358cd9afd0 xen/pci: add flag for PCI passthrough being possible
When running as a Xen PV guests passed through PCI devices only have a
chance to work if the Xen supplied memory map has some PCI space
reserved.

Add a flag xen_pv_pci_possible which will be set in early boot in case
the memory map has at least one area with the type E820_TYPE_RESERVED.

073828e954 ACPI: processor: Fix evaluating _PDC method when running as Xen dom0
In ACPI systems, the OS can direct power management, as opposed to the
firmware.  This OS-directed Power Management is called OSPM.  Part of
telling the firmware that the OS going to direct power management is
making ACPI "_PDC" (Processor Driver Capabilities) calls.  These _PDC
methods must be evaluated for every processor object.  If these _PDC
calls are not completed for every processor it can lead to
inconsistency and later failures in things like the CPU frequency
driver.

In a Xen system, the dom0 kernel is responsible for system-wide power
management.  The dom0 kernel is in charge of OSPM.  However, the
number of CPUs available to dom0 can be different than the number of
CPUs physically present on the system.

This leads to a problem: the dom0 kernel needs to evaluate _PDC for
all the processors, but it can't always see them.

In dom0 kernels, ignore the existing ACPI method for determining if a
processor is physically present because it might not be accurate.
Instead, ask the hypervisor for this information.

Fix this by introducing a custom function to use when running as Xen
dom0 in order to check whether a processor object matches a CPU that's
online.  Such checking is done using the existing information fetched
by the Xen pCPU subsystem, extending it to also store the ACPI ID.

This ensures that _PDC method gets evaluated for all physically online
CPUs, regardless of the number of CPUs made available to dom0.

251e90e7e3 xen: don't require virtio with grants for non-PV guests
Commit fa1f57421e ("xen/virtio: Enable restricted memory access using
Xen grant mappings") introduced a new requirement for using virtio
devices: the backend now needs to support the VIRTIO_F_ACCESS_PLATFORM
feature.

This is an undue requirement for non-PV guests, as those can be operated
with existing backends without any problem, as long as those backends
are running in dom0.

Per default allow virtio devices without grant support for non-PV
guests.

On Arm require VIRTIO_F_ACCESS_PLATFORM for devices having been listed
in the device tree to use grants.

Add a new config item to always force use of grants for virtio.

a603002eea virtio: replace restricted mem access flag with callback
Instead of having a global flag to require restricted memory access
for all virtio devices, introduce a callback which can select that
requirement on a per-device basis.

For convenience add a common function returning always true, which can
be used for use cases like SEV.

Per default use a callback always returning false.

As the callback needs to be set in early init code already, add a
virtio anchor which is builtin in case virtio is enabled.

fa1f57421e xen/virtio: Enable restricted memory access using Xen grant mappings
In order to support virtio in Xen guests add a config option XEN_VIRTIO
enabling the user to specify whether in all Xen guests virtio should
be able to access memory via Xen grant mappings only on the host side.

Also set PLATFORM_VIRTIO_RESTRICTED_MEM_ACCESS feature from the guest
initialization code on Arm and x86 if CONFIG_XEN_VIRTIO is enabled.

d1a928eac7 xen/unpopulated-alloc: Add mechanism to use Xen resource
The main reason of this change is that unpopulated-alloc
code cannot be used in its current form on Arm, but there
is a desire to reuse it to avoid wasting real RAM pages
for the grant/foreign mappings.

The problem is that system "iomem_resource" is used for
the address space allocation, but the really unallocated
space can't be figured out precisely by the domain on Arm
without hypervisor involvement. For example, not all device
I/O regions are known by the time domain starts creating
grant/foreign mappings. And following the advise from
"iomem_resource" we might end up reusing these regions by
a mistake. So, the hypervisor which maintains the P2M for
the domain is in the best position to provide unused regions
of guest physical address space which could be safely used
to create grant/foreign mappings.

Introduce new helper arch_xen_unpopulated_init() which purpose
is to create specific Xen resource based on the memory regions
provided by the hypervisor to be used as unused space for Xen
scratch pages. If arch doesn't define arch_xen_unpopulated_init()
the default "iomem_resource" will be used.

Update the arguments list of allocate_resource() in fill_list()
to always allocate a region from the hotpluggable range
(maximum possible addressable physical memory range for which
the linear mapping could be created). If arch doesn't define
arch_get_mappable_range() the default range (0,-1) will be used.

