Since ftrace has trampolines, don't use thunks for the __fentry__ site
but instead require that every function called from there includes
accounting. This very much includes all the direct-call functions.
Additionally, ftrace uses ROP tricks in two places:
- return_to_handler(), and
- ftrace_regs_caller() when pt_regs->orig_ax is set by a direct-call.
return_to_handler() already uses a retpoline to replace an
indirect-jump to defeat IBT, since this is a jump-type retpoline, make
sure there is no accounting done and ALTERNATIVE the RET into a ret.
ftrace_regs_caller() does much the same and gets the same treatment.

- Runtime verification infrastructure
This is the biggest change for this pull request. It introduces the
runtime verification that is necessary for running Linux on safety
critical systems. It allows for deterministic automata models to be
inserted into the kernel that will attach to tracepoints, where the
information on these tracepoints will move the model from state to state.
If a state is encountered that does not belong to the model, it will then
activate a given reactor, that could just inform the user or even panic
the kernel (for which safety critical systems will detect and can recover
from).
- Two monitor models are also added: Wakeup In Preemptive (WIP - not to be
confused with "work in progress"), and Wakeup While Not Running (WWNR).
- Added __vstring() helper to the TRACE_EVENT() macro to replace several
vsnprintf() usages that were all doing it wrong.
- eprobes now can have their event autogenerated when the event name is left
off.
- The rest is various cleanups and fixes.

Return trampoline must not use indirect branch to return; while this
preserves the RSB, it is fundamentally incompatible with IBT. Instead
use a retpoline like ROP gadget that defeats IBT while not unbalancing
the RSB.
And since ftrace_stub is no longer a plain RET, don't use it to copy

We don't need special hook for graph tracer entry point,
but instead we can use graph_ops::func function to install
the return_hooker.
This moves the graph tracing setup _before_ the direct
trampoline prepares the stack, so the return_hooker will
be called when the direct trampoline is finished.
This simplifies the code, because we don't need to take into
account the direct trampoline setup when preparing the graph
tracer hooker and we can allow function graph tracer on entries
registered with direct trampoline.

This ensures that a NOP is a NOP and not a random other instruction that
is also a NOP. It allows simplification of dynamic code patching that
wants to verify existing code before writing new instructions (ftrace,
jump_label, static_call, etc..).
Differentiating on NOPs is not a feature.
This pessimises 32bit (DONTCARE) and 32bit on 64bit CPUs (CARELESS).
32bit is not a performance target.
Everything x86_64 since AMD K10 (2007) and Intel IvyBridge (2012) is
fine with using NOPL (as opposed to prefix NOP). And per FEATURE_NOPL
being required for x86_64, all x86_64 CPUs can use NOPL. So stop
caring about NOPs, simplify things and get on with life.
[ The problem seems to be that some uarchs can only decode NOPL on a
single front-end port while others have severe decode penalties for
excessive prefixes. All modern uarchs can handle both, except Atom,
which has prefix penalties. ]
[ Also, much doubt you can actually measure any of this on normal
workloads. ]
After this, FEATURE_NOPL is unused except for required-features for
x86_64. FEATURE_K8 is only used for PTI.
[ bp: Kernel build measurements showed ~0.3s slowdown on Sandybridge
which is hardly a slowdown. Get rid of X86_FEATURE_K7, while at it. ]

If a direct hook is attached to a function that ftrace also has a function
attached to it, then it is required that the ftrace_ops_list_func() is used
to iterate over the registered ftrace callbacks. This will also include the
direct ftrace_ops helper, that tells ftrace_regs_caller where to return to
(the direct callback and not the function that called it).
As this direct helper is only to handle the case of ftrace callbacks
attached to the same function as the direct callback, the ftrace callback
allocated trampolines (used to only call them), should never be used to
return back to a direct callback.
Only copy the portion of the ftrace_regs_caller that will return back to
what called it, and not the portion that returns back to the direct caller.
The direct ftrace_ops must then pick the ftrace_regs_caller builtin function
as its own trampoline to ensure that it will never have one allocated for
it (which would not include the handling of direct callbacks).

I was able to trigger it on other machines, when I added to the kernel
command line of both "ftrace=function" and "trace_options=func_stack_trace".
The cause is the "ftrace=function" would register the function tracer
and create a trampoline, and it will set it as executable and
read-only. Then the "trace_options=func_stack_trace" would then update
the same trampoline to include the stack tracer version of the function
tracer. But since the trampoline already exists, it updates it with
text_poke_bp(). The problem is that text_poke_bp() called while
system_state == SYSTEM_BOOTING, it will simply do a memcpy() and not
the page mapping, as it would think that the text is still read-write.
But in this case it is not, and we take a fault and crash.
Instead, lets keep the ftrace trampolines read-write during boot up,
and then when the kernel executable text is set to read-only, the
ftrace trampolines get set to read-only as well.

