After relatively recent changes in the ACPI-based PCI hotplug
(ACPIPHP) code, the acpiphp_check_host_bridge() executed for PCI
host bridges via acpi_pci_root_scan_dependent() doesn't do anything
useful, because those bridges do not have hotplug contexts. That
happens by mistake, so fix it by making acpiphp_enumerate_slots()
add hotplug contexts to PCI host bridges too and modify
acpiphp_remove_slots() to drop those contexts for host bridges
as appropriate.

When a new device is added below a hotplug bridge, the bridge's secondary
bus speed and the device's bus speed must match. The shpchp driver
previously checked the bridge's *primary* bus speed, not the secondary bus
speed.
This caused hot-add errors like:
shpchp 0000:00:03.0: Speed of bus ff and adapter 0 mismatch
Check the secondary bus speed instead.

Some PCI functions used to be marked __devinit. When CONFIG_HOTPLUG was
not set, these functions were discarded after boot. A few callers of these
__devinit functions were marked __ref to indicate that they could safely
call the __devinit functions even though the callers were not __devinit.
But CONFIG_HOTPLUG and __devinit are now gone, and the need for the __ref
annotations is also gone, so remove them. Relevant historical commits:

Numerical values stored in the device tree are encoded in Big Endian and
should be byte swapped when running in Little Endian.
The RPA hotplug module should convert those values as well.
Note that in rpaphp_get_drc_props(), the comparison between indexes[i+1]
and *index is done using the BE values (whatever is the current endianess).
This doesn't matter since we are checking for equality here. This way only
the returned value is byte swapped.
RPA also made RTAS calls which implies BE values to be used. According to
the patch done in RTAS (http://patchwork.ozlabs.org/patch/336865), no
additional conversion is required in RPA.

In case of a spurious "cmd completed", pcie_write_cmd() does not clear it,
but yet expects more "cmd completed" events to be generated. This does not
happen because the previous (spurious) event has not been acknowledged.

None of these files are actually using any __init type directives and hence
don't need to include <linux/init.h>. Most are just a left over from
__devinit and __cpuinit removal, or simply due to code getting copied from
one driver to the next.

Every pci_dev is on a valid pci_bus, so we don't need to test whether
dev->bus is NULL or not.
The only exceptions are a few legacy cases like alpha_core_agp_setup(),
parisc_agp_setup(), and megaraid's make_local_pdev(), where we allocate a
pci_dev with a NULL bus pointer. These are dubious uses (especially the
megaraid one), and I don't think it's possible to exercise this
pci_configure_slot() path with any of them.

In order to avoid the need to register special ACPI dock
operations for SATA devices add a .uevent() callback pointer to
struct acpi_hotplug_context and make dock_hotplug_event() use that
callback if available. Also rename the existing .event() callback
in struct acpi_hotplug_context to .notify() to avoid possible
confusion in the future.

Instead of requiring a set of special dock operations to be registered
via register_hotplug_dock_device() for each ACPI dock device, it is
much more straightforward to use callback pointers from the devices'
hotplug contexts if available.
For this reason, modify dock_hotplug_event() to use callback pointers
from the hotplug contexts of ACPI devices and fall back to using the
special dock operarions only if those callbacks are missing. Also
make the ACPI-based PCI hotplug (ACPIPHP) subsystem set the .fixup
callback pointer in the hotplug contexts of devices handled by it to
a new function, acpiphp_post_dock_fixup(), so that the dock station
driver can use the callbacks from those contexts instead of special
dock operations registered via register_hotplug_dock_device().
Along with the above changes drop the ACPIPHP's dock operations that
are not necessary any more.

Rework the ACPI dock station driver to store ACPI device object
pointers instead of ACPI handles in its internal data structures.
The purpose is moslty to make subsequent simplifications possible,
but also this allows the overall code size to be reduced slightly.

In order for the ACPI dock station code to be able to use the
callbacks pointed to by the ACPI device objects' hotplug contexts
add a .fixup() callback pointer to struct acpi_hotplug_context.
That callback will be useful to handle PCI devices located in
dock stations.

In case a card is physically yanked out, it should immediately be removed,
regardless of the "surprise" capability bit. Thus:
- Always handle the physical removal - regardless of the "surprise" bit.

Currently, ACPIPHP does not add hotplug context to devices that
should be handled by the native PCI hotplug (PCIeHP) code. The
reason why was because PCIeHP didn't know about the devices'
connections with ACPI and would not clean up things properly
during an eject of an ACPI-backed device, for example.
However, after recent changes that made the ACPI core create struct
acpi_device objects for all namespace nodes regardless of the
underlying devices' status and added PCI rescan-remove locking to
both ACPIPHP and PCIeHP, that concern is not valid any more.
Namely, after those changes PCIeHP need not care about the ACPI
side of things any more and it should be serialized with respect to
ACPIPHP and they won't be running concurrently with each other in
any case.
For this reason, make ACPIPHP to add its hotplug context to
all devices with ACPI companions, even the ones that should be
handled by PCIeHP in principle. That may work around hotplug
issues on some systems where PCIeHP is supposed to work, but it
doesn't and the ACPI hotplug signaling works instead.

The ACPI specification (ACPI 5.0A, Section 6.3.7) says:
_STA may return bit 0 clear (not present) with bit 3 set (device is
functional). This case is used to indicate a valid device for which
no device driver should be loaded (for example, a bridge device.)
Children of this device may be present and valid. OSPM should
continue enumeration below a device whose _STA returns this bit
combination.
Evidently, some BIOSes follow that and return 0x0A from _STA, which
causes problems to happen when they trigger bus check or device check
notifications for those devices too. Namely, ACPIPHP thinks that they
are gone and may drop them, for example, if such a notification is
triggered during a resume from system suspend.
To fix that, modify ACPICA to regard devies as present and
functioning if _STA returns both the ACPI_STA_DEVICE_ENABLED
and ACPI_STA_DEVICE_FUNCTIONING bits set for them.

Today it is there is no protection around pciehp_enable_slot() and
pciehp_disable_slot() to ensure that they complete before another
hot-plug operation can be done on that particular slot.
This patch introduces the slot->hotplug_lock to ensure that any hotplug
operations (add / remove) complete before another hotplug event can begin
processing on that particular slot.

Today, this is how all the hotplug and unplug events work:
Hotplug / Removal needs to be done
=> Set slot->state (protected by slot->lock) to either
POWERON_STATE (for enabling) or POWEROFF_STATE (for disabling).
=> Submit the work item for pciehp_power_thread() to slot->wq.
Problem:
There is a problem if the hotplug events can happen fast enough that
they do not give SW enough time to add or remove the new devices.
=> Assume: Event for unplug comes (e.g. surprise removal). But
before the pciehp_power_thread() work item was executed, the
card was replaced by another card, causing surprise hotplug event.
=> What goes wrong:
=> The hot-removal event sets slot->state to POWEROFF_STATE, and
schedules the pciehp_power_thread().
=> The hot-add event sets slot->state to POWERON_STATE, and
schedules the pciehp_power_thread().
=> Now the pciehp_power_thread() is scheduled twice, and on both
occasions it will find POWERON_STATE and will try to add the
devices on the slot, and will fail complaining that the devices
already exist.
=> Why this is a problem: If the device was replaced between the hot
removal and hot-add, then we should unload the old driver and
reload the new one. This does not happen today. The kernel or the
driver is not even aware that the device was replaced.
The problem is that the pciehp_power_thread() only looks at the
slot->state which would only contain the *latest* state - not
the actual event (add / remove) that was the intent of the IRQ
handler who submitted the work.
What this patch does:
=> Hotplug events pass on an actual request (for addition or removal)
to pciehp_power_thread() which is local to that work item
submission.
=> pciehp_power_thread() does not need to look at slote->state and
hence no locks needed in that.
=> Essentially this results in all the hotplug and unplug events
"replayed" by pciehp_power_thread().

