This patch fixes missing includes from a number of .c files because
the code (wrongfully) depended on prom.h including them. The include
of linux/of_address.h was removed in microblaze prom.h in commit
"of/address: Clean up function declarations" (sha1 id 22ae782f8), but
not fixed in some callers. This patch fixes them up.

Commit b45cfba4e9 (vt,console,kdb:
implement atomic console enter/leave functions) introduced the ability
to atomically change the console mode with kernel mode setting but did
not preserve the state of the console_blanked variable.
The console_blanked variable must be restored when executing the
con_debug_leave() or further kernel mode set changes (such as using
chvt X) will fail to correctly set the state of console.

NSC_GPIO is X86_32 only. The other driver (SCx200_GPIO) that selects

There are different types of a fifo which can not handled in C without a
lot of overhead. So i decided to write the API as a set of macros, which
is the only way to do a kind of template meta programming without C++.
This macros handles the different types of fifos in a transparent way.
There are a lot of benefits:
- Compile time handling of the different fifo types
- Better performance (a save put or get of an integer does only generate
9 assembly instructions on a x86)
- Type save
- Cleaner interface, the additional kfifo_..._rec() functions are gone
- Easier to use
- Less error prone
- Different types of fifos: it is now possible to define a int fifo or
any other type. See below for an example.
- Smaller footprint for none byte type fifos
- No need of creating a second hidden variable, like in the old DEFINE_KFIFO
The API was not changed.
There are now real in place fifos where the data space is a part of the
structure. The fifo needs now 20 byte plus the fifo space. Dynamic
assigned or allocated create a little bit more code.
Most of the macros code will be optimized away and simple generate a
function call. Only the really small one generates inline code.
Additionally you can now create fifos for any data type, not only the
"unsigned char" byte streamed fifos.
There is also a new kfifo_put and kfifo_get function, to handle a single
element in a fifo. This macros generates inline code, which is lit bit
larger but faster.
I know that this kind of macros are very sophisticated and not easy to
maintain. But i have all tested and it works as expected. I analyzed the
output of the compiler and for the x86 the code is as good as hand written
assembler code. For the byte stream fifo the generate code is exact the
same as with the current kfifo implementation. For all other types of
fifos the code is smaller before, because the interface is easier to use.
The main goal was to provide an API which is very intuitive, save and easy
to use. So linux will get now a powerful fifo API which provides all what
a developer needs. This will save in the future a lot of kernel space,
since there is no need to write an own implementation. Most of the device
driver developers need a fifo, and also deep kernel development will gain
benefit from this API.
Here are the results of the text section usage:

As you can see, kfifo_put/kfifo_get creates a little bit more code than
kfifo_in/kfifo_out, but it is much faster (the code is inline).
The code is complete hand crafted and optimized. The text section size is
as small as possible. You get all the fifo handling in only 3 kb. This
includes type safe fix size records, dynamic records and DMA handling.
This should be the final version. All requested features are implemented.
Note: Most features of this API doesn't have any users. All functions
which are not used in the next 9 months will be removed. So, please adapt
your drivers and other sources as soon as possible to the new API and post
it.

This patch:
User of the kernel fifo should never bypass the API and directly access
the fifo structure. Otherwise it will be very hard to maintain the API.

This is one of the most interesting users of module parameters in the
tree, so weaning it off the old-style non-const module_param_call
scheme is a useful exercise.
I was confused by set_param_int/get_param_int (vs. the normal
param_set_int and param_get_int), so I renamed set_param_int to
set_param_timeout, and re-used param_get_int directly instead of
get_param_int. I also implemented param_check_wdog_ifnum and
param_check_timeout, so now the ifnum_to_use and timeout/pretimeout
parameters can just use plain module_param().

When the console has been redirected, a hangup of the tty
will cause tty_release to be called under the big tty_mutex,
which leads to a deadlock because hangup is also called
under the BTM.
This moves the BTM deeper into the tty_hangup function so
we can close the redirected tty without holding the BTM.
In case of pty, we now need to drop the BTM before
calling tty_vhangup.

The tty locking now follows the rules for mutexes, so
we can replace the BKL usage with a new subsystem
wide mutex.
Using a regular mutex here will change the behaviour
when blocked on the BTM from spinning to sleeping,
but that should not be visible to the user.
Using the mutex also means that all the BTM is now
covered by lockdep.

Some wait_until_sent versions require the big
tty mutex, others don't and some callers of
wait_until_sent already hold it while other don't.
That leads to recursive use of the BTM in these
functions, which we're trying to get rid of.
This turns all cleans up the locking there so
that the driver's wait_until_sent function
never takes the BTM itself if it is already
called with that lock held.

We need to release the BTM in paste_selection() when
sleeping in tty_ldisc_ref_wait to avoid deadlocks
with tty_ldisc_enable.
In tty_set_ldisc, we now always grab the BTM before
taking the ldisc_mutex in order to avoid AB-BA
deadlocks between the two.
tty_ldisc_halt potentially blocks on a workqueue
function that takes the BTM, so we must release
the BTM before calling it.

tty_mutex is never taken with the BTM held, except for
two corner cases that are worked around here.
We give up the BTM before calling tty_release() in the
error path of tty_open().
Similarly, we reorder the locking in ptmx_open()
to get tty_mutex before the BTM.

