from the pci ehci driver. Without this patch we get a compile error if

Hub-initiated LPM is not good for USB communications devices. Comms
devices should be able to tell when their link can go into a lower power
state, because they know when an incoming transmission is finished.
Ideally, these devices would slam their links into a lower power state,
using the device-initiated LPM, after finishing the last packet of their
data transfer.
If we enable the idle timeouts for the parent hubs to enable
hub-initiated LPM, we will get a lot of useless LPM packets on the bus
as the devices reject LPM transitions when they're in the middle of
receiving data. Worse, some devices might blindly accept the
hub-initiated LPM and power down their radios while they're in the
middle of receiving a transmission.
The Intel Windows folks are disabling hub-initiated LPM for all USB
communications devices under a xHCI USB 3.0 host. In order to keep
the Linux behavior as close as possible to Windows, we need to do the
same in Linux.
Set the disable_hub_initiated_lpm flag for for all USB communications
drivers. I know there aren't currently any USB 3.0 devices that
implement these class specifications, but we should be ready if they do.

All Intel xHCI host controllers support USB 3.0 Link Power Management.
The Panther Point xHCI host controller needs the xHCI driver to
calculate the U1 and U2 timeout values, because it will blindly accept a
MEL that would cause scheduling issues.
The Lynx Point xHCI host controller will reject MEL values that are too
high, but internally it implements the same algorithm that is needed for
Panther Point xHCI.
Simplify the code paths by just having the xHCI driver calculate what
the U1/U2 timeouts should be. Comments on the policy are in the code.

The choice of U1 and U2 timeouts for USB 3.0 Link Power Management (LPM)
is highly host controller specific. Here are a few examples of why it's
host specific:
1. Setting the U1/U2 timeout too short may cause the link to go into
U1/U2 in between service intervals, which some hosts may tolerate,
and some may not.
2. The host controller has to modify its bus schedule in order to take
into account the Maximum Exit Latency (MEL) to bring all the links
from the host to the device into U0. If the MEL is too big, and it
takes too long to bring the links into an active state, the host
controller may not be able to service periodic endpoints in time.
3. Host controllers may also have scheduling limitations that force
them to disable U1 or U2 if a USB device is behind too many tiers of
hubs.
We could take an educated guess at what U1/U2 timeouts may work for a
particular host controller. However, that would result in a binary
search on every new configuration or alt setting installation, with
multiple failed Evaluate Context commands. Worse, the host may blindly
accept the timeouts and just fail to update its schedule for U1/U2 exit
latencies, which could result in randomly delayed periodic transfers.
Since we don't want to cause jitter in periodic transfers, or delay
config/alt setting changes too much, lay down a framework that xHCI
vendors can extend in order to add their own U1/U2 timeout policies.
To extend the framework, they will need to:
- Modify the PCI init code to add a new xhci->quirk for their host, and
set the XHCI_LPM_SUPPORT quirk flag.
- Add their own vendor-specific hooks, like the ones that will be added
in xhci_call_host_update_timeout_for_endpoint() and
xhci_check_tier_policy()
- Make the LPM enable/disable methods call those functions based on the
xhci->quirk for their host.
An example will be provided for the Intel xHCI host controller in the
next patch.

We want to do everything we can to ensure that USB 3.0 Link Power
Management (LPM) can be disabled when it is enabled. If LPM can't be
disabled, we can't suspend USB 3.0 devices, or reset them. To make sure
we can submit the command to disable LPM, allocate a command in the
xhci_hcd structure, and reserve one TRB on the command ring.
We only need one command per xHCI driver instance, because LPM is only
disabled or enabled while the USB core is holding the bandwidth_mutex
that is shared between the xHCI USB 2.0 and USB 3.0 roothubs. The
bandwidth_mutex will be held until the command completes, or times out.

The upcoming USB 3.0 Link PM patches will introduce new API to enable
and disable low-power link states. We must be able to disable LPM in
order to reset a device, or place the device into U3 (device suspend).
Therefore, we need to make sure the Evaluate Context command to disable
the LPM timeouts can't fail due to there being no room on the command
ring.
Introduce a new flag to the function that queues the Evaluate Context
command, command_must_succeed. This tells the ring handler that a TRB
has already been reserved for the command (by incrementing
xhci->cmd_ring_reserved_trbs), and basically ensures that prepare_ring()
won't fail. A similar flag was already implemented for the Configure
Endpoint command queuing function.
All functions that currently call xhci_configure_endpoint() to issue an
Evaluate Context command pass "false" for the "must_succeed" parameter,
so this patch should have no effect on current xHCI driver behavior.

