I've just discovered that the useful-sounding has_transparent_hugepage()
is actually an architecture-dependent minefield: on some arches it only
builds if CONFIG_TRANSPARENT_HUGEPAGE=y, on others it's also there when
not, but on some of those (arm and arm64) it then gives the wrong
answer; and on mips alone it's marked __init, which would crash if
called later (but so far it has not been called later).
Straighten this out: make it available to all configs, with a sensible
default in asm-generic/pgtable.h, removing its definitions from those
arches (arc, arm, arm64, sparc, tile) which are served by the default,
adding #define has_transparent_hugepage has_transparent_hugepage to
those (mips, powerpc, s390, x86) which need to override the default at
runtime, and removing the __init from mips (but maybe that kind of code
should be avoided after init: set a static variable the first time it's
called).

Performing an MTHC0 instruction without XPA being present will trigger a
reserved instruction exception, therefore conditionalise the use of this
instruction when building TLB handlers (build_update_entries()), and in
__update_tlb().
This allows an XPA kernel to run on non XPA hardware without that
instruction implemented, just like it can run on XPA capable hardware
without XPA in use (with the noxpa kernel argument) or with XPA not
configured in hardware.

The XPA case in iPTE_SW or's in software mode bits to the pte_low value
(which is what actually ends up in the high 32 bits of EntryLo...). It
does this presuming that only bits in the upper 16 bits of the 32 bit
pte_low value will be set. Make this assumption explicit with a BUG_ON.
A similar assumption is made for the hardware mode bits, which are or'd
in with a single ori instruction. Make that assumption explicit with a

Rather than hardcode a scratch register for the XPA case in iPTE_SW,
pass one through from the work registers allocated by the caller. This
allows for the XPA path to function correctly regardless of the work
registers in use.
Without doing this there are cases (where KScratch registers are
unavailable) in which iPTE_SW will incorrectly clobber $1 despite it
already being in use for the PTE or PTE pointer.

For XPA kernels build_update_entries() uses $1 (at) as a scratch
register, but doesn't arrange for it to be preserved, so it will always
be clobbered by the TLB refill exception. Although this register
normally has a very short lifetime that doesn't cross memory accesses,
TLB refills due to instruction fetches (either on a page boundary or
after preemption) could clobber live data, and its easy to reproduce
the clobber with a little bit of assembler code.
Note that the use of a hardware page table walker will partly mask the
problem, as the TLB refill handler will not always be invoked.
This is fixed by avoiding the use of the extra scratch register. The
pte_high parts (going into the lower half of the EntryLo registers) are
loaded and manipulated separately so as to keep the PTE pointer around
for the other halves (instead of storing in the scratch register), and
the pte_low parts (going into the high half of the EntryLo registers)
are masked with 0x00ffffff using an ext instruction (instead of loading
0x00ffffff into the scratch register and AND'ing).

There are 2 distinct cases in which a kernel for a MIPS32 CPU
(CONFIG_CPU_MIPS32=y) may use 64 bit physical addresses
(CONFIG_PHYS_ADDR_T_64BIT=y):
- 36 bit physical addressing as used by RMI Alchemy & Netlogic XLP/XLR
CPUs.
- MIPS32r5 eXtended Physical Addressing (XPA).
These 2 cases are distinct in that they require different behaviour from
the kernel - the EntryLo registers have different formats. Until Linux
v4.1 we only supported the first case, with code conditional upon the 2
aforementioned Kconfig variables being set. Commit c5b367835c ("MIPS:
Add support for XPA.") added support for the second case, but did so by
modifying the code that existed for the first case rather than treating
the 2 cases as distinct. Since the EntryLo registers have different
formats this breaks the 36 bit Alchemy/XLP/XLR case. Fix this by
splitting the 2 cases, with XPA cases now being conditional upon
CONFIG_XPA and the non-XPA case matching the code as it existed prior to
commit c5b367835c ("MIPS: Add support for XPA.").

("MIPS: Implement Read Inhibit/eXecute Inhibit") we've had a mixture of
_PAGE_READ & _PAGE_NO_READ bits. Rather than keep both around, switch
away from using _PAGE_READ to determine page presence & instead invert
the use to _PAGE_NO_READ. Wherever we formerly had no definition for
_PAGE_NO_READ, change what was _PAGE_READ to _PAGE_NO_READ. The end
result is that we consistently use _PAGE_NO_READ to determine whether a
page is readable, regardless of whether RI/XI is implemented.

kernels where XPA couldn't be enabled (either nohtw or the hardware

was only set to 8 bytes (an extra shift of 1) in config_htw_params() if
PageGrain.ELPA (enable large physical addressing) is set. On an XPA
kernel though the size of PTEs is fixed at 8 bytes regardless of whether
XPA could actually be enabled.