We can use the port mutex for this and also for the hangup path so removing
the problematic use of the hangup mutex in this driver. Fix up the locking
on the various port flags while we are at it.
Ultimately this driver needs to be using tty_port_ helpers which would sort
this out far better.

The riscom8 board uses lock_kernel to protect bits of the port setting
ioctl logic. We can use the port mutex for this as the logic is internal
and will also lock set versus open (a locking property that has been lost
somewhere along the way)

Jesse's initial patch commit said:
"At panic time (i.e. when oops_in_progress is set) we should try a bit
harder to update the screen and make sure output gets to the VT, since
some drivers are capable of flipping back to it.
So make sure we try to unblank and update the display if called from a
panic context."
I've enhanced this to add a flag to the vc that console layer can set to
indicate they want this behaviour to occur. This also adds support to
fbcon for that flag and adds an fb flag for drivers to indicate they want
to use the support. It enables this for KMS drivers.

These are the changes needed for the kernel to support
LINEMODE in the server.
There is a new bit in the termios local flag word, EXTPROC.
When this bit is set, several aspects of the terminal driver
are disabled. Input line editing, character echo, and mapping
of signals are all disabled. This allows the telnetd to turn
off these functions when in linemode, but still keep track of
what state the user wants the terminal to be in.
New ioctl:
TIOCSIG Generate a signal to processes in the
current process group of the pty.
There is a new mode for packet driver, the TIOCPKT_IOCTL bit.
When packet mode is turned on in the pty, and the EXTPROC bit
is set, then whenever the state of the pty is changed, the
next read on the master side of the pty will have the TIOCPKT_IOCTL
bit set. This allows the process on the server side of the pty
to know when the state of the terminal has changed; it can then
issue the appropriate ioctl to retrieve the new state.
Since the original BSD patches accompanied the source code for telnet
I've left that reference here, but obviously the feature is useful for
any remote terminal protocol, including ssh.
The corresponding feature has existed in the BSD tty driver since 1989.

kernel will die on some platform when switch from vga mode to framebuffer
mode. The reason of this bug is that bind_con_driver reset vc->vc_origin
to (unsigned long)vc->vc_screenbuf.
On vgacon vc->vc_origin is not releated to vc->vc_screenbuf,if set
vc->vc_origin to vc->vc_screenbuf,kernel will die on vc_do_resize.

misc_deregister() returns an error only when it attempts to unregister
the device that is not registered. This is the driver's bug.
Most of the drivers don't check the return value of misc_deregister().
(It is not bad thing because most of kernel *_unregister() API always
succeed and do not return value)
So it is better to indicate the error by WARN_ON() in misc_deregister().

Make /dev/console get initialised before any initialisation routine that
invokes modprobe because if modprobe fails, it's going to want to open
/dev/console, presumably to write an error message to.
The problem with that is that if the /dev/console driver is not yet
initialised, the chardev handler will call request_module() to invoke
modprobe, which will fail, because we never compile /dev/console as a
module.
This will lead to a modprobe loop, showing the following in the kernel
log:

This can happen, for example, when the built in md5 module can't find
the built in cryptomgr module (because the latter fails to initialise).
The md5 module comes before the call to tty_init(), presumably because
'crypto' comes before 'drivers' alphabetically.
Fix this by calling tty_init() from chrdev_init().

The kernel console interface stores the number of lines it is
configured to use. The kdb debugger can greatly benefit by knowing how
many lines there are on the console for the pager functionality
without having the end user compile in the setting or have to
repeatedly change it at run time.

Some BIOSes will claim a large chunk of stolen space. Unless we
reclaim it, our aperture for remapping buffer objects will be
constrained. So clamp the stolen space to 32M and ignore the rest.
Fixes https://bugzilla.kernel.org/show_bug.cgi?id=15469 among others.
Adding the ignored stolen memory back into the general pool using the
memory hotplug code is left as an exercise for the reader.

A side-effect of being able to use custom page allocations with the
sg_table is that it cannot reap the partially constructed scatterlist if
fails to allocate a page. So we need to call sg_free_table() ourselves
if sg_alloc_table() fails.

When "onboard video memory" is set do "disabled" in BIOS on Asus P4P800-VM
board (i865G), kernel oopses with memory corruption:

This patch moves the declaration of of_get_address(), of_get_pci_address(),
and of_pci_address_to_resource() out of arch code and into the common
linux/of_address header file.
This patch also fixes some of the asm/prom.h ordering issues. It still
includes some header files that it ideally shouldn't be, but at least the
ordering is consistent now so that of_* overrides work.

Re-order structure entropy_store to remove 8 bytes of padding on
64 bit builds, so shrinking this structure from 72 to 64 bytes
and allowing it to fit into one cache line.

According to the specification found at
http://intellinuxgraphics.org/VOL_1_graphics_core.pdf, the PCI config
space register I830_GMCH_CTRL is a mirror of GMCH Graphics
Control. The correct macro for isolating the aperture size bits is
therefore I830_GMCH_GMS_MASK along with the attendant changes to the
case statement.

