4daf1ef3c6 scsi: pm80xx: Convert 'long' mdelay to msleep
For delays longer than 20ms [um]delay isn't recommended.

pm80xx_chip_soft_rst starts off with a 500ms delay before it even gets
around to checking for the results of the reset. As long as it's at least
500ms it doesn't matter what the scheduler is doing.  The delay in the
pm8001_exec_internal_task_abort does nothing, and theory is this is a delay
to avoid a double-free.

ce21c63ee9 scsi: pm80xx: Fix for SATA device discovery
Driver was missing complete() call in mpi_sata_completion which result in
SATA abort error handling timing out. That causes the device to be left in
the in_recovery state so subsequent commands sent to the device fail and
the OS removes access to it.

c9c13ba428 PCI: Add PCI_STD_NUM_BARS for the number of standard BARs
Code that iterates over all standard PCI BARs typically uses
PCI_STD_RESOURCE_END.  However, that requires the unusual test
"i <= PCI_STD_RESOURCE_END" rather than something the typical
"i < PCI_STD_NUM_BARS".

Add a definition for PCI_STD_NUM_BARS and change loops to use the more
idiomatic C style to help avoid fencepost errors.

afcd609e8e scsi: pm80xx: remove redundant assignments to variable rc
There are several occasions where variable rc is being initialized with a
value that is never read and error is being re-assigned a little later on.
Clean up the code by removing rc entirely and just returning the return
value from the call to pm8001_issue_ssp_tmf

196ba6629c scsi: pm80xx: Fixed kernel panic during error recovery for SATA drive
Disabling the SATA drive interface cause kernel panic. When the drive
Interface is disabled, device should be deregistered after aborting all
pending I/Os. Also changed the port recovery timeout to 10000 ms for
PM8006 controller.

Many times in libsas, and in LLDDs which use libsas, the check for an
expander device is re-implemented or open coded.

Use dev_is_expander() instead. We rename this from
sas_dev_type_is_expander() to not spill so many lines in referencing.

2a3d4eb8e2 scsi: flip the default on use_clustering
Most SCSI drivers want to enable "clustering", that is merging of
segments so that they might span more than a single page.  Remove the
ENABLE_CLUSTERING define, and require drivers to explicitly set
DISABLE_CLUSTERING to disable this feature.

72349b62a5 scsi: pm80xx: Fixed system hang issue during kexec boot
When the firmware is not responding, execution of kexec boot causes a system
hang. When firmware assertion happened, driver get notified with interrupt
vector updated in MPI configuration table. Then, the driver will read
scratchpad register and set controller_fatal_error flag to true.

6396bb2215 treewide: kzalloc() -> kcalloc()
The kzalloc() function has a 2-factor argument form, kcalloc(). This
patch replaces cases of:

        kzalloc(a * b, gfp)

with:
        kcalloc(a * b, gfp)

as well as handling cases of:

        kzalloc(a * b * c, gfp)

with:

        kzalloc(array3_size(a, b, c), gfp)

as it's slightly less ugly than:

        kzalloc_array(array_size(a, b), c, gfp)

This does, however, attempt to ignore constant size factors like:

        kzalloc(4 * 1024, gfp)

though any constants defined via macros get caught up in the conversion.

Any factors with a sizeof() of "unsigned char", "char", and "u8" were
dropped, since they're redundant.

b24413180f License cleanup: add SPDX GPL-2.0 license identifier to files with no license
Many source files in the tree are missing licensing information, which
makes it harder for compliance tools to determine the correct license.

By default all files without license information are under the default
license of the kernel, which is GPL version 2.

Update the files which contain no license information with the 'GPL-2.0'
SPDX license identifier.  The SPDX identifier is a legally binding
shorthand, which can be used instead of the full boiler plate text.

This patch is based on work done by Thomas Gleixner and Kate Stewart and
Philippe Ombredanne.

