The reset of data lines when the card is removed from the cage results in
a failure.The failure is seen if the card is removed from the cage when TC
is pending after a CMD with data received CC.The reset logic leaves the
controller in a state where niether a TC is received nor DTO.
The rest code can be safely removed here since it is taken care in the IRQ
handler.

percpu.h is included by sched.h and module.h and thus ends up being
included when building most .c files. percpu.h includes slab.h which
in turn includes gfp.h making everything defined by the two files
universally available and complicating inclusion dependencies.
percpu.h -> slab.h dependency is about to be removed. Prepare for
this change by updating users of gfp and slab facilities include those
headers directly instead of assuming availability. As this conversion
needs to touch large number of source files, the following script is
used as the basis of conversion.

The script does the followings.
* Scan files for gfp and slab usages and update includes such that
only the necessary includes are there. ie. if only gfp is used,
gfp.h, if slab is used, slab.h.
* When the script inserts a new include, it looks at the include
blocks and try to put the new include such that its order conforms
to its surrounding. It's put in the include block which contains
core kernel includes, in the same order that the rest are ordered -
alphabetical, Christmas tree, rev-Xmas-tree or at the end if there
doesn't seem to be any matching order.
* If the script can't find a place to put a new include (mostly
because the file doesn't have fitting include block), it prints out
an error message indicating which .h file needs to be added to the
file.
The conversion was done in the following steps.
1. The initial automatic conversion of all .c files updated slightly
over 4000 files, deleting around 700 includes and adding ~480 gfp.h
and ~3000 slab.h inclusions. The script emitted errors for ~400
files.
2. Each error was manually checked. Some didn't need the inclusion,
some needed manual addition while adding it to implementation .h or
embedding .c file was more appropriate for others. This step added
inclusions to around 150 files.
3. The script was run again and the output was compared to the edits
from #2 to make sure no file was left behind.
4. Several build tests were done and a couple of problems were fixed.
e.g. lib/decompress_*.c used malloc/free() wrappers around slab
APIs requiring slab.h to be added manually.
5. The script was run on all .h files but without automatically
editing them as sprinkling gfp.h and slab.h inclusions around .h
files could easily lead to inclusion dependency hell. Most gfp.h
inclusion directives were ignored as stuff from gfp.h was usually
wildly available and often used in preprocessor macros. Each
slab.h inclusion directive was examined and added manually as
necessary.
6. percpu.h was updated not to include slab.h.
7. Build test were done on the following configurations and failures
were fixed. CONFIG_GCOV_KERNEL was turned off for all tests (as my
distributed build env didn't work with gcov compiles) and a few
more options had to be turned off depending on archs to make things
build (like ipr on powerpc/64 which failed due to missing writeq).
* x86 and x86_64 UP and SMP allmodconfig and a custom test config.
* powerpc and powerpc64 SMP allmodconfig
* sparc and sparc64 SMP allmodconfig
* ia64 SMP allmodconfig
* s390 SMP allmodconfig
* alpha SMP allmodconfig
* um on x86_64 SMP allmodconfig
8. percpu.h modifications were reverted so that it could be applied as
a separate patch and serve as bisection point.
Given the fact that I had only a couple of failures from tests on step
6, I'm fairly confident about the coverage of this conversion patch.
If there is a breakage, it's likely to be something in one of the arch
headers which should be easily discoverable easily on most builds of
the specific arch.

Convert the device_terminate_all() operation on the
DMA engine to a generic device_control() operation
which can now optionally support also pausing and
resuming DMA on a certain channel. Implemented for the
COH 901 318 DMAC as an example.

msmsdcc_enable_clocks() was incorrectly being called depending on
the state of host->clks_on. This means the busclk idle timer was never
being deleted if the clock was already on.. Bogus.
Also fixes a possible double clk disable if the call to
del_timer_sync() in msmsdcc_disable_clocks() raced with
the busclk timer.

Based on an original patch by Brent DeGraaf:
"Previous versions of the SD driver were beset with excessive command
timeouts. These timeouts were silent by default, but happened
frequently, especially during heavy system activity and concurrent
access of two or more SD devices. Worst case, these timeouts would
occasionally hit at the end of a successful write, resulting in false
failures that could adversely affect journaling file systems if timing
was unfortunate. This update tightens the association and timing between
dma transfers and the commands that trigger them by utilizing a new api
implemented in the datamover. In addition, it also fixes a dma cache
coherency issue that was exposed during testing of this fix that
occasionally resulted in card corruption. Processing of results in the
interrupt status routine was modified to process command results prior to
data because overwritten command results were observed during testing
since the data section can result in command issuances of its own.
This change also eliminates the software command timeout, relying entirely
on the hardware version, since the software timeout was found to cause
problems of its own after extensive testing (having hardware timer and
software timers addressing the same issue was found to cause a race
condition under heavy system load)."
This change originally added PROG_DONE handling, which has been split out
into a separate patch. Also on our platform, the data mover driver maintains
coherency to ensure API reliability, so the above mentioned cache corruption
issue was not an issue for us.