Fix subsequent suspends by issuing tpm_continue_selftest during resume.
Otherwise, the tpm chip seems to be not fully initialized and will reject
the save state command during suspend, thus preventing the whole system
to suspend.

Both of_bus_type and of_platform_bus_type are just #define aliases
for the platform bus. This patch removes all references to them and
switches to the of_register_platform_driver()/of_unregister_platform_driver()
API for registering.
Subsequent patches will convert each user of of_register_platform_driver()
into plain platform_drivers without the of_platform_driver shim. At which
point the of_register_platform_driver()/of_unregister_platform_driver()
functions can be removed.

The use of "hvc_con_driver" as the name for a file-static "struct
console" with a ".setup" field pointing to an __init function causes
a modpost warning, since a non-initdata structure points to init code.
Using "hvc_console" as the name triggers the hacky "*_console"
workaround in modpost to silence the warning, and is the same thing
that most of the other console drivers already do.
I made the same change in hvsi.c since I happened to notice it was
likely to suffer from the same problem.

None of these changes fix any actual bugs, but are just various cleanups
that fell out along the way. In particular, some unused #defines and
includes are removed, PREFETCH_STRIDE is added (the default is right for
our shipping chips, but wrong for our next generation), our tile-specific
prefetching code is removed so the (identical) generic prefetching code
can be used instead, a comment is fixed to be proper GPL and not just a
"paste GPL here" token, a "//" comment is converted to "/* */", etc.

Fix a regression introduced by ae74e823cb ("ipmi: add parameter to limit
CPU usage in kipmid").
Some systems were seeing CPU usage go up dramatically with the recent
changes to try to reduce timer usage in the IPMI driver. This was traced
down to schedule_timeout_interruptible(1) being changed to
schedule_timeout_interruptbile(0). Revert that part of the change.

The ipmi code will never register a PCI or Open Firmware driver if a
hardcoded device is provided by the user by providing device addresses via
the module parameters. This can cause us to attempt to unregister a
driver that was never registered, resulting in an oops. Keep track of
registration in order to avoid this.
Fixes a post-2.6.34 regression.

This patch adds a missing element of the ReadPubEK command output,
that prevents future overflow of this buffer when copying the
TPM output result into it.
Prevents a kernel panic in case the user tries to read the
pubek from sysfs.

This reverts commit 962400e8fd, which was
entirely bogus.
The code used to multiply the character offset by "vc->vc_cols", and
that's actually correct, because 'd' itself is an 'unsigned short'. So
the pointer arithmetic already takes the size of a VGA character into
account. Changing it to use vc_size_row (which is just "vc_cols"
shifted up to take the size of the character into account) ends up
multiplying with the VGA character size twice.
This got reported as bugs for various other subsystems, because what it
actually results in is writing the 16-bit vc_video_erase_char pattern
(usually 0x0720: 0x07 is the default attribute, 0x20 is ASCII space)
into some random other allocation.
So Markus ended up reporting this as a ext4 bug, while to Torsten Kaiser
it looked like a problem with KMS or libata. Jeff Chua saw it in
different places.
And finally - Justin Mattock had slab poisoning enabled, and saw it as a
slab poison overwritten. And bisected and reverted this to verify the
buggy commit.

This change is the core kernel support for TILEPro and TILE64 chips.
No driver support (except the console driver) is included yet.
This includes the relevant Linux headers in asm/; the low-level
low-level "Tile architecture" headers in arch/, which are
shared with the hypervisor, etc., and are build-system agnostic;
and the relevant hypervisor headers in hv/.

The code uses vc->vc_cols instead of vc->vc_size_row by mistake, it
will cause half of the region which is going to clear remain
uncleared.
The issue happens in background consoles, so it's hard to observe.

In gsm_dlci_data_kick() we call gsm_dlci_data_sweep() with the
"gsm->tx_lock" held so we can't lock it again inside
gsm_dlci_data_sweep(). I removed that lock from and added one to
gsmld_write_wakeup() instead. The sweep function is only called from
those two places.

When a program that has a virtio port opened and blocked for a write
operation, a port hot-unplug event will later led to a crash when
SIGTERM was sent to the program. Fix that.

When removing a port we don't check if a program was blocked for read.
This leads to a crash when SIGTERM is sent to the program after
hot-unplugging the port.

This fixes a sporadic oops at boot on G5 Power Macs. The table_end
variable has the address of the last byte of the table. Adding on
PAGE_SIZE means we flush too much, and if the page after the table
is not mapped for any reason, the kernel will oops. Instead we add
on 1 because flush_dcache_range() interprets its second argument as
the first byte past the range to be flushed.

Ramoops, like mtdoops, can log oops/panic information but in RAM. It can
be used with persistent RAM for systems without flash support. In
addition, for this systems, with this driver, it's no more needed add to
the kernel the mtd subsystem with advantage in footprint.
It can be used in a very easy way with persistent RAM for systems without
flash support. For these systems, with this driver, it is no longer
required to cinlude mtd subsystem with an advantage in footprint. In
addition, you can save flash space and store this information only in RAM.

If run_to_completion flag is set, it means that we are running in a
single-threaded mode, and thus no locks are held.
This fixes a deadlock when IPMI notifier is being called during panic.

