Some architectures and irqchip drivers modify the cpumask returned by
irq_data_get_affinity_mask, usually by copying in to it. This is
problematic for uniprocessor configurations, where the affinity mask
should be constant, as it is known at compile time.
Add and use a setter for the affinity mask, following the pattern of
irq_data_update_effective_affinity. This allows the getter function to
return a const cpumask pointer.

Add the missing logic to allow Lasi, WAX and Dino to set the
CPU affinity. This fixes IRQ migration to other CPUs when a
CPU is shutdown which currently holds the IRQs for one of those
chips.

kobject_init_and_add() takes reference even when it fails.
According to the doc of kobject_init_and_add():
If this function returns an error, kobject_put() must be called to
properly clean up the memory associated with the object.
Fix memory leak by calling kobject_put().

There are currently 2 ways to create a set of sysfs files for a
kobj_type, through the default_attrs field, and the default_groups
field. Move the parisc pdc_stable sysfs code to use default_groups
field which has been the preferred way since aa30f47cf6 ("kobject: Add
support for default attribute groups to kobj_type") so that we can soon
get rid of the obsolete default_attrs field.

kernel.h is being used as a dump for all kinds of stuff for a long time.
Here is the attempt to start cleaning it up by splitting out panic and
oops helpers.
There are several purposes of doing this:
- dropping dependency in bug.h
- dropping a loop by moving out panic_notifier.h
- unload kernel.h from something which has its own domain
At the same time convert users tree-wide to use new headers, although for
the time being include new header back to kernel.h to avoid twisted
indirected includes for existing users.

Split out all the bits that are purely for dma_map_ops implementations
and related code into a new <linux/dma-map-ops.h> header so that they
don't get pulled into all the drivers. That also means the architecture
specific <asm/dma-mapping.h> is not pulled in by <linux/dma-mapping.h>
any more, which leads to a missing includes that were pulled in by the
x86 or arm versions in a few not overly portable drivers.

This API is the equivalent of alloc_pages, except that the returned memory
is guaranteed to be DMA addressable by the passed in device. The
implementation will also be used to provide a more sensible replacement
for DMA_ATTR_NON_CONSISTENT flag.
Additionally dma_alloc_noncoherent is switched over to use dma_alloc_pages
as its backend.

We found that callers of dma_get_seg_boundary mostly do an ALIGN
with page mask and then do a page shift to get number of pages:
ALIGN(boundary + 1, 1 << shift) >> shift
However, the boundary might be as large as ULONG_MAX, which means
that a device has no specific boundary limit. So either "+ 1" or
passing it to ALIGN() would potentially overflow.
According to kernel defines:

#define ALIGN(x, a) ALIGN_MASK(x, (typeof(x))(a) - 1)
We can simplify the logic here into a helper function doing:

This patch introduces and applies dma_get_seg_boundary_nr_pages()
as an overflow-free helper for the dma_get_seg_boundary() callers
to get numbers of pages. It also takes care of the NULL dev case
for non-DMA API callers.

When using kexec the SBA IOMMU IBASE might still have the RE
bit set. This triggers a WARN_ON when trying to write back the

request_irq() is preferred over setup_irq(). Invocations of setup_irq()
occur after memory allocators are ready.
Per tglx[1], setup_irq() existed in olden days when allocators were not
ready by the time early interrupts were initialized.
Hence replace setup_irq() by request_irq().

This breaks booting from sata_sil24 with the recent DMA change.
According to James Bottomley this was in to improve performance by
kicking the device into 32 bit descriptors, which are usually more
efficient, especially with older dual descriptor format cards like we
have on parisc systems.
Remove it for now to make DMA working again.