There are several places where the USB core needs to disable USB 3.0
Link PM:
- usb_bind_interface
- usb_unbind_interface
- usb_driver_claim_interface
- usb_port_suspend/usb_port_resume
- usb_reset_and_verify_device
- usb_set_interface
- usb_reset_configuration
- usb_set_configuration
Use the new LPM disable/enable functions to temporarily disable LPM
around these critical sections.
We need to protect the critical section around binding and unbinding USB
interface drivers. USB drivers may want to disable hub-initiated USB
3.0 LPM, which will change the value of the U1/U2 timeouts that the xHCI
driver will install. We need to disable LPM completely until the driver
is bound to the interface, and the driver has a chance to enable
whatever alternate interface setting it needs in its probe routine.
Then re-enable USB3 LPM, and recalculate the U1/U2 timeout values.
We also need to disable LPM in usb_driver_claim_interface,
because drivers like usbfs can bind to an interface through that
function. Note, there is no way currently for userspace drivers to
disable hub-initiated USB 3.0 LPM. Revisit this later.
When a driver is unbound, the U1/U2 timeouts may change because we are
unbinding the last driver that needed hub-initiated USB 3.0 LPM to be
disabled.
USB LPM must be disabled when a USB device is going to be suspended.
The USB 3.0 spec does not define a state transition from U1 or U2 into
U3, so we need to bring the device into U0 by disabling LPM before we
can place it into U3. Therefore, call usb_unlocked_disable_lpm() in
usb_port_suspend(), and call usb_unlocked_enable_lpm() in
usb_port_resume(). If the port suspend fails, make sure to re-enable
LPM by calling usb_unlocked_enable_lpm(), since usb_port_resume() will
not be called on a failed port suspend.
USB 3.0 devices lose their USB 3.0 LPM settings (including whether USB
device-initiated LPM is enabled) across device suspend. Therefore,
disable LPM before the device will be reset in
usb_reset_and_verify_device(), and re-enable LPM after the reset is
complete and the configuration/alt settings are re-installed.
The calculated U1/U2 timeout values are heavily dependent on what USB
device endpoints are currently enabled. When any of the enabled
endpoints on the device might change, due to a new configuration, or new
alternate interface setting, we need to first disable USB 3.0 LPM, add
or delete endpoints from the xHCI schedule, install the new interfaces
and alt settings, and then re-enable LPM. Do this in usb_set_interface,
usb_reset_configuration, and usb_set_configuration.
Basically, there is a call to disable and then enable LPM in all
functions that lock the bandwidth_mutex. One exception is
usb_disable_device, because the device is disconnecting or otherwise
going away, and we should not care about whether USB 3.0 LPM is enabled.

There are various functions within the USB core that will need to
disable USB 3.0 link power states. For example, when a USB device
driver is being bound to an interface, we need to disable USB 3.0 LPM
until we know if the driver will allow hub-initiated LPM transitions.
Another example is when the USB core is switching alternate interface
settings. The USB 3.0 timeout values are dependent on what endpoints
are enabled, so we want to ensure that LPM is disabled until the new alt
setting is fully installed.
Multiple functions need to disable LPM, and those functions can even be
nested. For example, usb_bind_interface() could disable LPM, and then
call into the driver probe function, which may attempt to switch to a
different alt setting. Therefore, we need to keep a count of the number
of functions that require LPM to be disabled at any point in time.
Introduce two new USB core API calls, usb_disable_lpm() and
usb_enable_lpm(). These functions increment and decrement a new
variable in the usb_device, lpm_disable_count. If usb_disable_lpm()
fails, it will call usb_enable_lpm() in order to balance the
lpm_disable_count.
These two new functions must be called with the bandwidth_mutex locked.
If the bandwidth_mutex is not already held by the caller, it should
instead call usb_unlocked_disable_lpm() and usb_enable_lpm(), which take
the bandwidth_mutex before calling usb_disable_lpm() and
usb_enable_lpm(), respectively.
Introduce a new variable (timeout) in the usb3_lpm_params structure to
keep track of the currently enabled U1/U2 timeout values. When
usb_disable_lpm() is called, and the USB device has the U1 or U2
timeouts set to a non-zero value (meaning either device-initiated or
hub-initiated LPM is enabled), attempt to disable LPM, regardless of the
state of the lpm_disable_count. We want to ensure that all callers can
be guaranteed that LPM is disabled if usb_disable_lpm() returns zero.
Otherwise the following scenario could occur:
1. Driver A is being bound to interface 1. usb_probe_interface()
disables LPM. Driver A doesn't care if hub-initiated LPM is enabled, so
even though usb_disable_lpm() fails, the probe of the driver continues,
and the bandwidth mutex is dropped.
2. Meanwhile, Driver B is being bound to interface 2.
usb_probe_interface() grabs the bandwidth mutex and calls
usb_disable_lpm(). That call should attempt to disable LPM, even
though the lpm_disable_count is set to 1 by Driver A.
For usb_enable_lpm(), we attempt to enable LPM only when the
lpm_disable_count is zero. If some step in enabling LPM fails, it will
only have a minimal impact on power consumption, and all USB device
drivers should still work properly. Therefore don't bother to return
any error codes.
Don't enable device-initiated LPM if the device is unconfigured. The
USB device will only accept the U1/U2_ENABLE control transfers in the
configured state. Do enable hub-initiated LPM in that case, since
devices are allowed to accept the LGO_Ux link commands in any state.
Don't enable or disable LPM if the device is marked as not being LPM
capable. This can happen if:
- the USB device doesn't have a SS BOS descriptor,
- the device's parent hub has a zeroed bHeaderDecodeLatency value, or
- the xHCI host doesn't support LPM.