least 3 core clock cycles for the writes to take effect. *sigh*

Based on a patch from Brent DeGraaf:
"The datamover supports channels which can be shared amongst devices.
As a result, the actual data transfer may occur some time after the
request is queued up. Some devices such as mmc host controllers
will timeout if a command is issued too far in advance of the actual
transfer, so if dma to other devices on the same channel is already
in progress or queued up, the added delay can cause pending transfers
to fail before they start. This change extends the api to allow a
user callback to be invoked just before the actual transfer takes
place, thus allowing actions directly associated with the dma
transfer, such as device commands, to be invoked with precise timing.
Without this mechanism, there is no way for a driver to realize
this timing. Also adds a user pointer to the command structure for use
by the caller to reference information that may be needed by the
callback routine for proper identification and processing associated
with that specific request. This change is necessary to fix problems
associated with excessive command timeouts and race conditions in the
mmc driver."
This patch also fixes all the callers of msm_dmov_enqueue_cmd() to
ensure their callback function is NULL.

It appears that in some cases there may be a delay on the ARM9 in enabling our clock.
As a result, we may put the controller into a bad state. Delay 10us after enabling
clocks to let the peripheral settle. Note - this is all imperical.
Also ensure set_ios() callback grabs the host lock.

This patch changes the tmio_mmc driver to wait 100ms
before checking the card detect status. This type of
delay is quite common among mmc drivers, it seems that
most hardware platforms need to give the hardware some
time to settle before checking card availabilty.
Hotplug is half-broken without this patch on the sh7724
Ecovec board. Hot insertion seems ok but eject is never
detected without this patch.

On SuperH platforms the SDHI controller does not produce any command IRQs after
a completed IO. This leads to card-detect interrupts staying disabled. Do not
disable card-detect interrupts on DATA IRQs.

This card reader doesn't advertise, however DMA works well. Probably
windows SDHCI driver assumes that all readers support DMA and thus we see
that bug.

We used to manage features and differences on a per-cpu basis. As several
cpus share the same mci revision, this patch aggregates cpus that have the
same IP revision in one defined constant. We use the
at91mci_is_mci1rev2xx() funtion name not to mess with newer Atmel sd/mmc
IP called "MCI2". _rev2 naming could have been confusing...

According to the datasheets AT91SAM9261 does not support SDIO interrupts,
and AT91SAM9260/9263 have an erratum requiring 4bit mode while using slot
B for the interrupt to work.

This patch is setting some max_ variables for the IO elevator, so the
elevator will put requests for large data blocks to the driver. This is
critical for
a) speed
and
b) wear leveling of the flash chip controller: Otherwise the controller
will treat the SD card badly with millions of single 4 KByte write
commands. This will lead to a shorter life time for the SD cards.

Convert the read to use the DMA buffer as well. The old code was doing
double-buffering DMA with the PDC; no way to make it work. Replace it
with a single-PDC approach. It also simplify things removing the need for
a pre_dma_read() function.

The TX DMA buffer is allocated only once, because the
allocation/deallocation of the buffer for EACH chunk of data is
time-consuming and prone to memory fragmentation.
Using a coherent DMA buffer avoids extra data cache calls.

If no platform_data was givin to the device it's going to use it's default
platform data struct which has all fields initialized to zero. As a
result the driver is going to try to request gpio0 both as write protect
and card detect pin. Which of course will fail and makes the driver
unusable
Previously to the introduction of no_wprotect and no_detect the behavior
was to assume that if no platform data was given there is no write protect
or card detect pin. This patch restores that behavior.

sdhci_set_adma_desc() is using byte-writes to write data in a specified
order into memory. Change to using __le16 for the two byte and __le32 for
the four byte cases and use the cpu_to_{le16,le32} to do the conversion
before writing.
This will reduce the size of the code and the number of writes as we no
longer need to chop the data up before writing.

The code to write the ADMA descriptor into memory is repeated several
times throughout sdhci_adma_table_pre, and thus should be moved into a
common function. This will also be useful if the patch to make the write
more efficient is accepted.