The behaviour on x86 won't be changed by current patch as both
arch_xen_unpopulated_init() and arch_get_mappable_range()
are not implemented for it.

Also fallback to allocate xenballooned pages (balloon out RAM
pages) if we do not have any suitable resource to work with
(target_resource is invalid) and as the result we won't be able
to provide unpopulated pages on a request.

9dd060afe2 xen/balloon: Bring alloc(free)_xenballooned_pages helpers back
This patch rolls back some of the changes introduced by commit
121f2faca2 "xen/balloon: rename alloc/free_xenballooned_pages"
in order to make possible to still allocate xenballooned pages
if CONFIG_XEN_UNPOPULATED_ALLOC is enabled.

On Arm the unpopulated pages will be allocated on top of extended
regions provided by Xen via device-tree (the subsequent patches
will add required bits to support unpopulated-alloc feature on Arm).
The problem is that extended regions feature has been introduced
into Xen quite recently (during 4.16 release cycle). So this
effectively means that Linux must only use unpopulated-alloc on Arm
if it is running on "new Xen" which advertises these regions.
But, it will only be known after parsing the "hypervisor" node
at boot time, so before doing that we cannot assume anything.

In order to keep working if CONFIG_XEN_UNPOPULATED_ALLOC is enabled
and the extended regions are not advertised (Linux is running on
"old Xen", etc) we need the fallback to alloc_xenballooned_pages().

This way we wouldn't reduce the amount of memory usable (wasting
RAM pages) for any of the external mappings anymore (and eliminate
XSA-300) with "new Xen", but would be still functional ballooning
out RAM pages with "old Xen".

Also rename alloc(free)_xenballooned_pages to xen_alloc(free)_ballooned_pages
and make xen_alloc(free)_unpopulated_pages static inline in xen.h
if CONFIG_XEN_UNPOPULATED_ALLOC is disabled.

121f2faca2 xen/balloon: rename alloc/free_xenballooned_pages
alloc_xenballooned_pages() and free_xenballooned_pages() are used as
direct replacements of xen_alloc_unpopulated_pages() and
xen_free_unpopulated_pages() in case CONFIG_XEN_UNPOPULATED_ALLOC isn't
defined.

Guard both functions with !CONFIG_XEN_UNPOPULATED_ALLOC and rename them
to the xen_*() variants they are replacing. This allows to remove some
ifdeffery from the xen.h header file. Adapt the prototype of the
functions to match.

9e2369c06c xen: add helpers to allocate unpopulated memory
To be used in order to create foreign mappings. This is based on the
ZONE_DEVICE facility which is used by persistent memory devices in
order to create struct pages and kernel virtual mappings for the IOMEM
areas of such devices. Note that on kernels without support for
ZONE_DEVICE Xen will fallback to use ballooned pages in order to
create foreign mappings.

The newly added helpers use the same parameters as the existing
{alloc/free}_xenballooned_pages functions, which allows for in-place
replacement of the callers. Once a memory region has been added to be
used as scratch mapping space it will no longer be released, and pages
returned are kept in a linked list. This allows to have a buffer of
pages and prevents resorting to frequent additions and removals of
regions.

If enabled (because ZONE_DEVICE is supported) the usage of the new
functionality untangles Xen balloon and RAM hotplug from the usage of
unpopulated physical memory ranges to map foreign pages, which is the
correct thing to do in order to avoid mappings of foreign pages depend
on memory hotplug.

Note the driver is currently not enabled on Arm platforms because it
would interfere with the identity mapping required on some platforms.

8cee3974b3 xen/pvh: Move Xen specific PVH VM initialization out of common file
We need to refactor PVH entry code so that support for other hypervisors
like Qemu/KVM can be added more easily.

This patch moves the small block of code used for initializing Xen PVH
virtual machines into the Xen specific file. This initialization is not
going to be needed for Qemu/KVM guests. Moving it out of the common file
is going to allow us to compile kernels in the future without CONFIG_XEN
that are still capable of being booted as a Qemu/KVM guest via the PVH
entry point.

b24413180f License cleanup: add SPDX GPL-2.0 license identifier to files with no license
Many source files in the tree are missing licensing information, which
makes it harder for compliance tools to determine the correct license.

