9d5043d93d misc: hpilo: rename device creation loop variable
In `ilo_probe()`, the loop variable `minor` isn't really the minor
device number, it's the channel or slot number.  Rename it to `slot` for
consistency.

1c9a697bc9 misc: hpilo: fix inconsistent device numbers
Each iLO device is allocated `max_ccb` minor device numbers (one for
each channel).  When `ilo_probe()` calls `device_create()` in a loop,
the minor device numbers passed to `device_create()` start at 0.  For
consistency with the call to `cdev_add()`, and for consistency with the
calls to `device_destroy()` from `ilo_remove()`, the minor device
numbers passed to `device_create()` should start at the value in the
variable `start`.  Fix it.

This is a logical bug rather than an actual bug, because the number of
supported devices is `MAX_ILO_DEV` which is defined as `1`.

fd06978b06 misc: hpilo: make ilo_class a static const structure
Now that the driver core allows for struct class to be in read-only
memory, move the ilo_class structure to be declared at build time
placing it into read-only memory, instead of having to be dynamically
allocated at boot time.

cec24b8b6b Char/Misc drivers for 6.4-rc1
Here is the "big" set of char/misc and other driver subsystems for
 6.4-rc1.
 
 It's pretty big, but due to the removal of pcmcia drivers, almost breaks
 even for number of lines added vs. removed, a nice change.
 
 Included in here are:
   - removal of unused PCMCIA drivers (finally!)
   - Interconnect driver updates and additions
   - Lots of IIO driver updates and additions
   - MHI driver updates
   - Coresight driver updates
   - NVMEM driver updates, which required some OF updates
   - W1 driver updates and a new maintainer to manage the subsystem
   - FPGA driver updates
   - New driver subsystem, CDX, for AMD systems
   - lots of other small driver updates and additions
 
 All of these have been in linux-next for a while with no reported
 issues.

1aaba11da9 driver core: class: remove module * from class_create()
The module pointer in class_create() never actually did anything, and it
shouldn't have been requred to be set as a parameter even if it did
something.  So just remove it and fix up all callers of the function in
the kernel tree at the same time.

23d51b8181 misc: hpilo: map iLO shared memory by PCI revision id
Starting from iLO ASIC 'Neches' with subsystem device id 0x00E4,
bar 5 is used for shared memory region mapping instead of bar 2
because bar 2 is made inaccessible after system POST for security
reason.

As this holds true for future iLO ASIC generations, it does not
make sense to map shared memory region according to the subsystem
device id of each following generations.

Map iLO shared memory region with PCI revision id that maps to the
iLO ASIC generation, starting from Neches (Rev 7).

The wrappers in include/linux/pci-dma-compat.h should go away.

The patch has been generated with the coccinelle script below and has been
hand modified to replace GFP_ with a correct flag.
It has been compile tested.

When memory is allocated in 'ilo_ccb_setup()' GFP_ATOMIC must be used
because a spin_lock is hold in 'ilo_open()' before calling
'ilo_ccb_setup()'

fadbfc38dd hpilo: Replace one-element array with flexible-array member
There is a regular need in the kernel to provide a way to declare
having a dynamically sized set of trailing elements in a structure.
Kernel code should always use “flexible array members”[1] for these
cases. The older style of one-element or zero-length arrays should
no longer be used[2].

For this particular case, it is important to notice that the cachelines
change from 7 to 6 after the flexible-array conversion:

9b6dba7002 misc: hpilo: Do not claim unsupported hardware
CL2600/CL2800 servers leveraged Proliant hardware but are targeted to a
different market segment and come with a different firmware base. Based
upon targeted market needs, the servers de-featured certain aspects of iLO.

As a result, hpilo driver still claims the hardware but is not functional,
so we decided to blacklist it with SSID 0x0289 to reduce confusion to
customers.

bc7de897f2 misc: hpilo: Exclude unsupported device via blacklist
Instead of having explicit if statements excluding devices,
use a pci_device_id table of devices to blacklist.

HPE will put out minor updates to the iLO using the same device
info except for the subsystem device id. hpilo driver takes the
approach to claim based upon {Vendor, Device, SubVendor} and it
allows old software to work on new hardware without patching.

As our primary way to support our customers is via distros, the
patching process could take months to go upstream and then
backported to multiple releases of multiple distros.

This approach worked fairly well as this is only the second time
in 10+ years that we need to blacklist an instance.

values as the POLL* constants do.  But they keyword here is "almost".
For various bad reasons they aren't the same, and epoll() doesn't
actually work quite correctly in some cases due to this on Sparc et al.

The next patch from Al will sort out the final differences, and we
should be all done.

ebf1b764aa misc: hpilo: ignore auxiliary HP iLO BMC's
This patch ignores auxiliary HP Lights-Out (iLO) management controllers.

All HP iLO controllers that have had the PCI subsystem device ID set to 0x1979
by the BIOS are ignored. Also changes default number of channels to 16 and
bumps the version of the module from 1.3 to 1.4.

bcdee04ea7 MISC: hpilo, remove pci_disable_device
pci_disable_device(pdev) used to be in pci remove function. But this
PCI device has two functions with interrupt lines connected to a
single pin. The other one is a USB host controller. So when we disable
the PIN there e.g. by rmmod hpilo, the controller stops working. It is
because the interrupt link is disabled in ACPI since it is not
refcounted yet. See acpi_pci_link_free_irq called from
acpi_pci_irq_disable.

It is not the best solution whatsoever, but as a workaround until the
ACPI irq link refcounting is sorted out this should fix the reported
errors.