This patch solves nasty problem original driver has.
Original goal of the ricoh_mmc was to disable this device because then,
mmc cards can be read using standard SDHCI controller, thus avoiding
writing of yet another driver.
However, the act of disablement, makes other pci functions that belong to
this controller (xD and memstick) shift up one level, thus pci core has
now wrong idea about these devices.
To fix this issue, this patch moves the driver into the pci quirk section,
thus it is executes before the pci is enumerated, and therefore solving
that issue, also same sequence of commands is performed on resume for same
reasons.

Although the hardware supports a 4/8bit SD interface and the driver
unconditionally advertises all hardware caps to the MMC core, not all
datalines may actually be wired up. This patch introduces another
field to au1xmmc platform data allowing platforms to disable certain
advanced host controller features.

DMA can only be done from physical addresses; move the "virt_to_phys"
source/destination buffer address translation from the dbdma queueing
functions (since the hardware can only DMA to/from physical addresses)
to their respective users.

EMMC can have two voltage supplies, Vcc and VccQ
which are implemented in the code as consumer
supplies vmmc and vmmc_aux.
If the regulator that supplies vmmc_aux is shared
with other consumers, then sending it to sleep
will disrupt those consumers. However, the
TWL4030-family regulators may have OFF remapped
to SLEEP, in which case 'regulator_disable()'
will put the regulator to sleep only when all
consumers are disabled - which is the desired
behaviour.
This patch adds a platform data field to allow
that option.

This patch moves the setup code for GPIO's and Voltage
Regulators from the board file mmc-twl4030.c to the
driver omap_hsmmc.c. PBIAS and other system control
configuration remains in the board file.
Moving GPIO code to the driver makes the board initialisation
code independent of when GPIO's are defined. That makes the
board initialisation now entirely independent of its original
twl4030 roots.
Moving Voltage Regulator code to the driver allows for further
development of regulator support in the core MMC code. It also
permits the MMC core to be compiled as a module, because the
board code no longer calls MMC core functions.

In the U300 some hardware bug makes the status flag not come up
signalling a successful write (or anything else, like an error, for
that matter) on write requests. This little quirk makes the writes
work on U300.

This new revision of the IP adds some improvements to the MCI already
present in several Atmel SOC.
Some new registers are added and a particular way of handling DMA
interaction lead to a new sequence in function call which is backward
compatible: On MCI2, we must set the DMAEN bit to enable the DMA
handshaking interface. This must happen before the data transfer command
is sent.
A new function is able to differentiate MCI2 code and is based on
knowledge of processor id (cpu_is_xxx()).

Allow the use of another DMA controller driver in atmel-mci sd/mmc driver.
This adds a generic dma_slave pointer to the mci platform structure where
we can store DMA controller information. In atmel-mci we use information
provided by this structure to initialize the driver (with new helper
functions that are architecture dependant).
This also adds at32/avr32 chip modifications to cope with this new access
method.

This is still in use especially to develop SDIO device drivers on laptop

Currently DaVinci EDMA driver supports multiple EDMA channel controller
instances. edma_alloc_channel() api returns a 32 bit value which has the
channel controller number in MSB and the EDMA channel number in LSB. The
variables which store the value returned by edma_alloc_channel() have to
be 32 bit wide now.

Add better support for omitting either the card detect or the write
protect GPIOs if the board does not support it. Add the fields
no_wprotect and no_detect to the platform data which when set indicate the
absence of the respective GPIOs.
Note, this also fixes a minor bug where it tries to free IRQ0 if there is
no detect gpio available.

Fixes for the DMA transfer mode of the driver to try and improve the state
of the code:
- Ensure that dma_complete is set during the end of the command phase
so that transfers do not stall awaiting the completion
- Update the DMA debugging to provide a bit more useful information
such as how many DMA descriptors where not processed and print the
DMA addresses in hexadecimal.
- Fix the DMA channel request code to actually request DMA for the
S3CMCI block instead of whatever '0' signified.
- Add fallback to PIO if we cannot get the DMA channel, as many of the
devices with this block only have a limited number of DMA channels.
- Only try and claim and free the DMA channel if we are trying to use it.
This improves the driver DMA code to the point where it can now identify a
card and read the partition table. However the DMA can still stall when
trying to move data between the host and memory.

Add a selection for the data transfer mode of the s3cmci driver, allowing
for either a configuration or rumtime selection of the use of the DMA or
PIO transfer code.
The PIO only mode is 476 bytes smaller than the driver with both methods
compiled in.

The controller supports SDIO IRQ detection so add support for hardware
assisted SDIO interrupt detection for the SDIO core. This improves the
response time for SDIO interrupts and thus the transfer rate from devices
such as the Marvel 8686.
As a note, it does seem that the controller will miss an IRQ than is held
asserted, so there are some manual checks to see if the SDIO interrupt is
active after a transfer.

