71d5049b05 x86/mm: Flush global TLB when switching to trampoline page-table
Move the switching code into a function so that it can be re-used and
add a global TLB flush. This makes sure that usage of memory which is
not mapped in the trampoline page-table is reliably caught.

Also move the clearing of CR4.PCIDE before the CR3 switch because the
cr4_clear_bits() function will access data not mapped into the
trampoline page-table.

a729691b54 x86/reboot: Limit Dell Optiplex 990 quirk to early BIOS versions
When this platform was relatively new in November 2011, with early BIOS
revisions, a reboot quirk was added in commit 6be30bb7d7 ("x86/reboot:
Blacklist Dell OptiPlex 990 known to require PCI reboot")

However, this quirk (and several others) are open-ended to all BIOS
versions and left no automatic expiry if/when the system BIOS fixed the
issue, meaning that nobody is likely to come along and re-test.

What is really problematic with using PCI reboot as this quirk does, is
that it causes this platform to do a full power down, wait one second,
and then power back on. This is less than ideal if one is using it for
boot testing and/or bisecting kernels when legacy rotating hard disks
are installed.

It was only by chance that the quirk was noticed in dmesg - and when
disabled it turned out that it wasn't required anymore (BIOS A24), and a
default reboot would work fine without the "harshness" of power cycling the
machine (and disks) down and up like the PCI reboot does.

Doing a bit more research, it seems that the "newest" BIOS for which the
issue was reported[1] was version A06, however Dell[2] seemed to suggest
only up to and including version A05, with the A06 having a large number of
fixes[3] listed.

As is typical with a new platform, the initial BIOS updates come frequently
and then taper off (and in this case, with a revival for CPU CVEs); a
search for O990-A<ver>.exe reveals the following dates:

        A02     16 Mar 2011
        A03     11 May 2011
        A06     14 Sep 2011
        A07     24 Oct 2011
        A10     08 Dec 2011
        A14     06 Sep 2012
        A16     15 Oct 2012
        A18     30 Sep 2013
        A19     23 Sep 2015
        A20     02 Jun 2017
        A23     07 Mar 2018
        A24     21 Aug 2018

While it's overkill to flash and test each of the above, it would seem
likely that the issue was contained within A0x BIOS versions, given the
dates above and the dates of issue reports[4] from distros. So rather than
just throw out the quirk entirely, limit the scope to just those early BIOS
versions, in case people are still running systems from 2011 with the
original as-shipped early A0x BIOS versions.

8ec9069a43 x86/idt: Remove address argument from idt_invalidate()
There is no reason to specify any specific address to idt_invalidate(). It
looks mostly like an artifact of unifying code done differently by
accident. The most "sensible" address to set here is a NULL pointer -
virtual address zero, just as a visual marker.

This also makes it possible to mark the struct desc_ptr in idt_invalidate()
as static const.

- Support for userspace to emulate Xen hypercalls
 - Raise the maximum number of user memslots
 - Scalability improvements for the new MMU. Instead of the complex
   "fast page fault" logic that is used in mmu.c, tdp_mmu.c uses an
   rwlock so that page faults are concurrent, but the code that can run
   against page faults is limited. Right now only page faults take the
   lock for reading; in the future this will be extended to some
   cases of page table destruction. I hope to switch the default MMU

 - Cleanups for MAXPHYADDR checks

 - On AMD, use VMLOAD/VMSAVE to save and restore host state
 - Stop using deprecated jump label APIs
 - Workaround for AMD erratum that made nested virtualization unreliable
 - Support for LBR emulation in the guest
 - Support for communicating bus lock vmexits to userspace

ed72736183 x86/reboot: Force all cpus to exit VMX root if VMX is supported
Force all CPUs to do VMXOFF (via NMI shootdown) during an emergency
reboot if VMX is _supported_, as VMX being off on the current CPU does
not prevent other CPUs from being in VMX root (post-VMXON). This fixes
a bug where a crash/panic reboot could leave other CPUs in VMX root and
prevent them from being woken via INIT-SIPI-SIPI in the new kernel.

4b2d8ca920 x86/reboot: Add Zotac ZBOX CI327 nano PCI reboot quirk
On this system the M.2 PCIe WiFi card isn't detected after reboot, only
after cold boot. reboot=pci fixes this behavior. In [0] the same issue
is described, although on another system and with another Intel WiFi
card. In case it's relevant, both systems have Celeron CPUs.

Add a PCI reboot quirk on affected systems until a more generic fix is
available.