Disable the link notification (in addition to presence detect
notifications) across the slot reset since the reset could flap the link,
and we don't want to treat it as hot unplug followed by a hotplug.

It does not make much sense to refuse to disable a slot if an adapter is
not present or the latch is open. If an adapter is not present, it provides
an even better reason to disable the device slot.
This is specially a problem for link state hot-plug, because some ports use
in band mechanism for presence detection. Thus when link goes down,
presence detect also goes down. We _want_ that the removal should take
place in such case.
Thus remove the checks for adapter and latch in pciehp_disable_slot()

We need future link up events for hot-add, thus don't disable the link
permanently during device removal. Also, remove the static functions that
are now left unused.
This reverts part of 2debd92899 ("PCI: pciehp: Disable/enable link during
slot power off/on"). This was discussed at the URL below, where it was
revealed that it was done for a bug in a PCIe repeater chip on that
particular platform.

A lot of systems do not have the fancy buttons and LEDs, and instead
want to rely only on the Link state change events to drive the hotplug
and removal state machinery.

This patch adds support for that functionality. Here are the details
about the patch itself:
* Define and use interrupt events for linkup / linkdown.
* Make the pcie_isr() also look at link events, and direct control to
corresponding (new) link state change handler function.
* Introduce the functions to handle link-up and link-down events and
queue the add / removal work in the slot->wq to be processed by
pciehp_power_thread()
As a side note, this patch also fixes the bug

Since acpi_device_hotplug() assumes that ACPI handles of device
objects passed to it will not become invalid while acpi_scan_lock
is being held, make acpiphp_disable_slot() acquire acpi_scan_lock,
because it generally causes _EJ0 to be executed for one of the
devices in the slot and that may cause its ACPI handle to become
invalid.

Since the only existing caller of acpiphp_check_host_bridge(),
which is acpi_pci_root_scan_dependent(), already has a struct
acpi_device pointer needed to obtain the ACPIPHP context, it
doesn't make sense to execute acpi_bus_get_device() on its
handle in acpiphp_handle_to_bridge() just in order to get that
pointer back.
For this reason, modify acpiphp_check_host_bridge() to take
a struct acpi_device pointer as its argument and rearrange the
code accordingly.

Since acpi_bus_notify() is executed on all notifications for all
devices anyway, make it execute acpi_device_hotplug() for all
hotplug events instead of installing notify handlers pointing to
the same function for all hotplug devices.
This change reduces both the size and complexity of ACPI-based device
hotplug code. Moreover, since acpi_device_hotplug() only does
significant things for devices that have either an ACPI scan handler,
or a hotplug context with .eject() defined, and those devices
had notify handlers pointing to acpi_hotplug_notify_cb() installed
before anyway, this modification shouldn't change functionality.

The ACPI-based PCI hotplug (ACPIPHP) code currently attaches its
hotplug context objects directly to ACPI namespace nodes representing
hotplug devices. However, after recent changes causing struct
acpi_device to be created for every namespace node representing a
device (regardless of its status), that is not necessary any more.
Moreover, it's vulnerable to the theoretical issue that the ACPI
handle passed in the context between handle_hotplug_event() and
hotplug_event_work() may become invalid in the meantime (as a
result of a concurrent table unload).
In principle, this issue might be addressed by adding a non-empty
release handler for ACPIPHP hotplug context objects analogous to
acpi_scan_drop_device(), but that would duplicate the code in that
function and in acpi_device_del_work_fn(). For this reason, it's
better to modify ACPIPHP to attach its device hotplug contexts to
struct device objects representing hotplug devices and make it
use acpi_hotplug_notify_cb() as its notify handler. At the same
time, acpi_device_hotplug() can be modified to dispatch the new
.hp.event() callback pointing to acpiphp_hotplug_event() from ACPI
device objects associated with PCI devices or use the generic
ACPI device hotplug code for device objects with matching scan
handlers.
This allows the existing code duplication between ACPIPHP and the
ACPI core to be reduced too and makes further ACPI-based device
hotplug consolidation possible.

After recent PCI core changes related to the rescan/remove locking,
the code sections under crit_sect mutexes from ACPIPHP slot objects
are always executed under the general PCI rescan/remove lock.
For this reason, the crit_sect mutexes are simply redundant, so drop
them.

After recent modifications of the ACPI core making it create a struct
acpi_device object for every namespace node representing a device
regardless of the current status of that device the ACPIPHP code
can store a struct acpi_device pointer instead of an ACPI handle
in struct acpiphp_context. This immediately makes it possible to
avoid making potentially costly calls to acpi_bus_get_device() in
two places and allows some more simplifications to be made going
forward.
The reason why that is correct is because ACPIPHP only installs
hotify handlers for namespace nodes that exist when
acpiphp_enumerate_slots() is called for their parent bridge.
That only happens if the parent bridge has an ACPI companion
associated with it, which means that the ACPI namespace scope
in question has been scanned already at that point. That, in
turn, means that struct acpi_device objects have been created
for all namespace nodes in that scope and pointers to those
objects can be stored directly instead of their ACPI handles.

If a struct acpi_device pointer is passed to acpiphp_no_hotplug()
instead of an ACPI handle, the function won't need to call
acpi_bus_get_device(), which may be costly, any more. Then,
trim_stale_devices() can call acpiphp_no_hotplug() passing
the struct acpi_device object it already has directly to that
function.
Make those changes and update slot_no_hotplug() accordingly.

After recent PCI core changes related to the rescan/remove locking,
the ACPIPHP's disable_slot() function is only called under the
general PCI rescan/remove lock, so it doesn't have to use
dev_in_slot() any more to avoid race conditions. Make it simply
walk the devices on the bus and drop the ones in the slot being
disabled and drop dev_in_slot() which has no more users.
Moreover, to avoid problems described in the changelog of commit
29ed1f29b6 (PCI: pciehp: Fix null pointer deref when hot-removing
SR-IOV device), make disable_slot() carry out the list walk in
reverse order.

If a PCI bridge with an ACPIPHP context attached is removed via
sysfs, the code path executed as a result is the following:

Now, if a dock event affecting one of the bridge's child devices
occurs (roughly at the same time), it will lead to the following code
path:
acpi_dock_deferred_cb
dock_notify
handle_eject_request
hot_remove_dock_devices
dock_hotplug_event
hotplug_event (dereferences context)
That may lead to a kernel crash in hotplug_event() if it is executed
after the last kfree() in the bridge removal code path.
To prevent that from happening, add a wrapper around hotplug_event()
called dock_event() and point the .handler pointer in acpiphp_dock_ops
to it. Make that wrapper retrieve the device's ACPIPHP context using
acpiphp_get_context() (instead of taking it from the data argument)
under acpiphp_context_lock and check if the parent bridge's
is_going_away flag is set. If that flag is set, it will return
immediately and if it is not set it will grab a reference to the
device's parent bridge before executing hotplug_event().
Then, in the above scenario, the reference to the parent bridge
held by dock_event() will prevent free_bridge() from being executed
for it until hotplug_event() returns.