There are several different exit latencies associated with coming out of
the U1 or U2 lower power link state.
Device Exit Latency (DEL) is the maximum time it takes for the USB
device to bring its upstream link into U0. That can be found in the
SuperSpeed Extended Capabilities BOS descriptor for the device. The
time it takes for a particular link in the tree to exit to U0 is the
maximum of either the parent hub's U1/U2 DEL, or the child's U1/U2 DEL.
Hubs introduce a further delay that effects how long it takes a child
device to transition to U0. When a USB 3.0 hub receives a header
packet, it takes some time to decode that header and figure out which
downstream port the packet was destined for. If the port is not in U0,
this hub header decode latency will cause an additional delay for
bringing the child device to U0. This Hub Header Decode Latency is
found in the USB 3.0 hub descriptor.
We can use DEL and the header decode latency, along with additional
latencies imposed by each additional hub tier, to figure out the exit
latencies for both host-initiated and device-initiated exit to U0.
The Max Exit Latency (MEL) is the worst-case time it will take for a
host-initiated exit to U0, based on whether U1 or U2 link states are
enabled. The ping or packet must traverse the path to the device, and
each hub along the way incurs the hub header decode latency in order to
figure out which device the transfer was bound for. We say worst-case,
because some hubs may not be in the lowest link state that is enabled.
See the examples in section C.2.2.1.
Note that "HSD" is a "host specific delay" that the power appendix
architect has not been able to tell me how to calculate. There's no way
to get HSD from the xHCI registers either, so I'm simply ignoring it.
The Path Exit Latency (PEL) is the worst-case time it will take for a
device-initiate exit to U0 to place all the links from the device to the
host into U0.
The System Exit Latency (SEL) is another device-initiated exit latency.
SEL is useful for USB 3.0 devices that need to send data to the host at
specific intervals. The device may send an NRDY to indicate it isn't
ready to send data, then put its link into a lower power state. If it
needs to have that data transmitted at a specific time, it can use SEL
to back calculate when it will need to bring the link back into U0 to
meet its deadlines.
SEL is the worst-case time from the device-initiated exit to U0, to when
the device will receive a packet from the host controller. It includes
PEL, the time it takes for an ERDY to get to the host, a host-specific
delay for the host to process that ERDY, and the time it takes for the
packet to traverse the path to the device. See Figure C-2 in the USB
3.0 bus specification.
Note: I have not been able to get good answers about what the
host-specific delay to process the ERDY should be. The Intel HW
developers say it will be specific to the platform the xHCI host is
integrated into, and they say it's negligible. Ignore this too.
Separate from these four exit latencies are the U1/U2 timeout values we
program into the parent hubs. These timeouts tell the hub to attempt to
place the device into a lower power link state after the link has been
idle for that amount of time.
Create two arrays (one for U1 and one for U2) to store mel, pel, sel,
and the timeout values. Store the exit latency values in nanosecond
units, since that's the smallest units used (DEL is in us, but the Hub
Header Decode Latency is in ns).
If a USB 3.0 device doesn't have a SuperSpeed Extended Capabilities BOS
descriptor, it's highly unlikely it will be able to handle LPM requests
properly. So it's best to disable LPM for devices that don't have this
descriptor, and any children beneath it, if it's a USB 3.0 hub. Warn
users when that happens, since it means they have a non-compliant USB
3.0 device or hub.
This patch assumes a simplified design where links deep in the tree will
not have U1 or U2 enabled unless all their parent links have the
corresponding LPM state enabled. Eventually, we might want to allow a
different policy, and we can revisit this patch when that happens.

Refactor the code that sets the usb_device flag to indicate the device
support link power management (lpm_capable). The current code sets
lpm_capable unconditionally if the USB devices have a USB 2.0 Extended
Capabilities Descriptor. USB 3.0 devices can also have that descriptor,
but the xHCI driver code that uses lpm_capable will not run the USB 2.0
LPM test for devices under the USB 3.0 roothub. Therefore, it's fine
only set lpm_capable for high speed devices in this refactoring.

The BOS descriptor is normally fetched and stored in the usb_device->bos
during enumeration. USB 3.0 roothubs don't undergo enumeration, but we
need them to have a BOS descriptor, since each xHCI host has a different
U1 and U2 exit latency. Make sure to fetch the BOS descriptor for USB
3.0 roothubs. It will be freed when the roothub usb_device is released.

USB 3.0 hubs can be put into a mode where the hub can automatically
request that the link go into a deeper link power state after the link
has been idle for a specified amount of time. Each of the new USB 3.0
link states (U1 and U2) have their own timeout that can be programmed
per port.
Change the xHCI roothub emulation code to handle the request to set the
U1 and U2 timeouts.

When the xHCI driver needs to clean up memory (perhaps due to a failed
register restore on resume from S3 or resume from S4), it needs to reset
the number of reserved TRBs on the command ring to zero. Otherwise,
several resume cycles (about 30) with a UAS device attached will
continually increment the number of reserved TRBs, until all command
submissions fail because there isn't enough room on the command ring.
This patch should be backported to kernels as old as 2.6.32,
that contain the commit 913a8a344f

Using kfifos for ID management was limiting the number of QPs and
preventing NP384 MPI jobs. So replace it with a simple bitmap
allocator.
Remove IDs from the IDR tables before deallocating them. This bug was
causing the BUG_ON() in insert_handle() to fire because the ID was
getting reused before being removed from the IDR table.

Add module option db_fc_threshold which is the count of active QPs
that trigger automatic db flow control mode. Automatically transition
to/from flow control mode when the active qp count crosses
db_fc_theshold.

