Allow runtime suspend parameters override within the LLD itself instead of
waiting for userspace to control the power management.

The data structure used for log messages is so large that it can cause a
boot failure. Since allocations from that data structure can fail anyway,
use kmalloc() / kfree() instead of that data structure.

Gcc-9 complains for a memset across pointer boundaries, which happens as
the code tries to allocate a flexible array on the stack. Turns out we
cannot do this without relying on gcc-isms, so with this patch we'll embed
the fc_rport_priv structure into fcoe_rport, can use the normal
'container_of' outcast, and will only have to do a memset over one
structure.

This allows a list of requests to be issued, with the LLD only writing the
hardware doorbell when necessary, after the last request was prepared.
This is more efficient if we have lists of requests to issue, particularly
on virtualized hardware, where writing the doorbell is more expensive than
on real hardware.
The use case for this is plugged IO, where blk-mq flushes a batch of
requests all at once.
The API is the same as for blk-mq, just with blk-mq concepts tweaked to
fit the SCSI subsystem API: the "last" flag in blk_mq_queue_data becomes a
flag in scsi_cmnd, while the queue_num in the commit_rqs callback is
extracted from the hctx and passed as a parameter.
The only complication is that blk-mq uses different plugging heuristics
depending on whether commit_rqs is present or not. So we have two
different sets of blk_mq_ops and pick one depending on whether the
scsi_host template uses commit_rqs or not.

This is the final round of mostly small fixes in our initial submit.
It's mostly minor fixes and driver updates. The only change of note
is adding a virt_boundary_mask to the SCSI host and host template to
parametrise this for NVMe devices instead of having them do a call in
slave_alloc. It's a fairly straightforward conversion except in the
two NVMe handling drivers that didn't set it who now have a virtual
infinity parameter added.

Following discussion on linux-next, we believe our version to be more
accurate than the one in the tree, so the resolution is to take our
version for all the SPDX conflicts"
Note on the SPDX license tag conversion conflicts: the SCSI tree had
done its own SPDX conversion, which in some cases conflicted with the
treewide ones done by Thomas & co.
In almost all cases, the conflicts were purely syntactic: the SCSI tree
used the old-style SPDX tags ("GPL-2.0" and "GPL-2.0+") while the
treewide conversion had used the new-style ones ("GPL-2.0-only" and
"GPL-2.0-or-later").
In these cases I picked the new-style one.
In a few cases, the SPDX conversion was actually different, though. As
explained by James above, and in more detail in a pre-pull-request
thread:
"The other problem is actually substantive: In the libsas code Luben
Tuikov originally specified gpl 2.0 only by dint of stating:
* This file is licensed under GPLv2.
In all the libsas files, but then muddied the water by quoting GPLv2
verbatim (which includes the or later than language). So for these
files Christoph did the conversion to v2 only SPDX tags and Thomas
converted to v2 or later tags"
So in those cases, where the spdx tag substantially mattered, I took the
SCSI tree conversion of it, but then also took the opportunity to turn
the old-style "GPL-2.0" into a new-style "GPL-2.0-only" tag.
Similarly, when there were whitespace differences or other differences
to the comments around the copyright notices, I took the version from
the SCSI tree as being the more specific conversion.
Finally, in the spdx conversions that had no conflicts (because the
treewide ones hadn't been done for those files), I just took the SCSI
tree version as-is, even if it was old-style. The old-style conversions
are perfectly valid, even if the "-only" and "-or-later" versions are
perhaps more descriptive.

Many times in libsas, and in LLDDs which use libsas, the check for an
expander device is re-implemented or open coded.
Use dev_is_expander() instead. We rename this from
sas_dev_type_is_expander() to not spill so many lines in referencing.

According to the SAS spec, an expander device shall transmit BROADCAST
(CHANGE) from at least one phy in each expander port other than the
expander port that is the cause for transmitting BROADCAST (CHANGE).
As such, for when the link is lost for a root PHY attached to an expander
PHY, we get no broadcast event.
This causes an issue for libsas, in that we will not revalidate the domain
for these events.
As a solution, for when a root PHY is formed or deformed from a root port,
insert a broadcast event to trigger a domain revalidation.