The timeouts in IPMI are in the 1-5 second range in message handling, so a
1 second timeout is a reasonable thing to do. This should help with
reducing power consumption on idle systems.

Some odd systems may have multiple BMCs, and we want to be able to support
them. Let's make the assumption that if a system legitimately has
multiple BMCs then each BMC's SI will be of the same type, and also that
we won't see multiple SIs of the same type unless we have multiple BMCs.
If these hold true then we should register all SIs of the same type.

If we're not currently in the middle of a transaction, and if we have
interrupts, there's no real reason to poll the controller more frequently
than the core IPMI code does. Set the interrupt_disabled flag
appropriately as the interrupt state changes, and make the timeout code
reset itself only if the transaction is incomplete or we have no
interrupts.

The ipmi spec provides an ordering for si discovery. Change the driver to
match, with the exception of preferring smbios to SPMI as HPs (at least)
contain accurate information in the former but not the latter.

On G33 and above, the size of the GTT space is determined by the GMCH
control register. Prior to this revision, the size is determined by the
size of the aperture. So we must careful to map and fill the appropriate
range depending on chipset.

For misc devices, inode->i_cdev doesn't point to the device drivers own
data. Link between file operations and device driver internal data is
lost. Pass pointer to misc device struct via file private data for driver

Handle out-of-range indices before reading what they refer to. And don't

drivers/char/hangcheck-timer.c is doubly broken. When the overflown value
of TIMER_FREQ is abnormally low, it spams the syslog with KERN_CRIT
messages "Hangcheck: hangcheck value past margin!" But whether it happens
or not depends on HZ and lpj in a complex way. People have hit it
occasionally as far as google search can tell.
First, the following line overflows unsigned long:
# define TIMER_FREQ (HZ*loops_per_jiffy)
Second, and more importantly, loops_per_jiffy has little to do with the
con= version from the the time scale of get_cycles() (aka rdtsc) to the
time scale of jiffies.
The attached patch resolves both of the problems.

.name, .match_table and .owner are duplicated in both of_platform_driver
and device_driver. This patch is a removes the extra copies from struct
of_platform_driver and converts all users to the device_driver members.
This patch is a pretty mechanical change. The usage model doesn't change
and if any drivers have been missed, or if anything has been fixed up
incorrectly, then it will fail with a compile time error, and the fixup
will be trivial. This patch looks big and scary because it touches so
many files, but it should be pretty safe.

Add an implementation of GSM 0710 MUX. The implementation currently supports
- Basic and advanced framing (as either end of the link)
- UI or UIH data frames
- Adaption layer 1-4 (1 and 2 via tty, 3 and 4 as skbuff lists)
- Modem and control messages including the correct retry process
- Flow control
and exposes the MUX channels as a set of virtual tty devices including modem
signals. This is an experimental driver.

In each case, the first argument to send_control_msg or __send_control_msg,
respectively, has either not been successfully allocated or has been freed
at the point of the call. In the first case, the first argument, port, is
only used to access the portdev and id fields, in order to call
__send_control_msg. Thus it seems possible instead to call
__send_control_msg directly. In the second case, the call to
__send_control_msg is moved up to a place where it seems like the first
argument, portdev, has been initialized sufficiently to make the call to
__send_control_msg meaningful.
This has only been compile tested.

The VIRTIO_CONSOLE_RESIZE control message sent to us by the host now
contains the new {rows, cols} values for the console. This ensures each
console port gets its own size, and we don't depend on the config-space
rows and cols values at all now.

With support for multiple consoles, just using one {rows,cols} pair in
the config space is not going to suffice.
Store each console's size as part of the console struct.
This changes the behaviour for one case when multiport is not enabled:
when notifier_add_vio() is called, the console size is taken from that
of the last config-space update instead of fetching it afresh from the
config space.
Also add a helper to update the size in the console struct as we'll need
to use the same code to update the size via control messages when
multiport support is enabled.

If the host port is not open, a write() should either just return if the
file is opened in non-blocking mode, or block till the host port is
opened.
Also, don't spin till host consumes data for nonblocking ports. For
non-blocking ports, we can do away with the spinning and reclaim the
buffers consumed by the host on the next write call or on the condition
that'll make poll return.

If we're using multiport, there's no point in always creating a console
port. Create the console port only if the host doesn't support
multiport.

Instead of the host and guest independently enumerating ports, switch to
a control message to add ports where the host supplies the port number
so there's no ambiguity or a possibility of a race between the host and
the guest port numbers.
We now no longer need the 'nr_ports' config value. Since no kernel has
been released with the MULTIPORT changes yet, we have a chance to fiddle
with the config space without adding compatibility features.
This is beneficial for management software, which would now be able to
instantiate ports at known locations and avoid problems that arise with
implicit numbering in the host and the guest. This removes the 'guessing
game' part of it, and management software can now actually indicate
which id to spawn a particular port on.

hvc_remove() has some bug which freezes other active hvc ports when one
port is removed.
So disable calling of hvc_remove() which deregisters a port with the
hvc_console.
If the hvc_console code calls into our get_chars() routine as a result
of a poll operation, we will return -EPIPE and the hvc_console code will
then do the necessary cleanup.
This call will be restored when the bug in hvc_remove() is found and
fixed.

