79e3094c53 mm/filemap: drop check for truncated page after I/O
If the I/O completed successfully, the page will remain Uptodate, even
if it is subsequently truncated.  If the I/O completed with an error,
this check would cause us to retry the I/O if the page were truncated
before we woke up.  There is no need to retry the I/O; the I/O to fill
the page failed, so we can legitimately just return -EIO.

This code was originally added by commit 56f0d5fe6851 ("[PATCH]
readpage-vs-invalidate fix") in 2005 (this commit ID is from the
linux-fullhistory tree; it is also commit ba1f08f14b52 in tglx-history).

At the time, truncate_complete_page() called ClearPageUptodate(), and so
this was fixing a real bug.  In 2008, commit 84209e02de ("mm: dont clear
PG_uptodate on truncate/invalidate") removed the call to
ClearPageUptodate, and this check has been unnecessary ever since.

It doesn't do any real harm, but there's no need to keep it.

d31fa86a27 mm/filemap: use filemap_read_page in filemap_fault
After splitting generic_file_buffered_read() into smaller parts, it turns
out we can reuse one of the parts in filemap_fault().  This fixes an
oversight -- waiting for the I/O to complete is now interruptible by a
fatal signal.  And it saves us a few bytes of text in an unlikely path.

7a60d6d7b3 mm: use filemap_range_needs_writeback() for O_DIRECT reads
For the generic page cache read helper, use the better variant of checking
for the need to call filemap_write_and_wait_range() when doing O_DIRECT
reads.  This avoids falling back to the slow path for IOCB_NOWAIT, if
there are no pages to wait for (or write out).

63135aa386 mm: provide filemap_range_needs_writeback() helper
Patch series "Improve IOCB_NOWAIT O_DIRECT reads", v3.

An internal workload complained because it was using too much CPU, and
when I took a look, we had a lot of io_uring workers going to town.

For an async buffered read like workload, I am normally expecting _zero_
offloads to a worker thread, but this one had tons of them.  I'd drop
caches and things would look good again, but then a minute later we'd
regress back to using workers.  Turns out that every minute something
was reading parts of the device, which would add page cache for that
inode.  I put patches like these in for our kernel, and the problem was
solved.

Don't -EAGAIN IOCB_NOWAIT dio reads just because we have page cache
entries for the given range.  This causes unnecessary work from the
callers side, when the IO could have been issued totally fine without
blocking on writeback when there is none.

This patch (of 3):

For O_DIRECT reads/writes, we check if we need to issue a call to
filemap_write_and_wait_range() to issue and/or wait for writeback for any
page in the given range.  The existing mechanism just checks for a page in
the range, which is suboptimal for IOCB_NOWAIT as we'll fallback to the
slow path (and needing retry) if there's just a clean page cache page in
the range.

Provide filemap_range_needs_writeback() which tries a little harder to
check if we actually need to issue and/or wait for writeback in the range.

Pull network filesystem helper library updates from David Howells:
 "Here's a set of patches for 5.13 to begin the process of overhauling
  the local caching API for network filesystems. This set consists of
  two parts:

  (1) Add a helper library to handle the new VM readahead interface.

      This is intended to be used unconditionally by the filesystem
      (whether or not caching is enabled) and provides a common
      framework for doing caching, transparent huge pages and, in the
      future, possibly fscrypt and read bandwidth maximisation. It also
      allows the netfs and the cache to align, expand and slice up a
      read request from the VM in various ways; the netfs need only
      provide a function to read a stretch of data to the pagecache and
      the helper takes care of the rest.

  (2) Add an alternative fscache/cachfiles I/O API that uses the kiocb
      facility to do async DIO to transfer data to/from the netfs's
      pages, rather than using readpage with wait queue snooping on one
      side and vfs_write() on the other. It also uses less memory, since
      it doesn't do buffered I/O on the backing file.

      Note that this uses SEEK_HOLE/SEEK_DATA to locate the data
      available to be read from the cache. Whilst this is an improvement
      from the bmap interface, it still has a problem with regard to a
      modern extent-based filesystem inserting or removing bridging
      blocks of zeros. Fixing that requires a much greater overhaul.

