08aff53582 usb/musb: remove conditional compilation of clk code
With addition of dummy clk_*() calls for non CONFIG_HAVE_CLK cases in
clk.h, there is no need to have clk code enclosed in #ifdef
CONFIG_HAVE_CLK, #endif macros.

musb also has these dummy macros defined locally.  Remove them as they
aren't required anymore.

ded017ee6c usb: phy: fix return value check of usb_get_phy
usb_get_phy will return -ENODEV if it's not able to find the phy. Hence
fixed all the callers of usb_get_phy to check for this error condition
instead of relying on a non-zero value as success condition.

c83a8542b5 usb: musb: move otg specific initializations from twl to glue
Moved otg specific state(OTG_STATE_B_IDLE, OTG_STATE_A_IDLE) initializations
from twl to glue. These initializations are removed from twl4030 and
twl6030 and moved to the mailbox API defined in glue.
This is part of the cleanup in preparation to make use of usb2 phy
driver.

1e5acb8d61 usb: musb: move work_struct(otg_notifier_work) from core to omap glue
Commit 712d8e(fixes pm_runtime calls while atomic by using a work
queue. musb pm_runtime_get_sync call happens in interrupt context
on cable attach case. That can result in re-enabling the interrupts and
cause side affect. To avoid this deferred processing is used)

While the issue and the work queue implementation is specific to omap
(omap2430.c), the work_struct is defined as a member of struct musb
(musb_core.h). Hence moved the work_struct from musb_core to omap
glue.

Add a linked list for keeping multiple PHY instances with different
types so that we can have separate USB2 and USB3 PHYs on one single
board. _get_phy_ has been changed so that the controller gets
the transceiver by type. _remove_phy_ has been added to let the phy
be removed from the phy list.

2ed9127cff usb: musb: host: Disable MUSB DMA mode incase of DMA channel request failure
Currently in case of MUSB DMA channel request failure we are not
clearing MUSB_RXCSR_DMAENAB, MUSB_RXCSR_H_AUTOREQ and
MUSB_RXCSR_AUTOCLEAR bits of MUSB RXCSR of MUSB DMA. Which is
causing failure in receipt of data packets in next transfer.

Fix is to disable the MUSB DMA mode and related bits incase of
DMA channel request fails

abf710e655 usb: musb: handle nuked ep dma interrupt
User can trigger disabling of gadget at run time while the
transfers are going on.
Eg: 1: rmmod of musb driver while transfers are going on

Eg: 2: On android doing:
 echo 0       > /sys/class/android_usb/android0/enable
While a big file transfer is going on via PTP/MTP.

In such a case, musb_gadget_disable() calls nuke()
but the dma interrupt may still happen for an endpoint since hw
would raise the interrupt in anycase.

 It's touches a lot of different parts of the kernel, all USB drivers,
 due to some API cleanups (getting rid of the ancient err() macro) and
 some changes that are needed for USB 3.0 power management updates.
 
 There are also lots of new drivers, pimarily gadget, but others as well.
 We deleted a staging driver, which was nice, and finally dropped the
 obsolete usbfs code, which will make Al happy to never have to touch
 that again.
 
 There were some build errors in the tree that linux-next found a few
 days ago, but those were fixed by the most recent changes (all were due
 to us not building with CONFIG_PM disabled.)

TI81XX platform has two musb interfaces and uses CPPI4.1 DMA engine.
It has builtin USB PHYs as AM35x. The current set of patches adds support
for one instance and only in PIO mode.

f5db9c6a3d m68k: Make sure {read,write}s[bwl]() are always defined
drivers/usb/musb/musb_io.h provides default implementations for
{read,write}s[bwl]() on most platforms, some of which will conflict soon
with platform-specific counterparts on m68k.

To avoid having to add more platform-specific checks to musb_io.h later,
make sure {read,write}s[bwl]() are always defined on m68k, and disable the
default implementations in musb_io.h on m68k, like is already done for
several other architectures.

pm_runtime_get_sync returns a signed integer. In case of errors
it returns a negative value. This patch fixes the error check
by making it signed instead of unsigned thus preventing register
access if get_sync_fails. Also passes the error cause to the
debug message.

c04352a590 usb: musb: fix some runtime_pm issues
When runtime_pm was originally added, it was done in rather confusing
way: omap2430_musb_init() (called from musb_init_controller) would do
runtime_pm_get_sync() and musb_init_controller() itself would do
runtime_pm_put to balance it out. This is not only confusing but also
wrong if non-omap2430 glue layer is used.

