69c7b2fe4c libceph: fix race between delayed_work() and ceph_monc_stop()
The way the delayed work is handled in ceph_monc_stop() is prone to
races with mon_fault() and possibly also finish_hunting().  Both of
these can requeue the delayed work which wouldn't be canceled by any of
the following code in case that happens after cancel_delayed_work_sync()
runs -- __close_session() doesn't mess with the delayed work in order
to avoid interfering with the hunting interval logic.  This part was
missed in commit b5d91704f5 ("libceph: behave in mon_fault() if
cur_mon < 0") and use-after-free can still ensue on monc and objects
that hang off of it, with monc->auth and monc->monmap being
particularly susceptible to quickly being reused.

To fix this:

- clear monc->cur_mon and monc->hunting as part of closing the session
  in ceph_monc_stop()
- bail from delayed_work() if monc->cur_mon is cleared, similar to how
  it's done in mon_fault() and finish_hunting() (based on monc->hunting)
- call cancel_delayed_work_sync() after the session is closed

1cb6422eca ceph: Annotate struct ceph_monmap with __counted_by
Prepare for the coming implementation by GCC and Clang of the __counted_by
attribute. Flexible array members annotated with __counted_by can have
their accesses bounds-checked at run-time checking via CONFIG_UBSAN_BOUNDS
(for array indexing) and CONFIG_FORTIFY_SOURCE (for strcpy/memcpy-family
functions).

As found with Coccinelle[1], add __counted_by for struct ceph_monmap.
Additionally, since the element count member must be set before accessing
the annotated flexible array member, move its initialization earlier.

a341131eb3 libceph: drop ->monmap and err initialization
Call to build_initial_monmap() is one stone two birds.  Explicitly it
initializes err variable. Implicitly it initializes ->monmap via call to
kzalloc().  We should only declare err and ->monmap is taken care of by
ceph_monc_init() prototype.

4972cf605f libceph, ceph: disambiguate ceph_connection_operations handlers
Since a few years, kernel addresses are no longer included in oops
dumps, at least on x86.  All we get is a symbol name with offset and
size.

This is a problem for ceph_connection_operations handlers, especially
con->ops->dispatch().  All three handlers have the same name and there
is little context to disambiguate between e.g. monitor and OSD clients
because almost everything is inlined.  gdb sneakily stops at the first
matching symbol, so one has to resort to nm and addr2line.

Some of these are already prefixed with mon_, osd_ or mds_.  Let's do
the same for all others.

00498b9941 libceph: introduce connection modes and ms_mode option
msgr2 supports two connection modes: crc (plain) and secure (on-wire
encryption).  Connection mode is picked by server based on input from
client.

Introduce ms_mode option:

  ms_mode=legacy        - msgr1 (default)
  ms_mode=crc           - crc mode, if denied fail
  ms_mode=secure        - secure mode, if denied fail
  ms_mode=prefer-crc    - crc mode, if denied agree to secure mode
  ms_mode=prefer-secure - secure mode, if denied agree to crc mode

ms_mode affects all connections, we don't separate connections to mons
like it's done in userspace with ms_client_mode vs ms_mon_client_mode.

For now the default is legacy, to be flipped to prefer-crc after some
time.

a5cbd5fc22 libceph, ceph: get and handle cluster maps with addrvecs
In preparation for msgr2, make the cluster send us maps with addrvecs
including both LEGACY and MSGR2 addrs instead of a single LEGACY addr.
This means advertising support for SERVER_NAUTILUS and also some older
features: SERVER_MIMIC, MONENC and MONNAMES.

MONNAMES and MONENC are actually pre-argonaut, we just never updated
ceph_monmap_decode() for them.  Decoding is unconditional, see commit
23c625ce30 ("libceph: assume argonaut on the server side").

SERVER_MIMIC doesn't bear any meaning for the kernel client.

Since ceph_decode_entity_addrvec() is guarded by encoding version
checks (and in msgr2 case it is guarded implicitly by the fact that
server is speaking msgr2), we assume MSG_ADDR2 for it.

4d8b8fb494 libceph: simplify ceph_monc_handle_map()
ceph_monc_handle_map() confuses static checkers which report a
false use-after-free on monc->monmap, missing that monc->monmap and
client->monc.monmap is the same pointer.

Use monc->monmap consistently and get rid of "old", which is redundant.

b726ec972c libceph: make ceph_pr_addr take an struct ceph_entity_addr pointer
GCC9 is throwing a lot of warnings about unaligned accesses by
callers of ceph_pr_addr. All of the current callers are passing a
pointer to the sockaddr inside struct ceph_entity_addr.