Multiport support was disabled for 2.6.34 because we wanted to introduce
a new ABI and since we didn't have any released kernel with the older
ABI and were out of the merge window, it didn't make sense keeping the
older ABI around.
Now we revert the patch disabling multiport and rework the ABI in the
following patches.

The current initialisation code probes 'unsupported' AGP devices
simply by calling its own probe function. It does not lock these
devices or even check whether another driver is already bound to
them.
We must use the device core to manage this. So if the specific
device id table didn't match anything and agp_try_unsupported=1,
switch the device id table and call driver_attach() again.

SIS 760 is listed in the device tables for both amd64-agp and sis-agp.
amd64-agp is apparently preferable since it has workarounds for some
BIOS misconfigurations that sis-agp doesn't handle.

Push down the bkl from procfs's ioctl main handler to its users.
Only three procfs users implement an ioctl (non unlocked) handler.
Turn them into unlocked_ioctl and push down the Devil inside.

This patch pushes the ACPI dependency into the device driver code
itself. Now, even without ACPI/PNP enabled, the device can be registered
using the TIS specified memory space. This will however result in the
lack of access to the BIOS event log, being the only implication of such
ACPI removal.

The BSD ringbuffer support that is landing in this branch
significantly conflicts with the Ironlake PIPE_CONTROL fix on master,
and requires it to be tested successfully anyway.
As a fourth step, remove any remaining usages of
dev_node_t from drivers:
- ipwireless can be simplified a bit, as we do not need
to pass around the (write-only) dev_node_t around.
- avma1_cs can be simplified as well, if we only keep the
minor number around as "priv" data, not a full-fledged
struct.

dev_node_t was only used to transport some minor/major numbers
from the PCMCIA device drivers to deprecated userspace helpers.
However, only a few drivers made use of it, and the userspace
helpers are deprecated anyways. Therefore, get rid of dev_node_t .
As a first step, remove any usage of dev_node_t from drivers which
only wrote to this typedef/struct, but did not make use of it.

Instead of the old pcmcia_request_irq() interface, drivers may now
choose between:
- calling request_irq/free_irq directly. Use the IRQ from *p_dev->irq.
- use pcmcia_request_irq(p_dev, handler_t); the PCMCIA core will
clean up automatically on calls to pcmcia_disable_device() or
device ejection.
- drivers still not capable of IRQF_SHARED (or not telling us so) may
use the deprecated pcmcia_request_exclusive_irq() for the time
being; they might receive a shared IRQ nonetheless.

This patch pushes the ACPI dependency into the device driver code
itself. Now, even without ACPI/PNP enabled, the device can be registered
using the TIS specified memory space. This will however result in the
lack of access to the bios event log, being the only implication of such
ACPI removal.

Convert most AGP chipset to use scratch page as default entries.
This help avoiding GPU querying 0 address and trigger computer
fault. With KMS and memory manager we bind/unbind AGP memory
constantly and it seems that some GPU are still doing AGP
traffic even after GPU report being idle with the memory segment.
Tested (radeon GPU KMS + Xorg + compiz + glxgears + quake3) on :
- SIS 1039:0001 & 1039:0003
- Intel 865 8086:2571

The ftrace_dump_on_oops kernel parameter, sysctl and sysrq let one
dump every cpu buffers when an oops or panic happens.
It's nice when you have few cpus but it may take ages if have many,
plus you miss the real origin of the problem in all the cpu traces.
Sometimes, all you need is to dump the cpu buffer that triggered the
opps, most of the time it is our main interest.
This patch modifies ftrace_dump_on_oops to handle this choice.
The ftrace_dump_on_oops kernel parameter, when it comes alone, has
the same behaviour than before. But ftrace_dump_on_oops=orig_cpu
will only dump the buffer of the cpu that oops'ed.
Similarly, sysctl kernel.ftrace_dump_on_oops=1 and
echo 1 > /proc/sys/kernel/ftrace_dump_on_oops keep their previous
behaviour. But setting 2 jumps into cpu origin dump mode.

This is essentially the last piece of code that's tying intel-gtt.c
to intel-agp.c. Extract the probe code into it's own function so that
it can be moved to intel-gtt.c.
This requires some slight changes in the ordering of device probe function.
This patch just implements that for better bisectability in case this
introduces bugs.
The biggest change is that the gmch/gtt code doesn't execute a pci
resource fixup anymore. I've dug around in historical git trees, and
this change is to support the agp port on an old HP server with the
i440 intel chipset. So only needed for the agp driver.

intel-agp.c contains actually two different drivers: An agp driver
for _physical_ agp slots an the gtt driver that is used by the intel
drm modules. Split them to prevent any further confusion.
This patch just moves the code and includes intel-gtt.c in intel-agp.c
Later patches will untangle these two drivers further.

Instead of keeping SysRq support inside of legacy keyboard driver split
it out into a separate input handler (filter). This stops most SysRq input
events from leaking into evdev clients (some events, such as first SysRq
scancode - not keycode - event, are still leaked into both legacy keyboard
and evdev).