If a PCI bridge with an ACPIPHP context attached is removed via
sysfs, the code path executed as a result is the following:
pci_stop_and_remove_bus_device_locked
pci_remove_bus
pcibios_remove_bus
acpi_pci_remove_bus
acpiphp_remove_slots
cleanup_bridge
put_bridge
free_bridge
acpiphp_put_context (for each child, under context lock)
kfree (child context)
Now, if a hotplug notify is dispatched for one of the bridge's
children and the timing is such that handle_hotplug_event() for
that notify is executed while free_bridge() above is running,
the get_bridge(context->func.parent) in handle_hotplug_event()
will not really help, because it is too late to prevent the bridge
from going away and the child's context may be freed before
hotplug_event_work() scheduled from handle_hotplug_event()
dereferences the pointer to it passed via the data argument.
That will cause a kernel crash to happpen in hotplug_event_work().
To prevent that from happening, make handle_hotplug_event()
check the is_going_away flag of the function's parent bridge
(under acpiphp_context_lock) and bail out if it's set. Also,
make cleanup_bridge() set the bridge's is_going_away flag under
acpiphp_context_lock so that it cannot be changed between the
check and the subsequent get_bridge(context->func.parent) in
handle_hotplug_event().
Then, in the above scenario, handle_hotplug_event() will notice
that context->func.parent->is_going_away is already set and it
will exit immediately preventing the crash from happening.

Commit 9217a98467 (ACPI / hotplug / PCI: Use global PCI rescan-remove
locking) modified ACPIPHP to protect its PCI device removal and addition
code paths from races against sysfs-driven rescan and remove operations
with the help of PCI rescan-remove locking. However, it overlooked the
fact that hotplug_event_work() is not the only caller of hotplug_event()
which may also be called by dock_hotplug_event() and that code path
is missing the PCI rescan-remove locking. This means that, although
the PCI rescan-remove lock is held as appropriate during the handling
of events originating from handle_hotplug_event(), the ACPIPHP's
operations resulting from dock events may still suffer the race
conditions that commit 9217a98467 was supposed to eliminate.
To address that problem, move the PCI rescan-remove locking from
hotplug_event_work() to hotplug_event() so that it is used regardless
of the way that function is invoked.

According to the changelog of commit 29ed1f29b6 (PCI: pciehp: Fix null
pointer deref when hot-removing SR-IOV device) it is unsafe to walk the
bus->devices list of a PCI bus and remove devices from it in direct order,
because that may lead to NULL pointer dereferences related to virtual
functions.
For this reason, change all of the bus->devices list walks in
acpiphp_glue.c during which devices may be removed to be carried out in
reverse order.

The changes in the ACPI-based PCI hotplug (ACPIPHP) subsystem made
during the 3.12 development cycle uncovered a problem with VGA
switcheroo that on some systems, when the device-specific method
(ATPX in the radeon case, _DSM in the nouveau case) is used to turn
off the discrete graphics, the BIOS generates ACPI hotplug events for
that device and those events cause ACPIPHP to attempt to remove the
device from the system (they are events for a device that was present
previously and is not present any more, so that's what should be done
according to the spec). Then, the system stops functioning correctly.
Since the hotplug events in question were simply silently ignored
previously, the least intrusive way to address that problem is to
make ACPIPHP ignore them again. For this purpose, introduce a new
ACPI device flag, no_hotplug, and modify ACPIPHP to ignore hotplug
events for PCI devices whose ACPI companions have that flag set.
Next, make the radeon and nouveau switcheroo detection code set the
no_hotplug flag for the discrete graphics' ACPI companion.

Previously, the caller checked ATTN_LED() or PWR_LED() to see whether the
slot has indicators before setting the indicator state. That clutters the
caller unnecessarily, so this moves the test inside the callees. The test
may not even be necessary; per spec it should be harmless to try to turn on
a non-existent LED. But checking first does avoid unnecessary hotplug
commands.

There's not much point in checking the return value from every config space
access because the only likely errors are design-time things like unaligned

Replace direct inclusions of <acpi/acpi.h>, <acpi/acpi_bus.h> and
<acpi/acpi_drivers.h>, which are incorrect, with <linux/acpi.h>
inclusions and remove some inclusions of those files that aren't
necessary.
First of all, <acpi/acpi.h>, <acpi/acpi_bus.h> and <acpi/acpi_drivers.h>
should not be included directly from any files that are built for
CONFIG_ACPI unset, because that generally leads to build warnings about
undefined symbols in !CONFIG_ACPI builds. For CONFIG_ACPI set,
<linux/acpi.h> includes those files and for CONFIG_ACPI unset it
provides stub ACPI symbols to be used in that case.
Second, there are ordering dependencies between those files that always
have to be met. Namely, it is required that <acpi/acpi_bus.h> be included
prior to <acpi/acpi_drivers.h> so that the acpi_pci_root declarations the
latter depends on are always there. And <acpi/acpi.h> which provides
basic ACPICA type declarations should always be included prior to any other
ACPI headers in CONFIG_ACPI builds. That also is taken care of including
<linux/acpi.h> as appropriate.

acpiphp_enumerate_slots() walks ACPI namenamespace under
a PCI host bridge with callback register_slot().
register_slot() evaluates _ADR for all the device objects
and emits a warning message for any error. Some platforms
have _HID device objects (such as HPET and IPMI), which
trigger unnecessary warning messages.
This patch avoids emitting a warning message when a target
device object does not have _ADR.

Modify the ACPI namespace scanning code to register a struct
acpi_device object for every namespace node representing a device,
processor and so on, even if the device represented by that namespace
node is reported to be not present and not functional by _STA.
There are multiple reasons to do that. First of all, it avoids
quite a lot of overhead when struct acpi_device objects are
deleted every time acpi_bus_trim() is run and then added again
by a subsequent acpi_bus_scan() for the same scope, although the
namespace objects they correspond to stay in memory all the time
(which always is the case on a vast majority of systems).
Second, it will allow user space to see that there are namespace
nodes representing devices that are not present at the moment and may
be added to the system. It will also allow user space to evaluate
_SUN for those nodes to check what physical slots the "missing"
devices may be put into and it will make sense to add a sysfs
attribute for _STA evaluation after this change (that will be
useful for thermal management on some systems).
Next, it will help to consolidate the ACPI hotplug handling among
subsystems by making it possible to store hotplug-related information
in struct acpi_device objects in a standard common way.
Finally, it will help to avoid a race condition related to the
deletion of ACPI namespace nodes. Namely, namespace nodes may be
deleted as a result of a table unload triggered by _EJ0 or _DCK.
If a hotplug notification for one of those nodes is triggered
right before the deletion and it executes a hotplug callback
via acpi_hotplug_execute(), the ACPI handle passed to that
callback may be stale when the callback actually runs. One way
to work around that is to always pass struct acpi_device pointers
to hotplug callbacks after doing a get_device() on the objects in
question which eliminates the use-after-free possibility (the ACPI
handles in those objects are invalidated by acpi_scan_drop_device(),
so they will trigger ACPICA errors on attempts to use them).

Modify struct acpi_dev_node to contain a pointer to struct acpi_device
associated with the given device object (that is, its ACPI companion
device) instead of an ACPI handle corresponding to it. Introduce two
new macros for manipulating that pointer in a CONFIG_ACPI-safe way,
ACPI_COMPANION() and ACPI_COMPANION_SET(), and rework the
ACPI_HANDLE() macro to take the above changes into account.
Drop the ACPI_HANDLE_SET() macro entirely and rework its users to
use ACPI_COMPANION_SET() instead. For some of them who used to
pass the result of acpi_get_child() directly to ACPI_HANDLE_SET()
introduce a helper routine acpi_preset_companion() doing an
equivalent thing.
The main motivation for doing this is that there are things
represented by struct acpi_device objects that don't have valid
ACPI handles (so called fixed ACPI hardware features, such as
power and sleep buttons) and we would like to create platform
device objects for them and "glue" them to their ACPI companions
in the usual way (which currently is impossible due to the
lack of valid ACPI handles). However, there are more reasons
why it may be useful.
First, struct acpi_device pointers allow of much better type checking
than void pointers which are ACPI handles, so it should be more
difficult to write buggy code using modified struct acpi_dev_node
and the new macros. Second, the change should help to reduce (over
time) the number of places in which the result of ACPI_HANDLE() is
passed to acpi_bus_get_device() in order to obtain a pointer to the
struct acpi_device associated with the given "physical" device,
because now that pointer is returned by ACPI_COMPANION() directly.
Finally, the change should make it easier to write generic code that
will build both for CONFIG_ACPI set and unset without adding explicit
compiler directives to it.