New Loongson 3 CPU (since Loongson-3A R2, as opposed to Loongson-3A R1,
Loongson-3B R1 and Loongson-3B R2) has many enhancements, such as FTLB,
L1-VCache, EI/DI/Wait/Prefetch instruction, DSP/DSPv2 ASE, User Local
register, Read-Inhibit/Execute-Inhibit, SFB (Store Fill Buffer), Fast
TLB refill support, etc.
This patch introduce a config option, CONFIG_LOONGSON3_ENHANCEMENT, to
enable those enhancements which are not probed at run time. If you want
a generic kernel to run on all Loongson 3 machines, please say 'N'
here. If you want a high-performance kernel to run on new Loongson 3
machines only, please say 'Y' here.
Some additional explanations:
1) SFB locates between core and L1 cache, it causes memory access out
of order, so writel/outl (and other similar functions) need a I/O
reorder barrier.
2) Loongson 3 has a bug that di instruction can not save the irqflag,
so arch_local_irq_save() is modified. Since CPU_MIPSR2 is selected
by CONFIG_LOONGSON3_ENHANCEMENT, generic kernel doesn't use ei/di
at all.
3) CPU_HAS_PREFETCH is selected by CONFIG_LOONGSON3_ENHANCEMENT, so
MIPS_CPU_PREFETCH (used by uasm) probing is also put in this patch.

Loongson-3 maintains cache coherency by hardware, this means:
1) It's icache is coherent with dcache.
2) It's dcaches don't alias (maybe depend on PAGE_SIZE).
3) It maintains cache coherency across cores (and for DMA).
So we can skip most cache flush operations by setting relevant handlers
to `cache_noop' in `r4k_cache_init'.

It's possible for pages to become visible prior to update_mmu_cache
running if a thread within the same address space preempts the current
thread or runs simultaneously on another CPU. That is, the following
scenario is possible:
CPU0 CPU1
write to page
flush_dcache_page
flush_icache_page
set_pte_at
map page
update_mmu_cache
If CPU1 maps the page in between CPU0's set_pte_at, which marks it valid
& visible, and update_mmu_cache where the dcache flush occurs then CPU1s
icache will fill from stale data (unless it fills from the dcache, in
which case all is good, but most MIPS CPUs don't have this property).
Commit 4d46a67a3e ("MIPS: Fix race condition in lazy cache flushing.")
attempted to fix that by performing the dcache flush in
flush_icache_page such that it occurs before the set_pte_at call makes
the page visible. However it has the problem that not all code that
writes to pages exposed to userland call flush_icache_page. There are
many callers of set_pte_at under mm/ and only 2 of them do call
flush_icache_page. Thus the race window between a page becoming visible
& being coherent between the icache & dcache remains open in some cases.
To illustrate some of the cases, a WARN was added to __update_cache with
this patch applied that triggered in cases where a page about to be
flushed from the dcache was not the last page provided to
flush_icache_page. That is, backtraces were obtained for cases in which
the race window is left open without this patch. The 2 standout examples
follow.
When forking a process:

[ 15.293747] [<80413808>] handle_sys+0x128/0x14c
When exec'ing an ELF binary:
[ 14.445964] [<80417630>] __update_cache+0xcc/0x188
[ 14.451369] [<80538d88>] move_page_tables+0x414/0x498

[ 14.485137] [<80413808>] handle_sys+0x128/0x14c
These code paths write into a page, call flush_dcache_page then call
set_pte_at without flush_icache_page inbetween. The end result is that
the icache can become corrupted & userland processes may execute
unexpected or invalid code, typically resulting in a reserved
instruction exception, a trap or a segfault.
Fix this race condition fully by performing any cache maintenance
required to keep the icache & dcache in sync in set_pte_at, before the
page is made valid. This has the added bonus of ensuring the cache
maintenance always happens in one location, rather than being duplicated
in flush_icache_page & update_mmu_cache. It also matches the way other
architectures solve the same problem (see arm, ia64 & powerpc).

them by mapping them in for the duration of the cache maintenance, just
like in __flush_dcache_page. The code for that isn't shared because we
need the page address in __update_cache so sharing became messy. Given
that the entirity is an extra 5 lines, just duplicate it.

When flush_dcache_page is called on an executable page, that page is
about to be provided to userland & we can presume that the icache
contains no valid entries for its address range. However if the icache
does not fill from the dcache then we cannot presume that the pages
content has been written back as far as the memories that the dcache
will fill from (ie. L2 or further out).
This was being done for lowmem pages, but not for highmem which can lead
to icache corruption. Fix this by mapping highmem pages & flushing their
content from the dcache in __flush_dcache_page before providing the page
to userland, just as is done for lowmem pages.