Get FULL/EMPTY/DROP events from LLD. On FULL event, disable normal
user mode DB rings.
Add modify_qp semantics to allow user processes to call into the
kernel to ring doobells without overflowing.
Add DB Full/Empty/Drop stats.
Mark queues when created indicating the doorbell state.
If we're in the middle of db overflow avoidance, then newly created
queues should start out in this mode.
Bump the C4IW_UVERBS_ABI_VERSION to 2 so the user mode library can
know if the driver supports the kernel mode db ringing.

recover LLD EQs for DB drop interrupts. This includes adding a new
db_lock, a spin lock disabling BH too, used by the recovery thread and
the ring_tx_db() paths to allow db drop recovery.
Clean up initial DB avoidance code.
Add read_eq_indices() - this allows the LLD to use the PCIe mw to
efficiently read hw eq contexts.
Add cxgb4_sync_txq_pidx() - called by iw_cxgb4 to sync up the sw/hw
pidx value.
Add flush_eq_cache() and cxgb4_flush_eq_cache(). This allows iw_cxgb4
to flush the sge eq context cache before beginning db drop recovery.
Add module parameter, dbfoifo_int_thresh, to allow tuning the db
interrupt threshold value.
Add dbfifo_int_thresh to cxgb4_lld_info so iw_cxgb4 knows the threshold.
Add module parameter, dbfoifo_drain_delay, to allow tuning the amount
of time delay between DB FULL and EMPTY upcalls to iw_cxgb4.

Add platform-specific callback functions for interrupts. This is
needed to do a single read-clear of the CAUSE register and then call
out to platform specific functions for DB threshold interrupts and DB
drop interrupts.
Add t4_mem_win_read_len() - mem-window reads for arbitrary lengths.
This is used to read the CIDX/PIDX values from EC contexts during DB
drop recovery.
Add t4_fwaddrspace_write() - sends addrspace write cmds to the fw.
Needed to flush the sge eq context cache.

As iwlwifi use fat skbs, it should not pull too much data in skb->head,
and particularly no tcp data payload, or splice() is slower, and TCP
coalescing is disabled. Copying payload to userland also involves at
least two copies (part from header, part from fragment)
Each layer will pull its header from the fragment as needed.
(on 64bit arches, skb_tailroom(skb) at this point is 192 bytes)
With this patch applied, I have a major reduction of collapsed/pruned
TCP packets, a nice increase of TCPRcvCoalesce counter, and overall
better Internet User experience.
Small packets are still using a fragless skb, so that page can be reused
by the driver.

Palmas is a PMIC from Texas Instruments and this is the MFD part of the
driver for this chip. The PMIC has SMPS and LDO regulators, a general
purpose ADC, GPIO, USB OTG mode detection, watchdog and RTC features.

regulator_map_voltage_iterate() is for drivers implementing set_voltage_sel()
and list_voltage() to use it as their map_voltage() operation.
In this case, regulator_map_voltage_iterate() happen to be doing the same thing
as ab3100_get_best_voltage_index() function. So we can use it to replace
ab3100_get_best_voltage_index() function.

Here we setup the ab8500 regulator driver for DT. We first do
this in the normal way, by providing a match structure during
initialisation, but then we provide information so that
whilst probing we can use existing data structures to do DT
look-ups.

ab8500's probe() function is becoming quite large, so in the lead
up to Device Tree enablement which will fork the thread of execution
this patch splits it into 3 main areas; basic error checking will
remain in probe(), but regulator register initialisation and regulator
registration have been moved to their own functions which will
be called in sequence by probe() and the DT equivalent.

The reset date of the ST Micro version of PL031 is 2000-01-01. The
correct weekday for 2000-01-01 is saturday, but pl031 is initialized to
sunday. This may lead to alarm malfunction, so configure the correct
wday if RTC_DR indicates reset.

Pull two networking fixes from David S. Miller:
1) Thanks to Willy Tarreau and Eric Dumazet, we've unlocked a bug that's
been present in do_tcp_sendpages() since that function was written in
2002.
When we block to wait for memory we have to unconditionally try and
push out pending TCP data, otherwise we can block for an unreasonably
long amount of time.
2) Fix deadlock in e1000, fixes kernel bugzilla 43132

tcp: do_tcp_sendpages() must try to push data out on oom conditions
Commit 1cc0c998fd ("ACPI: Fix D3hot v D3cold confusion") introduced a
bug in __acpi_bus_set_power() and changed the behavior of
acpi_pci_set_power_state() in such a way that it generally doesn't work
as expected if PCI_D3hot is passed to it as the second argument.
First off, if ACPI_STATE_D3 (equal to ACPI_STATE_D3_COLD) is passed to
__acpi_bus_set_power() and the explicit_set flag is set for the D3cold
state, the function will try to execute AML method called "_PS4", which
doesn't exist.
Fix this by adding a check to ensure that the name of the AML method
to execute for transitions to ACPI_STATE_D3_COLD is correct in
__acpi_bus_set_power(). Also make sure that the explicit_set flag
for ACPI_STATE_D3_COLD will be set if _PS3 is present and modify
acpi_power_transition() to avoid accessing power resources for
ACPI_STATE_D3_COLD, because they don't exist.
Second, if PCI_D3hot is passed to acpi_pci_set_power_state() as the
target state, the function will request a transition to
ACPI_STATE_D3_HOT instead of ACPI_STATE_D3. However,
ACPI_STATE_D3_HOT is now only marked as supported if the _PR3 AML
method is defined for the given device, which is rare. This causes
problems to happen on systems where devices were successfully put
into ACPI D3 by pci_set_power_state(PCI_D3hot) which doesn't work
now. In particular, some unused graphics adapters are not turned
off as a result.
To fix this issue restore the old behavior of
acpi_pci_set_power_state(), which is to request a transition to
ACPI_STATE_D3 (equal to ACPI_STATE_D3_COLD) if either PCI_D3hot or
PCI_D3cold is passed to it as the argument.
This approach is not ideal, because generally power should not
be removed from devices if PCI_D3hot is the target power state,
but since this behavior is relied on, we have no choice but to
restore it at the moment and spend more time on designing a
better solution in the future.