0082517fa4 x86/reboot, efi: Use EFI reboot for Acer TravelMate X514-51T
Upon reboot, the Acer TravelMate X514-51T laptop appears to complete the
shutdown process, but then it hangs in BIOS POST with a black screen.

The problem is intermittent - at some points it has appeared related to
Secure Boot settings or different kernel builds, but ultimately we have
not been able to identify the exact conditions that trigger the issue to
come and go.

Besides, the EFI mode cannot be disabled in the BIOS of this model.

However, after extensive testing, we observe that using the EFI reboot
method reliably avoids the issue in all cases.

So add a boot time quirk to use EFI reboot on such systems.

339b2ae0cd x86/apic: Fix restoring boot IRQ mode in reboot and kexec/kdump
This is a regression fix.

Before, to fix erratum AVR31, the following commit:

  522e664644 ("x86/apic: Disable I/O APIC before shutdown of the local APIC")

... moved the lapic_shutdown() call to after disable_IO_APIC() in the reboot
and kexec/kdump code paths.

This introduced the following regression: disable_IO_APIC() not only clears
the IO-APIC, but it also restores boot IRQ mode by setting the
LAPIC/APIC/IMCR, calling lapic_shutdown() after disable_IO_APIC() will
disable LAPIC and ruin the possible virtual wire mode setting which
the code has been trying to do all along.

The consequence is that a KVM guest kernel always prints the warning below
during kexec/kdump as the kernel boots up:

  [    0.001000] WARNING: CPU: 0 PID: 0 at arch/x86/kernel/apic/apic.c:1467 setup_local_APIC+0x228/0x330
  [    ........]

  [    0.001000]  secondary_startup_64+0xa5/0xb0
  [    ........]
  [    0.001000] masked ExtINT on CPU#0

To fix this, just call clear_IO_APIC() to stop the IO-APIC where
disable_IO_APIC() was called, and call restore_boot_irq_mode() to
restore boot IRQ mode before a reboot or a kexec/kdump jump.

b24413180f License cleanup: add SPDX GPL-2.0 license identifier to files with no license
Many source files in the tree are missing licensing information, which
makes it harder for compliance tools to determine the correct license.

By default all files without license information are under the default
license of the kernel, which is GPL version 2.

Update the files which contain no license information with the 'GPL-2.0'
SPDX license identifier. The SPDX identifier is a legally binding
shorthand, which can be used instead of the full boiler plate text.

This patch is based on work done by Thomas Gleixner and Kate Stewart and
Philippe Ombredanne.

How this work was done:

Patches were generated and checked against linux-4.14-rc6 for a subset of
the use cases:
 - file had no licensing information it it.
 - file was a */uapi/* one with no licensing information in it,
 - file was a */uapi/* one with existing licensing information,

Further patches will be generated in subsequent months to fix up cases
where non-standard license headers were used, and references to license
had to be inferred by heuristics based on keywords.

The analysis to determine which SPDX License Identifier to be applied to
a file was done in a spreadsheet of side by side results from of the
output of two independent scanners (ScanCode & Windriver) producing SPDX
tag:value files created by Philippe Ombredanne. Philippe prepared the
base worksheet, and did an initial spot review of a few 1000 files.

The 4.13 kernel was the starting point of the analysis with 60,537 files
assessed. Kate Stewart did a file by file comparison of the scanner
results in the spreadsheet to determine which SPDX license identifier(s)
to be applied to the file. She confirmed any determination that was not
immediately clear with lawyers working with the Linux Foundation.

Criteria used to select files for SPDX license identifier tagging was:
 - Files considered eligible had to be source code files.
 - Make and config files were included as candidates if they contained >5
   lines of source
 - File already had some variant of a license header in it (even if <5
   lines).

All documentation files were explicitly excluded.

The following heuristics were used to determine which SPDX license
identifiers to apply.

 - when both scanners couldn't find any license traces, file was
   considered to have no license information in it, and the top level
   COPYING file license applied.

   For non */uapi/* files that summary was:

   SPDX license identifier                            # files
   ---------------------------------------------------|-------
   GPL-2.0                                              11139

   and resulted in the first patch in this series.

   If that file was a */uapi/* path one, it was "GPL-2.0 WITH
   Linux-syscall-note" otherwise it was "GPL-2.0". Results of that was:

   SPDX license identifier                            # files
   ---------------------------------------------------|-------
   GPL-2.0 WITH Linux-syscall-note                        930

   and resulted in the second patch in this series.

