Previously nbd_index_mutex was held during whole add/remove/lookup
operations in order to guarantee that partially initialized devices are
not reachable via idr_find() or idr_for_each(). But now that partially
initialized devices become reachable as soon as idr_alloc() succeeds,
we need to skip partially initialized devices. Since it seems that
all functions use refcount_inc_not_zero(&nbd->refs) in order to skip
destroying devices, update nbd->refs from zero to non-zero as the last
step of device initialization in order to also skip partially initialized
devices.

Syzbot hit WARNING in internal_create_group(). The problem was in
too big disk->first_minor.
disk->first_minor is initialized by value, which comes from userspace
and there wasn't any sanity checks about value correctness. It can cause
duplicate creation of sysfs files/links, because disk->first_minor will
be passed to MKDEV() which causes truncation to byte. Since maximum
minor value is 0xff, let's check if first_minor is correct minor number.
NOTE: the root case of the reported warning was in wrong error handling
in register_disk(), but we can avoid passing knowingly wrong values to
sysfs API, because sysfs error messages can confuse users. For example:
user passed 1048576 as index, but sysfs complains about duplicate
creation of /dev/block/43:0. It's not obvious how 1048576 becomes 0.
Log and reproducer for above example can be found on syzkaller bug
report page.

Now open_mutex is used to synchronize partition operations (e.g,
blk_drop_partitions() and blkdev_reread_part()), however it makes
nbd driver broken, because nbd may call del_gendisk() in nbd_release()
or nbd_genl_disconnect() if NBD_CFLAG_DESTROY_ON_DISCONNECT is enabled,
and deadlock occurs, as shown below:
// AB-BA dead-lock
nbd_genl_disconnect blkdev_open
nbd_disconnect_and_put
lock bd_mutex
// last ref
nbd_put
lock nbd_index_mutex
del_gendisk
nbd_open
try lock nbd_index_mutex
try lock bd_mutex
or
// AA dead-lock
nbd_release
lock bd_mutex
nbd_put
try lock bd_mutex
Instead of fixing block layer (e.g, introduce another lock), fixing
the nbd driver to call del_gendisk() in a kworker when
NBD_DESTROY_ON_DISCONNECT is enabled. When NBD_DESTROY_ON_DISCONNECT
is disabled, nbd device will always be destroy through module removal,
and there is no risky of deadlock.
To ensure the reuse of nbd index succeeds, moving the calling of
idr_remove() after del_gendisk(), so if the reused index is not found
in nbd_index_idr, the old disk must have been deleted. And reusing
the existing destroy_complete mechanism to ensure nbd_genl_connect()
will wait for the completion of del_gendisk().
Also adding a new workqueue for nbd removal, so nbd_cleanup()
can ensure all removals complete before exits.

We noticed that the user interface of Android devices becomes very slow
under memory pressure. This is because Android uses the zram driver on top
of the loop driver for swapping, because under memory pressure the swap
code alternates reads and writes quickly, because mq-deadline is the
default scheduler for loop devices and because mq-deadline delays writes by
five seconds for such a workload with default settings. Fix this by making
the kernel select I/O scheduler 'none' from inside add_disk() for loop
devices. This default can be overridden at any time from user space,
e.g. via a udev rule. This approach has an advantage compared to changing
the I/O scheduler from userspace from 'mq-deadline' into 'none', namely
that synchronize_rcu() does not get called.
This patch changes the default I/O scheduler for loop devices from
'mq-deadline' into 'none'.
Additionally, this patch reduces the Android boot time on my test setup
with 0.5 seconds compared to configuring the loop I/O scheduler from user
space.

Commit 6cc8e74308 ("loop: scale loop device by introducing per
device lock") re-opened a race window for NULL pointer dereference at
loop_validate_file() where commit 310ca162d7 ("block/loop: Use
global lock for ioctl() operation.") has closed.
Although we need to guarantee that other loop devices will not change
during traversal, we can't take remote "struct loop_device"->lo_mutex
inside loop_validate_file() in order to avoid AB-BA deadlock. Therefore,
introduce a global lock dedicated for loop_validate_file() which is
conditionally taken before local "struct loop_device"->lo_mutex is taken.