The use of the inw/outw functions by the cs89x0 platform driver
results in NULL pointer references on ARM platforms and
platforms that do not provide ISA-style programmed I/O accessors.
Using inw/outw also accesses the wrong address space on platforms
that have a PCI I/O space that is not identity-mapped into the
physical address space.

This patch is V2 of the Emma Mobile GPIO driver. This
driver is designed to be reusable between multiple SoCs
that share the same basic building block, but so far it
has only been used on Emma Mobile EV2.
Each driver instance handles 32 GPIOs with individually
maskable IRQs. The driver operates on two I/O memory
ranges and the 32 GPIOs are hooked up to two interrupts.
In the case of Emma Mobile EV2 this GPIO building block
is used as main external interrupt controller hooking up
159 GPIOS as 159 interrupts via 5 driver instances and
10 interrupts to the GIC and the Cortex-A9 Dual.

The RNC state machine would incorrectly transition from
SCI_RNC_AWAIT_SUSPENSION directly to SCI_RNC_INVALIDATING when a destruct
request was made. This would skip the increment of the suspension count
and the abort of pending TCs (although the invalidating state would at
least cleanup outstanding TCs).
Instead, the RNC will transition to SCI_RNC_SUSPENDED and then start the
destruction process.

In the link fail path, set IDEV_GONE for every device on the domain
when the last link in the port fails.
In the abort path functions like isci_reset_device, make sure that
there has not already been a detected domain failure with the device
by checking IDEV_GONE, before performing any kind of hard reset, SMP
phy control, or TMF operation.
The check for IDEV_GONE makes sure that the device in the abort path
really has control of the port with which it is associated. This
prevents starting hard resets at incorrect times and scheduling
unnecessary LUN resets for SATA devices.

There is an apparent HW lockup caused when the PE is disabled while there
is an outstanding TC in progress. This change puts the link into OOB to
force the TC to end before the PE is disabled.

This change adds timeouts to the RNC suspension wait. It makes the
suspend and resume timeouts the same.
The previous resume timeout of 5 ms was too short, and timeouts were
seen in resumptions of devices in the abort task/LUN reset path - which
would receive an RNC resumed message within a tenth of a second later.

Completion of I/Os during the one of the abort path interface calls
from libsas can drive remote device state changes and the resumption
of the device RNC. This is a problem when the abort path is
attempting to cleanup outstanding I/O at the same time - the resumption
can prevent the termination from occuring correctly.

In the libsas error path, SATA disks require extra handling in
libata to recover operation. However, libsas expects to be able
to immediately recover all outstanding I/O once the error handler
escalation stops. This patch fixes the condition where the libata
error handler is scheduled for operation but libsas has already
deleted the outstanding sas_tasks.

The RNC can be any of the states in the loop from suspended to
ready when the API "suspend" or "resume" are called. This change
adds destination states parameters that control the suspension /
resumption action of the RNC statemachine for those transition states.

This commit changes the means by which outstanding I/Os are handled
for cleanup.
The likelihood is that this commit will be broken into smaller pieces,
however that will be a later revision. Among the changes:
- All completion structures have been removed from the tmf and
abort paths.
- Now using one completed I/O list, with the I/O completed in host bit being
used to select error or normal callback paths.

Add comprehensive decode for all TC completions that generate RNC
suspensions.
Note that this commit also removes unconditional resumptions of ATAPI
devices when in the SCI_STP_DEV_ATAPI_ERROR state, and STP devices
when in the SCI_STP_DEV_IDLE state. This is because the SCI_STP_DEV_IDLE
and SCI_STP_DEV_ATAPI state entry functions manage the RNC resumption.

For STP devices under certain protocol conditions, an RNC will not
suspend until the current transfer state is broken with a SYNC/ESC
sequence from the SCU. The SYNC/ESC driven by expiration of the
SCU link layer hang detect timer, which has too small a dynamic
range to support slow SATA devices, so normally it is disabled.
This change enables the timer with the minimum period at the point
when the suspension is requested.
Note that there is potential collateral damage to other open
connections to slow SATA devices on the same port, since there
is no alternative but to enable the LLHANG timer on every phy in
the port for the current suspension request - there is no way to
tell on which phy the RNC in question is currently active.

The support for CONFIG_MCA is being removed, since the 20
year old hardware simply isn't capable of meeting today's
software demands on CPU and memory resources.
This commit removes any MCA specific net drivers, and removes
any MCA specific probe/support code from drivers that were
doing a dual ISA/MCA role.

The serial4 controller on bf60x is an enhanced version of serial controller
on bf5xx. MMR size is 32 bits other than 16 bits. MMR GCTL, MCR and LCR are
combined into one control MMR. MSR and LSR are combined into one status MMR.
This patch adapts current bf5xx serial driver to serial4 controller on bf60x.

Change the TX ring scheme such that the number of rings for untagged packets
and for tagged packets (per each of the vlan priorities) is the same, unlike

Queue selection is done as follows:
If the mqprio qdisc is operates on the interface, such that the core networking
code invoked the device setup_tc ndo callback, a mapping of skb->priority =>
queue set is forced - for both, tagged and untagged traffic.
Else, the egress map skb->priority => User priority is used for tagged traffic, and

OEM parameters [1] are parsed from the platform option-rom / efi
driver. By default the driver was validating the parameters for the
dual-controller case, but in single-controller case only the first set
of parameters may be valid.
Limit the validation to the number of actual controllers detected
otherwise the driver may fail to parse the valid parameters leading to
driver-load or runtime failures.