 - if a file had some form of licensing information in it, and was one
   of the */uapi/* ones, it was denoted with the Linux-syscall-note if
   any GPL family license was found in the file or had no licensing in
   it (per prior point). Results summary:

   SPDX license identifier                            # files
   ---------------------------------------------------|------
   GPL-2.0 WITH Linux-syscall-note                       270
   GPL-2.0+ WITH Linux-syscall-note                      169
   ((GPL-2.0 WITH Linux-syscall-note) OR BSD-2-Clause)    21
   ((GPL-2.0 WITH Linux-syscall-note) OR BSD-3-Clause)    17
   LGPL-2.1+ WITH Linux-syscall-note                      15
   GPL-1.0+ WITH Linux-syscall-note                       14
   ((GPL-2.0+ WITH Linux-syscall-note) OR BSD-3-Clause)    5
   LGPL-2.0+ WITH Linux-syscall-note                       4
   LGPL-2.1 WITH Linux-syscall-note                        3
   ((GPL-2.0 WITH Linux-syscall-note) OR MIT)              3
   ((GPL-2.0 WITH Linux-syscall-note) AND MIT)             1

   and that resulted in the third patch in this series.

 - when the two scanners agreed on the detected license(s), that became
   the concluded license(s).

 - when there was disagreement between the two scanners (one detected a
   license but the other didn't, or they both detected different
   licenses) a manual inspection of the file occurred.

 - In most cases a manual inspection of the information in the file
   resulted in a clear resolution of the license that should apply (and
   which scanner probably needed to revisit its heuristics).

 - When it was not immediately clear, the license identifier was
   confirmed with lawyers working with the Linux Foundation.

 - If there was any question as to the appropriate license identifier,
   the file was flagged for further research and to be revisited later
   in time.

In total, over 70 hours of logged manual review was done on the
spreadsheet to determine the SPDX license identifiers to apply to the
source files by Kate, Philippe, Thomas and, in some cases, confirmation
by lawyers working with the Linux Foundation.

Kate also obtained a third independent scan of the 4.13 code base from
FOSSology, and compared selected files where the other two scanners
disagreed against that SPDX file, to see if there was new insights. The
Windriver scanner is based on an older version of FOSSology in part, so
they are related.

Thomas did random spot checks in about 500 files from the spreadsheets
for the uapi headers and agreed with SPDX license identifier in the
files he inspected. For the non-uapi files Thomas did random spot checks
in about 15000 files.

In initial set of patches against 4.14-rc6, 3 files were found to have
copy/paste license identifier errors, and have been fixed to reflect the
correct identifier.

Additionally Philippe spent 10 hours this week doing a detailed manual
inspection and review of the 12,461 patched files from the initial patch
version early this week with:
 - a full scancode scan run, collecting the matched texts, detected
   license ids and scores
 - reviewing anything where there was a license detected (about 500+
   files) to ensure that the applied SPDX license was correct
 - reviewing anything where there was no detection but the patch license
   was not GPL-2.0 WITH Linux-syscall-note to ensure that the applied
   SPDX license was correct

This produced a worksheet with 20 files needing minor correction. This
worksheet was then exported into 3 different .csv files for the
different types of files to be modified.

These .csv files were then reviewed by Greg. Thomas wrote a script to
parse the csv files and add the proper SPDX tag to the file, in the
format that the file expected. This script was further refined by Greg
based on the output to detect more types of files automatically and to
distinguish between header and source .c files (which need different
comment types.) Finally Greg ran the script using the .csv files to
generate the patches.

186f43608a x86/kernel: Audit and remove any unnecessary uses of module.h
Historically a lot of these existed because we did not have
a distinction between what was modular code and what was providing
support to modules via EXPORT_SYMBOL and friends. That changed
when we forked out support for the latter into the export.h file.

This means we should be able to reduce the usage of module.h
in code that is obj-y Makefile or bool Kconfig. The advantage
in doing so is that module.h itself sources about 15 other headers;
adding significantly to what we feed cpp, and it can obscure what
headers we are effectively using.

Since module.h was the source for init.h (for __init) and for
export.h (for EXPORT_SYMBOL) we consider each obj-y/bool instance
for the presence of either and replace as needed. Build testing
revealed some implicit header usage that was fixed up accordingly.