The HP Dino PCI controller chip can be used in two variants: as on-board
controller (e.g. in B160L), or on an Add-On card ("Card-Mode") to bridge
PCI components to systems without a PCI bus, e.g. to a HSC/GSC bus. One
such Add-On card is the HP HSC-PCI Card which has one or more DEC Tulip
PCI NIC chips connected to the on-card Dino PCI controller.
Dino in Card-Mode has a big disadvantage: All PCI memory accesses need
to go through the DINO_MEM_DATA register, so Linux drivers will not be
able to use the ioremap() function. Without ioremap() many drivers will
not work, one example is the tulip driver which then simply crashes the
kernel if it tries to access the ports on the HP HSC card.
This patch disables the HP HSC card if it finds one, and as such
fixes the kernel crash on a HP D350/2 machine.

parisc is the only architecture that sets ARCH_NO_COHERENT_DMA_MMAP
when an MMU is enabled. AFAIK this is because parisc CPUs use VIVT
caches, which means exporting normally cachable memory to userspace is
relatively dangrous due to cache aliasing.
But normally cachable memory is only allocated by dma_alloc_coherent
on parisc when using the sba_iommu or ccio_iommu drivers, so just
remove the .mmap implementation for them so that we don't have to set
ARCH_NO_COHERENT_DMA_MMAP, which I plan to get rid of.

While the default ->mmap and ->get_sgtable implementations work for the
majority of our dma_map_ops impementations they are inherently safe
for others that don't use the page allocator or CMA and/or use their
own way of remapping not covered by the common code. So remove the
defaults if these methods are not wired up, but instead wire up the
default implementations for all safe instances.

Pull parisc fixes from Helge Deller:
- Fix crashes when accessing PCI devices on some machines like C240 and
J5000. The crashes were triggered because we replaced cache flushes
by nops in the alternative coding where we shouldn't for some
machines.
- Dave fixed a race in the usage of the sr1 space register when used to
load the coherence index.
- Use the hardware lpa instruction to to load the physical address of
kernel virtual addresses in the iommu driver code.
- The kernel may fail to link when CONFIG_MLONGCALLS isn't set. Solve

Most I/O in the kernel is done using the kernel offset mapping.
However, there is one API that uses aliased kernel address ranges:

For this reason, we should use the hardware lpa instruction to load the

I believe we only use the vmap/vmalloc API with old PA 1.x processors
which don't have a sba, so we don't hit this problem.
Tested on c3750, c8000 and rp3440.

We only support I/O to kernel space. Using %sr1 to load the coherence
index may be racy unless interrupts are disabled. This patch changes the
code used to load the coherence index to use implicit space register
selection. This saves one instruction and eliminates the race.
Tested on rp3440, c8000 and c3750.

Similar to commit bcf3f1752a ("parisc: Hide Diva-built-in serial aux and
graphics card") it's better to hide the built-in serial AUX port at bootup.
When not hiding the port, the Linux serial driver will try to manage this port
and fails on a A500 server like this:

Main motivation was that managed_page_count_lock handling was complicating
things. It was discussed in length here,

Instead provide a selectable HAVE_PCI symbol that indicates availability
of PCI support, and a FORCE_PCI symbol to for PCI on and the handle the
rest in drivers/pci.

This patch adds the necessary code to patch a running kernel at runtime
to improve performance.
The current implementation offers a few optimizations variants:
- When running a SMP kernel on a single UP processor, unwanted assembler
statements like locking functions are overwritten with NOPs. When
multiple instructions shall be skipped, one branch instruction is used
instead of multiple nop instructions.
- In the UP case, some pdtlb and pitlb instructions are patched to
become pdtlb,l and pitlb,l which only flushes the CPU-local tlb
entries instead of broadcasting the flush to other CPUs in the system
and thus may improve performance.
- fic and fdc instructions are skipped if no I- or D-caches are
installed. This should speed up qemu emulation and cacheless systems.
- If no cache coherence is needed for IO operations, the relevant fdc
and sync instructions in the sba and ccio drivers are replaced by
nops.
- On systems which share I- and D-TLBs and thus don't have a seperate
instruction TLB, the pitlb instruction is replaced by a nop.
Live-patching is done early in the boot process, just after having run
the system inventory. No drivers are running and thus no external
interrupts should arrive. So the hope is that no TLB exceptions will
occur during the patching. If this turns out to be wrong we will
probably need to do the patching in real-mode.