Many times we use 8 for SAS address length, while we already have a macro
for this - SAS_ADDR_SIZE.
Replace instances of this with the macro. However, don't touch the SAS
address array sizes sas.h, as these are defined according to the SAS spec.
Some missing whitespaces are also added, and whitespace indentation
in sas_hash_addr() is also fixed (see sas_hash_addr()).

Currently the FC-NVMe driver is leverating the SCSI FC transport class to
access the remote ports. Which means that all FC-NVMe remote ports will be
visible to the fc transport layer, but due to missing definitions the port
roles will always be 'unknown'. This patch adds the missing definitions to
the fc transport class to that the port roles are correctly displayed.

Add a new event type - an FPIN event.
Add a new routine, fc_host_fpin_rcv(), that lldd's call when an FPIN is
received. The routine processes the fpin payload. For now, the routine
only logs an FPIN event.

There are two routines generating transport events that do the same thing
with only a couple of values set differently.
Refactor so there's a single routine doing the netlink operations to send
the event. All the differences are passed as arguments. Export the symbol
so the generic routine can be called by llds.
Modify the existing two event routines to use the helper.

commit 1917d42d14 ("fcoe: use enum for fip_mode") introduces a separate
enum for the fip_mode that shall be used during initialisation handling
until it is passed to fcoe_ctrl_link_up to set the initial fip_state. That
change was incomplete and gcc quietly converted in various places between
the fip_mode and the fip_state enum values with implicit enum conversions,
which fortunately cannot cause any issues in the actual code's execution.
clang however warns about these implicit enum conversions in the scsi
drivers. This commit consolidates the use of the two enums, guided by
clang's enum-conversion warnings.
This commit now completes the use of the fip_mode: It expects and uses
fip_mode in {bnx2fc,fcoe}_interface_create and fcoe_ctlr_init, and it calls
fcoe_ctrl_set_set() with the correct values in fcoe_ctlr_link_up(). It
also breaks the association between FIP_MODE_AUTO and FIP_ST_AUTO to
indicate these two enums are distinct.

The root cause is that the _IOC macro can generate really large numbers,
which don't fit into type 'int', which is used for the cmd parameter in
the ioctls in scsi_host_template. My research into how GCC and Clang are
handling this at a low level didn't prove fruitful. However, looking at
the rest of the kernel tree, all ioctls use an 'unsigned int' for the
cmd parameter, which will fit all of the _IOC values in the scsi/ata
subsystems.
Make that change because none of the ioctls expect a negative value for
any command, it brings the ioctls inline with the reset of the kernel,
and it removes ambiguity, which is never good when dealing with compilers.

The same effects can be achieved by setting the dma_boundary to
PAGE_SIZE - 1 and the max_segment_size to PAGE_SIZE, so shift those
settings into the drivers. Note that in many cases the setting might
be bogus, but this keeps the status quo.

segment size in a nice an easy way, so that the driver doesn't

Most SCSI drivers want to enable "clustering", that is merging of
segments so that they might span more than a single page. Remove the
ENABLE_CLUSTERING define, and require drivers to explicitly set
DISABLE_CLUSTERING to disable this feature.

The problem is that SCMD_STATE_COMPLETE is supposed to be bit number 0
and not a mask like "(1 << 0)". It is used like this:
if (test_and_set_bit(SCMD_STATE_COMPLETE, &scmd->state))
The test_and_set_bit() has a shift built in so it's a double left shift
and uses bit number 1 instead of number 0. This bug is harmless because
it's done consistently and it doesn't clash with any other flags.

The scsi timeout error handling had been directly updating the block
layer's request state to prevent a error handling and a natural completion
from completing the same request twice. Fix this layering violation
by having scsi control the fate of its commands with scsi owned flags
rather than use blk-mq's.

To avoid introducing problems like those fixed in commit f7068114d4
("sr: pass down correctly sized SCSI sense buffer"), this creates a macro
wrapper for scsi_execute() that verifies the size of the sense buffer
similar to what was done for command string sizes in commit 3756f6401c
("exec: avoid gcc-8 warning for get_task_comm").
Another solution could be to add a length argument to scsi_execute(),
but this function already takes a lot of arguments and Jens was not fond
of that approach.
Additionally, this moves the SCSI_SENSE_BUFFERSIZE definition into
scsi_device.h, and removes a redundant include for scsi_device.h from
scsi_cmnd.h.