Note that some bool/obj-y instances remain since module.h is
the header for some exception table entry stuff, and for things
like __init_or_module (code that is tossed when MODULES=n).

b279d67df8 x86/nmi: Save regs in crash dump on external NMI
Now, multiple CPUs can receive an external NMI simultaneously by
specifying the "apic_extnmi=all" command line parameter. When we take
a crash dump by using external NMI with this option, we fail to save
registers into the crash dump. This happens as follows:

  CPU 0                              CPU 1
  ================================   =============================
  receive an external NMI
  default_do_nmi()                   receive an external NMI
    spin_lock(&nmi_reason_lock)      default_do_nmi()
    io_check_error()                   spin_lock(&nmi_reason_lock)
      panic()                            busy loop
      ...
        kdump_nmi_shootdown_cpus()
          issue NMI IPI -----------> blocked until IRET
                                         busy loop...

Here, since CPU 1 is in NMI context, an additional NMI from CPU 0
remains unhandled until CPU 1 IRETs. However, CPU 1 will never execute
IRET so the NMI is not handled and the callback function to save
registers is never called.

To solve this issue, we check if the IPI for crash dumping was issued
while waiting for nmi_reason_lock to be released, and if so, call its
callback function directly. If the IPI is not issued (e.g. kdump is
disabled), the actual behavior doesn't change.

58c5661f21 panic, x86: Allow CPUs to save registers even if looping in NMI context
Currently, kdump_nmi_shootdown_cpus(), a subroutine of crash_kexec(),
sends an NMI IPI to CPUs which haven't called panic() to stop them,
save their register information and do some cleanups for crash dumping.
However, if such a CPU is infinitely looping in NMI context, we fail to
save its register information into the crash dump.

For example, this can happen when unknown NMIs are broadcast to all
CPUs as follows:

  CPU 0                             CPU 1
  ===========================       ==========================
  receive an unknown NMI
  unknown_nmi_error()
    panic()                         receive an unknown NMI
      spin_trylock(&panic_lock)     unknown_nmi_error()
      crash_kexec()                   panic()
                                        spin_trylock(&panic_lock)
                                        panic_smp_self_stop()
                                          infinite loop
        kdump_nmi_shootdown_cpus()
          issue NMI IPI -----------> blocked until IRET
                                          infinite loop...

Here, since CPU 1 is in NMI context, the second NMI from CPU 0 is
blocked until CPU 1 executes IRET. However, CPU 1 never executes IRET,
so the NMI is not handled and the callback function to save registers is
never called.

In practice, this can happen on some servers which broadcast NMIs to all
CPUs when the NMI button is pushed.

To save registers in this case, we need to:

  a) Return from NMI handler instead of looping infinitely
  or
  b) Call the callback function directly from the infinite loop

Inherently, a) is risky because NMI is also used to prevent corrupted
data from being propagated to devices. So, we chose b).

This patch does the following:

1. Move the infinite looping of CPUs which haven't called panic() in NMI
   context (actually done by panic_smp_self_stop()) outside of panic() to
   enable us to refer pt_regs. Please note that panic_smp_self_stop() is
   still used for normal context.

2. Call a callback of kdump_nmi_shootdown_cpus() directly to save
   registers and do some cleanups after setting waiting_for_crash_ipi which
   is used for counting down the number of CPUs which handled the callback

2965faa5e0 kexec: split kexec_load syscall from kexec core code
There are two kexec load syscalls, kexec_load another and kexec_file_load.
 kexec_file_load has been splited as kernel/kexec_file.c. In this patch I
split kexec_load syscall code to kernel/kexec.c.

And add a new kconfig option KEXEC_CORE, so we can disable kexec_load and
use kexec_file_load only, or vice verse.

The original requirement is from Ted Ts'o, he want kexec kernel signature
being checked with CONFIG_KEXEC_VERIFY_SIG enabled. But kexec-tools use
kexec_load syscall can bypass the checking.

Vivek Goyal proposed to create a common kconfig option so user can compile
in only one syscall for loading kexec kernel. KEXEC/KEXEC_FILE selects
KEXEC_CORE so that old config files still work.

Because there's general code need CONFIG_KEXEC_CORE, so I updated all the
architecture Kconfig with a new option KEXEC_CORE, and let KEXEC selects
KEXEC_CORE in arch Kconfig. Also updated general kernel code with to
kexec_load syscall.

80313b3078 x86/reboot: Add ASRock Q1900DC-ITX mainboard reboot quirk
The ASRock Q1900DC-ITX mainboard (Baytrail-D) hangs randomly in
both BIOS and UEFI mode while rebooting unless reboot=pci is
used. Add a quirk to reboot via the pci method.