Coherence Manager 3 (CM3) as present in I6400 can fill icache lines
effectively from dirty dcaches, so there is no need to flush dirty lines

Set the MIPS_CACHE_IC_F_DC flag such that cpu_has_ic_fills_f_dc

It is still necessary to handle icache coherency in flush_cache_range()
and copy_to_user_page() when the icache fills from the dcache, even
though the dcache does not need to be written back. However when this
handling was added in commit 2eaa7ec286 ("[MIPS] Handle I-cache
coherency in flush_cache_range()"), it did not do any icache flushing
when it fills from dcache.
Therefore fix r4k_flush_cache_range() to run
local_r4k_flush_cache_range() without taking into account whether icache
fills from dcache, so that the icache coherency gets handled. Checks are
also added in local_r4k_flush_cache_range() so that the dcache blast
doesn't take place when icache fills from dcache.
A test to mmap a page PROT_READ|PROT_WRITE, modify code in it, and
mprotect it to VM_READ|VM_EXEC (similar to case described in above
commit) can hit this case quite easily to verify the fix.
A similar check was added in commit f8829caee3 ("[MIPS] Fix aliasing
bug in copy_to_user_page / copy_from_user_page"), so also fix
copy_to_user_page() similarly, to call flush_cache_page() without taking
into account whether icache fills from dcache, since flush_cache_page()
already takes that into account to avoid performing a dcache flush.

Pull x86 protection key support from Ingo Molnar:
"This tree adds support for a new memory protection hardware feature
that is available in upcoming Intel CPUs: 'protection keys' (pkeys).
There's a background article at LWN.net:
https://lwn.net/Articles/643797/
The gist is that protection keys allow the encoding of
user-controllable permission masks in the pte. So instead of having a
fixed protection mask in the pte (which needs a system call to change
and works on a per page basis), the user can map a (handful of)
protection mask variants and can change the masks runtime relatively
cheaply, without having to change every single page in the affected
virtual memory range.
This allows the dynamic switching of the protection bits of large
amounts of virtual memory, via user-space instructions. It also
allows more precise control of MMU permission bits: for example the
executable bit is separate from the read bit (see more about that
below).
This tree adds the MM infrastructure and low level x86 glue needed for
that, plus it adds a high level API to make use of protection keys -
if a user-space application calls:
mmap(..., PROT_EXEC);
or
mprotect(ptr, sz, PROT_EXEC);
(note PROT_EXEC-only, without PROT_READ/WRITE), the kernel will notice
this special case, and will set a special protection key on this
memory range. It also sets the appropriate bits in the Protection
Keys User Rights (PKRU) register so that the memory becomes unreadable
and unwritable.
So using protection keys the kernel is able to implement 'true'
PROT_EXEC on x86 CPUs: without protection keys PROT_EXEC implies
PROT_READ as well. Unreadable executable mappings have security
advantages: they cannot be read via information leaks to figure out
ASLR details, nor can they be scanned for ROP gadgets - and they
cannot be used by exploits for data purposes either.
We know about no user-space code that relies on pure PROT_EXEC
mappings today, but binary loaders could start making use of this new
feature to map binaries and libraries in a more secure fashion.
There is other pending pkeys work that offers more high level system
call APIs to manage protection keys - but those are not part of this
pull request.
Right now there's a Kconfig that controls this feature
(CONFIG_X86_INTEL_MEMORY_PROTECTION_KEYS) that is default enabled
(like most x86 CPU feature enablement code that has no runtime
overhead), but it's not user-configurable at the moment. If there's
any serious problem with this then we can make it configurable and/or
flip the default"

The success of CMA allocation largely depends on the success of
migration and key factor of it is page reference count. Until now, page
reference is manipulated by direct calling atomic functions so we cannot
follow up who and where manipulate it. Then, it is hard to find actual
reason of CMA allocation failure. CMA allocation should be guaranteed
to succeed so finding offending place is really important.
In this patch, call sites where page reference is manipulated are
converted to introduced wrapper function. This is preparation step to
add tracepoint to each page reference manipulation function. With this
facility, we can easily find reason of CMA allocation failure. There is
no functional change in this patch.
In addition, this patch also converts reference read sites. It will
help a second step that renames page._count to something else and
prevents later attempt to direct access to it (Suggested by Andrew).

In current scache init cache line_size is determined from
cpu config register, however if there there no scache
then mips_sc_probe_cm3 function populates a invalid line_size of 2.
The invalid line_size can cause a NULL pointer deference
during r4k_dma_cache_inv as r4k_blast_scache is populated
based on line_size. Scache line_size of 2 is invalid option in
r4k_blast_scache_setup.

Commit c014d164f2 ("MIPS: Add platform callback before initializing
the L2 cache") added a platform_early_l2_init function in order to allow
platforms to probe for the CM before L2 initialisation is performed, so
that CM GCRs are available to mips_sc_probe.
That commit actually fails to do anything useful, since it checks
mips_cm_revision to determine whether it should call mips_cm_probe but
the result of mips_cm_revision will always be 0 until mips_cm_probe has
been called. Thus the "early" mips_cm_probe call never occurs.
Fix this & drop the useless weak platform_early_l2_init function by
simply calling mips_cm_probe from setup_arch. For platforms that don't
select CONFIG_MIPS_CM this will be a no-op, and for those that do it
removes the requirement for them to call mips_cm_probe manually
(although doing so isn't harmful for now).