  This is a step towards overhauling the fscache API. The change is
  opt-in on the part of the network filesystem. A netfs should not try
  to mix the old and the new API because of conflicting ways of handling
  pages and the PG_fscache page flag and because it would be mixing DIO
  with buffered I/O. Further, the helper library can't be used with the
  old API.

  This does not change any of the fscache cookie handling APIs or the
  way invalidation is done at this time.

  In the near term, I intend to deprecate and remove the old I/O API
  (fscache_allocate_page{,s}(), fscache_read_or_alloc_page{,s}(),
  fscache_write_page() and fscache_uncache_page()) and eventually
  replace most of fscache/cachefiles with something simpler and easier
  to follow.

  This patchset contains the following parts:

   - Some helper patches, including provision of an ITER_XARRAY iov
     iterator and a function to do readahead expansion.

   - Patches to add the netfs helper library.

   - A patch to add the fscache/cachefiles kiocb API.

   - A pair of patches to fix some review issues in the ITER_XARRAY and
     read helpers as spotted by Al and Willy.

  Jeff Layton has patches to add support in Ceph for this that he
  intends for this merge window. I have a set of patches to support AFS
  that I will post a separate pull request for.

  With this, AFS without a cache passes all expected xfstests; with a
  cache, there's an extra failure, but that's also there before these
  patches. Fixing that probably requires a greater overhaul. Ceph also
  passes the expected tests.

  I also have patches in a separate branch to tidy up the handling of
  PG_fscache/PG_private_2 and their contribution to page refcounting in
  the core kernel here, but I haven't included them in this set and will
  route them separately"

ed98b0159f mm/filemap: fix mapping_seek_hole_data on THP & 32-bit
No problem on 64-bit, or without huge pages, but xfstests generic/285
and other SEEK_HOLE/SEEK_DATA tests have regressed on huge tmpfs, and on
32-bit architectures, with the new mapping_seek_hole_data().  Several
different bugs turned out to need fixing.

u64 cast to stop losing bits when converting unsigned long to loff_t
(and let's use shifts throughout, rather than mixed with * and /).

Use round_up() when advancing pos, to stop assuming that pos was already
THP-aligned when advancing it by THP-size.  (This use of round_up()
assumes that any THP has THP-aligned index: true at present and true
going forward, but could be recoded to avoid the assumption.)

Use xas_set() when iterating away from a THP, so that xa_index stays in
synch with start, instead of drifting away to return bogus offset.

Check start against end to avoid wrapping 32-bit xa_index to 0 (and to
handle these additional cases, seek_data or not, it's easier to break
the loop than goto: so rearrange exit from the function).

2d11e73815 mm/filemap: fix find_lock_entries hang on 32-bit THP
No problem on 64-bit, or without huge pages, but xfstests generic/308
hung uninterruptibly on 32-bit huge tmpfs.

Since commit 0cc3b0ec23 ("Clarify (and fix) in 4.13 MAX_LFS_FILESIZE
macros"), MAX_LFS_FILESIZE is only a PAGE_SIZE away from wrapping 32-bit
xa_index to 0, so the new find_lock_entries() has to be extra careful
when handling a THP.

For readahead_expand(), we need to modify the file ra_state, so pass it
down by adding it to the ractl.  We have to do this because it's not always
the same as f_ra in the struct file that is already being passed.

Add three functions to manipulate PG_private_2:

 (*) set_page_private_2() - Set the flag and take an appropriate reference
     on the flagged page.

 (*) end_page_private_2() - Clear the flag, drop the reference and wake up
     any waiters, somewhat analogously with end_page_writeback().

 (*) wait_on_page_private_2() - Wait for the flag to be cleared.

Wrappers will need to be placed in the netfs lib header in the patch that
adds that.

[This implements a suggestion by Linus[1] to not mix the terminology of
 PG_private_2 and PG_fscache in the mm core function]

Changes:
v7:
- Use compound_head() in all the functions to make them THP safe[6].

v5:
- Add set and end functions, calling the end function end rather than
  unlock[3].
- Keep a ref on the page when PG_private_2 is set[4][5].