The ftrace_regs_caller() trampoline does something 'funny' when there
is a direct-caller present. In that case it stuffs the 'direct-caller'
address on the return stack and then exits the function. This then
results in 'returning' to the direct-caller with the exact registers
we came in with -- an indirect tail-call without using a register.
This however (rightfully) confuses objtool because the function shares
a few instruction in order to have a single exit path, but the stack
layout is different for them, depending through which path we came
there.
This is currently cludged by forcing the stack state to the non-direct
case, but this generates actively wrong (ORC) unwind information for
the direct case, leading to potential broken unwinds.
Fix this issue by fully separating the exit paths. This results in
having to poke a second RET into the trampoline copy, see
ftrace_regs_caller_ret.
This brings us to a second objtool problem, in order for it to
perceive the 'jmp ftrace_epilogue' as a function exit, it needs to be
recognised as a tail call. In order to make that happen,
ftrace_epilogue needs to be the start of an STT_FUNC, so re-arrange
code to make this so.
Finally, a third issue is that objtool requires functions to exit with
the same stack layout they started with, which is obviously violated
in the direct case, employ the new HINT_RET_OFFSET to tell objtool
this is an expected exception.
Together, this results in generating correct ORC unwind information
for the ftrace_regs_caller() function and it's trampoline copies.

avoids having a second/different copy of that code around.
This also avoids ftrace violating the (new) W^X rule and avoids

As testing for direct calls from the function graph tracer adds a little
overhead (which is a lot when tracing every function), add a counter that
can be used to test if function_graph tracer needs to test for a direct
caller or not.
It would have been nicer if we could use a static branch, but the static
branch logic fails when used within the function graph tracer trampoline.

Possible unsafe locking scenario:
CPU0 CPU1
---- ----
lock(text_mutex);
lock(cpu_hotplug_lock.rw_sem);
lock(text_mutex);
lock(cpu_hotplug_lock.rw_sem);
It is similar problem that has been solved by the commit 2d1e38f566
("kprobes: Cure hotplug lock ordering issues"). Many locks are involved.
To be on the safe side, text_mutex must become a low level lock taken
after cpu_hotplug_lock.rw_sem.
This can't be achieved easily with the current ftrace design.
For example, arm calls set_all_modules_text_rw() already in
ftrace_arch_code_modify_prepare(), see arch/arm/kernel/ftrace.c.
This functions is called:
+ outside stop_machine() from ftrace_run_update_code()
+ without stop_machine() from ftrace_module_enable()
Fortunately, the problematic fix is needed only on x86_64. It is
the only architecture that calls set_all_modules_text_rw()
in ftrace path and supports livepatching at the same time.
Therefore it is enough to move text_mutex handling from the generic
kernel/trace/ftrace.c into arch/x86/kernel/ftrace.c:
ftrace_arch_code_modify_prepare()
ftrace_arch_code_modify_post_process()
This patch basically reverts the ftrace part of the problematic
commit 9f255b632b ("module: Fix livepatch/ftrace module
text permissions race"). And provides x86_64 specific-fix.
Some refactoring of the ftrace code will be needed when livepatching
is implemented for arm or nds32. These architectures call
set_all_modules_text_rw() and use stop_machine() at the same time.

Nicolai Stange discovered[1] that if live kernel patching is enabled, and the
function tracer started tracing the same function that was patched, the
conversion of the fentry call site during the translation of going from
calling the live kernel patch trampoline to the iterator trampoline, would
have as slight window where it didn't call anything.
As live kernel patching depends on ftrace to always call its code (to
prevent the function being traced from being called, as it will redirect
it). This small window would allow the old buggy function to be called, and
this can cause undesirable results.
Nicolai submitted new patches[2] but these were controversial. As this is
similar to the static call emulation issues that came up a while ago[3].
But after some debate[4][5] adding a gap in the stack when entering the
breakpoint handler allows for pushing the return address onto the stack to
easily emulate a call.

Live kernel patching is not implemented on x86_32, thus the emulate
calls are only for x86_64.

Since alloc_module() will not set the pages as executable soon, set
ftrace trampoline pages as executable after they are allocated.
For the time being, do not change ftrace to use the text_poke()
interface. As a result, ftrace still breaks W^X.
