33fe43cfd9 powerpc/8xx: Manage _PAGE_ACCESSED through APG bits in L1 entry
When _PAGE_ACCESSED is not set, a minor fault is expected.
To do this, TLB miss exception ANDs _PAGE_PRESENT and _PAGE_ACCESSED
into the L2 entry valid bit.

To simplify the processing and reduce the number of instructions in
TLB miss exceptions, manage it as an APG bit and get it next to
_PAGE_GUARDED bit to allow a copy in one go. Then declare the
corresponding groups as handling all accesses as user accesses.
As the PP bits always define user as No Access, it will generate
a fault.

29daf869cb powerpc/8xx: Always fault when _PAGE_ACCESSED is not set
The kernel expects pte_young() to work regardless of CONFIG_SWAP.

Make sure a minor fault is taken to set _PAGE_ACCESSED when it
is not already set, regardless of the selection of CONFIG_SWAP.

This adds at least 3 instructions to the TLB miss exception
handlers fast path. Following patch will reduce this overhead.

Also update the rotation instruction to the correct number of bits
to reflect all changes done to _PAGE_ACCESSED over time.

cf209951fa powerpc/8xx: Map linear memory with huge pages
Map linear memory space with 512k and 8M pages whenever
possible.

Three mappings are performed:
- One for kernel text
- One for RO data
- One for the rest

Separating the mappings is done to be able to update the
protection later when using STRICT_KERNEL_RWX.

The ITLB miss handler now need to also handle huge TLBs
unless kernel text in pinned.

c8bef10a9f powerpc/8xx: Refactor kernel address boundary comparison
Now that linear and IMMR dedicated TLB handling is gone, kernel
boundary address comparison is similar in ITLB miss handler and
in DTLB miss handler.

Create a macro named compare_to_kernel_boundary.

When TASK_SIZE is strictly below 0x80000000 and PAGE_OFFSET is
above 0x80000000, it is enough to compare to 0x8000000, and this
can be done with a single instruction.

Using not. instruction, we get to use 'blt' conditional branch as
when doing a regular comparison:

0x00000000 <= addr <= 0x7fffffff ==>
0xffffffff >= NOT(addr) >= 0x80000000
The above test corresponds to a 'blt'

Otherwise, do a regular comparison using two instructions.

400dc0f861 powerpc/8xx: Drop special handling of Linear and IMMR mappings in I/D TLB handlers
Up to now, linear and IMMR mappings are managed via huge TLB entries
through specific code directly in TLB miss handlers. This implies
some patching of the TLB miss handlers at startup, and a lot of
dedicated code.

Remove all this specific dedicated code.

For now we are back to normal handling via standard 4k pages. In the
next patches, linear memory mapping and IMMR mapping will be managed
through huge pages.

684c1664e0 powerpc/8xx: Always pin TLBs at startup.
At startup, map 32 Mbytes of memory through 4 pages of 8M,
and PIN them inconditionnaly. They need to be pinned because
KASAN is using page tables early and the TLBs might be
dynamically replaced otherwise.

Remove RSV4I flag after installing mappings unless
CONFIG_PIN_TLB_XXXX is selected.

136a9a0f74 powerpc/8xx: Don't set IMMR map anymore at boot
Only early debug requires IMMR to be mapped early.

No need to set it up and pin it in assembly. Map it
through page tables at udbg init when necessary.

If CONFIG_PIN_TLB_IMMR is selected, pin it once we
don't need the 32 Mb pinned RAM anymore.

f76c8f6d25 powerpc/8xx: Add function to set pinned TLBs
Pinned TLBs cannot be modified when the MMU is enabled.

Create a function to rewrite the pinned TLB entries with MMU off.

To set pinned TLB, we have to turn off MMU, disable pinning,
do a TLB flush (Either with tlbie and tlbia) then reprogam
the TLB entries, enable pinning and turn on MMU.

If using tlbie, it cleared entries in both instruction and data
TLB regardless whether pinning is disabled or not.
If using tlbia, it clears all entries of the TLB which has
disabled pinning.

To make it easy, just clear all entries in both TLBs, and
reprogram them.

The function takes two arguments, the top of the memory to
consider and whether data is RO under _sinittext.
When DEBUG_PAGEALLOC is set, the top is the end of kernel rodata.
Otherwise, that's the top of physical RAM.

Everything below _sinittext is set RX, over _sinittext that's RW.

b250c8c08c powerpc/8xx: Manage 512k huge pages as standard pages.
At the time being, 512k huge pages are handled through hugepd page
tables. The PMD entry is flagged as a hugepd pointer and it
means that only 512k hugepages can be managed in that 4M block.
However, the hugepd table has the same size as a normal page
table, and 512k entries can therefore be nested with normal pages.