Reviewed-by: Christoph Hellwig <hch@lst.de>
Currently rbd_quiesce_lock() holds lock_rwsem for read while blocking
on releasing_wait completion. On the I/O completion side, each image
request also needs to take lock_rwsem for read. Because rw_semaphore
implementation doesn't allow new readers after a writer has indicated
interest in the lock, this can result in a deadlock if something that
needs to take lock_rwsem for write gets involved. For example:
1. watch error occurs
2. rbd_watch_errcb() takes lock_rwsem for write, clears owner_cid and
releases lock_rwsem
3. after reestablishing the watch, rbd_reregister_watch() takes
lock_rwsem for write and calls rbd_reacquire_lock()
4. rbd_quiesce_lock() downgrades lock_rwsem to for read and blocks on
releasing_wait until running_list becomes empty
5. another watch error occurs
6. rbd_watch_errcb() blocks trying to take lock_rwsem for write
7. no in-flight image request can complete and delete itself from
running_list because lock_rwsem won't be granted anymore
A similar scenario can occur with "lock has been acquired" and "lock
has been released" notification handers which also take lock_rwsem for
write to update owner_cid.
We don't actually get anything useful from sitting on lock_rwsem in
rbd_quiesce_lock() -- owner_cid updates certainly don't need to be
synchronized with. In fact the whole owner_cid tracking logic could
probably be removed from the kernel client because we don't support
proxied maintenance operations.

Skipping the "lock has been released" notification if the lock owner
is not what we expect based on owner_cid can lead to I/O hangs.
One example is our own notifications: because owner_cid is cleared
in rbd_unlock(), when we get our own notification it is processed as
unexpected/duplicate and maybe_kick_acquire() isn't called. If a peer
that requested the lock then doesn't go through with acquiring it,
I/O requests that came in while the lock was being quiesced would
be stalled until another I/O request is submitted and kicks acquire
from rbd_img_exclusive_lock().
This makes the comment in rbd_release_lock() actually true: prior to
this change the canceled work was being requeued in response to the
"lock has been acquired" notification from rbd_handle_acquired_lock().

Problem:
On reconfigure of device, there is no way to defend if the backend
storage is matching with the initial backend storage.
Say, if an initial connect request for backend "pool1/image1" got
mapped to /dev/nbd0 and the userspace process is terminated. A next
reconfigure request within NBD_ATTR_DEAD_CONN_TIMEOUT is allowed to
use /dev/nbd0 for a different backend "pool1/image2"
For example, an operation like below could be dangerous:
$ sudo rbd-nbd map --try-netlink rbd-pool/ext4-image
/dev/nbd0
$ sudo blkid /dev/nbd0
/dev/nbd0: UUID="bfc444b4-64b1-418f-8b36-6e0d170cfc04" TYPE="ext4"
$ sudo pkill -9 rbd-nbd
$ sudo rbd-nbd attach --try-netlink --device /dev/nbd0 rbd-pool/xfs-image
/dev/nbd0
$ sudo blkid /dev/nbd0
/dev/nbd0: UUID="d29bf343-6570-4069-a9ea-2fa156ced908" TYPE="xfs"
Solution:
Provide a way for userspace processes to keep some metadata to identify
between the device and the backend, so that when a reconfigure request is
made, we can compare and avoid such dangerous operations.
With this solution, as part of the initial connect request, backend
path can be stored in the sysfs per device config, so that on a reconfigure
request it's easy to check if the backend path matches with the initial
connect backend path.
Please note, ioctl interface to nbd will not have these changes, as there
won't be any reconfigure.