There is a (dubious?) lost irq workaround in sci_controller_isr() that
effectively nullifies attempts to disable interrupts. Until the
workaround can be re-evaluated add some infrastructure to prevent the
interrupt handler from inadvertantly re-enabling interrupts.
The failure mode was interrupts continuing to run after the driver had
been removed and its iomappings torn down.

The call to wait_for_start() is meant to ensure that all links have been
given a chance to come up before letting the kernel proceed with
probing. However, the implementation is not correctly syncing with the
port configuration agent. In the MPC case the ports are hard-coded, in
the APC case we need to wait for the port-configuration to form ports
from the started phys.
Towards that end increase the timeout for the APC agent to form ports,
and delay start complete until all phys are out of link-training.

Based on an original implementation by Ed Nadolski and Artur Wojcik
In preparation for S3/S4 support refactor initialization so that
driver-load and resume-from-suspend can share the common init path of
isci_host_init(). Organize the initialization into objects that are
self-contained to the driver (initialized by isci_host_init) versus
those that have some upward registration (initialized at allocation time
asd_sas_phy, asd_sas_port, dma allocations). The largest change is
moving the the validation of the oem and module parameters from
isci_host_init() to isci_host_alloc().
The S3/S4 approach being taken is that libsas will be tasked with
remembering the state of the domain and the lldd is free to be
forgetful. In the case of isci we'll just re-init using a subset of the
normal driver load path.

Per the SAS spec, several types of BROADCAST CHANGE primitives
must cause re-discovery of the originating expander.
Only the standard BROADCAST CHANGE primitive was being
sent to the LIBSAS layer. The other BC primitives have been
added to the sci_phy_event_handler()

Neither "acceptor_values" nor "proposer_values" can be NULL here when
scanning the value lists for incoming iSCSI login parameters such as
HeaderDigest=CRC32C,None.
Smatch complains because we are not allowed to pass NULL pointers to
strchr(). Also I removed a second later check for "!acceptor_values"
because it gets checked on the next line in the do while condition.

This patch fixes a bug in the handling of FILEIO w/ underlying block_device
resize operations where the original fd_dev->fd_dev_size was incorrectly being
used in fd_get_blocks() for READ_CAPACITY response payloads.
This patch avoids using fd_dev->fd_dev_size for FILEIO devices with
an underlying block_device, and instead changes fd_get_blocks() to
get the sector count directly from i_size_read() as recommended by hch.

Currently, 'res->flags' handlings are wrong in three respects:
* the driver _modifies_ the contents of platform data
* res->flags is set up, but not used anywhere in the driver
* request_irq() always takes VBUS_IRQ_FLAGS, regardless of refs->flags
This patch tries to fix this with a policy: If a platform IRQ resource
is available, give preference to its IRQ flag(s) over a default one
(VBUS_IRQ_FLAGS).

gpio_vbus is designed to be able to get an IRQ number for VBUS change
interrupt either (1) through platform_get_resource(IORESOURCE_IRQ) or
(2) by processing gpio_to_irq(pdata->gpio_vbus), in probe() function.
On the other hand, gpio_vbus_set_peripheral() and gpio_vbus_remove()
are always doing gpio_to_irq(pdata->gpio_vbus) to get an IRQ number.
This is not just inconsistent, but also broken. There is no guarantee
that an IRQ number obtained by platform_get_resource() is equal to
gpio_to_irq(pdata->gpio_vbus).
Cache an IRQ number in probe() function, and use it where necessary.

USB2 LPM is disabled when device begin to suspend and enabled after device
is resumed. That's because USB spec does not define the transition from
U1/U2 state to U3 state.
If usb_port_suspend() fails, usb_port_resume() is never called, and USB2 LPM
is disabled in this situation. Enable USB2 LPM if port suspend fails.
This patch should be backported to kernels as old as 3.2, that contain
the commit 65580b4321 "xHCI: set USB2
hardware LPM".

Sergio reported that when he recorded audio from a USB headset mic
plugged into the USB 3.0 port on his ASUS N53SV-DH72, the audio sounded
"robotic". When plugged into the USB 2.0 port under EHCI on the same
laptop, the audio sounded fine. The device is:
Bus 002 Device 004: ID 046d:0a0c Logitech, Inc. Clear Chat Comfort USB Headset
The problem was tracked down to the Fresco Logic xHCI host controller
not correctly reporting short transfers on isochronous IN endpoints.
The driver would submit a 96 byte transfer, the device would only send
88 or 90 bytes, and the xHCI host would report the transfer had a
"successful" completion code, with an untransferred buffer length of 8
or 6 bytes.
The successful completion code and non-zero untransferred length is a
contradiction. The xHCI host is supposed to only mark a transfer as
successful if all the bytes are transferred. Otherwise, the transfer
should be marked with a short packet completion code. Without the EHCI
bus trace, we wouldn't know whether the xHCI driver should trust the
completion code or the untransferred length. With it, we know to trust
the untransferred length.
Add a new xHCI quirk for the Fresco Logic host controller. If a
transfer is reported as successful, but the untransferred length is
non-zero, print a warning. For the Fresco Logic host, change the
completion code to COMP_SHORT_TX and process the transfer like a short
transfer.
This should be backported to stable kernels that contain the commit
f5182b4155 "xhci: Disable MSI for some
Fresco Logic hosts." That commit was marked for stable kernels as old
as 2.6.36.

