6f6d604b4e nvmet: allow bdev in buffered_io mode
Allow block device to be configured in the buffered I/O mode by using
the file backend. In this way now we can use cache for the block
device namespace which shows significant performance improvement.

We update the block device ns enable function and return early when
buffered_io flag is set.

2caecd62ea nvmet: use i_size_read() to set size for file-ns
Instead of calling vfs_getattr() use i_size_read() to read the size of
file so we can read the size of not only file type but also block type
with one call. This is needed to implement buffered_io support for the
NVMeOF block device backend.

We also change return type of function nvmet_file_ns_revalidate() from
int to void, since this function does not return any meaning value.

49add4966d block: pass a block_device and opf to bio_init
Pass the block_device that we plan to use this bio for and the
operation to bio_init to optimize the assignment.  A NULL block_device
can be passed, both for the passthrough case on a raw request_queue and
to temporarily avoid refactoring some nasty code.

07888c665b block: pass a block_device and opf to bio_alloc
Pass the block_device and operation that we plan to use this bio for to
bio_alloc to optimize the assignment.  NULL/0 can be passed, both for the
passthrough case on a raw request_queue and to temporarily avoid
refactoring some nasty code.

Also move the gfp_mask argument after the nr_vecs argument for a much
more logical calling convention matching what most of the kernel does.

30e32f300b nvmet-tcp: fix possible list corruption for unexpected command failure
nvmet_tcp_handle_req_failure needs to understand weather to prepare
for incoming data or the next pdu. However if we misidentify this, we
will wait for 0-length data, and queue the response although nvmet_req_init
already did that.

The particular command was namespace management command with no data,
which was incorrectly categorized as a command with incapsule data.

Also, add a code comment of what we are trying to do here.

af21250bb5 nvmet-tcp: fix memory leak when performing a controller reset
If a reset controller is executed while the initiator
is performing some I/O the driver may leak the memory allocated
for the commands' iovec.

Make sure that nvmet_tcp_uninit_data_in_cmds() releases
all the memory.

a208fc5672 nvmet-tcp: fix a race condition between release_queue and io_work
If the initiator executes a reset controller operation while
performing I/O, the target kernel will crash because of a race condition
between release_queue and io_work;
nvmet_tcp_uninit_data_in_cmds() may be executed while io_work
is running, calling flush_work() was not sufficient to
prevent this because io_work could requeue itself.

Fix this bug by using cancel_work_sync() to prevent io_work
from requeuing itself and set rcv_state to NVMET_TCP_RECV_ERR to
make sure we don't receive any more data from the socket.

page_frag_free() won't completely release the memory
allocated for the commands, the cache page must be explicitly

117d5b6d00 nvmet: use struct_size over open coded arithmetic
As noted in the "Deprecated Interfaces, Language Features, Attributes,
and Conventions" documentation [1], size calculations (especially
multiplication) should not be performed in memory allocator (or similar)
function arguments due to the risk of them overflowing. This could lead
to values wrapping around and a smaller allocation being made than the
caller was expecting. Using those allocations could lead to linear
overflows of heap memory and other misbehaviors.

In this case this is not actually dynamic size: all the operands
involved in the calculation are constant values. However it is better to
refactor this anyway, just to keep the open-coded math idiom out of
code.

So, use the struct_size() helper to do the arithmetic instead of the
argument "size + count * size" in the kmalloc() function.

This code was detected with the help of Coccinelle and audited and fixed
manually.

c7d792f9b8 nvmet-rdma: implement get_max_queue_size controller op
Limit the maximal queue size for RDMA controllers. Today, the target
reports a limit of 1024 and this limit isn't valid for some of the RDMA
based controllers. For now, limit RDMA transport to 128 entries (the
max queue depth configured for Linux NVMe/RDMA host).

Future general solution should use RDMA/core API to calculate this size
according to device capabilities and number of WRs needed per NVMe IO
request.

When a port is removed through configfs, any connected controllers
are starting teardown flow asynchronously and can still send commands.
This causes a use-after-free bug for any command that dereferences
req->port (like in nvmet_parse_io_cmd).

To fix this, wait for all the teardown scheduled works to complete
(like release_work at rdma/tcp drivers). This ensures there are no
active controllers when the port is eventually removed.