1ce873abed hpilo: Despecificate driver from iLO generation
This driver supports iLO, iLO2 and iLO3. However, comments and Kconfig
reference only iLO and iLO2. Let's just call it "iLO" to avoid having to
update strings for each iLO generation. This is similar to the change made
to hpwdt in commit 36e3ff44ce.

6038f373a3 llseek: automatically add .llseek fop
All file_operations should get a .llseek operation so we can make
nonseekable_open the default for future file operations without a
.llseek pointer.

The three cases that we can automatically detect are no_llseek, seq_lseek
and default_llseek. For cases where we can we can automatically prove that
the file offset is always ignored, we use noop_llseek, which maintains
the current behavior of not returning an error from a seek.

New drivers should normally not use noop_llseek but instead use no_llseek
and call nonseekable_open at open time.  Existing drivers can be converted
to do the same when the maintainer knows for certain that no user code
relies on calling seek on the device file.

The generated code is often incorrectly indented and right now contains
comments that clarify for each added line why a specific variant was
chosen. In the version that gets submitted upstream, the comments will
be gone and I will manually fix the indentation, because there does not
seem to be a way to do that using coccinelle.

Some amount of new code is currently sitting in linux-next that should get
the same modifications, which I will do at the end of the merge window.

5a0e3ad6af include cleanup: Update gfp.h and slab.h includes to prepare for breaking implicit slab.h inclusion from percpu.h
percpu.h is included by sched.h and module.h and thus ends up being
included when building most .c files.  percpu.h includes slab.h which
in turn includes gfp.h making everything defined by the two files
universally available and complicating inclusion dependencies.

percpu.h -> slab.h dependency is about to be removed.  Prepare for
this change by updating users of gfp and slab facilities include those
headers directly instead of assuming availability.  As this conversion
needs to touch large number of source files, the following script is
used as the basis of conversion.

The script does the followings.

* Scan files for gfp and slab usages and update includes such that
  only the necessary includes are there.  ie. if only gfp is used,
  gfp.h, if slab is used, slab.h.

* When the script inserts a new include, it looks at the include
  blocks and try to put the new include such that its order conforms
  to its surrounding.  It's put in the include block which contains
  core kernel includes, in the same order that the rest are ordered -
  alphabetical, Christmas tree, rev-Xmas-tree or at the end if there
  doesn't seem to be any matching order.

* If the script can't find a place to put a new include (mostly
  because the file doesn't have fitting include block), it prints out
  an error message indicating which .h file needs to be added to the
  file.

The conversion was done in the following steps.

1. The initial automatic conversion of all .c files updated slightly
   over 4000 files, deleting around 700 includes and adding ~480 gfp.h
   and ~3000 slab.h inclusions.  The script emitted errors for ~400
   files.

2. Each error was manually checked.  Some didn't need the inclusion,
   some needed manual addition while adding it to implementation .h or
   embedding .c file was more appropriate for others.  This step added
   inclusions to around 150 files.

3. The script was run again and the output was compared to the edits
   from #2 to make sure no file was left behind.

4. Several build tests were done and a couple of problems were fixed.
   e.g. lib/decompress_*.c used malloc/free() wrappers around slab
   APIs requiring slab.h to be added manually.

5. The script was run on all .h files but without automatically
   editing them as sprinkling gfp.h and slab.h inclusions around .h
   files could easily lead to inclusion dependency hell.  Most gfp.h
   inclusion directives were ignored as stuff from gfp.h was usually
   wildly available and often used in preprocessor macros.  Each
   slab.h inclusion directive was examined and added manually as
   necessary.

6. percpu.h was updated not to include slab.h.

7. Build test were done on the following configurations and failures
   were fixed.  CONFIG_GCOV_KERNEL was turned off for all tests (as my
   distributed build env didn't work with gcov compiles) and a few
   more options had to be turned off depending on archs to make things
   build (like ipr on powerpc/64 which failed due to missing writeq).

   * x86 and x86_64 UP and SMP allmodconfig and a custom test config.
   * powerpc and powerpc64 SMP allmodconfig
   * sparc and sparc64 SMP allmodconfig
   * ia64 SMP allmodconfig
   * s390 SMP allmodconfig
   * alpha SMP allmodconfig
   * um on x86_64 SMP allmodconfig

8. percpu.h modifications were reverted so that it could be applied as
   a separate patch and serve as bisection point.

Given the fact that I had only a couple of failures from tests on step
6, I'm fairly confident about the coverage of this conversion patch.
If there is a breakage, it's likely to be something in one of the arch
headers which should be easily discoverable easily on most builds of
the specific arch.

891f7d73ea hpilo: reduce frequency of IO operations
Change hpilo open and close logic to spin for 10usec between checking device,
rather than every usec.

Because the loop is coded to take up to 10ms, it seemed prudent to
increase the interval between polling the device, to reduce the load on
the system and allow more other work to happen.

c073b2db00 hpilo: open/close fix
The device can take a while to respond to an open/close request, so
increase the time kernel will wait for response (1 ms to 10ms).

Also, properly clean up a channel on a failed open, by calling the channel
close routine.  Just freeing the memory isn't sufficient, the device needs
to be informed that the channel is no longer open, and the device memory
cleared of references to freed dma buffer.

A driver for the HP iLO/iLO2 management processor, which allows userspace
programs to query the management processor.  Programs can open a channel
to the device (/dev/hpilo/dXccbN), and use this to send/receive queries.  
The O_EXCL open flag is used to indicate that a particular channel cannot
be shared between processes.  This driver will replace various packages
HP has shipped, including hprsm and hp-ilo.
