mm: Rename arch pte_mkwrite()'s to pte_mkwrite_novma()
The x86 Shadow stack feature includes a new type of memory called shadow stack. This shadow stack memory has some unusual properties, which requires some core mm changes to function properly. One of these unusual properties is that shadow stack memory is writable, but only in limited ways. These limits are applied via a specific PTE bit combination. Nevertheless, the memory is writable, and core mm code will need to apply the writable permissions in the typical paths that call pte_mkwrite(). The goal is to make pte_mkwrite() take a VMA, so that the x86 implementation of it can know whether to create regular writable or shadow stack mappings. But there are a couple of challenges to this. Modifying the signatures of each arch pte_mkwrite() implementation would be error prone because some are generated with macros and would need to be re-implemented. Also, some pte_mkwrite() callers operate on kernel memory without a VMA. So this can be done in a three step process. First pte_mkwrite() can be renamed to pte_mkwrite_novma() in each arch, with a generic pte_mkwrite() added that just calls pte_mkwrite_novma(). Next callers without a VMA can be moved to pte_mkwrite_novma(). And lastly, pte_mkwrite() and all callers can be changed to take/pass a VMA. Start the process by renaming pte_mkwrite() to pte_mkwrite_novma() and adding the pte_mkwrite() wrapper in linux/pgtable.h. Apply the same pattern for pmd_mkwrite(). Since not all archs have a pmd_mkwrite_novma(), create a new arch config HAS_HUGE_PAGE that can be used to tell if pmd_mkwrite() should be defined. Otherwise in the !HAS_HUGE_PAGE cases the compiler would not be able to find pmd_mkwrite_novma(). No functional change. sh: Avoid using IRQ0 on SH3 and SH4
IRQ0 is no longer returned by platform_get_irq() and its ilk -- they now
return -EINVAL instead. However, the kernel code supporting SH3/4-based
SoCs still maps the IRQ #s starting at 0 -- modify that code to start the
IRQ #s from 16 instead.
The patch should mostly affect the AP-SH4A-3A/AP-SH4AD-0A boards as they
indeed are using IRQ0 for the SMSC911x compatible Ethernet chip.

locking/atomic: sh: add preprocessor symbols
Some atomics can be implemented in several different ways, e.g. FULL/ACQUIRE/RELEASE ordered atomics can be implemented in terms of RELAXED atomics, and ACQUIRE/RELEASE/RELAXED can be implemented in terms of FULL ordered atomics. Other atomics are optional, and don't exist in some configurations (e.g. not all architectures implement the 128-bit cmpxchg ops). Subsequent patches will require that architectures define a preprocessor symbol for any atomic (or ordering variant) which is optional. This will make the fallback ifdeffery more robust, and simplify future changes. Add the required definitions to arch/sh. locking/atomic: make atomic*_{cmp,}xchg optional
Most architectures define the atomic/atomic64 xchg and cmpxchg operations in terms of arch_xchg and arch_cmpxchg respectfully. Add fallbacks for these cases and remove the trivial cases from arch code. On some architectures the existing definitions are kept as these are used to build other arch_atomic*() operations. The arm and sh versions of this file are identical to the generic versions and can just be removed. The drivers that actually use the sh3 specific version also include cpu/gpio.h directly, with the exception of magicpanelr2, which is easily fixed. sh/mm: support __HAVE_ARCH_PTE_SWP_EXCLUSIVE
Let's support __HAVE_ARCH_PTE_SWP_EXCLUSIVE by using bit 6 in the PTE, reducing the swap type in the !CONFIG_X2TLB case to 5 bits. Generic MM currently only uses 5 bits for the type (MAX_SWAPFILES_SHIFT), so the stolen bit is effectively unused. Interrestingly, the swap type in the !CONFIG_X2TLB case could currently overlap with the _PAGE_PRESENT bit, because there is a sneaky shift by 1 in __pte_to_swp_entry() and __swp_entry_to_pte(). Bit 0-7 in the architecture specific swap PTE would get shifted to bit 1-8 in the PTE. As generic MM uses 5 bits only, this didn't matter so far. While at it, mask the type in __swp_entry(). irq_to_gpio() is legacy and unused API, remove it for good. This leaves gpio_to_irq() as it's used yet in many places. Nevertheless, removal of its counterpart is a good signal to whoever even trying to consider using them that do not. stackprotector: actually use get_random_canary()
The RNG always mixes in the Linux version extremely early in boot. It also always includes a cycle counter, not only during early boot, but each and every time it is invoked prior to being fully initialized. Together, this means that the use of additional xors inside of the various stackprotector.h files is superfluous and over-complicated. Instead, we can get exactly the same thing, but better, by just calling `get_random_canary()`. mm: remove kern_addr_valid() completely
Most architectures (except arm64/x86/sparc) simply return 1 for kern_addr_valid(), which is only used in read_kcore(), and it calls copy_from_kernel_nofault() which could check whether the address is a valid kernel address. provide arch_test_bit_acquire for architectures that define test_bit
Some architectures define their own arch_test_bit and they also need
arch_test_bit_acquire, otherwise they won't compile. We also clean up
the code by using the generic test_bit if that is equivalent to the
arch-specific version.