I don't claim to understand the tty layer, but it seems like hvc_open and
hvc_close should be balanced in their kref reference counting.
Right now we get a kref every call to hvc_open:

The get_buf() API sets the second arg to the number of bytes *written*
by the other side; in this case it should be zero as these are output buffers.
lguest gets this right (obviously kvm's console doesn't), resulting in
continual buildup of console writes.

Commit 148f948ba8 (vfs: Introduce new
helpers for syncing after writing to O_SYNC file or IS_SYNC inode) broke
the raw driver.
We now call through generic_file_aio_write -> generic_write_sync ->
vfs_fsync_range. vfs_fsync_range has:
if (!fop || !fop->fsync) {
ret = -EINVAL;
goto out;
}
But drivers/char/raw.c doesn't set an fsync method.
We have two options: fix it or remove the raw driver completely. I'm
happy to do either, the fact this has been broken for so long suggests it
is rarely used.
The patch below adds an fsync method to the raw driver. My knowledge of
the block layer is pretty sketchy so this could do with a once over.
If we instead decide to remove the raw driver, this patch might still be
useful as a backport to 2.6.33 and 2.6.32.

I hit this when we had a bug in IDR for a few days. Basically sysfs would
fail to create new inodes since it uses an IDR and therefore class_create
would fail.
While we are unlikely to see this fail we may as well handle it instead of
oopsing.

rs_init() is failing to restore interrupts on two error paths, and is
incorrectly calling tty_unregister_driver() with local interrupts
disabled.
Fix these things by disabling interrupts later, after the reauest_irq()
calls.

percpu.h is included by sched.h and module.h and thus ends up being
included when building most .c files. percpu.h includes slab.h which
in turn includes gfp.h making everything defined by the two files
universally available and complicating inclusion dependencies.
percpu.h -> slab.h dependency is about to be removed. Prepare for
this change by updating users of gfp and slab facilities include those
headers directly instead of assuming availability. As this conversion
needs to touch large number of source files, the following script is
used as the basis of conversion.

The script does the followings.
* Scan files for gfp and slab usages and update includes such that
only the necessary includes are there. ie. if only gfp is used,
gfp.h, if slab is used, slab.h.
* When the script inserts a new include, it looks at the include
blocks and try to put the new include such that its order conforms
to its surrounding. It's put in the include block which contains
core kernel includes, in the same order that the rest are ordered -
alphabetical, Christmas tree, rev-Xmas-tree or at the end if there
doesn't seem to be any matching order.
* If the script can't find a place to put a new include (mostly
because the file doesn't have fitting include block), it prints out
an error message indicating which .h file needs to be added to the
file.
The conversion was done in the following steps.
1. The initial automatic conversion of all .c files updated slightly
over 4000 files, deleting around 700 includes and adding ~480 gfp.h
and ~3000 slab.h inclusions. The script emitted errors for ~400
files.
2. Each error was manually checked. Some didn't need the inclusion,
some needed manual addition while adding it to implementation .h or
embedding .c file was more appropriate for others. This step added
inclusions to around 150 files.
3. The script was run again and the output was compared to the edits
from #2 to make sure no file was left behind.
4. Several build tests were done and a couple of problems were fixed.
e.g. lib/decompress_*.c used malloc/free() wrappers around slab
APIs requiring slab.h to be added manually.
5. The script was run on all .h files but without automatically
editing them as sprinkling gfp.h and slab.h inclusions around .h
files could easily lead to inclusion dependency hell. Most gfp.h
inclusion directives were ignored as stuff from gfp.h was usually
wildly available and often used in preprocessor macros. Each
slab.h inclusion directive was examined and added manually as
necessary.
6. percpu.h was updated not to include slab.h.
7. Build test were done on the following configurations and failures
were fixed. CONFIG_GCOV_KERNEL was turned off for all tests (as my
distributed build env didn't work with gcov compiles) and a few
more options had to be turned off depending on archs to make things
build (like ipr on powerpc/64 which failed due to missing writeq).
* x86 and x86_64 UP and SMP allmodconfig and a custom test config.
* powerpc and powerpc64 SMP allmodconfig
* sparc and sparc64 SMP allmodconfig
* ia64 SMP allmodconfig
* s390 SMP allmodconfig
* alpha SMP allmodconfig
* um on x86_64 SMP allmodconfig
8. percpu.h modifications were reverted so that it could be applied as
a separate patch and serve as bisection point.
Given the fact that I had only a couple of failures from tests on step
6, I'm fairly confident about the coverage of this conversion patch.
If there is a breakage, it's likely to be something in one of the arch
headers which should be easily discoverable easily on most builds of
the specific arch.

When the host lets us know what 'name' a port is assigned, we create the
sysfs 'name' attribute. Generate a 'change' event after this so that
udev wakes up and acts on the rules for virtio-ports (currently there's
only one rule that creates a symlink from the 'name' to the actual char
device).

starting in 2.6.33. This was observed when using
console=ttyUSB0,115200 as a boot argument with an FTDI device. The
error is:
ftdi_sio ttyUSB0: ftdi_submit_read_urb - failed submitting read urb, error -22
The handling of the ASYNCB_INITIALIZED changed in 2.6.32 such that in
tty_port_shutdown() it always clears the flag if it is set. The fix
is to add a variable to the tty_port struct to indicate when the tty
port is a console.