This was used by the ide, scsi and networking code in the past to
determine if they should bounce payloads. Now that the dma mapping
always have to support dma to all physical memory (thanks to swiotlb
for non-iommu systems) there is no need to this crude hack any more.

Carlo Pisani noticed that his C3600 workstation behaved unstable during heavy
I/O on the PCI bus with a VIA VT6421 IDE/SATA PCI card.
To avoid such instability, this patch switches the LBA PCI bus from Hard Fail
mode into Soft Fail mode. In this mode the bus will return -1UL for timed out
MMIO transactions, which is exactly how the x86 (and most other architectures)
PCI busses behave.

Diva GSP card has built-in serial AUX port and ATI graphic card which simply
don't work and which both don't have external connectors. User Guides even
mention that those devices shouldn't be used.
So, prevent that Linux drivers try to enable those devices.

Many source files in the tree are missing licensing information, which
makes it harder for compliance tools to determine the correct license.
By default all files without license information are under the default
license of the kernel, which is GPL version 2.
Update the files which contain no license information with the 'GPL-2.0'
SPDX license identifier. The SPDX identifier is a legally binding
shorthand, which can be used instead of the full boiler plate text.
This patch is based on work done by Thomas Gleixner and Kate Stewart and
Philippe Ombredanne.
How this work was done:
Patches were generated and checked against linux-4.14-rc6 for a subset of
the use cases:
- file had no licensing information it it.
- file was a */uapi/* one with no licensing information in it,
- file was a */uapi/* one with existing licensing information,
Further patches will be generated in subsequent months to fix up cases
where non-standard license headers were used, and references to license
had to be inferred by heuristics based on keywords.
The analysis to determine which SPDX License Identifier to be applied to
a file was done in a spreadsheet of side by side results from of the
output of two independent scanners (ScanCode & Windriver) producing SPDX
tag:value files created by Philippe Ombredanne. Philippe prepared the
base worksheet, and did an initial spot review of a few 1000 files.
The 4.13 kernel was the starting point of the analysis with 60,537 files
assessed. Kate Stewart did a file by file comparison of the scanner
results in the spreadsheet to determine which SPDX license identifier(s)
to be applied to the file. She confirmed any determination that was not
immediately clear with lawyers working with the Linux Foundation.
Criteria used to select files for SPDX license identifier tagging was:
- Files considered eligible had to be source code files.
- Make and config files were included as candidates if they contained >5
lines of source
- File already had some variant of a license header in it (even if <5
lines).
All documentation files were explicitly excluded.
The following heuristics were used to determine which SPDX license
identifiers to apply.
- when both scanners couldn't find any license traces, file was
considered to have no license information in it, and the top level
COPYING file license applied.
For non */uapi/* files that summary was:

- when the two scanners agreed on the detected license(s), that became
the concluded license(s).
- when there was disagreement between the two scanners (one detected a
license but the other didn't, or they both detected different
licenses) a manual inspection of the file occurred.
- In most cases a manual inspection of the information in the file
resulted in a clear resolution of the license that should apply (and
which scanner probably needed to revisit its heuristics).
- When it was not immediately clear, the license identifier was
confirmed with lawyers working with the Linux Foundation.
- If there was any question as to the appropriate license identifier,
the file was flagged for further research and to be revisited later
in time.
In total, over 70 hours of logged manual review was done on the
spreadsheet to determine the SPDX license identifiers to apply to the
source files by Kate, Philippe, Thomas and, in some cases, confirmation
by lawyers working with the Linux Foundation.
Kate also obtained a third independent scan of the 4.13 code base from
FOSSology, and compared selected files where the other two scanners
disagreed against that SPDX file, to see if there was new insights. The
Windriver scanner is based on an older version of FOSSology in part, so
they are related.
Thomas did random spot checks in about 500 files from the spreadsheets
for the uapi headers and agreed with SPDX license identifier in the
files he inspected. For the non-uapi files Thomas did random spot checks
in about 15000 files.
In initial set of patches against 4.14-rc6, 3 files were found to have
copy/paste license identifier errors, and have been fixed to reflect the
correct identifier.
Additionally Philippe spent 10 hours this week doing a detailed manual
inspection and review of the 12,461 patched files from the initial patch
version early this week with:
- a full scancode scan run, collecting the matched texts, detected
license ids and scores
- reviewing anything where there was a license detected (about 500+
files) to ensure that the applied SPDX license was correct
- reviewing anything where there was no detection but the patch license
was not GPL-2.0 WITH Linux-syscall-note to ensure that the applied
SPDX license was correct
This produced a worksheet with 20 files needing minor correction. This
worksheet was then exported into 3 different .csv files for the
different types of files to be modified.
These .csv files were then reviewed by Greg. Thomas wrote a script to
parse the csv files and add the proper SPDX tag to the file, in the
format that the file expected. This script was further refined by Greg
based on the output to detect more types of files automatically and to
distinguish between header and source .c files (which need different
comment types.) Finally Greg ran the script using the .csv files to
generate the patches.