There are two different interfaces for queuing up work items on the
ACPI hotplug workqueue, alloc_acpi_hp_work() used by PCI and PCI host
bridge hotplug code and acpi_os_hotplug_execute() used by the common
ACPI hotplug code and docking stations. They both are somewhat
cumbersome to use and work slightly differently.
The users of alloc_acpi_hp_work() have to submit a work function that
will extract the necessary data items from a struct acpi_hp_work
object allocated by alloc_acpi_hp_work() and then will free that
object, while it would be more straightforward to simply use a work
function with one more argument and let the interface take care of
the execution details.
The users of acpi_os_hotplug_execute() also have to deal with the
fact that it takes only one argument in addition to the work function
pointer, although acpi_os_execute_deferred() actually takes care of
the allocation and freeing of memory, so it would have been able to
pass more arguments to the work function if it hadn't been
constrained by the connection with acpi_os_execute().
Moreover, while alloc_acpi_hp_work() makes GFP_KERNEL memory
allocations, which is correct, because hotplug work items are
always queued up from process context, acpi_os_hotplug_execute()
uses GFP_ATOMIC, as that is needed by acpi_os_execute(). Also,
acpi_os_execute_deferred() queued up by it waits for the ACPI event
workqueues to flush before executing the work function, whereas
alloc_acpi_hp_work() can't do anything similar. That leads to
somewhat arbitrary differences in behavior between various ACPI
hotplug code paths and has to be straightened up.
For this reason, replace both alloc_acpi_hp_work() and
acpi_os_hotplug_execute() with a single interface,
acpi_hotplug_execute(), combining their behavior and being more
friendly to its users than any of the two.

Commit 2dc4128 (ACPI / hotplug / PCI: Avoid doing too much for
spurious notifies) changed the enable_slot() to check return value of
pci_scan_slot() and if it is zero return early from the function. It
means that there were no new devices in this particular slot.
However, if a device appeared deeper in the hierarchy the code now
ignores it causing things like Thunderbolt chaining fail to recognize
new devices.
The problem with Alex Williamson's machine was solved with commit
a47d8c8 (ACPI / hotplug / PCI: Avoid parent bus rescans on spurious
device checks) and hence we should be able to restore the original
functionality that we always rescan on bus check notification.
On a device check notification we still check what acpiphp_rescan_slot()
returns and on zero bail out early.

The WARN_ON() in acpiphp_enumerate_slots() triggers unnecessarily for
devices whose bridges are going to be handled by native PCIe hotplug
(pciehp) and the simplest way to prevent that from happening is to
drop the WARN_ON().

acpi_has_method() is a new ACPI API introduced to check
the existence of an ACPI control method.
It can be used to replace acpi_get_handle() in the case that
1. the calling function doesn't need the ACPI handle of the control method.
and
2. the calling function doesn't care the reason why the method is unavailable.
Convert acpi_get_handle() to acpi_has_method()

In the current ACPIPHP notify handler we always go directly for a
rescan of the parent bus if we get a device check notification for
a device that is not a bridge. However, this obviously is
overzealous if nothing really changes, because this way we may rescan
the whole PCI hierarchy pretty much in vain.
That happens on Alex Williamson's machine whose ACPI tables contain
device objects that are supposed to coresspond to PCIe root ports,
but those ports aren't physically present (or at least they aren't
visible in the PCI config space to us). The BIOS generates multiple
device check notifies for those objects during boot and for each of
them we go straight for the parent bus rescan, but the parent bus is
the root bus in this particular case. In consequence, we rescan the
whole PCI bus from the top several times in a row, which is
completely unnecessary, increases boot time by 50% (after previous
fixes) and generates excess dmesg output from the PCI subsystem.
Fix the problem by checking if we can find anything new in the
slot corresponding to the device we've got a device check notify
for and doing nothig if that's not the case.
The spec (ACPI 5.0, Section 5.6.6) appears to mandate this behavior,
as it says:
Device Check. Used to notify OSPM that the device either appeared
or disappeared. If the device has appeared, OSPM will re-enumerate
from the parent. If the device has disappeared, OSPM will
invalidate the state of the device. OSPM may optimize out
re-enumeration.
Therefore, according to the spec, we are free to do nothing if
nothing changes.

The spec suggests that we should use _OST to notify the platform
about the status of notifications it sends us, for example so that
it doesn't repeate a notification that has been handled already.
This turns out to help reduce the amount of diagnostic output from
the ACPIPHP subsystem and speed up boot on at least one system that
generates multiple device check notifies for PCIe devices on the root
bus during boot.

a hotplug device and in those cases we should avoid doing all of the
configuration work needed when something actually changes. To that
end, check the return value of pci_scan_slot() in enable_slot() and
bail out early if it is 0.

In acpiphp_bus_add() we first remove device objects corresponding to
the given handle and the ACPI namespace branch below it, which are
then re-created by acpi_bus_scan(). This used to be done to clean
up after surprise removals, but now we do the cleanup through
trim_stale_devices() which checks if the devices in question are
actually gone before removing them, so the device hierarchy trimming
in acpiphp_bus_add() is not necessary any more and, moreover, it may
lead to problems if it removes device objects corresponding to
devices that are actually present.
For this reason, remove the leftover acpiphp_bus_trim() from
acpiphp_bus_add().

All users of pcie_bus_configure_settings() pass arguments of the form
"bus, bus->self->pcie_mpss". The "mpss" argument is redundant since we
can easily look it up internally. In addition, all callers check
"bus->self" for NULL, which we can also do internally.
This patch simplifies the interface and the callers. No functional change.

After commit bbd34fc (ACPI / hotplug / PCI: Register all devices
under the given bridge) register_slot() is called for all PCI
devices under a given bridge that have corresponding objects in
the ACPI namespace, but it calls acpiphp_register_hotplug_slot()
only for devices satisfying specific criteria. Still,
cleanup_bridge() calls acpiphp_unregister_hotplug_slot() for all
objects created by register_slot(), although it should only call it
for the ones that acpiphp_register_hotplug_slot() has been called
for (successfully). This causes a NULL pointer to be dereferenced
by the acpiphp_unregister_hotplug_slot() executed by cleanup_bridge()
if the object it is called for has not been passed to
acpiphp_register_hotplug_slot().
To fix this problem, check if the 'slot' field of the object passed
to acpiphp_unregister_hotplug_slot() in cleanup_bridge() is not NULL,
which only is the case if acpiphp_register_hotplug_slot() has been
executed for that object. In addition to that, make register_slot()
reset the 'slot' field to NULL if acpiphp_register_hotplug_slot() has
failed for the given object to prevent stale pointers from being
used by acpiphp_unregister_hotplug_slot().

PCIe hotplug has a bus per slot, so we can just use a normal
secondary bus reset. However, if a slot supports surprise removal,
a bus reset can be seen as a presence detection change triggering
a hot-remove followed by a hot-add. Disable presence detection from
triggering an interrupt or being polled around the bus reset.

