We currently have 6 (!) separate naming variants to name temporary instruction
buffers that are used for code patching:
- insnbuf
- insnbuff
- insn_buff
- insn_buffer
- ibuf
- ibuffer
These are used as local variables, percpu fields and function parameters.
Standardize all the names to a single variant: 'insn_buff'.

Standardize warning, error, and success printout format

Rename distill.awk to objdump_reformat.awk because it more
clearly expresses its purpose of re-formatting the output
of objdump so that insn_decoder_test can read it.

Rename test_get_len test command to insn_decoder_test
as it a more meaningful name. This also changes some
comments in related files.
Note that this also removes the paragraph about
writing to the Free Software Foundation's mailing
address.