Let's define page_mapped() to be true for compound pages if any
sub-pages of the compound page is mapped (with PMD or PTE).
On other hand page_mapcount() return mapcount for this particular small
page.
This will make cases like page_get_anon_vma() behave correctly once we
allow huge pages to be mapped with PTE.
Most users outside core-mm should use page_mapcount() instead of
page_mapped().

Tail page refcounting is utterly complicated and painful to support.
It uses ->_mapcount on tail pages to store how many times this page is
pinned. get_page() bumps ->_mapcount on tail page in addition to
->_count on head. This information is required by split_huge_page() to
be able to distribute pins from head of compound page to tails during
the split.
We will need ->_mapcount to account PTE mappings of subpages of the
compound page. We eliminate need in current meaning of ->_mapcount in
tail pages by forbidding split entirely if the page is pinned.
The only user of tail page refcounting is THP which is marked BROKEN for
now.
Let's drop all this mess. It makes get_page() and put_page() much
simpler.

Recent changes to how GFP_ATOMIC is defined seems to have broken the
condition to use mips_alloc_from_contiguous() in
mips_dma_alloc_coherent().
I couldn't bottom out the exact change but I think it's this commit
d0164adc89 ("mm, page_alloc: distinguish between being unable to
sleep, unwilling to sleep and avoiding waking kswapd").
GFP_ATOMIC has multiple bits set and the check for !(gfp & GFP_ATOMIC)
isn't enough.
The reason behind this condition is to check whether we can potentially
do a sleeping memory allocation. Use gfpflags_allow_blocking() instead
which should be more robust.

Commit 748e787eb6 ("MIPS: Optimize TLB refill for RI/XI
configurations.") stopped explicitly clearing the bits used by software
in PTEs by making use of a rotate instruction that rotates them into the
fill bits of the Entry{Lo,Hi} register. This can only work if there are
actually enough fill bits in the register to cover the software
maintained bits, otherwise we end up writing those bits into the upper
bits of the PFN or PFNX field of the Entry{Lo,Hi} register.
Fix this by detecting the number of fill bits present in the
Entry{Lo,Hi} registers & explicitly clearing the software bits where
necessary.

If there is a DMA zone (usually 24bit = 16MB I believe), but no DMA32
zone, as is the case for some 32-bit kernels, then massage_gfp_flags()
will cause DMA memory allocated for devices with a 32..63-bit
coherent_dma_mask to fall back to using __GFP_DMA, even though there may
only be 32-bits of physical address available anyway.
Correct that case to compare against a mask the size of phys_addr_t
instead of always using a 64-bit mask.

MAARs should be initialised on each CPU (or rather, core) in the system
in order to achieve consistent behaviour & performance. Previously they
have only been initialised on the boot CPU which leads to performance
problems if tasks are later scheduled on a secondary CPU, particularly
if those tasks make use of unaligned vector accesses where some CPUs
don't handle any cases in hardware for non-speculative memory regions.
Fix this by recording the MAAR configuration from the boot CPU and
applying it to secondary CPUs as part of their bringup.

Most architectures do not support non-coherent allocations and either
define dma_{alloc,free}_noncoherent to their coherent versions or stub
them out.
Openrisc uses dma_{alloc,free}_attrs to implement them, and only Mips
implements them directly.
This patch moves the Openrisc version to common code, and handles the
DMA_ATTR_NON_CONSISTENT case in the mips dma_map_ops instance.
Note that actual non-coherent allocations require a dma_cache_sync
implementation, so if non-coherent allocations didn't work on
an architecture before this patch they still won't work after it.

Since 2009 we have a nice asm-generic header implementing lots of DMA API
functions for architectures using struct dma_map_ops, but unfortunately
it's still missing a lot of APIs that all architectures still have to
duplicate.
This series consolidates the remaining functions, although we still need
arch opt outs for two of them as a few architectures have very
non-standard implementations.
This patch (of 5):
The coherent DMA allocator works the same over all architectures supporting
dma_map operations.
This patch consolidates them and converges the minor differences:
- the debug_dma helpers are now called from all architectures, including
those that were previously missing them
- dma_alloc_from_coherent and dma_release_from_coherent are now always
called from the generic alloc/free routines instead of the ops
dma-mapping-common.h always includes dma-coherent.h to get the defintions
for them, or the stubs if the architecture doesn't support this feature
- checks for ->alloc / ->free presence are removed. There is only one
magic instead of dma_map_ops without them (mic_dma_ops) and that one
is x86 only anyway.
Besides that only x86 needs special treatment to replace a default devices
if none is passed and tweak the gfp_flags. An optional arch hook is provided
for that.

The generic implementation of dma_map_ops.mmap(), dma_common_mmap(),
is not correct for non-coherent devices. It expects to be passed a
virtual address previously returned by dma_alloc_coherent(), which for
a non-coherent device will return a KSEG1 address. It then attempts to
convert that virtual address to a physical address using virt_to_page()
which will yield an incorrect address.
Also, dma_common_mmap() does not handle the DMA_ATTR_WRITE_COMBINE
attribute, and therefore dma_mmap_writecombine() will not actually set
the appropriate pgprot_t flags for write combining.
This patch adds an implementation of dma_map_ops.mmap() that correctly
handles KSEG1 addresses, and enables write combining when requested.