The current code only associates with the existing blkcg when aio is used
to access the backing file. This patch covers all types of i/o to the
backing file and also associates the memcg so if the backing file is on
tmpfs, memory is charged appropriately.
This patch also exports cgroup_get_e_css and int_active_memcg so it can be
used by the loop module.

Patch series "Charge loop device i/o to issuing cgroup", v14.
The loop device runs all i/o to the backing file on a separate kworker
thread which results in all i/o being charged to the root cgroup. This
allows a loop device to be used to trivially bypass resource limits and
other policy. This patch series fixes this gap in accounting.
A simple script to demonstrate this behavior on cgroupv2 machine:
'''
#!/bin/bash
set -e
CGROUP=/sys/fs/cgroup/test.slice
LOOP_DEV=/dev/loop0
if [[ ! -d $CGROUP ]]
then
sudo mkdir $CGROUP
fi
grep oom_kill $CGROUP/memory.events
# Set a memory limit, write more than that limit to tmpfs -> OOM kill
sudo unshare -m bash -c "
echo \$\$ > $CGROUP/cgroup.procs;
echo 0 > $CGROUP/memory.swap.max;
echo 64M > $CGROUP/memory.max;
mount -t tmpfs -o size=512m tmpfs /tmp;
dd if=/dev/zero of=/tmp/file bs=1M count=256" || true
grep oom_kill $CGROUP/memory.events
# Set a memory limit, write more than that limit through loopback
# device -> no OOM kill
sudo unshare -m bash -c "
echo \$\$ > $CGROUP/cgroup.procs;
echo 0 > $CGROUP/memory.swap.max;
echo 64M > $CGROUP/memory.max;
mount -t tmpfs -o size=512m tmpfs /tmp;
truncate -s 512m /tmp/backing_file
losetup $LOOP_DEV /tmp/backing_file
dd if=/dev/zero of=$LOOP_DEV bs=1M count=256;
losetup -D $LOOP_DEV" || true
grep oom_kill $CGROUP/memory.events
'''
Naively charging cgroups could result in priority inversions through the
single kworker thread in the case where multiple cgroups are
reading/writing to the same loop device. This patch series does some
minor modification to the loop driver so that each cgroup can make forward
progress independently to avoid this inversion.
With this patch series applied, the above script triggers OOM kills when
writing through the loop device as expected.
This patch (of 3):
Existing uses of loop device may have multiple cgroups reading/writing to
the same device. Simply charging resources for I/O to the backing file
could result in priority inversion where one cgroup gets synchronously
blocked, holding up all other I/O to the loop device.
In order to avoid this priority inversion, we use a single workqueue where
each work item is a "struct loop_worker" which contains a queue of struct
loop_cmds to issue. The loop device maintains a tree mapping blk css_id
-> loop_worker. This allows each cgroup to independently make forward
progress issuing I/O to the backing file.
There is also a single queue for I/O associated with the rootcg which can
be used in cases of extreme memory shortage where we cannot allocate a
loop_worker.
The locking for the tree and queues is fairly heavy handed - we acquire a
per-loop-device spinlock any time either is accessed. The existing
implementation serializes all I/O through a single thread anyways, so I
don't believe this is any worse.

Commit c76f48eb5c ("block: take bd_mutex around delete_partitions in
del_gendisk") adds disk->part0->bd_mutex in del_gendisk(), this way
causes the following AB/BA deadlock between removing loop and opening
loop:
1) loop_control_ioctl(LOOP_CTL_REMOVE)
-> mutex_lock(&loop_ctl_mutex)
-> del_gendisk
-> mutex_lock(&disk->part0->bd_mutex)
2) blkdev_get_by_dev
-> mutex_lock(&disk->part0->bd_mutex)
-> lo_open
-> mutex_lock(&loop_ctl_mutex)
Add a new Lo_deleting state to remove the need for clearing
->private_data and thus holding loop_ctl_mutex in the ioctl
LOOP_CTL_REMOVE path.
Based on an analysis and earlier patch from
Ming Lei <ming.lei@redhat.com>.

