82995cc6c5 libceph, rbd, ceph: convert to use the new mount API
Convert the ceph filesystem to the new internal mount API as the old
one will be obsoleted and removed. This allows greater flexibility in
communication of mount parameters between userspace, the VFS and the
filesystem.

See Documentation/filesystems/mount_api.txt for more information.

[ Numerous string handling, leak and regression fixes; rbd conversion
  was particularly broken and had to be redone almost from scratch. ]

cf73d882cc libceph: use ceph_kvmalloc() for osdmap arrays
osdmap has a bunch of arrays that grow linearly with the number of
OSDs.  osd_state, osd_weight and osd_primary_affinity take 4 bytes per
OSD. osd_addr takes 136 bytes per OSD because of sockaddr_storage.
The CRUSH workspace area also grows linearly with the number of OSDs.

Normally these arrays are allocated at client startup. The osdmap is
usually updated in small incrementals, but once in a while a full map
may need to be processed. For a cluster with 10000 OSDs, this means
a bunch of 40K allocations followed by a 1.3M allocation, all of which
are currently required to be physically contiguous. This results in
sporadic ENOMEM errors, hanging the client.

Go back to manually (re)allocating arrays and use ceph_kvmalloc() to
fall back to non-contiguous allocation when necessary.

10c12851a0 libceph: avoid a __vmalloc() deadlock in ceph_kvmalloc()
The vmalloc allocator doesn't fully respect the specified gfp mask:
while the actual pages are allocated as requested, the page table pages
are always allocated with GFP_KERNEL. ceph_kvmalloc() may be called
with GFP_NOFS and GFP_NOIO (for ceph and rbd respectively), so this may
result in a deadlock.

There is no real reason for the current PAGE_ALLOC_COSTLY_ORDER logic,
it's just something that seemed sensible at the time (ceph_kvmalloc()
predates kvmalloc()). kvmalloc() is smarter: in an attempt to reduce
long term fragmentation, it first tries to kmalloc non-disruptively.

Switch to kvmalloc() and set the respective PF_MEMALLOC_* flag using
the scope API to avoid the deadlock. Note that kvmalloc() needs to be
passed GFP_KERNEL to enable the fallback.

e8c99200b4 libceph: don't call crypto_free_sync_skcipher() on a NULL tfm
In set_secret(), key->tfm is assigned to NULL on line 55, and then
ceph_crypto_key_destroy(key) is executed.

ceph_crypto_key_destroy(key)
  crypto_free_sync_skcipher(key->tfm)
    crypto_free_skcipher(&tfm->base);

This happens to work because crypto_sync_skcipher is a trivial wrapper
around crypto_skcipher: &tfm->base is still 0 and crypto_free_skcipher()
handles that. Let's not rely on the layout of crypto_sync_skcipher.

This bug is found by a static analysis tool STCheck written by us.

a561372405 libceph: fix PG split vs OSD (re)connect race
We can't rely on ->peer_features in calc_target() because it may be
called both when the OSD session is established and open and when it's
not. ->peer_features is not valid unless the OSD session is open. If
this happens on a PG split (pg_num increase), that could mean we don't
resend a request that should have been resent, hanging the client
indefinitely.

In userspace this was fixed by looking at require_osd_release and
get_xinfo[osd].features fields of the osdmap. However these fields
belong to the OSD section of the osdmap, which the kernel doesn't
decode (only the client section is decoded).

Instead, let's drop this feature check. It effectively checks for
luminous, so only pre-luminous OSDs would be affected in that on a PG
split the kernel might resend a request that should not have been
resent. Duplicates can occur in other scenarios, so both sides should
already be prepared for them: see dup/replay logic on the OSD side and
retry_attempt check on the client side.

d9b9c89304 Lots of exciting things this time!
- support for rbd object-map and fast-diff features (myself). This
   will speed up reads, discards and things like snap diffs on sparse
   images.

 - ceph.snap.btime vxattr to expose snapshot creation time (David
   Disseldorp). This will be used to integrate with "Restore Previous
   Versions" feature added in Windows 7 for folks who reexport ceph
   through SMB.

 - security xattrs for ceph (Zheng Yan). Only selinux is supported
   for now due to the limitations of ->dentry_init_security().

 - support for MSG_ADDR2, FS_BTIME and FS_CHANGE_ATTR features (Jeff
   Layton). This is actually a single feature bit which was missing
   because of the filesystem pieces. With this in, the kernel client
   will finally be reported as "luminous" by "ceph features" -- it is
   still being reported as "jewel" even though all required Luminous
   features were implemented in 4.13.

 - stop NULL-terminating ceph vxattrs (Jeff Layton). The convention
   with xattrs is to not terminate and this was causing inconsistencies
   with ceph-fuse.

 - change filesystem time granularity from 1 us to 1 ns, again fixing
   an inconsistency with ceph-fuse (Luis Henriques).

 On top of this there are some additional dentry name handling and cap
 flushing fixes from Zheng. Finally, Jeff is formally taking over for
 Zheng as the filesystem maintainer.

f632a8170a Driver Core and debugfs changes for 5.3-rc1
Here is the "big" driver core and debugfs changes for 5.3-rc1

 It's a lot of different patches, all across the tree due to some api
 changes and lots of debugfs cleanups. Because of this, there is going
 to be some merge issues with your tree at the moment, I'll follow up
 with the expected resolutions to make it easier for you.

