The mmc_card_present() function helps the mmc core to track an internal
state of the card device. More importantly, it's not intended to be used by
mmc host drivers to check for an inserted card. Therefore, let's stop using
it and instead rely on checking for a valid pointer to a struct mmc_card,
as it should be good enough.

Suspend and resume of cards are handled by the protocol layer and
consequently the mmc_suspend|resume_host APIs are marked as deprecated.
This means we can simplify the suspend|resume callbacks by removing the
use of the deprecated APIs.

module_param(bool) used to counter-intuitively take an int. In
fddd5201 (mid-2009) we allowed bool or int/unsigned int using a messy
trick.
It's time to remove the int/unsigned int option. For this version
it'll simply give a warning, but it'll break next kernel version.

Add a driver for Elan Digital System's VUB300 chip which is a USB
connected SDIO/SDmem/MMC host controller. A VUB300 chip enables a USB 2.0
or USB 1.1 connected host computer to use SDIO/SD/MMC cards without the
need for a directly connected, for example via PCI, SDIO host controller.