5c211ba29d mm: add and use find_lock_entries
We have three functions (shmem_undo_range(), truncate_inode_pages_range()
and invalidate_mapping_pages()) which want exactly this function, so add
it to filemap.c.  Before this patch, shmem_undo_range() would split any
compound page which overlaps either end of the range being punched in both
the first and second loops through the address space.  After this patch,
that functionality is left for the second loop, which is arguably more
appropriate since the first loop is supposed to run through all the pages
quickly, and splitting a page can sleep.

Patch series "Overhaul multi-page lookups for THP", v4.

This THP prep patchset changes several page cache iteration APIs to only
return head pages.

 - It's only possible to tag head pages in the page cache, so only
   return head pages, not all their subpages.
 - Factor a lot of common code out of the various batch lookup routines
 - Add mapping_seek_hole_data()
 - Unify find_get_entries() and pagevec_lookup_entries()
 - Make find_get_entries only return head pages, like find_get_entry().

These are only loosely connected, but they seem to make sense together as
a series.

This patch (of 14):

Pagecache tags are used for dirty page writeback.  Since dirtiness is
tracked on a per-THP basis, we only want to return the head page rather
than each subpage of a tagged page.  All the filesystems which use huge
pages today are in-memory, so there are no tagged huge pages today.

to atomic global updates.  But it can make the statistics more accuracy
for the THP vmstat counters.

So we convert the NR_SHMEM_THPS account to pages.  This patch is
consistent with 8f182270df ("mm/swap.c: flush lru pvecs on compound page
arrival").  Doing this also can make the unit of vmstat counters more
unified.  Finally, the unit of the vmstat counters are pages, kB and
bytes.  The B/KB suffix can tell us that the unit is bytes or kB.  The
rest which is without suffix are pages.

bd8a1f3655 mm/filemap: support readpage splitting a page
For page splitting to succeed, the thread asking to split the page has to
be the only one with a reference to the page.  Calling
wait_on_page_locked() while holding a reference to the page will
effectively prevent this from happening with sufficient threads waiting on
the same page.  Use put_and_wait_on_page_locked() to sleep without holding
a reference to the page, then retry the page lookup after the page is
unlocked.

Since we now get the page lock a little earlier in filemap_update_page(),
we can eliminate a number of duplicate checks.  The original intent
(commit ebded02788 ("avoid unnecessary calls to lock_page when waiting
for IO to complete during a read")) behind getting the page lock later was
to avoid re-locking the page after it has been brought uptodate by another
thread.  We still avoid that because we go through the normal lookup path
again after the winning thread has brought the page uptodate.

cbd59c48ae mm/filemap: use head pages in generic_file_buffered_read
Add filemap_get_read_batch() which returns the head pages which represent
a contiguous array of bytes in the file.  It also stops when encountering
a page marked as Readahead or !Uptodate (but does return that page) so it
can be handled appropriately by filemap_get_pages().  That lets us remove
the loop in filemap_get_pages() and check only the last page.

Before that commit, there was a try_charge() and commit_charge() in
__add_to_page_cache_locked().  These two separated charge functions were
replaced by a single mem_cgroup_charge().  However, it forgot to add a
matching mem_cgroup_uncharge() when the xarray insertion failed with the
page released back to the pool.

Fix this by adding a mem_cgroup_uncharge() call when insertion error

9d3af4b448 mm: Pass 'address' to map to do_set_pte() and drop FAULT_FLAG_PREFAULT
Rather than modifying the 'address' field of the 'struct vm_fault'
passed to do_set_pte(), leave that to identify the real faulting address
and pass in the virtual address to be mapped by the new pte as a
separate argument.

This makes FAULT_FLAG_PREFAULT redundant, as a prefault entry can be
identified simply by comparing the new address parameter with the
faulting address, so remove the redundant flag at the same time.

46bdb4277f mm: Allow architectures to request 'old' entries when prefaulting
Commit 5c0a85fad9 ("mm: make faultaround produce old ptes") changed
the "faultaround" behaviour to initialise prefaulted PTEs as 'old',
since this avoids vmscan wrongly assuming that they are hot, despite
having never been explicitly accessed by userspace. The change has been
shown to benefit numerous arm64 micro-architectures (with hardware
access flag) running Android, where both application launch latency and
direct reclaim time are significantly reduced (by 10%+ and ~80%
respectively).