 Other than the debugfs cleanups, in this set of changes we have:
 	- bus iteration function cleanups (will cause build warnings
 	  with s390 and coresight drivers in your tree)
 	- scripts/get_abi.pl tool to display and parse Documentation/ABI
 	  entries in a simple way
 	- cleanups to Documenatation/ABI/ entries to make them parse
 	  easier due to typos and other minor things
 	- default_attrs use for some ktype users
 	- driver model documentation file conversions to .rst
 	- compressed firmware file loading
 	- deferred probe fixes

 All of these have been in linux-next for a while, with a bunch of merge
 issues that Stephen has been patient with me for. Other than the merge
 issues, functionality is working properly in linux-next :)

It turns out that it breaks booting with an encrypted volume, and Eric
biggers reports that it also breaks the fscrypt tests [1] and loading of
in-kernel X.509 certificates [2].

The root cause of all the breakage is likely the same, but David Howells
is off email so rather than try to work it out it's getting reverted in
order to not impact the rest of the merge window.

Pull keyring ACL support from David Howells:
 "This changes the permissions model used by keys and keyrings to be
  based on an internal ACL by the following means:

   - Replace the permissions mask internally with an ACL that contains a
     list of ACEs, each with a specific subject with a permissions mask.
     Potted default ACLs are available for new keys and keyrings.

     ACE subjects can be macroised to indicate the UID and GID specified
     on the key (which remain). Future commits will be able to add
     additional subject types, such as specific UIDs or domain
     tags/namespaces.

     Also split a number of permissions to give finer control. Examples
     include splitting the revocation permit from the change-attributes
     permit, thereby allowing someone to be granted permission to revoke
     a key without allowing them to change the owner; also the ability
     to join a keyring is split from the ability to link to it, thereby
     stopping a process accessing a keyring by joining it and thus
     acquiring use of possessor permits.

   - Provide a keyctl to allow the granting or denial of one or more

Merge tag 'keys-namespace-20190627' of git://git.kernel.org/pub/scm/linux/kernel/git/dhowells/linux-fs

Pull keyring namespacing from David Howells:
 "These patches help make keys and keyrings more namespace aware.

  Firstly some miscellaneous patches to make the process easier:

   - Simplify key index_key handling so that the word-sized chunks
     assoc_array requires don't have to be shifted about, making it
     easier to add more bits into the key.

   - Cache the hash value in the key so that we don't have to calculate
     on every key we examine during a search (it involves a bunch of
     multiplications).

   - Allow keying_search() to search non-recursively.

  Then the main patches:

   - Make it so that keyring names are per-user_namespace from the point
     of view of KEYCTL_JOIN_SESSION_KEYRING so that they're not
     accessible cross-user_namespace.

     keyctl_capabilities() shows KEYCTL_CAPS1_NS_KEYRING_NAME for this.

   - Move the user and user-session keyrings to the user_namespace
     rather than the user_struct. This prevents them propagating
     directly across user_namespaces boundaries (ie. the KEY_SPEC_*
     flags will only pick from the current user_namespace).

   - Make it possible to include the target namespace in which the key
     shall operate in the index_key. This will allow the possibility of
     multiple keys with the same description, but different target
     domains to be held in the same keyring.

     keyctl_capabilities() shows KEYCTL_CAPS1_NS_KEY_TAG for this.

   - Make it so that keys are implicitly invalidated by removal of a
     domain tag, causing them to be garbage collected.

   - Institute a network namespace domain tag that allows keys to be
     differentiated by the network namespace in which they operate. New
     keys that are of a type marked 'KEY_TYPE_NET_DOMAIN' are assigned
     the network domain in force when they are created.

   - Make it so that the desired network namespace can be handed down
     into the request_key() mechanism. This allows AFS, NFS, etc. to
     request keys specific to the network namespace of the superblock.

     This also means that the keys in the DNS record cache are
     thenceforth namespaced, provided network filesystems pass the
     appropriate network namespace down into dns_query().

     For DNS, AFS and NFS are good, whilst CIFS and Ceph are not. Other
     cache keyrings, such as idmapper keyrings, also need to set the
     domain tag - for which they need access to the network namespace of
     the superblock"

Invalid object maps are not trusted, but still updated. Note that we
never iterate, resize or invalidate object maps. If object-map feature

2e12256b9a keys: Replace uid/gid/perm permissions checking with an ACL
Replace the uid/gid/perm permissions checking on a key with an ACL to allow
the SETATTR and SEARCH permissions to be split. This will also allow a
greater range of subjects to represented.

============
WHY DO THIS?
============

The problem is that SETATTR and SEARCH cover a slew of actions, not all of
which should be grouped together.

For SETATTR, this includes actions that are about controlling access to a
key:

 (1) Changing a key's ownership.

 (2) Changing a key's security information.

 (3) Setting a keyring's restriction.

And actions that are about managing a key's lifetime:

 (4) Setting an expiry time.

 (5) Revoking a key.

and (proposed) managing a key as part of a cache:

 (6) Invalidating a key.

Managing a key's lifetime doesn't really have anything to do with
controlling access to that key.

Expiry time is awkward since it's more about the lifetime of the content
and so, in some ways goes better with WRITE permission. It can, however,
be set unconditionally by a process with an appropriate authorisation token
for instantiating a key, and can also be set by the key type driver when a
key is instantiated, so lumping it with the access-controlling actions is
probably okay.

As for SEARCH permission, that currently covers:

 (1) Finding keys in a keyring tree during a search.

 (2) Permitting keyrings to be joined.

 (3) Invalidation.

But these don't really belong together either, since these actions really
need to be controlled separately.

Finally, there are number of special cases to do with granting the
administrator special rights to invalidate or clear keys that I would like
to handle with the ACL rather than key flags and special checks.