Hot-removing a device with SR-IOV enabled causes a null pointer dereference
in v3.9 and v3.10.
This is a regression caused by ba518e3c17 ("PCI: pciehp: Iterate over all
devices in slot, not functions 0-7"). When we iterate over the
bus->devices list, we first remove the PF, which also removes all the VFs
from the list. Then the list iterator blows up because more than just the
current entry was removed from the list.
ac205b7bb7 ("PCI: make sriov work with hotplug remove") works around a
similar problem in pci_stop_bus_devices() by iterating over the list in
reverse, so the VFs are stopped and removed from the list first, before the
PF.
This patch changes pciehp_unconfigure_device() to iterate over the list in
reverse, too.

We currently enable PCI bridges after scanning a bus and assigning
resources. This is often done in arch code.
This patch changes this so we don't enable a bridge until necessary, i.e.,
until we enable a PCI device behind the bridge. We do this in the generic
pci_enable_device() path, so this also removes the arch-specific code to
enable bridges.

Now that acpiphp_check_bridge() always enumerates devices behind the
bridge, there is no need to do that for each sub-bridge anymore like
it is done in the current ACPI-based PCI hotplug (ACPIPHP) code.
Given this we don't need check_sub_bridges() anymore, so drop that
function completely.
This also simplifies the ACPIPHP code a bit.

Do not acquire bridge_mutex around the addition of a slot to its
bridge's list of slots and arount the addition of a function to
its slot's list of functions, because that doesn't help anything
right now (those lists are walked without any locking anyway).
However, acquire bridge_mutex around the list walk in
acpiphp_remove_slots() and use list_for_each_entry() there,
because we terminate the walk as soon as we find the first matching
entry. This prevents that list walk from colliding with bridge
addition and removal.

The current implementation of acpiphp_check_bridge() is pretty dumb:
- It enables a slot if it's not enabled and the slot status is
ACPI_STA_ALL.
- It disables a slot if it's enabled and the slot status is not
ACPI_STA_ALL.
This behavior is not sufficient to handle the Thunderbolt daisy
chaining case properly, however, because in that case the bus
behind the already enabled slot needs to be rescanned for new
devices.
For this reason, modify acpiphp_check_bridge() so that slots are
disabled and stopped if they are not in the ACPI_STA_ALL state.
For slots in the ACPI_STA_ALL state, devices behind them that don't
respond are trimmed using a new function, trim_stale_devices(),
introduced specifically for this purpose. That function walks
the given bus and checks each device on it. If the device doesn't
respond, it is assumed to be gone and is removed.
Once all of the stale devices directy behind the slot have been
removed, acpiphp_check_bridge() will start looking for new devices
that might have appeared on the given bus. It will do that even if
the slot is already enabled (SLOT_ENABLED is set for it).
In addition to that, make the bus check notification ignore
SLOT_ENABLED and go for enable_device() directly if bridge is NULL,
so that devices behind the slot are re-enumerated in that case too.
This change is based on earlier patches from Kirill A Shutemov
and Mika Westerberg.

Currently, enable_device() checks the return value of pci_scan_slot()
and returns immediately if that's 0 (meaning that no new functions
have been found in the slot). However, if one of the functions in
the slot is a bridge, some new devices may appear below it even if
the bridge itself is present continuously, so it generally is
necessary to do the rescan anyway just in case. [In particular,
that's necessary with the Thunderbolt daisy chaining in which case
new devices may be connected to the existing ones down the chain.]
The correctness of this change relies on the ability of
pcibios_resource_survey_bus() to detect if it has already been called
for the given bus and to skip it if so. Failure to do that will lead
to resource allocation conflicts.

With Thunderbolt you can daisy chain devices: connect new devices to
an already plugged one. In that case the "hotplug slot" is already
enabled, but we still want to look for new PCI devices behind it.
Reuse enable_device() to scan for new PCI devices on enabled slots
and push the SLOT_ENABLED check up into acpiphp_enable_slot().

The ACPI-based PCI hotplug (acpiphp) core code need not and really
should not execute _PS0 and _PS3 directly for devices it handles.
First of all, it is not necessary to put devices into D3 after
acpi_bus_trim() has walked through them, because
acpi_device_unregister() invoked by it puts each device into D3cold
before returning. Thus after disable_device() the slot should be
powered down already.
Second, calling _PS0 directly on ACPI device objects may not be
appropriate, because it may require power resources to be set up in
a specific way in advance and that must be taken care of by the ACPI
core. Thus modify acpiphp_bus_add() to power up the device using
the appropriate interface after it has run acpi_bus_scan() on its
handle.
After that, the functions executing _PS0 and _PS3, power_on_slot()
and power_off_slot(), are not necessary any more, so drop them
and update the code calling them accordingly. Also drop the
function flags related to device power states, since they aren't
useful any more too.

Modify handle_hotplug_event() to avoid queing up the execution of
handle_hotplug_event_work_fn() as a work item on kacpi_hotplug_wq
for non-hotplug events, such as ACPI_NOTIFY_DEVICE_WAKE. Move
the code printing diagnostic messages for those events into
handle_hotplug_event().
In addition to that, remove the bogus comment about how the core
should distinguish between hotplug and non-hotplug events and
queue them up on different workqueues. The core clearly cannot
know in advance what events will be interesting to the given
caller of acpi_install_notify_handler().

Both acpiphp_disable_slot() and acpiphp_eject_slot() are always
called together so instead of calling each separately we can
consolidate them into one function acpiphp_disable_and_eject_slot()
that does both (but it will return success on _EJ0 failures that
were ignored in the majority of call sites anyway).

The ACPI handle stored in struct acpiphp_func is also stored in the
struct acpiphp_context object containing it and it is trivial to get
from a struct acpiphp_func pointer to the handle field of the outer
struct acpiphp_context.
Hence, the handle field of struct acpiphp_func is redundant, so drop
it and provide a helper function, func_to_handle(), allowing it
users to get the ACPI handle for the given struct acpiphp_func
pointer.

The only bridge flag used by the ACPI-based PCI hotplug (ACPIPHP)
code is BRIDGE_HAS_EJ0, but it is only used by the event handling
function hotplug_event() and if that flag is set, the corresponding
function flag FUNC_HAS_EJ0 is set as well, so that bridge flag is
redundant.
For this reason, drop BRIDGE_HAS_EJ0 and all code referring to it
and since it is the only bridge flag defined, drop the flags field
from struct acpiphp_bridge entirely.

If the slot unique number is passed as an additional argument to
acpiphp_register_hotplug_slot(), the 'sun' field in struct
acpiphp_slot is only used by ibm_[s|g]et_attention_status(),
but then it's more efficient to store it in struct slot.
Thus move the 'sun' field from struct acpiphp_slot to struct slot
changing its data type to unsigned int in the process, and redefine
acpiphp_register_hotplug_slot() to take the slot number as separate
argument.

Rework register_slot() to create a struct acpiphp_func object for
every function it is called for and to create acpiphp slots for all
of them. Although acpiphp_register_hotplug_slot() is only called for
the slots whose functions are identified as "ejectable", so that user
space can manipulate them, the ACPIPHP notify handler,
handle_hotplug_event(), is now installed for all of the registered
functions (that aren't dock stations) and hotplug events may be
handled for all of them.
As a result, essentially, all PCI bridges represented by objects in
the ACPI namespace are now going to be "hotplug" bridges and that may
affect resources allocation in general, although it shouldn't lead to
problems.
This allows the code to be simplified substantially and addresses
the problem where bus check or device check notifications for some
PCI bridges or devices are not handled, because those devices are
not recognized as "ejectable" or there appear to be no "ejectable"
devices under those bridges.

