6b458ac195 USB fixes for 3.16-rc5
Here are some small USB fixes, PHY driver fixes (they ended up in this

eb82a3d846 phy: omap-usb2: Balance pm_runtime_enable() on probe failure and remove
If probe fails then we need to call pm_runtime_disable() to balance
out the previous pm_runtime_enable() call. Else it will cause
unbalanced pm_runtime_enable() call in the succeding probe call.

This anomaly was observed when the call to devm_phy_create() failed
with -EPROBE_DEFER.

Balance out the pm_runtime_enable() call in .remove() as well.

e73b49f1c4 phy: core: Fix error path in phy_create()
Prevent resources from being freed twice in case device_add() call
fails within phy_create(). Also use ida_simple_remove() instead of
ida_remove() as we had used ida_simple_get() to allocate the ida.

When NO_IOMEM is enabled (e.g. score architecture), some drivers which
need HAS_IOMEM need notice about it, or it will report related warning:

59025887fb phy: Add new Exynos5 USB 3.0 PHY driver
Add a new driver for the USB 3.0 PHY on Exynos5 series of SoCs.
The new driver uses the generic PHY framework and will interact
with DWC3 controller present on Exynos5 series of SoCs.

Also, created a new header file in linux/mfd/syscon/ for
Exynos5 SoCs and put the required PMU offset definitions
for the basic available PHYs.

3fc03f3d72 phy: kona2: use 'select GENERIC_PHY' in Kconfig
All other phy drivers use 'select', while this one is
the only one to use 'depends on'. This is not a bug, just
slightly inconsistent, so let's change it to do things the
same way as everyone else.

We may also want to turn GENERIC_PHY into a silent option
that only ever gets turned on if another driver needs it.

743bb387a1 phy: fix kernel oops in phy_lookup()
The kernel oopses in phy_lookup() due to 'phy->init_data' being NULL if we
register PHYs from a device tree probing driver and then call phy_get() on a
device that has no representation in the device tree (e.g. a PCI device).
Checking the pointer before dereferening it and skipping an interation if
it's NULL prevents this kernel oops.

When OMAP_CONTROL_USB was renamed to OMAP_CONTROL_PHY (commit
14da699b), its dependencies were lost in the process. Nothing in the
commit message indicates that this removal was intentional, so I think
it was by accident and the dependencies should be restored.

d1481832f1 phy: exynos: fix building as a module
The top-level phy-samsung-usb2 driver may be configured as a
loadable module, which currently causes link errors because
of the dependency on the exynos{5250,4x12,4210}_usb2_phy_config
symbol. Solving this could be achieved by exporting these
symbols, but as the SoC-specific parts of the driver are not
currently built as modules, it seems better to just link
everything into one module and avoid the need for the export.

This patch adds support for the APM X-Gene SoC 15Gbps Multi-purpose PHY.
This is the physical layer interface for the corresponding host
controller. Currently, only external clock and Gen3 SATA mode
are supported.

56042e4e97 phy: ti-pipe3: Fix suspend/resume and module reload
Due to Errata i783, SATA breaks if its DPLL is idled. The recommeded
workaround to issue a softreset to the SATA controller doesn't seem to
work. Here we just prevent SATA DPLL from Idling and hence avoid
the issue altogether.

629138dbae phy: ti-pipe3: streamline PHY operations
Limit .power_on() and .power_off() to just control the
PHY power and not the DPLL. The DPLL will be enabled
in .init() and idled in .exit().

Don't reprogram the DPLL if it has been already locked
by the bootloader. This fixes a problem with SATA, where
it fails if SATA was used by the bootloader.

Enable the dra7x errata workaround for false disconnect problem
with USB2PHY. False disconnects were detected with some of the devices.
Reduce the sensitivity of the disconnect logic within the USB2PHY subsystem
to enusre these false disconnects are not registered.

While at that, pass proper flags for each SoC's. This is a common driver
used across OMAP4,OMAP5,DRA7xx and AM437x USB2PHY.

False disconnect workaround is currently applicable for only DRA7x.

0b3f3b2c77 phy: core: Add an exported of_phy_get function
Previously the of_phy_get function took a struct device * and
was declared static. It was impossible to call it from
another driver and thus it was impossible to get phy defined
for a given node. The old function was renamed to _of_phy_get
and was left for internal use. of_phy_get function was added
and it was exported. The function enables to get a phy for
a given device tree node.

ba4bdc9e1d PHY: sunxi: Add driver for sunxi usb phy
The Allwinner A1x / A2x SoCs have 2 or 3 usb phys which are all accessed
through a single set of registers. Besides this there are also some other
phy related bits which need poking, which are per phy, but shared between the
ohci and ehci controllers, so these are also controlled from this new phy
driver.

Registering phy_provider before creating the PHY can result in PHY

b51fbf9fb0 phy-core: Don't allow building phy-core as a module
include/phy/phy.h has stub code in there for when building without the
phy-core enabled. This is useful for generic drivers such as ahci-platform,
ehci-platoform and ohci-platform which have support for driving an optional
phy passed to them through the devicetree.

Since on some boards this phy functionality is not needed, being able to
disable the phy subsystem without needing a lot of #ifdef magic in the
driver using it is quite useful.

However this breaks when the module using the phy subsystem is build-in and
the phy-core is not, which leads to the build failing with missing symbol
errors in the linking stage of the zImage.

Which leads to gems such as this being added to the Kconfig for achi_platform:

	depends on GENERIC_PHY || !GENERIC_PHY

Rather then duplicating this code in a lot of places using the phy-core,
I believe it is better to simply not allow the phy-core to be built as a
module. The phy core is quite small and has no external dependencies, so
always building it in when enabling it should not be an issue.