The clear_imask() call should be used to clear the interrupt mask
register, as it may end up clearing the SDIO interrupt bit if this is
enabled.
Change all writes of zero to SDIIMSK register to use clear_imask() ready
for the SDIO updates.

Use the platform id list to match the three different versions of the
hardware block that this driver supports.
This will change the prefix of the console messages produced by this
driver to be prefixed by s3c-mci instead of the hardware block name, such
as s3c2440-mci.

The #ifdefs in the MMCI driver were erroneous and just masking
a bug in the U300 generic GPIO implementation. This removes the
ifdefs and fixes the U300 generic GPIO instead.

On an OLPC XO-1.5 development board with Via VX855 chipset, the sdhci
controller can take up to 12ms to stabilize its clock, but the current
timeout at which we give up on the controller is 10ms.
The patch increases the timeout delay rather than using a device-specific
quirk -- since we exit the loop when the clock comes up, increasing the
timeout value will only make us mdelay() longer in the errant case of a
device with a clock that is not stabilizing, which it seems worth waiting
a little longer for in general.

Unification of the atmel-mci driver to support the AT91 processors MCI
interface. The atmel-mci driver currently supports the AVR32 and this
patch adds AT91 support.
Add read/write proof selection switch dependent on chip availability of
this feature.
To use this new driver on a at91 the platform driver for your board needs
to be updated.

- Get rid of incomprehensible "if { for { if } }" construction for the
exponential divisor calculation. The first if statement isn't correct
at all, since it should check for "host->max_clk / pre_div / 16 >
clock". The error doesn't cause any bugs because the check in the for
loop does the right thing, and so the outer check becomes useless;
- For the linear divisor do the same: a single while statement is more
readable than for + if construction;
- Add dev_dbg() that prints desired and actual clock frequency.

MPC85xx SOCs have normal write-protect state reporting, so we shouldn't
hard-code the quirk.
Instead, look for "sdhci,wp-inverted" property, plus check for
mpc837x_{rdb,mds} machines since older device trees don't specify the new
property.

eSDHC fails to recognize some SDHS cards, throwing timeout errors:
mmc0: error -110 whilst initialising SD card
That's because we calculate timeout value in a wrong way: on eSDHC hosts
the timeout clock is derivied from the SD clock, which is set dynamically.
As David Vrabel suggested, deriving timeout clock from SD clock is a
common scheme, so let's implement DATA_TIMEOUT_USES_SDCLK quirk and use it
for eSDHC hosts.
Also, from now on we don't need esdhc_get_timeout_clock() callback, so
remove it.

SDHCI core tries to write HISPD bit into the host control register, but
the eSDHC controllers don't have that bit, and that causes all sorts of
misbehaviour when using 4-bit mode capable SD cards.

Linear divisor's values in a register start at 0 (zero means "divide by
1"). Before this patch the code didn't account that fact, so SD cards

get_min_clock() makes sense only with NONSTANDARD_CLOCK quirk and when
set_clock() callback is specified.
The patch should cause no functional changes, it just makes the code
self-documented and avoids any possible misuse of get_min_clock().

Commands like SWITCH (CMD6) send a response and then signal busy while the
operation is completed. These commands are expected to always succeed
(otherwise the response would have indicated an error).
Set an arbitrarily large data timeout value (100ms) for these commands to
ensure that premature timeouts do not occur.

Depending on the manufacturer, there is a small possibility that removing
a card while it is being written to, can render the card permanently
unusable. To prevent that, the card is made inaccessible when the cover
is open.

If an unexpected interrupt occurs while preparing the next request, an
oops can occur.
For example, a new request is setting up DMA for data transfer so
host->data is not NULL. An unexpected transfer complete (TC) interrupt
comes along and the interrupt handler sets host->data to NULL. Oops!
Prevent that by adding a spinlock.

Sometimes the controller unexpectedly produces a TC (transfer complete)
interrupt before the CC (command complete) interrupt for command 6
(SWITCH). This is a problem because the CC interrupt can get mixed up
with the next request. Add a hack for CMD6.