===============
WHAT IS CHANGED
===============

The SETATTR permission is split to create two new permissions:

 (1) SET_SECURITY - which allows the key's owner, group and ACL to be
     changed and a restriction to be placed on a keyring.

 (2) REVOKE - which allows a key to be revoked.

The SEARCH permission is split to create:

 (1) SEARCH - which allows a keyring to be search and a key to be found.

 (2) JOIN - which allows a keyring to be joined as a session keyring.

 (3) INVAL - which allows a key to be invalidated.

The WRITE permission is also split to create:

 (1) WRITE - which allows a key's content to be altered and links to be
     added, removed and replaced in a keyring.

 (2) CLEAR - which allows a keyring to be cleared completely. This is
     split out to make it possible to give just this to an administrator.

 (3) REVOKE - see above.

Keys acquire ACLs which consist of a series of ACEs, and all that apply are
unioned together. An ACE specifies a subject, such as:

 (*) Possessor - permitted to anyone who 'possesses' a key
 (*) Owner - permitted to the key owner
 (*) Group - permitted to the key group
 (*) Everyone - permitted to everyone

Note that 'Other' has been replaced with 'Everyone' on the assumption that
you wouldn't grant a permit to 'Other' that you wouldn't also grant to
everyone else.

Further subjects may be made available by later patches.

The ACE also specifies a permissions mask. The set of permissions is now:

	VIEW Can view the key metadata
	READ Can read the key content
	WRITE Can update/modify the key content
	SEARCH Can find the key by searching/requesting
	LINK Can make a link to the key
	SET_SECURITY	Can change owner, ACL, expiry
	INVAL		Can invalidate
	REVOKE		Can revoke
	JOIN		Can join this keyring
	CLEAR		Can clear this keyring

The KEYCTL_SETPERM function is then deprecated.

The KEYCTL_SET_TIMEOUT function then is permitted if SET_SECURITY is set,
or if the caller has a valid instantiation auth token.

The KEYCTL_INVALIDATE function then requires INVAL.

The KEYCTL_REVOKE function then requires REVOKE.

The KEYCTL_JOIN_SESSION_KEYRING function then requires JOIN to join an
existing keyring.

The JOIN permission is enabled by default for session keyrings and manually
created keyrings only.

======================
BACKWARD COMPATIBILITY
======================

To maintain backward compatibility, KEYCTL_SETPERM will translate the
permissions mask it is given into a new ACL for a key - unless
KEYCTL_SET_ACL has been called on that key, in which case an error will be
returned.

It will convert possessor, owner, group and other permissions into separate
ACEs, if each portion of the mask is non-zero.

SETATTR permission turns on all of INVAL, REVOKE and SET_SECURITY. WRITE
permission turns on WRITE, REVOKE and, if a keyring, CLEAR. JOIN is turned
on if a keyring is being altered.

The KEYCTL_DESCRIBE function translates the ACL back into a permissions
mask to return depending on possessor, owner, group and everyone ACEs.

It will make the following mappings:

 (1) INVAL, JOIN -> SEARCH

 (2) SET_SECURITY -> SETATTR

 (3) REVOKE -> WRITE if SETATTR isn't already set

 (4) CLEAR -> WRITE

Note that the value subsequently returned by KEYCTL_DESCRIBE may not match
the value set with KEYCTL_SETATTR.

=======
TESTING
=======

This passes the keyutils testsuite for all but a couple of tests:

 (1) tests/keyctl/dh_compute/badargs: The first wrong-key-type test now
     returns EOPNOTSUPP rather than ENOKEY as READ permission isn't removed
     if the type doesn't have ->read(). You still can't actually read the
     key.

 (2) tests/keyctl/permitting/valid: The view-other-permissions test doesn't
     work as Other has been replaced with Everyone in the ACL.

73b0140bf0 mm/gup: change GUP fast to use flags rather than a write 'bool'
To facilitate additional options to get_user_pages_fast() change the
singular write parameter to be gup_flags.

This patch does not change any functionality. New functionality will
follow in subsequent patches.

Some of the get_user_pages_fast() call sites were unchanged because they
already passed FOLL_WRITE or 0 for the write parameter.

NOTE: It was suggested to change the ordering of the get_user_pages_fast()
arguments to ensure that callers were converted. This breaks the current
GUP call site convention of having the returned pages be the final
parameter. So the suggestion was rejected.

b726ec972c libceph: make ceph_pr_addr take an struct ceph_entity_addr pointer
GCC9 is throwing a lot of warnings about unaligned accesses by
callers of ceph_pr_addr. All of the current callers are passing a
pointer to the sockaddr inside struct ceph_entity_addr.

Fix it to take a pointer to a struct ceph_entity_addr instead,
and then have the function make a copy of the sockaddr before
printing it.

187df76325 libceph: fix breakage caused by multipage bvecs
A bvec can now consist of multiple physically contiguous pages.
This means that bvec_iter_advance() can move to a different page while
staying in the same bvec (i.e. ->bi_bvec_done != 0).

The messenger works in terms of segments which can now be defined as
the smaller of a bvec and a page. The "more bytes to process in this
segment" condition holds only if bvec_iter_advance() leaves us in the
same bvec _and_ in the same page. On next bvec (possibly in the same
page) and on next page (possibly in the same bvec) we may need to set
->last_piece.

bb229bbb3b libceph: wait for latest osdmap in ceph_monc_blacklist_add()
Because map updates are distributed lazily, an OSD may not know about
the new blacklist for quite some time after "osd blacklist add" command
is completed. This makes it possible for a blacklisted but still alive
client to overwrite a post-blacklist update, resulting in data
corruption.