The error handling on a nullb->disk allocation currently jumps to
out_cleanup_disk that calls blk_cleanup_disk with a null pointer causing

The driver core ignores the return value of struct bus_type::remove()
because there is only little that can be done. To simplify the quest to
make this function return void, let struct vio_driver::remove() return
void, too. All users already unconditionally return 0, this commit makes
it obvious that returning an error code is a bad idea and should prevent
that future driver authors consider returning an error code.
Note there are two nominally different implementations for a vio bus:
one in arch/sparc/kernel/vio.c and the other in
arch/powerpc/platforms/pseries/vio.c. This patch only addresses the
former.

My UEK-derived config has 1030 files depending on pagemap.h before this
change. Afterwards, just 326 files need to be rebuilt when I touch
pagemap.h. I think blkdev.h is probably included too widely, but
untangling that dependency is harder and this solves my problem. x86
allmodconfig builds, but there may be implicit include problems on other
architectures.

In null_init, null_add_dev(dev) is called.
In null_add_dev, it calls null_free_zoned_dev(dev) to free dev->zones
via kvfree(dev->zones) in out_cleanup_zone branch and returns err.
Then null_init accept the err code and then calls null_free_dev(dev).
But in null_free_dev(dev), dev->zones is freed again by
null_free_zoned_dev().
My patch set dev->zones to NULL in null_free_zoned_dev() after
kvfree(dev->zones) is called, to avoid the double free.

Prior to commit 4a8c31a1c6 ("xen/blkback: rework connect_ring() to avoid
inconsistent xenstore 'ring-page-order' set by malicious blkfront"), the
behaviour of xen-blkback when connecting to a frontend was:
- read 'ring-page-order'
- if not present then expect a single page ring specified by 'ring-ref'
- else expect a ring specified by 'ring-refX' where X is between 0 and
1 << ring-page-order
This was correct behaviour, but was broken by the afforementioned commit to
become:
- read 'ring-page-order'
- if not present then expect a single page ring (i.e. ring-page-order = 0)
- expect a ring specified by 'ring-refX' where X is between 0 and
1 << ring-page-order
- if that didn't work then see if there's a single page ring specified by
'ring-ref'
This incorrect behaviour works most of the time but fails when a frontend
that sets 'ring-page-order' is unloaded and replaced by one that does not
because, instead of reading 'ring-ref', xen-blkback will read the stale
'ring-ref0' left around by the previous frontend will try to map the wrong
grant reference.
This patch restores the original behaviour.

While the maximum size of each ramdisk is defined either as a module
parameter, or compile time default, it's impossible to know how many pages
have currently been allocated by each ram%d device, since they're
allocated when used and never freed.
This patch creates a new directory at this location:
/sys/kernel/debug/ramdisk_pages/
which will contain a file named "ram%d" for each instantiated ramdisk on
the system. The file is read-only, and read() will output the number of
pages currently held by that ramdisk.
We lose track how much memory a ramdisk is using as pages once used are
simply recycled but never freed.
In instances where we exhaust the size of the ramdisk with a file that
exceeds it, encounter ENOSPC and delete the file for mitigation; df would
show decrease in used and increase in available blocks but the since we
have touched all pages, the memory footprint of the ramdisk does not
reflect the blocks used/available count

RNBD can make double-queues for irq-mode and poll-mode.
For example, on 4-CPU system 8 request-queues are created,
4 for irq-mode and 4 for poll-mode.
If the IO has HIPRI flag, the block-layer will call .poll function
of RNBD. Then IO is sent to the poll-mode queue.
Add optional nr_poll_queues argument for map_devices interface.
To support polling of RNBD, RTRS client creates connections
for both of irq-mode and direct-poll-mode.
For example, on 4-CPU system it could've create 5 connections:
con[0] => user message (softirq cq)
con[1:4] => softirq cq
After this patch, it can create 9 connections:
con[0] => user message (softirq cq)
con[1:4] => softirq cq
con[5:8] => DIRECT-POLL cq

clang static analysis reports this problem
rnbd-clt.c:1212:11: warning: Branch condition evaluates to a

This is triggered in the find_and_get_or_create_sess() call
because the variable first is not initialized and the

This is false positive.
But the if-check can be reduced by initializing first to

sess will be valid or not. The not case is caught by
find_and_get_or_create_sess()'s caller rnbd_clt_map_device()
sess = find_and_get_or_create_sess(...);
if (IS_ERR(sess))
return ERR_CAST(sess);

setting it in find_or_create_sess() is not needed.