This confusion resulted in commit 772aed45b6 "usb: musb: fix
pm_runtime mismatch", that removed runtime_pm_put() from
musb_init_controller as that looked unbalanced, and also happened to
fix unrelated isp1704_charger crash. However this broke runtime PM
functionality (musb is now always powered, even without gadget active).

Avoid these confusing runtime pm dependences by making
musb_init_controller() and omap2430_musb_init() do their own runtime
get/put pairs; also cover error paths. Remove unneeded runtime_pm_put
in omap2430_remove too. isp1704_charger crash that motivated
772aed45b6 will be fixed by following patch.

e9e8c85e69 usb: musb: make modules behave better
There's really no point in doing all that
initcall trickery when we can safely let
udev handle module probing for us.

Remove all of that trickery, by moving everybody
to module_init() and making proper use of
platform_device_register() rather than
platform_device_probe().

This is not only unwanted, it's also happening on half-unitialized
state, after musb_shutdown() has returned, which sometimes causes
hardware to fail to work after reboot. Let's better properly stop
gadget on shutdown too.

This patch moves musb_gadget_cleanup out of musb_free(), which has 2
callsites: probe error path and musb_remove. On probe error path it was
superflous since musb_gadget_cleanup is called explicitly there, and
musb_remove() calls musb_shutdown(), so cleanup will get called as before.

Commit 0020afb369 (ARM: mach-davinci:
remove mach/memory.h) removed mach/memory.h for DaVinci which broke
DaVinci MUSB build.

mach/memory.h is not actually needed in davinci.c, so remove it.
While at it, also remove some more machine specific inclulde
files which are not needed for build.

Tested on DM644x EVM using USB card reader.

f557978745 usb: musb: drop superfluous pm_runtime calls around musb_shutdown
Since commit 4f9edd2d7e "usb: musb: Fix the crash issue during reboot"
musb_shutdown() does pm_runtime_get_sync/pm_runtime_put by itself, so
this no longer needs to be done by the caller. Also, musb_exit_debugfs()
doesn't access the device, so just drop those runtime_pm calls.

90ab5ee941 module_param: make bool parameters really bool (drivers & misc)
module_param(bool) used to counter-intuitively take an int.  In
fddd5201 (mid-2009) we allowed bool or int/unsigned int using a messy
trick.

It's time to remove the int/unsigned int option.  For this version
it'll simply give a warning, but it'll break next kernel version.

772aed45b6 usb: musb: fix pm_runtime mismatch
In musb_init_controller() there's a pm_runtime_put(), but there's no
pm_runtime_get(), which creates a mismatch that causes the driver to
sleep when it shouldn't.

This was introduced in 7acc619[1], but it wasn't triggered in my setup
until 18a2689[2] was merged to Linus' branch at point df0914[3]. IOW;
when PM is working as it was supposed to.

However, it seems most of the time this is used in a way that keeps the
counter above 0, so nobody noticed. Also, it seems to depend on the
configuration used in versions before 3.1, but not later (or in it).

I found the problem by loading isp1704_charger before any usb gadgets:

All versions after 2.6.39 are affected.

9a35f8767a usb: musb: allow building USB_MUSB_TUSB6010 as a module
Commit 1376d92f9 "usb: musb: allow musb and glue layers to be modules"
made the USB_MUSB_TUSB6010 option modular, but actually building
the driver as a module does not work, so various randconfig builds
actually fail. This changes all code that depends on the
option to also check for modular builds, and exports the necessary
symbols.

2e7fc3ba68 usb: musb: use a Kconfig choice to pick the right DMA method
The logic to allow only one DMA driver in MUSB is currently
flawed, because it also allows picking no DMA driver at all
and also not selecting PIO mode.

Using a choice statement makes this foolproof for now and
also simplifies the Makefile.

