A protected VM accessing ID_AA64ISAR2_EL1 gets punished with an UNDEF,
while it really should only get a zero back if the register is not
handled by the hypervisor emulation (as mandated by the architecture).
Introduce all the missing ID registers (including the unallocated ones),
and have them to return 0.

Moving kvm_pmu_events into the vcpu (and refering to it) broke the
somewhat unusual case where the kernel has no support for a PMU
at all.
In order to solve this, move things around a bit so that we can
easily avoid refering to the pmu structure outside of PMU-aware
code. As a bonus, pmu.c isn't compiled in when HW_PERF_EVENTS
isn't selected.

Instead of the host accessing hyp data directly, pass the pmu
events of the current cpu to hyp via the vcpu.
This adds 64 bits (in two fields) to the vcpu that need to be
synced before every vcpu run in nvhe and protected modes.
However, it isolates the hypervisor from the host, which allows
us to use pmu in protected mode in a subsequent patch.

There are a few cases in the nVHE code where we take the absolute
address of a symbol via a literal pool entry, and subsequently translate
it to another address space (PA, kimg VA, kernel linear VA, etc).
Originally, this literal was needed because we relied on a different
translation for absolute references, but this is no longer the case, so
we can simply use relative addressing instead. This removes a couple of
RELA entries pointing into the .text segment.

ESR_EL2 was defined as a 32-bit register in the initial release of the
ARM Architecture Manual for Armv8-A, and was later extended to 64 bits,
with bits [63:32] RES0. ARMv8.7 introduced FEAT_LS64, which makes use of
bits [36:32].
KVM treats ESR_EL1 as a 64-bit register when saving and restoring the
guest context, but ESR_EL2 is handled as a 32-bit register. Start
treating ESR_EL2 as a 64-bit register to allow KVM to make use of the
most significant 32 bits in the future.
The type chosen to represent ESR_EL2 is u64, as that is consistent with the
notation KVM overwhelmingly uses today (u32), and how the rest of the
registers are declared.

When pKVM is enabled, host memory accesses are translated by an identity
mapping at stage-2, which is populated lazily in response to synchronous
exceptions from 64-bit EL1 and EL0.
Extend this handling to cover exceptions originating from 32-bit EL0 as
well. Although these are very unlikely to occur in practice, as the
kernel typically ensures that user pages are initialised before mapping
them in, drivers could still map previously untouched device pages into
userspace and expect things to work rather than panic the system.

CPACR_EL1 has several bitfields for controlling traps for floating point
features to EL1, each of which has a separate bits for EL0 and EL1. Marc
Zyngier noted that we are not consistent in our use of defines to
manipulate these, sometimes using a define covering the whole field and
sometimes using defines for the individual bits. Make this consistent by
expanding the whole field defines where they are used (currently only in
the KVM code) and deleting them so that no further uses can be
introduced.

Future CPUs may implement a clearbhb instruction that is sufficient
to mitigate SpectreBHB. CPUs that implement this instruction, but
not CSV2.3 must be affected by Spectre-BHB.
Add support to use this instruction as the BHB mitigation on CPUs
that support it. The instruction is in the hint space, so it will
be treated by a NOP as older CPUs.

Speculation attacks against some high-performance processors can
make use of branch history to influence future speculation.
When taking an exception from user-space, a sequence of branches
or a firmware call overwrites or invalidates the branch history.
The sequence of branches is added to the vectors, and should appear
before the first indirect branch. For systems using KPTI the sequence
is added to the kpti trampoline where it has a free register as the exit
from the trampoline is via a 'ret'. For systems not using KPTI, the same
register tricks are used to free up a register in the vectors.
For the firmware call, arch-workaround-3 clobbers 4 registers, so
there is no choice but to save them to the EL1 stack. This only happens
for entry from EL0, so if we take an exception due to the stack access,
it will not become re-entrant.
For KVM, the existing branch-predictor-hardening vectors are used.
When a spectre version of these vectors is in use, the firmware call
is sufficient to mitigate against Spectre-BHB. For the non-spectre
versions, the sequence of branches is added to the indirect vector.

Now that we have SYM_FUNC_ALIAS() and SYM_FUNC_ALIAS_WEAK(), use those
to simplify and more consistently define function aliases across
arch/arm64.
Aliases are now defined in terms of a canonical function name. For
position-independent functions I've made the __pi_<func> name the
canonical name, and defined other alises in terms of this.
The SYM_FUNC_{START,END}_PI(func) macros obscure the __pi_<func> name,
and make this hard to seatch for. The SYM_FUNC_START_WEAK_PI() macro
also obscures the fact that the __pi_<func> fymbol is global and the
<func> symbol is weak. For clarity, I have removed these macros and used
SYM_FUNC_{START,END}() directly with the __pi_<func> name.
For example:
SYM_FUNC_START_WEAK_PI(func)
... asm insns ...
SYM_FUNC_END_PI(func)
EXPORT_SYMBOL(func)
... becomes:
SYM_FUNC_START(__pi_func)
... asm insns ...
SYM_FUNC_END(__pi_func)
SYM_FUNC_ALIAS_WEAK(func, __pi_func)
EXPORT_SYMBOL(func)
For clarity, where there are multiple annotations such as
EXPORT_SYMBOL(), I've tried to keep annotations grouped by symbol. For
example, where a function has a name and an alias which are both
exported, this is organised as:
SYM_FUNC_START(func)
... asm insns ...
SYM_FUNC_END(func)
EXPORT_SYMBOL(func)
SYM_FUNC_ALIAS(alias, func)
EXPORT_SYMBOL(alias)
For consistency with the other string functions, I've defined strrchr as
a position-independent function, as it can safely be used as such even
though we have no users today.
As we no longer use SYM_FUNC_{START,END}_ALIAS(), our local copies are
removed. The common versions will be removed by a subsequent patch.
There should be no functional change as a result of this patch.