The nvme-loop's admin queue may be freed and reallocated, and we have to
reset the flag of NVME_CTRL_ADMIN_Q_STOPPED so that the flag can match
with the quiesce state of the admin queue.

nvme-loop is the only driver to reallocate request queue, and not see
such usage in other nvme drivers.

e7006de6c2 nvme: code command_id with a genctr for use-after-free validation
We cannot detect a (perhaps buggy) controller that is sending us
a completion for a request that was already completed (for example
sending a completion twice), this phenomenon was seen in the wild
a few times.

So to protect against this, we use the upper 4 msbits of the nvme sqe
command_id to use as a 4-bit generation counter and verify it matches
the existing request generation that is incrementing on every execution.

The 16-bit command_id structure now is constructed by:

0755d3be2d nvme-tcp: can't set sk_user_data without write_lock
The sk_user_data pointer is supposed to be modified only while
holding the write_lock "sk_callback_lock", otherwise
we could race with other threads and crash the kernel.

we can't take the write_lock in nvmet_tcp_state_change()
because it would cause a deadlock, but the release_work queue
will set the pointer to NULL later so we can simply remove
the assignment.

ae5e6886b4 nvme: use return value from blk_execute_rq()
We don't have an nvme status to report if the driver's .queue_rq()
returns an error without dispatching the requested nvme command. Check
the return value from blk_execute_rq() for all passthrough commands so
the caller may know their command was not successful.

If the command is from the target passthrough interface and fails to
dispatch, synthesize the response back to the host as a internal target
error.

3c3ee16532 nvmet: use NVMET_MAX_NAMESPACES to set nn value
For Spec regarding MNAN value:-

If the controller supports Asymmetric Namespace Access Reporting, then
this field shall be set to a non-zero value that is less than or equal
to the NN value.

Instead of using subsys->max_nsid that gets calculated dynamically,
use NVMET_MAX_NAMESPACES value to report NN. This way we will maintain
the MNAN value spec compliant with NN.

Without this patch, code results in the following error :-

aaf2e048af nvmet: add ZBD over ZNS backend support
NVMe TP 4053 – Zoned Namespaces (ZNS) allows host software to
communicate with a non-volatile memory subsystem using zones for NVMe
protocol-based controllers. NVMeOF already support the ZNS NVMe
Protocol compliant devices on the target in the passthru mode. There
are generic zoned block devices like  Shingled Magnetic Recording (SMR)
HDDs that are not based on the NVMe protocol.

This patch adds ZNS backend support for non-ZNS zoned block devices as
NVMeOF targets.

This support includes implementing the new command set NVME_CSI_ZNS,
adding different command handlers for ZNS command set such as NVMe
Identify Controller, NVMe Identify Namespace, NVMe Zone Append,
NVMe Zone Management Send and NVMe Zone Management Receive.

With the new command set identifier, we also update the target command
effects logs to reflect the ZNS compliant commands.

ab5d0b38c0 nvmet: add Command Set Identifier support
NVMe TP 4056 allows controllers to support different command sets.
NVMeoF target currently only supports namespaces that contain
traditional logical blocks that may be randomly read and written. In
some applications there is a value in exposing namespaces that contain
logical blocks that have special access rules (e.g. sequentially write
required namespace such as Zoned Namespace (ZNS)).

In order to support the Zoned Block Devices (ZBD) backend, controllers
need to have support for ZNS Command Set Identifier (CSI).

In this preparation patch, we adjust the code such that it can now
support the default command set identifier. We update the namespace data
structure to store the CSI value which defaults to NVME_CSI_NVM
that represents traditional logical blocks namespace type.

The CSI support is required to implement the ZBD backend for NVMeOF
with host side NVMe ZNS interface, since ZNS commands belong to
the different command set than the default one.

9a01b58c22 nvmet: add nvmet_req_bio put helper for backends
In current code there exists two backends which are using inline bio
optimization, that adds a duplicate code for freeing the bio.

For Zoned Block Device backend we also use the same optimzation and it
will lead to having duplicate code in the three backends: generic
bdev, passsthru, and generic zns.

Add a helper function to avoid duplicate code and update the respective
backends.

0d148efdf0 nvmet: allow mn change if subsys not discovered
Currently, once the subsystem's model_number is set for the first time
there is no way to change it. However, as long as no connection was
established to nvmf target, there is no reason for such restriction and
we should allow to change the subsystem's model_number as many times as
needed.