Fix it to take a pointer to a struct ceph_entity_addr instead,
and then have the function make a copy of the sockaddr before
printing it.

bb229bbb3b libceph: wait for latest osdmap in ceph_monc_blacklist_add()
Because map updates are distributed lazily, an OSD may not know about
the new blacklist for quite some time after "osd blacklist add" command
is completed.  This makes it possible for a blacklisted but still alive
client to overwrite a post-blacklist update, resulting in data
corruption.

Waiting for latest osdmap in ceph_monc_blacklist_add() and thus using
the post-blacklist epoch for all post-blacklist requests ensures that
all such requests "wait" for the blacklist to come into force on their
respective OSDs.

acafe7e302 treewide: Use struct_size() for kmalloc()-family
One of the more common cases of allocation size calculations is finding
the size of a structure that has a zero-sized array at the end, along
with memory for some number of elements for that array. For example:

If we go without an established session for a while, backoff delay will
climb to 30 seconds.  The keepalive timeout is also 30 seconds, so it's
pretty easily hit after a prolonged hunting for a monitor: we don't get
a chance to send out a keepalive in time, which means we never get back
a keepalive ack in time, cutting an established session and attempting
to connect to a different monitor every 30 seconds:

facb9f6eba libceph: un-backoff on tick when we have a authenticated session
This means that if we do some backoff, then authenticate, and are
healthy for an extended period of time, a subsequent failure won't
leave us starting our hunting sequence with a large backoff.

b24413180f License cleanup: add SPDX GPL-2.0 license identifier to files with no license
Many source files in the tree are missing licensing information, which
makes it harder for compliance tools to determine the correct license.

By default all files without license information are under the default
license of the kernel, which is GPL version 2.

Update the files which contain no license information with the 'GPL-2.0'
SPDX license identifier.  The SPDX identifier is a legally binding
shorthand, which can be used instead of the full boiler plate text.

This patch is based on work done by Thomas Gleixner and Kate Stewart and
Philippe Ombredanne.

How this work was done:

Patches were generated and checked against linux-4.14-rc6 for a subset of
the use cases:
 - file had no licensing information it it.
 - file was a */uapi/* one with no licensing information in it,
 - file was a */uapi/* one with existing licensing information,

Further patches will be generated in subsequent months to fix up cases
where non-standard license headers were used, and references to license
had to be inferred by heuristics based on keywords.

The analysis to determine which SPDX License Identifier to be applied to
a file was done in a spreadsheet of side by side results from of the
output of two independent scanners (ScanCode & Windriver) producing SPDX
tag:value files created by Philippe Ombredanne.  Philippe prepared the
base worksheet, and did an initial spot review of a few 1000 files.

The 4.13 kernel was the starting point of the analysis with 60,537 files
assessed.  Kate Stewart did a file by file comparison of the scanner
results in the spreadsheet to determine which SPDX license identifier(s)
to be applied to the file. She confirmed any determination that was not
immediately clear with lawyers working with the Linux Foundation.

Criteria used to select files for SPDX license identifier tagging was:
 - Files considered eligible had to be source code files.
 - Make and config files were included as candidates if they contained >5
   lines of source
 - File already had some variant of a license header in it (even if <5
   lines).

All documentation files were explicitly excluded.

The following heuristics were used to determine which SPDX license
identifiers to apply.

 - when both scanners couldn't find any license traces, file was
   considered to have no license information in it, and the top level
   COPYING file license applied.

   For non */uapi/* files that summary was:

   and resulted in the first patch in this series.

   and that resulted in the third patch in this series.

 - when the two scanners agreed on the detected license(s), that became
   the concluded license(s).

 - when there was disagreement between the two scanners (one detected a
   license but the other didn't, or they both detected different
   licenses) a manual inspection of the file occurred.

 - In most cases a manual inspection of the information in the file
   resulted in a clear resolution of the license that should apply (and
   which scanner probably needed to revisit its heuristics).

 - When it was not immediately clear, the license identifier was
   confirmed with lawyers working with the Linux Foundation.

 - If there was any question as to the appropriate license identifier,
   the file was flagged for further research and to be revisited later
   in time.

In total, over 70 hours of logged manual review was done on the
spreadsheet to determine the SPDX license identifiers to apply to the
source files by Kate, Philippe, Thomas and, in some cases, confirmation
by lawyers working with the Linux Foundation.

Kate also obtained a third independent scan of the 4.13 code base from
FOSSology, and compared selected files where the other two scanners
disagreed against that SPDX file, to see if there was new insights.  The
Windriver scanner is based on an older version of FOSSology in part, so
they are related.

Thomas did random spot checks in about 500 files from the spreadsheets
for the uapi headers and agreed with SPDX license identifier in the
files he inspected. For the non-uapi files Thomas did random spot checks
in about 15000 files.