How this work was done:

Patches were generated and checked against linux-4.14-rc6 for a subset of
the use cases:
 - file had no licensing information it it.
 - file was a */uapi/* one with no licensing information in it,
 - file was a */uapi/* one with existing licensing information,

Further patches will be generated in subsequent months to fix up cases
where non-standard license headers were used, and references to license
had to be inferred by heuristics based on keywords.

The analysis to determine which SPDX License Identifier to be applied to
a file was done in a spreadsheet of side by side results from of the
output of two independent scanners (ScanCode & Windriver) producing SPDX
tag:value files created by Philippe Ombredanne.  Philippe prepared the
base worksheet, and did an initial spot review of a few 1000 files.

The 4.13 kernel was the starting point of the analysis with 60,537 files
assessed.  Kate Stewart did a file by file comparison of the scanner
results in the spreadsheet to determine which SPDX license identifier(s)
to be applied to the file. She confirmed any determination that was not
immediately clear with lawyers working with the Linux Foundation.

Criteria used to select files for SPDX license identifier tagging was:
 - Files considered eligible had to be source code files.
 - Make and config files were included as candidates if they contained >5
   lines of source
 - File already had some variant of a license header in it (even if <5
   lines).

All documentation files were explicitly excluded.

The following heuristics were used to determine which SPDX license
identifiers to apply.

 - when both scanners couldn't find any license traces, file was
   considered to have no license information in it, and the top level
   COPYING file license applied.

   For non */uapi/* files that summary was:

 - when the two scanners agreed on the detected license(s), that became
   the concluded license(s).

 - when there was disagreement between the two scanners (one detected a
   license but the other didn't, or they both detected different
   licenses) a manual inspection of the file occurred.

 - In most cases a manual inspection of the information in the file
   resulted in a clear resolution of the license that should apply (and
   which scanner probably needed to revisit its heuristics).

 - When it was not immediately clear, the license identifier was
   confirmed with lawyers working with the Linux Foundation.

 - If there was any question as to the appropriate license identifier,
   the file was flagged for further research and to be revisited later
   in time.

In total, over 70 hours of logged manual review was done on the
spreadsheet to determine the SPDX license identifiers to apply to the
source files by Kate, Philippe, Thomas and, in some cases, confirmation
by lawyers working with the Linux Foundation.

Kate also obtained a third independent scan of the 4.13 code base from
FOSSology, and compared selected files where the other two scanners
disagreed against that SPDX file, to see if there was new insights.  The
Windriver scanner is based on an older version of FOSSology in part, so
they are related.

Thomas did random spot checks in about 500 files from the spreadsheets
for the uapi headers and agreed with SPDX license identifier in the
files he inspected. For the non-uapi files Thomas did random spot checks
in about 15000 files.

In initial set of patches against 4.14-rc6, 3 files were found to have
copy/paste license identifier errors, and have been fixed to reflect the
correct identifier.

Additionally Philippe spent 10 hours this week doing a detailed manual
inspection and review of the 12,461 patched files from the initial patch
version early this week with:
 - a full scancode scan run, collecting the matched texts, detected
   license ids and scores
 - reviewing anything where there was a license detected (about 500+
   files) to ensure that the applied SPDX license was correct
 - reviewing anything where there was no detection but the patch license
   was not GPL-2.0 WITH Linux-syscall-note to ensure that the applied
   SPDX license was correct

This produced a worksheet with 20 files needing minor correction.  This
worksheet was then exported into 3 different .csv files for the
different types of files to be modified.