Unfortunately, we will have to revisit this when we start
supporting multiple ARM platforms in a single kernel binary,
because at that point we will actually need to select
multiple DMA drivers and pick the right one at run-time.

This commit renames the “speed” field of the usb_gadget_driver
structure to “max_speed”.  This is so that to make it more
apparent that the field represents the maximum speed gadget
driver can support.

This also make the field look more like fields with the same
name in usb_gadget and usb_composite_driver structures.  All
of those represent the *maximal* speed given entity supports.

After this commit, there are the following fields in various
structures:
* usb_gadget::speed - the current connection speed,
* usb_gadget::max_speed - maximal speed UDC supports,
* usb_gadget_driver::max_speed - maximal speed gadget driver
  supports, and
* usb_composite_driver::max_speed - maximal speed composite
  gadget supports.

bfe0658b40 usb: udc: Fix gadget driver's speed check in various UDC drivers
Several UDC drivers had a gadget driver's speed sanity check of the
form of:

	driver->speed != USB_SPEED_HIGH

or:

	driver->speed != USB_SPEED_HIGH && driver->speed != USB_SPEED_FULL

As more and more gadget drivers support USB SuperSpeed, driver->speed
may be set to USB_SPEED_SUPER and UDC driver should handle such gadget
correctly.  The above checks however fail to recognise USB_SPEED_SUPER
as a valid speed.

This commit changes the two checks to:

	driver->speed < USB_SPEED_HIGH

or:

	driver->speed < USB_SPEED_FULL

respectively.

5d193ce8f1 usb: musb: PM: fix context save/restore in suspend/resume path
Currently the driver tries to save context in the suspend path, but
will cause an abort if the device is already runtime suspended.  This
happens, for example, if MUSB loaded/compiled-in, in host mode, but no
USB devices are attached.  MUSB will be runtime suspended, but then
attempting a system suspend will crash due to the context save
being attempted while the device is disabled.

On OMAP, as of v3.1, the driver's ->runtime_suspend() callback will be
called late in the suspend path (by the PM domain layer) if the driver
is not already runtime suspended, ensuring a full shutdown.

Therefore, the context save is not needed in the ->suspend() method
since it will be called in the ->runtime_suspend() method anyways
(similarily for resume.)

NOTE: this leaves the suspend/resume methods basically empty (with
      some FIXMEs and comments, but I'll leave it to the maintainers
      to decide whether to remove them.

968b822c00 USB: Remove the SAW_IRQ hcd flag
The HCD_FLAG_SAW_IRQ flag was introduced in order to catch IRQ routing
errors: If an URB was unlinked and the host controller hadn't gotten
any IRQs, it seemed likely that the IRQs were directed to the wrong
vector.

This warning hasn't come up in many years, as far as I know; interrupt
routing now seems to be well under control.  Therefore there's no
reason to keep the flag around any more.  This patch (as1495) finally
removes it.

de76cc2ba2 musb_gadget: Fix for spurious interrupts on endpoint zero.
There is a multi-year old bug in the MUSB hardware which is not documented.
It causes spurious interrupts and have various symptoms, like endless
"SetupEnd came in a wrong ep0stage" messages. The fix is taken from the
FreeBSD's musb driver.

How to reproduce:
For example issue clear-stall on a couple of endpoints very fast,
like one request per 125us. After a while the bug triggers and the
musb-chip becomes unusable until next re-enumeration.

0ae52d5458 usb: musb: Enable DMA mode1 RX for transfers without short packets
This patch enables DMA mode1 for the RX path when we know
there won't be any short packets. We check that by looking
into the short_no_ok flag, if it's true we enable mode1, otherwise
we use mode0 to transfer the data.

This will result in a throughput performance gain of around
40% for USB mass-storage/mtp use cases.

In case one "forgot" to load the receiver i.e. doing

he ends up with:

The problem is that musb_free() tries to figure out what was
initializued and what wasn't and clean up only the initialized part.
This works well for usb_del_gadget_udc() but device_unregister() can't
deal with it. Therefore we rely on the fact the we always have a parent
device and only then remove the device.
I broke this in 0f91349 ("usb: gadget: convert all users to the new udc
infrastructure")