Waiting for latest osdmap in ceph_monc_blacklist_add() and thus using
the post-blacklist epoch for all post-blacklist requests ensures that
all such requests "wait" for the blacklist to come into force on their
respective OSDs.

6b41d4d9cb libceph: use struct_size() for kmalloc() in crush_decode()
One of the more common cases of allocation size calculations is finding
the size of a structure that has a zero-sized array at the end, along
with memory for some number of elements for that array. For example:

struct foo {
    int stuff;
    struct boo entry[];
};

instance = kmalloc(sizeof(struct foo) + count * sizeof(struct boo), GFP_KERNEL);

Instead of leaving these open-coded and prone to type mistakes, we can
now use the new struct_size() helper:

instance = kmalloc(struct_size(instance, entry, count), GFP_KERNEL);

This code was detected with the help of Coccinelle.

0fd3fd0a9b libceph: handle an empty authorize reply
The authorize reply can be empty, for example when the ticket used to
build the authorizer is too old and TAG_BADAUTHORIZER is returned from
the service. Calling ->verify_authorizer_reply() results in an attempt
to decrypt and validate (somewhat) random data in au->buf (most likely
the signature block from calc_signature()), which fails and ends up in
con_fault_finish() with !con->auth_retry.  The ticket isn't invalidated
and the connection is retried again and again until a new ticket is
obtained from the monitor:

  libceph: osd2 192.168.122.1:6809 bad authorize reply

                                mutex_unlock(&con->mutex)
  set KEEPALIVE_PENDING
  set WRITE_PENDING

This triggers warnings in clear_standby() when either ceph_con_send()
or ceph_con_keepalive() get to clearing STANDBY next time.

I don't see a reason to condition queue_con() call on the previous
value of KEEPALIVE_PENDING, so move the setting of KEEPALIVE_PENDING
into the critical section -- unlike WRITE_PENDING, KEEPALIVE_PENDING
could have been a non-atomic flag.

433b0a1295 libceph: use MSG_SENDPAGE_NOTLAST with ceph_tcp_sendpage()
Prevent do_tcp_sendpages() from calling tcp_push() (at least) once per
page. Instead, arrange for tcp_push() to be called (at least) once per
data payload. This results in more MSS-sized packets and fewer packets
overall (5-10% reduction in my tests with typical OSD request sizes).
See commits 2f53384424 ("tcp: allow splice() to build full TSO
packets"), 35f9c09fe9 ("tcp: tcp_sendpages() should call tcp_push()
once") and ae62ca7b03 ("tcp: fix MSG_SENDPAGE_NOTLAST logic") for
details.

Here is an example of a packet size histogram for 128K OSD requests
(MSS = 1448, top 5):

Before:

     SIZE    COUNT
     1448   777700
      952   127915
     1200    39238
     1219     9806
       21     5675

After:

     SIZE    COUNT
     1448   897280
       21     6201
     1019     2797
      643     2739
      376     2479

We could do slightly better by explicitly corking the socket but it's
not clear it's worth it.

1f6b821aef libceph: drop last_piece logic from write_partial_message_data()
last_piece is for the last piece in the current data item, not in the
entire data payload of the message. This is harmful for messages with
multiple data items. On top of that, we don't need to signal the end
of a data payload either because it is always followed by a footer.

We used to signal "more" unconditionally, until commit fe38a2b67b
("libceph: start defining message data cursor"). Part of a large
series, it introduced cursor->last_piece and also mistakenly inverted
the hint by passing last_piece for "more". This was corrected with
commit c2cfa19400 ("libceph: Fix ceph_tcp_sendpage()'s more boolean
usage").

As it is, last_piece is not helping at all: because Nagle algorithm is
disabled, for a simple message with two 512-byte data items we end up
emitting three packets: front + first data item, second data item and
footer. Go back to the original pre-fe38a2b67bc6 behavior -- a single
packet in most cases.

7e241f647d libceph: fall back to sendmsg for slab pages
skb_can_coalesce() allows coalescing neighboring slab objects into
a single frag:

  return page == skb_frag_page(frag) &&
         off == frag->page_offset + skb_frag_size(frag);

ceph_tcp_sendpage() can be handed slab pages. One example of this is
XFS: it passes down sector sized slab objects for its metadata I/O. If
the kernel client is co-located on the OSD node, the skb may go through
loopback and pop on the receive side with the exact same set of frags.
When tcp_recvmsg() attempts to copy out such a frag, hardened usercopy
complains because the size exceeds the object's allocated size:

  usercopy: kernel memory exposure attempt detected from ffff9ba917f20a00 (kmalloc-512) (1024 bytes)

Although skb_can_coalesce() could be taught to return false if the
resulting frag would cross a slab object boundary, we already have
a fallback for non-refcounted pages. Utilize it for slab pages too.

aa563d7bca iov_iter: Separate type from direction and use accessor functions
In the iov_iter struct, separate the iterator type from the iterator
direction and use accessor functions to access them in most places.

Convert a bunch of places to use switch-statements to access them rather
then chains of bitwise-AND statements. This makes it easier to add further
iterator types. Also, this can be more efficient as to implement a switch
of small contiguous integers, the compiler can use ~50% fewer compare
instructions than it has to use bitwise-and instructions.

Further, cease passing the iterator type into the iterator setup function.
The iterator function can set that itself. Only the direction is required.

setup_request_data() adds message data items to both request and reply

This went unnoticed for years but now that message data items are
preallocated, it promptly crashes in ceph_msg_data_add(). Amend the
signature to make it clear that setup_request_data() operates on both
request and reply messages. Also, remove data_len assert -- we have
another one in prepare_write_message().