These .csv files were then reviewed by Greg.  Thomas wrote a script to
parse the csv files and add the proper SPDX tag to the file, in the
format that the file expected.  This script was further refined by Greg
based on the output to detect more types of files automatically and to
distinguish between header and source .c files (which need different
comment types.)  Finally Greg ran the script using the .csv files to
generate the patches.

to pass the timer pointer explicitly. This requires adding a pointer to

0b6df110b3 scsi: pm80xx: panic on ncq error cleaning up the read log.
when there's an error in 'ncq mode' the host has to read the ncq error
log (10h) to clear the error state. however, the ccb that is setup for
doing this doesn't setup the ccb so that the previous state is
cleared. if the ccb was previously used for an IO n_elems is set and
pm8001_ccb_task_free() treats this as the signal to go free a
scatter-gather list (that's already been freed).

bc1371c181 scsi: pm8001: fix double free in pm8001_pci_probe
In function pm8001_pci_probe(), on errors that the control flow jumps to
label err_out_ha_free, function pm8001_free() is called. In pm8001_free(),
scsi_host_put() is called to release shost, which keeps the return value
of scsi_host_alloc(). After pm8001_free() returns, kfree() is called to
free shost again, resulting in a double free bug. This patch removes
scsi_host_put() from pm8001_free() and explicitly calls scsi_host_put()
to release Scsi_Host in need.

e83596b41c pm80xx: Remove bogus address masking in pm8001_ioremap()
It is unclear what the original intent of the masking was, but it is
clearly incorrect to truncate a physical address before calling
ioremap().  On systems where there are valid physical address bits above
bit-31 (arm64 for example) the result is an eventual OOPs when
initializing the driver.

Remove the bogus code to fix it.

This patch changes the !blk-mq path to the same defaults as the blk-mq
I/O path by always enabling block tagging, and always using host wide
tags.  We've had blk-mq available for a few releases so bugs with

2a188cb42b pm80xx: remove the SCSI host before detaching from SAS transport
Previously, when this module was unloaded via 'rmmod' with at least one
drive attached, the SCSI error handler thread would become stuck in an
infinite recovery loop and lockup the system, necessitating a reboot.

Once the SAS layer is detached, the driver will fail any subsequent
commands since the target devices are removed.  However, removing the
SCSI host generates a SYNCHRONIZE CACHE (10) command, which was failed
and left the error handler no method of recovery.

This patch simply removes the SCSI host first so that no more commands
can come down, prior to cleaning up the SAS layer.  Note that the stack
is built up with the SCSI host first, and then the SAS layer.  Perhaps
it should be reversed for symmetry, so that commands cannot be sent to
the pm80xx driver prior to attaching the SAS layer?

What was really strange about this bug was that it was introduced at
commit cff549e486 ("[SCSI]: proper state checking and module refcount
handling in scsi_device_get").  This commit appears to tinker with how
the reference counting is performed for SCSI device objects.  My theory
is that prior to this commit, the refcount for a device object was
blindly incremented at some point during the teardown process which
coincidentially made the device stick around during the procedure, which
also coincidentially made any commands sent to the driver not fail
(since the device was technically still "there").  After this commit was
applied, my theory is the refcount for the device object is not being
incremented at a specific point anymore, which makes the device go away,
and thus made the pm80xx driver fail any subsequent commands.

c913df3f3d pm80xx: avoid a panic if MSI(X) interrupts are disabled
If MSI(X) interrupts are disabled via the kernel command line
(pci=nomsi), the pm8001 driver will kernel panic because it does not
detect that MSI interrupts are disabled and will soldier on and attempt to
configure MSI interrupts anyways.  This leads to a kernel panic, most
likely because a required data structure is not available down the
line.  Using the pci_msi_enabled() function in order to detect if MSI
interrupts are enabled before configuring them resolves this issue and
avoids a kernel panic when the module is loaded.  Additionally, the
irq_vector structure must be initialized when legacy interrupts are
being used otherwise legacy interrupts will simply not function and
result in another panic.

b650a8806e pm80xx: wait a minimum of 500ms before issuing commands to SPCv
The documentation for the 8070 and 8072 SPCv chip explicitly states that
a minimum of 500ms must elapse before issuing commands, otherwise the
SPCv may not process them and the firmware may get into an unrecoverable
state requiring a reboot.  While the Linux guys will probably think this
is 'racy', it is called out in the chip documentation and inserting this
delay makes power management function properly.