Commit 2623c7a5f2 ("libata: add refcounting to ata_host") v4.17+ introduced
refcounting to ata_host and will increase or decrease the refcount when
adding or deleting transport ATA port.
Now the ata host for libsas is embedded in domain_device, and the ->kref
member is not initialized. Afer we add ata transport class, ata_host_get()
will be called when adding transport ATA port and a warning will be

And also when removing transport ATA port ata_host_put() will be called and
another similar warning will be triggered. If the refcount decreased to
zero, the ata host will be freed. But this ata host is only part of
domain_device, it cannot be freed directly.
So we have to change this embedded static ata host to a dynamically
allocated ata host and initialize the ->kref member. To use ata_host_get()
and ata_host_put() in libsas, we need to move the declaration of these
functions to the public libata.h and export them.

The BLK_EH_NOT_HANDLED implies nothing happen, but very often that

On Fujitsu ETERNUS systems, sense code ABORTED COMMAND with ASC/Q C1/01
is used to indicate temporary condition where the storage-internal path
to a target is switched from one controller to another. SCSI commands
that return with this error code must be retried unconditionally
(i.e. without the "maybe_retry" logic in scsi_decide_disposition);
otherwise dm-multipath might initiate a failover from a healthy path
e.g. for REQ_FAILFAST_DEV commands.
Introduce a new blist flag for this case.

EMC Symmetrix returns 'internal target error' for a variety of
conditions, most of which will be transient. So we should always retry
it, even with failfast set. Otherwise we'd get spurious path flaps with
multipath.

The SCSI PRE-FETCH (10 or 16) command is present both on hard disks
and some SSDs. It is useful when the address of the next block(s) to
be read is known but it is not following the LBA of the current READ
(so read-ahead won't help). It returns two "good" SCSI Status values.
If the requested blocks have fitted (or will most likely fit (when
the IMMED bit is set)) into the disk's cache, it returns CONDITION
MET. If it didn't (or will not) fit then it returns GOOD status.
The goal of this patch is to stop the SCSI subsystem treating the
CONDITION MET SCSI status as an error. The current state makes the
PRE-FETCH command effectively unusable via pass-throughs.

and the storvsc performance improvement with interrupt handling which
was dropped from the previous fixes pull request. We also have two
regressions: one is a double call_rcu() in ATA error handling and the
other is a missed conversion to BLK_STS_OK in
__scsi_error_from_host_byte().

The patch introduces an additional field in the scsi_host_template
structure - struct attribute_group **sdev_group. This field allows to
define groups of attributes. It will provide an ability to use binary
attributes as well as device attributes and to group them under
subfolders if necessary.

Since the SRP_LOGIN_REQ defined in the SRP standard is larger than
what fits in the RDMA/CM login request private data, introduce a new
login request format for the RDMA/CM.
Note: since srp_daemon and ibsrpdm rely on the subnet manager and
since there is no equivalent of the IB subnet manager in non-IB
networks, login has to be performed manually for non-IB networks.

In commit 87c8331fcf ("[SCSI] libsas: prevent domain rediscovery
competing with ata error handling") introduced disco mutex to prevent
rediscovery competing with ata error handling and put the whole
revalidation in the mutex. But the rphy add/remove needs to wait for the
error handling which also grabs the disco mutex. This may leads to dead
lock.So the probe and destruct event were introduce to do the rphy
add/remove asynchronously and out of the lock.
The asynchronously processed workers makes the whole discovery process
not atomic, the other events may interrupt the process. For example,
if a loss of signal event inserted before the probe event, the
sas_deform_port() is called and the port will be deleted.
And sas_port_delete() may run before the destruct event, but the
port-x:x is the top parent of end device or expander. This leads to
a kernel WARNING such as:

Make probe and destruct a direct call in the disco and revalidate function,
but put them outside the lock. The whole discovery or revalidate won't
be interrupted by other events. And the DISCE_PROBE and DISCE_DESTRUCT
event are deleted as a result of the direct call.
Introduce a new list to destruct the sas_port and put the port delete after
the destruct. This makes sure the right order of destroying the sysfs
kobject and fix the warning above.
In sas_ex_revalidate_domain() have a loop to find all broadcasted
device, and sometimes we have a chance to find the same expander twice.
Because the sas_port will be deleted at the end of the whole revalidate
process, sas_port with the same name cannot be added before this.
Otherwise the sysfs will complain of creating duplicate filename. Since
the LLDD will send broadcast for every device change, we can only
process one expander's revalidation.