Alan pointed out a race in the code where hvc_remove is invoked. The
recent virtio_console work is the first user of hvc_remove().
Alan describes it thus:
The hvc_console assumes that a close and remove call can't occur at the
same time.
In addition tty_hangup(tty) is problematic as tty_hangup is asynchronous
itself....

The problems with this patch include:
1. There's at least one subtlety I overlooked - switching
between X servers (i.e. from one X VT to another) still requires
the cooperation of both X servers. I was assuming that KMS
eliminated this.
2. It hasn't been tested at all (no X server patch exists which
uses the new mode).
As he was the original author of the patch, I'll revert it.

This CPU should be coherent with graphics in this direction, though
flushing graphics caches are still required. Fixes a system reset on
module load on Sandybridge with 4G+ memory.

de957628ce changed setting of the
x86_init.iommu.iommu_init function ptr only when GART IOMMU is
found.
One side effect of it is that num_k8_northbridges
is not initialized anymore if not explicitly
called. This resulted in uninitialized pointers in
<arch/x86/kernel/cpu/intel_cacheinfo.c:amd_calc_l3_indices()>,
for example, which uses the num_k8_northbridges thing through
node_to_k8_nb_misc().
Fix that through an initcall that runs right after the PCI
subsystem and does all the scanning. Then, remove initialization
in gart_iommu_init() which is a rootfs_initcall and we're
running before that.
What is more, since num_k8_northbridges is being used in other
places beside GART IOMMU, include it whenever we add AMD CPU
support. The previous dependency chain in kconfig contained
K8_NB depends on AGP_AMD64|GART_IOMMU
which was clearly incorrect. The more natural way in terms of
hardware dependency should be
AGP_AMD64|GART_IOMMU depends on K8_NB depends on CPU_SUP_AMD &&
PCI. Make it so Number One!

test whether mn->next is NULL.
Unless that test is redundant too, your patch fixes a potential NULL

In some cases kipmid can use a lot of CPU. This adds a way to tune the
CPU used by kipmid to help in those cases. By setting kipmid_max_busy_us
to a value between 100 and 500, it is possible to bring down kipmid CPU
load to practically 0 without loosing too much ipmi throughput
performance. Not setting the value, or setting the value to zero,
operation is unaffected.

The device driver must allocate memory for IUCV buffers with GFP_DMA,
because IUCV cannot address memory above 2GB (31bit addresses only).
Because the IUCV ignores the higher bits of the address, sending and
receiving IUCV data with this driver might cause memory corruptions.

This new VT mode (VT_PROCESS_AUTO) does everything that VT_PROCESS does
except that it doesn't wait for a VT_RELDISP ioctl before switching
away from a VT with that mode.
If the X server eventually uses this new mode, debugging and crash
recovery should become easier. This is because even when currently in
the VT of a frozen X server it would still be possible to switch out
by doing SysRq-r and then CTRL-<number of a text vt>, sshing in and
doing chvt <number of a text vt>, or any other method of VT switching.
The general concensus on #xorg-devel seems to be that it should be
safe to use this with X now that we have KMS.
This also moves the VT_ACKACQ define to a more appropriate place,
for clarity's sake.

We allocate during interrupts so while our buffering is normally diced up
small anyway on some hardware at speed we can pressure the VM excessively
for page pairs. We don't really need big buffers to be linear so don't try
so hard.
In order to make this work well we will tidy up excess callers to request_room,
which cannot itself enforce this break up.

This was noticed by Matthias Urlichs and he proposed a fix. This patch
does the fixing a different way to avoid introducing several new race
conditions into the code.
The problem case is TTY_DRIVER_RESET_TERMIOS = 0. In that case while we
abort the ldisc change, the hangup processing has not cleaned up and restarted
the ldisc either.
We can't restart the ldisc stuff in the set_ldisc as we don't know what
the hangup did and may touch stuff we shouldn't as we are no longer
supposed to influence the tty at that point in case it has been re-opened
before we get rescheduled.
Instead do it the simple way. Always re-init the ldisc on the hangup, but
use TTY_DRIVER_RESET_TERMIOS to indicate that we should force N_TTY.

lock so did clever (and wrong) things to work around the non need to avoid

The Nozomi tty handling is very broken on the open/close side (See
http://bugzilla.kernel.org/show_bug.cgi?id=13024 for one example). In
particular it marks the tty as closed on the first close() not on the last.
Most of the logic is pretty solid except for the open/close path so switch
to the tty_port helpers and let them do all the heavy lifting. This is also
fixes all the POSIX behaviour violations in the open/close paths.