Unfortunately, commit 315d09bf30 ("Revert "mm: make faultaround
produce old ptes"") reverted the change due to it being identified as
the cause of a ~6% regression in unixbench on x86. Experiments on a
variety of recent arm64 micro-architectures indicate that unixbench is
not affected by the original commit, which appears to yield a 0-1%
performance improvement.

Since one size does not fit all for the initial state of prefaulted
PTEs, introduce arch_wants_old_prefaulted_pte(), which allows an
architecture to opt-in to 'old' prefaulted PTEs at runtime based on
whatever criteria it may have.

f9ce0be71d mm: Cleanup faultaround and finish_fault() codepaths
alloc_set_pte() has two users with different requirements: in the
faultaround code, it called from an atomic context and PTE page table
has to be preallocated. finish_fault() can sleep and allocate page table
as needed.

PTL locking rules are also strange, hard to follow and overkill for
finish_fault().

Let's untangle the mess. alloc_set_pte() has gone now. All locking is
explicit.

The price is some code duplication to handle huge pages in faultaround
path, but it should be fine, having overall improvement in readability.

3644e2d2dd mm/filemap: fix infinite loop in generic_file_buffered_read()
If iter->count is 0 and iocb->ki_pos is page aligned, this causes
nr_pages to be 0.

Then in generic_file_buffered_read_get_pages() find_get_pages_contig()
returns 0 - because we asked for 0 pages, so we call
generic_file_buffered_read_no_cached_page() which attempts to add a page
to the page cache, which fails with -EEXIST, and then we loop. Oops...

15b4473617 mm/lru: revise the comments of lru_lock
Since we changed the pgdat->lru_lock to lruvec->lru_lock, it's time to fix
the incorrect comments in code.  Also fixed some zone->lru_lock comment
error from ancient time.  etc.

I struggled to understand the comment above move_pages_to_lru() (surely
it never calls page_referenced()), and eventually realized that most of
it had got separated from shrink_active_list(): move that comment back.

b8eddff888 mm: memcontrol: add file_thp, shmem_thp to memory.stat
As huge page usage in the page cache and for shmem files proliferates in
our production environment, the performance monitoring team has asked for
per-cgroup stats on those pages.

We already track and export anon_thp per cgroup.  We already track file
THP and shmem THP per node, so making them per-cgroup is only a matter of
switching from node to lruvec counters.  All callsites are in places where
the pages are charged and locked, so page->memcg is stable.

06c0444290 mm/filemap.c: generic_file_buffered_read() now uses find_get_pages_contig
Convert generic_file_buffered_read() to get pages to read from in batches,
and then copy data to userspace from many pages at once - in particular,
we now don't touch any cachelines that might be contended while we're in
the loop to copy data to userspace.

This is is a performance improvement on workloads that do buffered reads
with large blocksizes, and a very large performance improvement if that
file is also being accessed concurrently by different threads.

On smaller reads (512 bytes), there's a very small performance improvement
(1%, within the margin of error).

723ef24b9b mm/filemap/c: break generic_file_buffered_read up into multiple functions
Patch series "generic_file_buffered_read() improvements", v2.

generic_file_buffered_read() has turned into a real monstrosity to work
with.  And it's a major performance improvement, for both small random and
large sequential reads.  On my test box, 4k buffered random reads go from
~150k to ~250k iops, and the improvements to big sequential reads are even
bigger.

This incorporates the fix for IOCB_WAITQ handling that Jens just posted as
well, also factors out lock_page_for_iocb() to improve handling of the
various iocb flags.

This is prep work for changing generic_file_buffered_read() to use
find_get_pages_contig() to batch up all the pagecache lookups.

This patch should be functionally identical to the existing code and
changes as little as of the flow control as possible.  More refactoring
could be done, this patch is intended to be relatively minimal.

Twice now, when exercising ext4 looped on shmem huge pages, I have crashed
on the PF_ONLY_HEAD check inside PageWaiters(): ext4_finish_bio() calling
end_page_writeback() calling wake_up_page() on tail of a shmem huge page,
no longer an ext4 page at all.