The TLB registers are dumped in a couble of places:
- sysrq_tlbdump_single() - when dumping TLB state.
- do_mcheck() - in response to a machine check error.
The main TLB registers also differ between r3k and r4k, but r4k appears
to be assumed.

Execution of break instruction, trap instructions, emulation of unaligned
loads or floating point instructions - anything that tries to read the
instruction's opcode from userspace - needs read access to a page.
RIXI (Read Inhibit / Execute Inhibit) support however allows the creation of
pags that are executable but not readable. On such a mapping the attempted
load of the opcode by the kernel is going to cause an endless loop of
page faults.
The quick workaround for this is to disable the combinations that the kernel
currently isn't able to handle which are executable mappings.

MT_SMP is not the only SMP option for MT cores. The MT_SMP option
allows more than one VPE per core to appear as a secondary CPU in the
system. Because of how CM works, it propagates the address-based
cache ops to the secondary cores but not the index-based ones.
Because of that, the code does not use IPIs to flush the L1 caches on
secondary cores because the CM would have done that already. However,
the CM functionality is independent of the type of SMP kernel so even in
non-MT kernels, IPIs are not necessary. As a result of which, we change
the conditional to depend on the CM presence. Moreover, since VPEs on
the same core share the same L1 caches, there is no need to send an
IPI on all of them so we calculate a suitable cpumask with only one
VPE per core.

This replaces the plain loop over the sglist array with for_each_sg()
macro which consists of sg_next() function calls. Since MIPS doesn't
select ARCH_HAS_SG_CHAIN, it is not necessary to use for_each_sg() in
order to loop over each sg element. But this can help find problems
with drivers that do not properly initialize their sg tables when
CONFIG_DEBUG_SG is enabled.

Commit c5b367835c ("MIPS: Add support for XPA.") added generation of a
shift by _PAGE_PRESENT_SHIFT in build_pte_present() and
build_pte_writable(), however except for the XPA case this is always
zero making it unnecessary.
Make the shift conditional upon _PAGE_PRESENT_SHIFT being non-zero to
save an instruction in those cases.

Commit c5b367835c ("MIPS: Add support for XPA.") changed
build_pte_present() and build_pte_writable() to assume a constant offset
of _PAGE_READ and _PAGE_WRITE relative to _PAGE_PRESENT, however this is
no longer true for some MIPS32R2 builds since commit be0c37c985
("MIPS: Rearrange PTE bits into fixed positions.") which moved the
_PAGE_READ PTE bit away from the _PAGE_PRESENT bit, with the _PAGE_WRITE
bit falling into its place.
Make use of the _PAGE_READ and _PAGE_WRITE definitions to calculate the
correct mask to apply instead of hard coding 3 (for _PAGE_PRESENT |
_PAGE_READ) or 5 (for _PAGE_PRESENT | _PAGE_WRITE).

Most R3k processor implementations have their 8 first TLB entries fixed
as wired, so we always skip them in TLB invalidation. That however
means any leftover entries present there at boot will stay throughout
the life of the kernel, unless replaced with new ones.
So rename `local_flush_tlb_all' to `local_flush_tlb_from' and make it
accept the TLB entry to start from. Then use 0 initially at bootstrap,
and the first regular entry later on, bypassing any wired entries.
Wrap the latter arrangement into a new `local_flush_tlb_all' entry
point.
There is no need to disable interrupts in the call made from `tlb_init'
because it's made before the interrupt subsystem has been initialised;
this is also true for secondary processors, should we ever support R3k
SMP. So move this piece of code to new `local_flush_tlb_all'.

Introduce faulthandler_disabled() and use it to check for irq context and
disabled pagefaults (via pagefault_disable()) in the pagefault handlers.
Please note that we keep the in_atomic() checks in place - to detect
whether in irq context (in which case preemption is always properly
disabled).
In contrast, preempt_disable() should never be used to disable pagefaults.
With !CONFIG_PREEMPT_COUNT, preempt_disable() doesn't modify the preempt
counter, and therefore the result of in_atomic() differs.
We validate that condition by using might_fault() checks when calling
might_sleep().
Therefore, add a comment to faulthandler_disabled(), describing why this
is needed.
faulthandler_disabled() and pagefault_disable() are defined in
linux/uaccess.h, so let's properly add that include to all relevant files.

When an architecture fully supports randomizing the ELF load location,
a per-arch mmap_rnd() function is used to find a randomized mmap base.
In preparation for randomizing the location of ET_DYN binaries
separately from mmap, this renames and exports these functions as
arch_mmap_rnd(). Additionally introduces CONFIG_ARCH_HAS_ELF_RANDOMIZE
for describing this feature on architectures that support it
(which is a superset of ARCH_BINFMT_ELF_RANDOMIZE_PIE, since s390
already supports a separated ET_DYN ASLR from mmap ASLR without the
ARCH_BINFMT_ELF_RANDOMIZE_PIE logic).

