8f31fa53d3 scsi: hpsa: Remove pci-dma-compat wrapper API
The legacy API wrappers in include/linux/pci-dma-compat.h should go away as
they create unnecessary midlayering for include/linux/dma-mapping.h API.
Instead use dma-mapping.h API directly.

The patch has been generated with the coccinelle script below. Compile
tested.

cb17c1b69b scsi: hpsa: Don't bother with vmalloc for BIG_IOCTL_Command_struct
"BIG" in the name refers to the amount of data being transferred, _not_ the
size of structure itself; it's 140 or 144 bytes (for 32bit and 64bit hosts
resp.). IOCTL_Command_struct is 136 or 144 bytes large...

No point whatsoever turning that into dynamic allocation, let alone vmalloc
one. Just keep it as local variable...

3e16e83a62 scsi: hpsa: correct race condition in offload enabled
Correct race condition where ioaccel is re-enabled before the raid_map is
updated. For RAID_1, RAID_1ADM, and RAID 5/6 there is a BUG_ON called which
is bad.

 - Change event thread to disable ioaccel only. Send all requests down the
   RAID path instead.

 - Have rescan thread handle offload_enable.

 - Since there is only one rescan allowed at a time, turning
offload_enabled on/off should not be racy. Each handler queues up a
rescan if one is already in progress.

  - For timing diagram, offload_enabled is initially off due to a change
    (transformation: splitmirror/remirror), ...

  otbe = offload_to_be_enabled
  oe   = offload_enabled

  ...
T6 - rescan thread has bypassed code to reload new raid map.
  ...
T7 - event thread runs and sets offload_to_be_enabled
  ...
  T9 - rescan thread turns on offload_enabled.
T10- request comes in and goes down ioaccel path.
  T11- BUG_ON.

 - After the patch is applied, ioaccel_enabled can only be re-enabled in
   the re-scan thread.

"Seven fixes to four drivers with no core changes.

The mpt3sas one is theoretical until we get a CPU that goes up to 64
bits physical, the qla2xxx one fixes an oops in a driver
initialization error leg and the others are mostly cosmetic"

[ The fcoe patches may be worth highlighting - they may be "just"
cleanups, but they simplify and fix the odd fc_rport_priv structure
handling rules so that the new gcc-9 warnings about memset crossing
  structure boundaries are gone.

The old code was hard for humans to understand too, and really

The big merge conflict this time around is the SPDX licence tags.
Following discussion on linux-next, we believe our version to be more
accurate than the one in the tree, so the resolution is to take our
version for all the SPDX conflicts"

Note on the SPDX license tag conversion conflicts: the SCSI tree had
done its own SPDX conversion, which in some cases conflicted with the
treewide ones done by Thomas & co.

In almost all cases, the conflicts were purely syntactic: the SCSI tree
used the old-style SPDX tags ("GPL-2.0" and "GPL-2.0+") while the
treewide conversion had used the new-style ones ("GPL-2.0-only" and
"GPL-2.0-or-later").

In these cases I picked the new-style one.

In a few cases, the SPDX conversion was actually different, though. As
explained by James above, and in more detail in a pre-pull-request
thread:

"The other problem is actually substantive: In the libsas code Luben
Tuikov originally specified gpl 2.0 only by dint of stating:

  * This file is licensed under GPLv2.

In all the libsas files, but then muddied the water by quoting GPLv2
verbatim (which includes the or later than language). So for these
files Christoph did the conversion to v2 only SPDX tags and Thomas
converted to v2 or later tags"

So in those cases, where the spdx tag substantially mattered, I took the
SCSI tree conversion of it, but then also took the opportunity to turn
the old-style "GPL-2.0" into a new-style "GPL-2.0-only" tag.

Similarly, when there were whitespace differences or other differences
to the comments around the copyright notices, I took the version from
the SCSI tree as being the more specific conversion.

Finally, in the spdx conversions that had no conflicts (because the
treewide ones hadn't been done for those files), I just took the SCSI
tree version as-is, even if it was old-style. The old-style conversions
are perfectly valid, even if the "-only" and "-or-later" versions are
perhaps more descriptive.

6f4e626fb0 scsi: ata: Use unsigned int for cmd's type in ioctls in scsi_host_template
Clang warns several times in the scsi subsystem (trimmed for brevity):

drivers/scsi/hpsa.c:6209:7: warning: overflow converting case value to

        case CCISS_GETHEARTBEAT:
             ^

The root cause is that the _IOC macro can generate really large numbers,
which don't fit into type 'int', which is used for the cmd parameter in
the ioctls in scsi_host_template. My research into how GCC and Clang are
handling this at a low level didn't prove fruitful. However, looking at
the rest of the kernel tree, all ioctls use an 'unsigned int' for the
cmd parameter, which will fit all of the _IOC values in the scsi/ata
subsystems.

Make that change because none of the ioctls expect a negative value for
any command, it brings the ioctls inline with the reset of the kernel,
and it removes ambiguity, which is never good when dealing with compilers.

Merge tag 'scsi-misc' of git://git.kernel.org/pub/scm/linux/kernel/git/jejb/scsi

Pull SCSI updates from James Bottomley:
"This is mostly updates of the usual drivers: UFS, esp_scsi, NCR5380,
  qla2xxx, lpfc, libsas, hisi_sas.

In addition there's a set of mostly small updates to the target
subsystem a set of conversions to the generic DMA API, which do have
some potential for issues in the older drivers but we'll handle those
  as case by case fixes.

A new myrs driver for the DAC960/mylex raid controllers to replace the
block based DAC960 which is also being removed by Jens in this merge
  window.

Plus the usual slew of trivial changes"

[ "myrs" stands for "MYlex Raid Scsi". Obviously. Silly of me to even
wonder. There's also a "myrb" driver, where the 'b' stands for
'block'. Truly, somebody has got mad naming skillz. - Linus ]

6396bb2215 treewide: kzalloc() -> kcalloc()
The kzalloc() function has a 2-factor argument form, kcalloc(). This
patch replaces cases of:

        kzalloc(a * b, gfp)

with:
        kcalloc(a * b, gfp)

as well as handling cases of:

        kzalloc(a * b * c, gfp)

with:

        kzalloc(array3_size(a, b, c), gfp)

as it's slightly less ugly than:

        kzalloc_array(array_size(a, b), c, gfp)

This does, however, attempt to ignore constant size factors like:

        kzalloc(4 * 1024, gfp)

though any constants defined via macros get caught up in the conversion.

Any factors with a sizeof() of "unsigned char", "char", and "u8" were
dropped, since they're redundant.

0d98ba8d70 scsi: hpsa: disable device during shutdown
'Commit cc27b735ad ("PCI/portdrv: Turn off PCIe services during
shutdown")' has been added to kernel to shutdown pending PCIe port service
interrupts during reboot so that a newly started kexec kernel wouldn't
observe pending interrupts.

pcie_port_device_remove() is disabling the root port and switches by
calling pci_disable_device() after all PCIe service drivers are shutdown.

This has been found to cause crashes on HP DL360 Gen9 machines during
reboot due to hpsa driver not clearing the bus master bit during the
shutdown procedure by calling pci_disable_device().

Disable device as part of the shutdown sequence.

fd9adc402b SCSI fixes on 20180327
Two driver fixes (ibmvfc, iscsi_tcp) and a USB fix for devices that
give the wrong return to Read Capacity and cause a huge log spew. The
remaining 5 patches all try to fix commit 84676c1f21
"genirq/affinity: assign vectors to all possible CPUs") which broke
 the non-mq I/O path.