There are separate handling event functions for hotplug bridges and
for hotplug functions, but they may be combined into one common
hotplug event handling function which simplifies the code slightly.
That also allows a theoretical bug to be dealt with which in
principle may occur if a hotplug bridge is on a dock station, because
in that case the bridge-specific notification should be used instead
of the function-specific one, but the dock station always uses the
latter.

Using the hotplug context objects introduced previously rework the

for ACPI device objects corresponding to the hotplug PCI devices are
handled by one function, handle_hotplug_event(), which recognizes
whether it has to handle a bridge or a function.
In addition to code size reduction it allows some ugly pieces of code
where notify handlers have to be uninstalled and installed again to
go away. Moreover, it fixes a theoretically possible race between
handle_hotplug_event() and free_bridge() tearing down data structures
for the same handle.

When either a new hotplug bridge or a new hotplug function is added
by the ACPI-based PCI hotplug (ACPIPHP) code, attach a context object
to its ACPI handle to store hotplug-related information in it. To
start with, put the handle's bridge and function pointers into that
object. Count references to the context objects and drop them when
they are not needed any more.
First of all, this makes it possible to find out if the given bridge
has been registered as a function already in a much more
straightforward way and acpiphp_bridge_handle_to_function() can be
dropped (Yay!).
This also will allow some more simplifications to be made going
forward.

When a new ACPIPHP function is added by register_slot() and the
notify handler cannot be installed for it, register_slot() returns an
error status without cleaning up, which causes the entire namespace
walk in acpiphp_enumerate_slots() to be aborted, although it still
may be possible to successfully install the function notify handler
for other device objects under the given brigde.
To address this issue make register_slot() return success after
a new function has been added, even if the addition of the notify
handler for it has failed.

The acpiphp_enumerate_slots() function is now split into two parts,
acpiphp_enumerate_slots() proper and init_bridge_misc() which is
only called by the former. If these functions are combined,
it is possible to make the code easier to follow and to clean up
the error handling (to prevent memory leaks on error from
happening in particular), so do that.

Since acpi_pci_slot_enumerate() and acpiphp_enumerate_slots() can get
the ACPI device handle they need from bus->bridge, it is not
necessary to pass that handle to them as an argument.
Drop the second argument of acpi_pci_slot_enumerate() and
acpiphp_enumerate_slots(), rework them to obtain the ACPI handle
from bus->bridge and make acpi_pci_add_bus() and
acpi_pci_remove_bus() entirely symmetrical.

The only user of the ACPI dock notifier chain is the ACPI-based PCI
hotplug (acpiphp) driver that uses it to carry out post-dock fixups
needed by some systems with broken _DCK. However, it is not
necessary to use a separate notifier chain for that, as it can be
simply replaced with a new callback in struct acpi_dock_ops.
For this reason, add a new .fixup() callback to struct acpi_dock_ops
and make hotplug_dock_devices() execute it for all dock devices with
hotplug operations registered. Accordingly, make acpiphp point that
callback to the function carrying out the post-dock fixups and
do not register a separate dock notifier for each device
registering dock operations. Finally, drop the ACPI dock notifier
chain that has no more users.

For the workqueue creation interfaces that do not expect format strings,
make sure they cannot accidently be parsed that way. Additionally, clean
up calls made with a single parameter that would be handled as a format
string. Many callers are passing potentially dynamic string content, so
use "%s" in those cases to avoid any potential accidents.

The disable slot implementation on s390 currently just detaches the
pci function from the partition - without informing the pci layer.
Fix this by calling pci_stop_and_remove_bus_device prior to the
operation.

The interactions between the ACPI dock driver and the ACPI-based PCI
hotplug (acpiphp) are currently problematic because of ordering
issues during hot-remove operations.
First of all, the current ACPI glue code expects that physical
devices will always be deleted before deleting the companion ACPI
device objects. Otherwise, acpi_unbind_one() will fail with a
warning message printed to the kernel log, for example:

This means, in particular, that struct pci_dev objects have to
be deleted before the struct acpi_device objects they are "glued"
with.
Now, the following happens the during the undocking of an ACPI-based
dock station:
1) hotplug_dock_devices() invokes registered hotplug callbacks to
destroy physical devices associated with the ACPI device objects
depending on the dock station. It calls dd->ops->handler() for
each of those device objects.
2) For PCI devices dd->ops->handler() points to
handle_hotplug_event_func() that queues up a separate work item
to execute _handle_hotplug_event_func() for the given device and
returns immediately. That work item will be executed later.
3) hotplug_dock_devices() calls dock_remove_acpi_device() for each
device depending on the dock station. This runs acpi_bus_trim()
for each of them, which causes the underlying ACPI device object
to be destroyed, but the work items queued up by
handle_hotplug_event_func() haven't been started yet.
4) _handle_hotplug_event_func() queued up in step 2) are executed
and cause the above failure to happen, because the PCI devices
they handle do not have the companion ACPI device objects any
more (those objects have been deleted in step 3).
The possible breakage doesn't end here, though, because
hotplug_dock_devices() may return before at least some of the
_handle_hotplug_event_func() work items spawned by it have a
chance to complete and then undock() will cause _DCK to be
evaluated and that will cause the devices handled by the
_handle_hotplug_event_func() to go away possibly while they are
being accessed.
This means that dd->ops->handler() for PCI devices should not point
to handle_hotplug_event_func(). Instead, it should point to a
function that will do the work of _handle_hotplug_event_func()
synchronously. For this reason, introduce such a function,
hotplug_event_func(), and modity acpiphp_dock_ops to point to
it as the handler.
Unfortunately, however, this is not sufficient, because if the dock
code were not changed further, hotplug_event_func() would now
deadlock with hotplug_dock_devices() that called it, since it would
run unregister_hotplug_dock_device() which in turn would attempt to
acquire the dock station's hp_lock mutex already acquired by
hotplug_dock_devices().
To resolve that deadlock use the observation that
unregister_hotplug_dock_device() won't need to acquire hp_lock
if PCI bridges the devices on the dock station depend on are
prevented from being removed prematurely while the first loop in
hotplug_dock_devices() is in progress.
To make that possible, introduce a mechanism by which the callers of
register_hotplug_dock_device() can provide "init" and "release"
routines that will be executed, respectively, during the addition
and removal of the physical device object associated with the
given ACPI device handle. Make acpiphp use two new functions,
acpiphp_dock_init() and acpiphp_dock_release(), that call
get_bridge() and put_bridge(), respectively, on the acpiphp bridge
holding the given device, for this purpose.
In addition to that, remove the dock station's list of
"hotplug devices" and make the dock code always walk the whole list
of "dependent devices" instead in such a way that the loops in
hotplug_dock_devices() and dock_event() (replacing the loops over
"hotplug devices") will take references to the list entries that
register_hotplug_dock_device() has been called for. That prevents
the "release" routines associated with those entries from being
called while the given entry is being processed and for PCI
devices this means that their bridges won't be removed (by a
concurrent thread) while hotplug_event_func() handling them is
being executed.
This change is based on two earlier patches from Jiang Liu.

On x86 platforms, the kernel respects PCI resource assignments from
the BIOS and only reassigns resources for unassigned BARs at boot
time. However, with the ACPI-based hotplug (acpiphp), it ignores the
BIOS' PCI resource assignments completely and reassigns all resources
by itself. This causes differences in PCI resource allocation
between boot time and runtime hotplug to occur, which is generally
undesirable and sometimes actively breaks things.
Namely, if there are enough resources, reassigning all PCI resources
during runtime hotplug should work, but it may fail if the resources
are constrained. This may happen, for instance, when some PCI
devices with huge MMIO BARs are involved in the runtime hotplug
operations, because the current PCI MMIO alignment algorithm may
waste huge chunks of MMIO address space in those cases.
On the Alexander's Sony VAIO VPCZ23A4R the BIOS allocates limited
MMIO resources for the dock station which contains a device
(graphics adapter) with a 256MB MMIO BAR. An attempt to reassign
that during runtime hotplug causes the dock station MMIO window to be
exhausted and acpiphp fails to allocate resources for the majority
of devices on the dock station as a result.
To prevent that from happening, modify acpiphp to follow the boot
time resources allocation behavior so that the BIOS' resource
assignments are respected during runtime hotplug too.