Always use the track buffer that is already used for addresses outside
the 16MB address capability of the floppy controller. This allows to
remove a lot of code that relies on kernel virtual addresses. With
this gone there is just a single place left that looks at the bio,
which can be converted to memcpy_{from,to}_page, thus removing the need
for the extra block-layer bounce buffering for highmem pages.

null_blk: fix command timeout completion handling
block: only update parent bi_status when bio fail
Memory backed or zoned null block devices may generate actual request
timeout errors due to the submission path being blocked on memory
allocation or zone locking. Unlike fake timeouts or injected timeouts,
the request submission path will call blk_mq_complete_request() or
blk_mq_end_request() for these real timeout errors, causing a double
completion and use after free situation as the block layer timeout
handler executes blk_mq_rq_timed_out() and __blk_mq_free_request() in
blk_mq_check_expired(). This problem often triggers a NULL pointer
dereference such as:
BUG: kernel NULL pointer dereference, address: 0000000000000050
RIP: 0010:blk_mq_sched_mark_restart_hctx+0x5/0x20

This problem very often triggers when running the full btrfs xfstests
on a memory-backed zoned null block device in a VM with limited amount
of memory.
Avoid this by executing blk_mq_complete_request() in null_timeout_rq()
only for commands that are marked for a fake timeout completion using
the fake_timeout boolean in struct null_cmd. For timeout errors injected
through debugfs, the timeout handler will execute
blk_mq_complete_request()i as before. This is safe as the submission
path does not execute complete requests in this case.
In null_timeout_rq(), also make sure to set the command error field to
BLK_STS_TIMEOUT and to propagate this error through to the request
completion.

Ones successfully obtained should not be overwritten, but instead left
for xen_blkbk_unmap_prepare() to pick up and put.

commit 0d8359620d ("zram: support page writeback") introduced two
problems. It overwrites writeback_store's return value as kstrtol's
return value, which makes return value zero so user could see zero as
return value of write syscall even though it wrote data successfully.
It also breaks index value in the loop in that it doesn't increase the
index any longer. It means it can write only first starting block index
so user couldn't write all idle pages in the zram so lose memory saving
chance.
This patch fixes those issues.

writeback_store's return value is overwritten by submit_bio_wait's return
value. Thus, writeback_store will return zero since there was no IO
error. In the end, write syscall from userspace will see the zero as
return value, which could make the process stall to keep trying the write
until it will succeed.

There exists multiple path may do zram compaction concurrently.
1. auto-compaction triggered during memory reclaim
2. userspace utils write zram<id>/compaction node
So, multiple threads may call zs_shrinker_scan/zs_compact concurrently.
But pages_compacted is a per zsmalloc pool variable and modification
of the variable is not serialized(through under class->lock).
There are two issues here:
1. the pages_compacted may not equal to total number of pages
freed(due to concurrently add).
2. zs_shrinker_scan may not return the correct number of pages
freed(issued by current shrinker).
The fix is simple:
1. account the number of pages freed in zs_compact locally.
2. use actomic variable pages_compacted to accumulate total number.

module parameter 'virtblk_queue_depth' was firstly introduced for
testing/benchmarking purposes described in commit fc4324b459
("virtio-blk: base queue-depth on virtqueue ringsize or module param").
And currently 'virtblk_queue_depth' is used as a saved value for the
first probed device.
Since we have different virtio-blk devices which have different
capabilities, it requires that we support per-device queue depth instead
of per-module. So defaultly use vq free elements if module parameter
'virtblk_queue_depth' is not set.