Fix the 74K D-cache alias erratum workaround so that it actually works.
Our current code sets MIPS_CACHE_VTAG for the D-cache, but that flag
only has any effect for the I-cache. Additionally MIPS_CACHE_PINDEX is
set for the D-cache if CP0.Config7.AR is also set for an affected
processor, leading to confusing information in the bootstrap log (the
flag isn't used beyond that).
So delete the setting of MIPS_CACHE_VTAG and rely on MIPS_CACHE_ALIASES,
set in a common place, removing I-cache coherency issues seen in GDB
testing with software breakpoints, gdbserver and ptrace(2), on affected
systems.
While at it add a little piece of explanation of what CP0.Config6.SYND
is so that people do not have to chase documentation.

The lazy cache flushing implemented in the MIPS kernel suffers from a
race condition that is exposed by do_set_pte() in mm/memory.c.
A pre-condition is a file-system that writes to the page from the CPU
in its readpage method and then calls flush_dcache_page(). One example
is ubifs. Another pre-condition is that the dcache flush is postponed
in __flush_dcache_page().
Upon a page fault for an executable mapping not existing in the
page-cache, the following will happen:
1. Write to the page
2. flush_dcache_page
3. flush_icache_page
4. set_pte_at
5. update_mmu_cache (commits the flush of a dcache-dirty page)
Between steps 4 and 5 another thread can hit the same page and it will
encounter a valid pte. Because the data still is in the L1 dcache the CPU
will fetch stale data from L2 into the icache and execute garbage.
This fix moves the commit of the cache flush to step 3 to close the
race window. It also reduces the amount of flushes on non-executable
mappings because we never enter __flush_dcache_page() for non-aliasing
CPUs.
Regressions can occur in drivers that mistakenly relies on the
flush_dcache_page() in get_user_pages() for DMA operations.

This patch rearranges the PTE bits into fixed positions for R2
and later cores. In the past, the TLB handling code did runtime
checking of RI/XI and adjusted the shifts and rotates in order
to fit the largest PFN value into the PTE. The checking now
occurs when building the TLB handler, thus eliminating those
checks. These new arrangements also define the largest possible
PFN value that can fit in the PTE. HUGE page support is only
available for 64-bit cores. Layouts of the PTE bits are now:
64-bit, R1 or earlier: CCC D V G [S H] M A W R P
32-bit, R1 or earler: CCC D V G M A W R P
64-bit, R2 or later: CCC D V G RI/R XI [S H] M A W P

Currently we have many duplicates in definitions around
follow_huge_addr(), follow_huge_pmd(), and follow_huge_pud(), so this
patch tries to remove the m. The basic idea is to put the default
implementation for these functions in mm/hugetlb.c as weak symbols
(regardless of CONFIG_ARCH_WANT_GENERAL_HUGETL B), and to implement
arch-specific code only when the arch needs it.
For follow_huge_addr(), only powerpc and ia64 have their own
implementation, and in all other architectures this function just returns
ERR_PTR(-EINVAL). So this patch sets returning ERR_PTR(-EINVAL) as
default.
As for follow_huge_(pmd|pud)(), if (pmd|pud)_huge() is implemented to
always return 0 in your architecture (like in ia64 or sparc,) it's never
called (the callsite is optimized away) no matter how implemented it is.
So in such architectures, we don't need arch-specific implementation.
In some architecture (like mips, s390 and tile,) their current
arch-specific follow_huge_(pmd|pud)() are effectively identical with the
common code, so this patch lets these architecture use the common code.
One exception is metag, where pmd_huge() could return non-zero but it
expects follow_huge_pmd() to always return NULL. This means that we need
arch-specific implementation which returns NULL. This behavior looks
strange to me (because non-zero pmd_huge() implies that the architecture
supports PMD-based hugepage, so follow_huge_pmd() can/should return some
relevant value,) but that's beyond this cleanup patch, so let's keep it.
Justification of non-trivial changes:
- in s390, follow_huge_pmd() checks !MACHINE_HAS_HPAGE at first, and this
patch removes the check. This is OK because we can assume MACHINE_HAS_HPAGE
is true when follow_huge_pmd() can be called (note that pmd_huge() has
the same check and always returns 0 for !MACHINE_HAS_HPAGE.)
- in s390 and mips, we use HPAGE_MASK instead of PMD_MASK as done in common
code. This patch forces these archs use PMD_MASK, but it's OK because
they are identical in both archs.
In s390, both of HPAGE_SHIFT and PMD_SHIFT are 20.
In mips, HPAGE_SHIFT is defined as (PAGE_SHIFT + PAGE_SHIFT - 3) and
PMD_SHIFT is define as (PAGE_SHIFT + PAGE_SHIFT + PTE_ORDER - 3), but
PTE_ORDER is always 0, so these are identical.

break the kernel for *all* existing MIPS CPUs that implement the
CP0_PageGrain[IEC] bit. They cause the TLB exception handlers to be
generated without the legacy execute-inhibit handling, but never set
the CP0_PageGrain[IEC] bit to activate the use of dedicated exception
vectors for execute-inhibit exceptions. The result is that upon
detection of an execute-inhibit violation, we loop forever in the TLB
exception handlers instead of sending SIGSEGV to the task.
If we are generating TLB exception handlers expecting separate
vectors, we must also enable the CP0_PageGrain[IEC] feature.
The bug was introduced in kernel version 3.17.