By default all files without license information are under the default
license of the kernel, which is GPL version 2.

Update the files which contain no license information with the 'GPL-2.0'
SPDX license identifier.  The SPDX identifier is a legally binding
shorthand, which can be used instead of the full boiler plate text.

This patch is based on work done by Thomas Gleixner and Kate Stewart and
Philippe Ombredanne.

How this work was done:

Patches were generated and checked against linux-4.14-rc6 for a subset of
the use cases:
 - file had no licensing information it it.
 - file was a */uapi/* one with no licensing information in it,
 - file was a */uapi/* one with existing licensing information,

Further patches will be generated in subsequent months to fix up cases
where non-standard license headers were used, and references to license
had to be inferred by heuristics based on keywords.

The analysis to determine which SPDX License Identifier to be applied to
a file was done in a spreadsheet of side by side results from of the
output of two independent scanners (ScanCode & Windriver) producing SPDX
tag:value files created by Philippe Ombredanne.  Philippe prepared the
base worksheet, and did an initial spot review of a few 1000 files.

The 4.13 kernel was the starting point of the analysis with 60,537 files
assessed.  Kate Stewart did a file by file comparison of the scanner
results in the spreadsheet to determine which SPDX license identifier(s)
to be applied to the file. She confirmed any determination that was not
immediately clear with lawyers working with the Linux Foundation.

Criteria used to select files for SPDX license identifier tagging was:
 - Files considered eligible had to be source code files.
 - Make and config files were included as candidates if they contained >5
   lines of source
 - File already had some variant of a license header in it (even if <5
   lines).

All documentation files were explicitly excluded.

The following heuristics were used to determine which SPDX license
identifiers to apply.

 - when both scanners couldn't find any license traces, file was
   considered to have no license information in it, and the top level
   COPYING file license applied.

   For non */uapi/* files that summary was:

 - when the two scanners agreed on the detected license(s), that became
   the concluded license(s).

 - when there was disagreement between the two scanners (one detected a
   license but the other didn't, or they both detected different
   licenses) a manual inspection of the file occurred.

 - In most cases a manual inspection of the information in the file
   resulted in a clear resolution of the license that should apply (and
   which scanner probably needed to revisit its heuristics).

 - When it was not immediately clear, the license identifier was
   confirmed with lawyers working with the Linux Foundation.

 - If there was any question as to the appropriate license identifier,
   the file was flagged for further research and to be revisited later
   in time.

In total, over 70 hours of logged manual review was done on the
spreadsheet to determine the SPDX license identifiers to apply to the
source files by Kate, Philippe, Thomas and, in some cases, confirmation
by lawyers working with the Linux Foundation.

Kate also obtained a third independent scan of the 4.13 code base from
FOSSology, and compared selected files where the other two scanners
disagreed against that SPDX file, to see if there was new insights.  The
Windriver scanner is based on an older version of FOSSology in part, so
they are related.

Thomas did random spot checks in about 500 files from the spreadsheets
for the uapi headers and agreed with SPDX license identifier in the
files he inspected. For the non-uapi files Thomas did random spot checks
in about 15000 files.

In initial set of patches against 4.14-rc6, 3 files were found to have
copy/paste license identifier errors, and have been fixed to reflect the
correct identifier.

Additionally Philippe spent 10 hours this week doing a detailed manual
inspection and review of the 12,461 patched files from the initial patch
version early this week with:
 - a full scancode scan run, collecting the matched texts, detected
   license ids and scores
 - reviewing anything where there was a license detected (about 500+
   files) to ensure that the applied SPDX license was correct
 - reviewing anything where there was no detection but the patch license
   was not GPL-2.0 WITH Linux-syscall-note to ensure that the applied
   SPDX license was correct

This produced a worksheet with 20 files needing minor correction.  This
worksheet was then exported into 3 different .csv files for the
different types of files to be modified.

These .csv files were then reviewed by Greg.  Thomas wrote a script to
parse the csv files and add the proper SPDX tag to the file, in the
format that the file expected.  This script was further refined by Greg
based on the output to detect more types of files automatically and to
distinguish between header and source .c files (which need different
comment types.)  Finally Greg ran the script using the .csv files to
generate the patches.