There exists a race where we can be attempting to create a new nbd
configuration while a previous configuration is going down, both
configured with DESTROY_ON_DISCONNECT. Normally devices all have a
reference of 1, as they won't be cleaned up until the module is torn
down. However with DESTROY_ON_DISCONNECT we'll make sure that there is
only 1 reference (generally) on the device for the config itself, and
then once the config is dropped, the device is torn down.
The race that exists looks like this
TASK1 TASK2
nbd_genl_connect()
idr_find()
refcount_inc_not_zero(nbd)
* count is 2 here ^^
nbd_config_put()
nbd_put(nbd) (count is 1)
setup new config
check DESTROY_ON_DISCONNECT
put_dev = true
if (put_dev) nbd_put(nbd)
* free'd here ^^
In nbd_genl_connect() we assume that the nbd ref count will be 2,
however clearly that won't be true if the nbd device had been setup as
DESTROY_ON_DISCONNECT with its prior configuration. Fix this by getting
rid of the runtime flag to check if we need to mess with the nbd device
refcount, and use the device NBD_DESTROY_ON_DISCONNECT flag to check if
we need to adjust the ref counts. This was reported by syzkaller with

There's an I/O error on fsync() in a detached loop device
if it has been previously attached.
The issue is write cache is enabled in the attach path in
loop_configure() but it isn't disabled in the detach path;
thus it remains enabled in the block device regardless of
whether it is attached or not.
Now fsync() can get an I/O request that will just be failed
later in loop_queue_rq() as device's state is not 'Lo_bound'.
So, disable write cache in the detach path.
Do so based on the queue flag, not the loop device flag for
read-only (used to enable) as the queue flag can be changed
via sysfs even on read-only loop devices (e.g., losetup -r.)
Test-case:
# DEV=/dev/loop7
# IMG=/tmp/image
# truncate --size 1M $IMG
# losetup $DEV $IMG
# losetup -d $DEV
Before:
# strace -e fsync parted -s $DEV print 2>&1 | grep fsync
fsync(3) = -1 EIO (Input/output error)

The goto label fail_queue is needed to cleanup the queue allocation
when devm_platform_ioremap_resource() or alloc_disk() fails, either of
these two functions are not dependent on the queue variable which is
allocated prior to these calls.
Allocate the queue variable after successful alloc_disk(). Return
error directly when devm_platform_ioremap_resource() or alloc_disk()
fail. Remove fail_queue label and a call to the blk_cleanup_queue().
Direct return from these two functions allows us to remove the local
variable err and allocating queue after alloc_disk() allows us to
remove the local variable queue so we use disk->queue directly.

The function uses a goto-based loop, which may lead to an earlier error
getting discarded by a later iteration. Exit this ad-hoc loop when an
error was encountered.
The out-of-memory error path additionally fails to fill a structure
field looked at by xen_blkbk_unmap_prepare() before inspecting the
handle which does get properly set (to BLKBACK_INVALID_HANDLE).
Since the earlier exiting from the ad-hoc loop requires the same field
filling (invalidation) as that on the out-of-memory path, fold both
paths. While doing so, drop the pr_alert(), as extra log messages aren't
going to help the situation (the kernel will log oom conditions already
anyway).

This issue was originally fixed in 09954bad4 ("floppy: refactor open()
flags handling").
The fix as a side-effect, however, introduce issue for open(O_ACCMODE)
that is being used for ioctl-only open. I wrote a fix for that, but
instead of it being merged, full revert of 09954bad4 was performed,
re-introducing the O_NDELAY / O_NONBLOCK issue, and it strikes again.
This is a forward-port of the original fix to current codebase; the
original submission had the changelog below:
====
Commit 09954bad4 ("floppy: refactor open() flags handling"), as a
side-effect, causes open(/dev/fdX, O_ACCMODE) to fail. It turns out that
this is being used setfdprm userspace for ioctl-only open().
Reintroduce back the original behavior wrt !(FMODE_READ|FMODE_WRITE)
modes, while still keeping the original O_NDELAY bug fixed.