0d9c1ab3be libceph: preallocate message data items
Currently message data items are allocated with ceph_msg_data_create()
in setup_request_data() inside send_request().  send_request() has never
been allowed to fail, so each allocation is followed by a BUG_ON:

  data = ceph_msg_data_create(...);
  BUG_ON(!data);

It's been this way since support for multiple message data items was
added in commit 6644ed7b7e ("libceph: make message data be a pointer")
in 3.10.

There is no reason to delay the allocation of message data items until
the last possible moment and we certainly don't need a linked list of
them as they are only ever appended to the end and never erased. Make
ceph_msg_new2() take max_data_items and adapt the rest of the code.

The current requirement is that ceph_osdc_alloc_messages() should be

that it is called when OSD op codes are known.

3b83f60da6 libceph: enable fallback to ceph_msg_new() in ceph_msgpool_get()
ceph_msgpool_get() can fall back to ceph_msg_new() when it is asked for
a message whose front portion is larger than pool->front_len. However
the caller always passes 0, effectively disabling that code path. The
allocation goes to the message pool and returns a message with a front
that is smaller than requested, setting us up for a crash.

One example of this is a directory with a large number of snapshots.
If its snap context doesn't fit, we oops in encode_request_partial().

894868330a libceph: don't consume a ref on pagelist in ceph_msg_data_add_pagelist()
Because send_mds_reconnect() wants to send a message with a pagelist
and pass the ownership to the messenger, ceph_msg_data_add_pagelist()
consumes a ref which is then put in ceph_msg_data_destroy(). This
makes managing pagelists in the OSD client (where they are wrapped in
ceph_osd_data) unnecessarily hard because the handoff only happens in
ceph_osdc_start_request() instead of when the pagelist is passed to
ceph_osd_data_pagelist_init(). I counted several memory leaks on
various error paths.

Fix up ceph_msg_data_add_pagelist() and carry a pagelist ref in

6daca13d2e libceph: add authorizer challenge
When a client authenticates with a service, an authorizer is sent with
a nonce to the service (ceph_x_authorize_[ab]) and the service responds
with a mutation of that nonce (ceph_x_authorize_reply). This lets the
client verify the service is who it says it is but it doesn't protect
against a replay: someone can trivially capture the exchange and reuse
the same authorizer to authenticate themselves.

Allow the service to reject an initial authorizer with a random
challenge (ceph_x_authorize_challenge). The client then has to respond
with an updated authorizer proving they are able to decrypt the
service's challenge and that the new authorizer was produced for this
specific connection instance.

The accepting side requires this challenge and response unconditionally
if the client side advertises they have CEPHX_V2 feature bit.

This addresses CVE-2018-1128.

6da2ec5605 treewide: kmalloc() -> kmalloc_array()
The kmalloc() function has a 2-factor argument form, kmalloc_array(). This
patch replaces cases of:

        kmalloc(a * b, gfp)

with:
        kmalloc_array(a * b, gfp)

as well as handling cases of:

        kmalloc(a * b * c, gfp)

with:

        kmalloc(array3_size(a, b, c), gfp)

as it's slightly less ugly than:

        kmalloc_array(array_size(a, b), c, gfp)

This does, however, attempt to ignore constant size factors like:

        kmalloc(4 * 1024, gfp)

though any constants defined via macros get caught up in the conversion.

Any factors with a sizeof() of "unsigned char", "char", and "u8" were
dropped, since they're redundant.

The tools/ directory was manually excluded, since it has its own
implementation of kmalloc().

The Coccinelle script used for this was:

// Fix redundant parens around sizeof().

Pull overflow updates from Kees Cook:
 "This adds the new overflow checking helpers and adds them to the
  2-factor argument allocators. And this adds the saturating size
  helpers and does a treewide replacement for the struct_size() usage.
  Additionally this adds the overflow testing modules to make sure
  everything works.

  I'm still working on the treewide replacements for allocators with
  "simple" multiplied arguments:

     *alloc(a * b, ...) -> *alloc_array(a, b, ...)

  and

     *zalloc(a * b, ...) -> *calloc(a, b, ...)

  as well as the more complex cases, but that's separable from this
  portion of the series. I expect to have the rest sent before -rc1
  closes; there are a lot of messy cases to clean up.

  Summary:

   - Introduce overflow test module (Rasmus, Kees)

acafe7e302 treewide: Use struct_size() for kmalloc()-family
One of the more common cases of allocation size calculations is finding
the size of a structure that has a zero-sized array at the end, along
with memory for some number of elements for that array. For example:

struct foo {
    int stuff;
    void *entry[];
};

instance = kmalloc(sizeof(struct foo) + sizeof(void *) * count, GFP_KERNEL);

Instead of leaving these open-coded and prone to type mistakes, we can
now use the new struct_size() helper:

instance = kmalloc(struct_size(instance, entry, count), GFP_KERNEL);

This patch makes the changes for kmalloc()-family (and kvmalloc()-family)
uses. It was done via automatic conversion with manual review for the
"CHECKME" non-standard cases noted below, using the following Coccinelle
script:

// pkey_cache = kmalloc(sizeof *pkey_cache + tprops->pkey_tbl_len *
//                      sizeof *pkey_cache->table, GFP_KERNEL);

a86f009f10 libceph: allocate the locator string with GFP_NOFAIL
calc_target() isn't supposed to fail with anything but POOL_DNE, in
which case we report that the pool doesn't exist and fail the request
with -ENOENT. Doing this for -ENOMEM is at the very least confusing
and also harmful -- as the preceding requests complete, a short-lived
locator string allocation is likely to succeed after a wait.