da2dd6184b pm80xx: configure PHY settings based on subsystem vendor ID
Previuosly, all PMC Sierra 80xx controllers are assumed to be a
motherboard controller, except if the subsystem vendor ID was equal to
PCI_VENDOR_ID_ADAPTEC.  The driver then attempts to load PHY settings
from NVRAM.  While this may be correct behavior for most controllers, it
does not work with Adaptec and ATTO controllers since they do not store
PHY settings in NVRAM and choose to use either custom PHY settings or
chip defaults.  Loading random values from NVRAM may cause the
controllers to malfunction in this edge case.

8414cd8057 pm80xx: Add PORT RECOVERY TIMEOUT support
PORT RECOVERY TIMEOUT is the maximum time between the controller's
detection of the PHY down until the receipt of the ID_Frame (from the
same remote SAS port). If the time expires before the ID_FRAME is
received, the port is considered INVALID and can be removed. The
IOP_EVENT_PORT_RECOVERY_TIMER_TMO event is reported following the
IOP_EVENT_ PHY_DOWN event when the PHY/port does not recover after
Port Recovery Time.

3b700e3411 pm80xx: Fix for Incorrect DMA Unmapping of SG List
In pm8001_ccb_task_free(), the dma unmapping is done based on
ccb->n_elem value. This should be initialized to zero in the
task_abort(). Otherwise, pm8001_ccb_task_free() will try for
dma_unmap_sg() which is invalid for task abort and can lead to
kernel crash.

efc3c1df5f scsi: remove ->change_queue_type method
Since we got rid of ordered tag support in 2010 the prime use case of
switching on and off ordered tags has been obsolete.  The other function
of enabling/disabling tagging entirely has only been correctly implemented

79855d1785 libsas: remove task_collector mode
The task_collector mode (or "latency_injector", (C) Dan Willians) is an
optional I/O path in libsas that queues up scsi commands instead of
directly sending it to the hardware.  It generall increases latencies
to in the optiomal case slightly reduce mmio traffic to the hardware.

Only the obsolete aic94xx driver and the mvsas driver allowed to use
it without recompiling the kernel, and most drivers didn't support it
at all.

Remove the giant blob of code to allow better optimizations for scsi-mq
in the future.

c40ecc12cf scsi: avoid ->change_queue_depth indirection for queue full tracking
All drivers use the implementation for ramping the queue up and down, so
instead of overloading the change_queue_depth method call the
implementation diretly if the driver opts into it by setting the
track_queue_depth flag in the host template.

Note that a few drivers validated the new queue depth in their
change_queue_depth method, but as we never go over the queue depth
set during slave_configure or the sysfs file this isn't nessecary
and can safely be removed.

2ecb204d07 scsi: always assign block layer tags if enabled
Allow a driver to ask for block layer tags by setting .use_blk_tags in the
host template, in which case it will always see a valid value in
request->tag, similar to the behavior when using blk-mq.  This means even
SCSI "untagged" commands will now have a tag, which is especially useful
when using a host-wide tag map.

9f17609968 pm8001: Fix hibernation issue
During hibernation, the HBA firmware may lose power and forget the device
id info.   This causes the HBA to reject IO upon resume.   The fix is
to call the libsas power management routines to make the domain device
forgetful.

2b01d816f5 [SCSI] pm80xx: Spinlock fix
spin_lock_irqsave for the HBA lock is called in one function where flag
is local to that function. Another function is called from the first
function where lock has to be released using spin_unlock_irqrestore for
calling task_done of libsas. In the second function also flag is declared
and used. For calling task_done there is no need to enable the irq. So
instead of using spin_lock_irqsave and spin_unlock_irqrestore, spin_lock
and spin_unlock is used now. This also avoids passing the flags across all
the functions where HBA lock is being used. Also removed redundant code.