When a card is not in use, the voltage regulator can be put to sleep.
This is an alternative to powering the card off, when powering off is not
safe because the card might be replaced without the driver being aware of
it.
That situation happens if:
- the card is removable i.e. not eMMC
- and there is no card detect
- and there is a cover switch but the cover is open

This patch is preparation for adding context save and restore support.
Keep track of the current power mode so that the context restore function
can avoid restoring the context for a card if the power has been switched
off. If the power is off, the card must be reinitialized anyway which
will re-establish the context.

omap_mmc_probe lives in .init.text, so using platform_driver_register to
register it is wrong because binding a device after the init memory is
discarded (e.g. via sysfs) results in an oops.
As requested by David Brownell platform_driver_probe is used instead of
moving the probe function to .devinit.text as proposed initially. This
saves some memory, but devices registered after the driver is probed are
not bound (probably there are none) and binding via sysfs isn't possible.

This enables the use of a regulator to power the MMCI/PL180
PrimeCell. The OCR mask is calculated and voltage is set using
the new MMC core functions for discovering voltage ranges
in regulators. The platform translate_vdd function which basically
controls the 4 lines out of the PL180 is disabled if you use a
regulator instead.

This moves the mmci platform data definition struct away from
arch/arm/include/asm/mach/mmc.h into the more proper place among
the other primecells in include/linux/amba/mmci.h and at the same
time renames it to "mmci.h", and also the struct in this file
confusingly named mmc_platform_data has been renamed
mmci_platform_data for clarity.

This makes it possible to pass down the host controller
capabilities for the MMCI driver using the platform data. It
also provides the capabilties for the U300 implementation as an
example, and makes sure the 4bit wide mode is set if this is
requested by the ios() now that we can actually set that
capability for a platform.

This breaks out the clock divider set-up code from the
mmci_set_ios() code and surrounds the two register
writes with a host lock so we don't get collisions if
(in future code) two code paths want to change the
clock divider at the same time as can be the case if
we get something like pre/post- clock frequency change
notifications soonish.

This moves the primecell vendor enum definition inside vic.c
out to linux/amba/bus.h where it belongs and replace any
occurances of specific vendor ID:s with the respective enums
instead.

The MMC block needs 3 external datas to work :
- is the MMC card put in "read-only mode" ?
- is a MMC card inserted or removed ?
- enable power towards the MMC card
Several platforms provide these controls through
gpios. Expand the platform_data to request and use these
gpios is set up by board code.

The recent addition of optional gpiolib support to check if a

It needs this ifdef to become optional so that U300 compiles,

the code allready uses flush_kernel_dcache_page(). This patch updates the
driver to the recent sg API changes which require that either SG_MITER_TO_SG
or SG_MITER_FROM_SG is set. SG_MITER_TO_SG calls flush_kernel_dcache_page()
in sg_mitter_stop()

Since commit 8dfd0374be ("MMC core: limit
minimum initialization frequency to 400kHz") MMC core checks for minimum
frequency, and that causes following messages flood when using eSDHC
controllers:
...
mmc0: Minimum clock frequency too high for identification mode
mmc0: Minimum clock frequency too high for identification mode
...
The warnings are legitimate, since if we'd use 133 MHz clocks for standard
SDHCI controllers, we'd not able to scale frequency down to 400 kHz.
But eSDHC controllers have a non-standard SD clock management, so we can
divide clock by 256 * 16, not just 256.
This patch introduces get_min_clock() callback for sdhci core and
implements it for sdhci-of driver, and thus fixes the issue.

Standard data flow for MMC/SD/SDIO cards requires that the mvsdio
controller be set for big endian operation. This is causing problems
with buffers which length is not a multiple of 4 bytes as the last
partial word doesn't get shifted all the way and stored properly in
memory. Let's compensate for this.

Use gpiolib where available (and when valid GPIOs are provided) for

Add a new spi_master.flags word listing constraints relevant to that
controller. Define the first constraint bit: a half duplex restriction.
Include that constraint in the OMAP1 MicroWire controller driver.
Have the mmc_spi host be the first customer of this flag. Its coding
relies heavily on full duplex transfers, so it must fail when the
underlying controller driver won't perform them.
(The spi_write_then_read routine could use it too: use the
temporarily-withdrawn full-duplex speedup unless this flag is set, in
which case the existing code applies. Similarly, any spi_master
implementing only SPI_3WIRE should set the flag.)

The SDHCI controller found in the VX855ES requires 10ms
delay between applying power and applying clock.
This issue has been discovered and documented by the OLPC XO1.5 team.

Some hosts (hardware configurations, or particular SD/MMC slots) may
not support 4-bit bus. For example, on MPC8569E-MDS boards we can
switch between serial (1-bit only) and nibble (4-bit) modes, thought
we have to disable more peripherals to work in 4-bit mode.
Along with some small core changes, this patch modifies sdhci-of
driver, so that now it looks for "sdhci,1-bit-only" property in the
device-tree, and if specified we enable a proper quirk.