Now all libsas works are queued to scsi host workqueue, include sas
event work post by LLDD and sas discovery work, and a sas hotplug flow
may be divided into several works, e.g libsas receive a
PORTE_BYTES_DMAED event, currently we process it as following steps:
sas_form_port --- run in work in shost workq
sas_discover_domain --- run in another work in shost workq
...
sas_probe_devices --- run in new work in shost workq
We found during hot-add a device, libsas may need run several
works in same workqueue to add device in system, the process is
not atomic, it may interrupt by other sas event works, like
PHYE_LOSS_OF_SIGNAL.
This patch is preparation of execute libsas sas event in sync. We need
to use different workqueue to run sas event and disco event. Otherwise
the work will be blocked for waiting another chained work in the same
workqueue.

If the PHY burst too many events, we will alloc a lot of events for the
worker. This may leads to memory exhaustion.
Dan Williams suggested to shut down the PHY if the events reached the
threshold, because in this case the PHY may have gone into some
erroneous state. Users can re-enable the PHY by sysfs if they want.
We cannot use the fixed memory pool because if we run out of events, the
shut down event and loss of signal event will lost too. The events still
need to be allocated and processed in this case.

Now libsas hotplug work is static, every sas event type has its own
static work, LLDD driver queues the hotplug work into shost->work_q. If
LLDD driver burst posts lots hotplug events to libsas, the hotplug
events may pending in the workqueue like

In this case, a new PORTE_BYTES_DMAED event coming, libsas try to queue
it to shost->work_q, but this work is already pending, so it would be
lost. Finally, libsas delete the related sas port and sas devices, but
LLDD driver expect libsas add the sas port and devices(last sas event).
This patch use dynamic allocated work to avoid this issue.

If scsi_eh_scmd_add() is called concurrently with
scsi_host_queue_ready() while shost->host_blocked > 0 then it can
happen that neither function wakes up the SCSI error handler. Fix
this by making every function that decreases the host_busy counter
wake up the error handler if necessary and by protecting the
host_failed checks with the SCSI host lock.

The rps_resp buffer in ata_device is a DMA target, but it isn't
explicitly cacheline aligned. Due to this, adjacent fields can be
overwritten with stale data from memory on non-coherent architectures.
As a result, the kernel is sometimes unable to communicate with an SATA
device behind a SAS expander.
Fix this by ensuring that the rps_resp buffer is cacheline aligned.
This issue is similar to that fixed by Commit 84bda12af3 ("libata:
align ap->sector_buf") and Commit 4ee34ea3a1 ("libata: Align
ata_device's id on a cacheline").

The contexts from which a SCSI device can be quiesced or resumed are:
* Writing into /sys/class/scsi_device/*/device/state.
* SCSI parallel (SPI) domain validation.
* The SCSI device power management methods. See also scsi_bus_pm_ops.
It is essential during suspend and resume that neither the filesystem
state nor the filesystem metadata in RAM changes. This is why while
the hibernation image is being written or restored that SCSI devices
are quiesced. The SCSI core quiesces devices through scsi_device_quiesce()
and scsi_device_resume(). In the SDEV_QUIESCE state execution of
non-preempt requests is deferred. This is realized by returning
BLKPREP_DEFER from inside scsi_prep_state_check() for quiesced SCSI
devices. Avoid that a full queue prevents power management requests
to be submitted by deferring allocation of non-preempt requests for
devices in the quiesced state. This patch has been tested by running
the following commands and by verifying that after each resume the
fio job was still running:

Many source files in the tree are missing licensing information, which
makes it harder for compliance tools to determine the correct license.
By default all files without license information are under the default
license of the kernel, which is GPL version 2.
Update the files which contain no license information with the 'GPL-2.0'
SPDX license identifier. The SPDX identifier is a legally binding
shorthand, which can be used instead of the full boiler plate text.
This patch is based on work done by Thomas Gleixner and Kate Stewart and
Philippe Ombredanne.
How this work was done:
Patches were generated and checked against linux-4.14-rc6 for a subset of
the use cases:
- file had no licensing information it it.
- file was a */uapi/* one with no licensing information in it,
- file was a */uapi/* one with existing licensing information,
Further patches will be generated in subsequent months to fix up cases
where non-standard license headers were used, and references to license
had to be inferred by heuristics based on keywords.
The analysis to determine which SPDX License Identifier to be applied to
a file was done in a spreadsheet of side by side results from of the
output of two independent scanners (ScanCode & Windriver) producing SPDX
tag:value files created by Philippe Ombredanne. Philippe prepared the
base worksheet, and did an initial spot review of a few 1000 files.
The 4.13 kernel was the starting point of the analysis with 60,537 files
assessed. Kate Stewart did a file by file comparison of the scanner
results in the spreadsheet to determine which SPDX license identifier(s)
to be applied to the file. She confirmed any determination that was not
immediately clear with lawyers working with the Linux Foundation.
Criteria used to select files for SPDX license identifier tagging was:
- Files considered eligible had to be source code files.
- Make and config files were included as candidates if they contained >5
lines of source
- File already had some variant of a license header in it (even if <5
lines).
All documentation files were explicitly excluded.
The following heuristics were used to determine which SPDX license
identifiers to apply.
- when both scanners couldn't find any license traces, file was
considered to have no license information in it, and the top level
COPYING file license applied.
For non */uapi/* files that summary was:

- when the two scanners agreed on the detected license(s), that became
the concluded license(s).
- when there was disagreement between the two scanners (one detected a
license but the other didn't, or they both detected different
licenses) a manual inspection of the file occurred.
- In most cases a manual inspection of the information in the file
resulted in a clear resolution of the license that should apply (and
which scanner probably needed to revisit its heuristics).
- When it was not immediately clear, the license identifier was
confirmed with lawyers working with the Linux Foundation.
- If there was any question as to the appropriate license identifier,
the file was flagged for further research and to be revisited later
in time.
In total, over 70 hours of logged manual review was done on the
spreadsheet to determine the SPDX license identifiers to apply to the
source files by Kate, Philippe, Thomas and, in some cases, confirmation
by lawyers working with the Linux Foundation.
Kate also obtained a third independent scan of the 4.13 code base from
FOSSology, and compared selected files where the other two scanners
disagreed against that SPDX file, to see if there was new insights. The
Windriver scanner is based on an older version of FOSSology in part, so
they are related.
Thomas did random spot checks in about 500 files from the spreadsheets
for the uapi headers and agreed with SPDX license identifier in the
files he inspected. For the non-uapi files Thomas did random spot checks
in about 15000 files.
In initial set of patches against 4.14-rc6, 3 files were found to have
copy/paste license identifier errors, and have been fixed to reflect the
correct identifier.
Additionally Philippe spent 10 hours this week doing a detailed manual
inspection and review of the 12,461 patched files from the initial patch
version early this week with:
- a full scancode scan run, collecting the matched texts, detected
license ids and scores
- reviewing anything where there was a license detected (about 500+
files) to ensure that the applied SPDX license was correct
- reviewing anything where there was no detection but the patch license
was not GPL-2.0 WITH Linux-syscall-note to ensure that the applied
SPDX license was correct
This produced a worksheet with 20 files needing minor correction. This
worksheet was then exported into 3 different .csv files for the
different types of files to be modified.
These .csv files were then reviewed by Greg. Thomas wrote a script to
parse the csv files and add the proper SPDX tag to the file, in the
format that the file expected. This script was further refined by Greg
based on the output to detect more types of files automatically and to
distinguish between header and source .c files (which need different
comment types.) Finally Greg ran the script using the .csv files to
generate the patches.

In preparation for unconditionally passing the struct timer_list pointer to
all timer callbacks, switch to using the new timer_setup() and from_timer()
to pass the timer pointer explicitly. This requires adding a pointer to
hold the timer's target task, as there isn't a link back from slow_task.

As per SAM there is a status precedence, with any sense code 29/XX
taking second place just after an ACA ACTIVE status. Additionally, each
target might prefer to not queue any unit attention conditions, but just
report one. Due to the above, this will be that one with the highest
precedence. This results in the sense code 29/XX effectively
overwriting any other unit attention. Hence we should report the
power-on reset to userland so that it can take appropriate action.