7243b93345 xen/pvh: Bootstrap PVH guest
Start PVH guest at XEN_ELFNOTE_PHYS32_ENTRY address. Setup hypercall
page, initialize boot_params, enable early page tables.

Since this stub is executed before kernel entry point we cannot use
variables in .bss which is cleared by kernel. We explicitly place
variables that are initialized here into .data.

While adjusting xen_hvm_init_shared_info() make it use cpuid_e?x()
instead of cpuid() (wherever possible).

063334f305 xen/x86: Remove PVH support
We are replacing existing PVH guests with new implementation.

We are keeping xen_pvh_domain() macro (for now set to zero) because
when we introduce new PVH implementation later in this series we will
reuse current PVH-specific code (xen_pvh_gnttab_setup()), and that
code is conditioned by 'if (xen_pvh_domain())'. (We will also need
a noop xen_pvh_domain() for !CONFIG_XEN_PVH).

72a9b18629 xen: Remove event channel notification through Xen PCI platform device
Ever since commit 254d1a3f02 ("xen/pv-on-hvm kexec: shutdown watches
from old kernel") using the INTx interrupt from Xen PCI platform
device for event channel notification would just lockup the guest
during bootup.  postcore_initcall now calls xs_reset_watches which
will eventually try to read a value from XenStore and will get stuck
on read_reply at XenBus forever since the platform driver is not
probed yet and its INTx interrupt handler is not registered yet. That
means that the guest can not be notified at this moment of any pending
event channels and none of the per-event handlers will ever be invoked
(including the XenStore one) and the reply will never be picked up by
the kernel.

The exact stack where things get stuck during xenbus_init:

-xenbus_init
 -xs_init
  -xs_reset_watches
   -xenbus_scanf
    -xenbus_read
     -xs_single
      -xs_single
       -xs_talkv

Vector callbacks have always been the favourite event notification
mechanism since their introduction in commit 38e20b07ef ("x86/xen:
event channels delivery on HVM.") and the vector callback feature has
always been advertised for quite some time by Xen that's why INTx was
broken for several years now without impacting anyone.

Luckily this also means that event channel notification through INTx
is basically dead-code which can be safely removed without impacting
anybody since it has been effectively disabled for more than 4 years
with nobody complaining about it (at least as far as I'm aware of).

This commit removes event channel notification through Xen PCI
platform device.

ddc416cbc4 xen/pvh/x86: Define what an PVH guest is (v3).
Which is a PV guest with auto page translation enabled
and with vector callback. It is a cross between PVHVM and PV.

The Xen side defines PVH as (from docs/misc/pvh-readme.txt,
with modifications):

"* the guest uses auto translate:
 - p2m is managed by Xen
 - pagetables are owned by the guest
 - mmu_update hypercall not available
* it uses event callback and not vlapic emulation,
* IDT is native, so set_trap_table hcall is also N/A for a PVH guest.

For a full list of hcalls supported for PVH, see pvh_hypercall64_table
in arch/x86/hvm/hvm.c in xen.  From the ABI prespective, it's mostly a
PV guest with auto translate, although it does use hvm_op for setting
callback vector."

Also we use the PV cpuid, albeit we can use the HVM (native) cpuid.
However, we do have a fair bit of filtering in the xen_cpuid and
we can piggyback on that until the hypervisor/toolstack filters
the appropiate cpuids. Once that is done we can swap over to
use the native one.

We setup a Kconfig entry that is disabled by default and
cannot be enabled.

Note that on ARM the concept of PVH is non-existent. As Ian
put it: "an ARM guest is neither PV nor HVM nor PVHVM.
It's a bit like PVH but is different also (it's further towards
the H end of the spectrum than even PVH).". As such these
options (PVHVM, PVH) are never enabled nor seen on ARM
compilations.

initial Xen support") PV on HVM guests can be xen_initial_domain.
However PV on HVM guests might have an unitialized xen_start_info, so
check before accessing its fields.

4c071ee526 arm: initial Xen support
- Basic hypervisor.h and interface.h definitions.
- Skeleton enlighten.c, set xen_start_info to an empty struct.
- Make xen_initial_domain dependent on the SIF_PRIVILIGED_BIT.

The new code only compiles when CONFIG_XEN is set, that is going to be
added to arch/arm/Kconfig in patch #11 "xen/arm: introduce CONFIG_XEN on
ARM".