Add support for mmap'ing buffers via dmabuf. For handling mmap of
cached buffers correctly, fault handling and PTE shootdown are used
to track dirty pages and automagically handle cache flushes before
dma access to the buffer.

This is a first step for adding ECC support for pstore RAM backend: we
will use the persistent_ram routines, kindly provided by Google.
Basically, persistent_ram is a set of helper routines to deal with the
[optionally] ECC-protected persistent ram regions.
A bit of Makefile, Kconfig and header files adjustments were needed
because of the move.

Under certain scenarios, it's possible that bursty manageability traffic
over the BMC-to-OS path may overrun the internal manageability receive
buffer causing dropped manageability packets. Clearing this bit prevents
this situation by interrupting coalescing to allow manageability traffic
through.

The code seems to want to look at the last byte where the HW puts some
information. Since the skb->data area is never seen by the HW I guess it
does not work as expected. We pass the page address to the HW so I
*think* in order to get to the last byte where the information might be
one should use the page buffer and take a look.
This is of course not more than just compile tested.

It's been broken forever (i.e. it's not scheduling in a power
aware fashion), as reported by Suresh and others sending
patches, and nobody cares enough to fix it properly ...
so remove it to make space free for something better.
There's various problems with the code as it stands today, first
and foremost the user interface which is bound to topology
levels and has multiple values per level. This results in a
state explosion which the administrator or distro needs to
master and almost nobody does.
Furthermore large configuration state spaces aren't good, it
means the thing doesn't just work right because it's either
under so many impossibe to meet constraints, or even if
there's an achievable state workloads have to be aware of
it precisely and can never meet it for dynamic workloads.
So pushing this kind of decision to user-space was a bad idea
even with a single knob - it's exponentially worse with knobs
on every node of the topology.
There is a proposal to replace the user interface with a single
3 state knob:
sched_balance_policy := { performance, power, auto }
where 'auto' would be the preferred default which looks at things
like Battery/AC mode and possible cpufreq state or whatever the hw
exposes to show us power use expectations - but there's been no
progress on it in the past many months.
Aside from that, the actual implementation of the various knobs
is known to be broken. There have been sporadic attempts at
fixing things but these always stop short of reaching a mergable
state.
Therefore this wholesale removal with the hopes of spurring
people who care to come forward once again and work on a
coherent replacement.

According to the comment, errata 23 says that the memory we allocate
can't cross a 64KiB boundary. In case of jumbo frames we allocate
complete pages which can never cross the 64KiB boundary because
PAGE_SIZE should be a multiple of 64KiB so we stop either before the
boundary or start after it but never cross it. Furthermore the check
seems bogus because it looks at skb->data which is not seen by the HW
at all because we only pass the DMA address of the page we allocated. So
I *think* the workaround is not required here.

__napi_schedule might raise softirq but nothing
causes do_softirq to trigger, so it does not in fact
run. As a result,
the error message "NOHZ: local_softirq_pending 08"
sometimes occurs during boot of a KVM guest when the network service is
started and we are oom:

Further, receive queue processing might get delayed
indefinitely until some interrupt triggers:
virtio_net expected napi to be run immediately.
One way to cause do_softirq to be executed is by
invoking local_bh_enable(). As __napi_schedule is
normally called from bh or irq context, this
seems to make sense: disable bh before __napi_schedule
and enable afterwards.
In fact it's a very complicated way of calling do_softirq(),
and works since this function is only used when we are not
in interrupt context. It's not hot at all, in any ideal scenario.

When the balloon module is removed, we deflate the balloon, reclaiming
all the pages that were given to the host. However, we don't update the
config values for the new balloon size, resulting in the host showing
outdated balloon values.
The size update is done after each leak and fill operation, only the
module removal case was left out.

If a port was open before going into one of the sleep states, the port
can continue normal operation after restore. However, the host has to
be told that the guest side of the connection is open to restore
pre-suspend state.
This wasn't noticed so far due to a bug in qemu that was fixed recently
(which marked the guest-side connection as always open).

With the exception of the detached field, sg_mutex no longer adds any
locking. detached handling has been broken before and is still broken
and this patch does not seem to make things worse than they were to
begin with.
However, I have observed cases of tasks being blocked for >200s waiting
for sg_mutex. So the removal clearly adds value for very little cost.

Changes since v1: set_exclude now returns the new value, which gets
rid of the comma expression and the operator precedence bug. Thanks
to Douglas for spotting it.
sdp->exclude was previously protected by the BKL. The sg_mutex, which
replaced the BKL, only semi-protected it, as it was missing from
sg_release() and sg_proc_seq_show_debug(). Take an explicit spinlock
for it.

srp->done is protected by sfp->rq_list_lock everywhere, except for this
one case. Result can be that the wake-up happens before the cacheline
with the changed srp->done has arrived, so the waiter can go back to
sleep and never be woken up again.
The wait_event_interruptible() means that anyone trying to debug this
unlikely race will likely notice everything working fine again, as the
next signal will unwedge things. Evil.

This patch adds support for ALUA MI_REPORT_TARGET_PGS extended header
format defined within SPC-4. It changes target core ALUA emulation logic
within target_emulate_report_target_port_groups() to support both the
extended and original length only header formats.
It includes adding a new 'implict_trans_secs' attribute for each ALUA
target port group to control the value returned to the application client
for an recommended implict translation timeout in seconds. By default
this value is currently set to zero, and limited up to 255 by virtue of
using a single byte in the extended header format.
This value is used by target_emulate_report_target_port_groups() within
the extended header logic to set IMPLICIT TRANSITION TIME as defined by
spc4r30.