Move standard macro definitions for the zone types and zone conditions
to scsi_proto.h together with the definitions related to the REPORT
ZONES command. While at it, define all values in the enums to be clear.
Also remove unnecessary includes in sd_zbc.c.

SBC-4 states:
"A MAXIMUM UNMAP LBA COUNT field set to a non-zero value indicates the
maximum number of LBAs that may be unmapped by an UNMAP command"
"A MAXIMUM WRITE SAME LENGTH field set to a non-zero value indicates
the maximum number of contiguous logical blocks that the device server
allows to be unmapped or written in a single WRITE SAME command."
Despite the spec being clear on the topic, some devices incorrectly
expect WRITE SAME commands with the UNMAP bit set to be limited to the
value reported in MAXIMUM UNMAP LBA COUNT in the Block Limits VPD.
Implement a blacklist option that can be used to accommodate devices
with this behavior.

If a pass-through request is submitted then blk_get_request()
initializes that request by calling scsi_initialize_rq(). Also call this
function for filesystem requests. Introduce CMD_INITIALIZED to keep
track of whether or not a request has already been initialized.

Simplify the SMP passthrough code by switching it to the generic bsg-lib
helpers that abstract away the details of the request code, and gets
drivers out of seeing struct scsi_request.
For the libsas host SMP code there is a small behavior difference in
that we now always clear the residual len for successful commands,
similar to the three other SMP handler implementations. Given that
there is no partial command handling in the host SMP handler this should
not matter in practice.

Introduce struct scsi_vpd for the VPD page length, data and the RCU head
that will be used to free the VPD data. Use kfree_rcu() instead of
kfree() to free VPD data. Move the VPD buffer pointer check inside the
RCU read lock in the sysfs code. Only annotate pointers that are shared
across threads with __rcu. Use rcu_dereference() when dereferencing an
RCU pointer. This patch suppresses about twenty sparse complaints about
the vpd_pg8[03] pointers. This patch also fixes a race condition, namely
that updating of the VPD pointers and length variables in struct
scsi_device was not atomic with reference to the code reading these

Since commit e9c787e65c ("scsi: allocate scsi_cmnd structures as
part of struct request") struct request and struct scsi_cmnd are
adjacent. This means that there is now an alternative to reading
req->special to convert a pointer to a prepared request into a
SCSI command pointer, namely by using blk_mq_rq_to_pdu(). Make
this change where appropriate. Although this patch does not
change any functionality, it slightly improves performance and
slightly improves readability.

Releasing a zone write lock only when the write commnand that acquired
the lock completes can cause deadlocks due to potential command
reordering if the lock owning request is requeued and not executed. This
problem exists only with the scsi-mq path as, unlike the legacy path,
requests are moved out of the dispatch queue before being prepared and
so before locking a zone for a write command.
Since sd_uninit_cmnd() is now always called when a request is requeued,
call sd_zbc_write_unlock_zone() from that function for write requests
that acquired a zone lock instead of from sd_done(). Acquisition of a zone
lock by a write command is indicated using the new command
flag SCMD_ZONE_WRITE_LOCK.

Scsi_cmnd is an unsuitable argument for eh_device_reset_handler(),
eh_target_reset_handler(), and eh_host_reset_handler() which do not have
the scope of one single SCSI command. These callbacks tend to use
fc_block_scsi_eh() requiring scsi_cmnd. In order to start decoupling
above eh callbacks from scsi_cmnd, introduce a new variant of the
function called fc_block_rport() taking an fc_rport as argument.
Refactor the old fc_block_scsi_eh() to simply delegate to
fc_block_rport().

The addition of the STARGET_REMOVE state had the side effect of
introducing a race condition that can cause a crash.
scsi_target_reap_ref_release() checks the starget->state to
see if it still in STARGET_CREATED, and if so, skips calling
transport_remove_device() and device_del(), because the starget->state
is only set to STARGET_RUNNING after scsi_target_add() has called
device_add() and transport_add_device().
However, if an rport loss occurs while a target is being scanned,
it can happen that scsi_remove_target() will be called while the
starget is still in the STARGET_CREATED state. In this case, the
starget->state will be set to STARGET_REMOVE, and as a result,
scsi_target_reap_ref_release() will take the wrong path. The end
result is a panic:

Just wire up the generic TCG OPAL infrastructure to the SCSI disk driver
and the Security In/Out commands.
Note that I don't know of any actual SCSI disks that do support TCG OPAL,
but this is required to support ATA disks through libata.

libsas uses scsi_queue_work() to queue its internal event notifications.
scsi_queue_work() can return -EINVAL if the work queue doesn't exist and
it does call queue_work() which can return false if the work is already
queued.
Make the SAS event code capable of returning errors up to the caller,
which is handy when changing to dynamically allocated work in libsas
as well, as discussed here: https://lkml.org/lkml/2017/6/14/121.