Signed-off-by: Ralf Baechle <ralf@linux-mips.org>
The core VM already knows about VM_FAULT_SIGBUS, but cannot return a
"you should SIGSEGV" error, because the SIGSEGV case was generally
handled by the caller - usually the architecture fault handler.
That results in lots of duplication - all the architecture fault
handlers end up doing very similar "look up vma, check permissions, do
retries etc" - but it generally works. However, there are cases where
the VM actually wants to SIGSEGV, and applications _expect_ SIGSEGV.
In particular, when accessing the stack guard page, libsigsegv expects a
SIGSEGV. And it usually got one, because the stack growth is handled by
that duplicated architecture fault handler.
However, when the generic VM layer started propagating the error return
from the stack expansion in commit fee7e49d45 ("mm: propagate error
from stack expansion even for guard page"), that now exposed the
existing VM_FAULT_SIGBUS result to user space. And user space really
expected SIGSEGV, not SIGBUS.
To fix that case, we need to add a VM_FAULT_SIGSEGV, and teach all those
duplicate architecture fault handlers about it. They all already have
the code to handle SIGSEGV, so it's about just tying that new return
value to the existing code, but it's all a bit annoying.
This is the mindless minimal patch to do this. A more extensive patch
would be to try to gather up the mostly shared fault handling logic into
one generic helper routine, and long-term we really should do that
cleanup.
Just from this patch, you can generally see that most architectures just
copied (directly or indirectly) the old x86 way of doing things, but in
the meantime that original x86 model has been improved to hold the VM
semaphore for shorter times etc and to handle VM_FAULT_RETRY and other
"newer" things, so it would be a good idea to bring all those
improvements to the generic case and teach other architectures about
them too.

There is a potential race when probing the TLB in TLBL/M/S exception
handlers for a matching entry. Between the time we hit a TLBL/S/M
exception and the time we get to execute the TLBP instruction, the
HTW may have replaced the TLB entry we are interested in hence the TLB
probe may fail. However, in the existing handlers, we never checked the
status of the TLBP (ie check the result in the C0/Index register). We
fix this by adding such a check when the core implements the HTW. If
we couldn't find a matching entry, we return back and try again.

The microMIPS microassembler is only suitable for configurations where
the kernel itself is built to microMIPS machine code and not where only
user microMIPS software is supported. The former is controlled with the
CPU_MICROMIPS configuration setting, whereas SYS_SUPPORTS_MICROMIPS is
used for the latter.
Not only that, but with a given microMIPS vs standard MIPS kernel
configuration only one microassembler is needed, that matches the ISA
selected -- CP0.Config3.ISAOnExc is mandatory on microMIPS processors,
so there is never a need to mix microMIPS and standard MIPS code.
Consequently build only the microassembler that matches the ISA selected
for the kernel.

In commit 8393c524a2 (MIPS: tlbex: Fix a missing statement for
HUGETLB), the TLB Refill handler was fixed so that non-OCTEON targets
would work properly with huge pages. The change was incorrect in that
it broke the OCTEON case.
The problem is shown here:
xxx0: df7a0000 ld k0,0(k1)

xxxc0: df610000 ld at,0(k1)
xxxc4: 335a0ff0 andi k0,k0,0xff0
xxxc8: e825ffcd bbit1 at,0x5,0x0
xxxcc: 003ad82d daddu k1,at,k0
.
.
.
In the non-octeon case there is a destructive test for the huge PTE
bit, and then at 0, $k0 is reloaded (that is what the 8393c524a2
patch added).
In the octeon case, we modify k1 in the branch delay slot, but we
never need k0 again, so the new load is not needed, but since k1 is
modified, if we do the load, we load from a garbage location and then
get a nested TLB Refill, which is seen in userspace as either SIGBUS
or SIGSEGV (depending on the garbage).
The real fix is to only do this reloading if it is needed, and never
where it is harmful.

Multicore MIPSes without I/D hardware coherency suffered from a race
condition in the page fault handler. The page table entry was
published before any pending lazy D-cache flush was committed, hence
it allowed execution of stale page cache data by other VPEs in the
system.
To make the cache handling safe we need to perform flushing already in
the set_pte_at function. MIPSes without coherent I-caches can get a
small increase in flushes due to the unavailability of the execute
flag in set_pte_at.