The problem is that PageWriteback is not accompanied by a page reference
(as the NOTE at the end of test_clear_page_writeback() acknowledges): as
soon as TestClearPageWriteback has been done, that page could be removed
from page cache, freed, and reused for something else by the time that
wake_up_page() is reached.

Matthew Wilcox suggested avoiding or weakening the PageWaiters() tail
check; but I'm paranoid about even looking at an unreferenced struct page,
lest its memory might itself have already been reused or hotremoved (and
wake_up_page_bit() may modify that memory with its ClearPageWaiters()).

Then on crashing a second time, realized there's a stronger reason against
that approach.  If my testing just occasionally crashes on that check,
when the page is reused for part of a compound page, wouldn't it be much
more common for the page to get reused as an order-0 page before reaching
wake_up_page()?  And on rare occasions, might that reused page already be
marked PageWriteback by its new user, and already be waited upon?  What
would that look like?

It would look like BUG_ON(PageWriteback) after wait_on_page_writeback()
in write_cache_pages() (though I have never seen that crash myself).

And prior to 2a9127fcf2 ("mm: rewrite wait_on_page_bit_common() logic")
this would have been much less likely: before that, wake_page_function()'s
non-exclusive case would stop walking and not wake if it found Writeback
already set again; whereas now the non-exclusive case proceeds to wake.

I have not thought of a fix that does not add a little overhead: the
simplest fix is for end_page_writeback() to get_page() before calling
test_clear_page_writeback(), then put_page() after wake_up_page().

Was there a chance of missed wakeups before, since a page freed before
reaching wake_up_page() would have PageWaiters cleared?  I think not,
because each waiter does hold a reference on the page.  This bug comes
when the old use of the page, the one we do TestClearPageWriteback on,
had *no* waiters, so no additional page reference beyond the page cache
(and whoever racily freed it).  The reuse of the page has a waiter
holding a reference, and its own PageWriteback set; but the belated
wake_up_page() has woken the reuse to hit that BUG_ON(PageWriteback).

13bd691421 mm: mark async iocb read as NOWAIT once some data has been copied
Once we've copied some data for an iocb that is marked with IOCB_WAITQ,
we should no longer attempt to async lock a new page. Instead make sure
we return the copied amount, and let the caller retry, instead of
returning -EIOCBQUEUED for a new page.

This should only be possible with read-ahead disabled on the below
device, and multiple threads racing on the same file. Haven't been able
to reproduce on anything else.

There are a number of places in the VM which assume that a THP is a PMD in
size.  That's true today, and remains true after this patch series, but
this is a prerequisite for switching to arbitrary-sized THPs.
thp_nr_pages() still returns either HPAGE_PMD_NR or 1, but will be changed

This is hard to reproduce with mainline, but happens regularly with the
THP patchset (as so many more THPs are created).  This solution is take
from the THP patchset.  It splits the shadow entry into order-0 pieces at
the time that we bring a new page into cache.

stable(ish) part of the merge window to rearrange code.

27a83a609b mm/filemap: fix filemap_map_pages for THP
We dereference page->mapping and page->index directly after calling
find_subpage() and these fields are not valid for tail pages.  While
commit 4101196b19 ("mm: page cache: store only head pages in i_pages")
introduced the call to find_subpage(), the problem existed prior to this;
I'm going to suggest all the way back to when THPs first existed.

The user-visible effects of this are almost negligible.  To hit it, you
have to mmap a tmpfs file at an unaligned address and then it's only a
disabled optimisation causing page faults to happen more frequently than
they otherwise would.

Fix this by keeping both head and page pointers and checking the
appropriate one.  We could use page_mapping() and page_to_index(), but
that's higher overhead.

c8d317aa18 io_uring: fix async buffered reads when readahead is disabled
The async buffered reads feature is not working when readahead is
turned off. There are two things to concern:

- when doing retry in io_read, not only the IOCB_WAITQ flag but also
  the IOCB_NOWAIT flag is still set, which makes it goes to would_block
  phase in generic_file_buffered_read() and then return -EAGAIN. After
  that, the io-wq thread work is queued, and later doing the async
  reads in the old way.

- even if we remove IOCB_NOWAIT when doing retry, the feature is still
  not running properly, since in generic_file_buffered_read() it goes to
  lock_page_killable() after calling mapping->a_ops->readpage() to do
  IO, and thus causing process to sleep.