When a PCI host bridge device receives a Bus Check notification, we
must re-enumerate starting with the bridge to discover changes (devices
that have been added or removed).
Prior to 668192b678 ("PCI: acpiphp: Move host bridge hotplug to
pci_root.c"), this happened in _handle_hotplug_event_bridge(). After that
commit, _handle_hotplug_event_bridge() is not installed for host bridges,
and the host bridge notify handler, _handle_hotplug_event_root() did not
re-enumerate.
This patch adds re-enumeration to _handle_hotplug_event_root().
This fixes cases where we don't notice the addition or removal of
PCI devices, e.g., the PCI-to-USB ExpressCard in the bugzilla below.

The __must_check (gcc "warn_unused_result") attribute only makes sense
when compiling the *caller* of the function, so the attribute should
appear on the declaration in the header file, not on the definition.

Now acpiphp_enumerate_slots() and acpiphp_remove_slots() may be invoked
concurrently by the PCI core, so add a bridge_mutex and reference count
mechanism to protect acpiphp bridge/slot/function data structures.
To avoid deadlock, handle_hotplug_event_bridge() will requeue the
hotplug event onto the kacpi_hotplug_wq by calling alloc_acpi_hp_work().
But the workaround has introduced a minor race window because the
'bridge' passed to _handle_hotplug_event_bridge() may have already been
destroyed when _handle_hotplug_event_bridge() is actually executed by
the kacpi_hotplug_wq. So hold a reference count on the passed 'bridge'.
Fix the same issue for handle_hotplug_event_func() too.

Use PCI_EXP_SLTCAP_PSN mask to make it easier to find where the
Physical Slot Number is used.
The Physical Slot Number is bits 31:19 of the Slot Capabilities Register,
and slot_cap is a u32, so the mask is technically unnecessary, but it's
helpful for human readers.

cpci_hotplug_init() and cpci_hotplug_exit() are defined in
cpci_hotplug_core.c but had extern declarations in pci_hotplug_core.c.
This puts the declarations in a header file included both places so
the compiler can help keep everything consistent.

Previously the acpiphp driver registered itself as an ACPI PCI subdriver,
so its callbacks were invoked when creating/destroying PCI root
buses to manage ACPI-based PCI hotplug slots. But it doesn't handle
P2P bridge hotplug events, so it will cause strange behaviour if there
are hotplug slots associated with a hot-removed P2P bridge.
This patch fixes this issue by:
1) Directly hooking into PCI core to update hotplug slot devices when
creating/destroying PCI buses through:
pci_{add|remove}_bus() -> acpi_pci_{add|remove}_bus()
2) Getting rid of unused ACPI PCI subdriver-related code
It also cleans up unused code in the acpiphp driver.

Convert acpiphp to be builtin only, with no module option.
Previously, when HOTPLUG_PCI_ACPI=m, users could disable acpiphp by
removing the module or preventing it from loading. That can't be done
if acpiphp is builtin statically, so this adds an "acpiphp.disable"
kernel parameter. If a user needs to use this parameter, it is a bug,
and we want to hear about it.

Currently function disable_device() detects slot state by checking
existence of PCI function 0. It's unreliable because the PCI device
for function 0 may be removed through the sysfs interface. If that
happens, it will cause powering off a hotplug slot without destroying
all PCI devices.
On the other hand, it won't hurt us except wasting some computation
power if the check is removed, because all code of disable_device()
is self-protected. So remove the check.

Loading the pci hotplug module when no devices are present will fail
but unfortunately some hotplug callbacks stay registered to the pci
bus level. Fix this by not letting module loading fail when no pci
devices are present and provide proper {de}registration functions
for these callbacks.

This changeset is aimed at fixing a few different but related
problems in the ACPI hotplug infrastructure.
First of all, since notify handlers may be run in parallel with
acpi_bus_scan(), acpi_bus_trim() and acpi_bus_hot_remove_device()
and some of them are installed for ACPI handles that have no struct
acpi_device objects attached (i.e. before those objects are created),
those notify handlers have to take acpi_scan_lock to prevent races
from taking place (e.g. a struct acpi_device is found to be present
for the given ACPI handle, but right after that it is removed by
acpi_bus_trim() running in parallel to the given notify handler).
Moreover, since some of them call acpi_bus_scan() and
acpi_bus_trim(), this leads to the conclusion that acpi_scan_lock
should be acquired by the callers of these two funtions rather by
these functions themselves.
For these reasons, make all notify handlers that can handle device
addition and eject events take acpi_scan_lock and remove the
acpi_scan_lock locking from acpi_bus_scan() and acpi_bus_trim().
Accordingly, update all of their users to make sure that they
are always called under acpi_scan_lock.
Furthermore, since eject operations are carried out asynchronously
with respect to the notify events that trigger them, with the help
of acpi_bus_hot_remove_device(), even if notify handlers take the
ACPI scan lock, it still is possible that, for example,
acpi_bus_trim() will run between acpi_bus_hot_remove_device() and
the notify handler that scheduled its execution and that
acpi_bus_trim() will remove the device node passed to
acpi_bus_hot_remove_device() for ejection. In that case, the struct
acpi_device object obtained by acpi_bus_hot_remove_device() will be
invalid and not-so-funny things will ensue. To protect agaist that,
make the users of acpi_bus_hot_remove_device() run get_device() on
ACPI device node objects that are about to be passed to it and make
acpi_bus_hot_remove_device() run put_device() on them and check if
their ACPI handles are not NULL (make acpi_device_unregister() clear
the device nodes' ACPI handles for that check to work).
Finally, observe that acpi_os_hotplug_execute() actually can fail,
in which case its caller ought to free memory allocated for the
context object to prevent leaks from happening. It also needs to
run put_device() on the device node that it ran get_device() on
previously in that case. Modify the code accordingly.

dead code left in acpiphp. So remove those dead code.
Now companion ACPI devices are always created before corresponding
PCI devices. And the ACPI event handle_hotplug_event_bridge() will be
installed only if it has associated PCI device. So remove dead code to
handle bridge hot-adding in function handle_hotplug_event_bridge().

With commit 4f535093cf "PCI: Put pci_dev in device tree as
early as possible", companion ACPI devices should be created before
creating corresponding PCI devices, otherwise it will break the ACPI
PCI binding logic.
Without this patch, the /sys/bus/pci/devices/.../firmware_node symlink
is missing after hot-removing and hot-adding a device with acpiphp.

Could have root bus hot-added later and there may be slots that need
acpiphp.
The result returned by acpiphp_get_num_slots() is meaningless, because
the bridge the slots are under may be added after this function has been
called, so drop acpiphp_get_num_slots() and the code using it.

The acpiphp driver is confusing because it contains partial support for PCI
host bridge hotplug as well as support for hotplug of PCI devices.
This patch moves the host bridge hot-add support to pci_root.c and adds
hot-remove support in pci_root.c.
How to test it: if sci_emu patch is applied, find out root bus number to
ACPI root name mapping from dmesg or /sys. To remove root bus:
echo "\_SB.PCIB 3" > /sys/kernel/debug/acpi/sci_notify
To add back root bus:
echo "\_SB.PCIB 1" > /sys/kernel/debug/acpi/sci_notify

When system support hotplug bridge with children hotplug slots, we need
to make sure that parent bridge get preallocated resource so later when
device is plugged into children slot, those children devices will get
resource allocated.
We do not meet this problem, because for PCIe hotplug card, when acpiphp
is used, pci_scan_bridge will set that for us when detect hotplug bit in
slot cap.

