The FSL driver is mangling the iommu_groups to not have a group for its
PCI bridge/controller (eg the thing passed to fsl_add_bridge()). Robin
says this is so FSL could work with VFIO which would be blocked by having
a probed driver on the platform_device in the same group. This is
supported by comments from FSL:

.. PCIe devices share the same device group as the PCI controller. This
becomes a problem while assigning the devices to the guest, as you are
required to unbind all the PCIe devices including the controller from the
host. PCIe controller can't be unbound from the host, so we simply delete
the controller iommu_group.
However, today, we use driver_managed_dma to allow PCI infrastructure
devices that are 'security safe' to co-exist in groups and still allow
VFIO to work. Set this flag for the fsl_pci_driver.
Change fsl_pamu_device_group() so that it no longer removes the controller
from any groups. For check_pci_ctl_endpt_part() mode this creates an extra
group that contains only the controller.
Otherwise force the controller's single group to be the group of all the
PCI devices on the controller's hose. VFIO continues to work because of
driver_managed_dma.
Remove the iommu_group_remove_device() calls from fsl_pamu and lightly
restructure its fsl_pamu_device_group() function.

fsl_pamu_device_group() is only called if dev->iommu_group is NULL, so
iommu_group_get() always returns NULL. Remove this test and just allocate
a group. Call generic_device_group() for this like the other drivers.

For those IOMMU drivers that don't provide default domain support, add an
implementation of set_platform_dma_ops callback so that the IOMMU core
could return the DMA control to platform DMA ops. At the same time, with
the set_platform_dma_ops implemented, there is no need for detach_dev.
Remove it to avoid dead code.

Following the new rules in include/linux/iommu.h kdocs, EINVAL now can be
used to indicate that domain and device are incompatible by a caller that
treats it as a soft failure and tries attaching to another domain.
On the other hand, there are ->attach_dev callback functions returning it
for obvious device-specific errors. They will result in some inefficiency
in the caller handling routine.
Update these places to corresponding errnos following the new rules.

Move the domain specific operations out of struct iommu_ops into a new
structure that only has domain specific operations. This solves the
problem of needing to know if the method vector for a given operation
needs to be retrieved from the device or the domain. Logically the domain
ops are the ones that make sense for external subsystems and endpoint
drivers to use, while device ops, with the sole exception of domain_alloc,
are IOMMU API internals.

The only thing that fsl_pamu_window_enable does for the current caller
is to fill in the prot value in the only dma_window structure, and to
propagate a few values from the iommu_domain_geometry struture into the
dma_window. Remove the dma_window entirely, hardcode the prot value and
otherwise use the iommu_domain_geometry structure instead.
Remove the now unused ->domain_window_enable iommu method.

The default geometry is the same as the one set by qman_port given
that FSL_PAMU depends on having 64-bit physical and thus DMA addresses.
Remove the support to update the geometry and remove the now pointless
geom_size field.

The external interface to get/set window attributes is already
abstracted behind iommu_domain_{get,set}_attr(), so there's no real
reason for the internal interface to be different. Since we only have
one window-based driver anyway, clean up the core code by just moving
the DOMAIN_ATTR_WINDOWS handling directly into the PAMU driver.

Commit 68a17f0be6 introduced an initialization order
problem, where devices are linked against an iommu which is
not yet initialized. Fix it by initializing the iommu-device
before the iommu-ops are registered against the bus.

Currently a PAMU driver patch is very likely to receive some
checkpatch complaints about the code in the context of the
patch. This patch is an attempt to fix most of that and make
the driver more readable
Also fixed a subset of the sparse and coccinelle reported
issues.

is_power_of_2 requires an unsigned long parameter which would
lead to truncation of 64 bit values on 32 bit architectures.
__ffs also expects an unsigned long parameter thus won't work
for 64 bit values on 32 bit architectures.