In initial set of patches against 4.14-rc6, 3 files were found to have
copy/paste license identifier errors, and have been fixed to reflect the
correct identifier.

Additionally Philippe spent 10 hours this week doing a detailed manual
inspection and review of the 12,461 patched files from the initial patch
version early this week with:
 - a full scancode scan run, collecting the matched texts, detected
   license ids and scores
 - reviewing anything where there was a license detected (about 500+
   files) to ensure that the applied SPDX license was correct
 - reviewing anything where there was no detection but the patch license
   was not GPL-2.0 WITH Linux-syscall-note to ensure that the applied
   SPDX license was correct

This produced a worksheet with 20 files needing minor correction.  This
worksheet was then exported into 3 different .csv files for the
different types of files to be modified.

These .csv files were then reviewed by Greg.  Thomas wrote a script to
parse the csv files and add the proper SPDX tag to the file, in the
format that the file expected.  This script was further refined by Greg
based on the output to detect more types of files automatically and to
distinguish between header and source .c files (which need different
comment types.)  Finally Greg ran the script using the .csv files to
generate the patches.

Improve accuracy of statfs reporting for Ceph filesystems comprising
exactly one data pool. In this case, the Ceph monitor can now report
the space usage for the single data pool instead of the global data
for the entire Ceph cluster. Include support for this message in
mon_client and leverage it in ceph/super.

Separate osdmap handling from decoding and iterating over a bag of maps
in a fresh MOSDMap message.  This sets up the scene for the updated OSD
client.

Of particular importance here is the addition of pi->was_full, which
can be used to answer "did this pool go full -> not-full in this map?".
This is the key bit for supporting pool quotas.

We won't be able to downgrade map_sem for much longer, so drop
downgrade_write().

b5d91704f5 libceph: behave in mon_fault() if cur_mon < 0
This can happen if __close_session() in ceph_monc_stop() races with
a connection reset.  We need to ignore such faults, otherwise it's
likely we would take !hunting, call __schedule_delayed() and end up
with delayed_work() executing on invalid memory, among other things.

The (two!) con->private tests are useless, as nothing ever clears
con->private.  Nuke them.

bee3a37c47 libceph: reschedule tick in mon_fault()
Doing __schedule_delayed() in the hunting branch is pointless, as the
tick will have already been scheduled by then.

What we need to do instead is *reschedule* it in the !hunting branch,
after reopen_session() changes hunt_mult, which affects the delay.
This helps with spacing out connection attempts and avoiding things
like two back-to-back attempts followed by a longer period of waiting
around.

Unless we are in the process of setting up a client (i.e. connecting to
the monitor cluster for the first time), apply a backoff: every time we
want to reopen a session, increase our timeout by a multiple (currently
2); when we complete the connection, reduce that multipler by 50%.

Don't try to reconnect to the same monitor when we fail to establish
a session within a timeout or it's lost.

For that, pick_new_mon() needs to see the old value of cur_mon, so
don't clear it in __close_session() - all calls to __close_session()
but one are followed by __open_session() anyway.  __open_session() is
only called when a new session needs to be established, so the "already
open?" branch, which is now in the way, is simply dropped.

82dcabad75 libceph: revamp subs code, switch to SUBSCRIBE2 protocol
It is currently hard-coded in the mon_client that mdsmap and monmap
subs are continuous, while osdmap sub is always "onetime".  To better
handle full clusters/pools in the osd_client, we need to be able to
issue continuous osdmap subs.  Revamp subs code to allow us to specify
for each sub whether it should be continuous or not.

Although not strictly required for the above, switch to SUBSCRIBE2
protocol while at it, eliminating the ambiguity between a request for
"every map since X" and a request for "just the latest" when we don't
have a map yet (i.e. have epoch 0).  SUBSCRIBE2 feature bit is now
required - it's been supported since pre-argonaut (2010).

Move "got mdsmap" call to the end of ceph_mdsc_handle_map() - calling
in before we validate the epoch and successfully install the new map
can mess up mon_client sub state.

02ac956c42 libceph: move debugfs initialization into __ceph_open_session()
Our debugfs dir name is a concatenation of cluster fsid and client
unique ID ("global_id").  It used to be the case that we learned
global_id first, nowadays we always learn fsid first - the monmap is
sent before any auth replies are.  ceph_debugfs_client_init() call in
ceph_monc_handle_map() is therefore never executed and can be removed.

Its counterpart in handle_auth_reply() doesn't really belong there
either: having to do monc->client and unlocking early to work around
lockdep is a testament to that.  Move it into __ceph_open_session(),
where it can be called unconditionally.