Setting the phy state for hard reset response.
After sending hard reset for a device ,phy down event sets
the phy state to zero but for phy up event it will not set
the phy state again.This will cause problem to successive

e1e819cc96 [SCSI] pm8001: clean up unnecessary MSI/MSI-X capability find
PCI core will initialize device MSI/MSI-X capability in
pci_msi_init_pci_dev(). So device driver should use
pci_dev->msi_cap/msix_cap to determine whether the device
support MSI/MSI-X instead of using
pci_find_capability(pci_dev, PCI_CAP_ID_MSI/MSIX).
Access to PCIe device config space again will consume more time.

9504a92392 [SCSI] pm80xx: fix Adaptec 71605H hang
The IO command size is 128 bytes for these new controllers as opposed to 64
for the old 8001 controller.

The Adaptec out-of-tree driver did this correctly. After comparing the two
this turned out to be the crucial difference.

So don't hardcode the IO command size, instead use pm8001_ha->iomb_size as
that is the correct value for both old and new controllers.

Remove the arbitrary expectation in libsas that all SCSI commands are 16 bytes
or less.  Instead do all copies via cmd->cmd_len (and use a pointer to this in
the libsas task instead of a copy).  Note that this still doesn't enable > 16
byte CDB support in the underlying drivers because their internal format has
to be fixed and the wire format of > 16 byte CDBs according to the SAS spec is
different.  the libsas drivers (isci, aic94xx, mvsas and pm8xxx are all
updated for this change.

aa9f8328fc [SCSI] sas: unify the pointlessly separated enums sas_dev_type and sas_device_type
These enums have been separate since the dawn of SAS, mainly because the
latter is a procotol only enum and the former includes additional state
for libsas.  The dichotomy causes endless confusion about which one you
should use where and leads to pointless warnings like this:

drivers/scsi/mvsas/mv_sas.c: In function 'mvs_update_phyinfo':
drivers/scsi/mvsas/mv_sas.c:1162:34: warning: comparison between 'enum sas_device_type' and 'enum sas_dev_type' [-Wenum-compare]

Fix by eliminating one of them.  The one kept is effectively the sas.h
one, but call it sas_device_type and make sure the enums are all
properly namespaced with the SAS_ prefix.

Performing pci_free_consistent in tasklet had result in a core dump. So
allocated a new memory region for it. Fix for passing proper address
and operation in firmware flash update.

This is a followup to a patch provided by Jack Wang on September 21 2011.

After increasing the CAN_QUEUE to 510 in pm8001 we discovered some performance
degredation from time to time. We needed to increase the MPI queue to
compensate and ensure we never hit that limit. We also needed to double
the margin to support event and administrivial commands that take from
the pool resulting in an occasional largely unproductive command completion
with soft error to the caller when the command pool is overloaded temporarily.

There is a possble racing scenario.

'process_oq' is called by two routines, as shown below.

process_oq() --> process_one_iomb() --> mpi_sata_completion()

In 'mpi_sata_completion', "pm8001_ha->lock" is first released.
It means lock is taken before,  which is true for
the context A, as 'pm8001_ha->lock' is taken in 'pm8001_chip_isr()'

But for context B there is no lock taken before and pm8001_ha->lock
is unlocked in 'mpi_sata_completion()'. This may unlock the lock
taken in context A. Possible racing ??

If 'pm8001_ha->lock' is taken in 'process_oq()' instead of
'pm8001_chip_isr' then the above issue can be avoided.

Static checker is giving following warning:

The code flow is as shown below:
process_oq() --> process_one_iomb --> mpi_sata_completion

In 'mpi_sata_completion'
the first call for 'spin_unlock_irqrestore()' is with flags=0,
which is as good as 'spin_unlock_irq()' ( unconditional interrupt
enabling).