The Spectre-BHB workaround adds a firmware call to the vectors. This
is needed on some CPUs, but not others. To avoid the unaffected CPU in
a big/little pair from making the firmware call, create per cpu vectors.
The per-cpu vectors only apply when returning from EL0.
Systems using KPTI can use the canonical 'full-fat' vectors directly at
EL1, the trampoline exit code will switch to this_cpu_vector on exit to
EL0. Systems not using KPTI should always use this_cpu_vector.
this_cpu_vector will point at a vector in tramp_vecs or
__bp_harden_el1_vectors, depending on whether KPTI is in use.

CPUs vulnerable to Spectre-BHB either need to make an SMC-CC firmware
call from the vectors, or run a sequence of branches. This gets added
to the hyp vectors. If there is no support for arch-workaround-1 in
firmware, the indirect vector will be used.
kvm_init_vector_slots() only initialises the two indirect slots if
the platform is vulnerable to Spectre-v3a. pKVM's hyp_map_vectors()
only initialises __hyp_bp_vect_base if the platform is vulnerable to
Spectre-v3a.
As there are about to more users of the indirect vectors, ensure
their entries in hyp_spectre_vector_selector[] are always initialised,
and __hyp_bp_vect_base defaults to the regular VA mapping.
The Spectre-v3a check is moved to a helper
kvm_system_needs_idmapped_vectors(), and merged with the code
that creates the hyp mappings.

At the moment, the VMID algorithm will send an SGI to all the
CPUs to force an exit and then broadcast a full TLB flush and
I-Cache invalidation.
This patch uses the new VMID allocator. The benefits are:
- Aligns with arm64 ASID algorithm.
- CPUs are not forced to exit at roll-over. Instead,
the VMID will be marked reserved and context invalidation
is broadcasted. This will reduce the IPIs traffic.
- More flexible to add support for pinned KVM VMIDs in
the future.
With the new algo, the code is now adapted:
- The call to update_vmid() will be done with preemption
disabled as the new algo requires to store information
per-CPU.

Cortex-A510's erratum #2077057 causes SPSR_EL2 to be corrupted when
single-stepping authenticated ERET instructions. A single step is
expected, but a pointer authentication trap is taken instead. The
erratum causes SPSR_EL1 to be copied to SPSR_EL2, which could allow
EL1 to cause a return to EL2 with a guest controlled ELR_EL2.
Because the conditions require an ERET into active-not-pending state,
this is only a problem for the EL2 when EL2 is stepping EL1. In this case
the previous SPSR_EL2 value is preserved in struct kvm_vcpu, and can be
restored.

When any exception other than an IRQ occurs, the CPU updates the ESR_EL2
register with the exception syndrome. An SError may also become pending,
and will be synchronised by KVM. KVM notes the exception type, and whether
an SError was synchronised in exit_code.
When an exception other than an IRQ occurs, fixup_guest_exit() updates
vcpu->arch.fault.esr_el2 from the hardware register. When an SError was
synchronised, the vcpu esr value is used to determine if the exception
was due to an HVC. If so, ELR_EL2 is moved back one instruction. This
is so that KVM can process the SError first, and re-execute the HVC if
the guest survives the SError.
But if an IRQ synchronises an SError, the vcpu's esr value is stale.
If the previous non-IRQ exception was an HVC, KVM will corrupt ELR_EL2,
causing an unrelated guest instruction to be executed twice.
Check ARM_EXCEPTION_CODE() before messing with ELR_EL2, IRQs don't
update this register so don't need to check.

KVM/arm64 fixes for 5.17, take #1
- Correctly update the shadow register on exception injection when
running in nVHE mode
- Correctly use the mm_ops indirection when performing cache invalidation
from the page-table walker
- Restrict the vgic-v3 workaround for SEIS to the two known broken
implementations
Injecting an exception into a guest with non-VHE is risky business.
Instead of writing in the shadow register for the switch code to
restore it, we override the CPU register instead. Which gets
overriden a few instructions later by said restore code.
The result is that although the guest correctly gets the exception,
it will return to the original context in some random state,
depending on what was there the first place... Boo.
Fix the issue by writing to the shadow register. The original code
is absolutely fine on VHE, as the state is already loaded, and writing
to the shadow register in that case would actually be a bug.

Contrary to what df652bcf11 ("KVM: arm64: vgic-v3: Work around GICv3
locally generated SErrors") was asserting, there is at least one other
system out there (Cavium ThunderX2) implementing SEIS, and not in
an obviously broken way.
So instead of imposing the M1 workaround on an innocent bystander,
let's limit it to the two known broken Apple implementations.

CMOs issued from EL2 cannot directly use the kernel helpers,
as EL2 doesn't have a mapping of the guest pages. Oops.
Instead, use the mm_ops indirection to use helpers that will
perform a mapping at EL2 and allow the CMO to be effective.