(We used to call ceph_object_locator_to_pg() for a pi lookup. In
theory that could fail with -ENOENT, hence the "ret != -ENOENT" warning
being removed.)

c843d13cae libceph: make abort_on_full a per-osdc setting
The intent behind making it a per-request setting was that it would be
set for writes, but not for reads. As it is, the flag is set for all
fs/ceph requests except for pool perm check stat request (technically
a read).

ceph_osdc_abort_on_full() skips reads since the previous commit and
I don't see a use case for marking individual requests.

Don't consider reads for aborting and use ->base_oloc instead of
->target_oloc, as done in __submit_request().

Strictly speaking, we shouldn't be aborting FULL_TRY/FULL_FORCE writes
either. But, there is an inconsistency in FULL_TRY/FULL_FORCE handling
on the OSD side [1], so given that neither of these is used in the
kernel client, leave it for when the OSD behaviour is sorted out.

4eea0fefd7 libceph: use for_each_request() in ceph_osdc_abort_on_full()
Scanning the trees just to see if there is anything to abort is
unnecessary -- all that is needed here is to update the epoch barrier
first, before we start aborting. Simplify and do the update inside the
loop before calling abort_request() for the first time.

The switch to for_each_request() also fixes a bug: homeless requests
weren't even considered for aborting.

88bc1922c2 libceph: defer __complete_request() to a workqueue
In the common case, req->r_callback is called by handle_reply() on the
ceph-msgr worker thread without any locks. If handle_reply() fails, it
is called with both osd->lock and osdc->lock. In the map check case,
it is called with just osdc->lock but held for write. Finally, if the
request is aborted because of -ENOSPC or by ceph_osdc_abort_requests(),
it is called directly on the submitter's thread, again with both locks.

req->r_callback on the submitter's thread is relatively new (introduced
in 4.12) and ripe for deadlocks -- e.g. writeback worker thread waiting
on itself:

ceph_con_workfn() validates con->state before calling try_read() and

notably in process_message() and ceph_con_in_msg_alloc(), opening the
window for ceph_con_close() to sneak in, close the connection and
release con->sock. When try_write() is called on the assumption that
con->state is still valid (i.e. not STANDBY or CLOSED), a NULL sock
gets passed to the networking stack:

pretty easily hit after a prolonged hunting for a monitor: we don't get

to connect to a different monitor every 30 seconds:

5359a17d27 libceph, rbd: new bio handling code (aka don't clone bios)
The reason we clone bios is to be able to give each object request
(and consequently each ceph_osd_data/ceph_msg_data item) its own
pointer to a (list of) bio(s). The messenger then initializes its
cursor with cloned bio's ->bi_iter, so it knows where to start reading
from/writing to. That's all the cloned bios are used for: to determine
each object request's starting position in the provided data buffer.

Introduce ceph_bio_iter to do exactly that -- store position within bio
list (i.e. pointer to bio) + position within that bio (i.e. bvec_iter).

db2196a589 libceph: eliminate overflows in ceph_calc_file_object_mapping()
bl, stripeno and objsetno should be u64 -- otherwise large enough files
get corrupted. How large depends on file layout:

- 4M-objects layout (default): any file over 16P
- 64K-objects layout (smallest possible object size): any file over 512T

Only CephFS is affected, rbd doesn't use ceph_calc_file_object_mapping()
yet. Fortunately, CephFS has a max_file_size configurable, the default
for which is way below both of the above numbers.

Reimplement the logic from scratch with no layout validation -- it's

Greg Kroah-Hartman
b24413180f License cleanup: add SPDX GPL-2.0 license identifier to files with no license
Many source files in the tree are missing licensing information, which
makes it harder for compliance tools to determine the correct license.

By default all files without license information are under the default
license of the kernel, which is GPL version 2.

Update the files which contain no license information with the 'GPL-2.0'
SPDX license identifier. The SPDX identifier is a legally binding
shorthand, which can be used instead of the full boiler plate text.

This patch is based on work done by Thomas Gleixner and Kate Stewart and
Philippe Ombredanne.

How this work was done:

Patches were generated and checked against linux-4.14-rc6 for a subset of
the use cases:
 - file had no licensing information it it.
 - file was a */uapi/* one with no licensing information in it,
 - file was a */uapi/* one with existing licensing information,

Further patches will be generated in subsequent months to fix up cases
where non-standard license headers were used, and references to license
had to be inferred by heuristics based on keywords.

The analysis to determine which SPDX License Identifier to be applied to
a file was done in a spreadsheet of side by side results from of the
output of two independent scanners (ScanCode & Windriver) producing SPDX
tag:value files created by Philippe Ombredanne. Philippe prepared the
base worksheet, and did an initial spot review of a few 1000 files.

The 4.13 kernel was the starting point of the analysis with 60,537 files
assessed. Kate Stewart did a file by file comparison of the scanner
results in the spreadsheet to determine which SPDX license identifier(s)
to be applied to the file. She confirmed any determination that was not
immediately clear with lawyers working with the Linux Foundation.

Criteria used to select files for SPDX license identifier tagging was:
 - Files considered eligible had to be source code files.
 - Make and config files were included as candidates if they contained >5
   lines of source
 - File already had some variant of a license header in it (even if <5
   lines).

All documentation files were explicitly excluded.

The following heuristics were used to determine which SPDX license
identifiers to apply.