So for better performance 'spin_unlock_irqrestore()' can be replaced
with 'spin_unlock_irq()' and 'spin_lock_irqsave()' can be replaced by
'spin_lock_irq()'.

libsas ata error handling is already async but this does not help the
scan case.  Move initial link recovery out from under host->scan_mutex,
and delay synchronization with eh until after all port probe/recovery
work has been queued.

Device ordering is maintained with scan order by still calling
sas_rphy_add() in order of domain discovery.

Since we now scan the domain list when invoking libata-eh we need to be
careful to check for fully initialized ata ports.

f41a0c441c [SCSI] libsas: fix sas_find_local_phy(), take phy references
In the direct-attached case this routine returns the phy on which this
device was first discovered.  Which is broken if we want to support
wide-targets, as this phy reference can become stale even though the
port is still active.

In the expander-attached case this routine tries to lookup the phy by
scanning the attached sas addresses of the parent expander, and BUG_ONs
if it can't find it.  However since eh and the libsas workqueue run
independently we can still be attempting device recovery via eh after
libsas has recorded the device as detached.  This is even easier to hit
now that eh is blocked while device domain rediscovery takes place, and
that libata is fed more timed out commands increasing the chances that
it will try to recover the ata device.

Arrange for dev->phy to always point to a last known good phy, it may be
stale after the port is torn down, but it will catch up for wide port
reconfigurations, and never be NULL.

Each libsas driver (mvsas, pm8001, and isci) has invented a different
method for managing the ap->lock.  The lock is held by the ata
->queuecommand() path.  mvsas drops it prior to acquiring any internal
locks which allows it to hold its internal lock across calls to
task->task_done().  This capability is important as it is the only way
the driver can flush task->task_done() instances to guarantee that it no
longer has any in-flight references to a domain_device at
->lldd_dev_gone() time.

When an lldd invokes ->notify_port_event() it can trigger a chain of libsas
events to:

  1/ form the port and find the direct attached device

  2/ if the attached device is an expander perform domain discovery

A call to flush_workqueue() will only flush the initial port formation work.
Currently libsas users need to call scsi_flush_work() up to the max depth of
chain (which will grow from 2 to 3 when ata discovery is moved to its own
discovery event).  Instead of open coding multiple calls switch to use
drain_workqueue() to flush sas work.

drain_workqueue() does not handle new work submitted during the drain so
libsas needs a bit of infrastructure to hold off unchained work submissions
while a drain is in flight.  A lldd ->notify() event is considered 'unchained'
while a sas_discover_event() is 'chained'.  As Tejun notes:

  "For now, I think it would be best to add private wrapper in libsas to
   support deferring unchained work items while draining."

5954d7380f [SCSI] pm8001: deficient responses to IO_XFER_ERROR_BREAK and IO_XFER_OPEN_RETRY_TIMEOUT
IO_XFER_ERROR_BREAK and IO_XFER_OPEN_RETRY_TIMEOUT are deficient of the
required actions as outlined in the programming manual for the pm8001. Due to
the overlapping code requirements of these recovery responses, we found it
necessary to bundle them together into one patch.

When a break is received during the command phase (ssp_completion), this is a
result of a timeout or interruption on the bus. Logic suggests that we should
retry the command.

When a break is received during the data-phase (ssp_event), the task must be
aborted on the target or it will retain a data-phase lock turning the target
reticent to all future media commands yet will successfully respond to TUR,
INQUIRY and ABORT leading eventually to target failure through several
abort-cycle loops.

The open retry interval is exceedingly short resulting in occasional target
drop-off during expander resets or when targets push-back during bad-block
remapping. Increased effective timeout from 130ms to 1.5 seconds for each try
so as to trigger after the administrative inquiry/tur timeout in the scsi
subsystem to keep error-recovery harmonics to a minimum.

When an open retry timeout event is received, the action required by the
targets is to issue an abort for the outstanding command then logic suggests
we retry the command as this state is usually an indication of a credit block
or busy condition on the target.