In addition, in order to simplfy the changes and make the model number
flow more similar to the rest of the attributes in the Identify
Controller data structure, we set a default value for the model number
at the initiation of the subsystem.

According to the NVM specification, the serial_number should be 20 bytes
(bytes 23:04 of the Identify Controller data structure), and should
contain only ASCII characters.

In accordance, the serial_number size is changed to 20 bytes and before
any attempt to store a new value in serial_number we check that the
input is valid - i.e. contains only ASCII characters, is not empty and
does not exceed 20 bytes.

2a4a910aa4 nvmet-fc: do not check for invalid target port in nvmet_fc_handle_fcp_rqst()
When parsing a request in nvmet_fc_handle_fcp_rqst() we should not
check for invalid target ports; if we do the command is aborted
from the fcp layer, causing the host to assume a transport error.
Rather we should still forward this request to the nvmet layer, which
will then correctly fail the command with an appropriate error status.

fec356a61a nvmet: fix memory leak in nvmet_alloc_ctrl()
When creating ctrl in nvmet_alloc_ctrl(), if the cntlid_min is larger
than cntlid_max of the subsystem, and jumps to the
"out_free_changed_ns_list" label, but the ctrl->sqs lack of be freed.
Fix this by jumping to the "out_free_sqs" label.

85428beac8 nvmet: seset ns->file when open fails
Reset the ns->file value to NULL also in the error case in
nvmet_file_ns_enable().

The ns->file variable points either to file object or contains the
error code after the filp_open() call. This can lead to following
problem:

When the user first setups an invalid file backend and tries to enable
the ns, it will fail. Then the user switches over to a bdev backend
and enables successfully the ns. The first received I/O will crash the
system because the IO backend is chosen based on the ns->file value:

Nothing can stop a host from submitting invalid commands. The target
just needs to respond with an appropriate status, but that's not a
target error. Demote invalid command messages to the debug level so

a97157440e nvme: move the fabrics queue ready check routines to core
queue_rq() in pci only checks if the dispatched queue (nvmeq) is ready,
e.g. not being suspended. Since nvme_alloc_admin_tags() in reset flow
restarts the admin queue, users are able to submit admin commands to a
controller before reset_work() completes. Commands submitted under this
condition may interfere with commands that performs identify, IO queue
setup in reset_work(), and may result in a hang described in the
following patch.

As seen in the fabrics, user commands are prevented from being executed
under inproper controller states. We may reuse this logic to maintain a
clear admin queue during reset_work().

Issue following command:
nvme set-feature -f 0xf -v 0 /dev/nvme1n1 # disable keep-alive timer
nvme admin-passthru -o 0x18 /dev/nvme1n1  # send keep-alive command
will make keep-alive timer fired and thus delete the controller like
below:

Avoid this by not queuing delayed keep-alive if it is disabled when

d8e7b462f5 nvmet-tcp: enable optional queue idle period tracking
Add 'idle_poll_period_usecs' option used by io_work() to support
network devices enabled with advanced interrupt moderation
supporting a relaxed interrupt model. It was discovered that
such a NIC used on the target was unable to support initiator
connection establishment, caused by the existing io_work()
flow that immediately exits after a loop with no activity and
does not re-queue itself.

With this new option a queue is assigned a period of time
that no activity must occur in order to become 'idle'.  Until
the queue is idle the work item is requeued.

The new module option is defined as changeable making it
flexible for testing purposes.

The pre-existing legacy behavior is preserved when no module option
for idle_poll_period_usecs is specified.

f4b9e6c90c nvme: use driver pdu command for passthrough
All nvme transport drivers preallocate an nvme command for each request.
Assume to use that command for nvme_setup_cmd() instead of requiring
drivers pass a pointer to it. All nvme drivers must initialize the
generic nvme_request 'cmd' to point to the transport's preallocated
nvme_command.

The generic nvme_request cmd pointer had previously been used only as a
temporary copy for passthrough commands. Since it now points to the
command that gets dispatched, passthrough commands must directly set it
up prior to executing the request.

According to the NVM specifications, the model number size should be
40 bytes (bytes 63:24 of the Identify Controller data structure).
Therefore, any attempt to store a value into model_number which

de5878048e nvmet: remove unnecessary ctrl parameter
The function nvmet_ctrl_find_get() accepts out pointer to nvmet_ctrl
structure. This function returns the same error value from two places
that is :- NVME_SC_CONNECT_INVALID_PARAM | NVME_SC_DNR.