On the 8xx, TLB loading is performed by software and allthough the
page tables are organised to match the L1 and L2 level defined by
the HW, all TLB entries have both L1 and L2 independent entries.
It means that even if two TLB entries are associated with the same
PMD entry, they can be loaded with different values in L1 part.

The L1 entry contains the page size (PS field):
- 00 for 4k and 16 pages
- 01 for 512k pages
- 11 for 8M pages

By adding a flag for hugepages in the PTE (_PAGE_HUGE) and copying it
into the lower bit of PS, we can then manage 512k pages with normal
page tables:
- PMD entry has PS=11 for 8M pages
- PMD entry has PS=00 for other pages.

As a PMD entry covers 4M areas, a PMD will either point to a hugepd
table having a single entry to an 8M page, or the PMD will point to
a standard page table which will have either entries to 4k or 16k or
512k pages. For 512k pages, as the L1 entry will not know it is a
512k page before the PTE is read, there will be 128 entries in the
PTE as if it was 4k pages. But when loading the TLB, it will be
flagged as a 512k page.

Note that we can't use pmd_ptr() in asm/nohash/32/pgtable.h because
it is not defined yet.

In ITLB miss, we keep the possibility to opt it out as when kernel
text is pinned and no user hugepages are used, we can save several
instruction by not using r11.

In DTLB miss, that's just one instruction so it's not worth bothering
with it.

a891c43b97 powerpc/8xx: Prepare handlers for _PAGE_HUGE for 512k pages.
Prepare ITLB handler to handle _PAGE_HUGE when CONFIG_HUGETLBFS
is enabled. This means that the L1 entry has to be kept in r11
until L2 entry is read, in order to insert _PAGE_HUGE into it.

Also move pgd_offset helpers before pte_update() as they
will be needed there in next patch.

d3efcd38c0 powerpc/8xx: Drop CONFIG_8xx_COPYBACK option
CONFIG_8xx_COPYBACK was there to help disabling copyback cache mode
for debuging hardware. But nobody will design new boards with 8xx now.

All 8xx platforms select it, so make it the default and remove
the option.

Also remove the Mx_RESETVAL values which are pretty useless and hide
the real value while reading code.

a4031afb9d powerpc/8xx: Fix clearing of bits 20-23 in ITLB miss
In ITLB miss handled the line supposed to clear bits 20-23 on the L2
ITLB entry is buggy and does indeed nothing, leading to undefined
value which could allow execution when it shouldn't.

Properly do the clearing with the relevant instruction.

head_8xx.S has entries for all exceptions from 0x100 to 0x1f00.
Several of them do not exist and are never generated by the 8xx
in accordance with the documentation.

Remove those entry points to make some room for future growing
exception code.

Today, the STACK_END_MAGIC is set on init_stack in start_kernel().

To avoid a false 'Thread overran stack, or stack corrupted' message
on early Oopses, setup STACK_END_MAGIC as soon as possible.

b86fb88855 powerpc/32: implement fast entry for syscalls on non BOOKE
This patch implements a fast entry for syscalls.

Syscalls don't have to preserve non volatile registers except LR.

This patch then implement a fast entry for syscalls, where
volatile registers get clobbered.

As this entry is dedicated to syscall it always sets MSR_EE
and warns in case MSR_EE was previously off

It also assumes that the call is always from user, system calls are
unexpected from kernel.

The overall series improves null_syscall selftest by 12,5% on an 83xx
and by 17% on a 8xx.

8a23fdec3d powerpc/32: Refactor EXCEPTION entry macros for head_8xx.S and head_32.S
EXCEPTION_PROLOG is similar in head_8xx.S and head_32.S

This patch creates head_32.h and moves EXCEPTION_PROLOG macro
into it. It also converts it from a GCC macro to a GAS macro
in order to ease refactorisation with 40x later, since
GAS macros allows the use of #ifdef/#else/#endif inside it.
And it also has the advantage of not requiring the uggly "; \"
at the end of each line.

This patch also moves EXCEPTION() and EXC_XFER_XXXX() macros which
are also similar while adding START_EXCEPTION() out of EXCEPTION().

2edb16efc8 powerpc/32: Add KASAN support
This patch adds KASAN support for PPC32. The following patch
will add an early activation of hash table for book3s. Until
then, a warning will be raised if trying to use KASAN on an
hash 6xx.

To support KASAN, this patch initialises that MMU mapings for
accessing to the KASAN shadow area defined in a previous patch.

An early mapping is set as soon as the kernel code has been
relocated at its definitive place.

Then the definitive mapping is set once paging is initialised.

For modules, the shadow area is allocated at module_alloc().