 - when both scanners couldn't find any license traces, file was
   considered to have no license information in it, and the top level
   COPYING file license applied.

   For non */uapi/* files that summary was:

   SPDX license identifier                            # files
   ---------------------------------------------------|-------
   GPL-2.0                                              11139

   and resulted in the first patch in this series.

   If that file was a */uapi/* path one, it was "GPL-2.0 WITH
   Linux-syscall-note" otherwise it was "GPL-2.0". Results of that was:

   SPDX license identifier                            # files
   ---------------------------------------------------|-------
   GPL-2.0 WITH Linux-syscall-note                        930

   and resulted in the second patch in this series.

 - if a file had some form of licensing information in it, and was one
   of the */uapi/* ones, it was denoted with the Linux-syscall-note if
   any GPL family license was found in the file or had no licensing in
   it (per prior point). Results summary:

   SPDX license identifier                            # files
   ---------------------------------------------------|------
   GPL-2.0 WITH Linux-syscall-note                       270
   GPL-2.0+ WITH Linux-syscall-note                      169
   ((GPL-2.0 WITH Linux-syscall-note) OR BSD-2-Clause)    21
   ((GPL-2.0 WITH Linux-syscall-note) OR BSD-3-Clause)    17
   LGPL-2.1+ WITH Linux-syscall-note                      15
   GPL-1.0+ WITH Linux-syscall-note                       14
   ((GPL-2.0+ WITH Linux-syscall-note) OR BSD-3-Clause)    5
   LGPL-2.0+ WITH Linux-syscall-note                       4
   LGPL-2.1 WITH Linux-syscall-note                        3
   ((GPL-2.0 WITH Linux-syscall-note) OR MIT)              3
   ((GPL-2.0 WITH Linux-syscall-note) AND MIT)             1

   and that resulted in the third patch in this series.

 - when the two scanners agreed on the detected license(s), that became
   the concluded license(s).

 - when there was disagreement between the two scanners (one detected a
   license but the other didn't, or they both detected different
   licenses) a manual inspection of the file occurred.

 - In most cases a manual inspection of the information in the file
   resulted in a clear resolution of the license that should apply (and
   which scanner probably needed to revisit its heuristics).

 - When it was not immediately clear, the license identifier was
   confirmed with lawyers working with the Linux Foundation.

 - If there was any question as to the appropriate license identifier,
   the file was flagged for further research and to be revisited later
   in time.

In total, over 70 hours of logged manual review was done on the
spreadsheet to determine the SPDX license identifiers to apply to the
source files by Kate, Philippe, Thomas and, in some cases, confirmation
by lawyers working with the Linux Foundation.

Kate also obtained a third independent scan of the 4.13 code base from
FOSSology, and compared selected files where the other two scanners
disagreed against that SPDX file, to see if there was new insights. The
Windriver scanner is based on an older version of FOSSology in part, so
they are related.

Thomas did random spot checks in about 500 files from the spreadsheets
for the uapi headers and agreed with SPDX license identifier in the
files he inspected. For the non-uapi files Thomas did random spot checks
in about 15000 files.

In initial set of patches against 4.14-rc6, 3 files were found to have
copy/paste license identifier errors, and have been fixed to reflect the
correct identifier.

Additionally Philippe spent 10 hours this week doing a detailed manual
inspection and review of the 12,461 patched files from the initial patch
version early this week with:
 - a full scancode scan run, collecting the matched texts, detected
   license ids and scores
 - reviewing anything where there was a license detected (about 500+
   files) to ensure that the applied SPDX license was correct
 - reviewing anything where there was no detection but the patch license
   was not GPL-2.0 WITH Linux-syscall-note to ensure that the applied
   SPDX license was correct

This produced a worksheet with 20 files needing minor correction. This
worksheet was then exported into 3 different .csv files for the
different types of files to be modified.

These .csv files were then reviewed by Greg. Thomas wrote a script to
parse the csv files and add the proper SPDX tag to the file, in the
format that the file expected. This script was further refined by Greg
based on the output to detect more types of files automatically and to
distinguish between header and source .c files (which need different
comment types.) Finally Greg ran the script using the .csv files to
generate the patches.

29a0cfbf91 libceph: don't allow bidirectional swap of pg-upmap-items
This reverts most of commit f53b7665c8 ("libceph: upmap semantic
changes").

We need to prevent duplicates in the final result. For example, we
can currently take

  [1,2,3] and apply [(1,2)] and get [2,2,3]

or

  [1,2,3] and apply [(3,2)] and get [1,2,2]

The rest of the system is not prepared to handle duplicates in the
result set like this.

The reverted piece was intended to allow

  [1,2,3] and [(1,2),(2,1)] to get [2,1,3]

to reorder primaries. First, this bidirectional swap is hard to
implement in a way that also prevents dups. For example, [1,2,3] and
[(1,4),(2,3),(3,4)] would give [4,3,4] but would we just drop the last
step we'd have [4,3,3] which is also invalid, etc. Simpler to just not
handle bidirectional swaps. In practice, they are not needed: if you
just want to choose a different primary then use primary_affinity, or
pg_upmap (not pg_upmap_items).

069f3222ca crush: implement weight and id overrides for straw2
bucket_straw2_choose needs to use weights that may be different from
weight_items. For instance to compensate for an uneven distribution
caused by a low number of values. Or to fix the probability biais
introduced by conditional probabilities (see
http://tracker.ceph.com/issues/15653 for more information).