We hijacked the pm8001_handle_event work queue handler so that it will handle
task as an argument instead of device for the workers in support of the
deferred handling outlined above.

Moderate to Heavy bad-path testing on a 2.6.32 vintage kernel, compile-testing
on scsi-misc-2.6 kernel ...

d95d00016f [SCSI] pm8001: Add FUNC_GET_EVENTS
Jack noticed I dropped a patch fragment associated with a flags automatic
variable in mpi_set_phys_g3_with_ssc (ooops) and that the pre-emptive locking
that piggy-backed this patch was not in-fact necessary because of underlying
atomic accesses to the hardware. Here is the updated patch fixing these two
issues.

The pm8001 driver is missing the FUNC_GET_EVENTS handler in the phy control
function. Since the pm8001_bar4_shift function was not designed to be called
at runtime, added locking surrounding the adjustment for all accesses.

5c4fb76af3 [SCSI] pm8001: fix lockup on phy_control hard reset.
pm8001_phy_control PHY_FUNC_HARD_RESET locks up on second try via
smp_phy_control because response HW_EVENT_PHY_START_STATUS fails to complete
previous command. The PM8001F_RUN_TIME flag is not treated as a bit, but a
state in all readers, yet once we are operational or in the run time state,
the flags use a bit-set operation.

ac013ed1cb [SCSI] isci: export phy events via ->lldd_control_phy()
Allow the sas-transport-class to update events for local phys via a new
PHY_FUNC_GET_EVENTS command to ->lldd_control_phy().  Fixup drivers that
are not prepared for new enum phy_func values, and unify
->lldd_control_phy() error codes.

These are the SAS defined phy events that are reported in a
smp-report-phy-error-log command:
 * /sys/class/sas_phy/<phyX>/invalid_dword_count
 * /sys/class/sas_phy/<phyX>/running_disparity_error_count
 * /sys/class/sas_phy/<phyX>/loss_of_dword_sync_count
 * /sys/class/sas_phy/<phyX>/phy_reset_problem_count

6fbc769219 [SCSI] pm8001: missing break statements
Code Inspection: found two missing break directives. First one will
result in not retrying an a task that report
IO_OPEN_CNX_ERROR_HW_RESOURCE_BUSY, the second will result in cosmetic
debug printk conflicting statement stutter. Because checkpatch.pl came
up with a warning regarding unnecessary space before a newline on one of
the fragments associated with the diff context, I took the liberty of
fixing all the cases of this issue in the pair of files touched by this
defect. These cosmetic changes hide the break changes :-(

To help focus, break changes are in pm8001_hwi.c fragment line 1649 for
the IO_OPEN_CNX_ERROR_HW_RESOURCE_BUSY case statement and pm8001_sas.c
line 1000 deals with the conflicting debug print stutter.

On the pm8001, when a device is in the process of going away (device
power off or hot plug), depending on the timing, the driver would return
SAS_PHY_DOWN as the return value to the queuecommand DEV_IS_GONE logic.
The net result is an near infinite retry (especially if SAS debugging is
enabled), the logs will fill with:

429305e465 [SCSI] pm8001: simplify workqueue usage
pm8001 manages its own list of pending works and cancel them on device
free.  It is unnecessarily complex and has a race condition - the
works are canceled but not synced, so the work could still be running
during and after the data structures are freed.

This patch simplifies workqueue usage.

* A driver specific workqueue pm8001_wq is created to serve these
  work items.

* To avoid confusion, the "queue" suffixes are dropped from work items
  and functions.

* Delayed queueing was never used.  pm8001_work now uses work_struct
  instead.

* The driver no longer keeps track of pending works.  All pm8001_works
  are queued to pm8001_wq and the workqueue is flushed as necessary.

flush_scheduled_work() usage is removed during conversion.

df64d3caab [SCSI] Unify SAM_ and SAM_STAT_ macros
We have two separate definitions for identical constants with nearly the
same name.  One comes from the generic headers in scsi.h; the other is
an enum in libsas.h ... it's causing confusion about which one is
correct (fortunately they both are).