Stanse found a potential null dereference in mgsl_put_char and
mgsl_write. There is a check for tty being NULL, but it is
dereferenced earlier.
Actually, tty cannot be NULL in .write and .put_char, so remove

Fix transmit bug that could drop send data if write() called close to
serial transmitter going idle after sending previous data. Bug is caused
by incorrect use of device information member tx_count.
Driver originally processed one data block (write call) at a time, waiting
for transmit idle before sending more. tx_count recorded how much data
was loaded in DMA buffers on write(), and was cleared on send completion.
tx_count use was overloaded to record accumulated data from put_char()
callback when transmitter was idle.
A bug was introduced when transmit code was reworked to allow multiple
blocks of data in the tx DMA buffers which keeps transmitter from going
idle between blocks. tx_count was set to size of last block loaded,
cleared when tx went idle, and monitored to know when to restart
transmitter without proper synchronization. tx_count could be cleared
when unsent data remained in DMA buffers and transmitter required
restarting, effectively dropping unsent data.
Solution:
1. tx_count now used only to track accumulated data from put_char
2. DMA buffer state tracked by direct inspection of descriptors

Make sure that the appropriate AGP module is loaded and probed before
trying to set up the DRM. The DRM already depends on the AGP core,
but in this case we know the specific AGP driver we need too, and can
help users avoid the trap of loading the AGP driver after the DRM
driver.

Instead of allocating just one buffer for a port's in_vq, fill
the entire in_vq with buffers so the host need not stall while
an application consumes the data and makes the buffer available
again for the host.

With MULTIPORT support, the control queue is an integral part of the
functioning of the device. If we can't get any buffers allocated, the
host won't be able to relay important information and the device may not
function as intended.
Ensure 'probe' doesn't succeed until the control queue has at least one
buffer allocated for its ivq.

If unused data exists in in_vq, ensure we flush that first and then
detach unused buffers, which will ensure all buffers from the in_vq are
removed.
Also ensure we free the buffers after detaching them.

If the 'nr_ports' variable in the config space is updated to a higher
value, that means new ports have been hotplugged.
Introduce a new workqueue to handle such updates and create new ports.

The host can set a name for ports so that they're easily discoverable
instead of going by the /dev/vportNpn naming. This attribute will be
placed in /sys/class/virtio-ports/vportNpn/name. udev scripts can then
create symlinks to the port using the name.

Add a guest_connected field that ensures only one process
can have a port open at a time.
This also ensures we don't have a race when we later add support for
dropping buffers when closing the char dev and buffer caching is turned
off for the particular port.

Allow guest userspace applications to open, read from, write to, poll
the ports via the char dev interface.
When a port gets opened, a notification is sent to the host via a
control message indicating a connection has been established. Similarly,
on closing of the port, a notification is sent indicating disconnection.

The char device will be used as an interface by applications on the
guest to communicate with apps on the host.
The devices created are placed in /dev/vportNpn where N is the
virtio-console device number and n is the port number for that device.
One dynamic major device number is allocated for each device and minor
numbers are allocated for the ports contained within that device.
The file operation for the char devs will be added in the following
commits.

This commit adds a new feature, MULTIPORT. If the host supports this
feature as well, the config space has the number of ports defined for
that device. New ports are spawned according to this information.
The config space also has the maximum number of ports that can be
spawned for a particular device. This is useful in initializing the
appropriate number of virtqueues in advance, as ports might be
hot-plugged in later.
Using this feature, generic ports can be created which are not tied to
hvc consoles.
We also open up a private channel between the host and the guest via
which some "control" messages are exchanged for the ports, like whether
the port being spawned is a console port, resizing the console window,
etc.
Next commits will add support for hotplugging and presenting char
devices in /dev/ for bi-directional guest-host communication.

Console ports could be hot-added. Also, with the new multiport support,
a port is identified as a console port only if the host sends a control
message.
Move the console port init into a separate function so it can be invoked
from other places.

Move out console-specific stuff into a separate struct from 'struct
port' as we need to maintain two lists: one for all the ports (which
includes consoles) and one only for consoles since the hvc callbacks
only give us the vtermno.
This makes console handling cleaner.

The ChangeLog file under drivers/char is 30K of stuff dedicated to the
mid-90's TTY exploits of Ted Ts'o; it has been updated once since 1998
- and that was in 2001. It's interesting history, but we don't
normally carry that kind of history inline with the code. Let's remove
it.

When suspending, tpm_infineon calls the generic suspend function of the
TPM framework. However, the TPM framework does not return and the system
hangs upon suspend. When sending the necessary command "TPM_SaveState"
directly within the driver, suspending and resuming works fine.

This reverts commit 7036251180 ("tty: fix race in tty_fasync") and
commit b04da8bfdf ("fnctl: f_modown should call write_lock_irqsave/
restore") that tried to fix up some of the fallout but was incomplete.
It turns out that we really cannot hold 'tty->ctrl_lock' over calling
__f_setown, because not only did that cause problems with interrupt
disables (which the second commit fixed), it also causes a potential
ABBA deadlock due to lock ordering.
Thanks to Tetsuo Handa for following up on the issue, and running
lockdep to show the problem. It goes roughly like this:
- f_getown gets filp->f_owner.lock for reading without interrupts
disabled, so an interrupt that happens while that lock is held can
cause a lockdep chain from f_owner.lock -> sighand->siglock.
- at the same time, the tty->ctrl_lock -> f_owner.lock chain that
commit 7036251180 introduced, together with the pre-existing
sighand->siglock -> tty->ctrl_lock chain means that we have a lock
dependency the other way too.
So instead of extending tty->ctrl_lock over the whole __f_setown() call,
we now just take a reference to the 'pid' structure while holding the
lock, and then release it after having done the __f_setown. That still
guarantees that 'struct pid' won't go away from under us, which is all
we really ever needed.