- blk-cgroup iteration teardown resched fix (Baolin)
- NVMe pull request from Christoph:
- add another Write Zeroes quirk (Chaitanya Kulkarni)
- handle a no path available corner case (Daniel Wagner)
- use the proper RCU aware list_add helper (Chao Leng)
- bcache regression fix (Coly)
- bdev->bd_size_lock IRQ fix. This will be fixed in drivers for 5.12,
but for now, we'll make it IRQ safe (Damien)
- null_blk zoned init fix (Damien)
- add_partition() error handling fix (Dinghao)
- s390 dasd kobject fix (Jan)
- nbd fix for freezing queue while adding connections (Josef)
- tag queueing regression fix (Ming)
- revert of a patch that inadvertently meant that we regressed write
performance on raid (Maxim)"

nvme-multipath: Early exit if no path is available
nvme-pci: add the DISABLE_WRITE_ZEROES quirk for a SPCC device
bcache: only check feature sets when sb->version >= BCACHE_SB_VERSION_CDEV_WITH_FEATURES
block: fix bd_size_lock use
blk-cgroup: Use cond_resched() when destroy blkgs
Revert "block: simplify set_init_blocksize" to regain lost performance
nbd: freeze the queue while we're adding connections
s390/dasd: Fix inconsistent kobject removal
block: Fix an error handling in add_partition
blk-mq: test QUEUE_FLAG_HCTX_ACTIVE for sbitmap_shared in hctx_may_queue
To avoid potential compilation problems, replaced the badly written
MB_TO_SECTS() macro (missing parenthesis around the argument use) with
the inline function mb_to_sects(). And while at it, simplify the
calculation of the total number of zones of the device using the
round_up() macro.

Pull xen fixes from Juergen Gross:
- A fix for a regression introduced in 5.11 resulting in Xen dom0
having problems to correctly initialize Xenstore.
- A fix for avoiding WARN splats when booting as Xen dom0 with
CONFIG_AMD_MEM_ENCRYPT enabled due to a missing trap handler for the
#VC exception (even if the handler should never be called).
- A fix for the Xen bklfront driver adapting to the correct but
unexpected behavior of new qemu.

Replace pci_read_config_word() with pcie_capability_read_word().

Currently, loop device has only one global lock: loop_ctl_mutex.
This becomes hot in scenarios where many loop devices are used.
Scale it by introducing per-device lock: lo_mutex that protects
modifications of all fields in struct loop_device.
Keep loop_ctl_mutex to protect global data: loop_index_idr, loop_lookup,
loop_add.
The new lock ordering requirement is that loop_ctl_mutex must be taken
before lo_mutex.

This is inline with the specification described in blkif.h:
* discard-granularity: should be set to the physical block size if
node is not present.
* discard-alignment, discard-secure: should be set to 0 if node not
present.
This was detected as QEMU would only create the discard-granularity
node but not discard-alignment, and thus the setup done in
blkfront_setup_discard would fail.
Fix blkfront_setup_discard to not fail on missing nodes, and also fix
blkif_set_queue_limits to set the discard granularity to the physical
block size if none is specified in xenbus.

When setting up a device, we can krealloc the config->socks array to add
new sockets to the configuration. However if we happen to get a IO
request in at this point even though we aren't setup we could hit a UAF,
as we deref config->socks without any locking, assuming that the
configuration was setup already and that ->socks is safe to access it as
we have a reference on the configuration.
But there's nothing really preventing IO from occurring at this point of
the device setup, we don't want to incur the overhead of a lock to
access ->socks when it will never change while the device is running.
To fix this UAF scenario simply freeze the queue if we are adding
sockets. This will protect us from this particular case without adding
any additional overhead for the normal running case.
