6a492b0f23 SCSI misc on 20160727
This update includes the usual round of driver updates (fcoe, lpfc,
 ufs, qla2xxx, hisi_sas).  The most important other change is removing
 the flag to allow non-blk_mq on a per host basis (it's unused); there
 is still a global module parameter for all of SCSI just in case.  The
 rest are an assortment of minor fixes and typo updates.

necessary in the caller but still lets it do the constant-folding.

When running in VN2VN mode there is no central instance which would send
out any FIP VLAN discovery notifications. So this patch adds a new sysfs
attribute 'fip_vlan_responder' which will activate a FIP VLAN discovery

4613c5f1df scsi/osd: open code blk_make_request
I wish the OSD code could simply use blk_rq_map_* helpers like
everyone else, but the complex nature of deciding if we have
DATA IN and/or DATA OUT buffers might make this impossible
(at least for a mere human like me).

But using blk_rq_append_bio at least allows sharing the setup code
between request with or without dat a buffers, and given that this
is the last user of blk_make_request it allows getting rid of that
somewhat awkward interface.

01f90dd9e0 sd: don't use the ALL_TG_PT bit for reservations
These only work if the we use the same initiator ID for all path,
which might not be true if we use different protocols, or even just
different HBAs.

Instead dm-mpath will grow support to register all path manually
later in this series.

64ce60cab2 hpsa: correct skipping masked peripherals
The SA controller spins down RAID drive spares.

A REGNEWD event causes an inquiry to be sent to all physical
drives. This causes the SA controller to spin up the spare.

The controller suspends all I/O to a logical volume until
the spare is spun up. The spin-up can take over 50 seconds.

This can result in one or both of the following:
 - SML sends down aborts and resets to the logical volume
   and can cause the logical volume to be off-lined.
 - a negative impact on the logical volume's I/O performance
   each time a REGNEWD is triggered.

During hot swap of PCI device, there can be PCI error on device,
during normal driver unload. The race between normal driver unload and
driver unload due to PCI error, can lead to system crash.Fix is to check