Currently, we enumerate devices in a slot with pci_scan_slot(), then
iterate through all the devices we found by looking for functions 0-7. But
that's wrong for ARI devices, which may have function numbers up to 255.
This means that when we hot-add an ARI device, pciehp only initializes
functions 0-7, and other functions don't work correctly. Additionally, if
we hot-remove the device, pciehp only removes functions 0-7, leaving stale
pci_dev structures for any other functions.
This patch fixes the problem by iterating over devices in a slot by using
the upstream bridge's "bus->devices" list instead.

The only difference between acpi_bus_scan() and acpi_bus_add() is the
invocation of acpi_update_all_gpes() in the latter which in fact is
unnecessary, because acpi_update_all_gpes() has already been called
by acpi_scan_init() and the way it is implemented guarantees the next
invocations of it to do nothing.
For this reason, drop acpi_bus_add() and make all its callers use
acpi_bus_scan() directly instead of it. Additionally, rearrange the
code in acpi_scan_init() slightly to improve the visibility of the
acpi_update_all_gpes() call in there.

When we have an SHPC-capable bridge with a second SHPC-capable bridge
below it, pushing the upstream bridge's attention button causes a
deadlock.
The deadlock happens because we use the shpchp_wq workqueue to run
shpchp_pushbutton_thread(), which uses shpchp_disable_slot() to remove
devices below the upstream bridge. When we remove the downstream bridge,
we call shpc_remove(), the shpchp driver's .remove() method. That calls
flush_workqueue(shpchp_wq), which deadlocks because the
shpchp_pushbutton_thread() work item is still running.
This patch avoids the deadlock by creating a workqueue for every slot
and removing the single shared workqueue.

Use non-ordered workqueue for attention button events.
Attention button events on each slot can be handled asynchronously. So
we should use non-ordered workqueue. This patch also removes ordered
workqueue in shpchp as a result.
486b10b9f4 ("PCI: pciehp: Handle push button event asynchronously") made
the same change to pciehp. I split this out from a patch by Yijing Wang
<wangyijing@huawei.com> so we fix one thing at a time and to make the
shpchp history correspond more closely with the pciehp history.

The callers of acpi_bus_add() usually assume that if it has
succeeded, then a struct acpi_device object has been attached to
the handle passed as the first argument. Unfortunately, however,
this assumption is wrong, because acpi_bus_scan(), and acpi_bus_add()
too as a result, may return a pointer to a different struct
acpi_device object on success (it may be an object corresponding to
one of the descendant ACPI nodes in the namespace scope below that
handle).
For this reason, the callers of acpi_bus_add() who care about
whether or not a struct acpi_device object has been created for
its first argument need to check that using acpi_bus_get_device()
anyway, so the second argument of acpi_bus_add() is not really
useful for them. The same observation applies to acpi_bus_scan()
executed directly from acpi_scan_init().
Therefore modify the relevant callers of acpi_bus_add() to check the
existence of the struct acpi_device in question with the help of
acpi_bus_get_device() and drop the no longer necessary second
argument of acpi_bus_add(). Accordingly, modify acpi_scan_init() to
use acpi_bus_get_device() to get acpi_root and drop the no longer
needed second argument of acpi_bus_scan().

Notice that acpi_bus_add() uses only 2 of its 4 arguments and
redefine its header to match the body. Update all of its callers as
necessary and observe that this leads to quite a number of removed
lines of code (Linus will like that).
Add a kerneldoc comment documenting acpi_bus_add() and wonder how
its callers make wrong assumptions about the second argument (make
note to self to take care of that later).

The ACPI PCI root bridge driver was the only ACPI driver implementing
the .start() callback, which isn't used by any ACPI drivers any more
now.
For this reason, acpi_start_single_object() has no purpose any more,
so remove it and all references to it. Also remove
acpi_bus_start_device(), whose only purpose was to call
acpi_start_single_object().
Moreover, since after the removal of acpi_bus_start_device() the
only purpose of acpi_bus_start() remains to call
acpi_update_all_gpes(), move that into acpi_bus_add() and drop
acpi_bus_start() too, remove its header from acpi_bus.h and
update all of its former users accordingly.
This change was previously proposed in a different from by
Yinghai Lu.

Add SCLP PCI configure/deconfigure and implement a PCI hotplug
controller (s390_pci_hpc). The hotplug controller creates a slot
for every PCI function in stand-by or configured state. The PCI
functions are named after the PCI function ID (fid). By writing to
the power attribute in /sys/bus/pci/slots/<fid>/power the PCI function
is moved to stand-by or configured state. If moved to the configured
state the device is automatically scanned by the s390 PCI layer.

Commit 0d52f54e2e (PCI / ACPI: Make acpiphp
ignore root bridges using PCIe native hotplug) added code that made the
acpiphp driver completely ignore PCIe root complexes for which the kernel
had been granted control of the native PCIe hotplug feature by the BIOS
through _OSC. Later commit 619a5182d1
"PCI hotplug: Always allow acpiphp to handle non-PCIe bridges" relaxed
the constraints to allow acpiphp driver handle non-PCIe bridges under
such a complex. The constraint needs to be relaxed further to allow
acpiphp driver to handle PCIe ports without native PCIe hotplug capability.
Some MR-IOV switch chipsets, such PLX8696, support multiple virtual PCIe
switches and may migrate downstream ports among virtual switches. To
migrate a downstream port from the source virtual switch to the target, the
port needs to be hot-removed from the source and hot-added into the target.
The pciehp driver can't be used here because there are no slots within the
virtual PCIe switch. So acpiphp driver is used to support downstream port
migration. A typical configuration is as below:

Here acpiphp driver will be used to handle root ports and upstream port

This patch changes .add/.remove interfaces of acpi_pci_driver.
In the current implementation acpi_handle is passed as a parameter
of .add/.remove interface. However, the acpi_pci_root structure
contains more useful information than just the acpi_handle. This
enables us to avoid some useless lookups in each acpi_pci_driver.
Note: This changes interfaces used by acpi_pci_register_driver(), an
exported symbol. This patch updates all the in-kernel users, but any
out-of-kernel acpi_pci_register_driver() users will need updates.

Function eeh_rmv_from_parent_pe() could be called by the path of
either normal PCI hotplug, or EEH recovery. For the former case,
we need purge the corresponding PE on removal of the associated
PE bus.
The patch tries to cover that by passing more information to function
pcibios_remove_pci_devices() so that we know if the corresponding PE
needs to be purged or be marked as "invalid".

Following code has a race window between pci_find_bus() and pci_get_slot()
if PCI hotplug operation happens between them which removes the pci_bus.
So use PCI hotplug safe interface pci_get_domain_bus_and_slot() instead,
which also reduces code complexity.

register & 0x0B == 0x04 is never true, the code will always take the
(identical) 0x00 path anyway. Remove the dead code paths.

Previously, the driver ignored resume unless the pciehp_force module_param
was specified. On some laptops that means that interrupts are not
delivered after S3, so card removals and insertions are not handled.
This patch makes the driver handle resume regardless of pciehp_force.

The fakephp driver was scheduled for removal in 2011.
Fakephp presented /sys/bus/pci/slots/.../power files for every PCI
function. Writing "0" to one of these files logically removed the device
from the system. The PCI core now provides the same functionality with
/sys/bus/pci/devices/.../remove.