This patch fixes the MAINTENANCE_IN service action type checks to only
look at the proper lower 5 bits of cdb byte 1. This addresses the case
where MI_REPORT_TARGET_PGS w/ extended header using the upper three bits of
cdb byte 1 was not processed correctly in transport_generic_cmd_sequencer,
as well as the three cases for standby, unavailable, and transition ALUA
primary access state checks.
Also add MAINTENANCE_IN to the excluded list in transport_generic_prepare_cdb()
to prevent the PARAMETER DATA FORMAT bits from being cleared.

Introduce the module_comedi_usb_driver macro, and the
associated register/unregister functions, which is a
convenience macro for comedi usb driver modules similar
to module_platform_driver. It is intended to be used by
drivers where the init/exit section does nothing but
register/unregister the comedi driver and associated usb
driver. By using this macro it is possible to eliminate
a few lines of boilerplate code per comedi usb driver.
Also, when registering the usb driver check for failure
and unregister the comedi driver.

Use del_timer_sync to remove timer before mddev_suspend finishes.
We don't want a timer going off after an mddev_suspend is called. This is
especially true with device-mapper, since it can call the destructor function
immediately following a suspend. This results in the removal (kfree) of the
structures upon which the timer depends - resulting in a very ugly panic.
Therefore, we add a del_timer_sync to mddev_suspend to prevent this.

raid10 stores dev_sectors in 'conf' separately from the one in
'mddev' because it can have a very significant effect on block
addressing and so need to be updated carefully.
However raid10_resize isn't updating it at all!
To update it correctly, we need to make sure it is a proper
multiple of the chunksize taking various details of the layout
in to account.
This calculation is currently done in setup_conf. So split it
out from there and call it from raid10_resize as well.
Then set conf->dev_sectors properly.

Move the module_init/module_exit routines and the associated
struct comedi_drive to the end of the source. This is more
typical of how other drivers are written and removes the need
for the forward declarations.
Refactor some of the other functions to remove the remaining
forward declarations.
Convert the driver to use the module_comedi_driver() macro
which makes the code smaller and a bit simpler.

source. This is more typical of how other drivers are written
and removes the need for the forward declarations.
Refactor some of the other functions to remove the remaining
forward declarations.
Remove some unused code stubs that are in #if 0 / #endif
blocks.
Convert the driver to use the module_comedi_pci_driver() macro
which makes the code smaller and a bit simpler.

Pull networking tree from David Miller:
1) ptp_pch driver build broke during this merge window due to missing
slab.h header, fix from Geery Uytterhoeven.
2) If ipset passes in a bogus hash table size we crash because the size
is not validated properly. Compounding this, gcc-4.7 can miscompile
ipset such that even when the user specifies legitimate parameters
the tool passes in an out-of-range size to the kernel.
Fix from Jozsef Kadlecsik.
3) Users have reported that the netdev watchdog can trigger with pch_gbe
devices, and it turns out this is happening because of races in the
TX path of the driver leading to the transmitter hanging. Fix from
Eric Dumazet, reported and tested by Andy Cress.
4) Novatel USB551L devices match the generic class entries for the cdc
ethernet USB driver, but they don't work because they have generic
descriptors and thus need FLAG_WWAN to function properly.
Add the necessary ID table entry to fix this, from Dan Williams.
5) A recursive locking fix in the USBNET driver added a new problem, in
that packet list traversal is now racy and we can thus access
unlinked SKBs and crash.
Avoid this situation by adding some extra state tracking, from Ming
Lei.
6) The rtlwifi conversion to asynchronous firmware loading is racy, fix
by reordering the probe procedure. From Larry Finger.

7) Fix regressions with bluetooth keyboards by notifying userland
properly when the security level changes, from Gustavo Padovan.
8) Bluetooth needs to make sure device connected events are emitted
before other kinds of events, otherwise userspace will think there is
no baseband link yet and therefore abort the sockets associated with
that connection.

This patch unifies speed setting for both supported tablets. Functionality
of "wacom_poke" (used only by Graphire) is now in "wacom_set_features".
Reporting speed for both tablets can be changed by somethinkg like:
echo 1 > /sys/class/bluetooth/hci0/hci0:1/{device No}/speed
Accepted values:
0 - low speed,
1 - high speed.
The way of changing reporting speed is the same for Graphire and Intuos4 WL.

Add option to change reporting speed for Intuos4 WL. The option is only
internal to the module, but it will be extended to allow control over sysfs,
as it is already implemented for Graphire.

That check is valid only for Wacom Graphire, as the device raports always
start with 0x03. Intuos4 WL high-speed raports begin with 0x04, so
the check would be filtering out valid reports.

The qeth layer3 driver is notified about IP address changes.
Changes concerning qeth driven network interfaces have to be
forwarded to the OSA-card. This includes IP addresses of VLAN
interfaces with a qeth device as base device. Function
qeth_l3_verify_vlan_dev() determines if the net_device of the
IP event belongs to a vlan device belonging to a qeth device
as real device. This function is broken starting with commit
7ff0bcf676 , which means IP
addresses of VLAN devices are no longer set at the base qeth
device. The patch repairs function qeth_l3_verify_vlan_dev().