Fix this by eliminating the libsas.h duplicate

2471b89406 [SCSI] pm8001: potential null dereference in pm8001_dev_gone_notify()
In the original code we dereferenced "pm8001_dev" before checking if it
was null.  This patch moves the dereference inside the condition.

This was found by a static checker (smatch).  I looked, but I couldn't
tell if "pm8001_dev" dev was ever actually null.  The approach in this
patch seemed like the safest response.

5a0e3ad6af include cleanup: Update gfp.h and slab.h includes to prepare for breaking implicit slab.h inclusion from percpu.h
percpu.h is included by sched.h and module.h and thus ends up being
included when building most .c files.  percpu.h includes slab.h which
in turn includes gfp.h making everything defined by the two files
universally available and complicating inclusion dependencies.

percpu.h -> slab.h dependency is about to be removed.  Prepare for
this change by updating users of gfp and slab facilities include those
headers directly instead of assuming availability.  As this conversion
needs to touch large number of source files, the following script is
used as the basis of conversion.

The script does the followings.

* Scan files for gfp and slab usages and update includes such that
  only the necessary includes are there.  ie. if only gfp is used,
  gfp.h, if slab is used, slab.h.

* When the script inserts a new include, it looks at the include
  blocks and try to put the new include such that its order conforms
  to its surrounding.  It's put in the include block which contains
  core kernel includes, in the same order that the rest are ordered -
  alphabetical, Christmas tree, rev-Xmas-tree or at the end if there
  doesn't seem to be any matching order.

* If the script can't find a place to put a new include (mostly
  because the file doesn't have fitting include block), it prints out
  an error message indicating which .h file needs to be added to the
  file.

The conversion was done in the following steps.

1. The initial automatic conversion of all .c files updated slightly
   over 4000 files, deleting around 700 includes and adding ~480 gfp.h
   and ~3000 slab.h inclusions.  The script emitted errors for ~400
   files.

2. Each error was manually checked.  Some didn't need the inclusion,
   some needed manual addition while adding it to implementation .h or
   embedding .c file was more appropriate for others.  This step added
   inclusions to around 150 files.

3. The script was run again and the output was compared to the edits
   from #2 to make sure no file was left behind.

4. Several build tests were done and a couple of problems were fixed.
   e.g. lib/decompress_*.c used malloc/free() wrappers around slab
   APIs requiring slab.h to be added manually.

5. The script was run on all .h files but without automatically
   editing them as sprinkling gfp.h and slab.h inclusions around .h
   files could easily lead to inclusion dependency hell.  Most gfp.h
   inclusion directives were ignored as stuff from gfp.h was usually
   wildly available and often used in preprocessor macros.  Each
   slab.h inclusion directive was examined and added manually as
   necessary.

6. percpu.h was updated not to include slab.h.

7. Build test were done on the following configurations and failures
   were fixed.  CONFIG_GCOV_KERNEL was turned off for all tests (as my
   distributed build env didn't work with gcov compiles) and a few
   more options had to be turned off depending on archs to make things
   build (like ipr on powerpc/64 which failed due to missing writeq).

   * x86 and x86_64 UP and SMP allmodconfig and a custom test config.
   * powerpc and powerpc64 SMP allmodconfig
   * sparc and sparc64 SMP allmodconfig
   * ia64 SMP allmodconfig
   * s390 SMP allmodconfig
   * alpha SMP allmodconfig
   * um on x86_64 SMP allmodconfig

8. percpu.h modifications were reverted so that it could be applied as
   a separate patch and serve as bisection point.

Given the fact that I had only a couple of failures from tests on step
6, I'm fairly confident about the coverage of this conversion patch.
If there is a breakage, it's likely to be something in one of the arch
headers which should be easily discoverable easily on most builds of
the specific arch.