Move this to the caller so we can change the return type to nvmet_ctrl.

Now that we can changed the return type, instead of taking out pointer
to the nvmet_ctrl structure remove that function parameter and return
the valid nvmet_ctrl pointer on success and NULL on failure.

Also, add and rename the goto labels for more readability with comments.

abec6561fc nvme-rdma: Fix a use after free in nvmet_rdma_write_data_done
In nvmet_rdma_write_data_done, rsp is recoverd by wc->wr_cqe and freed by
nvmet_rdma_release_rsp(). But after that, pr_info() used the freed
chunk's member object and could leak the freed chunk address with
wc->wr_cqe by computing the offset.

d9f273b758 nvmet: model_number must be immutable once set
In case we have already established connection to nvmf target, it
shouldn't be allowed to change the model_number. E.g. if someone will
identify ctrl and get model_number of "my_model" later on will change
the model_numbel via configfs to "my_new_model" this will break the NVMe
specification for "Get Log Page – Persistent Event Log" that refers to
Model Number as: "This field contains the same value as reported in the
Model Number field of the Identify Controller data structure, bytes
63:24."

Although it doesn't mentioned explicitly that this field can't be
changed, we can assume it.

So allow setting this field only once: using configfs or in the first
identify ctrl operation.

3a1f7c79ae nvmet: make nvmet_find_namespace() req based
The six callers of nvmet_find_namespace() duplicate the error log page
update and status setting code for each call on failure.

All callers are nvmet requests based functions, so we can pass req
to the nvmet_find_namesapce() & derive ctrl from req, that'll allow us
to update the error log page in nvmet_find_namespace(). Now that we
pass the request we can also get rid of the local variable in
nvmet_find_namespace() and use the req->ns and return the error code.

Replace the ctrl parameter with nvmet_req for nvmet_find_namespace(),
centralize the error log page update for non allocated namesapces, and
return uniform error for non-allocated namespace.

The nvmet_find_namespace() takes nsid parameter which is from NVMe
commands structures such as get_log_page, identify, rw and common. All
these commands have same offset for the nsid field.

Derive nsid from req->cmd->common.nsid) & remove the extra parameter
from the nvmet_find_namespace().

Lastly now we associate the ns to the req parameter that we pass to the
nvmet_find_namespace(), rename nvmet_find_namespace() to
nvmet_req_find_ns().

aa0aff604a nvmet: return uniform error for invalid ns
For nvmet_find_namespace() error case we have inconsistent error code
mapping in the function nvmet_get_smart_log_nsid() and
nvmet_set_feat_write_protect().

There is no point in retrying for the invalid namesapce from the host
side. Set the error code to the NVME_SC_INVALID_NS | NVME_SC_DNR which
matches what we have in nvmet_execute_identify_desclist().

0fbcfb089a nvmet-tcp: fix potential race of tcp socket closing accept_work
When we accept a TCP connection and allocate an nvmet-tcp queue we should
make sure not to fully establish it or reference it as the connection may
be already closing, which triggers queue release work, which does not
fence against queue establishment.

In order to address such a race, we make sure to check the sk_state and
contain the queue reference to be done underneath the sk_callback_lock
such that the queue release work correctly fences against it.

cb8563f5c7 nvmet-tcp: fix out-of-bounds access when receiving multiple h2cdata PDUs
When the host sends multiple h2cdata PDUs, we keep track on
the receive progress and calculate the scatterlist index and
offsets.

The issue is that sg_offset should only be kept for the first
iov entry we map in the iovec as this is the difference between
our cursor and the sg entry offset itself.

In addition, the sg index was calculated wrong because we should
not round up when dividing the command byte offset with PAG_SIZE.

9ceb786353 nvmet-rdma: Fix list_del corruption on queue establishment failure
When a queue is in NVMET_RDMA_Q_CONNECTING state, it may has some
requests at rsp_wait_list. In case a disconnect occurs at this
state, no one will empty this list and will return the requests to
free_rsps list. Normally nvmet_rdma_queue_established() free those
requests after moving the queue to NVMET_RDMA_Q_LIVE state, but in
this case __nvmet_rdma_queue_disconnect() is called before. The
crash happens at nvmet_rdma_free_rsps() when calling
list_del(&rsp->free_list), because the request exists only at
the wait list. To fix the issue, simply clear rsp_wait_list when
destroying the queue.