The symbolic name VLC_SA_RECEIVE_CREDENTIAL is not used anywhere in the
kernel. Additionally, since SPC 5 the RECEIVE CREDENTIAL command is
obsolete. The VLC_SA_RECEIVE_CREDENTIAL definition is misleading since
it occurs outside the list of other variable length CDB service action
codes (READ_32, WRITE_32, ...). Hence remove this definition.

Instead of explicitly calling scsi_req_init() after blk_get_request(),
call that function from inside blk_get_request(). Add an
.initialize_rq_fn() callback function to the block drivers that need
it. Merge the IDE .init_rq_fn() function into .initialize_rq_fn()
because it is too small to keep it as a separate function. Keep the
scsi_req_init() call in ide_prep_sense() because it follows a
blk_rq_init() call.

Serializing SCSI device state changes avoids that two state changes can
occur concurrently, e.g. the state changes in scsi_target_block() and
__scsi_remove_device(). This serialization is essential to make patch
"Make __scsi_remove_device go straight from BLOCKED to DEL" work
reliably.
Enable this mechanism for all scsi_target_*block() callers but not for
the scsi_internal_device_unblock() calls from the mpt3sas driver because
that driver can call scsi_internal_device_unblock() from atomic context.

Dereferencing shost from scsi_exit_rq() is not safe because the SCSI
host may already have been freed when scsi_exit_rq() is called.
Increasing the shost reference count in scsi_init_rq() and dropping that
reference in scsi_exit_rq() is nontrivial since scsi_host_dev_release()
may sleep and since scsi_exit_rq() may be called from interrupt
context. Since scsi_exit_rq() only needs a single bit from shost, copy
that bit into struct scsi_cmnd.

Currently we use nornal Linux errno values in the block layer, and while
we accept any error a few have overloaded magic meanings. This patch
instead introduces a new blk_status_t value that holds block layer specific
status codes and explicitly explains their meaning. Helpers to convert from
and to the previous special meanings are provided for now, but I suspect
we want to get rid of them in the long run - those drivers that have a
errno input (e.g. networking) usually get errnos that don't know about
the special block layer overloads, and similarly returning them to userspace
will usually return somethings that strictly speaking isn't correct
for file system operations, but that's left as an exercise for later.
For now the set of errors is a very limited set that closely corresponds
to the previous overloaded errno values, but there is some low hanging
fruite to improve it.
blk_status_t (ab)uses the sparse __bitwise annotations to allow for sparse
typechecking, so that we can easily catch places passing the wrong values.

Use get_unaligned_be32 and get_unaligned_be64 to obtain values from the
sense buffer instead of open coding the operations. Also change the
function return value to a bool and fix the function signature
declaration to remove spaces triggering checkpatch warnings.
No functional change is introduced by this patch.

This passes on the scsi_cmnd result field to users of passthrough
requests. Currently we abuse req->errors for this purpose, but that
field will go away in its current form.
Note that the old IDE code abuses the errors field in very creative
ways and stores all kinds of different values in it. I didn't dare
to touch this magic, so the abuses are brought forward 1:1.

This patch allows scsi drivers that expose virturalized fibre channel
devices but that do not expose rports to successfully rescan the scsi
bus via echo "- - -" > /sys/class/scsi_host/hostX/scan. Drivers can
create a pseudo rport and indicate FC_PORT_ROLE_FCP_DUMMY_INITIATOR as
the rport's role in fc_rport_identifiers. This insures that a valid
scsi_target_id is assigned to the newly created rport and it can meet
the requirements of fc_user_scan_tgt calling scsi_scan_target.