The problem is very intermittent and hard to debug, it might succeed
rebooting just fine 40 times in a row - but fails half a dozen times
the next day. It seems to be slightly less common in BIOS CSM mode
than native UEFI (with the CSM disabled), but it does happen in either
mode. Since I've started testing this patch in late january, rebooting
has been 100% reliable.

Most of the time it already hangs during POST, but occasionally it
might even make it through the bootloader and the kernel might even
start booting, but then hangs before the mode switch. The same symptoms
occur with grub-efi, gummiboot and grub-pc, just as well as (at least)
kernel 3.16-3.19 and 4.0-rc6 (I haven't tried older kernels than 3.16).
Upgrading to the most current mainboard firmware of the ASRock
Q1900DC-ITX, version 1.20, does not improve the situation.

( Searching the web seems to suggest that other Bay Trail-D mainboards
  might be affected as well. )

5be44a6fb1 x86: Remove the PCI reboot method from the default chain
Steve reported a reboot hang and bisected it back to this commit:

  a4f1987e4c x86, reboot: Add EFI and CF9 reboot methods into the default list

He heroically tested all reboot methods and found the following:

  reboot=t       # triple fault                  ok
  reboot=k       # keyboard ctrl                 FAIL
  reboot=b       # BIOS                          ok
  reboot=a       # ACPI                          FAIL
  reboot=e # EFI FAIL [system has no EFI]
  reboot=p       # PCI 0xcf9                     FAIL

And I think it's pretty obvious that we should only try PCI 0xcf9 as a
last resort - if at all.

The other observation is that (on this box) we should never try
the PCI reboot method, but close with either the 'triple fault'
or the 'BIOS' (terminal!) reboot methods.

Thirdly, CF9_COND is a total misnomer - it should be something like
CF9_SAFE or CF9_CAREFUL, and 'CF9' should be 'CF9_FORCE' ...

So this patch fixes the worst problems:

 - it orders the actual reboot logic to follow the reboot ordering
   pattern - it was in a pretty random order before for no good
   reason.

 - it fixes the CF9 misnomers and uses BOOT_CF9_FORCE and
   BOOT_CF9_SAFE flags to make the code more obvious.

 - it tries the BIOS reboot method before the PCI reboot method.
   (Since 'BIOS' is a terminal reboot method resulting in a hang
    if it does not work, this is essentially equivalent to removing
    the PCI reboot method from the default reboot chain.)

 - just for the miraculous possibility of terminal (resulting
   in hang) reboot methods of triple fault or BIOS returning
   without having done their job, there's an ordering between
   them as well.

a4f1987e4c x86, reboot: Add EFI and CF9 reboot methods into the default list
Reboot is the last service linux OS provides to the end user. We are
supposed to make this function more robust than today. This patch adds
all of the known reboot methods into the default attempt list. The
machines requiring reboot=efi or reboot=p or reboot=bios get a chance
to reboot automatically now.

If there is a new reboot method emerged, we are supposed to add it to
the default list as well, instead of adding the endless dmidecode entry.

If one method required is in the default list in this patch but the
machine reboot still hangs, that means some methods ahead of the
required method cause the system hangs, then reboot the machine by
passing reboot= arguments and submit the reboot dmidecode table quirk.

We are supposed to remove the reboot dmidecode table from the kernel,
but to be safe, we keep it. This patch prevents us from adding more.
If you happened to have a machine listed in the reboot dmidecode
table and this patch makes reboot work on your machine, please submit
a patch to remove the quirk.

The default reboot order with this patch is now:

    ACPI > KBD > ACPI > KBD > EFI > CF9_COND > BIOS

Because BIOS and TRIPLE are mutually exclusive (either will either
work or hang the machine) that method is not included.

[ hpa: as with any changes to the reboot order, this patch will have
  to be monitored carefully for regressions. ]

522e664644 x86/apic: Disable I/O APIC before shutdown of the local APIC
In reboot and crash path, when we shut down the local APIC, the I/O APIC is
still active. This may cause issues because external interrupts
can still come in and disturb the local APIC during shutdown process.

To quiet external interrupts, disable I/O APIC before shutdown local APIC.

6d9153bbce x86/reboot: Correct pr_info() log message in the set_bios/pci/kbd_reboot()
commit:

  c767a54ba0 x86/debug: Add KERN_<LEVEL> to bare printks, convert printks to pr_<level>

broke the log messages in the set_bios/pci/kbd_reboot() functions, by
putting the reboot method string and quirk entry's ident string in the
wrong order. This patch fixes it.