5ef64cc898 mm: allow a controlled amount of unfairness in the page lock
Commit 2a9127fcf2 ("mm: rewrite wait_on_page_bit_common() logic") made
the page locking entirely fair, in that if a waiter came in while the
lock was held, the lock would be transferred to the lockers strictly in
order.

That was intended to finally get rid of the long-reported watchdog
failures that involved the page lock under extreme load, where a process
could end up waiting essentially forever, as other page lockers stole
the lock from under it.

It also improved some benchmarks, but it ended up causing huge
performance regressions on others, simply because fair lock behavior
doesn't end up giving out the lock as aggressively, causing better
worst-case latency, but potentially much worse average latencies and
throughput.

Instead of reverting that change entirely, this introduces a controlled
amount of unfairness, with a sysctl knob to tune it if somebody needs
to.  But the default value should hopefully be good for any normal load,
allowing a few rounds of lock stealing, but enforcing the strict
ordering before the lock has been stolen too many times.

There is also a hint from Matthieu Baerts that the fair page coloring
may end up exposing an ABBA deadlock that is hidden by the usual
optimistic lock stealing, and while the unfairness doesn't fix the
fundamental issue (and I'm still looking at that), it avoids it in
practice.

The amount of unfairness can be modified by writing a new value to the
'sysctl_page_lock_unfairness' variable (default value of 5, exposed
through /proc/sys/vm/page_lock_unfairness), but that is hopefully
something we'd use mainly for debugging rather than being necessary for
any deep system tuning.

This whole issue has exposed just how critical the page lock can be, and
how contended it gets under certain locks.  And the main contention
doesn't really seem to be anything related to IO (which was the origin
of this lock), but for things like just verifying that the page file
mapping is stable while faulting in the page into a page table.

76cd61739f mm/error_inject: Fix allow_error_inject function signatures.
'static' and 'static noinline' function attributes make no guarantees that
gcc/clang won't optimize them. The compiler may decide to inline 'static'
function and in such case ALLOW_ERROR_INJECT becomes meaningless. The compiler
could have inlined __add_to_page_cache_locked() in one callsite and didn't
inline in another. In such case injecting errors into it would cause
unpredictable behavior. It's worse with 'static noinline' which won't be
inlined, but it still can be optimized. Like the compiler may decide to remove
one argument or constant propagate the value depending on the callsite.

To avoid such issues make sure that these functions are global noinline.

Since commit bbddabe2e4 ("mm: filemap: only do access activations on
reads"), mark_page_accessed() is called for reads only.  But the idle flag
is cleared by mark_page_accessed() so the idle flag won't get cleared if
the page is write accessed only.

Basically idle page tracking is used to estimate workingset size of
workload, noticeable size of workingset might be missed if the idle flag
is not maintained correctly.

It seems good enough to just clear idle flag for write operations.

2a9127fcf2 mm: rewrite wait_on_page_bit_common() logic
It turns out that wait_on_page_bit_common() had several problems,
ranging from just unfair behavioe due to re-queueing at the end of the
wait queue when re-trying, and an outright bug that could result in
missed wakeups (but probably never happened in practice).

This rewrites the whole logic to avoid both issues, by simply moving the
logic to check (and possibly take) the bit lock into the wakeup path
instead.

That makes everything much more straightforward, and means that we never
need to re-queue the wait entry: if we get woken up, we'll be notified
through WQ_FLAG_WOKEN, and the wait queue entry will have been removed,
and everything will have been done for us.

dd3e6d5039 mm: add support for async page locking
Normally waiting for a page to become unlocked, or locking the page,
requires waiting for IO to complete. Add support for lock_page_async()
and wait_on_page_locked_async(), which are callback based instead. This
allows a caller to get notified when a page becomes unlocked, rather
than wait for it.

We add a new iocb field, ki_waitq, to pass in the necessary data for this
to happen. We can unionize this with ki_cookie, since that is only used
for polled IO. Polled IO can never co-exist with async callbacks, as it is
(by definition) polled completions. struct wait_page_key is made public,
and we define struct wait_page_async as the interface between the caller
and the core.