For 64bit kernels the lmmio_space_offset of the host bridge window
isn't set correctly on systems with dino/cujo PCI host bridges.
This leads to not assigned memory bars and failing drivers, which
need to use these bars.

will have to be picked up from other headers and a couple of .c files.
Create a trivial placeholder <linux/sched/signal.h> file that just
maps to <linux/sched.h> to make this patch obviously correct and
bisectable.
Include the new header in the files that are going to need it.

The dma-mapping core and the implementations do not change the DMA
attributes passed by pointer. Thus the pointer can point to const data.
However the attributes do not have to be a bitfield. Instead unsigned
long will do fine:
1. This is just simpler. Both in terms of reading the code and setting
attributes. Instead of initializing local attributes on the stack
and passing pointer to it to dma_set_attr(), just set the bits.
2. It brings safeness and checking for const correctness because the
attributes are passed by value.

I/O resource descriptor, 'desc' in struct resource, needs to be
initialized to zero by default. Some drivers call kmalloc() to
allocate a resource entry, but do not initialize it to zero by
memset(). Change these drivers to call kzalloc(), instead.

The cause of the crash is not exhaustion of the IOMMU space, there is
plenty of free pages. The function sba_alloc_range is called with size
0x11000, thus the pages_needed variable is 0x11. The function
sba_search_bitmap is called with bits_wanted 0x11 and boundary size is
0x10 (because dma_get_seg_boundary(dev) returns 0xffff).
The function sba_search_bitmap attempts to allocate 17 pages that must not
cross 16-page boundary - it can't satisfy this requirement
(iommu_is_span_boundary always returns true) and fails even if there are
many free entries in the IOMMU space.
How did it happen that we try to allocate 17 pages that don't cross
16-page boundary? The cause is in the function iommu_coalesce_chunks. This
function tries to coalesce adjacent entries in the scatterlist. The
function does several checks if it may coalesce one entry with the next,
one of those checks is this:

It is possible that (startsg->length + dma_len > max_seg_size) is false
(we are just near the 0x10000 max_seg_size boundary), so the funcion
decides to coalesce this entry with the next entry. When the coalescing
succeeds, the function performs

And now, because of non-zero dma_offset, dma_len is greater than 0x10000.
iommu_alloc_range (a pointer to sba_alloc_range) is called and it attempts
to allocate 17 pages for a device that must not cross 16-page boundary.
To fix the bug, we must make sure that dma_len after addition of
dma_offset and alignment doesn't cross the segment boundary. I.e. change

This patch makes this change (it precalculates max_seg_boundary at the
beginning of the function iommu_coalesce_chunks). I also added a check
that the mapping length doesn't exceed dma_get_seg_boundary(dev) (it is
not needed for Promise TX2+ SATA, but it may be needed for other devices
that have dma_get_seg_boundary lower than dma_get_max_seg_size).