We introduce a weight_set for each straw2 bucket to set the desired
weight for a given item at a given position. The weight of a given item
when picking the first replica (first position) may be different from
the weight the second replica (second position). For instance the weight
matrix for a given bucket containing items 3, 7 and 13 could be as
follows:

          position 0   position 1

item 3     0x10000      0x100000
item 7     0x40000       0x10000
item 13    0x40000       0x10000

When crush_do_rule picks the first of two replicas (position 0), item 7,
3 are four times more likely to be choosen by bucket_straw2_choose than
item 13. When choosing the second replica (position 1), item 3 is ten
times more likely to be choosen than item 7, 13.

By default the weight_set of each bucket exactly matches the content of
item_weights for each position to ensure backward compatibility.

bucket_straw2_choose compares items by using their id. The same ids are
also used to index buckets and they must be unique. For each item in a
bucket an array of ids can be provided for placement purposes and they
are used instead of the ids. If no replacement ids are provided, the
legacy behavior is preserved.

1c2e7b451b libceph: apply_upmap()
Previously, pg_to_raw_osds() didn't filter for existent OSDs because
raw_to_up_osds() would filter for "up" ("up" is predicated on "exists")
and raw_to_up_osds() was called directly after pg_to_raw_osds(). Now,
with apply_upmap() call in there, nonexistent OSDs in pg_to_raw_osds()
output can affect apply_upmap(). Introduce remove_nonexistent_osds()
to deal with that.

6f428df47d libceph: pg_upmap[_items] infrastructure
pg_temp and pg_upmap encodings are the same (PG -> array of osds),
except for the incremental remove: it's an empty mapping in new_pg_temp
for pg_temp and a separate old_pg_upmap set for pg_upmap.  (This isn't
to allow for empty pg_upmap mappings -- apparently, pg_temp just wasn't
looked at as an example for pg_upmap encoding.)

Reuse __decode_pg_temp() for decoding pg_upmap and new_pg_upmap.
__decode_pg_temp() stores into pg_temp union member, but since pg_upmap
union member is identical, reading through pg_upmap later is OK.

CURRENT_TIME is not y2038 safe. The macro will be deleted and all the
references to it will be replaced by ktime_get_* apis.

struct timespec is also not y2038 safe. Retain timespec for timestamp
representation here as ceph uses it internally everywhere. These
references will be changed to use struct timespec64 in a separate patch.

The current_fs_time() api is being changed to use vfs struct inode* as
an argument instead of struct super_block*.

Set the new mds client request r_stamp field using ktime_get_real_ts()
instead of using current_fs_time().

Also, since r_stamp is used as mtime on the server, use timespec_trunc()
to truncate the timestamp, using the right granularity from the
superblock.

This api will be transitioned to be y2038 safe along with vfs.

19809c2da2 mm, vmalloc: use __GFP_HIGHMEM implicitly
__vmalloc* allows users to provide gfp flags for the underlying
allocation. This API is quite popular

  $ git grep "=[[:space:]]__vmalloc\|return[[:space:]]*__vmalloc" | wc -l
  77

The only problem is that many people are not aware that they really want
to give __GFP_HIGHMEM along with other flags because there is really no
reason to consume precious lowmemory on CONFIG_HIGHMEM systems for pages
which are mapped to the kernel vmalloc space. About half of users don't
use this flag, though. This signals that we make the API unnecessarily
too complex.

This patch simply uses __GFP_HIGHMEM implicitly when allocating pages to
be mapped to the vmalloc space. Current users which add __GFP_HIGHMEM
are simplified and drop the flag.

58eb7932ae libceph: add an epoch_barrier field to struct ceph_osd_client
Cephfs can get cap update requests that contain a new epoch barrier in
them. When that happens we want to pause all OSD traffic until the right
map epoch arrives.

Add an epoch_barrier field to ceph_osd_client that is protected by the
osdc->lock rwsem. When the barrier is set, and the current OSD map
epoch is below that, pause the request target when submitting the
request or when revisiting it. Add a way for upper layers (cephfs)
to update the epoch_barrier as well.

If we get a new map, compare the new epoch against the barrier before
kicking requests and request another map if the map epoch is still lower
than the one we want.

If we get a map with a full pool, or at quota condition, then set the
barrier to the current epoch value.

fc36d0a42c libceph: abort already submitted but abortable requests when map or pool goes full
When a Ceph volume hits capacity, a flag is set in the OSD map to
indicate that, and a new map is sprayed around the cluster. With cephfs
we want it to shut down any abortable requests that are in progress with
an -ENOSPC error as they'd just hang otherwise.

Add a new ceph_osdc_abort_on_full helper function to handle this. It
will first check whether there is an out-of-space condition in the
cluster and then walk the tree and abort any request that has
r_abort_on_full set with a -ENOSPC error. Call this new function
directly whenever we get a new OSD map.

a1f4020aab libceph: allow requests to return immediately on full conditions if caller wishes
Usually, when the osd map is flagged as full or the pool is at quota,
write requests just hang. This is not what we want for cephfs, where
it would be better to simply report -ENOSPC back to userland instead
of stalling.

If the caller knows that it will want an immediate error return instead
of blocking on a full or at-quota error condition then allow it to set a
flag to request that behavior.

Set that flag in ceph_osdc_new_request (since ceph.ko is the only caller),
and on any other write request from ceph.ko.

A later patch will deal with requests that were submitted before the new
map showing the full condition came in.

osd_request_timeout specifies how many seconds to wait for a response
from OSDs before returning -ETIMEDOUT from an OSD request. 0 (default)
means no limit.

osd_request_timeout is osdkeepalive-precise -- in-flight requests are
swept through every osdkeepalive seconds. With ack vs commit behaviour
gone, abort_request() is really simple.