If a failed command is retried and fails again we need
to enter SCSI EH, otherwise we will never be able to
recover the command.
To detect this situation we must not clear scmd->eh_eflags
when EH finishes but rather make it persistent throughout
the lifetime of the command.

The current medium access timeout counter will be increased for
each command, so if there are enough failed commands we'll hit
the medium access timeout for even a single device failure and
the following kernel message is displayed:
sd H:C:T:L: [sdXY] Medium access timeout failure. Offlining disk!
Fix this by making the timeout per EH run, ie the counter will
only be increased once per device and EH run.

Commit 669f044170 ("scsi: srp_transport: Move queuecommand() wait code
to SCSI core") can make scsi_internal_device_block() sleep. However,
the mpt3sas driver can call this function from an interrupt
handler. Hence add a second argument to scsi_internal_device_block()
that restores the old behavior of this function for the mpt3sas handler.

There's a rather long standing regression from the commit "libiscsi:
Reduce locking contention in fast path"
Depending on iSCSI target behavior, it's possible to hit the case in
iscsi_complete_task where the task is still on a pending list
(!list_empty(&task->running)). When that happens the task is removed
from the list while holding the session back_lock, but other task list
modification occur under the frwd_lock. That leads to linked list
corruption and eventually a panicked system.
Rather than back out the session lock split entirely, in order to try
and keep some of the performance gains this patch adds another lock to
maintain the task lists integrity.
Major enterprise supported kernels have been backing out the lock split
for while now, thanks to the efforts at IBM where a lab setup has the
most reliable reproducer I've seen on this issue. This patch has been
tested there successfully.

Instead define the timeout behavior purely based on the host_template
eh_timed_out method and wire up the existing transport implementations
in the host templates. This also clears up the confusion that the
transport template method overrides the host template one, so some
drivers have to re-override the transport template one.

Provide fc_bsg_to_rport() helper that will become handy when we're

Export fc_bsg_jobdone so drivers can use it directly instead of doing
the round-trip via struct fc_bsg_job::job_done() and use it in the
LLDDs. That way we can also unify the interfaces of fc_bsg_jobdone and
bsg_job_done.
As we've converted all LLDDs over to use fc_bsg_jobdone() directly, we
can remove the function pointer from struct fc_bsg_job as well.

When we're receiving a timeout we should be checking for queue
full status; if there are still some packets pending we should
be resetting the counter to ensure we're not missing out any
packets which are still queued.

When a sequence times out we have no idea what happened to the
frame. And we do not know if we will ever receive the frame.
Hence we cannot re-use the xid as we would risk data corruption
if the xid had been re-used and the timed out frame would be
received after that.
So we need to quarantine the xid until the lport is reset.
Yes, I know this will (eventually) deplete the xid pool.
But for now it's the safest method.

A lot of the REQ_* flags are only used on struct requests, and only of
use to the block layer and a few drivers that dig into struct request
internals.
This patch adds a new req_flags_t rq_flags field to struct request for
them, and thus dramatically shrinks the number of common requests. It
also removes the unfortunate situation where we have to fit the fields
from the same enum into 32 bits for struct bio and 64 bits for
struct request.

Implement ZBC support functions to setup zoned disks, both
host-managed and host-aware models. Only zoned disks that satisfy
the following conditions are supported:
1) All zones are the same size, with the exception of an eventual
last smaller runt zone.
2) For host-managed disks, reads are unrestricted (reads are not
failed due to zone or write pointer alignement constraints).
Zoned disks that do not satisfy these 2 conditions are setup with
a capacity of 0 to prevent their use.
The function sd_zbc_read_zones, called from sd_revalidate_disk,
checks that the device satisfies the above two constraints. This
function may also change the disk capacity previously set by
sd_read_capacity for devices reporting only the capacity of
conventional zones at the beginning of the LBA range (i.e. devices
reporting rc_basis set to 0).
The capacity message output was moved out of sd_read_capacity into
a new function sd_print_capacity to include this eventual capacity
change by sd_zbc_read_zones. This new function also includes a call
to sd_zbc_print_zones to display the number of zones and zone size
of the device.

When running in VN2VN mode there is no central instance which would send
out any FIP VLAN discovery notifications. So this patch adds a new sysfs
attribute 'fip_vlan_responder' which will activate a FIP VLAN discovery
responder.