Add initialisation for Memory Accessibility Attribute Registers. Generic
code cannot know the platform-specific requirements with regards to
speculative accesses, so it simply calls a platform_maar_init function
which platforms with MAARs are expected to implement by calling the
provided write_maar_pair function & returning the number of MAAR pairs
used. A weak default implementation will simply use no MAAR pairs. Any
present but unused MAAR pairs are then marked invalid, effectively
disabling them.
The end result of this patch is that MAARs are all marked invalid, until
platforms implement the platform_maar_init function.

The Hardware Page Table Walker aims to speed up TLB refill exceptions
by handling them in the hardware level instead of having a software
TLB refill handler. However, a TLB refill exception can still be
thrown in certain cases such as, synchronus exceptions, or address
translation or memory errors during the HTW operation. As a result of
which, HTW must not be considered a complete replacement for the TLB
refill software handler, but rather a fast-path for it.
For HTW to work, the PWBase register must contain the task's page
global directory address so the HTW will kick in on TLB refill
exceptions.
Due to HTW being a separate engine embedded deep in the CPU pipeline,
we need to restart the HTW everytime a PTE changes to avoid HTW
fetching a old entry from the page tables. It's also necessary to
restart the HTW on context switches to prevent it from fetching a
page from the previous process. Finally, since HTW is using the
entryhi register to write the translations to the TLB, it's necessary
to stop the HTW whenever the entryhi changes (eg for tlb probe
perations) and enable it back afterwards.
== Performance ==
The following trivial test was used to measure the performance of the
HTW. Using the same root filesystem, the following command was used
to measure the number of tlb refill handler executions with and
without (using 'nohtw' kernel parameter) HTW support. The kernel was
modified to use a scratch register as a counter for the TLB refill
exceptions.
find /usr -type f -exec ls -lh {} \;

TLB refill exceptions: 12306

Avoid SMP cacheflushes. This is a minor optimization of startup but
will also avoid smp_call_function from doing stupid things when called
from a CPU that is not yet marked online.
missed an appropriate cache flush of TLB refill handler because that time it was
at fixed location CAC_BASE. After years the refill handler in EBASE vector
is not at that location and can be allocated in some another memory and needs
I-cache sync as other TLB exception vectors.
Besides that, the new function - local_flash_icache_range() was introduced
to avoid SMP cacheflushes.

So far BCM47XX can only detect amount of HIGHMEM. It still requires
adding (registering) and well-testing before enabling by default.

This reverts commit d7a887a73d.
Function add_temporary_entry is needed by bcm47xx to support highmem. We
need to add a temporary entry to check for amount of RAM.
The only change made in this revert was replacing (ENTER|EXIT)_CRITICAL.

We check that the struct vm_area_struct pointer vma is NULL and then
dereference it a few lines below. The intent was to make sure vma is
not NULL but this is not necessary since the bug pre-dates GIT history
and seem to never have caused a problem. The tlb-4k and tlb-8k versions
of local_flush_tlb_page() don't bother checking if vma is NULL, also
vma is dereferenced before being passed to local_flush_tlb_page(),
thus it is safe to remove this NULL check.

Code in a switch statement in probe_pcache checks the CPU type twice
unnecessarily for processor implementations that have the alias removal
feature reported by the CP0 Config7.AR and Config7.IAR bits. This change
rewrites the affected fragment avoiding the extraneous check and improving
readability.

In commit 2c8c53e28f (MIPS: Optimize TLB handlers for Octeon CPUs)
build_r4000_tlb_refill_handler() is modified. But it doesn't compatible
with the original code in HUGETLB case. Because there is a copy & paste
error and one line of code is missing. It is very easy to produce a bug
with LTP's hugemmap05 test.

'lw' opcode for it. Fix it by using the correct 'lh' opcode.

Pull MIPS updates from Ralf Baechle:
- three fixes for 3.15 that didn't make it in time
- limited Octeon 3 support.
- paravirtualization support
- improvment to platform support for Netlogix SOCs.
- add support for powering down the Malta eval board in software
- add many instructions to the in-kernel microassembler.
- add support for the BPF JIT.
- minor cleanups of the BCM47xx code.
- large cleanup of math emu code resulting in significant code size
reduction, better readability of the code and more accurate
emulation.
- improvments to the MIPS CPS code.
- support C3 power status for the R4k count/compare clock device.
- improvments to the GIO support for older SGI workstations.
- increase number of supported CPUs to 256; this can be reached on
certain embedded multithreaded ccNUMA configurations.
- various small cleanups, updates and fixes

Currently hugepage migration is available for all archs which support
pmd-level hugepage, but testing is done only for x86_64 and there're
bugs for other archs. So to avoid breaking such archs, this patch
limits the availability strictly to x86_64 until developers of other
archs get interested in enabling this feature.
Simply disabling hugepage migration on non-x86_64 archs is not enough to
fix the reported problem where sys_move_pages() hits the BUG_ON() in
follow_page(FOLL_GET), so let's fix this by checking if hugepage
migration is supported in vma_migratable().