8c1fc5abdc powerpc: Rename THREAD_INFO to TASK_STACK
This patch renames THREAD_INFO to TASK_STACK, because it is in fact
the offset of the pointer to the stack in task_struct so this pointer
will not be impacted by the move of THREAD_INFO.

Also make it available on 64-bit, as we'll need it there when we
activate THREAD_INFO_IN_TASK.

8f54a6f740 powerpc/kconfig: make _etext and data areas alignment configurable on 8xx
On 8xx, large pages (512kb or 8M) are used to map kernel linear
memory. Aligning to 8M reduces TLB misses as only 8M pages are used
in that case. We make 8M the default for data.

This patchs allows the user to do it via Kconfig.

d5f17ee964 powerpc/8xx: don't disable large TLBs with CONFIG_STRICT_KERNEL_RWX
This patch implements handling of STRICT_KERNEL_RWX with
large TLBs directly in the TLB miss handlers.

To do so, etext and sinittext are aligned on 512kB boundaries
and the miss handlers use 512kB pages instead of 8Mb pages for
addresses close to the boundaries.

It sets RO PP flags for addresses under sinittext.

e4470bd6a4 powerpc/8xx: Map 32Mb of RAM at init.
At the time being, initial MMU setup allows 24 Mbytes
of DATA and 8 Mbytes of code.

Some debug setup like CONFIG_KASAN generate huge
kernels with text size over the 8M limit and data over the
24 Mbytes limit.

Here is an 8xx kernel compiled with CONFIG_KASAN_INLINE for
one of my boards:

Commit 8c8c10b90d ("powerpc/8xx: fix handling of early NULL pointer
dereference") moved the loading of r6 earlier in the code. As some
functions are called inbetween, r6 needs to be loaded again with the
address of swapper_pg_dir in order to set PTE pointers for
the Abatron BDI.

74fabcadfd powerpc/8xx: don't use r12/SPRN_SPRG_SCRATCH2 in TLB Miss handlers
This patch reworks the TLB Miss handler in order to not use r12
register, hence avoiding having to save it into SPRN_SPRG_SCRATCH2.

In the DAR Fixup code we can now use SPRN_M_TW, freeing
SPRN_SPRG_SCRATCH2.

Then SPRN_SPRG_SCRATCH2 may be used for something else in the future.

6a8f911b50 powerpc/8xx: Use hardware assistance in TLB handlers
Today, on the 8xx the TLB handlers do SW tablewalk by doing all
the calculation in ASM, in order to match with the Linux page
table structure.

The 8xx offers hardware assistance which allows significant size
reduction of the TLB handlers, hence also reduces the time spent
in the handlers.

However, using this HW assistance implies some constraints on the
page table structure:
- Regardless of the main page size used (4k or 16k), the
level 1 table (PGD) contains 1024 entries and each PGD entry covers
a 4Mbytes area which is managed by a level 2 table (PTE) containing
also 1024 entries each describing a 4k page.
- 16k pages require 4 identifical entries in the L2 table
- 512k pages PTE have to be spread every 128 bytes in the L2 table
- 8M pages PTE are at the address pointed by the L1 entry and each
8M page require 2 identical entries in the PGD.

This patch modifies the TLB handlers to use HW assistance for 4K PAGES.

Before that patch, the mean time spent in TLB miss handlers is:
- ITLB miss: 80 ticks
- DTLB miss: 62 ticks
After that patch, the mean time spent in TLB miss handlers is:
- ITLB miss: 72 ticks
- DTLB miss: 54 ticks
So the improvement is 10% for ITLB and 13% for DTLB misses

5af543be14 powerpc/8xx: Temporarily disable 16k pages and hugepages
In preparation of making use of hardware assistance in TLB handlers,
this patch temporarily disables 16K pages and hugepages. The reason
is that when using HW assistance in 4K pages mode, the linux model
fit with the HW model for 4K pages and 8M pages.

However for 16K pages and 512K mode some additional work is needed
to get linux model fit with HW model.
For the 8M pages, they will naturaly come back when we switch to
HW assistance, without any additional handling.
In order to keep the following patch smaller, the removal of the
current special handling for 8M pages gets removed here as well.

Therefore the 4K pages mode will be implemented first and without
support for 512k hugepages. Then the 512k hugepages will be brought
back. And the 16K pages will be implemented in the following step.

8cfe4f5242 powerpc/8xx: Move SW perf counters in first 32kb of memory
In order to simplify time critical exceptions handling 8xx
specific SW perf counters, this patch moves the counters into
the beginning of memory. This is possible because .text is readable
and the counters are never modified outside of the handlers.

By doing this, we avoid having to set a second register with
the upper part of the address of the counters.
